▼ It is known that the birth process and initial life exposures, such as feeding, may have an important impact on the acquisition of bacterial communities throughout the human body, including the gut. Preterm infants usually have special dietary needs and undergo increased oxidative stress related to intensive care, which can ultimately impair their gastrointestinal microbial colonization and microbial diversity in the bowel. Using molecular techniques, we analyzed the fecal microbiome of 20 preterm infants and tested the association between bacterial communities and feeding type, as well as levels of F2-isoprostanes. We found that feeding influences the fecal microbiome of preterm infants, however more research is needed to clarify the role of human milk fortifiers in this process. We also observed preliminary evidence of an association between microbial composition and oxidative stress, indicating that future studies in this area should be conducted.

▼ La neuro-inflammation induite par l'inflammation systémique ou générée en réponse à une lésion cérébrale aiguë a des conséquences cliniques néfastes : elle est mise en cause dans l'aggravation des lésions cérébrales aiguës chez l'homme, aussi bien chez l'adulte que chez l'enfant. La microglie est l'effecteur cérébral principal de cette réponse inflammatoire, et peut présenter selon les situations un profil neurotoxique ou, au contraire, anti-inflammatoire et régulateur. La compréhension des mécanismes d'activation microgliale et de leurs conséquences est capitale pour une meilleure prise en charge des malades. La première partie de ce travail de thèse s'intéresse aux conséquences de l'inflammation néonatale associée à la prématurité sur la réponse microgliale à l'âge adulte, face à de nouvelles agressions cérébrales que sont l'inflammation systémique et les lésions cérébrales aiguës. Dans un modèle murin d'inflammation néonatale, nous avons mis en évidence d'importantes modifications du transcriptome microglial une fois ces souris adultes. De plus, un stimulus inflammatoire à l'âge adulte modifie le profil d'activation microgliale, le pic des marqueurs pro-inflammatoires et immuno-régulateurs survenant plus précocement et intensément, démontrant l'existence d'une mémoire du système immunitaire inné cérébral. Ces modifications dans le profil d'activation microgliale s'accompagnent dans un modèle de lésion cérébrale excitotoxique d'une majoration de la taille des lésions de la substance blanche. Un traitement par mélatonine des souriceaux prévient cette aggravation. La deuxième partie de ce travail a consisté à caractériser in vitro le profil d'activation microgliale en réponse à une stimulation par HMGB1, une alarmine relarguée lors de la mort cellulaire et donc présente en cas de lésion cérébrale aiguë mais aussi de lésions extra-crâniennes associées. Nous avons montré que le profil d'activation microgliale dépend du type d'HMGB1 utilisé. Les microglies exposées à la forme recombinante de chez Sigma présentent un profil transcriptomique proinflammatoire mais une baisse des taux de cytokines sécrétées dans le milieu. Ces résultats mettent en évidence l'importance de l'inflammation et de l'activation microgliale dans le pronostic des lésions cérébrales et offrent la possibilité de mettre en place des stratégies neuroprotectrices innovantes

▼ It has been documented that children who are born prematurely are at risk of experiencing motor skills deficits early in life; however, little is known about the relationships between early perinatal risk factors and later motor abilities. The current investigation attempted to gain better understanding regarding the influence of gestational age and sex on early motor development among a cohort of preschool-aged children born prior to 34 weeks gestation (N = 104). Additionally, relationships between motor performance and other abilities, namely cognitive and language, were examined. As hypothesized, degree of gestational immaturity was significantly associated with poorer performance on specific motor tasks. Additionally, a female advantage was found on select fine and gross motor tasks. Examination of associations between performances in different neuropsychological domains revealed that motor performance contributed to explained variance in cognitive and language outcome, above and beyond the variance accounted for by perinatal sociodemographic and medical risk factors. The implications of these findings are discussed.

▼ Premature birth has been shown to be associated with various deficits in neuropsychological functioning during early childhood; however, few studies have attempted to understand the variables that contribute to variability in performance among children born prematurely. The purpose of the current study was to investigate the relationships between specific perinatal factors and language and cognitive outcome measures in a group of preschool-aged children born prematurely (N=47). As predicted, there were significant relationships between sex and specific outcome measures, with boys performing more poorly than girls; however, contrary to hypotheses, significant relationships failed to be found between outcome measures and both gestational age and intrauterine growth rate. The overall implications of these findings for the development of preschool-aged children born prematurely are discussed.

▼ Multiples are thought to be at increased risk for developmental outcome deficits in the preschool years, following preterm birth. However, little research has been conducted to determine whether this group remains at higher risk in the age of artificial reproductive techniques and the modern NICU. The purpose of this study was to investigate whether multiplicity is a risk factor for neuropsychological outcome deficits in a sample of 118 preschoolers (49 multiples, 75 singletons) born prematurely (<34 weeks gestation). As predicted, there were significant relationships between multiplicity and outcome measures, however, the direction of the effects were opposite to our prediction, with multiplies demonstrating superior performance. Additionally, significant multiplicity by sex interactions revealed a female twin advantage over male twin and singleton performance. In this preschool age sample, preterm multiples were not at a disadvantage when compared to singleton counterparts, demonstrating either equivalent or superior neuropsychological performance.

▼ Preterm birth is a common cause of neurodevelopmental disorders in childhood. Little is known about the outcome of infants with severe retinopathy of prematurity (ROP) and multiple infections in the postnatal period. This thesis describes their associations with abnormal brain development using multi-modal MR imaging and standardized outcome assessments. Those infants with severe ROP had delayed brain maturation of the posterior white matter and optic radiations, with poorer 18 month cognitive and motor outcomes. Compared to fewer episodes of infection, three or more postnatal infections was associated with delayed maturation of the posterior limb of the internal capsule (PLIC), corpus callosum and the optic radiations, with poorer 36 month motor outcomes. Our findings support that severe ROP and multiple postnatal infections in very preterm newborns are associated with decreased brain maturation and poorer neurodevelopmental outcomes, and that advancements in these disorders have the potential to improve outcomes.

