▼ A proposta deste estudo foi desenvolver ferramentas que permitam um exame de palpação completo utilizando o algômetro e verificar a validade e confiabilidade intra e interexaminadores. Neste estudo foram examinados 80 indivíduos, sendo 40 pacientes sintomáticos que freqüentavam a clínica de prótese da Faculdade de Odontologia da Universidade de São Paulo, divididos em 20 mulheres e 20 homens com idade média de 43 anos e 40 voluntários assintomáticos divididos em 20 mulheres e 20 homens com idade média de 40 anos. Os pacientes foram examinados com um algômetro digital, ferramenta de palpação intra-oral e em forma de pinçamento em três sessões diferentes de exames com intervalo de uma semana entre si. Na primeira sessão os pacientes sintomáticos foram avaliados por um examinador, e este grupo foi denominado F1. Após uma semana, na segunda sessão, os mesmos pacientes foram reavaliados pelo mesmo examinador, e foram denominados grupoF2, após 30 minutos um segundo examinador avaliou os mesmos pacientes, com os resultados formou-se o grupoJ. Após uma semana, na terceira sessão, os 40 voluntários assintomáticos (controle) foram avaliados pelo examinador F, grupo denominado C. A validade do algômetro foi avaliada comparando-se grupoF2 x grupoC (sintomáticos x assintomáticos). A confiabilidade intra-examinador foi avaliada comparando-se grupoF1 x grupoF2 e a confiabilidade interexaminadores comparando-se grupoF2xgrupoJ. Após a aplicação de testes estatísticos a validade do algômetro foi comprovada, a confiabilidade intraexaminador foi considerada boa, porém a confiabilidade interexaminadores foi de aproximadamente 50%, mostrando que para se atingir altos níveis de confiabilidade é necessário um programa de calibragem e uma padronização da taxa de aplicação da força.
The purpose of this study was to develop tools that allow a complete palpation examination using an algometer to evaluate validity and reliability. This study examined 80 subjects, 40 symptomatics patients from the school of Dentistry, University of São Paulo, divided in 20 women and 20 men, mean age 40 years, were examined using an eletronic algometer, intra-oral tool and pincer ? type pressure tool. The symptomatics patients were examinated in tree different sessions with intervals of one week. In the first session, an examiner (F1 group) evaluated the simptomatics patients. After one week, in the second session, the same examiner (F2 group) evaluated the same patients, 30 minutes later a second examiner evaluated the same patients (J group). One week later, in the third session, the first examiner (C group) evaluated 40 healthy volunteers. The validity of pressure digital algometer was obtained comparing F2 group x C group. The reliability intra examiner was obtained comparing F1 group x F2 group and the reliability inter examiners comparing F1 group x J group. ▼ O objetivo deste trabalho foi analisar a influência da ansiedade e do estresse no limiar de dor à pressão (LDP) de músculos mastigatórios, numa amostra de estudantes universitários em diferentes períodos do ano letivo. Para este propósito, foram selecionados 45 indivíduos, sendo 29 estudantes, que apresentavam DTM de origem miogênica seguindo critérios de inclusão propostos pelo Research Diagnostic Criteria (RDC) e 16 que não apresentavam características de DTM, do gênero feminino, equilibrados em relação à idade. Utilizando um algômetro (KRATOS®) foram realizadas tomadas bilaterais dos limiares de dor à pressão (LDP) dos indivíduos da amostra nos músculos masseter, temporal anterior, médio e posterior. Além disso, os participantes foram solicitados a responder questionários multidimensionais, através do Inventário de Ansiedade de Beck (BAI) e o Inventário de Sintomas de Stress de Lipp (ISSL), para mensurar reações emocionais ou afetivas em situações que causem estresse e/ou ansiedade. Também, o nível de dor foi registrado pela Escala de Análise Visual (EAV). Todos os exames foram realizados em quatro momentos distintos (T1, T2, T3 e T4) tendo como parâmetro o período de avaliações acadêmicas da Faculdade de Odontologia de Bauru. Os dados obtidos foram submetidos à análise estatística (ANOVA, Teste de Tukey, Teste de Friedman e Mann-Whitney), em um nível de significância de 5%. A comparação entre os diferentes tempos do estudo nos 2 grupos mostrou diferença estatisticamente significativa (p<0,05), sendo que o período das avaliações mostrou maiores níveis de estresse e ansiedade e menores valores de LDP. Sob o ponto de vista do músculo nos diferentes grupos e nos diferentes tempos, foram encontradas diferenças estatisticamente significativas (p<0,05). Os resultados da Escala de Análise Visual mostraram diferenças estatisticamente significantes entre o grupo sintomático e o grupo assintomático em T1, T2, T3 e T4. Com relação ao estresse e ansiedade, não houve diferenças estatisticamente significante entre os grupos. Houve, no entanto, uma associação entre o aumento do estresse e da ansiedade e diminuição dos valores de LDP em cada tempo. Concluiu-se que existe relação entre estresse e ansiedade e LDP tanto para indivíduos assintomáticos quanto para sintomáticos com DTM de origem miogênica.
