▼ ABSTRACT Background: Acute poisoning remains a common method employed for suicide worldwide. World Health Organization (WHO) estimates that about 0.3 million people die every year worldwide due to various poisoning agents. Methodology: The study was a cross-sectional descriptive study. A retrospective survey of acute poisoning cases presenting at U.T.H medical emergency department was undertaken. This was conducted by identifying and reviewing all acute poisoning cases that presented at this department during the period 1st January, 2015 to 31th December, 2015 (a period of 12 months). A total of 131 patients’ medical records and charts on poisoning were reviewed. The data was thereafter analyzed by using statistical package of social science (SPSS). All statistical tests were at 5% significance level. Results: Of the 131 acute poisoning cases reviewed, 51.1% were female. Most patients (53.4%) were single and 47.3% were unemployed. Acute poisoning cases occurred more in the age-group of 20-39 years (49.6%). Most of poisoning cases (58.8%) occurred in areas associated with poor socio-economic status. Organophosphate poisoning accounted for (38.2%) of all the poisoning cases. Other common agents/drugs used were paracetamol tablets (7.6%), antibiotics (6.1%), antiretroviral (ARVs) (3.1%) and household chemicals (11.5%). Thirty four patients (26%) took more than one drug/ agent. Out of 109 patients who had the reason for their poisoning stated, 57 patients had relationship disharmony (32 married, 25 unmarried). Fourteen patients had family issues which mainly resulted from conflicts with parents and grandparents. The psychosocial problems of the 16 patients were not specified and accidental poisoning was seen in 5 patients. Majority of the patients (85.5%) were discharged without any complications and the mortality rate was observed at 4.6%. Low GCS (< 8) and hypothermia were common clinical features amongst patients that got admitted. The association between mortality and poisoning associated with alcohol was found to be statistically significant (p=0.04). Conclusion: Acute poisoning is still a common phenomenon, majorly amongst females in the age group of 20 – 39 years, with poor quality of life. Relationship disharmony is the most common reason for poisoning. Organophosphate poisoning accounted for most of the poisoning cases. Drug/agent used can only be curtailed to a minimal extent. Prevention through early detection of vulnerable patients and early psychological management should be our goal.

▼ Introduction:Chemical poisoning has been identified as a significant global public health problem resulting in over 350, 000 deaths due to unintentional poisoning. Each year nearly a million people die as a result of suicide, and chemicals account for a significant number of these deaths. Although data on the occurrence of poisoning on a global scale may seem to be well established, updated data from developing countries like Zambia is largely unexplored. Methods:Data on poisoning was retrospectively collected from two referral hospitals in Lusaka from 1st January to 31st December, 2012. A pretested data collection form was used to collect demographic and epidemiologic data surrounding cases including toxic agents used, route of exposure to toxic agent, circumstance (deliberate/accidental) and outcome (recovery/death) of poisoning. All analysis was done using STATA version 13. Results:A total of 1, 031 poisoning cases were reviewed resulting in a prevalence of 1.1 per 100 admissions. Of the cases reviewed men (n=592, 57%) dominated females (n=439, 43%). Deliberate poisoning was common among those aged between 20-30 years (n=231, 52%) while accidental poisoning (n=292, 64%) characterised most of the cases aged between 0-12 years (P-value <0.001). There were a total of 27 deaths representing a 2.6 per 100 case mortality rate. Pesticides (n=187, 18%), alcohol (n=158, 15%) and pharmaceuticals (n=123, 12%) were associated with a larger proportion of poisoning cases. The majority (n=682, 66%) of cases reported were from peri urban residences of Lusaka District. Conclusion:The prevalence of chemical poisoning established in this study was lower than that found in the African region while the mortality was higher than that observed elsewhere. Common toxic agents involved in poisoning were pesticides, alcohol and pharmaceuticals, these also had higher case fatality rates. The significant predictors of deliberate/accidental poisoning were age, route of exposure and toxic agent used while those of recovery/death were sex, age and toxic agent used (more so for pesticides).

