▼ The periodontitis is an inflammatory disease condition caused by a variety of oral microorganisms. A hyper reactive host response regulation is triggering the disease progression in a small part of the world wide population – around 10% at most – resulting either in a local or generalized aggressive form of manifestation. Bleeding is a final act in a process of tissue and capillary damage initiated by bacterial presence and modulated by pro-inflammatory cytokines. Calibrated assignment of bleeding intensity as a parameter for the severity of the disease is considered golden standard for diagnosing gingivitis – an inflammatory condition restricted to the soft tissues. Periodontal inflammation regularly causes attachment loss and is accompanied by loss of alveolar bone and a phenomenon named “pocketing”. The severity of this disease condition is currently characterized by assessing either the probing pocket depths (PPD) or the attachment loss (AL). The evaluation of bleeding tendency as Bleeding on Probing (BoP) is currently an additional dichotomous finding. The aim of the present study was to assess the suitability of a visual calibration of a Pocket Bleeding Index (PoB-Index) to reflect the real inflammatory status of each single probed periodontal pocket. By matching single grades of the index suggested – i.e. the single visually assigned bleeding grades - with parameters such as bleeding duration (stop watch) and amount of blood sampled by capillary tube correlation coefficients were calculated. Correlations between PoB-Index and the probing depth and PoBI and the amount of gingival crevicular fluid (GCF) sampled as exudates that follow a gradient were calculated also to objectify the visual calibration of bleeding intensity. The capillary dilatation is initiated as first step in a cascade of inflammatory reactions in front of all clinical signs and results in a measurable increase of the GCF. Since periodontitis progresses episodically, i.e. phases of activity are followed by phases of inactivity the capillary dilatation follows the same pattern. The mean age of 17 participating patients recruited was 32 years; 10 of them were non-smokers whereas 7 were smoking. All patients enrolled did not have any periodontitis therapy or received any antibiotics prescribed for the last 12 months. All patients underwent intensive instructions in oral hygiene repeatedly, thus the allocated plaque index remained for the duration of the study < 25% (API < 25%). After completion of instrumentation (S/RP) all patients received Amoxicillin (500 mg) and Metronidazol (250 mg) x 3 daily for 10 days per os prescribed. The assessment of the study parameters was carried out at baseline after completion of the initial therapy (before S/RP), at month 3, 6 and 9 past active therapy. In total 575 sites were evaluated and monitored. For improving the reproducibility, all measurements were undertaken under equal conditions in all patients. ▼ INTRODUCTION Ultrasound imaging plays a major role in evaluation of pregnancies especially in the first trimester. Vaginal bleeding in early pregnancy is a relatively common presentation in the acute gynaecological setting, and ultrasonography remains the safest and fastest way of evaluating these patients. OBJECTIVE The aim of the study was to determine the ultrasound findings inpatients presenting with vaginal bleeding in the first trimester and their proportions. MATERIALS AND METHODS A cross-sectional descriptive study carried out at the University of Nairobi, Department of Diagnostic Imaging and Radiation Medicine (DDIRM, UON) and at Kenyatta National Hospital (KNH). All Patients presenting to the radiology department for ultrasound due to vaginal bleeding in the first trimester, between April and December, 2013, were evaluated with a pelvic ultrasound, either transabdominal, transvaginal or both. A data collection sheet was used to record the pelvic sonographic findings and possible predisposing factors associated with the vaginal bleeding. The data was analysed using the Stata computer software package and the results presented in the form of tables, pie charts and bar charts. RESULTS A total of 231 patients were recruited into the study. The age range was 16 – 47 years with a mean of 27.83 years and a median of 27 years with a standard deviation of 5.686. Most patients were in the 26 to 35 year age group (133 cases; 57.6%). All 231 patients had abnormal findings on ultrasound, but 6 of them had more than one finding, giving a