The aim of this research was to evaluate the influence of stress and anxiety on the Pressure Pain Threshold (PPT) of the masticatory muscles and on the subjective pain report of dental students of the Bauru School of Dentistry (University of São Paulo, Brazil) at different situations. ▼ Este trabalho objetivou avaliar a prevalência de subgrupos da Disfunção Temporomandibular (DTM) e hábitos parafuncionais orais em indivíduos portadores de zumbido subjetivo, sendo comparados com indivíduos sem zumbido. Duzentos indivíduos, de ambos os gêneros, com idade entre 18 a 60 anos, participaram do estudo. Inicialmente, foram subdivididos, com auxílio do exame otorrinolaringológico, em grupo experimental (n=100), indivíduos portadores de zumbido subjetivo, e grupo controle (n=100), voluntários sem qualquer queixa de zumbido. A análise da prevalência de DTM, bem como o estudo dos seus sinais e sintomas, foi feita segundo os critérios de diagnóstico Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD). O Limiar de Dor à Pressão (LDP) dos músculos masseter e temporais (anterior, médio e posterior) foram medidos bilateralmente, por meio de um algômetro KRATOS®, e a dor subjetiva pela Escala de Análise Visual (EAV). O estudo da presença dos hábitos parafuncionais orais, apertamento dentário diurno e bruxismo do sono, foi feito através do auto-relato e da aplicação de exame clínico de bruxismo, respectivamente. Os dados obtidos foram submetidos à análise estatística (Testes Qui-Quadrado, t de Student e Mann-Whitney e Coeficiente de Correlação de Spearman), em um nível de significância de 5%. Foi encontrada uma maior prevalência de DTM no grupo experimental quando comparado ao grupo controle (p<0,05). De uma maneira geral, os sinais e sintomas de DTM avaliados foram mais prevalentes no grupo experimental em comparação ao grupo controle, como a presença de dor à palpação na ATM e de ruídos articulares na abertura bucal, maior tempo de dor e maior grau de severidade de dor crônica. O resultado médio da EAV encontrado foi estatisticamente maior no grupo experimental (p<0,05), contudo os LDPs não apresentaram diferença significativa entre os grupos. A análise dos hábitos parafuncionais mostrou diferença estatisticamentente significativa da presença de bruxismo noturno entre os grupos, sendo mais prevalente no grupo experimental (p<0,05). Houve uma correlação positiva estatisticamente significativa entre a severidade do zumbido e a severidade da dor crônica (p<0,05) e entre o tempo de zumbido e o tempo de dor (p<0,05). Conclui-se que parece existir uma forte correlação entre os sinais e sintomas da DTM e o zumbido de caráter subjetivo.
