Objetivo - Comparar índices 3D Power Doppler (PD3D) da circulação úteroplacentária (UPC) no primeiro e segundo trimestre em pacientes que desenvolveram pré-eclâmpsia (PE) e aquelas que não fizeram e testar a hipótese de que os parâmetros da vascularização e intensidade de fluxo da placenta, tal como determinado por PD3D, são diferentes em gestações normais em comparação com pré-eclâmpsia. Método - Foi realizado um estudo observacional prospectivo usando PD3D para avaliar a perfusão da placenta em 96 gestantes que fizeram o exame ecográfico de rotina entre 11 e 14 semanas. O índice vascular (VI), o índice de fluxo (FI) e o índice de fluxo vascular (VFI) por histograma Doppler tridimensional foram calculados. Todas pacientes repetiram o exame entre 16 e 20 semanas. Após o nascimento as pacientes foram classificadas como normais ou com pré-eclâmpsia. Resultados - Os índices vasculares placentários incluindo VI, FI e VFI foram significativamente menores em placentas com PE em comparação com os controles no exame realizado no segundo trimestre (p <0,001). Não houve diferença estatística nas pacientes examinadas no primeiro trimestre. Conclusão - Nossos resultados sugerem que a avaliação de índices vasculares placentários com Power Doppler 3D no segundo trimestre tem o potencial para detectar as mulheres em risco para o desenvolvimento posterior de PE.

Objective - To compare 3D power Doppler indices (3DPD) of utero-placental circulation (UPC) in the first and second quarter in patients who developed preeclampsia and those who did not and to test the hypothesis that the parameters of vascularization and placenta flow intensity, as determined by three-dimensional ultrasound (3D) are different in normal pregnancies compared with preeclampsia. Methods - A prospective observational study using 3D power Doppler was performed to evaluate the placental perfusion in 96 pregnant women who came to do the ultrasound routine between 11 and 14 weeks. The placental vascular index (VI), flow index (FI), blood vessels and blood flow index (VFI) by three-dimensional Doppler histogram were calculated. All patients repeated the exam between 16 and 20 weeks. The outcome was scored as normal or preeclamptic. Results - The placental vascular indices including VI, FI and VFI were significantly lower in preeclamptic placentas compared with controls in the study performed in the second trimester (p <0.001). There was no statistical difference in the patients examined in the first trimester. Conclusion - Our findings suggest that 3D-Power Doppler assessment of placental vascular indices in the second trimester has the potential to detect women at risk for subsequent development of PE.

▼ Objective: To determine the role of 3-Dimensional Ultrasound(3-D) and Power Doppler (3-DPD) in the diagnosis of the uterusmalignancy and to examine the diagnostic accuracy of this newultrasonographic technique in the detection of uterine cancer.Methods: A preoperative analysis by 3-D and 3-DPD wasperformed between November 2003 and November 2012 of 636 womenreferred uterine bleeding within the first Department of Obstetrics andGynecology, University of Athens, in Alexandra Hospital. The age ofthe women studied was 47,418,8 years with a range (18-72 years).Three dimensional ultrasonographic method is a new diagnostictechnique that provides precise and additional information of theregion of interest by the rotation of the mechanical transducerthrough 360ο. Multiple sections of uterine pathology, rotation in thethree axis (x, y, z), translation and reconstruction of (3-D) plasticimages allow more meticulous and detailed evaluation, especiallywhen modes used.Comprehensive (3-D) display allows interactive qualitative andquantitative analysis of the tumor microcirculation in threedimensions.In the present study, were analyzed 4 sonomorphologicalcriteria (endometrial thickness, endometrial volume, subendometrialhallo, intracavitary fluid) and 3 criteria by 3-DPD (vessel’sarchitecture, branching pattern and tumoral blood flow (R.I.)), basedon Kurjak’s scoring system. When it was necessary some patientswere evaluated more detailed with imaging techniques (CT scan,M.R.I.), tumor markers, and at last were noted the family andpersonal history and analyzed demographic considerations.All of the women were operated and comparison of theultrasound findings with the histopathological diagnosis wasconducted. The malignant of the uterus lesions were classified according to the International Federation of Gynecologists andObstetricians (F.I.G.O).Statistical analysis: The diagnostic accuracy of 3-D ultrasoundwas estimated by R.O.C. curve and for statistical analysis of the data,was used the x²-test.Results: In the present study was included the largest numberof patients (636 women) according to international literature. Thewomen that were in reproductive age were examined independently ofthe phase of their menstrual cycle (proliferative or secretory) thusproviding a more objective result.The histopathological diagnoses were 450 benign tumors of theuterus and 186 malignant. The combined use of 3-D and 3-DPD wasshowed 430 benign and 206 malignant tumors of the uterus. A cut offvalue of 6 was used for the discrimination of benign and malignanttumors based on a specific scoring system concerning 3-Dultrasonographic criteria.Three dimensional methods diagnosed 12 malignant tumors asbenign (false negative) and 32 benign tumors of the uterus asmalignant (false positive). ▼ Acoustic properties, such as the characteristic impedance and the complex propagation constant, of porous materials have been traditionally characterized based on pressure-based measurement techniques using microphones. Although the microphone techniques have evolved since their introduction, the most general form of the microphone technique employs two microphones in characterizing the acoustic field for one continuous medium. The shortcomings