▼ Research on the efficacy of hypnosis has been limited due to the lack of a sham hypnosis (placebo) for comparison to use as a control in randomized clinical trials. Instead, researchers have had to use a variety of controls, resulting in inconsistency that greatly limits researchers’ methodology and makes it difficult to compare study results or make aggregate statements regarding hypnosis’ efficacy. This study consisted of three primary aims: 1) to evaluate whether white noise can be considered an "inert" procedure; 2) to evaluate the credibility of a model of sham hypnosis that uses white noise as a potential form of "hypnosis" when presented within the hypnotic context; and 3) to explore the relationships between participant characteristics and specific outcome measures. Seventy-five undergraduate students were randomized to one of three groups: hypnosis; sham (white noise presented in the context of hypnosis); or control (white noise in the absence of hypnotic context). Measures of interest involved participants’ ratings of: 1) therapist’s professionalism; 2) the consistency of the environment with hypnosis; 3) subjects’ perception that they received hypnosis; 4) subjects' evaluation of the procedure as pleasant, relaxing, and beneficial 5) participants' perceived acceptability (evaluated by assessing perception of the procedure as acceptable, ethical, effective, of the procedure they received; and 6) shifts in relaxation resulting from each procedure. In each of the variables of interest, subjects who received sham hypnosis and those who received a hypnotic induction demonstrated significant differences from those assigned to the white noise control, with effect sizes ranging from .165 to .852. However, there were no significant differences between participants' ratings of the sham and hypnosis procedure in any of these domains. Taken together, results support the feasibility of using white noise as an inert procedure that, given the proper environmental context, can serve as a credible sham hypnosis. ▼ This thesis focuses on animal models of placebo analgesia. Placebo analgesia is the experience of reduced pain, or pain relief, without the delivery of a pain-relieving drug or actual reduction of the painful stimulus. Research into the underlying neuro-mechanisms of placebo analgesia have been limited by unreliable animal models of the effect and attempts to replicate human placebo testing paradigms in animals have had mixed outcomes. In many cases, a tolerance to analgesic has been observed instead. The project had two main aims. The first was to replicate a recent model of placebo analgesia. The second aim of the project was to conduct a meta-analysis of the existing published literature and determine the overall effect size of placebo analgesia in rodent models. Experiment 1 sought to replicate a previous model of rodent placebo analgesia. Results from experiment 1 did not support previous findings and showed no evidence for placebo analgesia in rodents. Experiment 2 was a second attempt to replicate a previous model of placebo analgesia. The second attempt adjusted the potential limitations from experiment 1 such as cue chamber intensity and re-ran the protocol using rodents of the same sex and strain as the original paper. Results from experiment 2 were similar to experiment 1 and did not support the hypotheses or provide any evidence for placebo analgesia in rodents. Experiment 3 used a simplified version of the behavioural conditioning methodology from experiments 1 and 2, but was modified to reduce handling stress, to increase the salience of the to-be-conditioned placebo cue, and to reduce stress-induced analgesia. Results from experiment 3 found no evidence for placebo analgesia in rodents was observed. The results from 45 studies across 21 papers were meta-analysed and showed a moderate effect size for placebo analgesia in rodents, and moderator analysis revealed that cue type was a significant factor in the development of the effect in rodents. Using a context cue box as the cue type was found to be the most effective approach to establishing placebo analgesia in rodents. The current research has highlighted the replicability problem in pre-clinical research and as such it is recommended that animal studies looking at placebo analgesia aim to include more methodological detail in their reporting, particularly in regard to cue type. A limitation of the current project was the exclusion of drug tolerance literature that results in nocebo responses. Future research should compare and contrast this body of work more closely with research into placebo analgesia in rodents.

