▼ L'objectif de l'étude était de comparer les
analyses de cas chassé-croisé (CCO) et de cas-contrôle du temps
(CCT) afin d'évaluer la relation entre l'usage de prescriptions et
le risque de blessures traumatiques en utilisant des bases de
données populationnelles incluant les données du Registre de
Traumatisme de Québec, d'hospitalisation du MED-ECHO et la RAMQ.
Les cas ayant rapporté une blessure entre janvier-2000 et
décembre-2000 ont été inclus dans l'analyse. Pour chaque cas, un
contrôle a été apparié sur l'âge, le sexe, le niveau du centre de
traumatologie et la date de sortie de l'hôpital. 2,417 cas et 2,417
contrôles ont été appariés dans l'étude. Les résultats ont démontré
que l'analyse de CCO est 1,5 fois plus susceptible de produire une
association négative pour le risque de blessure comparativement à
l'analyse CCT. Un faible niveau d'accord (62.4%) sur les
estimations des risques a également été évalué par les deux
méthodes.

▼ Prescription drug abuse has reached epidemic proportions and trends indicate that the number of adolescents who abuse these drugs will continue to increase. Every adolescent appear to be at risk and this epidemic does not appear to be defined by known risk profiles for substance abuse. Nearly one in every five 12th grade students used
prescription drugs for non-medical reasons at least once in lifetime according to the Monitoring the Future study. This is a significant public health problem as
prescription drug abuse in the adolescence can continue into the adult life and puts adolescents at risk of substance abuse and other comorbidities. The purpose of this study was to identify individual, family, school and community level factors responsible for the initiation and maintenance of
prescription drug abuse behaviors in Illinois 12th grade students.
A cross-sectional study design was used with the secondary data on the 12th graders’
prescription drug use from the 2010 Illinois Youth Survey. Multilevel logistic regression was performed to account for the multistage sampling and hierarchical structure of the data, using software such as HLM, STATA, and SPSS. Abuse of any
prescription drug and of subclasses such as uppers, downers, steroids and other drugs were the dependent variables. Individual level variables such as age, gender, race, socioeconomic status, having clear college/future plans, other substance/over the counter (OTC) drug abuse, depression, gambling, family rules about alcohol and drug use, perceived peer drug use; school level variables such as the percent of -White students, students in the low socioeconomic status category, students reporting bullying, students reporting unsafe at school; and school district level variables such as community norms towards substance abuse, community activities, safety, location and availability of alcohol, tobacco and other drugs (ATOD) were examined for their influence on
prescription drug abuse in Illinois 12th graders.
Results indicate that Illinois 12th graders (11.4%) had a lower prevalence than the US prevalence (15.4%) in the year 2009. After adjusting for all the other variables in the multilevel logistic regression model, Illinois 12th graders with past year ATOD use (Odds ratio (OR): 6.095, 95% CI: 2.708,13.717), past year OTC drug use (OR: 6.081, 95% CI: 3.952,9.358), past year depression (OR: 2.381, 95% CI: 1.475, 3.844), and past year gambling (OR: 1.684, 95% CI: 1.069, 2.655) were found to have significantly higher odds of
prescription drug abuse in 12th graders. Other individual level variables and all school/ school district level variables were found to be not significantly associated with
prescription drug abuse in 12th graders.
Prescription drug abuse in Illinois 12th graders was significantly different from prevalence in US 12th graders and was found to be significantly associated with gender, past year ATOD use, OTC use, gambling, depression, and perceived peer drug use. Most of the variation in

Introduction: Drugs are important components of health care and play crucial role in saving life.
Drug
prescription is the product of most medical consultations. Despite the importance of good
quality prescriptions erroneous prescribing habits are not uncommon worldwide. Illegible or incorrect
prescription can result in loss of the patient, physician and pharmacist time and may cause therapeutic
error or drug toxicity. Hence the objective of this study was to assess the nature of
prescription
deficiencies (omission and error) in production of
prescription with regard to Patient identification,
drug information, prescriber’s identification.
Methods: This was a prospective, cross- sectional hospital based study. All prescriptions issued to
non admitted patients below 14 yrs of age visiting pediatric regular OPD, emergency and follow up
clinics were collected from pediatric emergency and special clinic every day and assessed for their
quality based on the nationally adopted format approved by Food Medicine and Healthcare
Administration and Control Authority . Data was collected from May 1 to 31/2014 until the desired
sample size is reached. Data was analyzed by SPSS for windows version 20.
Results: Total number of
prescription studied was 246. With regard to patient identification, 78% of
prescriptions were lack patient age, 80.5% sex and 86.2% patient weight. 50.8% of the prescriptions
were without drug formulation and 19.9% drug generic name. 30% of the prescriptions were issued
with erroneous dose. Prescribers name was not documented in 45.5% of the prescriptions and date of
prescriptions issued was not written in 65% of the prescriptions. Based on the predefined ….
Conclusion and recommendation: From this study production of
prescription quality is
unsatisfactory as most of the prescriptions lack the essential elements. Patients are not well identified
drug information is incomplete and prescribes identification is lacking in most of the prescriptions
studied. And most of the prescriptions with poor quality are from clinics attended by senior residents.
As a specialized teaching hospital the department should device corrective measure to enhance
prescription quality, the hospital has to have
prescription error reporting system and this calls for
medication error study to evaluate the impact of
prescription quality in the patient care.