The aim of this study was to study the prevalence of Temporomandibular Disorders (TMD) subgroups and oral parafunctional habits, daytime clenching and sleep bruxism, in patients with subjective tinnitus when compared to a group of asymptomatic volunteers. ▼ Background: Pain is a primary clinical concern for most people. Pain is the most common reason for seeking any form of health assistance be it medical, dental, physiotherapeutic or alternative disciplines. Pain threshold is defined as the lowest application of a stimulus that is perceived as pain. Experimental pain studies use a range of pain challenges including electrical, heat or cold, ischaemic and pressure. Some carry a higher potential risk of tissue injury or the sensations experienced are less acceptable to subjects. Pressure pain threshold (PPT), measured by a simple mechanical algometer is an attractive alternative well-suited for non-invasive repeated measurements on multiple sites not limited to limbs over short time intervals in a relaxed setting. Since 2000, the University of Technology Sydney had conducted eight PPT studies and collected over 47,500 baseline PPT measurements on 262 healthy subjects at 24 regional sites with three or four PPT readings for each site at each session of four to eight occasions of at least one week apart. Research Study One included seven studies with over 32,000 pre-intervention PPT measures on 235 healthy subjects at 17 sites with three PPT measures at each occasion for four consecutive occasions. These data were being analysed to develop comprehensive epidemiological profiles that assess relationships between PPT with subject variables (gender, age, BMI) and duration of temporal sessions. Research Study Two assessed the PPT at two affected and two non-affected sites of 20 patients with lateral epicondylitis. Research Study Three examined the inter-device reliability between mechanical and electronic algometers at six sites of 17 subjects.
Aims: Research Study One explored the temporal stability of possible relationships between subject variables of gender, age and BMI, the duration of temporal sessions with the regional PPT at each measurement site. Research Study Two assessed the regional PPT measures at LI10 and LI11 of the affected and non-affected elbows for subjects with lateral epicondylitis. Research Study Three examined the inter-device reliability of a mechanical and an electronic algometers of same measurement parameters: circular rubber plunger of 1cm2 and force application rate of 1kg/s.
Methods: Research Study One: All studies used the same protocol including the same model algometer, tip dimensions, application rates, rest interval between measurement cycles and at least seven days between each of four data collection visits. Regional PPT measurement sites included sites on head, neck and limbs. Data analyses used GLM and the alternative non-parametric tests wherever applicable. ▼ La manipulation vertébrale est connue pour son effet clinique sur les douleurs musculo squelettiques. Or, ses mécanismes d’action sur la douleur sont encore peu connus. C’est pourquoi ce projet doctoral a cherché à déterminer l’existence d’un effet spécifique sur le seuil de la douleur à la pression, chez des sujets asymptomatiques, et s’il existe, d’en préciser sa taille et sa durée. Nos deux revues systématiques et critiques de la littérature ont mis en évidence la présence d’un effet régional spécifique de la manipulation vertébrale sur cette perception douloureuse dans la population asymptomatique, avec une taille de l’effet moyenne et de courte durée. Cependant, ces résultats sont en contradiction avec notre essai expérimental sur l’effet d’une manipulation vertébrale thoracique contrôlé par placébo dans cette population. Cette différence s’explique par la qualité de la manœuvre placébo, avec la vérification de l’aveuglement réussi durant l’expérimentation par un questionnaire. En conclusion, nos travaux n’ont pas réussi à reproduire un effet spécifique de la manipulation vertébrale sur le seuil de la perception douloureuse à la pression, suggérant que les mécanismes d’action de la manipulation sont similaires à ceux impliqués dans une manoeuvre placebo. Par conséquent, l’importance principale de ce travail est d’avoir montré l’importance de l’effet placebo et comment les résultats d’un essai peuvent dépendre de la manière dont l’intervention placébo est réalisée et évaluée.