total of 237 findings. The commonest finding was incomplete abortion accounting for 38.7% of cases, followed by ectopic pregnancy accounting for 28.1%, while the least common finding was uterine fibroids in pregnancy accounting for 0.4%. First trimester bleeding was associated with a high rate of pregnancy loss with 206 of the patients (89.2%) having pregnancy loss. CONCLUSION Vaginal bleeding during the first trimester of pregnancy was associated with high rates of pregnancy failure in the emergency setting, with incomplete abortion and ectopic pregnancy being common sonographic findings. One should therefore have a high suspicion index for these conditions and apply appropriate investigative procedures and management.

▼ Defects in haemostasis, specifically in the quantitative or qualitative properties of von Willebrand factor (VWF), result in the most commonly inherited bleeding disorder, von Willebrand disease (VWD). Of the three types of VWD, type 1 is the most common and has the mildest phenotype. Type 1 VWD has a symptomatic prevalence of approximately 1 in 1000, but a much smaller number of patients who are actually diagnosed. This discrepancy is a result of a lack of standardized diagnostic criteria. The objective of the Self-Administered Bleeding Assessment Tool (Self-BAT) Study is to develop and optimize, a standardized, self-administered BAT to quantify bleeding history, and to distinguish between normal and abnormal bleeding. In Phase 1 of the study, the medical terminology in the expert-administered ISTH BAT was changed to a grade four reading level to produce the first version of the Self-BAT. Three subsequent versions of the Self-BAT were generated during optimization based on focus group feedback and statistical analysis. The Self-BAT was administered to a total of 38 control subjects and 20 subjects affected with type 1 VWD in Phase 1. Demographic information on all the subjects (gender, age) was collected and blood was drawn for determination of VWF antigen level, VWF ristocetin cofactor activity and factor VIII level. Bleeding scores calculated from the ISTH BAT and Self-BAT showed an excellent correlation of 0.869 in the final version. In Phase 2 of the study, the Self-BAT was administered to 27 control subjects and 23 affected subjects with the objective to simplify the questionnaire by identifying questions which can discriminate between unaffected and affected subjects. It was determined through category-total, category-category and item-item correlations that there was not sufficient evidence to reduce the Self-BAT. In conclusion, the Self-BAT functions as a simple and effective tool to distinguish between normal and abnormal bleeding and is suitable for adults to administer themselves. The Self-BAT has now moved on to prospective validation studies where it will be tested on subjects who have been referred to tertiary care, but not yet received a diagnosis, in order to test its ability to distinguish between normal individuals and those affected with type 1 VWD and, potentially, other mild bleeding disorders.

▼ Introduction Capsule endoscopy (CE) is a first-line diagnostic tool for known or suspected small bowel bleeding (SBB), and its use has over time been expanded to include panenteric imaging. It offers advantages over conventional endoscopy in minimal invasiveness and ease of use. However several drawbacks remain including the lack of modalities other than imaging, inability to control or propel the capsule, lesser image quality compared to conventional endoscopy and labour-intensiveness of data interpretation. Aims and objectives This thesis aims to explore the ways in which use of CE can be optimised in the current clinical or “real world” context, focusing on its use in gastrointestinal bleeding and working within current resource and technological limitations. Methods A review and analysis of the existing literature was undertaken, examining the present state of CE technology and identifying current gaps in knowledge. Meta-analyses were undertaken examining the effectiveness of the two main methods of image enhancement in CE: the use of bowel preparation and currently available rudimentary computer-aided diagnosis. The following studies then looked into how to better select patients who should be prioritised for CE examination – a pertinent issue in today’s resource-stretched healthcare systems. A retrospective study was carried out to examine the effects of altering the timing of CE examination in patients