que les patients avec une maladie artérielle périphérique (MAP)
soient traités médicalement afin de réduire la survenue
d’évènements cardiovasculaires majeurs. Objectif : Identifier les
facteurs associés à la prescription des thérapies préventives
recommandées. Méthode : Les patients avec une MAP (n=362) traités
consécutivement par angiodilatation fémoropoplitée entre 2008 et
2010 dans un centre tertiaire (CHU de Québec, Canada) ont été
inclus dans l’étude. L’issue clinique primaire était la
prescription de trois thérapies combinées. Résultats : Au total,
52% des patients recevaient la thérapie combinée. La présence d’au
moins trois facteurs de risque cardiovasculaire (Rapport de cotes
(RC)=4,51; IC 95% : 2,76-7,37) était le facteur le plus fortement
associé à la prise des thérapies combinées. Conclusion : La prise
en charge du risque cardiovasculaire des patients avec une MAP est
encore sous-optimale. Une meilleure compréhension des barrières et
des facilitateurs à l’application des recommandations est toujours
nécessaire.
Introduction: Guidelines recommend that patients
with peripheral arterial disease (PAD) should be medically treated
to reduce the occurrence of serious cardiovascular events.
Objective: To identified factors associated with the prescription
of preventive therapies in patients with PAD. Methods: Consecutive
patients with PAD (n=362) who underwent peripheral percutaneous
transluminal angioplasty between 2008 and 2010 in one tertiary care
center (CHU de Quebec, Canada) were considered for the study. Data
were collected from the medical charts. The main outcome was the
combined prescription of three therapies. Results: A total of 52%
of the patients received the three combined therapies. Having at
least three cardiovascular risk factors (Odds Ratio (OR)=4.51; 95%
CI: 2.76-7.37) was the factor most strongly associated with the
prescription of the combined therapies. Conclusion: Canadian
patients with PAD are still not optimally managed. We still need to
better understand the barriers and facilitators to the application
of the guidelines in PAD patients.

▼ Pushing, pulling,
lifting and carrying items are performed routinely in
many trades within a military environment, contributing
to 41% of all manual handling tasks (Jones et al., 2000;
Sharp et al., 2009). These strength‐related tasks place
significant physical demands upon the soldier (Sharp et
al., 1993). However, unlike other occupations, these task
demands cannot be re‐engineered or reduced. In order to
adequately meet this diverse range of task demands,
soldiers must have sufficient physical capacity (Sharp et
al., 2009) . Currently, the ARMY undertake 36% of
training time in endurance running, swimming and pack
marching and 18% of training time performing non‐specific
resistance circuit training within the 80‐day, 12‐week
Basic Military Training regimen (Orme, 2005).
The distribution of time dedicated to strength
training does not match the significant strength daily
trade demands (Hendrickson et al., 2010). The actual
transfer of adaptations following Basic Military Training
regimen to improve occupational performance is currently
unknown. Therefore, the aim of this investigation was to
observe the effect of a modified version of the Basic
Military Training Program on occupational performance
(maximal box lift, maximal pack lift and repetitive
lift), generic strength assessments (6RM squat, 1RM bench
press, isometric and isokinetic knee extension/flexion,
grip strength), aerobic power (VO2peak), agility
(Illinois agility course), power (vertical jump) and
current military assessments of physical capacity (2.4 km
run, push ups and sit ups) in comparison to an
experimental training regimen. Forty
Six (33F;13M) university students were randomised into
two training regimen; military training (MT) and
experimental training (ET). Three training sessions were
performed for an 8‐week period. There were marked volume
(time) differences in endurance between MT and ET (75%
versus 15%) and strength (25% versus 85%) components of
training, but with three sessions of one hour performed
per week, total training time was matched.
Following the training intervention, no
significant interaction was observed for occupational
performance, however there was a significant interaction
observed for 6RM squat performance (P= 0.0001).
Interestingly, despite an 87% difference in endurance
volume (training time), there was no significant
interaction observed for 2.4 km endurance run time,
regardless of VO2peak. The results obtained from this
investigation reveal the effectiveness of high intensity
training conducted within a concurrent setting…

▼ The direction and intensity of consumer attitudes toward
prescription
prices by varying socio-economic positions and age groups
were measured by use of the semantic differential technique. The
potential relationships between consumer attitudes toward
prescription
prices and (1) consumer
prescription experience, (2) select consumer
knowledge, (3) select consumer beliefs, and (4) a consumer
value were investigated.
A random sample of 150 families was selected and the wife
within each family group was interviewed utilizing a prepared questionnaire.
The respondents were grouped into nine family classes by
varying socio-economic positions and age groups, and were
dichotomized according to varying family
prescription experience,
select respondent knowledge, and select respondent beliefs.
The results indicated that the general direction of respondent
attitudes toward
prescription prices was negative in all family
classes regardless of socio-economic position or age group.
The results were determined as showing no significant differences
between attitude means of respondents representing families
with above and below average
prescription experience in terms of
frequency and expenditures.
The results were determined as showing that respondents with
basically a correct knowledge of
prescription prices and
prescription
pricing procedures had significantly more negative attitudes toward
prescription prices than respondents with basically an incorrect
knowledge.
The results were determined as showing that respondents who
accepted selected beliefs concerning (1) physician rebates, (2) the
average
prescription price, and (3) pharmacy profits, generally had
significantly more negative attitudes toward
prescription prices than
respondents who did not accept the selected beliefs.
The results indicated that the direction of respondent attitudes
toward the value of
prescription drugs in maintaining health was intensely
positive regardless of the family class observed.

▼ Prescription opioids are a highly prescribed class of medication, which when administered over long durations can lead to negative effects (e.g., addiction, non-medical use).
Prescription opioids are often prescribed concomitantly with antihyperglycemic medication among patients with T2DM. Due to known associated negative effects of
prescription opioids, it is imperative to study the impact of long term opioid use of antihyperglycemic medication when prescribed concomitantly. Therefore, the primary aim of this study was to examine if there is any effect of long-term opioid use on antihyperglycemic medication adherence when the two medication classes are prescribed concomitantly among patients with T2DM. Additionally, this study also examined whether the duration of opioid use and antihyperglycemic adherence has any potential impact on subsequent diabetes related hospitalizations.