of determining the acoustic field based on only two microphones can be overcome by using numerous microphones. However, the use of a number of microphones requires a careful and intricate calibration procedure. This dissertation uses laser Doppler anemometry (LDA) to establish a new measurement technique which can resolve issues that microphone techniques have: First, it is based on a single sensor, thus the calibration is unnecessary when only overall ratio of the acoustic field is required for the characterization of a system. This includes the measurements of the characteristic impedance and the complex propagation constant of a system. Second, it can handle multiple positional measurements without calibrating the signal at each position. Third, it can measure three dimensional components of velocity even in a system with a complex geometry. Fourth, it has a flexible adaptability which is not restricted to a certain type of apparatus only if the apparatus is transparent. LDA is known to possess several disadvantages, such as the requirement of a transparent apparatus, high cost, and necessity of seeding particles. The technique based on LDA combined with a curvefitting algorithm is validated through measurements on three systems. First, the complex propagation constant of the air is measured in a rigidly terminated cylindrical pipe which has very low dissipation. Second, the radiation impedance of an open-ended pipe is measured. These two parameters can be characterized by the ratio of acoustic field measured at multiple locations. Third, the power dissipated in a variable RLC load is measured. The three experiments validate the LDA technique proposed. The utility of the LDA method is then extended to the measurement of the complex propagation constant of the air inside a 100 ppi reticulated vitreous carbon (RVC) sample. Compared to measurements in the available studies, the measurement with the 100 ppi RVC sample supports the LDA technique in that it can achieve a low uncertainty in the determined quantity. This dissertation concludes with using the LDA technique for modal decomposition of the plane wave mode and the (1,1) mode that are driven simultaneously. This modal decomposition suggests that the LDA technique surpasses microphone-based techniques, because they are unable to determine the acoustic field based on an acoustic model with unconfined propagation constants for each modal component.

Estudiar el rendimiento diagnóstico de la ecografía transrectal en escala de grises, power doppler y del contraste sonográfico en el diagnóstico precoz por imagen del cáncer de próstata localizado. 2º. Comparar la rentabilidad diagnóstica de la biopsia prostática aleatoria con la biopsia ecodirigida mediante power doppler y contraste sonográfico, en el diagnóstico precoz por imagen del cáncer de próstata localizado. Material y métodos: Se diseñó un estudio epidemiológico prospectivo y transversal de ámbito provincial, para estudiar el rendimiento diagnóstico de la ecografía transrectal en escala de grises (EG), Power Doppler (PD) y del contraste sonográfico (CS) en el diagnóstico precoz por imagen del cáncer de próstata localizado y comparar la rentabilidad diagnóstica de la biopsia prostática aleatoria (BA) frente a la biopsia ecodirigida mediante power doppler y contraste sonográfico en el diagnóstico precoz del cáncer de próstata localizado, sobre las personas adscritas al área de salud del Hospital Universitari i Politécnic La Fe de Valencia. El estudio se llevó a cabo por el Servicio de Urología del Hospital Universitari i Politécnic La Fe de Valencia. La adquisición de los datos se inició en Enero de 2002 y finalizó en Enero de 2010. El procesamiento de los datos se ha realizado diferenciando dos fases, una fase analítica y otra descriptiva. En la fase analítica, se comparó el rendimiento diagnóstico del tacto rectal, escala de grises, power doppler y contraste sonográfico, calculando los tests diagnósticos de Sensibilidad (S), Especificidad (E), Exactitud (Ex), Coeficientes de Probabilidad, Odds Ratio Diagnóstica (ORD) y Valores Predictivos (VP) para estas variables, en la población general y en función de su estadio clínico (cT1 y cT2), con el objetivo de valorar el rendimiento diagnóstico de la imagen ecográfica transrectal mediante cada uno de los elementos de trabajo (EG, PD y CS). En la división por estadios clínicos de la población, se excluyeron los casos con estadio clínico cT2b (“nódulo palpable bilateral”), por varios motivos: escaso número de pacientes englobados en este estadio (n:7), a que la presencia de un nódulo palpable bilateral probablemente supondría un mayor rendimiento diagnóstico en las variables de estudio por imagen y a que a estos pacientes, no se les administró contraste. Por todo ello, la división por estadios de la población quedó reducida al estadio cT1 (tacto rectal: “adenoma”) y al estadio cT2 (tacto rectal: “nódulo palpable”). ▼ Despite knowing the importance of maintaining steady blood flow to the vulnerable neonatal brain, there is currently no reliable way to quantify regional brain perfusion at the bedside. Consequently there are gaps in our understanding of the pathophysiological changes that occur at times when the neonate is at risk of neurological harm. Fractional Moving Blood Volume (FMBV), which is an index generated from the quantification of the power Doppler ultrasound signal in an area, is a technique to evaluate regional perfusion that has been validated in the human fetus. The quantified power Doppler signal in a selected area has been shown to correlate with the amount of blood flow in that region. This thesis investigates the application of FMBV to the neonatal brain and its value as a tool to evaluate perfusion.Initial studies contained within this thesis focused on the repeatability of FMBV when applied to power Doppler images in the neonate. I established a neonatal