Spinal manipulation is known for its clinical effect on musculoskeletal pain. Or, its mechanisms of action on pain are still little known. This is why this doctoral project sought to determine the existence of a specific effect on the threshold of pressure pain, in asymptomatic subjects, and if it exists, to specify its size and its duration. Our two systematic and critical reviews of the literature have highlighted the presence of a specific regional effect of vertebral manipulation on this painful perception in the asymptomatic population, with a medium effect size and short duration. However, these results contradict our experimental trial on the effect of placebo-controlled thoracic spinal manipulation in this population. This difference can be explained by the quality of the placebo maneuver, with the verification of successful blindness during the experiment by a questionnaire. In conclusion, our work has not succeeded in reproducing a specific effect of vertebral manipulation on the threshold of pain perception at pressure, suggesting that the mechanisms of action of manipulation are similar to those in a placebo maneuver. Therefore, the main importance of the work is to have shown the importance of the placebo effect and commenting on the results of a trial may depend on the way the placebo intervention is performed and indicated.

▼ Uvod: Radikulopatija je obično praćena bolovima i drugim senzornim i motornim poremećajima, uz smanjenje kvaliteta života u različitom obimu. Algometrija kao visokosenzitivna metoda pruža objektivan uvid u stepen bola, dok se upotrebom upitnika na jednostavan način mogu proceniti karakteristike bola i biopsihosocijalni status pacijenta. Cilj: Istraživanje je sprovedeno kako bi se izmerili prag bola i prag tolerancije na bol kod pacijenata sa cervikalnom i lumbalnom radikulopatijom i utvrdila moguća povezanost bola sa biopsihosocijalnim faktorima. Materijal i metode: Studijom je pre započinjanja i posle završavanja terapijskog ciklusa ispitano 60 pacijenata sa dijagnostikovanom cervikalnom radikulopatijom (30 muškaraca i 30 žena) i 60 pacijenata sa dijagnostikovanom lumbalnom radikulopatijom (30 muškaraca i 30 žena). Svi pacijenti su bili hospitalno lečeni u okviru Klinike za medicinsku rehabilitaciju, Kliničkog centra Vojvodine u Novom Sadu, a terapijski ciklus je u proseku trajao 14-21 dan. U istraživanju su korišćenitest za detekciju bola (Pain Detect Test), kratki upitnik o bolu (Brief Pain Inventory), indeks onesposobljenosti zbog bolova u vratu (Neck Disability Index), Kvebekova skala onesposobljenosti kod lumbalnog sindroma (Quebec Back Pain Disability Scale), bolnička skala za anksioznost i depresiju (Hospital Anxiety and Depression Scale), upitnik za procenu prisustva straha od fizičke aktivnosti/posla i njihovog izbegavanja (The Fear-Avoidance Beliefs Questionnaire) i skala katastrofizma bola (Pain Catastrophizing Scale). Rezultati: Nije uočena statistički značajna razlika algometrijskih vrednosti između pacijenata sa cervikalnom radikulopatijom i pacijenata sa lumbalnom radikulopatijom. Ustanovljeno je da osobe ženskog pola imaju niži prag bola i nižu toleranciju na bol od osoba muškog pola. Poređenjem algometrijskih vrednosti pre započinjanja i posle završavanja terapijskog ciklusa zapaža se da je program rehabilitacije povoljno uticao na pacijente sa lumbalnom radikulopatijom, dok je kod pacijenata sa cervikalnom radikulopatijom došlo do pogoršanja tegoba. Prisutna je negativna korelacija između vrednosti izmerenih algometrom i skorova za anksioznost, depresiju i strah od fizičke aktivnost i posla, što znači da biopsihosocijalni faktori u velikoj meri utiču na bolnost. Zaključci: Kvantifikovanje i mapiranje bola uz pomoć algometra i utvrđivanje biopsihosocijalnog statusa putem upitnika će omogućiti primenu adekvatne terapije kod pacijenata, koja je zasnovana na individualnom pristupu. Istovremeno bi primenjenom metodologijom bila postignuta bolja verifikacija rezultata rehabilitacionog programa.