▼ Syftet med studien var att undersöka om vårdpersonal använder fysisk aktivitet på recept(FaR) som behandling till personer med depression. Frågeställningar: Ordinerar vårdpersonal FaR som behandling till personer med depression?Arbetar vårdpersonalen utifrån Socialstyrelsens Nationella riktlinjer (2010) gällande patienter med depression? Metod Studien är gjord utifrån en kvalitativ metod och analyserad utefter en innehållsanalys. Intervjumetoden är av semistrukturerad art med en frågeguide på elva öppna frågor som underlag. Av de fyrtio kontaktade verksamhetschefer som i sin tur gick ut och frågade sina anställda om de var någon som ville ställa upp så intervjuades fyra stycken respondenter. Alla fyra informanterna var utbildade läkare inom allmänmedicin eller psykiatri som antingen arbetade inom primärvården (3) eller som specialist inom psykiatrin (1). Resultat Resultaten visar att alla intervjupersoner var positivt inställda till fysisk aktivitet som del i behandling till personer med depression. Även om själva FaR metoden ansågs som ett trubbigt och komplicerat verktyg så tyckte informanterna ändå att det gav positiva resultat för deras patienter att komma igång och aktivera sig. Det framkom att enklare budskap till vårdpersonal gällande de nationella riktlinjerna möjligen kan öka användningen av FaR för deprimerade inom hälso- och sjukvården, och på så sätt bidra till att fler patienter uppnår en förbättrad psykisk hälsa. I analysen framkom att de flesta hade många års erfarenhet av att arbeta med deprimerade. Slutsats Det kan rekommenderas att se över bristande rutiner i uppföljning med FaR för patienter som har depression samt en förenkling av journalinförandet och det digitala receptet. Med grund i denna studies resultat kan man dra slutsatsen att Socialstyrelsens nationella riktlinjer gällande personer med depression behöver lyftas fram och implementeras ytterligare i den svenska hälso- och sjukvården.

prescription d’antidépresseurs chez les personnes âgées. Au Canada, l’adéquation de la
prescription d’antidépresseurs dans la population des personnes âgées est peu étudiée. En médecine générale, près de la moitié des personnes âgées ayant une dépression ne reçoivent pas une
prescription appropriée d’antidépresseurs. Le but de ce mémoire était (1) : déterminer la proportion des personnes âgées qui reçoivent une
prescription d’antidépresseurs selon les recommandations cliniques canadiennes et (2) identifier les facteurs liés au patient et à l’organisation des services susceptibles d’influencer la
prescription adéquate d’antidépresseurs chez les personnes âgées qui consultent dans les services de médecine générale. Pour répondre à cet objectif, une étude a été réalisée à partir des données de l'enquête transversale (ESA- services) sur la santé des aînés et l'utilisation des services de santé menée auprès de 1811 personnes âgées de 65 ans et plus qui consultaient dans les services de médecine générale au Québec entre 2011-2013. Près de 20 % de ces personnes, couvertes par le régime public d’assurance médicaments de la Régie d’assurance maladie du Québec avaient une
prescription d’antidépresseurs. L’échantillon de cette étude incluait des personnes ayant reçu une
prescription d’antidépresseurs pendant au moins 90 jours. En fonction des recommandations cliniques canadiennes, l’adéquation de la
prescription d’antidépresseurs a été mesurée à l’aide de 3 critères : la dose adéquate, la durée adéquate et le suivi médical adéquat dans la phase aigüe du traitement. Nos résultats indiquent que 44 % des personnes ont reçu une
prescription adéquate d’antidépresseurs selon les 3 critères. En utilisant les régressions multivariées pour examiner les facteurs pouvant expliquer la probabilité de recevoir une
prescription adéquate d’antidépresseurs en contrôlant pour l’âge, le sexe, l'état matrimonial, le revenu, la scolarité, aucune de ces variables n'était associée à la probabilité de recevoir une
prescription adéquate d’antidépresseurs. Toutefois, lorsque l’on examine l’adéquation de la
prescription d’antidépresseurs au niveau de chacun des critères, les résultats de notre étude ont montré que les hommes et les personnes qui vivent en milieu urbain et métropolitain sont moins susceptibles de recevoir une dose adéquate d’antidépresseurs et que la détresse psychologique modérée à sévère était associée à la probabilité d’avoir un suivi adéquat. Aucun des facteurs organisationnels n’était associé à la
prescription adéquate d’antidépresseurs.

▼ Literature and research surrounding transgender people and healthcare is scant. In particular, there is very little information regarding the intersection of transgender people, pharmacy services, pharmacies, and pharmacists. The objective of this study was to describe the experiences of transgender people filling prescriptions in community pharmacies. Semi-structured interviews were conducted with eleven self-identifying transgender people. Participants were asked to describe their experiences filling prescriptions at their pharmacies. One theme identified was a struggle with obtaining the correct needles and/or syringes for participants using injectable hormones. Moreover, based on the professional experience of the author, systemic issues related to pharmacy software introduce difficulty and multiple opportunities for error when processing prescriptions for needles and syringes. A multi-pronged approach of addressing educational needs as well as streamlining the prescription fulfillment process within pharmacy software systems should be used in order to more fully prevent these errors in the future.

prescription drug policy areas, aiming to fill in the gap in the research toward reforming health care in a positive direction: (1) Medicare Part D program, (2) state
prescription drug monitoring programs (PDMPs), and (3) off-label prescribing practices.
The Medicare Part D program study examined Part D’s effects on health outcomes and Medicare spending for Parts A and B among Part D beneficiaries who previously lacked
prescription drug coverage, using Medicare Current Beneficiary Survey (MCBS) Cost and Use files for 2004-2007, which are combined with Medicare claims and other administrative data, and a difference-in-differences model, estimating Part D’s treatment effects between 2005 and 2007. The results showed that Part D benefits did not have statistically significant effects on health outcomes and Medicare Parts A and B spending among the study population. In subpopulation analysis, people who previously had hypertension showed slightly, but significantly, decreased general health status and increased Medicare spending for home health agency services.
The second study on PDMPs examined PDMPs’ effects in reducing drug overdose mortality rates in the United States during 1999-2010. The main data used are (1) de-identified individual-level mortality data obtained from the National Center for Health Statistics (NCHS) and (2) bridged-race population estimates data produced by the U.S. Census Bureau in collaboration with NCHS. The PDMP effects on drug overdose mortality rates were estimated using three different fixed effects models. The results suggested that PDMPs had little effect in reducing drug overdose mortality rates and might be related to unintended consequences, such as increased drug overdose deaths from illegal or unspecified drugs.
The third study on off-label prescribing of drugs aimed to investigate individuals’ understanding, experience, and preferences on off-label prescribing of drugs from the perspective of patient-centered care, employing concurrent, embedded mixed methods. The primary quantitative and qualitative data were collected through a survey, on-line and off-line, among community-dwelling senior citizens age 60 and older residing in a county in Pennsylvania. The study found that an absolute majority of individuals (96 percent of the respondents) wanted to be informed of off-label
prescription, and the reasons included: to make informed decision; to determine the existence and the level of scientific evidence supporting the off-label drug use; to have a partnership with physicians in their treatment; and because of their belief that making informed decisions is a patient’s right.