reference range for FMBV in the basal ganglia across a range of gestations. This involved a rigorous interrogation of the image acquisition and analysis sequence and the development of an application to analyse images. I also assessed temporal variation during the transitional period through the first 48 hours of life. Following on from this, I investigated the feasibility of using novel impedance indices to evaluate tissue resistance as a complementary tool in the evaluation of tissue perfusion.The results of these studies show that FMBV is a reproducible technique that can be applied to neonatal images. A reference range for basal ganglia FMBV has been defined for all neonates. In addition, serial assessment during transition demonstrated anticipated interval increases in FMBV. Finally, the generation of tissue impedance indices, as a potential adjunct to perfusion assessment, was feasible and reproducible.FMBV is applicable to the neonatal brain and an optimised program is now available that uses ultrasound data from neonatal images to calculate FMBV. There are now opportunities for assessment of FMBV in settings where direct evaluation of regional perfusion may provide insights into the pathogenesis of cerebral injury. In the future, the development of an application that calculates FMBV from three dimensional images offers the exciting possibility of evaluating perfusion by quantifying the signal from a volume of tissue rather than a two dimensional area. Sentinel lymph node (SLN) biopsy is the standard of care for breast cancer patients with non-palpable axillary lymph nodes. We evaluated the usefulness of contrast-enhanced ultrasonography in preoperative detection of malignant SLNs. Methods: 50 patients with breast cancer (median age: 60 years) underwent a color power Doppler ultrasonography with intravenous contrast (Sonovue®) preoperatively, and findings suggestive of metastatic disease to the SLN were documented. The final histopathological report and the radiological preoperative record were compared. Finally, the sensitivity, specificity and diagnostic accuracy of this evolving diagnostic modality were calculated. Results: Contrast-enhanced ultrasound scan identified a negative SLN in the axilla of 27 patients and final histopathology was negative for 30 cases in total, so negative predictive value was calculated as 90% and positive predictive value was 75%. Overall sensitivity was 83.33% and specificity was 84.38%. Moreover, the ability of contrast-enhanced ultrasound to differentiate between SLN status was only statistically significantly correlated with the actual final histopathological report (p < 0.001), while successful ultrasound prediction was not correlated with any factor. Conclusions: SLN status can be evaluated preoperatively using contrast-enhanced color Doppler ultrasonography with high accuracy.

Η βιοψία του λεμφαδένα φρουρού αποτελεί την ενδεδειγμένη θεραπευτική τακτική στην αντιμετώπιση των ασθενών με καρκίνο μαστού και μη-ψηλαφητούς λεμφαδένες στη μασχάλη. Μελετήσαμε σε πενήντα ασθενείς (διάμεση ηλικία 60 έτη) το λεμφαδένα φρουρό με έγχρωμο Doppler υπερηχοτομογράφημα με την προσθήκη σκιαγραφικού υπερήχων και συγκρίνουμε τα ακτινολογικά αποτελέσματα με την παθολογοανατομική βιοψία του αδένα. Καταγράφονται η ευαισθησία και η ειδικότητα της μεθόδου στο 83,33% και 84,38% αντίστοιχα. Η δυνατότητα του έγχρωμου Doppler υπερηχοτομογραφήματος να εκτιμήσει το λεμφαδένα φρουρό είναι στατιστικά σημαντική (p<0.001)και εξαρτάται στην πολυπαραγοντική ανάλυση μόνον από το αποτέλεσμα της τελικής βιοψίας του λεμφαδένα φρουρού. Συμπερασματικά η μέθοδος έχει αρκετή αξιοπιστία ώστε να χρησιμοποιηθεί προεγχειρητικά για την αξιολόγηση του λεμφαδένα φρουρού.

Retard de croissance intra-utérin (RCIU) et prééclampsie (PE) sont des complications majeures de la grossesse humaine et sont le plus souvent due à une insuffisance de vascularisation utéro-placentaire. Notre objectif était d'évaluer l’angiographie Doppler énergie tridimensionnelle (PDA) comme nouvel outil de dépistage de la PE et du RCIU et d’étude de la fonction placentaire et du RCIU sur modèles animaux. La corrélation entre les indices Doppler 3D et l'écoulement de sang réel au sein de l'unité utéro-placentaires a d'abord été évaluée sur modèle de brebis gravide, ainsi que l'impact des réglages de la machine. Un degré de corrélation plus élevé a été observé pour VI et VFI (r = 0,86 et 0,82 respectivement, p <0,0001) que pour FI (r = 0,64, p <0,0001).L'intérêt de la technique a été ensuite démontré dans un modèle de RCIU chez le lapin (femelles traitées par du L-NAME).Troisièmement, la valeur prédictive du PDA comme test de dépistage du RCIU et de la PE a été démontrée par la réalisation d'une étude prospective multicentrique chez 70 femmes enceintes à bas risque (AUC 0,95, 100% VAN avec une spécificité de 85% pour un seuil de 36.784 FI placentaire).

Intra-uterine growth retardation (IUGR) and preeclampsia (PE) are major complications of human pregnancy & are most often due to an insufficient utero-placental vascularization. Our aim was to evaluate the three-dimensional power Doppler angiography (PDA) as a new tool for the screening IUGR & PE & for the study placental function and IUGR in animal models. The correlation between 3D Doppler indices and the real blood flow within the utero-placental unit was first evaluated in the pregnant sheep model, as well as the impact of machine settings. A higher correlation degree was observed for VI and VFI (r = 0.86 and 0.82 respectively p<0.0001) than for FI (r = 0.64; p<0.0001).The interest of the technique was secondly demonstrated in a rabbit IUGR model (females treated with L-NAME).Thirdly, the predictive value of PDA as a screening test for IUGR & PE was demonstrated by conducting a prospective multicentric study in 70 low risk pregnant women (AUC 0.95, 100% NPV with a specificity of 85% for a 36.784 placental FI threshold).