▼ Introdução: Os pontos gatilho (PG) do esternocleidomastóideo (ECM) podem ser a causa de
dor na face e no crânio. A técnica músculo-energia (TME) pode ser utilizada na presença de
PG. Objectivo: Verificar qual o efeito imediato da TME, aplicada no ECM, na sensibilidade
dolorosa à pressão (SDP) do PG do ECM e nas amplitudes cervicais em comparação com uma
técnica placebo. Metodologia: Uma amostra voluntária de 52 indivíduos foi dividida
aleatoriamente por dois grupos. Inicialmente foi medida a SDP e as amplitudes dos
movimentos activos da coluna cervical. Após a aplicação da TME, com 20% da força máxima,
e da técnica placebo, nos respectivos grupos, a SDP e as amplitudes cervicais foram
reavaliadas. Resultados: Não existiram diferenças estatísticas significativas para afirmar que
os dados recolhidos antes e depois da aplicação da TME eram significativamente diferentes.
Conclusão: Os efeitos imediatos da TME, neste estudo, não foram significativos. No entanto, a
bibliografia aponta noutro sentido, tornando-se importante perceber de que forma podemos
melhorar a aplicação da TME, de forma a optimizar os seus efeitos.
Background: The sternocleidomastoid (SCM) trigger points (TrP) may be the cause of pain in
the face and skull. The muscle energy technique (MET) can be used in the presence of TrP.
Aim: Investigate what the immediate effect of MET, applied in the SCM, in the pressure pain
threshold (PPT) of SCM TrP and cervical range of motion (ROM) compared to a placebo
technique. Methods: A voluntary sample of 52 subjects was randomly divided into two
groups. Initially the PPT and the cervical active ROM were measured. After applying the
MET, with 20% of maximum strength, and placebo technique, in their respective groups, the
PPT and cervical ROM were reevaluated. Results: There were no significant statistical
differences to assert that the data collected before and after the application of MET were
significantly different. Conclusion: The immediate effect of MET, in this study, wasn’t
significant. However, the bibliography points in another direction, making it important to
understand how can we improve the MET application in order to optimize its effects.

▼ A preliminary investigation of
pressure pain sensitivity and the vascular effects of ACL injury
with subsequent surgical reconstruction was carried out using a non-randomized, cross-sectional approach. One investigational visit was conducted on 15 subjects greater than six months status-post ACL reconstruction, while 14 subjects without lower extremity injury served as controls. All participants were between 18-40 years old and free of cardiovascular disease. Information on
demographics, physical activity, medical history, and LE injury was collected for both groups.
Pressure pain threshold with
pressure algometry and arterial vascular dilation with
ultrasonography (flow-mediated dilation) were assessed in both the upper and lower extremities.
Hyperalgesia and impaired FMD was observed in the ACL group compared to the control.
This is a novel finding in human research and as developed can improve the understanding of
post-traumatic knee OA and a possible link to cardiovascular disease. Significant hyperalgesia
was observed local and contralateral to the ACL reconstruction. A trending pattern of
hyperalgesia was also observed at tibial measurement sites. FMD comparisons between groups
reached statistical significance, while, interestingly the greatest impairment in FMD occurred in
the popliteal artery contralateral to a unilateral ACL reconstruction. This contralateral arterial
impairment likely represents an imbalance between local inflammatory factors and spinally
mediated reflexes effecting smooth muscle control. Future research is recommended to expand
on the current project with larger sample sizes, more extensive investigation of inflammatory
cytokines, serum or urine analysis of type II cartilage byproducts and a more detailed analysis of
arterial function.