▼ Inflammation and oxidative stress are increasingly recognised as important independent mediators of preterm brain injury and have been implicated in the pathogenesis of cerebral palsy and cognitive impairment. Such exposures are common for the premature infant in whom infection and inflammatory morbidities occur in around 60%. Furthermore, many preterm infants require oxygen therapy and respiratory support due to lung immaturity. Epidemiological and experimental studies indicate that in addition to the independent effects of inflammation and extreme hyperoxia on the developing brain, inflammation preconditions the developing brain resulting in variable injury when exposed to subsequent hypoxia-ischaemia. However experimental studies employing exposure to more modest oxygen fluctuations are lacking. This thesis characterises a clinically relevant model of prematurity where the developing brain is exposed to low grade inflammation and oxygen fluctuation around a hyperoxic mean. We hypothesise that antenatal inflammation and postnatal oxygen fluctuation, both alone and in combination, have detrimental effects on developing white matter. Pregnant dams received intraperitoneal lipopolysaccharide (LPS) or saline on G18 and G19. Dams and their pups were then reared in room air or fluctuating hyperoxia (circa 10kPa) for seven days. We measured longitudinal brain and body growth in different experimental groups to 12 weeks. Whole brains were examined for mRNA expression of inflammatory cytokines (TNFα, IL-1β, IL-6 and IL-10) and markers of oxidative injury (iNOS, SOD2). To determine the effect of perinatal insults on developing white matter, we analysed the expression of myelin basic protein (MBP) and glial fibrillary acidic protein (GFAP) in the internal and external capsule. We also examined white matter tracts for differences in microglia (CD68), oligodendrocyte progenitor cells (NG2), oligodendroglial cells (Olig2) and cell death (cleaved caspase3). Behavioural studies (Morris Watermaze Test, Elevated Plus Test and Open Field Test) were undertaken at 12 weeks of age to detect any long-term functional difference between the groups. Antenatal inflammation reduces both brain and body growth at P7. This normalises by P14 unless this inflammatory insult has been followed by postnatal oxygen fluctuation, where brain and body growth restriction persists until P14. We defined our inflammatory response at P1 following antenatal inflammation and did not observe elevation of mRNA at P1. We demonstrated increased SOD2 at this time point, indicating a reparative process. At P7 we observed a significant reduction in the oxidative response following combined exposure to antenatal inflammation and postnatal oxygen fluctuation, indicating a potential limit to, or suppression of, the reparative process. In terms of white matter injury, antenatal inflammation reduces myelination at P7. There is no synergistic effect of inflammation and oxygen fluctuation on MBP immunohistochemistry at P7. and the incidence of premature live births is rising because of the improved perinatal
care. With the rising incidence of preterm births and the improving survival rates of (extremely)
very low birth weight neonates, efforts to decrease morbidity concerning short
and long term outcome remain a challenge in the neonatal intensive care unit (NICU).
Necrotizing enterocolitis (NEC) is the most common surgical emergency involving the
gastrointestinal tract of preterm neonates and affects 2-7% of all premature infants.
Both the incidence of NEC and its fatality rate are inversely related to birth weight and
gestational age.
Treatment is still limited to immediate restriction of enteral feeds and broad-spectrum
antibiotics. Although most cases of NEC are managed medically, an estimated 20-40%
of infants undergo surgery. Mortality rates from NEC range from 15-30% but mortality
rates for infants requiring surgery are as high as 50%, and are highest for the smallest,
most immature infants. Survivors of NEC are at increased risk for complications such
as short bowel syndrome and impaired neurodevelopment. Stoll and colleagues
reported that between 18 and 22 months of corrected gestational age, infants who
recovered from NEC in the postnatal period were at high risk for adverse outcomes,
including poor growth, cerebral palsy, vision and hearing impairment, and decreased
neuromotor development. Furthermore, infants who are surgically treated are more
likely to have growth impairment and adverse neurodevelopmental outcomes than
infants who were treated medically.

▼ Very low birth weight (VLBW; ≤1500 grams) preschoolers are at risk for cognitive and executive functioning (EF) difficulties. Maternal language quality may impact cognitive development in VLBW children. The aims of this study were to explore differences in maternal language (Spanish, English) and child cognitive abilities, and to explore associations between maternal language use (verbal scaffolding and structuring) and child cognitive abilities in VLBW preschoolers (3.5-4 years). Caregivers reported sociodemographic information. Cognitive abilities were measured using the WPPSI-III (VIQ, PIQ), Bear Dragon, and Gift Delay Peek scores. Results showed English speaking children scored higher on VIQ. For English speaking children, verbal scaffolding was positively correlated with VIQ, and structured statements were negatively associated with PIQ. When controlling for maternal education, the associations remained significant. Findings suggest maternal language use plays an important role in child cognitive development; these associations may vary by language group and be influenced by socioeconomic factors.

▼ Respiratory Syncytial Virus (RSV) infects the lower respiratory tract in children under the age of two years and is spread through droplet and contact with infected persons. An estimated 200,000 children suffer from complications of RSV annually worldwide. Palivizumab is a monoclonal antibody used to immunize children from RSV and has been on the market since 1988. In 2014, the American Academy of Pediatrics (AAP) updated its policy for recommendation of RSV in premature infants. The objective of this capstone is to propose an evaluation framework with an example of how it could have been applied to assess the impact of the AAP policy change on RSV vaccination use and RSV outcomes among premature infants.
The proposed evaluation framework would be a unique link between birth certificate records and surveys of parents/guardians of 32 week gestation premature infants or less in the metropolitan Atlanta area. The birth certificate data would identify “at risk” infants and would allow for selection of a sample of parents/guardians, both pre-policy change (August 1, 2013 to July 30, 2014) and post-policy change (August 1, 2014 to July 30, 2015). The primary endpoints would be: initiation and completion of the RSV vaccine series and RSV infection rates. Moderating variables would be obtained from birth certificate data (e.g. mother’s education and race) and survey data (e.g. attitudes toward vaccine acceptance).
The evaluation framework proposed in this Capstone can be used in future analyses of RSV vaccination policy changes. It can also be generalized to other geographic areas in the US and used for routine surveillance of RSV vaccination use and RSV outcomes.