Elle tient une place centrale dans la relation entre le patient et le médecin. En médecine générale en France, 78 % à 90 % des consultations aboutiraient à une
prescription médicamenteuse versus 72 % en Allemagne et 43 % aux Pays-Bas. Le médicament est un produit complexe soumis à des logiques industrielles et de santé publique contradictoires. Sa
prescription est l'aboutissement d'une décision médicale qui tient compte, dans le cadre de l'evidence-based-medicine, des données de la science, de l'expérience du médecin et des choix du patient. D'autres déterminants peuvent influencer la
prescription médicamenteuse : certaines caractéristiques des maladies, des patients, des médecins et du système de soin. Afin d'identifier ces déterminants, nous avons mené une revue systématique de la littérature pour rechercher les facteurs associés au nombre de médicaments prescrits en médecine générale. Puis, à partir de l'étude épidémiologique nationale ECOGEN, nous avons identifié les facteurs liés à la
prescription médicamenteuse en médecine générale en France. Pour étudier plus précisément la
prescription de médicaments chez les patients atteints de maladies chroniques, nous avons mené une étude épidémiologique évaluant les facteurs associés à la
prescription d'antibiotiques et d'anxiolytiques/hypnotiques aux patients asthmatiques en médecine générale en France et en Italie. Enfin, nous avons étudié la place des fonctionnalités liées à la
prescription médicamenteuse dans les logiciels médicaux en France. La revue de la littérature a montré que les déterminants du nombre de médicaments prescrits en médecine générale étaient souvent liés à l'existence d'une maladie chronique, aux caractéristiques sociodémographiques des patients (personnes âgées, femmes, faible statut socioéconomique et faible niveau d'éducation) et aux caractéristiques de travail du médecin tel que le nombre de consultations par jour. L'étude épidémiologique réalisée en médecine générale en France a montré qu'un médicament était plus fréquemment prescrit aux femmes (OR : 1,12 [1,04-1,20]) et aux individus âgés de plus de 60 ans (OR : 1,35 [1,20-1,53]) ou de moins de 15 ans (OR : 1,51 [1,29-1,78]. Les médecins recevant les visiteurs de l'industrie pharmaceutique étaient aussi plus susceptibles de prescrire un médicament (OR : 1,19 [1,01-1,41]). L'étude comparant l'Italie à la France a montré que les généralistes français prescrivaient plus d'anxiolytiques/hypnotiques (17.8% vs. 6.9%, p<0.0001) mais moins d'antibiotiques (37.1% vs. 42.2%, p<0.00001) que les généralistes italiens aux patients asthmatiques. ▼ There is growing evidence of the increasing use of medicines in older generations; however, research undertaken to critically examine specific differences in use among baby boomers and older people is limited. Understanding the complex factors that influence medicine use by baby boomers and older people is vital due to the particular risks associated with medicines use in older generations. In addition, exploring the differences in medicines usage between these generations will help to inform research and health policy to better meet the health care needs of the ageing population. This thesis, utilising South Australian (SA) population data, examines the difference in prevalence of use of medicines and the influence of social, demographic, chronic conditions and health related factors on the likelihood of being a user of
prescription medicines, over-the-counter medicines (OTC) or complementary and alternative medicines (CAM). The thesis also examines the links between medicines use and health outcomes with the evaluation of their beneficial or adverse effects assessed through comparison with quality of life (QoL), health services use and biochemical markers. Population data from the SA Health Omnibus Survey, an annual face-to-face survey of randomly selected respondents, and the North West Adelaide Health Study (NWAHS) were used. Univariable and multivariable logistic regression analyses were performed in order to investigate the relationships between medicines use and demographic characteristics (including gender, household size, income, education, area of residence, work status, marital status and country of birth). Linear regression analyses were used to determine the relationship between QoL and the independent variables. The results of the analysis show that the majority of SA baby boomers and older people take a combination of
prescription medicines and CAM and a large proportion of older people take a combination of
prescription medicines, OTC medicines and CAM. Unemployment was shown to be a key factor for the use of
prescription medicines and polypharmacy (five or more medicines), while gender and education were important factors for the use of CAM. The use of CAM and OTC medicines could potentially increase the rate of adverse effects or drug interactions. Therefore, it is important for prescribers to take a detailed medicines history prior to prescribing medicine. QoL was shown to be lower with increased use of
prescription medicines and OTC medicines. However only a weak association was found between the use of CAM and QoL. The specific type of
prescription medicines found to be associated with QoL were those prescribed for blood and blood forming organs, musculoskeletal system and nervous system. Cardiovascular disease (CVD) was used as an example of a chronic condition. People with CVD and without CVD had a similar QoL but the increase use of
prescription medicines, OTC medicines was associated with lower QoL. Further investigation for association of the use of medicines and QoL is warranted.