referred for likely SBB, using cases carried out at our tertiary care centre over the past decade. Outcomes were compared between patients who had undergone CE following negative bidirectional endoscopies, or negative upper gastrointestinal tract endoscopy only. Furthermore, building on existing work, a second study was undertaken using a prospectively-designed database to collect multicentre data on findings and outcomes in young patients referred for CE with iron deficiency anaemia. This study investigated factors predictive of small bowel neoplasia in this patient group. Finally, the effect of image visualisation quality on diagnostic certainty was investigated. CE images were processed to alter image parameters, and the resulting images presented to an iii international group of expert CE readers in order to determine thresholds for acceptable image quality and the effects of differing image quality in the parameters examined. Results Currently-available image enhancement techniques: (1) Use of bowel preparation: Laxative use did not improve the diagnostic yield of CE with odds ratio (OR) 1.1 for both overall and significant findings when comparing laxative use with pre-procedural fast only. However, subjectively-determined small bowel visualisation quality improved with the use of laxatives (OR 1.60 (95%CI 1.08–2.06)), NNT 14. ▼ Atrial fibrillation (AF) is the most common sustained cardiac rhythm disorder and is increasing in incidence and prevalence in ageing populations. It is estimated that 1.5-2% of the developed world suffers from AF. The risk of stroke and thromboembolism (TE) is increased 4-5 fold by non-valvular atrial fibrillation (NVAF) and nearly 15% of all strokes are caused by AF. Judicious use of antithrombotic therapy significantly reduces the risk of stroke for most patients who have AF. Conversely, underuse of antithrombotic therapy is associated with an increase in the rates of death and ischaemic events (stroke, transient ischaemic attack [TIA], or myocardial infarction [MI]). Despite the proven benefits of anticoagulant therapy among high-risk patients with AF, there have been frequent reports of discordance between guideline recommendations and anticoagulant prescribing patterns leading to problem of underutilisation in patients with AF in real-world practices. During the time period of this study there were limited data available on the characteristics, clinical management and outcomes of patients with AF from an Australian perspective. The available literature suggested that there was underutilisation of anticoagulant therapy, although this data came from relatively small observational trials in selected patient groups. The Commonwealth Review of Anticoagulation Therapies in AF in Australia identified that stroke prevention in individuals with AF required improvement, and highlighted a range of issues to be addressed related to the assessment of patients for stroke and bleeding risk, appropriate choice of antithrombotic agent(s) in patients with multiple comorbidities, and the monitoring of patients. The review stressed the need for local data on which to base recommendations regarding the treatment of AF. Meanwhile, little was known about the clinical outcomes and safety of direct oral anticoagulants (DOACs) outside the trial setting. In 2011, dabigatran’s sponsor launched a patient familiarisation program in which over 28,000 Australians were enrolled. Thus, we established that there was a critical need for local data regarding the safety of antithrombotic medications, including the DOACs, in the treatment of AF. We designed this study, the Tasmanian AF (TAF) Study, as a starting point to providing comprehensive data describing the outcomes of stroke prevention strategies in Tasmanian patients with AF. The TAF study is an ongoing retrospective study that enrols patients from 3 different hospitals in Tasmania, Australia; the Royal Hobart Hospital (RHH), Launceston General Hospital (LGH) and North West Regional Hospital (NWRH). The rationale behind this project was to derive local data on usage pattern of antithrombotic therapy, clinical outcomes and their safety profile in Australian sub-population - initially prior to the Pharmaceutical Benefits Scheme (PBS) listing of DOACs, and then to use the established database to monitor the impact of the introduction of the DOACs into clinical practice. Uvod: Krvarenje nakon perkutanih koronarnih intervencija (PCI) je važna komplikacija sa značajnim nepovoljnim uticajem na