▼ Objetivo: Determinar a influência da presença de cafeína no sangue de cordão umbilical na ocorrência de apneia. Métodos: Estudo de coorte prospectivo de recém-nascidos pretermos com peso de nascimento menor de 2.000 g. Os critérios de exclusão foram: mães que receberam opióides , ventilação mecânica durante os primeiros 4 dias de vida, malformação congênita cerebral e cardíaca maiores, asfixia perinatal, hemorragia peri-intraventricular severa, exsanguíneotransfusão antes do quarto dia de vida e uso de metilxantina antes da extubação. Os recém-nascidos foram divididos em: com e sem cafeína detectável no sangue de cordão umbilical e acompanhados nos primeiros quatro dias de vida para a ocorrência de apneia. Resultados: 87 com e 40 sem cafeína detectável no sangue de cordão umbilical foram estudados. A mediana da concentração de cafeína dos 87 pacientes com cafeína detectável no sangue de cordão umbilical foi 2,3 µg/ml (0,2-9,4 µg/ml). Não houve associação entre a ocorrência de apneia e a presença de cafeína no sangue de cordão umbilical. Recém-nascidos com cafeína detectável no cordão umbilical tiveram apnéia mais tarde (66.3 horas) do que aqueles com níveis indetectáveis (54.2 horas). Conclusão: a detecção de níveis de cafeína no sangue de cordão umbilical não diminuiu a ocorrência de apneia da prematuridade. Nós sugerimos que novos estudos com a administração de altas doses de cafeína para mães antes do parto prematuro, como estratégia para prevenir a apneia da prematuridade, devam ser realizados.
Objective: To determine the influence of presence of caffeine in umbilical cord blood on apnea occurrence. Methods: A prospective cohort study with preterm newborns with birth weight less than 2,000 g was undertaken. Exclusion criteria were: mothers that received opioids, mechanical ventilation during the first 4 days of life, cerebral and major cardiac malformations, perinatal asphyxia, severe periintraventricular hemorrhage, exchange transfusion before the fourth day of life, and those that received methylxantine prior to extubation. Neonates were divided in: with detectable and undetectable caffeine in umbilical cord blood. Newborns were followed for the first 4 days for occurrence of apnea spells. Results: 87 with and 40 without detectable caffeine in umbilical cord blood were studied. The median caffeine concentration of the 87 patients with detectable caffeine in umbilical blood was 2.3 µg/ml (0.2-9.4 µg/ml). There was no association between occurrence of apnea spells and presence of caffeine in umbilical cord blood. Neonates with detectable caffeine in umbilical blood had apnea later (66.3 ± 4.14 hours) than those with undetectable levels (54.2 ± 6.26 hours). Conclusion: The detected levels of caffeine in umbilical cord blood did not decrease the occurrence of apnea of prematurity. We suggest that further studies on administration of high dose of caffeine to mothers prior to a preterm delivery as a preventive measure for apnea of prematurity deserve to be conducted.

▼ The premature retinopathy (ROP) is a spreading vain disease which is developed from premature retina vascularization and its detection as well as an early treatment diminish scar sequels of the disease and reduce the risk of sighting loss. The ROP is a frequent disease with prevalence of less severe periods and spontaneous reversion. Nowadays there is a tendency of standardization on the concepts and actions to be followed in all neonatal care unities in Brazil that as a developing country has different characteristics in relation to neonatal care of developed countries. This study used the Brazilian guidelines proposal for screening and treatment of retinopathy of prematurity (ROP) created by the Group ROP Brazil which is based in the latest updatings on the theme. This study was proposed in order to analyze the incidence and major characteristics of ROP in the premature children population born in the Hospital Regional da Asa Sul (HRAS), during the period from July 2009 to December 2009, which aimed a better knowledge on this disease. It was realized a longitudinal prospective, graphical descriptive and analytical study in 50 premature children of this group. It was observed the incidence of ROP in any period of 32%, being 68% of ROP occurred in children born weight 1.000g or less and prevalence of periods more premature of ROP. It was also observed that 10% of incidence for treatment of ROP, being the treatment done through laser therapy for ROP, giving 100% of resolution of the disease. The follow up for the treatment began between 4 to 6 weeks of chronological age (CA) of premature with gestational age minor or equal to 32 weeks and/or born weight less or equal to 1.500g and vascular maturation of the retina average occurred in 10 weeks of chronological age (CA), period in which the follow up was recommendable. The reversion of the many stages of ROP for the vascular maturation began in even time and was completed lately in the most advanced stages. The weight showed a most significant risk factor for the presence of premature retinopathy. It was observed ROP with a major frequency related to the low born weight, gestational age and oxygen in the first month of life, which represent the most important risk factors. The exam of the eyes has to be initiated between the 4 to 6 weeks of life. The knowledge of the characteristics of this disease allows the early detection of ROP and the selection of the ideal moment to start the treatment.