▼ Lead poisoning contributes 0.9 per cent of developing countries’ disease burden. Lead poisoning is due to excessive lead exposure to the human body. In Zambia, Kabwe is one of the most affected towns due to its previous mining activities. Studies done by ZCCM-IH and others show that currently, 35 per cent of Kabwe’s population or 13,952 households in Katondo, Railways, Chowa, Kasanda and Makululu are exposed to high levels of lead with a median soil concentration of above 500mg/kg, 48 per cent of child bearing age women were exposed to lead poisoning. This prompted ZCCM-IH to conduct sensitization. However, health institutional data has continued to show a high number of children being diagnosed with lead poisoning. Therefore the study attempted to answer the research question whether women of child-bearing age in Kabwe’s high lead residential exposure areas know the precautionary measures against Lead poisoning in their homes. Women who were able to answer/list at least eight types of lead poisoning preventive measures were considered knowledgeable. The purpose of the study was to assess the extent of knowledge of precautionary measures among women of child-bearing age who stay in high lead poisoning exposure residential areas in Kabwe because of their triple roles they play in the prevention of lead poisoning in homes. The study surveyed 430 households using structured interview schedule in high lead endemic residential areas of Makululu, Chowa and Kasanda. Makululu and Kasanda had two Focus Group Discussions each. The sample included women aged 13-49 years old. Quantitative data was analyzed using SPSS computer software package. The Chi-square (χ²) test was used to test for significant associations between independent and dependent variables (p-value < 0.05). The results revealed that knowledge level of precautionary measures among women of child-bearing age was low at 40.9% (p-value = 0.0055) despite staying in high lead poisoning exposure residential areas and having heard of lead poisoning. This may explain why the number of children being diagnosed with lead poisoning is still high despite the sensitization and people are still constructing houses even in highly contaminated areas. It is recommended that the government through Zambia Environmental Management Agency and other stakeholders should come up with new policies to combat lead poisoning exposure and also ways of creating awareness so that lead poisoning can be prevented in endemic areas. Ministry of Mines and Energy should come up with policies to discourage people from scavenging old mine site for scrap metal and mineral ores. Kabwe Municipal Council should stop giving out plots in high lead exposure areas. Ministry of Community and Development Mother and Child Health at district level should support community health activities in Lead poisoning prevention.

Sindoor, an orange, maroon or red powder used for Hindu cultural and religious purposes, has been found to be adulterated with lead tetroxide, (Pb3O4, red lead). Red lead is often added to give the sindoor powder a deep red color. The adverse health effects caused by lead exposure have been widely documented and in children can include abdominal pain, learning disabilities reduced attention span, slowed growth and antisocial and delinquent behavior. Currently, there is no known safe level of lead exposure in children. We believe this is the first study to comprehensively assess lead (Pb) concentrations in sindoor and related powders from not only a wide geographical area in an entire state but also in those sindoor powders purchased directly from India. A total of 170 religious powders were purchased in different regions of the state (Northern, Central and Southern Jersey). Out of those 170 powders, 118 were sindoor and 52 were miscellaneous religious powders. Of the 118 powders, 95 were purchased from 66 South Asian stores in New Jersey while 23 were purchased in India. The miscellaneous powders were purchased in New Jersey. Laboratory analysis revealed how, among, U.S. sindoor powders, 79 (83.2%) of tested powders contained >1 ug/g Pb. The median lead concentration for tested U.S. sindoor powders was 3.0 ug/g Pb with a maximum Pb level of >300,000 ug/g Pb. For sindoor powders purchased in India, 18 (78.3%) samples contained >1 ug/g Pb. The median Pb concentrations for these sindoor powders was 12.7 ug/g Pb with a maximum Pb level of >300,000 ïug/g Pb. Two samples, purchased from Mumbai, contained >1% Pb. A third sample, also purchased from Mumbai, contained 0.5% ug/g Pb. For miscellaneous religious powders, 29 (55.8%) powders contained >1 ug/g Pb. The median Pb concentration for these powders was 1.1 ug/g Pb with a maximum Pb level of 61.0 ug/g Pb. Semi-quantitative x-ray fluorescence (XRF) and colormetric tests successfully identified lead in samples with higher Pb levels (i.e., >300,000 ug/g), but were not effective for samples with lower Pb levels. In summary, these results underscore the need for hazard surveillance of sindoor.