prognozu. Cilj: Da se proceni prediktivna vrednost preterano niske rezidualne reaktivnosti trombocita na dvojnu antiagregacionu terapiju aspirinom i klopidogrelom, za krvarenje nakon elektivne PCI. Metodologija i rezultati: Kod 481-nog uzastopnog bolesnika sa elektivnom PCI, izvršeno je testiranje funkcije trombocita pomoću Multiplejt analizatora, gde je ispitivana reaktivnost trombocita indukovana arahidonskom kiselinom (ASPI) i adenozin difosfatom (ADP), neposredno pre PCI i 24 časa nakon PCI. Pacijenti su praćeni prosečno 15.34±7.19 meseci. Primarni cilj studije bio je pojava svakog krvarenja, dok je sekundarni cilj bila pojava ishemijskog neželjenog srčanog događaja (MACE). Učestalost svih, BARK≤2, i BARK≥3 krvarenja prema BARK klasifikaciji je bila 19%, 18% i 1%, odgovarajuće. Grupe sa svim, i BARK≤2 krvarenjima, su imale niže prosečne vrednosti multiplejtADP testa nakon 24 časa, u poređenju sa grupom bez krvarenja: 45.30±18.63 U prema 50.99±19.01 U; p=0.005; i 45.75±18.96 U prema 50.99 ± 18.99 U; p=0.01; odgovarajuće. Ženski pol (HR 2.11; CI 1.37-3.25; p=0.001), prethodni infarkt miokarda (HR 0.56; CI 0.37-0.85; p=0.006), niska telesna težina (HR 0.78; CI 0.62-0.98; p=0.03), i vrednosti multiplejt ADP testa nakon 24 časa od PCI ispod medijane (HR 0.75; CI 0.61-0.93; p=0.009) su bili nezavisni prediktori svih krvarenja, određeni pomoću Koksove univarijantne analize. ▼ IntroductionTelangiectatic matting (TM) is a morphological description referring to vessels with a small diameter of less than 0.2 mm that can appear sporadically or in well-defined patches (hence the term ‘matting’) primarily on the lower limbs. The aetiology underlying TM remains uncertain however angiogenic and inflammatory mechanisms are believed to play a pathogenic role. The aim of this study was to investigate the pathogenesis of TM and identify possible risk factors. MethodsThis study had two parts. The clinical records of consecutive patients were retrospectively analysed to identify risk factors for TM. In the second part, the haemostatic and coagulation profile of 12 patients with TM were analysed and compared with 12 controls using standard coagulation tests, platelet function and a global assay of coagulation namely rotational thromboelastometry (ROTEM).ResultsIn 352 consecutive patients presenting to a phlebology practice, 25 patients had TM (7.1%). All 25 patients were female with the median age of 45 (27-57) years. A comprehensive medical history was taken. Amongst 27 possible risk factors assessed, statistically significant associations included recurrent epistaxis, easy bruising, hypersensitivity (eczema, hives, hay fever, rhinitis), previous treatment with sclerotherapy or vascular laser for lower limb veins and a family history of telangiectasias. The haemostatic and coagulation profile of patients with TM did not differ significantly from those without TM.ConclusionTM is associated with both hypersensitivity and a bleeding tendency. Our study revealed no significant increase in the incidence of haemostatic abnormalities in patients with TM compared with the control group. Given the significant association with hypersensitivity disorders we believe underlying mast cell hyperreactivity may contribute to both hypersensitivity and a bleeding tendency and predispose patients to TM. Further research studying mast cell activity is warranted to establish a definite link. ▼ The objective of this study was to determine the feeding habits of the Mazama pocket gopher (Thomomys mazama Merriam) in the pine region of south-central Oregon. Damage caused by pocket gophers to conifer regeneration was a major incentive for this research. A permanent reference collection, consisting of epithelial tissue from leaves, stems, and roots of local plants mounted on microscope slides, was prepared. Pocket gophers were collected on alternate months from July 1973 through September l974. Species of plants eaten were identified by comparing fragments of epithelial tissue in the stomach contents to the reference collection. Shrubs, common mullein (Verbascum thapsus L.), and ponderosa pine seedlings (Pinus ponderosa Dougl.) also were examined in the field monthly from December 1973 through September 1974 for evidence of above-ground feeding by gophers. The relative abundance of plants on the study area was estimated by frequency of occurrence and percentage cover. Preference indexes for frequently eaten foods were obtained by dividing the precentage composition in the diet by the plant cover