O efeito placebo caracteriza-se pela redução de um sintoma ou melhora de um quadro clínico que se obtém após a administração de uma substância farmacologicamente inerte (placebo) ou um procedimento sabidamente ineficaz. O efeito placebo tem sido observado em diferentes doenças e sintomas e a magnitude de resposta varia entre 30% a 70%. Alguns fatores que estão associados ao efeito placebo são conhecidos e diversas teorias tentam elucidar os mecanismos psicobiológicos envolvidos na resposta a placebo. No entanto, o entendimento ainda é limitado. Objetivos: a) Verificar se há característica de personalidades associadas à resposta a placebo em estudos com antidepressivos; b) Verificar se há associação de polimorfismos em genes do sistema serotonérgico com resposta a placebo. Métodos: Foram incluídos no estudo pacientes que participaram de ensaios clínicos com antidepressivos e placebo conduzidos no Instituto de Psiquiatria do Hospital de Clínicas da Faculdade de Medicina da USP (IPQ-HCFMUSP), entre 2000 e 2005. Os sujeitos (n=57) [pacientes deprimidos (n=14), pacientes fóbicos (n=22) e voluntários saudáveis (n=21)] foram considerados respondedores a placebo, de acordo com os critérios estabelecidos pelo estudo de origem. Para a análise das variáveis de personalidade os sujeitos responderam o Inventário de Temperamento e Caráter (ITC) e para a análise das variáveis genéticas foram escolhidos três genes polimórficos do sistema serotonérgico: do transportador de serotonina (SLC6A4), do receptor de serotonina subtipo 2A (HTR2A) e do receptor de serotonina subtipo 1B (HTR1B). Análise estatística: foi utilizado o modelo de análise discriminante para avaliar o comportamento das variáveis [idade, escolaridade, gênero e diagnóstico] e uma regressão logística para verificar quais destas variáveis explicavam as diferenças observadas entre os respondedores e não respondedores, bem como seu grau de importância. Resultados: Não houve diferenças estatisticamente significativas entre os respondedores e não respondedores a placebo, em relação à idade, escolaridade e gênero. Os respondedores a placebo apresentaram escores significativamente maiores nos fatores de personalidade busca de novidade e autotranscendência quando comparados aos não respondedores. Os resultados da análise genética mostraram uma sugestiva associação entre dois genes polimórficos [o 5HTTLPR (SLC6A4) e o gene 5HTR1B (G861C)] e a resposta a placebo. Conclusões: Os resultados sugerem que a resposta a placebo é influenciada por características de personalidade em sujeitos sadios e pacientes com diagnóstico de fobia social e depressão e por polimorfismos genéticos em sujeitos sadios.

▼ Il est largement reconnu que l'affectivité est en mesure d'influencer l'évolution de la douleur. D'une perspective clinique, les individus qui souffrnt de douleur chronique sont également affectés par des troubles affectifs reliés à la douleur. Plusieurs études ont également démontré un déficit au niveau des contrôles inhibiteurs diffus nociceptifs (CIDN) dans plusieurs cas de douleur chronique. D'un point de vue physiologique, la boucle spino-bulbo-spinale qui sous-tend les mécanismes endogènes descendants possède également des connexions avec des structures limbiques impliquées dans la composante affective de la douleur. De nombreuses études en neuro-imagerie supportent en fait que les CIDN pourraient être modulés par certaines régions du système limbique, notamment le cortex cingulé antérieur (CCA). Cependant, aucune étude n'a à ce jour investigué le lien entre la composante affective de la douleur et les CIDN. L'objectif de notre étude est donc de déterminer si la manipulation de la composante affective de la douleur, par le biais de suggestions spécifiques, est en mesure de moduler l'efficacité analgésique des CIDN. Trente-deux participants ont été recrutés par un échantillonnage de convenance. L'efficacité des CIDN a été évaluée à travers trois conditions du stimulus, soit une condition neutre (absence de suggestion), une condition où une augmentation de l'aspect désagréable est suggérée (condition A) et une condition où une diminution de l'aspect désagréable est suggérée (condition B). La réponse analgésique des CIDN a été déterminée par un test de douleur thermique et un test d'immersion de l'avant-bras (stimulation conditionnante) dans un bain d'eau froide. Les résultats démontrent une bonne réponse aux suggestions de la part des participants, indiquant que la manipulation cognitive a bien fonctionné. Les résultats relatifs à l'efficacité des CIDN ne démontrent aucun changement significatif entre la condition neutre et la condition A. Une diminution significative de la réponse analgésique par rapport à la condition neutre est néanmoins observée à la condition B. Ces résultats indiquent que l'augmentation de l'aspect désagréable perçu de la stimulation conditionnante n'évoque aucun changement au niveau de la réponse analgésique des CIDN, résultats qui pourraient être expliqués par un effet plafond. Cependant, une diminution