▼ Rodenticide use in agriculture can lead to the secondary poisoning of avian predators. Currently the Australian sugarcane industry has two rodenticides, Racumin® and Rattoff®, available for in-crop use but, like many agricultural industries, it lacks an ecologically-based method of determining the potential secondary poisoning risk the use of these rodenticides poses to avian predators. The material presented in this thesis addresses this by: a. determining where predator/prey interactions take place in sugar producing districts; b. quantifying the amount of rodenticide available to avian predators and the probability of encounter; and c. developing a stochastic model that allows secondary poisoning risk under various rodenticide application scenarios to be investigated. Results demonstrate that predator/prey interactions are highly constrained by environmental structure. Rodents used crops that provided high levels of canopy cover and therefore predator protection and poorly utilised open canopy areas. In contrast, raptors over-utilised areas with low canopy cover and low rodent densities, but which provided high accessibility to prey. Given this pattern of habitat use, and that industry baiting protocols preclude rodenticide application in open canopy crops, these results indicate that secondary poisoning can only occur if poisoned rodents leave closed canopy crops and become available for predation in open canopy areas. Results further demonstrate that after in-crop rodenticide application, only a small proportion of rodents available in open areas are poisoned and that these rodents carry low levels of toxicant. Coupled with the low level of rodenticide use in the sugar industry, the high toxic threshold raptors have to these toxicants and the low probability of encountering poisoned rodents, results indicate that the risk of secondary poisoning events occurring is minimal. A stochastic model was developed to investigate the effect of manipulating factors that might influence secondary poisoning hazard in a sugarcane agro-ecosystem. These simulations further suggest that in all but extreme scenarios, the risk of secondary poisoning is also minimal. Collectively, these studies demonstrate that secondary poisoning of avian predators associated with the use of the currently available rodenticides in Australian sugar producing districts is minimal. Further, the ecologically-based method of assessing secondary poisoning risk developed in this thesis has broader applications in other agricultural systems where rodenticide use may pose risks to avian predators.

▼ The entire human population is chronically exposed to various contaminants, including lead and chlorinated hydrocarbon pesticides. Studies were undertaken to assess the effects of chronic exposure to lead on the toxicities of two such pesticides, lindane and dieldrin. Mice were given lead as the acetate in drinking water, at levels of 75, 150 or 300 ppm lead for eight weeks. The acute LD50 of lindane (98 mg/kg) given in corn oil by oral intubation was not altered significantly (P > .05) by administration of 150 ppm lead. Similarly, the acute LD50 of dieldrin (45 mg/kg), given in corn oil by oral intubation, was not changed significantly (P > .05) by the same lead exposure. In subacute studies, lindane was given daily by oral intubatiop in corn oil at a level of 40 mg/kg for the final two weeks of lead exposure. The excretion of delta-aminolevulinic acid in the urine was increased in a dose-related fashion by lead but was unaffected by lindane. There was no evidence of significant interaction between the two agents. Urinary coproporphyrin excretion was increased slightly by both agents, and when lindane was given to mice receiving 300 ppm lead, levels rose from a control urinary concentration of 0.21 microgram per ml to 0.37 microgram per ml (P < .05). Urinary excretion of uroporphyrin was not altered by either agent. Thus, lead and lindane apparently interacted in an additive fashion at at least one step in the heme synthesis pathway, and failed to interact at at least two steps. Lead depressed blood hemoglobin concentration (from a control value of 13.5 grams per 100 ml to 12.8 grams per 100 ml), an effect predictable from the blockade of heme biosynthesis by the metal. Lindane did not affect hemoglobin concentration when given by itself but prevented the fall in concentration elicited by lead. Hematocrit was decreased in a dose-related fashion (from 43.7% to 41.1%) by lead, and lindane appeared to accentuate the decrease (40.0% when both were given). The agents were thus seen to interact at two or more sites, one of which was probably the heme biosynthetic pathway. Levels of hepatic microsomal cytochrome P-450 were depressed in a dose-related fashion upon administration of lead. In mice receiving 300 ppm lead, levels were depressed to about 50% of the control level. This effect suggested the possibility that individuals exposed to lead may suffer impairment