▼ Esta pesquisa refere-se ao estudo da constituição subjetiva, educação e desenvolvimento psicomotor de crianças que nasceram pré-termo e estiveram internadas em unidade de terapia intensiva neonatal, participando da posição “Mãe-Canguru”, em 2002, no hospital da Criança Conceição em Porto Alegre, Brasil. O objetivo principal deste estudo foi descrever de que forma os pais conduzem a educação de seus filhos que nasceram pré-termo e participaram da posição "Mãe-Canguru". A abordagem metodológica desta pesquisa foi qualitativa, descritiva e longitudinal do tipo estudo de caso. O estudo foi realizado com seis crianças dos 3 aos 5 anos de idade cronológica que nasceram pré-termo e participaram da posição "Mãe-Canguru" e com seus respectivos pais, através de visitas domiciliares semestrais. Os instrumentos utilizados para a coleta de dados deste estudo foram a entrevista semidirigida, a observação e o protocolo de avaliação do desenvolvimento psicomotor do Exame Neurológico Evolutivo (ENE). Os dados foram categorizados, descritos e analisados de forma longitudinal, conforme a evolução dos casos. As categorias foram analisadas através da perspectiva freudo-lacaniana, de acordo com os referenciais teóricos da linha de pesquisa Personalidade, Cultura, Psicanálise e Educação. Este estudo permitiu verificar a influência do significante prematuridade no discurso dos pais para a educação das crianças, bem como a importância da função paterna como representante da lei e da ordem simbólica para a organização subjetiva da criança. Foi observado uma “minimização” da autoridade de alguns pais das crianças que nasceram pré-termo em relação a significação do mundo para a criança.
The present research is the result of a study of subjective constitution, education and psychomotor development of preterm at Hospital da Criança Conceição in Porto Alegre, Brazil in 2002. The main objective of this study was to describe how children that were hospitalized in a neonatal intensive care unit and who participated in the “Mother-Kangaroo” position parents conduct the education of their preterm children who participated in the “Mother-Kangaroo” position. This research used case studies, and its methodological approach was qualitative, descriptive, and longitudinal. The study was conducted with six preterm children aged between 3 and 5 years who participated in the “Mother-Kangaroo” position and their parents by means of semestral house visits. The tools used for data collection were a semi-directed interview, direct observation, and a protocol of evaluation of psychomotor development of the Evolutive Neurological Exam (ENE). The data was longitudinally categorized, described, and analyzed according to the evolution of cases. The categories were analyzed through the Freudian-Lacanian perspective, following the theoretical referential of the research line Personality, Culture, Psychoanalysis, and Education. ▼ O contexto da prematuridade traz para as mães muitas incertezas em relação ao desenvolvimento de seu bebê e às dificuldades específicas para a interação da díade pelas particularidades inerentes à condição do recém-nascido pré-termo (RNPT), envolvendo profunda adaptação da mãe a mudanças decorrentes da internação e da situação em que se encontra o recém- nascido. Diante da complexidade, a situação de prematuridade pode acarretar para a mãe um sofrimento que poderá desenvolver sentimentos desestruturantes em função das dificuldades acarretadas pela internação e prematuridade. Este estudo tem por objetivo geral analisar o acolhimento feito às mães de RNPTs hospitalizados em Unidade de Terapia Intensiva Neona- tal -UTIN, de um Hospital Amigo da Criança. Como objetivos específicos: Descrever os sentimentos vivenciados pela mãe do RNPT, na perspectiva do acolhimento em UTIN, no Método Canguru e na Casa da Gestante, no contexto de um Hospital Amigo da Criança em Passos- MG; Identificar estratégias de enfrentamento utilizadas pelas mães para lidar com situações causadoras de sentimentos e dificuldades no contexto de um Hospital Amigo da Criança em Passos-MG; Compreender como se dá a humanização de tal atenção, tendo em vista as inter-relações e também compreender de que forma os processos de acolhimento estabelecem relações com a humanização do cuidado nos diferentes ambientes da mãe, em tal contexto. Trata- se de um estudo exploratório e descritivo de abordagem qualitativa, pautado no conceito de humanização, para o processo analítico na modalidade de análise temática. Participaram da pesquisa mães de recém-nascidos pré-termo internados na UTIN de um Hospital Amigo da Criança, utilizando o critério de saturação teórica. Foi realizada, de forma individual, uma entrevista semiestruturada, gravada em áudio e, após a transcrição, foram identificadas três categorias de análise e subcategorias, a partir das verbalizações das participantes. 1- O impacto da separação súbita após o nascimento: A mãe diante do processo de internação: preocupando-se com o filho e A incerteza da sobrevida: apoiando-se na espiritualidade; 2- Vivenciando o Método Canguru: Permanecendo perto do filho na vivência do Método Canguru, Restrições na vivência do Método Canguru; Sentindo-se impotente e desamparada e tendo outros filhos; Permanecendo perto do filho na vivência do Método Canguru; 3- Casa da Gestante:Recebendo apoio e aprendizado: Conversando com outras mães: trocando experiências, Apoio do PROMAI e Tendo apoio da equipe de enfermagem. ▼ Este estudo teve como objetivo conhecer experiências vividas por mães de bebês prematuros e entender as repercussões da prematuridade na maternidade e na construção do vínculo entre mãe e filho. Trata-se de uma pesquisa qualitativa, baseada na análise de conteúdo de questionários respondidos por 28 mães, cujos bebês estiveram internados em Unidade de Cuidados Intensivos. A Teoria do Apego foi utilizada como fundamentação teórica. Duas representações a respeito do exercício da maternidade foram destacadas e relacionadas aos comportamentos de cuidado. A representação da maternidade potente foi caracterizada pela percepção materna da eficiência do sistema cuidador. Esta representação revela a existência de mecanismos de enfrentamento saudável, o que repercute positivamente no vínculo mãe-bebê. A representação da maternidade impotente apontou para a dificuldade de integração da experiência da prematuridade : à : maternidade, interferindo nos comportamentos de cuidado com o filho. Cohcluiu-se que o enfrentamento da crise gerada pela prematuridade implica em um trabalho psíquico de elaboração da experiência, que pode permanecer em estado ativo durante muito tempo. A experiência da prematuridade não elaborada pode colocar em risco o , funcionamento saudável do sistema de cuidado e de apego. A capacidade individual , t de significar e elaborar as experiências foi ressaltada. Foi possível observar que as participantes ampliaram e modificaram positivamente os modelos operativos internos a respeito do cuidar e do enfrentamento de adversidades. As intervenções dos profissionais de saúde com esta população devem focalizar os mecanismos de enfrentamento, o processo de elaboração e os fatores facilitadores e dificultadores da experiência da maternidade prematura
This study aimed to assess the experience of mothers of premature infants and understand the repercussions of prematurity on both the motherhood experience and the formation of the mother-child bond. A qualitative research was carried out based on the content analysis of questionnaires completed by 28 mothers whose babies were admitted to Intensive Care Unit. The Attachment Theory was used as theoretical foundation. Two types of motherhood representations were pointed out and associated with caregiving behaviors. The potent motherhood representation wa characterized by the mother s perception of the efficiency of the caregiving system. This representation reveals the existence of healthy coping strategies, which reflects positively on the mother-child bond. The impotent motherhood representation indicates the difficulty of integrating the experience of prematurity with motherhood, which interferes in the caregiving behavior. The conclusion was thq c6ping with the crisis caused by prematurity requires a psychological internal change, so ttiat the experience, which can remain in an active state for a long time, is elaborated. ▼ This study examines the relationship between mothers and premature babies, during prolonged hospitalization in a neonatal intensive therapy unit (NITU). To consider the matter, an interview with the mother of an extreme premature baby was conducted. She recorded the hospitalization period using a scrapbook, i.e., a book through which hospitalization history is recorded in details, using customized pages and a wide variety of materials. The history of both mother and baby, here called Maria and Matheus, allowed us to explain the importance of the consideration by the speech pathologist/audiologist of such relationship in a hospital setting, having as main reference the aspects of the Winnicottian theory. The study approached particularly the concepts of primary mother concerns and holding, which are fundamental in the first moments of life, defined by the author as absolute dependency. Finally, the study concludes that, even in such an adverse situation as is baby s prolonged hospitalization, the mother is indeed able to develop her own mother style. Therefore, the entire team of professionals involved, including the speech pathologist/audiologist, is responsible for helping increase the offer of holding in an environment different from home; this certainly supports the development of healthy children, who are able to pass to the next stages of growth
O presente estudo trata da relação mãe e bebê prematuro, durante o período de internação prolongada em Unidade de Terapia Intensiva Neonatal (UTIN). Para refletir sobre essa questão, foi realizada uma entrevista com a mãe de um bebê prematuro extremo que fez um registro dos momentos de internação hospitalar, usando para isso um scrapbook - palavra inglesa que significa, literalmente, livro de retalhos e que registra uma história minuciosamente, em páginas personalizadas e com uma grande variedade de materiais. A história vivida por ambos, mãe e bebê, aqui denominados Maria e Mateus, nos permitiu elucidar a importância de o fonoaudiólogo contemplar esse vínculo no setting hospitalar, tendo como referência principal aspectos da teoria winnicottiana. Em especial, foram abordados os conceitos de preocupação materna primária e de holding, primordiais no momento inicial da vida, caracterizado pelo autor como de dependência absoluta. Por fim, foi possível concluir que, mesmo em uma situação tão adversa, como a internação hospitalar prolongada do bebê, a mãe pode sim desenvolver seu estilo de ser mãe, sendo que cabe a toda equipe de profissionais envolvidos, incluindo o fonoaudiólogo, facilitar a oferta de holding em um ambiente que é tão diferente da casa, o que certamente favorece o desenvolvimento de crianças saudáveis, em condições de atravessarem as próximas etapas de amadurecimento

▼ This study compares the effects of two types of breast milk on the weight gain of preterm infants. Twenty seven preterm newborns (born before thirty seven weeks) admitted into the UMC NICU were divided into two groups based on the type of milk they were receiving. The first group of preterm infants were fed with only their mothers’ breast milk while the second group were fed with a combination of their mothers’ breast milk and donated breast milk. Chart reviews and semi-structured questionnaires given to the mothers were used to obtain information on the infants and their mothers. Their weight gain was measured at 7, 14, and 21 days after they began feeding. There was no significant difference in the demographics of the groups. An independent sample T-test showed that at seven days and fourteen days, the infants fed with a combination of their mother’s and donated breast milk gained more weight than those fed with only their mother’s milk (P = .055and P = .087). There was no difference in weight gain at twenty one days (p =.496). Thus, this study suggests a trend that infants who receive combined milk types gain more weight in the first two weeks of feeding.