The purpose of this study was to explore the role of 3-dimensional (3D) power Doppler (PD) sonography to discriminate between benign and malignant cystic-solid and solid vascularized adnexal masses and to define cutoff values for 3D PD indices to be used in a clinical setting. METHODS. A total of 143 consecutive women (mean age, 50.4 years; range, 17-82 years) with diagnoses of cystic-solid or solid vascularized adnexal masses on B-mode and 2-dimensional PD sonography were evaluated by 3D PD sonography before surgery. Three-dimensional PD sonography was used to assess vascularization within papillary projections and solid areas with a virtual organ computer-aided analysis program. Three-dimensional PD vascular indices (vascularization index [VI], flow index [FI], and vascularizationflow index [VFI]) were automatically calculated. A definitive histologic diagnosis was obtained in each case. RESULTS. A total of 113 masses (74%) were malignant, and 39 (26%) were benign. Morphologic evaluation revealed 30 unilocular solid masses (19.7%), 43 multilocular solid masses (28.3%), and 79 mostly solid masses (52%). The mean VI (9.365% versus 3.3%; P < .001), FI (34.318 versus 28.794; P < .001), and VFI (3.233 versus 1.15; P < 0.01) were significantly higher in malignant tumors. No differences were found in the resistive index, pulsatility index, and peak systolic velocity. Receiver operating characteristic analysis revealed an area under the curve of 0.77 (95% confidence interval, 0.69-0.85), 0.71 (0.60-0.81), and 0.75 (0.66-0.83) for the VI, FI and VFI, respectively. For reducing the false-positive rate by almost one-third, sensitivity values for the VI (cutoff, 1.556%), FI (25.212), and VFI (0.323) were 92%, 95%, and 93%, respectively. CONCLUSIONS. Three-dimensional PD vascular indices could be helpful for reducing the false-positive rate in cystic-solid and solid vascularized adnexal masses. ▼ Enhanced blood perfusion in a tissue mass is an indication of neo-vascularity and potential malignancy. Ultrasonic pulsed Doppler imaging is a safe and economical modality for noninvasive monitoring of blood flow. However, weak blood echoes make it difficult to detect perfusion using standard methods without the expense of contrast enhancement. Additionally, imaging requires high sensitivity to slow, disorganized blood-flow patterns while simultaneously rejecting clutter and noise. An approach to address these challenges involves arranging acquisition data in a multi-dimensional structure to facilitate the characterization and separation of independent scattering sources. The resulting data array involves a linear combination of spatial, slow-time (kHz-order sampling), and frame-time (Hz-order sampling) coordinates. Applying an eigenfilter that exploits higher-order singular value decomposition (HOSVD) can technically transform the array and reduce the dimensions to yield power estimates for blood flow and perfusion that are well isolated from tissue clutter. Studies using microcirculation-mimicking simulations and phantoms enable the optimization of the filtering algorithm to maximize estimation efficiency. These techniques are applied to murine models of ischemia and melanoma at 24 MHz to form perfusion images. The results show enhancements of tissue perfusion maps, which help researchers access lesions without contrast enhancement. In a study aimed at peripheral artery disease (PAD), the enhanced sensitivity and specificity of ultrasonic-pulsed-Doppler imaging enable differentiation of perfusion between healthy and ischemic states. In addition, the use of the new ultrasound imaging coupled with other imaging modalities helps to illuminate the complex mechanism that mediates neovascularization in response to vascular occlusion. Consequently, these techniques have the potential to increase the effectiveness of existing medical imaging technologies in safe, cost-effective ways that promote sustainable medicine. Bu nedenle, hidrostatik basıncın (23 mmHg) mikrovasküler fonksiyonlar üzerindeki olası etkilerini değerlendirmek için kişiler yüzükoyun ve sırtüstü yatma pozisyonlarında iken kol ve bacağı düşey olarak sarkıtarak her iki ekstremitede de meydana gelen değişiklikleri inceledik. Bu çalışmaya 25 sağlıklı genç denek gönüllü olarak katıldı. Deri kan akımı, lokal hidrostatik basınç uygulanmasından önce ve sonra önkolun volar ve bacağın gaiter bölgelerinden laser Doppler akış ölçer (LDF) kullanılarak ölçüldü. Deri vasküler yatakların kontrol mekanizmalarını incelemek için denekler sırtüstü yatma pozisyonunda iken bazal durum ve hidrostatik faktör etkisiyle artan basınç altında lokal ısısal uyaran (42 °C) uygulandı. Hızlı Fourier dönüşümü (FFT) LDF sinyallerinin güç spektral yoğunluklarını (PSD) hesaplamak için kullanıldı. PSD eğrilerinin integrali 1’e normalize edildi ve kardiyak ( I: 0.6 – 2,0 Hz), solunum ( II: 0,6 – 0,145 Hz), miyojenik ( III: 0,145 – 0,052 Hz), nörojenik ( IV: 0,021 – 0,052 Hz), NO’ya bağımlı endoteliyal aktivite ( V: 0,0095 – 0,021 Hz) ve NO’dan bağımsız endoteliyal aktiviteye ( VI: 0,0005 – 0,0095 Hz) ait altı frekans bölgesinin integrali, her bir kontrol sistemin bağıl katkısını belirlemek için kullanıldı. Volar bölge gaiter bölge ile karşılaştırıldığında miyojenik ve endoteliyal osilasyonların gücü, hidrostatik basınç uygulanması sırasında vazodilatör uyaranla değişmemiştir. Sonuçlar açıkça, hidrostatik basınç nedeniyle volar bölgedeki kan akımının miyojenik ve endoteliyal kontrol mekanizmalarında önemli fonksiyon kaybının olduğunu göstermektedir. Yüksek kan basıncı nedeniyle ortaya çıkan mikrovasküler patolojileri belirlemek için en uygun kayıt bölgesinin volar bölge olduğu önerilmektedir. Les