Dans l’ordre international le statu quo des règles juridiques reste ambigu et décentralisé, à travers de ses sources, substances et niveaux de la juridicité évolutive, de jus cogen au soft-law. L’application des ces règles dans l’ordre juridque interne, entretemps avec les règles étatiques, reste un défi objectif pour l’Etat. Le cas du Cambodge en voie de développement confronte l’écart entre les pratiques nationales via ses organes, le nationalisme juridique et sa Constitution internationalisée, l’objectif de l’Etat pour sa légitimité d’intégrer dans la communauté internationale ; le context s’illustre par la position partagée avec d’autres nouveaux sujets du droit international de 20è siècles qui voulait un nouveau dynamique des règles internationales répondant à leurs besoins. La codification du droit international constitue la tendance prometteuse pour crystalyser et développer ce droit. La jurisprudence internationale sanctionne les droits étatiques de manière équivoque, en défendant néanmoins sa primauté juridique par des mécanismes du droit de la responsabilité, du droit des traités codifiés. Ceci reflèterait notre perspective du renforcement de l’application.
The research tries to escape from the long-standing theoretical enigma of the problematic relationship between the international legal order and the state legal order, to confront the legal reality, the critical effectiveness and the devoir-être of state vis-à-vis the international prescription which is in itself doubtful. In the international order, the status quo of legal rules remains ambiguous and decentralized, through its sources, substances and levels of evolutionary legality, from jus cogen to soft-law. The application of these rules in the state legal order, meanwhile with state rules, remains an objective challenge for the state. The case of Cambodia, a developing state, confronts the gap between national practices via its organs, legal nationalism and its internationalized Constitution, the objective of the State for its legitimacy to integrate into the international community; the context is illustrated by the position shared with other new subjects of international law of the 20th century that wanted a new dynamic of international rules to meet their needs. The codification of international law is the promising trend to crystallize and develop the law. International jurisprudence sanctions state’s rules in equivoque maner, while defending its legal primacy through mechanisms of the law of responsibility and the law of treaties. This would reflect our perspective of strengthening the application.

prescription drug (NMPD) use is becoming increasingly prevalent, particularly among adolescents. A critical step towards developing effective intervention efforts requires identifying adolescents who are at risk for NMPD use. An extensive literature suggests that both adolescents and adults with elevated anxiety sensitivity (AS) are at greater risk for problematic substance use, and a small body of work has identified similar links with NMPD use specifically among adults. However, most of this literature combines all
prescription drugs into a single category, and no work has evaluated the relation between AS and NMPD use among adolescents. The current study endeavors to further this area of research by examining the relation between AS (overall and subscales) and NMPD use among adolescents. The project evaluated 276 adolescents (age 9-19 years) enrolled in a residential treatment program on level of AS, including sub-dimensions (i.e., cognitive, physical, and social), and NMPD use across three categories of drugs: analgesic (e.g., Vicodin®), anxiolytic (e.g., Xanax®), and stimulant (e.g., Adderall®). A series of logistic regression models indicated that overall AS predicted nonmedical analgesic use, but not anxiolytic/sedative or stimulant use. A closer investigation of the AS subscales demonstrated that only the AS social subscale significantly predicted nonmedical analgesic and anxiolytic/sedative use. These results suggest that AS is related to NMPD use among adolescents, highlighting the need for future work to disaggregate the assessment of NMPD use into specific drug classes and explore the subscale dimensions of AS.

Within the past decade,
prescription drug abuse has emerged as a nationwide epidemic, with opioid-related poisoning deaths more than tripling since 1999. In an effort to bring this public health crisis under control, 43 states, including Tennessee, have enacted
prescription drug monitoring programs (PDMPs), computerized databases of DEA-controlled substance prescriptions filled at pharmacies within the given state. Such programs have been found to be effective in curbing
prescription opioid abuse by alerting prescribers to aberrant
prescription-filling activity. However, they are commonly underutilized and have workflow barriers that impede clinical use.
Ideally, PDMP queries could be generated seamlessly from within a medical enterprise’s electronic health record (EHR) system, using an application-programming interface (API) supplied by the state’s PDMP vendor. However, the enabling legislative language currently prohibits such access. Therefore, we developed and evaluated a Perl software program activated from within Vanderbilt University Medical Center’s EHR patient chart to send the properly coded/formatted user and patient-demographic information packets to the Tennessee PDMP website, without the use of an API. The program parses the returned data file for important
prescription information and displays the filtered information to the user. By allowing the query to occur in the background, the user’s tether time to the computer is decreased from 3 minutes to 10 seconds per query.
During the evaluation phase, we used a quasi-experimental intervention design with two alternating 2-week control and intervention periods. Twenty-eight ED attending physicians participated in the study and queried the PDMP at their clinical discretion. During integrated PDMP query tool availability, 5.9 % (169/2844) of emergency department patients were screened compared with 2.2 % (62/2786) during periods when the tool was not available (p<0.001, Pearson’s Chi square). Data was not viewed in 20% of integrated tool assisted queries. The EHR-integrated PDMP query tool was well regarded by study physicians as an enhancement to workflow.

However, antidepressants may interact with anticancer drugs or cause adverse reactions. The prescription practice of antidepressants to cancer patients in Australia is not well documented. Our systematic review and meta-analysis identified that the overall prevalence rate of antidepressants was 15.6% varied widely by world-region and gender. A retrospective case-control study was undertaken to determine the recent prescription practice of antidepressants to cancer and non-cancer patients in Australia. Mirtazapine was the highly prescribed antidepressants to cases, whereas Desvenlafaxine was prescribed to controls. Considerable variation in the prescribing patterns of antidepressants was identified. Prospective studies are needed to ascertain whether patients are being treated optimally.

pain is complex. Opioids have the potential to alleviate discomfort
and increase ones overall ability to function but, long term use
also has potential physical and psychological impacts. The purpose
of this study was to explore the lived experience of adults who use
prescription opioids to manage chronic non-cancer pain. Nine
participants were recruited and interviewed. Participants were
asked to describe how using
prescription opioids had affected their
lives. Interviews were recorded, transcribed and analyzed
thematically using Interpretative phenomenological analysis (IPA).
Eight themes emerged from the data: the process of decision making,
physical effects of using opioids, social consequences of using
opioids, Guilt, fears, ambivalence, self-protection, and
acceptance. Using opioids made pain more manageable and improved
function for most of the participants. Nevertheless, using opioids
was also associated with stigma, guilt, fears and ambivalence about
their future as persons with chronic pain.