▼ Pretende-se com o presente estudo verificar se existem mudanças na interacção mãe - bebé prematuro, entre a sua hospitalização na Unidade dos Cuidados Intermédios de Neonatologia, e após a sua alta, ao nível do seu sentimento de confiança, das suas intervenções verbais, do contacto pele-a-pele, e dos seus comportamentos. Os participantes foram quatro díades seleccionadas na Maternidade Alfredo da Costa em Lisboa, com bebés de baixo peso e com idade gestacional entre as 28 e as 35 semanas. Este trabalho assenta numa metodologia qualitativa, utilizando como instrumentos a Entrevista Anamnésica, a Entrevista R e a observação das díades pelo método de Esther Bick, com a sua posterior análise de conteúdo. Do material analisado, verificou-se existirem mudanças nestas díades nos momentos pré-hospitalar, para os momentos pós-alta hospitalar, as quais aumentam os seus comportamentos de fala, intrusividade, e uma diminuição dos seus sentimentos de confiança e contacto pele-a-pele.
The present study seeks to verify the existence of interactions changes between mother and preterm babies in the Intermediate Neonatal Unit Care, and after their hospital discharge, at a level of verbal interventions to the babies, mother intrusive behavior, skin to skin contact, and feeling of confidence. The present study is based on a qualitative methodology using as instruments, the Anamnesis Interview, Interview R, Dyads Observation by Esther Bick Method, and their subsequent content analysis. The participants were four dyads mother- preterm babies selected by Maternidade Alfredo da Costa in Lisbon, with gestacional age between 28 and 35 weeks. The results show us an increase at mother verbal interventions and intrusive behavior, as like as a decrease on skin to skin contact and sense of confidence, after the babies discharge.

▼ A prematuridade com todas as suas consequências, constitui um problema mundial. Portugal não é exceção, sendo a taxa de prematuridade nacional elevada em relação à média europeia.
No Serviço de Medicina Materno Fetal da Maternidade Dr. Alfredo da Costa, onde desempenhamos funções, o alvo dos cuidados de enfermagem inclui um número elevado de gravidezes de risco que terminam em partos prematuros.
A teoria da vinculação contribui para a compreensão da estruturação e manutenção dos laços afetivos do ser humano ao longo da vida. O processo de vinculação tem início durante a gravidez, no entanto, um parto prematuro pelas características do recém-nascido e pelo impacto negativo que tem nos pais, pode comprometê-lo. A transição para a parentalidade que constitui uma crise de maturação no ciclo de vida dos futuros pais, também é dificultada pela expectativa de um parto prematuro. A complexidade dos seus pensamentos sobre o impacto de serem pais, vai agravar-se com o stress e ansiedade inerentes ao internamento hospitalar e à incerteza prognóstica da gravidez e parto.
Ao verificar a lacuna existente no cuidar do casal com alto risco de parto pré-termo, foi desenvolvido um projeto de intervenção com o objetivo de facilitar a aquisição de competências de vinculação pelo casal grávido, perante o recém-nascido prematuro. Sendo os enfermeiros prestadores de cuidados, imprescindíveis para a concretização dos objetivos, foi necessário avaliar o seu grau de motivação e as suas competências sobre a prematuridade. Neste relatório de mestrado pretendemos dar a conhecer as atividades desenvolvidas para o alcance dos objetivos traçados no projeto de intervenção. Entre as atividades desenvolvidas contam-se: a elaboração de um protocolo de visita dos casais em risco de parto prematuro à Unidade de Cuidados Intensivos Neonatais; a efetivação e avaliação dessas visitas e a elaboração de um álbum seriado, para promover as competências no âmbito da parentalidade com o recém-nascido prematuro, nas grávidas/casais que não possam efetuar a visita. Da análise dos questionários aos enfermeiros, para colmatar as necessidades de formação diagnosticadas, resultaram duas ações de formação em contexto de serviço e um dossier sobre a prematuridade; ABSTRACT:

▼ Tem-se verificado que a possibilidade de aparecimento de problemas comportamentais, cognitivos e
motores podem estar relacionados com condições ambientais, familiares e educacionais as quais
determinam a interação entre a criança e o meio.
Objetivo: Analisar se um grupo de crianças prematuras de muito baixo peso (RNMBP) e de extremo
baixo peso (REBP), manifestam indicadores de psicopatologia e se estes estão relacionados com
fatores sociodemográficos, aspetos da história clinica e/ou desenvolvimental.
Instrumento: Para o presente estudo deu-se privilégio a uma análise quantitativa dos dados,
constatando algumas relações entre o perfil obtido do “Child Behavior Checklist para idades 1 ½ - 5”
( CBCL 1 ½ - 5”), elementos da ficha demográfica e história clinica.
Resultados: Verificou-se haver uma maior perceção por parte dos pais de problemas de
comportamento quando se tem um nível socioeconómico baixo, verificando-se a mesma associação na
idade gestacional, ou seja, a uma baixa idade gestacional está a perceção de problemas de
comportamento. Da exploração do baixo peso e duração do tempo de internamento, com problemas de
comportamento não se verificaram resultados significativos. Relativamente ao género, foram
encontradas diferenças estatisticamente significativas.
It has been found that the potential emergence of behavioral, cognitive and motor may be related to
environmental, educational and family which determine the interaction between the child and the
environment.
Objective: The aim of the study was to analyze a group of premature infants of very low birth weight
(VLBW) and extremely low birth weight (ELBW) manifest indicators of psychopathology and
whether these are related do socio-demographic factors, aspects of clinical history and/or
developmental.
Instruments: For the present study was given a privilege to a quantitative analysis of data, noting
some relationship between the profile obtained from the “Child Behavior Checklist for ages 1 ½ - 5” (
CBCL 1 ½ - 5”), components of elements of socio-demographic data sheet.
Results: Found to be a greater perception by parents of behavior problems when you have a low
socioeconomic level, verifying the same association in gestational age, in a lower gestational age is the
perception of behavior problems. Exploitation of low birth weight and duration of hospitalization, whit
behavior problems, there were no significant results. Regarding gender differences were statically
significant.