of oxidative metabolic pathways dependent upon cytochrome P-450. This could have the effect of increasing the toxicity of a compound taken into the body, or of increasing the duration of the action of the compound. Lindane increased levels of cytochrome P-450 an average of 83% over respective counterparts not receiving lindane. This effect was probably related to an induction of hepatic microsomal enzyme systems. Lindane, at the level given, overcame the effects of lead on cytochrome P-450 levels. ▼ Unintentional childhood poisoning is an important public health issue. Children aged 0-4 years are often analysed together as a group for unintentional poisoning risk for statistical purposes. However, these children are quite different in terms of their physical, cognitive and self-regulatory abilities. This thesis examines the contribution of child development to poisoning risk Three population-based epidemiology studies were conducted that used age as a proxy for development and controlled for other variables that might be contributing to a young child's poisoning risk. The findings of the three age pattern studies demonstrated strong support for the contribution of child development to poisoning events. Children aged 1-3 years had the highest level of risk in all three studies and the age patterns remained stable when analysed over time and by sex, socio-economic status and remoteness of residence. These findings warranted further investigation of child development as a risk factor for unintentional poisoning in children aged 1-3 years.A case-control study was designed to investigate child development and other potential risk factors for unintentional poisoning. It employed three control groups, injured, sick and healthy, to indentify risk factors specific to unintentional poisoning. Both questionnaire and observational measures were used to assess the factors. The importance of age and the role of development in unintentional poisoning risk were highlighted by the findings of the risk factor study. Several variables were identified as contributors to poisoning events which also may reflect a younger poisoning group. The study indicated that maternal use of more positive control, accessible poison storage locations and less parenting stress all contributed to unintentional poisoning. Less close supervision was also identified as a risk factor, indicating that proximity of supervision may be important for poison prevention. The findings of these four studies demonstrated that development plays a major role in unintentional poisoning risk for children aged 1-3 years. Taken together, information regarding age patterns and risk factors for unintentional poisoning in young children may help target prevention measures and identify areas warranting further research. ▼ The content of cadmium and lead in raw food crops consisting of cabbage, rape, tomato and maize collected from selected retail markets, and in daily diets collected from selected school dining halls in Kabwe and Lusaka was determined. Separate analyte solutions were obtained after wet digestion of samples followed by extraction and precipitation for cadmium and lead respectively . Measurements were done using flame atomic absorption spectrophotometry technique. The range of concentrations in p.g/kg d.w. of raw food crops from Kabwe and Lusaka (and food crop with lowest and highest mean) were respectively for cadmium N.D.- 125 (Cabbage - rape) and N.D.- 264 (maize - tomato), and for lead N.D.- 1 974 (tomato - rape) and N.D.- 1 948 (maize - rape) with N.D. meaning "not detected" concentration. Calculations based on quantities assumed to be portions of total food consumed daily yielded W.I., in (ig/kg b.w. for a person weighing 60 kg, ranging from 0.01 to 0.196 for cadmium and from 0.11 to 0.351 for lead. These values represent 1.4 to 2.8% and 0.44 to 14% P.T.W.I.(Provisional Tolerable Weekly Intake) for cadmium and lead respectively. The mean concentration in jag/kg d.w. of food composites corresponding to the numbers of daily diets collected in a week from each selected institution was, for cadmium, between 6 and 12 with the exception of Saint Paul's Secondary School which showed N.D. as mean; and for lead, N.D. in all institutions except Saint Paul's Secondary School which had 462 as mean. The weekly dietary intake of food actually consumed, in }J.g/kg b.w. for a 60 kg man, was estimated to range between N.D.and 0.77 with most of the values on the higher limit for cadmium, and between N.D. and 29.6 with all the values on the lower limit except one for lead. These values represent N.D. to 11% and N.D. to 118.6% P.T.W.I, for cadmium and lead respectively. Sample recoveries, with values bracketed between 80% and 110%, demonstrated the accuracy of flame atomic absorption spectrophotometry technique. A comparison of the levels and W.I. of cadmium and lead reported hi this study for raw food crops and diets with legal limits were found to be within the normal range.