pathologies vasculaires placentaires (PVP), prééclampsie (PE) et retard de croissance intra-utérin (RCIU), sont une des causes majeures de morbidité et mortalité maternelles et périnatales dans les pays développés. Un défaut de vascularisation utéro-placentaire est présent dans ces deux pathologies. Le Doppler énergie 3D (3DPD) est un outil facile à utiliser en clinique, les indices quantitatifs calculés à partir de volume utéro-placentaire semblent prometteurs pour dépistage de la PE. Le but principal de cette thèse est d’évaluer cette technique sur le plan de sa valeur discriminative entre situation physiologique et pathologique, ainsi que sur le plan de ses limites techniques en clinique et sur modèles animaux. La performance des indices 3DPD au 1er trimestre pour dépister la PE et/ou RCIU a été étudiée dans une étude prospective multicentrique. Une évaluation de la corrélation entre les indices 3DPD et les indices morphométrique a été ensuite réalisée chez les femmes enceintes à la fin de grossesse. Une corrélation entre le phénotype des villosités placentaire et la qualité de la perfusion placentaire a été mise en évidence. À la fin, l’intérêt de la technique a été démontré dans un modèle de RCIU chez brebis gestante.

Placental vascular pathologies (PVP), preeclampsia (PE) and intra uterine growth restriction (IUGR), are a major cause of maternal and perinatal morbidity and mortality in developed countries. A default of utero-placental vascularization is present in these two pathologies. 3D Power Doppler (3DPD) is an easy tool to use in clinic, its quantitative indices calculated from obtained utero-placental volume seem promising for PE prediction. The major objective of this thesis was to evaluate the discriminative value of this technique between physiological and pathological situations, as well as its limitations in clinical and animal models. The screening performance of PE and/or IUGR by 3DPD indices in the first trimester was studied in a prospective multicenter study. An evaluation of the correlation between 3DPD indices and morphometric indices was then carried out among pregnant women at term. At the end, the interest of the technique was demonstrated in an ewe model of IUGR (treated with testosterone).

Comparar la rentabilidad diagnóstica de la biopsia prostática aleatoria con la biopsia ecodirigida mediante power doppler y contraste sonográfico, en el diagnóstico precoz por imagen del cáncer de próstata localizado. Material y métodos: Se diseñó un estudio epidemiológico prospectivo y transversal de ámbito provincial, para estudiar el rendimiento diagnóstico de la ecografía transrectal en escala de grises (EG), Power Doppler (PD) y del contraste sonográfico (CS) en el diagnóstico precoz por imagen del cáncer de próstata localizado y comparar la rentabilidad diagnóstica de la biopsia prostática aleatoria (BA) frente a la biopsia ecodirigida mediante power doppler y contraste sonográfico en el diagnóstico precoz del cáncer de próstata localizado, sobre las personas adscritas al área de salud del Hospital Universitari i Politécnic La Fe de Valencia. El estudio se llevó a cabo por el Servicio de Urología del Hospital Universitari i Politécnic La Fe de Valencia. La adquisición de los datos se inició en Enero de 2002 y finalizó en Enero de 2010. El procesamiento de los datos se ha realizado diferenciando dos fases, una fase analítica y otra descriptiva. En la fase analítica, se comparó el rendimiento diagnóstico del tacto rectal, escala de grises, power doppler y contraste sonográfico, calculando los tests diagnósticos de Sensibilidad (S), Especificidad (E), Exactitud (Ex), Coeficientes de Probabilidad, Odds Ratio Diagnóstica (ORD) y Valores Predictivos (VP) para estas variables, en la población general y en función de su estadio clínico (cT1 y cT2), con el objetivo de valorar el rendimiento diagnóstico de la imagen ecográfica transrectal mediante cada uno de los elementos de trabajo (EG, PD y CS). En la división por estadios clínicos de la población, se excluyeron los casos con estadio clínico cT2b (“nódulo palpable bilateral”), por varios motivos: escaso número de pacientes englobados en este estadio (n:7), a que la presencia de un nódulo palpable bilateral probablemente supondría un mayor rendimiento diagnóstico en las variables de estudio por imagen y a que a estos pacientes, no se les administró contraste. Por todo ello, la división por estadios de la población quedó reducida al estadio cT1 (tacto rectal: “adenoma”) y al estadio cT2 (tacto rectal: “nódulo palpable”). Vários estudos usaram a ecografia como técnica de imagem capaz de diferenciar entre SpA e outras patologias reumáticas, de acordo com o padrão de envolvimento articular. No entanto, sinais ecográficos isolados de sinovite e erosões nas SpA, não diferem daqueles que são observados noutros distúrbios inflamatórios reumatológicos. A principal característica ecográfica específica e presente em todos os subtipos de SpA, parece ser a presença de entesites, caracteriza-se pelos achados ecograficos em escala de cinzentos: ecogenicidade, espessura, depósitos calcificados, entesofitos, rutura, erosões, irregularidades corticais e bursite. Com esta revisão bibliográfica pretendemos reunir e rever a informação existente sobre a utilidade da ecografia no diagnóstico da Espondilite anquilosante e Artrite psoriática. As palavras-chave “spondylitis, ankylosing” , “arthritis, psoriatic”, “musculoskeletal ultrasonography”, “power doppler sonography” e “classification criteria” foram utilizadas para a pesquisa de artigos científicos nas bases de dados PubMed e uptodate. Foi também realizada a leitura livros de texto onde este tema era abordado.