▼ A capacidade de nomeação táctil é a competência de obter a informação sensorial do
ambiente, de um objeto e/ou de movimentos do corpo, processar e integrar esses inputs
sensoriais no sistema nervoso central e usar a informação sensorial para reconhecer e
dar nome a um objeto. A exploração da capacidade de nomeação táctil vai permitir
verificar de uma forma empírica se o acesso à linguagem e se o percurso efectuado pela
informação ao nível linguístico é feito de acordo com o esperado. O presente estudo tem
como objectivo comparar a capacidade de nomeação táctil de crianças de 4 anos e 5
anos de idade com nascimento de termo e com nascimento pré-termo (entre as 28 e 36
semanas de gestação). Participaram no estudo 60 crianças, 20 prematuros e 40 de termo,
que frequentam jardins-de-infância da área da grande Lisboa, tendo sido avaliadas
através de uma tarefa de nomeação táctil de objectos comuns. Não foram verificadas
diferenças significativas entre o grupo de contraste e os prematuros, embora
tendencialmente os prematuros tenham obtido valores mais baixos, no limiar da
significância. Não se verificou correlação do desempenho na nomeação táctil com o
número de semanas de gestação, nem com o peso ao nascer. Também não foi verificada
correlação com os anos de escolaridade dos pais, em nenhum dos grupos. No grupo de
contraste não havia diferença entre rapazes e raparigas, mas nos prematuros o
desempenho destas foi melhor. Por limitações do estudo não foi possível valorizar as
diferenças nos resultados obtidos através da mão direita e da mão esquerda.
The capacity of tactile naming is the ability to obtain sensory information from
the environment, an object and/or body movements, process and integrate these sensory
inputs to the central nervous system and use sensory information to recognize and name
an object. Exploiting the ability to tactile naming recognition will allow to empirically
verify whether access to the language and the route information carried by the linguistic
level is done according to the expected. The present study aims to compare the ability of
tactile naming of children with 4 and 5 years of age with birth at term and birth at
preterm (between 28 and 36 weeks of gestation). Sixty children, 20 preterm and 40
term, attending kindergarten in the area of great Lisbon, born at term and preterm, have
been evaluated through a tactile naming task with common objects, participated in this
study. No significant differences between the group of premature and contrast were
observed, however the group of premature children have obtained lower values, in the
limit of significance. There was no correlation between performance on the tactile
naming with the number of weeks of gestation or with birth weight. Also no correlation
was verified through years of parental schooling in any groups. In contrast there was no
group difference between boys and girls, but the performance in preterm girls was

▼ O estudo em questão tem como objetivo principal, a caracterização do perfil desenvolvimental de bebés nascidos com muito baixo peso e extremo baixo peso no terceiro ano de vida. Deste modo, os objetivos específicos consistem em explorar o impacto de determinados fatores de risco biológico (Idade gestacional, Peso á nascença, tempo de internamento) nos resultados obtidos na escala de desenvolvimento no terceiro ano de vida, bem como explorar o impacto de determinados fatores ambientais (frequência de creche) nos resultados obtidos na escala de desenvolvimento no terceiro ano de vida.
Assim, procedeu-se á aplicação do instrumento Griffiths Mental Development Scales 0 – 2 years, que abrange idades, desde o nascimento até aos 24 meses e é composta por cinco subescalas, sendo elas, Coordenação do olho e mão, Audição e linguagem, Locomotora, Execução e Pessoal - Social, e o seu tempo de administração compreende cerca de 45 minutos.
Em modo de conclusão verificou-se que um maior tempo de internamento, um peso menor á nascença e uma idade gestacional menor, não influenciam os resultados de cada criança na Escala Desenvolvimental de Ruth Griffiths. Por outro lado, foi encontrada uma associação entre a frequência de creche e resultados mais elevados na Escala de Desenvolvimento de Ruth Griffiths, o que demonstra a importância da estimulação precoce em crianças nascidas prematuras.
The main goal of this study is to characterize the developmental profile of infants born with very low birth weight and extremely low birth weight in the third year of life. Thus, the specific objectives are to explore the impact of certain biological risk factors (gestational age, birth weight, length of stay) the results obtained in the scale of development in the third year of life, and to explore the impact of certain factors environmental (frequency daycare) results on the scale of development in the third year of life.
Thus, we proceeded to the application of the instrument Griffiths Mental Development Scales 0-2 years, covering ages from birth to 24 months and consists of five subscales them the eye and hand coordination, hearing and language, locomotion, execution and personal - social and your time management comprises approximately 45 minutes.
In so concluding it was found that a longer hospital stay, a lower weight will be born and a lower gestational age do not influence the results of each child in Ruth Griffiths Developmental Scale. Moreover, an association was found between the frequency of day care and higher results in Scale Development Ruth Griffiths, demonstrating the importance of early stimulation in preterm infants

O presente estudo teve como objetivos caracterizar sócio-demograficamente a amostra de
mães de crianças prematuras e obter dados clínicos em relação ao nascimento das crianças,
explorar os estilos de vinculação e as estratégias de coping em mães de crianças prematuras
em comparação com mães de crianças de termo, e ainda, verificar, dentro das estratégias de
coping, quais as mais usadas para cada estilo de vinculação para das duas amostras.
Para tal desenvolveu-se um estudo de carácter transversal com duas amostras, uma de 45
mães de crianças prematuras e outra de 45 mães de crianças de termo, ou seja, o grupo de
controlo. Os dados relativos à amostra de crianças prematuras foram recolhidos no
Departamento de Pedopsiquiatria e Saúde Mental da Infância e da Adolescência do Centro
Hospitalar do Porto enquanto que os dados das mães de crianças de termo foram recolhidos
no Agrupamento de Escolas de S. João da Madeira. Todas as participantes responderam a um
Questionário sócio-demográfico e clínico desenvolvido para o efeito, a Escala de Vinculação
do Adulto e o Brief COPE.
Os resultados apontam para a inexistência de diferenças entre os dois grupos quanto ao
estilo de vinculação, sendo que a maioria das mães apresenta um estilo de vinculação
“seguro”. Quanto às estratégias de coping apenas se verificaram diferenças na estratégia de
coping “aceitação”, sendo uma estratégia mais utilizada pelas mães de crianças prematuras.
Por último, verificou-se que as mães de crianças prematuras com vinculação “preocupada”
usam mais a estratégia “coping ativo” enquanto nas de termo são as que têm estilo de
vinculação “seguro”. Na estratégia “negação” as mães prematuras que mais utilizam são as
com estilo “desligado” e nas de termo “amedrontado”. Na estratégia “uso de substâncias” são
as mães prematuras com estilo “seguro” e nas de termo são as com estilo “amedrontado” que
mais a utilizam. Não se verificaram diferenças nas restantes estratégias. This study aimed to socio-demographically characterize the sample of mothers of premature
infants and obtain clinical data in relation to the birth of children, explore the styles of
attachment and coping strategies in mothers of premature infants compared with mothers of
term, and also verify what are the coping strategies most used for each style of binding for the
two samples.
For such it was developed a transversal study with two samples, one of 45 premature
infants’ mothers and other 45 term children’ mothers, i.e. the control group. Data regarding
the sample of premature infants were collected at the Child and Adolescent Psychiatry
Department of the Oporto’ Hospital Center whilst the data regarding mothers of term children