▼ The purpose of this study was to determine the effect of population
size, type of ownership, number of hours open per week,
pharmacy location, type of pharmacy, average annual store sales
volume, average number of prescriptions filled per day, patient profile
system and delivery service on
prescription price, dollar markup
and percent markup in Oregon; then to examine the relationship between
the selected socioeconomic variables and
prescription price,
dollar markup or percent markup among Oregon counties.
The
prescription price, dollar markup and percent markup were
obtained from the average of 300 prescriptions from each of 292 observed
pharmacies which were drawn by 1977 Survey of Oregon
Pharmacies conducted by Adult and Family Services, State of Oregon.
One-way analysis of variance was used to determine the effect of the
previous nine variables on
prescription price, dollar markup and
percent markup. Type of ownership, number of hours open per week, type of
pharmacy, average annual store sales volume, patient profile system
and delivery service affected the
prescription price, dollar markup
and percent markup significantly. The difference in size of city
population had significant effect only on
prescription price but not
on dollar markup and percent markup. Pharmacy location and number
of prescriptions filled per day appeared to cause significant effect on
dollar markup and percent markup but not on
prescription price.
The simple regression analysis reflected a significant relationship
between retail drug and proprietary sales and dollar markup
among Oregon counties. The stepwise regression also indicated
significant relationship between dollar markup and retail drug and
proprietary sales, per capita income per person and percentage age
over 65 years among Oregon counties. Both stepwise and simple
regression showed that there were no significant relationships among
all selected socioeconomic variables and either
prescription price
or percent markup.

▼ Background: Worldwide, prices for cancer drugs have been under downward pressure where several governments have mandated price cuts of branded and generic products. A better alternative to mandated price cuts would be the estimation of a launch price based on drug performance, cost effectiveness and a country’s ability to pay. In this study, the development of a global pricing index for new drugs that encompasses all of these attributes in patients with metastatic colorectal cancer (mCRC) is described. Methods: A pharmacoeconomic model was developed to simulate clinical outcomes in mCRC patients receiving chemotherapy with the addition of a “new drug” that improves survival by 1.4, 3 and 6 months. Cost and health state utility data were obtained from cancer centers and oncology nurses (total n=112) in Canada (n=24), Spain (n=24), India (n=24), South Africa (n=16) and Malaysia (n=24). A price per dose was estimated for each survival increment using a target value threshold of three times the per capita gross domestic product (GDP) for each country, as recommended by the World Health Organisation (WHO). Multivariable analysis was then used to develop the pricing index, which considers survival benefit, per capita GDP and income dispersion as measured by the Gini coefficient as predictor variables. Results: Higher survival benefits were associated with elevated drug prices, especially in wealthier countries such as Canada and Spain. For a nation like Argentina with a per capita GDP of 15,000 and a Gini coefficient of 51, it is estimated that for a drug which provides a 4 month survival benefit in mCRC, the value based price would be US 630 per dose. In contrast, the same drug in a wealthier country like Norway could command a price of US 2,775 and still be considered cost effective according to the WHO criteria. Conclusions: A global pricing index was presented that can be used to estimate a value based price in different countries for new drugs in mCRC. The application of this index to estimate a price based on cost effectiveness would be a good starting point for opening dialogue between the key stakeholders and a better alternative to governments’ mandated price cuts.

▼ Objectives: To assess the attitudes and culture of prescription drug misuse amongst undergraduate students. To assess students knowledge of, their perceived risks, and severity of misusing prescription drugs, and the accessibility of prescription drugs on campus. To examine the relationship between academic status and prescription drug misuse. To develop a Public Service Announcement to create awareness on this issue, and propose appropriate prevention programs using the Ecological Model of behavioral change. Methods: This is a cross-sectional study involving 50 undergraduate Drexel University students, ages 18 and older. Students were recruited from the Main Campus to complete an anonymous 35-question survey to assess their perceptions, beliefs, and attitudes towards prescription drug misuse. Results: 22% of students in this study reported misusing prescription drugs in their lifetime, while 44% of students reported abusing illicit drugs in their lifetime. Older students 24 years old and above are more likely to misuse prescription drugs than younger students. Most students reported stimulants (58%) and painkillers (52%) to be the most accessible prescription drugs on campus. 90% of students reported marijuana as the most accessible illegal drug. 70% of students feel that they could gain access to prescription drugs by asking other students on campus, with 40% of students claiming to know at least one student to ask for prescription drugs. In this study, academic status was not significantly correlated with prescription drug misuse; however, students with lower grade point average appeared to misuse prescription drugs more than students with higher grade point average. Perceived risks and severity of prescription drug misuse varied; however, majority of students reported some risks associated with prescription drug and illegal drug abuse. Conclusions: Lifetime misuse of prescription and illicit drugs is somewhat common amongst Drexel University undergraduate students surveyed at the Main Campus. This study found that students risk perceptions about drug misuse and their perceived ease of access to drugs could influence their choice to abuse prescription and illicit drugs. It was difficult proving statistical significance for some correlations due to the sample size of this study. Hence, a larger sample study on this topic is recommended in order to be able to generalize the findings to the rest of the Drexel University students. A PSA is being done to create awareness on this issue, and correct students misunderstanding of the effects of drug misuse and prevention programs should be put in place through the C.H.O.I.C.E.S Center at Drexel University.

medicines, especially when it starts to interfere in the
prescription. Monitoring
advertisements, as well as the observation of its influence on health professionals,
prescriptions become necessary because of this public health problem. OBJECTIVE:
The aim of this work was to analyze drug advertisements directed to gynecologists
and/or obstetricians doctors relating them to the current legislation as well as its
influence on
prescription. METHODOLOGY: The sample was composed of drug
advertisements divulged to the gynecologists and / or obstetricians doctors and was
analyzed according to RDC96/2008. To evaluate the influence of advertisement on
prescription, a questionnaire was administered to gynecologists / obstetricians
doctors and prescriptions of a public maternity were also evaluated. RESULTS AND
CONCLUSION: Concerning the advertisements analyzed, 48% complied fully with
current legislation and 52% of the advertisements analyzed were in accordance to
the RDC No. 96/2008 in most of the items evaluated. The doctors interviewed are
used to receive the visits of propagandists, even in the public service, receiving gifts
offered by the industry and believe that medicine advertisement tries to influence
prescribing. Many of them use the material provided by the propagandist as a source
for their
prescription, although they present a critical view about them information.
The use of trade name / mark on the
prescription is a common practice among the
doctors interviewed, even in the public service, suggesting there is an influence of
medicine advertisement on the
prescription