on the study area. Stomach content analysis revealed that 31 of the 45 plant species identified on the study area were used as food by gophers. Plant materials eaten included foliage, stems, and roots. Forbs comprised 55 percent of the vegetative cover in July 1973, and 61 percent in July 1974. They constituted 40 percent of the annual diet, and were eaten most heavily during the growing season. The most frequently eaten forbs were spreading groundsmoke (Gayophytum diffusum Mutt.), matted nama (Nama densum Lemmon), knotweed (Polygonum douglasii Greene), common thistle (Cirsium vulgare(Savi) Tenore), goosefoot violet (Viola purpurea Kell.), and velvet lupine (Lupinus leucophyllus Dougl.). Grasses comprised 41 percent of the vegetative cover in July 1973, and 30 percent in July 1974. They constituted 32 percent of the annual diet, and were eaten extensively during the dormant season. Cheatgrass (Bromus tectorum L.) was eaten only in May. Western needlegrass (Stipa occidentalis Thurb.) was eaten most heavily in summer and autumn, and mountain brome (Bromus carinatus H. & A.) was consumed most heavily during the dormant season. Stomach content analysis indicated that woody plants were eaten during the dormant season, but they constituted only a small part of the annual diet. Ponderosa pine was the most heavily eaten woody plant. Field observations supported these results. Evidence of gophers feeding above ground on woody plants was found only from December through April. In general, the diets of gophers collected during the growing season, July and September, were not significantly different. The diets of gophers collected during other sampling periods were significantly different (p<O.O5). Availability and relative abundance of foods were identified as important dietary determinants. ▼ In the last few years, many schools and universities have incorporated personal digital assistants (PDAs) into their teaching curricula, in an attempt to enhance students' learning experience and reduce instructors' workload. One of the most common uses of PDAs in the classroom is as a test administrator. This study compared the usability effectiveness, efficiency, and satisfaction of a PDA-based quiz application to that of standard paper-and-pencil quizzes in a university course in order to determine whether it was advisable to invest time and money in PDA-based testing. The effects of computer anxiety, age, gender, and ethnicity on usability were also evaluated, to ascertain that these factors do not discriminate against individuals taking PDA-based tests. Five quizzes were administered to students participating in an engineering introductory course. Of these, students took two PDA-based quizzes and three paper-and-pencil quizzes. One PDA-based quiz and one paper-and-pencil quiz were compared in terms of their effectiveness, measured as students' quiz scores and through a mental workload questionnaire; their efficiency, which was the time it took students to complete each quiz; and their satisfaction, evaluated using a subjective user satisfaction questionnaire. Computer anxiety was also measured, using an additional questionnaire. It was hypothesized that the PDA-based quiz would be more effective and efficient than the paper-and-pencil quiz and that students' satisfaction with the PDA-based quiz would be greater. The study showed the PDA-based quiz to be more efficient, that is, students completed it in less time than they needed to complete the paper-and-pencil quiz. No differences in effectiveness and satisfaction were found between the two quiz types. It was also hypothesized that for PDA-based quizzes, as computer anxiety increased, effectiveness and satisfaction would decrease; for paper-and-pencil quizzes there would be no relationship between computer anxiety and effectiveness and no relationship between computer anxiety and satisfaction. Findings showed an increase in quiz score (increase in effectiveness) and an increase in mental workload (decrease in effectiveness) as computer anxiety increased for both quiz types. No relationship was found between computer anxiety and satisfaction for either paper-and-pencil or PDA-based quizzes. The final hypothesis suggested that user satisfaction would be positively correlated with effectiveness (quiz score and mental workload) for both PDA-based and paper-and-pencil quizzes. No relationship was found between quiz score and satisfaction for either quiz type. User satisfaction was positively correlated with mental