▼ [EN] Preeclampsia leading to growth restriction is a bilateral process resulting from vascular placental insufficiency and affecting both the mother and the foetus. The causal treatment is birth, most commonly preterm and very often iatrogenic. Prematurity causes an increase in the disease burden among new-borns. Therefore, the ability to reach the 34th gestational week is essential. Diagnosis is based on a combination of clinical, paraclinical, ultrasound and Doppler findings. Accurate and timely diagnosis is necessary, in order to avoid, on one hand long-term damage quo ad vitam both in the mother and in the foetus, and on the other – unnecessary iatrogenic preterm birth. Recent data demonstrates that administering magnesium sulphate parenterally may reduce the incidence of infantile cerebral palsy. The aim of this study is to evaluate the effect of antenatal MgSO4 administration on peripartum haemorrhages, periventricular leukomalacia, and infantile cerebral palsy among preterm babies, born between 28+0 and 31+6 gestational weeks. //////////// //////////// [BG] Прееклампсията и свързаното с нея забавяне на растежа на плода е билатерален процес, засягащ както майката така и плода в основата, на който е васкуларната плацентарна инсуфициенция. Каузалното лечение е раждането, най-често преждевременно и много често ятрогенно. Недоносеността и незрелостта са причина за увеличена заболеваемост на новороденото и затова достигането на 34 г.с. е от есенциално значение. Диагнозата е най- често калейдоскоп от клиника, параклиника, ултразвукова и доплерова диагностика. Точната и навременна диагностика е необходима за да се избегнат от една страна дългосрочни щети quo ad vitam както за майката, така и на плода, а от друга страна да се избегне ненужното ятрогенно преждевременно раждане. В последно време се натрупаха данни, че чрез парентералната апликация на магнезиев сулфат може да се намали честотата на инфантилната церебралната пареза, която е тежко усложнение в следствие на перивентрикуларни мозъчни хеморагии, при преждевременно родени преди 32 г.с. В този случай инфузии с магнезиев сулфат се използват като невропофилактика. Настоящата дисертация е едно проучване на действието на антенаталната апликация на MgSO4 при недоносени от 28+0 до 31+ 6 г.с. с цел намаляване на перипарталните хеморагии, респективно на перивентрикуларната левкомалация и детската церебрална пареза.

▼ [EN] The retinopathy of prematurity (ROP) is an important reason for blindness of both eyes among the premature children in countries with semi-developed economies. The disease is potentially preventable through applying effective national screening programs. Aim The aim of the current work is to explore the regional and national characteristics of the retinopathy of prematurity and to apply algorithms for solving the problems connected with this disease. Materials and methods A retrospective research of the medical files of MHAT “Prof. S.Kirkovich” – Stara Zagora was held. The research covered the files of all registered, alive, premature children, who were treated in the Neontological section of the hospital between January 1998 and March 2002. A total of 803 premature children were analyzed.Active screening for the early finding of eye pathology among premature children was held in the period April 2002 – August 2010. The screening took place among the premature children in the Neonatological section of Hospital “Prof. S. Kirkovich” – Stara Zagora and the section for premature children of the centre for medical and social services for children in Stara Zagora. The total amount of the children in the screening research is 1029. Until 2015 a continuous tracing of the premature children, who had passed the screening research, was being held. The count of the tracked children is 569. The following examinations methods were used in Stara Zagora area: Clinical methods: Outward inspection of head, face, reactions for light of the pupils, transparency, indirect ophthalmoscopy after mydriasis, ocular acuity, ophthalmoscopy, ocular motility, refraction, biomicroscopy of anterior segment and tonometry. Screening methods: Outward inspection of head and face, reactions for light of the pupils, transparency and indirect ophthalmoscopy after mydriasis. Screening program: first ophthalmological inspection – during the first 2-4 weeks of the life of the premature children; further ophthalmological inspections – every 7-10 days until the process of vascularity of the retina is completed, once in a month until the age of 6 months, once every two months until the age of 1 year, and once in a year until the age of 6 years. A national research of the approach to retinopathy has taken place in the period April 2015 – September 2015, via questionnaire, which was filled by ophthalmologists, performing inspections of premature children from all 28 administrative regions in Bulgaria. An algorithm for examination of children with retinopathy of prematurity was developed. There is a national system, which contains data for the neonatal period, the period until the age of 6 months, the period until the age of 1 year, and the periods in the ages of 1-5 years, 6-12 years,13-18 years. Statistical approaches are used to analyze data. Results In the retrospective research, different phases of ROP were found by 1.7% of the premature children (14 out of 803). ▼ The purpose of the study was to determine the causes, of the increased perinatal
mortality, identify and describe other factors contributing to the increased perinatal
mortality rate in a rural healthcare facility situated in Sekhukhune district in Limpopo
province, and to formulate the recommendations that will reduce the perinatal mortality
rate based on the results. A quantitative, descriptive, cross-sectional and retrospective
design was conducted. The study population was one hundred and sixty two (162)
records of babies who died in the perinatal facility from the 1st January 2015 to the 31st
December 2015 with a gestational age of about 28 weeks or more. No sampling was
done, but a census was used. The sample comprised of one hundred and sixty two (162)
of all the records related to perinatal mortality. Data were collected from patients’ records
by using a checklist. Analysis of the data was performed by the IBM Statistical Package
for Social Sciences (SPSS) version 14 computer software. Frequency tables and pie
graphs were used to present the data.
The results indicated that 75.3% (n=122) of the records were associated with health
personnel as a factor contributing to perinatal mortality. Furthermore, preterm cases
accounted for 45.1% (n=73) and
prematurity accounted for 37.0% (n=60) of the cases of
perinatal mortality. Therefore, preterm births and
prematurity are risk factors that should
be managed immediately after birth, and all babies should be managed prior to being
transferred to the other healthcare institutions.
The recommendations are that the education of patients about early antenatal visit, signs
of labour and danger signs during pregnancy and training of healthcare workers on
record-keeping have to be done on a continuous basis. Managers should conduct quality
improvement programmes, benchmarking and implement maternal and neonatal
guidelines in the clinical area throughout pregnancy.