Reproduziu-se experimentalmente o envenenamento crotálico, através da inoculação, por via subcutânea, do veneno de Crotalus durissus terrificus (Cascavel sul-americana) em dez bovinos mestiços, com peso variando entre os 125 e 449 quilogramas e idade entre 12 e 36 meses. Dois animais na mesma faixa etária e padrão racial foram utilizados como controle. O animal que recebeu dose de 0,03 mg/kg p.v. foi a óbito 7h40min após a inoculação. A dose de 0,015 mg/kg p.v. induziu o óbito em quatro de sete bovinos inoculados, enquanto os dois animais que receberam 0,0075 mg/Kg, adoeceram discretamente e se recuperaram. Os sintomas tiveram início entre 1h30min e 13h45min após a inoculação. A evolução oscilou entre 5h25min e 44h59min para os animais que morreram e entre 33h15min e 17 dias entre os animais que se recuperaram. Os principais sinais nervosos observados foram diminuição da resposta aos estímulos externos, reflexos hipotônicos, arrastar dos cascos no solo, aparente letargia, paralisia do globo ocular e da língua, decúbito esternal e lateral. Verificou-se também adipsia constante e petéquias nas mucosas vaginal e conjuntival. Observaram-se discreto a moderado aumento do tempo de sangramento em seis animais e moderado aumento do tempo de tromboplastina parcial ativada em sete bovinos. Houve moderada leucocitose com neutrofilia, linfopenia relativa, eosinopenia, monocitose e discreto aumento do número de bastões. Foi evidenciado significativo aumento dos níveis séricos de creatinaquinase, da ordem de dez vezes. Não foram observadas alterações significativas através da urinálise. À necropsia constataram-se edema quase imperceptível no local da inoculação, discretas petéquias e sufusões no epicárdio, omento, vesícula biliar e mucosa da bexiga. Os exames histopatológicos revelaram necrose (hialinização) de grupos de miócitos ou em miócitos isolados em dez diferentes músculos esqueléticos examinados, próximos ou distantes do local de inoculação. Diante do quadro clínico-patológico foram feitas observações sobre o diagnóstico do envenenamento crotálico e sua diferenciação com enfermidades que cursam com paralisia e necrose muscular em bovinos do Brasil.

Crotalus poisoning was experimentally reproduced by inoculation of Crotalus durissus terrificus (South American Rattlesnake) venom by subcutaneous via in ten clinically healthy mixed breed bovine, weighing between 125 and 449 kg and aged 12 to 36 months. Two bovine of the same age range and breed standard were used as controls. The bovine that received a 0.03 mg/kg p.v. dose died 7h40min after inoculation. A 0.015 mg/kg p.v. dose provoked death in four out of seven inoculated bovine, while two bovine experimentally poisoned with 0.0075 mg/kg p.v. became mildly sick and recovered. Onset of symptoms occurred between 1h30min and 13h45min after inoculation. Evolution oscillated between 5h25min and 44h59min for bovine deaths and between 33h15min and 17 days for bovine that recovered. ▼ Lead (Pb) and cadmium (Cd) are naturally occurring heavy metals which have been redistributed to a large extent by man in the environment. These metals have certain common toxic sites of action on biological systems of mammals. Simultaneous exposure to Pb and Cd is highly probable due to common sources of exposure in the environment. The purpose of this study was to observe the effects of simultaneous, chronic exposure to Pb and Cd on pathology, tissue residues and susceptibility to viral challenge in mice. Mice were exposed, via the drinking water, to either Pb or Cd or both metals simultaneously at various doses for ten weeks and then challenged with encephalomyocarditis virus (EMCV). Mortality was enhanced in all groups of mice exposed to Pb and suppressed in groups of mice treated with Cd. Mice simultaneously coexposed to Pb and Cd had mortality rates intermediate to those observed after exposure to the individual metals. It is proposed that Pb suppresses immunity in mice by inhibition of basic metabolic functions deoendent on sulfhydrylcontaining enzymes. The effect of this action results in a decreased ability of cells to produce antibodies, interferon and anti-viral proteins. It is postulated that Cd enhances the immune response in mice by inhibition of zinc-dependent enzymes such as peptidases and RNA polymerase. This results in increased stability and persistance of antibodies, interferon and anti-viral proteins in addition to decreased ability of the virus to replicate. Coexposure to Pb and Cd did not appear to alter histopathologic lesions observed by light microscopy that were produced by exposure to Pb or Cd only. Pb and Cd residues in tissues of mice exposed to these metals, as compared to residues in tissues of mice which received Pb or Cd only, indicate that interaction occurs in regard to metabolism, storage and excretion of these elements. Coexposure