▼ O objetivo deste trabalho foi analisar a influência do ciclo menstrual nas alterações de limiar de dor à pressão (LDP) na musculatura mastigatória de mulheres com sinais e sintomas de Disfunção Temporomandibular (DTM). Inicialmente 47 voluntárias entre 18 e 40 anos participaram do estudo, das quais 36 foram incluídas no experimento: 15 com sinais e sintomas de DTM (7 sob terapia com contraceptivos orais (CO) e 8 sem CO) e 21 saudáveis, sem sinais e/ou sintomas de DTM (8 com CO e 13 sem CO). Os LDPs dos músculos masseter e temporais (anterior, médio e posterior), e do tendão de Aquiles foram medidos bilateralmente, por meio de um algômetro, durante 2 ciclos menstruais consecutivos, nas 4 diferentes fases: menstrual (dias 1-3), folicular (dias 5-9), periovulatória (dias 12-16) e lútea (dias 19-23). Em cada fase do ciclo, as voluntárias relataram sua dor em uma Escala de Análise Visual (EVA). Os resultados foram submetidos à análise de variância a 3 critérios para mensurações repetidas, a um nível de significância de 5%.Foram encontrados LDPs significativamente menores nos músculos temporal e masseter e no tendão de Aquiles das mulheres com DTM quando comparado às mulheres assintomáticas, independentemente da fase do ciclo e do uso de contraceptivos (p < 0,05). De uma maneira geral, os LDPs foram maiores em mulheres em terapia com contraceptivos orais, quando comparado às mulheres sem terapia. Parece não existir influência das diferentes fases do ciclo menstrual no LDP, independentemente da presença ou não de DTM.
The aim of this study was to investigate the influence of the menstrual cycle on the Pain Pressure Threshold (PPT) figures of the masticatory muscles in women with signs and symptoms of Temporomandibular Disorders (TMD). Forty-seven volunteers (ages between 18-40 years-old) were initially recruited for this purpose. According to the criteria adopted, 36 were included. The experimental group was composed of 15 women with myofascial pain (RDC/TMD) (7 under oral contraceptive medication), while 21 women with no TMD signs or symptoms (8 under oral contraceptive medication) composed the control group. The PPT values of masseter and temporalis (anterior, middle, and posterior regions) muscles, as well as the Achilles? tendon were bilaterally screened during two consecutive menstrual cycles, in the following phases: menstrual (day 1-3), follicular (day 5-9), periovulatory (day 12-16) and luteal (day 19-23). A visual analog scale (VAS) was used to address subjective pain in each menstrual phase. Data were submitted to 3-way ANOVA for repeated measurements, with a 5% significant level. The PPT values were significantly lower in the temporalis, masseter, and the Achilles? tendon of TMD patients when compared with the asymptomatic controls, regardless of the menstrual cycle phase or the use of oral contraceptives (p<.05). ▼ Introdução: A osteoartrite de joelhos (KOA) apresenta alta prevalência, principalmente em mulheres. Com o envelhecimento da população esta prevalência irá aumentar. Os tratamentos conservadores apresentam limitada eficácia em expressivo número de pacientes no curso do tratamento . A cirurgia de protetização apresenta altos custos, possibilidade de complicações pós-operatórias graves e ainda que a correção anatômica seja perfeita, em torno de 20% dos pacientes persistem com dor crônica pós-operatória. Portanto, é preciso avançar no conhecimento dos mecanismos fisiopatológicos e estudar novas abordagens terapêuticas para agregar às existentes, visando melhor manejo da dor e para restabelecer a função de maneira mais efetiva. Estas questões motivaram três questões centrais que origiram os três estudos que compõem esta tese. Estudo I: No primeiro estudo avaliamos os mecanismos pelos quais há perpetuação da dor na KOA. Para responder a esta questão buscou respostas aos seguintes objetivos: I) Comparar se a função da via da dor inibitório descendente está associada com o estado de inibição no sistema corticospinal, indexado pelo potencial evocado motor (MEP) e o período de silêncio cortical (CSP) em pacientes com KOA e controles saudáveis. II) Determinar se há correlação entre as medidas de inibição intracortical (CSP, MEP) com alterações na escala de dor numérica (NPS 0-10) na KOA durante a tarefa de modulação condicionada de dor (CPM-task) considerando o efeito da capacidade funcional auto-relatada avaliada pelo Western Ontário and McMaster Universities Index (WOMAC) e uso de analgésicos. Métodos: Estudo transversal, foram incluídas 21 pacientes femininas com KOA e 10 controles saudáveis com idade entre 19 a 75 anos. Os parâmetros de excitabilidade do córtex motor (MEP e CSP) foram avaliados utilizando a estimulação magnética trasncraniana (EMT). Avaliação de dor e a incapacidade pelo WOMAC e a NPS (0-10) durante a CPM-task. Resultados: A média ajustada (DP) do CSP observada em pacientes com OA foi 23,43% menor do que em indivíduos saudáveis [54,54 (16,10) vs. 70,94 (22,87)], respectivamente (P = 0,01). A função do sistema modulador descendente de dor avaliado pela alteração do NPS (0-10) durante o CPM-task foi negativamente correlacionada com o parâmetro de excitabilidade cortical indexado pelo CSP (P = 0,001). O CSP foi negativamente correlacionado com a dor e incapacidade avaliada pelo índice WOMAC. Conclusão: Foi observado um sistema inibitório descendente de dor enfraquecido, corroborando com os achados em outras patologias de dor crônica. To measure the immediate effects of a brief mindfulness body scan meditation on self-reported
pain, the nociceptive system and autonomic nervous system.