▼ A large body of literature shows that compared to children born at term, preterm- children are at increased risk for difficulties with inattention and hyperactivity. Less consistency exists, however, in the limited body of research exploring the contribution of early biological risk to behavioral disinhibition within the population of children born prematurely. Therefore, our goal was to examine perinatal variables that may influence activity level and hyperactivity among preterm preschoolers. Ninety-eight preterm (23.4 - 33.9 weeks gestation) preschoolers (3-4 years) participated in the study. Direct measures of inattention and hyperactivity as well as parental ratings were used to evaluate behavior. We used simultaneous linear regression analyses with gestational age, perinatal complications, and growth rate z-score (birth weight standardized by gestational age) as predictors of interest. Socioeconomic status, sex, multiple gestation, and age at testing were our "covariates." Surprisingly, we found that within our preterm sample, total number of complications was inversely related to the CBCL Externalizing Problems scale score. Sex, but not perinatal medical status, was significantly related to performance on the NEPSY-II Statue subtest, with males displaying reduced ability for motor inhibition. Preschoolers with a greater number of complications obtained lower Externalizing Problems scale scores, suggesting a link between increased perinatal risk and reduced behavioral initiation. The reduced motor inhibition in boys, however, is consistent with the expected male outcome disadvantage documented in the
prematurity literature.

▼ Children born prematurely or at very low birth weight (VLBW) have an increased risk for hypoxic ischemic brain injury (HI). HI refers to a lack of adequate blood and oxygen flow in the brain. HI can also occur in the term infant due to birth complications such as prolonged labor, placental dysfunction, or cord prolapse. In both populations (though exact patterns of neuropathology vary) brain damage is likely to occur in the form of decreased hippocampal and cortical volume, and enlargement of the ventricles (Kesler et al., 2004, Nagy et al., 2009). Resulting neuropathology can in turn lead to cognitive and behavioral deficits as children age. For example, children who suffer from HI are more likely to be diagnosed with developmental disabilities such as ADHD, as well as language and memory impairments throughout childhood.
Fortunately, hypoxia ischemia can be modeled in neonatal rats via carotid artery ligation, followed by a period of induced hypoxia. This animal model can produce brain injuries that roughly equate to functional injuries seen in term infants with HI when performed on P7, and also leads to similar cognitive and behavioral impairments. Using this P7 HI model to assess behavioral outcomes, our lab has previously reported deficits in HI males on a Morris water maze task (a measure of simple spatial learning and memory). Based on those findings, the current study sought to assess HI males on a more difficult memory task: the eight arm radial water maze, in which performance requires a heightened reliance on spatial working memory. This paradigm was modified to make the task progressively harder throughout 8 testing weeks, by way of opening additional arms every 2 weeks. Thus, testing proceeded as follows: Condition 1 (Weeks 1 and 2) – 3 arms open; Condition 2 (Weeks 3 and 4) – 5 arms open; Condition 3 (Weeks 5 and 6) – 8 arms open; and Condition 4 (Weeks 7 and 8) – 8 arms open, with a 1 hour delay between sample and test trials.
Results revealed that HI animals performed close to Sham levels in the earlier weeks of testing (Condition 1-3). However, during Week 2 of Condition 4, when a retention interval of 1 hour was introduced, we saw an emergent deficit in HI animals with more errors made. Further analysis of Weeks within each Condition revealed that both HI and Sham animals were learning the task as testing progressed. This combination of findings indicates that though both groups could learn the memory task, HI animals showed a deficit on the hardest Condition (implementation of an hour delay). Interestingly, though HI animals were making more errors than Shams in the later weeks of testing, they also were taking significantly
less time to make an arm choice – suggesting HI animals may act more impulsively. Taken together, our findings indicate a spatial working memory impairment in HI animals, as well as a potential attentional deficit that might be associated with the characteristics of ADHD in clinical populations. Pesquisas anteriores indicaram que, comparativamente às mães de termo, as
mães dos bebés prematuros apresentavam perceções mais negativas acerca da
gravidez e do parto, mostravam-se mais preocupadas com a saúde e desenvolvimento
do seu bebé, contudo mais confiantes quanto ao futuro e mais otimistas quanto às
suas capacidades para se relacionarem positivamente com os seus filhos. Embora
alguns estudos indiquem que relações entre estas representações estão associadas à
qualidade subsequente da relação mãe-filho(a), pouco sabemos sobre as
representações das mães dos bebés prematuríssimos (bebés muito e extremamente
prematuros).
Neste estudo, procurou-se investigar as representações maternas em díades
com bebés prematuríssimos (IG<32 semanas), comparando-as com as díades com
bebés prematuros (IG>32 e <36 semanas). Foi realizada, para o efeito, uma entrevista
materna nas primeiras 72 horas após o nascimento a 20 mães de bebés
prematuríssimos e 40 mães de bebés prematuros acerca das suas representações
relativamente à gravidez, parto, nascimento antecipado, maternidade e expectativa
sobre o futuro desenvolvimento do bebé.
É no grupo dos bebés prematuríssimos que se verifica uma maior percentagem
de mães que exprimem um sentimento de choque/pânico face à notícia da
prematuridade. No entanto, e curiosamente, existem neste grupo mais mães otimistas
quanto ao futuro dos filhos, se estes sobreviverem, do que no grupo das mães dos
bebés nascidos de pré-termo. O grupo das mães dos prematuros subdivide-se entre as
mães que antecipam imensas dificuldades e aquelas que não esperam dificuldades.
Os dados obtidos serão discutidos à luz das práticas de intervenção precoce
suportadas na evidência e na ação preventiva junto de pais de crianças nascidas de
extremo pré-termo.
ABSTRACT
Previous researches show that, compared with term mothers, mothers of
premature babies reported more negative perceptions about pregnancy and birth; they
revealed themselves more worried with their babies’ health and development, although
more confident about the future and more optimistic about their ability to engage
positively with their children. Even though some studies emphasize that relationships
among such representations are associated with the subsequent quality of the motherchild
relationship, not much is known about the representations of mothers of very
premature infants. This study attempted to investigate mother representations in dyads
with extremely premature babies (GA<32 weeks) in comparison with dyads with
premature babies (GA>32 and <36 weeks). For the purpose, interviews were carried
out in the first 72 hours after birth to 20 mothers of extremely premature babies and 40
mothers of premature babies about their representations related to pregnancy, birth,