workload, regardless of quiz type. The usability comparison of paper-and-pencil and PDA-based quizzes found the latter to be equal, if not superior, to the former. ▼ The use of mobile phones and other mobile devices are becoming widespread and almost all of these mobile devices have some sort of mobile Internet access. Due to the increase in mobile Internet usage, many websites need altering in order to become mobile compatible. Creating a mobile compatible version of a website is challenging due to formatting and capability restrictions imposed by the mobile device. Currently, one of the popular methods of creating a mobile compatible website involves the creation of a new, dedicated mobile version of the website. However, this approach can prove to be expensive, and repetitive, since a fully functional desktop version of the website often already exists. A second method involves the use of a transformation proxy to transform the traditional website into a mobile compatible version. This research develops a transformation framework that enables a web developer to create a single set of source files that can be used to render output compatible with both traditional and mobile devices. In developing this framework, capabilities and restrictions of the mobile device were examined. Furthermore, current mobile web development guidelines and best practices were discussed. This resulted in the development of a method to identify and outline areas of a traditional website for transformation into a mobile friendly format. Furthermore, a transformation engine that allowed processing of the traditional website into a mobile compatible website was developed. This transformation engine extracted the outlined areas, and rendered the extracted content, all while maintaining the website’s original functionality. The development of a prototype verified that the transformation concepts are valid, and provided for the development of guidelines and recommendations. ▼ I used field experiments to study how plants in two grassland communities colonized soil mounds made by the Camas pocket gopher, Thomomys bulbivorus (Richardson). I identified potential mound colonizers in each source of colonization (buried propagule bank, seed rain, and established vegetation) and then measured species specific rates of colonization on mounds built by T. bulbivorus. By selectively eliminating different avenues of colonization on artificial mounds, I estimated the relative and combined effects of colonization from (1) germination and growth of buried viable seeds and growth of root fragments in the soil; (2) germination of seeds raining onto the mounds; (3) emergence of buried vegetation and, (4) encroachment and establishment of adjacent vegetation. Artificial mounds were good mimics of mounds built by T. bulbivorus judged by their similarity in colonization rates and composition of colonizing species. I repeated the investigation in adjacent pasture and prairie communities differing in species composition and abundances to compare the effects of these differences on the colonization process. Composition and abundance of species in the expressed and potential vegetation varied considerably between pasture and prairie as did the two communities' response to identical gopher disturbances. Despite these differences, the relative contribution of each source of colonization was strikingly similar in the two communities. Vegetative encroachment and emergence contributed more to overall colonization rates (76% in the pasture; 75% in the prairie) than did establishment from seeds or buried root fragments. Emergence from underneath the mounds was favored by the shallow depth of mounds, minimal alteration of the substrate associated with mound building, and dominance of perennial species with erect growth forms. The small area and high perimeter to surface area ratio resulted in a high percent colonization from encroachment of surrounding vegetation. Colonization from the rain and bank contributed less to mound closure and may have been limited by a low abundance of propagules in those two sources. Successful colonists differed in their patterns of colonization. Festuca rubra, Agoseris heterophylla, Plantago lanceolata and Prune lla vulgaris colonized almost exclusively via emergence. ▼ In the upper Sonoran and transition zone areas of southern British Columbia populations of Perognathus parvus living in different habitats exist within a few miles of each other. The area was deglaciated about 