Early diagnosis of Spondylarthritis (SpA) is still a challenge, due to the low specificity and variations of clinical manifestations, in these pathologies. For this reason, imaging techniques have a very important role in the clinical evaluation and diagnosis of these diseases. High range ultrasound equipments with good diagnostic accuracy provide rheumatologists the necessary information for early identification of inflammation and structural damage, allow the management and daily monitoring of the disease and can monitor the response to therapies. The applicability of ultrasound to several joints and the possibility of dynamic evaluation of articular and peri-articular structures, contributed to consider this thecnique as a natural extension of the physical examination. Several studies have used ultrasound as an imaging technique that can differentiate between SpA and other rheumatic conditions, according to the pattern of joint involvement. ▼ The first aim of this thesis was to evaluate the added value of ultrasound in clinical decision making in patients with arthralgia, patients with psoriasis and monitoring RA patients. Our second aim was to increase sensitivity of power Doppler ultrasound for MCP joints. Early diagnosis of rheumatoid arthritis and thereby facilitating early initiation of effective disease-modifying drugs can slow down disease progression and diminish joint damage. With the introduction of the 2010 ACR/EULAR classification criteria for RA we are able to classify patients as having RA at an earlier stage. We described which cut point of the 2010 criteria would enable us to earlier identify RA patients among recent onset inflammatory arthritis patients. Part one – Ultrasound in clinical practice Since physical examination reached its maximum to identify synovitis, the first chapters of this thesis focus on the added value of ultrasound in daily clinical practice. Firstly, we described the association of ultrasound inflammation and the development of inflammatory arthritis in an early arthralgia cohort. Secondly, the frequency of ultrasound enthesitis in primary care psoriasis patients with musculoskeletal complaints was explored. The course of ultrasound inflammation and clinical findings in the feet in newly diagnosed RA patients was investigated next. In addition, the association of the presence of ultrasound synovitis and health status in RA patients who are in clinical remission was studied. And last we evaluated if ultrasound synovitis is a biomarker for clinical flare in RA patients who are tapering their medication. Part two – Experimental technical research The performance of the power Doppler modality of several ultrasound machines was compared by a flow phantom. The same flow phantom was used to compare conventional ultrasound with high-frame rate Doppler ultrasound. And we also give the results of high-frame rate Doppler ultrasound imaging in RA patients to evaluate whether it was possible to detect higher levels of vascularisation than with conventional ultrasound. ▼ Objective: To determine the role of 3-Dimensional Ultrasound (3-D) and Power Doppler (3-DPD) in the diagnosis of ovarian tumors and to examine the diagnostic accuracy of this new ultrasonographic technique in the detection of ovarian cancer. Methods: A preoperative analysis by 3-D and 3-DPD was performed between November 2003 and November 2006 of 318 women referred for the evaluation of a unilateral ovarian mass within the first Department of Obstetrics and Gynecology, University of Athens, in Alexandra Hospital. The age of the women studied was 47,418,8 years with a range (18-72 years). Three dimensional ultrasonographic method is a new diagnostic technique that provides precise and additional information of the region of interest by the rotation of the mechanical transducer through 360ο. Multiple sections of the ovarian tumor, rotation in the three axis (x, y, z), translation and reconstruction of (3-D) plastic images allow more meticulous and detailed evaluation, especially when modes used. Comprehensive (3-D) display allows interactive qualitative and quantitative analysis of the tumor microcirculation in three dimensions. In the present study, were analyzed sonomorphological criteria (wall structure, shadowing, septa <3mm, solid parts, echogenicity, the existence of peritoneal fluid, the relationship with surrounding structure and 3 criteria by (3-DPD) [Vessel’s architecture, branching pattern and tumoral blood flow (R.I.)], based on Kurjak’s scoring system. When it was necessary some patients were evaluated more detailed with imaging techniques (CT scan, M.R.I.), tumor markers, and at last were noted the family and personal history and analyzed demographic considerations. All of the women were operated and comparison of the ultrasound findings with the histopathological diagnosis was conducted. The malignant ovarian tumors were classified according to the International Federation of Gynecologists and Obstetricians (F.I.G.O). Statistical analysis: The diagnostic accuracy of 3-D ultrasound was estimated by R.O.C. curve and for statistical analysis of the data, was used the x²-test. Results: In the present study was included the largest number of patients (318 women) according to international literature. The women that were in reproductive age were examined independently of the phase of their menstrual cycle (proliferative or secretory) thus providing a more objective result. The histopathological diagnoses were 225 benign ovarian tumors and 93 malignant. The combined use of 3-D and 3-DPD was showed 215 benign and 103 malignant ovarian tumors. A cut off value of 6 was used for the discrimination of benign and malignant tumors based on a specific scoring system concerning 3-D ultrasonographic criteria. The sonographic appearance of a malignant ovarian tumor as characterized by 3-D and 3-DPD was: papillary protrusions, septa abnormal surface, abnormal echogenicity and vascularity in the centre or at the periphery with abnormal branching pattern, scattered vessels and vascular shunts. ▼ Operator-dependent instrument settings and the likelihood of image artifacts are two challenges for reliably using three-dimensional (3-D) power Doppler angiography in flow depiction and quantification applications. To address the operator-dependent settings challenge, an automated method for wall filter cut-off selection, the wall filter selection curve (WFSC) method, was developed using flow-phantom images. The flow-phantom WFSCs guided the development of a theoretical signal model relating color pixel density (CPD) and wall filter cut-off frequency. Simulations using the theoretical model were used to define criteria for the WFSC method to be applied to unprocessed power Doppler signals from 3-D vasculature. The adapted WFSC method was combined with a 3-D skeletonization and vessel network reconstruction method to present a two-stage processing method aimed at improving vascular detection, visualization and quantification. The two-stage method was evaluated using two in vivo models; a murine tumor model was used to