to Pb caused increased Cd accumulation at low doses. The effectwasreversed at high doses and Cd tissue residues are reduced. Pb tissue residues, except in kidneys, were generally lower in mice coexposed to Cd. Kidney Pb levels increased in these mice. It is significant that chronic coexposure to Pb and Cd at subclinical doses alters certain effects produced by exposure to only one of the metals. The indiscriminate use of pesticides poses a serious health hazard for a large human population as well as for the flora and fauna of this planet. The present work deals with the development of sensitive and selective methods for the pesticide residue analysis in different food and environmental samples. Pesticides and their degradation products have widely proliferated the land and water, they are hazardous not only to farm workers but also to population as a whole through contamination of food, drinking water and other environmental features. Many of these pesticides are known carcinogens, endocrine disruptors, teratogens, mutants etc.These accumulate in important human organs and lead to progressively growing toxic effects.The pesticides are a necessary evil. The use of pesticides is required to control pests and to increase crop yield and hence ensuring food security for growing populations across the world. The need of the hour is to develop such efficient, rapid, powerful, sensitive and economical methods which could detect pesticides in various matrices to very low detection levels. Solid Phase Extraction is an effective and time tested pre-concentration technique. SPE alongwith HPLC has been used for the separation and quantitative determination of phenylurea herbicides. Further SPE-HPLC-UV has been employed to extract Azadirachtin pesticide from neem tree parts. newlineA new technique; MEPS (Microextraction by Packed Sorbent); which is miniaturized version of SPE has been used for the determination of Endocrine disruptor and suspected endocrine disruptor pesticides in various biological and environmental samples. This technique ensures less ssample prepation time and requires low sample volumes; as low as 10µL, hence is particularly useful for analysis in biological matrices. Derivative Spectrophotometry (DS) and spectrophotometrry have been effectively put to use for the quantitative analysis of dithiocarbamate (DTCs) fungicides and the methods have been successfully used for their analysis in different food.

▼ The purpose of this research was to analyze the available pesticide poisoning data in Oregon from 1994 to 1998 in order to obtain a descriptive account of general incident information and of occupational exposures. These data were to be used by the Pesticide Analytical and Response Center for planning preventive strategies to reduce pesticide poisonings. Specific data from the five-year period were retrieved from two different software systems, dBaseIV and SPIDER, a National Institute of Occupational Safety and Health Institute program. They were then reconciled to each other, and analyzed with SPSS Version 10 for frequency information. Percentages from frequencies, and in some cases, population-based rates, were then calculated. Results indicated that the population usually affected by pesticides each year was non-Hispanic, female, and 40 to 49 years old. More pesticide poisonings took place in non-occupational settings than occupational settings for the five-year period as a whole. However, when reviewing each year, there were more occupational cases than non-occupational cases in 1994, 1996, and 1998. Marion and Multnomah counties experienced a higher number of poisonings than other counties. However, when rates per 100,000 people were calculated, Wheeler and Wasco, which are both rural, were the top two ranking counties, with rates of 12.7 and 4.4 respectively. Organophosphate insecticides were involved in poisonings more than other pesticides, with chlorpyrifos as the active ingredient most frequently seen for the five-year period. The target organs more frequently affected were those of the central nervous system, followed by the gastrointestinal system. Occupational incidence information showed that the top three occupations with greater frequencies of pesticide poisonings were general clerk, registered nurse, and farm worker. It was determined that all occupations were potentially at some risk of pesticide poisoning. Some occupations had work tasks that provided more exposure to pesticides and could be classified as high-risk, such as farm workers. Other typically low-risk occupations, such as nurses and general clerks, were exposed to pesticides from patients who were poisoning victims or exposed to unacceptable levels of residue as a result of structural spraying. Non-Hispanics were shown to have experienced the majority of pesticide poisonings for the