10,000 years ago setting a maximum time for occupancy by small mammals. This study was initiated to compare the adaptations of individuals of these populations to the different habitats and, if possible, comment on their evolution and distribution. Animals from an environmental gradient were examined in the field and laboratory. One end of the environmental continum (low area) was characterized by low rainfall, high temperatures and a long summer, the other (high area) by high rainfall, low temperatures and a long winter. Specimens from kill trapping indicated that the subspecies under consideration was morphologically variable with characteristics distributed in a checkerboard pattern. Analysis of stomach contents indicated that food during the summer was about equally divided between seeds, green vegetation and animal material. Seeds were the major item stored for winter food. Low area animals ate comparatively more green vegetation, possibly in response to greater water loss. Intensive live trapping and dissection of specimens indicated that high area females came into reproductive condition earlier in the spring and ceased reproducing earlier in the summer, producing fewer litters than the low area females. Young-of-year females in the low area reproduced while those in the high area did not. Average litter sizes (4.85) were the same. There was no postpartum estrus. Home ranges sizes of males (895 m²) in the high and low areas were the same. The ranges of the females (656 m²) were smaller. Burrows and home range centers were apparently randomly distributed and ranges of both sexes overlapped. Density was highest in the low area and decreased with altitude. Long term survival rates were high in all groups except low area young animals. Short term survival rates were highest in the winter, lowest in the spring and intermediate in the summer and fall. High area animals entered torpor earlier in the fall than did low area animals. Adults apparently entered torpor before young animals. In the laboratory animals tended to enter torpor during the dark period and leave torpor during the light period after 3-168 ([formula omitted] = 46) hours in torpor. Percent of time spent in torpor increased with time at 5 C and levelled off at about 60%. When maintained at low temperatures high area animals had significantly longer torpor periods and spent a greater proportion of time in torpor than did low area animals. Subjection to water stress indicated that the low area animals were better able to conserve water by reacting more quickly to dehydration. When dehydrated low area animals were able to maintain allow plasma osmotic concentration while high area animals were not. Nekoliko randomizovanih kliničkih studija je pokazalo da pacijenti sa velikim krvarenjem kao komplikacijom akutnih koronarnih sindroma (AKS) i perkutane koronarne intervencije (PKI) imaju lošiji klinički ishod, sa povećanim rizikom od nastanka glavnih neţeljenih kardijalnih dogaĎaja (MACE), koji obuhvataju smrtni ishod, infarkt miokarda, šlog i trombozu stenta, u odnosu na bolesnike bez krvarenja. Prema tome, prvencija krvarenja je postala isto toliko značajna kao prevencija ishemijskih dogaĎaja. MeĎutim, napore da se ovo sprovede ometa nedostatak jednobraznosti u definicijama krvarenja koje se danas koriste. U cilju standardizacije brojnih definicija krvarenja kod bolesnika koji primaju antitromboznu terapiju, tim naučnika okupljen oko Bleeding Academic Research Consortium-a (BARC) je nedavno konsenzusom doneo klasifikaciju krvarenja koristeći hijerarhijski pristup pri opisu stepena teţine krvarenja. Pošto se od nove jedinstvene definicije krvarenja očekuje da bude široko prihvaćena u kliničkim studijama i svakodnevnoj kliničkoj praksi, potrebno je da se njena vrednost potvrdi putem primene na već postojećim podacima o krvarenju ili podacima sakupljenim putem budućih kliničkih studija. Cilj: Glavni cilj ove studije je da ispita odnos izmeĎu bolničkog krvarenja procenjenog prema BARC klasifikaciji i kratkoročnog i dugoročnog mortaliteta/MACE-a kod neselektovane populacije bolesnika sa akutnim infarktom miokarda sa elevacijom segmenta ST (STAIM) podvrgnutih hitnoj koronarnoj angiografiji i lečenih metodom primarne PKI. Metode: Studija je obuhvatila 1808 konsekutivnih bolesnika sa STAIM, primljenih u periodu od avgusta 2009 do januara 2011 godine na Odeljenje urgentne kardiologije Kliničkog centra Srbije u Beogradu radi lečenja metodom primarne PKI. Podaci o bolesnicima sakupljeni su prospektivno