test the performance of the method in a flow quantification application and a chick embryo chorioallantoic membrane (CAM) model was used to evaluate the method’s value for flow depiction applications. Applying the WFSC method to flow-phantom images improved vessel delineation and vascular quantification to within 3% of the vascular volume fraction of the phantom. Criteria for the WFSC method from the simulations were to assess at least 100 cut-off frequencies and that the CPD variability should be less than 5% to ensure quantification accuracy. Large variations in the cut-off frequency selected using the WFSC among images acquired at different time points and across different animals in the murine tumor model signified the relevance of spatially and temporally adjusting the cut-off frequency. The two-stage method improved visualization of the vascular network and significantly reduced artifacts in both the tumor and CAM models in comparison to images using conventional Doppler processing. In the CAM model, vessel diameters measured in two-stage processed images were more accurate than measurements in images exported from a commercial scanner. The proposed signal processing methods increase accuracy and robustness of qualitative and quantitative studies using 3-D power Doppler angiography to assess vascular networks for flow depiction and quantification.

Les dispositifs d’isolation galvanique intégrés au sein des systèmes de commande d’interrupteurs de puissance doivent répondre à une demande accrue en performance, facilité d’intégration et efficacité énergétique. Les transducteurs ultrasonores capacitifs micro-usinés (cMUT : capacitive Micromachined Ultrasonic Transducer), capables d’émettre et de recevoir des ondes ultrasonores, semblent une alternative tout à fait nouvelle à la fonction d’isolation galvanique. Ces travaux de thèse ont pour objectif de démontrer la faisabilité d’un dispositif basé sur la technologie cMUT. Le principe de fonctionnement consiste à transmettre une information grâce à une communication par onde acoustique de volume entre deux réseaux de cMUT placés de part et d’autre d’un substrat. Nous focalisons, en premier lieu, ces travaux sur le processus de fabrication par micro-usinage de surface des cMUT ainsi que les techniques de réalisation des dispositifs en structure double face sur substrat de silicium. L’étude permet d’identifier le collage de substrat comme une solution de fabrication industrialisable. Suite à la réalisation des dispositifs, la caractérisation électro-mécanique des cMUT est une étapeessentielle à la validation de leur fonctionnalité en tant que dispositifs émetteurs. L’étude débute par uneévaluation des propriétés mécaniques du matériau constituant la membrane et qui impactent directementle comportement global des cMUT. Puis, la caractérisation du comportement statique et dynamique descMUT permet d’extraire les paramètres tels que la fréquence de résonance, la tension de collapse etl’efficacité électro-mécanique qui définissent le mode de pilotage d’un tel système.Finalement, la validation du concept de transmission et de détection d’ondes ultrasonores est réaliséegrâce à des mesures de vibrométrie laser Doppler. Les résultats apportent des éléments de réponse quantau mode de propagation des ondes et permettent d’identifier les topologies de meilleure efficacité entransmission acoustique. Enfin, l’intégration du prototype dans l’application de commanded’interrupteur de puissance démontre la faisabilité du concept de transformateur acoustique basé sur latechnologie cMUT.

Galvanic isolation devices integrated into switch command systems must be able to answer all of the increasing demand for performance, energetic efficiency and integration easiness. The capacitive micro machined ultrasonic transducers (cMUT), able to emit and receive ultrasounds, could be an entirely new alternative to the function of galvanic isolation. This work aims to demonstrate the feasibility of a cMUT-based device. The operating principle consists in transmitting information thanks to a bulk acoustic wave between two cMUT arrays located on both sides of a substrate. ▼ Acoustic communication is an enabling technology for many autonomous undersea systems, such as those used for ocean monitoring, offshore oil and gas industry, aquaculture, or port security. There are three main challenges in achieving reliable high-rate underwater communication: the bandwidth of acoustic channels is extremely limited, the propagation delays are long, and the Doppler distortions are more pronounced than those found in wireless radio channels. In this dissertation we focus on assessing the fundamental limitations of acoustic communication, and designing efficient signal processing methods that cam overcome these limitations.; We address the fundamental question of acoustic channel capacity (achievable rate) for single-input-multi-output (SIMO) acoustic channels using a per-path Rician fading model, and focusing on two scenarios: narrowband channels where the channel statistics can be approximated as frequency- independent, and wideband channels where the nominal path loss is frequency-dependent. In each scenario, we compare several candidate power allocation techniques, and show that assigning uniform power across all frequencies for the first scenario, and assigning uniform power across a selected frequency-band for the second scenario, are the best practical choices in most cases, because the long propagation delay renders the feedback information outdated for power allocation based on the estimated channel response. We quantify our results using the channel information extracted form the 2010 Mobile Acoustic Communications Experiment (MACE'10).; Next, we focus on achieving reliable high-rate communication over underwater acoustic channels. Specifically, we investigate orthogonal frequency division multiplexing (OFDM) as the state-of-the-art technique for dealing with frequency-selective multipath channels, and propose a class of methods that compensate for the time-variation of the underwater acoustic channel. These methods are based on multiple-FFT demodulation, and are implemented as partial (P), shaped (S), fractional (F), and Taylor series expansion (T) FFT demodulation. They replace the conventional FFT demodulation with a few FFTs and a combiner. The input to each FFT is a specific transformation of the input signal (P,S,F,T), while the combiner performs weighted summation of the FFT outputs. We design an adaptive algorithm of stochastic gradient type to learn the combiner weights for coherent and differentially coherent detection. The algorithm is cast into the framework of multiple receiving elements to take advantage of spatial diversity. Synthetic data, as well as experimental data from the MACE'10 experiment are used to demonstrate the performance of the proposed methods, showing significant improvement over conventional detection techniques with or without inter-carrier interference equalization (5 dB - 7 dB on average over multiple hours), as well as improved bandwidth efficiency.