Study design
A between-
subject, repeated measures, double-blinded, randomised controlled trial, with experimental and control interventions. A 10-minute intervention period was followed by a 15-minute rest period.
Participants
Thirty adults with chronic
pain (7 men and 23 women) recruited through advertising in local papers, web-based social media and professional networks.
Interventions
The experimental group followed a 10-minute audio recording of a mindfulness based body scan meditation. The control group listened to a 10-minute audio recording of text from an audio book in a pleasant, friendly voice whilst sitting quietly.
Main measures
The primary dependent variable for self-reported
pain was rating of
pain severity on a visual analogue scale. The primary dependent variables for nociception were:
pressure pain threshold recordings at a painful site and
pressure pain threshold recordings at a non-painful site. The primary dependent variables for the autonomic nervous system were: mean heart rate, heart rate variability, heart rate variability low frequency to high frequency power ratio, and skin conductance.
Results
There were no statistically significant differences between the group that listened to the experimental mindfulness tape and the group that listened to the control tape on any of the outcome measures.
Conclusion
In people with chronic
pain, a brief mindfulness body scan meditation has no effect on rating of
pain severity on a visual analogue scale,
pressure pain thresholds, mean heart rate, heart rate variability, heart rate variability low frequency to high frequency power ratio, or skin conductance when compared to a control group. Further research is required before determining whether brief mindfulness interventions are helpful in people experiencing chronic
pain.

▼ Pain and movement are intimately connected and nearly universal human experiences. However, our understanding of the extent, significance, and mechanisms of
pain-movement relationships is limited. While
pain is a normal, protective response to injury and potentially harmful stimuli, prolonged or dysfunctional neuromuscular adaptions in response to
pain can contribute to a variety of
pain conditions. Alternatively, movement (in the form of global physical activity, individual exercise programs, and/or specific motor learning/functional tasks) is often prescribed to help decrease
pain and improve function. While attempts have been made to show an effect of movement on
pain or to better understand altered movement strategies in response to
pain, much of the research has been limited to animal models or to those with specific persistent or chronic
pain conditions limiting generalizability and interpretability. Therefore, this research sought to advance current understanding of the relationships between physical activity and normal variability in centrally- and peripherally-mediated
pain in healthy adults. Additionally, we sought to characterize changes in reflexive motor responses in the upper extremity to an endogenous, naturally-occurring, long-lasting acute muscle
pain.
The results of these investigations indicate that greater, self-reported intense (i.e. vigorous) and leisure activity are more strongly associated with decreased
pain sensitivity than is
pain modulation or measured activity (via accelerometry). Future research is needed to determine directionality of these relationships. Further, reflexive motor responses to endogenous, acute muscle
pain in the upper extremity were not significantly altered indicating that changes in
pain-related, movement strategies may be more strongly influenced by supraspinal adaptations. These results may have value in improving understanding of
pain-related, movement sequelae and directing future research in this area.

▼ Background: The two separate studies reported in this thesis represent the third and fifth in an ongoing series that commenced at the College of Traditional Chinese Medicine (TCM), University of Technology Sydney (UTS) in 1999 into the effect of acupuncture on regional pressure pain threshold (PPT). The studies extended manual acupuncture research and were the first to include a study involving acupressure.