five-year period, with almost 62% of the total poisonings. Pesticide poisonings for Hispanics were shown to be only 8% of the total poisonings. Limitations in the study existed due to missing race data, which prevented the calculation of population-based rates to obtain a clearer picture. Thirty one percent of the total pesticide poisonings for the five-year period were not classified by race. ▼ Spores of various strains of Bacillus cereus are the causative agents of emetic and diarrheal foodborne illnesses. Typically, spores will survive thermal treatments that destroy vegetative cells, and then go on to germinate to form the vegetative cells that are associated with toxin production. The spore has to germinate in order to develop into the vegetative cells that produce toxins, hence a thorough understanding of the proteins and molecular mechanisms that underpin spore germination are of great significance from spore control perspectives. A major objective of this thesis was to use molecular genetic and fluorescence microscopy techniques to characterise the location and function of the GerP proteins in Bacillus cereus 14579. The GerP proteins have been identified from mutagenesis studies across the Bacilli as being implicated in spore germination, most likely by impacting upon the permeability of the spore coat. Data presented in this thesis reveal that the various GerP proteins all localise to the same inner-coat vicinity within the spore, as determined via the super-resolution ellipsoid localisation microscopy technique. The study also reveals that only the GerPA protein is required for the localisation of the other GerP proteins in the developing spore. A number of other coat and or germination associated proteins in B. cereus 14579 were examined in the course of this work. These include the GerN and GerT antiporters, which are both shown to have an involvement in inosine mediated spore germination in this strain. However, hypothetical interactions between antiporter proteins and the ‘linker-like’ N-terminal domain of the GerIA inosine-responsive germinant receptor protein appear unlikely since spores engineered with a truncated GerIA receptor subunit germinate normally. The protein encoded at locus BC1245 was also examined in this work, since it too had been implicated in spore germination. Data presented in this thesis indicate that this is not the case, and that the protein is a component of the spore coat. Overall, the work conducted in this project contributes to knowledge of spore assembly, spore structure and mechanisms that underpin germination, which ultimately, should permit the development of improved methodologies for spore control.

▼ Listeria monocytogenes is the causative agent of listeriosis, a severe foodborne infection that is increasing significantly in Europe and North America. A correlating factor contributing to the resurgence of listeriosis is the rise in consumption of cold-stored ready-to-eat (RTE) foods. The steady upsurge in disease requires more focused research to control the pathogen, L. monocytogenes. Currently, there is a plethora of diagnostic methods for the causative agent, however, each has limitations, one of which is the inability to correlate results across laboratories. This is a particular hindrance to an outbreak investigation in an age when food is transported widely across the globe. In this study, proteomic approaches were used to search for biomarkers that facilitate rapid characterisation of isolates against a background of differentially expressed proteins. A preamble to this investigation necessitated incorporation of an efficient lysis procedure to release maximum proteins. This was eventually achieved using a Listeria specific enzyme, endolysin, and a disruptive mechanical method. Liquid Chromatography-Mass Spectrometry/Mass Spectrometry (LC-MS/MS) data showed that bead beating and enzymatic lysis were the most efficient methods for analysis of the proteome. Dendrogram lineages, derived from MALD-TOF-MS spectra, strongly correlated with 16S rRNA analyses. Selective protein capture and analysis by MALD-TOF-MS (designated SELDI-TOF-MS) demonstrated considerable intraspecies diversity as revealed by dendrograms which were also visualised by „Heat Maps‟. One-dimensional polyacrylamide gel electrophoresis and LC-MS/MS analysis of seven L. monocytogenes isolates, led to the successful identification of two proteins; a hypothetical protein, designated lwe06778 and a phosphoribosyl-AMP cyclohydrolase which were uniquely present at 4°C. This finding suggests that L. monocytogenes depends on the histidine biosynthesis pathway in order to survive at cold temperatures. It is hypothesised that the addition of inhibitors, specific to both proteins in RTE cold foods may be a useful means for controlling outbreaks of listeriosis in the future.