The purpose of this study is to examine women with abnormal vaginal bleeding and thick endometrium (>5 mm, histologically proven to be endometrial hyperplasia or cancer with endometrial biopsy) with contrast-enhanced transvaginal colour Doppler ultrasound, in order to demonstrate efficiently the vascularity of these lesions, quantify it and differentiate benign from malignant lesions. Over a 40 months period 53 patients were examined with contrast-enhanced transvaginal colour Doppler ultrasound. Examination images were captured, saved via a computer system and processed with image analyzing software in order to define the maximum percentage of colored pixels over the total area of the lesion before and after the administration of contrast agent and to calculate the difference between them. All patients underwent hysterectomy. Six patients were excluded from the study as one of them suffered from diabetes mellitus (contraindication for contrast agent injection), one showed no endometrial abnormality at the time of the examination and in four patients we could not demonstrate effectively the borders of the lesion in any plane and make any calculations. 10 of our patients suffered from endometrial hyperplasia, 12 from grade I cancer, 41 from grade II cancer and 11 from grade III cancer. Statistical analysis of our data showed that before the contrast agent injection vascularity of hyperplasia cases indeed showed statistical difference from the vascularity of cancer cases. However, there was a significant overlap of values. Vascularity of cancer cases did not demonstrate any statistical difference. At contrast-enhanced transvaginal ultrasound statistical difference between cases of hyperplasia and carcinoma was greater and there was no overlap of values. Additionally, vascularity of grade I carcinoma showed statistical difference with both grade II and grade III carcinomas. In conclusion, contrast-enhanced transvaginal power Doppler ultrasound is more efficient in demonstrating vascularity of benign and malignant endometrial lesions than plain power Doppler imaging and significantly helps in the differential diagnosis amongst these lesions.

Σκοπός της έρευνας μας είναι η μελέτη ασθενών με μηνορραγία ή μητρορραγία και πάχυνση του ενδομητρίου οφειλόμενη σε ιστολογικά επιβεβαιωμένη με ενδομήτριο βιοψία αδενωματώδη υπερπλασία ή κακοήθεια με διακολπικό υπερυχογράφημα μετά την ενδοφλέβια χορήγηση σκιαγραφικού μέσου. Η υπόθεση της μελέτης είναι ότι η μέθοδος αυτή θα αναδείξει αποτελεσματικότερα την παθολογικά αυξημένη αγγείωση των κακοηθειών σε σχέση με την αδενωματώδη υπερπλασία συμβάλλοντας έτσι στην διαφορική τους διάγνωση. ▼ The research and development for robust, accurate and power-efficient sensors for the continuous monitoring of physiological vital signs of patients is of great importance. For example, accurate and continuous heart rate monitoring will not only provide key medical insights regarding the individual’s heart conditions, but it may also be critically important in providing life-saving treatments and therapies. This thesis, therefore, presents a study on several robust and low-power techniques for continuous heartbeat sensing using: (1) a wearable electrocardiogram (ECG) sensor; (2) an implantable cardioverter defibrillator (ICD); and (3) a Doppler-based non-contact vital signs (NCVS) bio-sensor. To realize effective wireless-assisted-living solutions and provide reliable ambulatory healthcare, low power and miniature cardiac monitoring sensor systems are essential. In this context, low-power and low-noise instrumentation amplifiers (INAs) were studied and designed to be a part of the bio-sensor analog front end (AFE) integrated circuit (IC). Circuit techniques and topologies for low-power and low-noise design were used to realize CMOS INAs, which would enable robust and battery friendly ECG bio-sensors. This thesis discusses the circuit design techniques, and presents the simulation and measurement results of the low-power CMOS INAs with and without the chopper techniques to reduce the 1/f noise. Doppler-based non-contact vital signs (NCVS) bio-sensor was also investigated for reliable and robust detection of heart rate. On the system level, the antenna evaluation of the non-contact vital sensor has been explored to understand the relationship between the antenna characteristics and heart-rate detection accuracy. The original PCB-based NCVS sensor system used in the study was designed by former students of our group, but this thesis reports the various efforts to make the system more reliable and robust.