▼ Topical steroids comprise the mainstay treatment of skin diseases. However, irrational
prescription of topical steroids is a common occurrence in clinical practice and they are being abused by health professionals and patients alike. This study was performed to assess the utilization pattern of topical steroids in ALERT Hospital, Addis Ababa. The study employed a descriptive, cross sectional study design, using both quantitative and qualitative methods between August and November, 2013. Data abstraction format was used to collect the data from a total of 660 patient records and an interview guide was employed to collect data qualitatively. The quantitative data was analyzed using SPSS version 16 and the qualitative data was analyzed thematically. The majority of patients were females 380(60.9%) and their mean age was 24.2 years. Eczematous dermatitis 211(30.0%) was the single most common skin diseases observed. However, infectious skin diseases were the most common skin dermatoses 230(33.8%) when combined. The most commonly prescribed classes of drugs were topical steroids and its combination 315(28.4%) followed by anti-fungal drugs 245(22.1%). There were a total of 1108 drugs prescribed, with an average of 1.77 drugs per
prescription. Topical steroids and its combination were found to be the most commonly prescribed drugs. A total of 315 topical steroids were prescribed and brand name was used in 307(97.5%) of them. The most commonly prescribed topical corticosteroids were betamethasone dipropionate 77(24.4%) and clobetasol propionate 73(23.2%), which are potent and very potent topical corticosteroids, respectively. Out of all the topical corticosteroids prescribed, the majority were potent 174(55.2%) and very potent 73(23.2%). Prescribers‘ adherence is to the ALERT Medicines Formulary and National Standard Treatment Guideline was found to be 100% and 58.1%, respectively. In most prescriptions, site of application, duration of treatment and quantity to be dispensed was inadequate; a practice that should be improved.

In May 2011, Portugal signed the Memorandum of Understanding in which it guarantees a reduction in the public spending on pharmaceuticals. The aim of this research work is to analyse the possibility of hospital cost reductions in HIV, oncology and rheumatism related pharmaceutical drugs, by better practice of medical physicians. The patterns of drug prescriptions were modelled using a relative distribution, stochastic dominance analysis and an ordered probit model. It was found that there is great divergence in expenditure levels of pharmaceutical products across hospitals and medical specialties. Outlier physicians tend to maintain their prescribing behaviour, even after the introduction of feedback. Therefore, there is room to reduce hospital costs of pharmaceutical products, by better practice.

besoins complexes suivis dans les GMF-UMF. Les problèmes liés à la
pharmacothérapie (PRP), la complexité du régime médicamenteux et
l’adhésion au traitement chez les patients ayant des besoins
complexes référés par leur médecin, ont été comparés avant et après
les interventions du pharmacien dans quatre GMF-UMF à Québec. Les
PRP identifiés ont été regroupés et décrits en utilisant la
classification de Strand. La complexité du régime médicamenteux a
été mesurée en utilisant l'indice de complexité du régime
médicamenteux (MRCI) tandis qu’on a mesuré l’adhésion à l’aide de
la proportion de jours couverts (PDC) en se basant sur
l'information du bilan comparatif des médicaments (BCM). Les
moyennes des MRCI et des PDC, avant et après les interventions des
pharmaciens, ont été comparées par un test-t sur échantillons
appariés. Dans cette étude, 92 patients ont été référés aux
pharmaciens, dont 64 ont été sélectionnés suivant les critères
d’inclusion, parmi lesquels, 56 patients ayant 2 BCM ont été inclus
dans l’analyse. L’échantillon se composait majoritairement de
femmes âgées ayant de multiples comorbidités, une polypharmacie et
un régime médicamenteux complexe. Le pharmacien détectait en
moyenne 7,2 PRP par patient. Ses interventions ont mené à une
diminution de la complexité du régime médicamenteux (différence des
moyennes (DM) de 5,44 au MRCI) et à une augmentation de l’adhésion
aux traitements (DM de 5,6 % pour la PDC). Les pharmaciens GMF-UMF
ont pu détecter et résoudre les PPR, diminuer la complexité du
régime médicamenteux et la non-adhésion aux traitements chez les
patients ayant des besoins complexes dans les cliniques de soins de
première ligne.
Recently, improvements in professional human
resources, including pharmacists, took place within several Family
Medicine Groups and Units (FMG-FMU) in Quebec. Our objective, is to
measure the impact of pharmacist’ interventions on the
pharmacotherapy for patients with complex needs monitored in
FMG-FMU in Quebec City. Drug related problems (DRPs), medication
regimen complexity and adherence to treatment in patients with
complex needs referred by their physician, have been compared
before and after the pharmacist' interventions in four FMG-FMU in
Quebec City. Identified DRP were grouped and described using Strand
classification. The medication regimen complexity was calculated
using the medication regimen complexity index (MRCI) while
adherence was measured using the proportion of days covered (PDC)

medicines, especially when it starts to interfere in the
prescription. Monitoring
advertisements, as well as the observation of its influence on health professionals,
prescriptions become necessary because of this public health problem. OBJECTIVE:
The aim of this work was to analyze drug advertisements directed to gynecologists
and/or obstetricians doctors relating them to the current legislation as well as its
influence on
prescription. METHODOLOGY: The sample was composed of drug
advertisements divulged to the gynecologists and / or obstetricians doctors and was
analyzed according to RDC96/2008. To evaluate the influence of advertisement on
prescription, a questionnaire was administered to gynecologists / obstetricians
doctors and prescriptions of a public maternity were also evaluated. RESULTS AND
CONCLUSION: Concerning the advertisements analyzed, 48% complied fully with
current legislation and 52% of the advertisements analyzed were in accordance to
the RDC No. 96/2008 in most of the items evaluated. The doctors interviewed are
used to receive the visits of propagandists, even in the public service, receiving gifts
offered by the industry and believe that medicine advertisement tries to influence
prescribing. Many of them use the material provided by the propagandist as a source
for their
prescription, although they present a critical view about them information.
The use of trade name / mark on the
prescription is a common practice among the
doctors interviewed, even in the public service, suggesting there is an influence of
medicine advertisement on the
prescription