▼ Thirty years since the removal of lead from gasoline, lead still poses a health risk. Children are most at-risk for adverse health outcomes caused by lead toxicity due to both behavioural (e.g., hand-to-mouth behaviour) and physiological differences (i.e., increased intake of lead by body weight, higher uptake rate and a higher vulnerability to the effects of lead) compared to adults (Yeoh et al., 2009). As a result, governments must identify children that may be at-risk of lead poisoning and develop practical methods to mitigate lead exposure. Before a government can develop a policy to help mitigate exposure of lead for children, we need to understand the spatial distribution of lead within the city. A popular spatial model used within air pollution research may allow more accurate, and more localized predictions than the most common interpolation method, kriging. Land use regression (LUR) is a technique leveraging multiple predictor variables to help estimate the spatial distribution of the dependent variable. By using historical sources of lead, LUR can be used to model soil lead levels (SLL) with localized variation. Unfortunately, spurious relationships can be the basis of a LUR model, which may lead to an overfitted spatial model resulting in a model with little generalizability and questionable ability to estimate the dependent variable at unobserved locations. Ultimately, Empirical Bayesian Kriging may be the best option for soil contamination research due to its ability to provide a smoothed prediction surface and its dependence on the spatial structure of the data to provide estimations. The benefit to society and the return on investment (ROI) is often the justification for lead remediation. Gould (2009) estimates a 17 to 221 ROI for every dollar spent on lead hazard control. One of the main components of this estimate of ROI comes from the decrease in intelligence quotient (IQ) that a child may experience as a consequence of lead toxicity. There are three main ways that a decrease in IQ can negatively impact the economy, (i) lower potential lifetime earnings, (ii) reduced tax revenues, and (iii) higher spending on special education (Gould, 2009). Since IQ has such a significant role in the ROI estimates, chapter 3 seeks to achieve a greater understanding of the relationship between blood lead levels (BLLs) and IQ. The loss of IQ points for an increase in blood lead concentration proposed by Lanphear et al. (2005) and referenced by Gould (2009) is significantly higher than what we found in our meta-analysis. Thus, the projected ROI proposed by Gould (2009) may be much lower than previously calculated. In the final chapter, the cost associated with permanent lead abatement is investigated based on ROI projections as a case study in Hamilton, Ontario. We show that, in most cases, permanent lead remediation is far too expensive for a municipal government. ▼ Acute kidney injury (AKI) following self-poisoning with herbicides is common and an important predictor of mortality. Early diagnosis may reduce mortality if successful treatments are developed. AKI is diagnosed by changes in serum creatinine (sCr), a surrogate of glomerular filtration rate (GFR). This delays diagnosis and underestimates extent of injury, so use of sCr to diagnose nephrotoxicity is suboptimal. We prospectively studied Sri Lankan subjects admitted to hospitals following deliberate drug/chemical ingestion and after snake envenomation. This thesis focuses on two herbicides, paraquat and glyphosate. Firstly, we hypothesised that the abrupt increase in sCr following paraquat poisoning was so rapid and was unlikely to be due solely to nephrotoxicity. We assessed this by comparing kinetics of sCr and serum cystatin C, and influences of non-renal factors. The increase in creatinine greatly exceeded that predicted or even possible with maximal decreases in GFR. We speculated this represents increased production of creatine and creatinine to meet energy demands following paraquat-induced oxidative stress. Creatinine was not a good marker of GFR and paraquat nephrotoxicity should be evaluated using more specific renal biomarkers.The utility of 10 structural injury biomarkers was then evaluated. Three (urinary cystatin C, NGAL and clusterin) of 10 showed a modest performance in detecting AKI. Most patients with AKI died due to multi-organ failure within 2 days and sCr predicted mortality independently of GFR. Therefore, any additional clinical utility of injury biomarkers to sCr was limited in paraquat poisoning.We assessed the influence of proteinuria on biomarker excretion. Albuminuria was associated with increased excretion of most biomarkers and biomarker cutoffs for prediction of death were higher. Diagnostic cutoffs for outcome prediction and stratification must be modified if albuminuria is due to comorbid conditions.Finally, the role of biomarkers in detecting nephrotoxicity and their added value to sCr was evaluated in 90 patients following glyphosate-surfactant herbicide (GPSH) poisoning. GPSH-induced nephrotoxicity was common but generally mild and reversible. It can be diagnosed within 8-16 hours by cytochrome C (uCytoC). The early increases in uCytoC and interleukin-18 support a primary mechanism of GPSH-induced nephrotoxicity involving mitochondrial toxicity and apoptosis and use of these biomarkers may identify mechanism-specific targets for treatments.