i uneti u jedinstvenu elektronsku bazu podataka. Η οξεία αιμορραγία ανώτερου πεπτικού (ΟΑΑΠ) παραμένει ένα από τα πιο συχνά και επείγοντα περιστατικά που συνοδεύεται με αυξημένη νοσηρότητα και θνητότητα. Σκοπός της μελέτης είναι να δειχθούν αλλαγές στα κλινικο-επιδημιολογικά χαρακτηριστικά των ασθενών με ΟΑΑΠ την τελευταία δεκαετία. Συλλέχθηκαν δεδομένα από όλους τους ασθενείς που εισήχθησαν στα νοσοκομεία του Νομού Αχαΐας με ΟΑΑΠ από 1η Ιανουαρίου ως την 31η Δεκεμβρίου 2005 και έγινε αναδρομική σύγκριση με δεδομένα από ασθενείς που εισήχθησαν πριν 10 έτη στην ίδια περιοχή την περίοδο από 1η Ιανουαρίου ως τη 31η Δεκεμβρίου 1995. Η επίπτωση των ασθενών με ΟΑΑΠ και των πεπτικών ελκών και στις δυο περιόδους υπολογίσθηκε με βάση δεδομένων στοιχείων από την Εθνική Στατιστική Υπηρεσία. Παρατηρήθηκε μείωση στην επίπτωση των ασθενών με ΟΑΑΠ από 162,9/100.000 πληθυσμού το 1995, σε 108,2 /100.000 πληθυσμού (RR=0,49, CI 95%=0,37-0,63) το 2005 και στην επίπτωση των ασθενών με αιμορραγία από πεπτικό έλκος (ΑΠΕ) από 104,8 /100.000 πληθυσμού σε 72,5 /100000 (RR=0,49, CI 95%=0,35-0,68). Η μείωση αυτή οφειλόταν κυρίως στη μείωση της επίπτωσης των ασθενών με αιμορραγία από δωδεκαδακτυλικό έλκος (ΑΔΕ)(από 66,7ασθενείς/100.000 σε 35,5/100.000 πληθυσμού), ενώ η επίπτωση της αιμορραγίας από γαστρικά έλκη παρέμεινε στα ίδια επίπεδα (από 33,1 /100.000 σε 34,4 /100.000). Η μέση ηλικία των ασθενών αυξήθηκε από 59,4±17,1 έτη σε 66,1±16,1, p<0.0001, όπως και η συν-νοσηρότητα των ασθενών. Το ποσοστό χρήσης μη στεροειδών αντιφλεγμονωδών φαρμάκων (ΜΣΑΦ) σε αυτούς τους ασθενείς παρέμεινε σταθερό (49,3% vs 48,2%), ενώ η χρήση από του στόματος αντιπηκτικών και αντιαιμοπεταλιακών φαρμάκων αυξήθηκε σημαντικά (από 2,2% σε 6,8%, p=0,001 και από 1,2% σε 10,8%, p<0,0001 αντίστοιχα). Η συχνότητα υποτροπής αιμορραγίας σε ασθενείς με ΑΠΕ ,καθώς και η συχνότητα της επείγουσας χειρουργικής αιμόστασής τους μειώθηκαν σημαντικά (από 12% σε 5,9%, p=0,02 και από 8,9% σε 3,4%, p=0,009, αντίστοιχα). Δεν ανεβρέθηκε στατιστικά σημαντική διαφορά στην συνολική θνητότητα (3,9% το 1995 vs 6,5 % το 2005). Η επίπτωση της ΟΑΑΠ κατά την τελευταία δεκαετία μειώθηκε σημαντικά κυρίως λόγω της μείωσης της επίπτωσης των ασθενών με ΑΔΕ. Οι ασθενείς αυτοί είναι πιο ηλικιωμένοι με αυξημένα συνοδά νοσήματα, αλλά χωρίς να έχει μεταβληθεί στατιστικά η θνητότητά τους τα τελευταία δέκα έτη.

▼ Factor VIII (FVIII) is a co-factor in the haemostatic system required for fibrin-rich clot formation. Inherited F8 gene defects result in haemophilia A (HA), one of the commonest inherited bleeding disorders. Acquired FVIII defects (acquired haemophilia A, AHA) occur through auto-antibody formation. FVIII antibodies (allo and auto-antibodies) are the greatest challenges facing the haemophilia treating physician. Prediction of risk of antibody formation is based on genetic and environmental factors. There is incomplete understanding of the total immune response to FVIII due to limitations in current laboratory methodology used for FVIII antibody testing. The aim of this this work was to assess clinical practices, laboratory methodology and high-throughput approaches to further characterise the immune response to FVIII. The key findings are as follows: 1) Sub-optimal compliance with targeted inhibitor screening following FVIII treatment was seen in non-severe HA in London haemophilia centres. 2) A national survey of AHA demonstrated heterogeneity in the management of immunosuppression and testing. 3) A FVIII ELISA was specific with a high negative predictive values for FVIII antibody detection in routine practice. 4) Pre-analytical heat treatment prior to antibody testing, improved sensitivity for auto-antibody detection and a systematic evaluation optimised incubation conditions for this modification. 5) A novel re-usable high-throughput peptide microarray, characterised B-cell epitopes of monoclonal and polyclonal FVIII antibodies, demonstrating immunodominant epitopes in regions of functional or structural importance. 6) A modified low volume RNA sample tube demonstrated feasibility for transcriptome analysis in patients with severe haemophilia A, providing a repository of transcriptome data for developing understanding of the allo-immune response to FVIII. Heterogeneity in clinical and laboratory practices limits interpretation of data from observational studies of FVIII antibodies. Improvements in detection and characterisation of FVIII antibodies, may further advance understanding of the total immune response to FVIII.