▼ Introduction: The concerns of possible resistance development by P. falciparum to most artemisinin-based combination therapies (ACT’s) is imminent and to an extent due to the non-uniform use of the ACT’s and drug pressure. This has necessitated the need to routinely monitor the drug efficacy every couple of years in all countries where malaria is endemic and studies on patterns of ACT prescription is also needed as an essential component to complement efficacy studies.
Objective: To describe the patterns of ACT prescription in the Ledzokuku-Krowor Municipal Assembly.
Method: A descriptive cross-sectional survey was carried out for the study. Five (5) health care facilities were randomly selected within the Ledzokuku-Krowor Municipal Assembly. All prescribers within the selected health care facilities were administered with structured questionnaires. Also, fifty (50) patient folders were randomly selected from a pool of 2017 confirmed malaria cases at the Out-Patient Department (OPD) in each of the five selected health care facilities. Data on the ACT types prescribed for patients who were diagnosed with malaria was abstracted.
Results: The most frequently prescribed ACT for uncomplicated and complicated malaria were artemether-lumefantrine 62.4% (156/250) and artesunate injection + artemether-lumefantrine 22.4% (56/250) respectively. The proportion of uncomplicated malaria cases correctly treated was 73.6% (184/250) and that for complicated malaria cases correctly treated was 85% (56/66). The most prevailing reasons given by all the prescribers that governed their choice of ACT prescribed included ready availability of the drugs 32.3% (20/62), its effectiveness 22.6% (14/62), side effect 19.4% (12/62) and cost 16.1% (10/62). Prescribers age (P<0.05), occupation (whether specialist or general practitioner) (P<0.001) and work experience (P<0.001) were found to be significantly associated with the ACT type they prescribed
Conclusion: This study indicates that there is implementation of artemisinin combination therapy (ACT) prescription for malaria cases in health facilities within the Ledzokuku-Krowor Municipal Assembly however there is the need for the National Malaria Control Program (NMCP) to roll out campaign programs on the various ACTs available in Ghana and the need to vary their prescription in order to prevent pressure on one type in order to delay possible resistance that could develop.

▼ Background: The Centers for Disease Control indicated that in 2009, 90 million prescriptions were written for antibiotics in the United States, with half of those being "unnecessary or inappropriate". The highest rate of antibiotic use was evident in children younger than 15 years old. Physician’s perception of parents’ expectation to receive antibiotic
prescription for their children is a significant predictor of overprescribing antibiotics.
Objective: The objective was to manipulate two factors (parents’ ‘perceived benefits of using antibiotics’ and their ‘perceived barriers to visit doctors without any expectation of antibiotic prescription’) and then evaluate whether their level of expectation would change after the manipulation.
Methods: A prospective experimental study was conducted using a structured data collection instrument. The purpose of the experiment was to manipulate two variables, perceived barriers and perceived benefits using four scenarios and keep other factors constant. Each
subject viewed four situations and expectation associated with each situation was evaluated. Subjects who had at least one child (age ≤ 5 years) during the study and who could speak, read and write English were selected for the study. Data were collected at shopping malls and parks in Houston, TX. Descriptive analyses and repeated measures mixed method covariance adjusted analyses were performed using SAS® 9.3. The a-priori significance level was set as 0.05 for all tests conducted.
Results: A total of 300 complete surveys were considered for analyses. The mean age for the sample was 30.36 (± 7.04) years; females represented 55.7% of the sample. The mean general expectation score (before reading any scenario) to receive antibiotic
prescription for children was 53.6 (± 25.7). The repeated measure mixed methods analyses indicated that there was 12 point reduction (p < 0.0001) in expectation score after removing perceived barriers from the situational scenarios. Almost 16 point decrease (p < 0.0001) in expectation score was observed after removing perceived benefits from the scenario. There was 18 point decrease (p < 0.0001) in expectation score after removing perceived barriers and perceived benefits from the situational scenario. The study result also indicated that general expectation toward an antibiotic
prescription, training in the healthcare field and parents’ preference for communication had significant effect on parents’ expectation.
Conclusions: Perceived barriers, perceived benefits alone and in combination have effect on parents’ expectation to receive antibiotic
prescription for children. Policy makers as well as intervention programs should consider these factors to enhance successful reduction of antibiotic expectations.

▼ The purpose of this study was to examine the obstacles and problems prescribers of exercise on prescription (EoP) in primary care in Bollnäs and Söderhamn experience when using the method. It also aimed at examining if the barriers identified in previous studies also will be found in Bollnäs and Söderhamns municipalities. A qualitative study was made and four interviews with the managers of health centers in Bollnäs and Söderhamns municipalities were completed. The only inclusion criteria was that it would be a licensed health care professional at the clinic who work with, and prescribe the EoP. A content analysis was performed, which categorized the statements made according to their content. The results of the study showed that prescribers experienced obstacles and problems with the method. The main reason was lack of time, particularly insufficient time to provide information on adequate physical activity. Another obstacle was difficulties to motivate patients to become more physically active, as well as interoperability issues with politicians and complicated prescription forms. The study confirmed previous research regarding the barriers identified. The basis for the study was small and it is therefore not possible to generalize from the results obtained.

▼ Direct-to-Consumer Advertising (DTCA) of prescription drugs has been a consistent controversial topic extensively debated in scholarly work by many authors. This thesis compares the arguments made by the proponents and opponents of this debate and comes to a conclusion that there is less justification in claims that DTCA benefits society. The thesis goes further in assessing the benefits from normative ethical principles, including using Rawls difference principle as a guide, to evaluate which side of the debate conforms more logically along the teachings of ethical philosophy. At the end it is apparent that the least advantaged members of society do not benefit from this aggressive marketing strategy. It concludes that the pharmaceutical industry makes exaggerated claims of providing the public increased autonomy when, in reality, it is trying to further its own cause of making huge profits for its shareholders. This thesis also reaches the conclusion that the industry uses patients as a means to achieve its own end, that end being unreasonable levels of profit.