Aims: Study I: To compare the effects of unilateral and bilateral needling of the same acupoint on regional PPT. In addition the effects of individual and combined needling of two distinct acupoints on regional PPT were examined. Study II: To compare the effects of manual acupuncture and acupressure on regional PPT.
Methods: The design was used in both studies that were dual blind within subjects experimental design with randomised repeated measures. There were 22 healthy subjects (11 males and 11 females) in study I and 24 healthy females in study II, and all subjects completed the four interventions used in their study. For Study I, the same needle application technique was used in four manual acupuncture interventions: Large Intestine 4 (LI4) unilaterally; LI4 bilaterally; Large Intestine 11 (LI11) unilaterally; and LI4 in conjunction with LI11 both unilaterally. For Study II, unilaterally applied manual acupuncture to LI4; to Spleen 4 (SP4); unilaterally applied acupressure to LI4 and to a nonacupoint (NAP) used in two previous studies at UTS. PPT was measured at ten regional measurement sites across the body before and after each intervention; the change was expressed as mean percentage change in PPT from preintervention to post intervention. Both within and between intervention comparisons were examined; visual analogue scales were used to measure participants' perceptions of pain; needling or acupressure sensations; tension, and anxiety; and changes in acupuncturist’s behaviour.
Results: Study I: following all four interventions, statistically significant increases in mean PPT were observed. These occurred at nine sites following LI4 either unilaterally or bilaterally; at six sites for the LI 11 intervention; and at five sites following the combined LI11 and LI4 intervention. These increases were significantly greater for the bilateral LI4 intervention than the unilateral LI4 intervention at only two sites. There were no statistically significant differences in the subjective perceptions among the four interventions. Study II: statistically significant increases in PPT were elicited by both acupressure interventions and acupuncture to SP4. There was minimal difference in the effectiveness of these three interventions. Surprisingly, acupuncture to LI4 was significantly less effective and only produced statistically significant elevations in mean PPT at three sites. The data and design were examined extensively to identify a cause for this finding which was at odds with those from study I and also from the four related studies of PPT and acupuncture to LI4 conducted at UTS. ▼ Prolonged standing is common in the workplace, and is a cause of
pain and discomfort in the feet, legs, and lower back. Anti-fatigue mats are often used in industry to reduce discomfort resulting from prolonged standing. However, there are currently no means for predicting the effectiveness of a particular mat in mitigating discomfort, and there is little understanding of how discomfort develops during prolonged standing. The main objective of this research was to investigate the cause of discomfort during prolonged standing, and the role of mats in reducing discomfort.
The effect of flooring material properties on discomfort and behavior was investigated during four-hour periods of standing. Touch sensitivity of the foot was also measured before and after standing to test for changes in sensitivity. Plantar
pressure was investigated as a potential mechanism for discomfort during standing by testing the
pain-
pressure threshold at different levels of constant pressures on the foot. Finally, the effects of flooring and shoes on plantar
pressure were studied.
During prolonged standing, mats reduced discomfort when compared to a hard floor, but no differences in discomfort were observed among mats. During standing, weight-shifting was correlated to discomfort and demonstrated lower statistical variance than subjective ratings of discomfort. These findings suggest that weight-shifting may offer a more sensitive measurement for discomfort than subjective ratings. Finally, as a result of prolonged standing, touch sensitivity of the feet increased, suggesting potential unintended bias in clinical touch sensitivity testing.
During
pain-
pressure threshold testing, elevated levels of constant plantar
pressure were associated with an earlier onset of
pain, indicating that plantar
pressure is a mechanism that contributes discomfort. Flooring significantly affected plantar
pressure during barefoot standing and walking, but when shoes were worn the effects of flooring on
pressure were very small or not significant. This suggests that for reducing plantar
pressure, attention should be focused on the design of footwear rather than the design of mats. Because flooring does not affect plantar
pressure, mats must reduce discomfort through some other unknown mechanism.