de l'aspect désagréable perçu réduit de façon significative l'efficacité des mécanismes descendants. Ces observations démontrent que la composante affective de la douleur est en mesure de moduler l'efficacité analgésique des CIDN, bien qu'une réponse analgésique efficace à un niveau basal soit plus difficile à potentialiser. Nos résultats rapportent des preuves supplémentaires sur le rôle de l'affectivité dans les réponses douloureuses, démontrant davantage son importance. ▼ Thesis title: The Effect of Choice on Placebo Treatment for Sleep Abstract: Recent studies suggest that choice may enhance the placebo effect in the treatment of acute conditions. However, in those studies, there is often a failure to detect a placebo effect in the absence of choice, making it unclear as to whether choice provides an advantage when a significant placebo effect exists without choice. To address this, this thesis examined the role of choice in placebo effects for a chronic condition where placebo effects exist independent of choice. First, a review of the literature on the placebo effect and choice effects is presented. Second, a meta-analysis was conducted to confirm that placebo effects are reliably produced for sleep difficulty without choice. The meta-analysis indicated statistically reliable small to moderate placebo effects in perceived sleep onset latency (SOL), total sleep time (TST), and global sleep quality (GSQ). Finally, an empirical study tested the influence of choice on the placebo effect for sleep difficulty. 117 participants with self-reported sleep difficulty were recruited under the guise of a hypnotic trial and randomized to receive: placebo treatment with single choice, daily choice, or no choice, or no treatment. Significant placebo effects were found for perceived insomnia severity, fatigue, and sleep quality, with treatment-induced expectancy fully mediating the placebo effects for insomnia severity and sleep quality, and partially mediating effects in fatigue. However, placebo effects were not found for perceived SOL and TST, nor for objective SOL and TST measured via actigraphy. There was no evidence of enhancement of the placebo effect by choice on any of the outcomes, nor were there any differences between making a single choice versus daily choices. These findings suggest that choice may not significantly strengthen placebo effects over longer treatment periods, or provide a significant advantage when placebo effects already exist without choice.

▼ The placebo literature suggests that people’s beliefs about: (i) what will occur when they ingest or do not ingest a drug, (ii) whether they have or have not ingested a drug, can elicit involuntary responses consistent with those beliefs independently of the pharmacological effects of the drug in question. This thesis aimed to test whether caffeine withdrawal symptoms could be affected by manipulating moderate to heavy coffee drinkers’ beliefs concerning: (i) the likelihood of experiencing caffeine withdrawal symptoms, (ii) how much caffeine they had in their system. Studies 1 and 2 demonstrated that the caffeine withdrawal symptoms experienced by 24-h abstinent caffeine consumers can be significantly reduced simply by their being told they had consumed caffeinated coffee, even though they had consumed only decaf. However while manipulating beliefs concerning the ingestion of caffeine did affect self-reported withdrawal symptoms, attempts to affect caffeine withdrawal by providing information about the likelihood of experiencing those symptoms was not. Study 3 suggested the possibility that placebo caffeine withdrawal relief may be a conditioned response, occurring in the absence of both conscious expectancies and caffeine. Studies 4 and 5 demonstrated the practical potential of blinded dose tapering regimens, with participants who were made aware of their true dose reduction schedule showing steeper rates of withdrawal increase over time than (i) participants who were deceived about their dose reduction schedule, and (ii) participants who were given no information about their dose reduction schedule. The studies in this thesis indicate that individuals’ beliefs about how much of a drug they have in their system can have a significant effect on withdrawal symptoms. Interventions that utilise this knowledge, such as the blind dose tapering procedure modelled in studies 4 and 5, may reduce withdrawal and help addicted individuals transition to a drug-free state.

▼ Fragile X Syndrome (FXS) is a disorder caused by a congenital mutation of the FMR1 gene on the X chromosome. FXS is associated with moderate to severe intellectual disability and is one known cause of autism spectrum disorders. There are no approved medications to treat FXS symptoms. In 2013, Seaside Therapeutics completed two Phase 3 studies of an investigational medication, STX209, for treatment of social withdrawal in FXS. Efficacy results for these studies were not positive. Clinical trials of psychoactive drugs often fail to show a statistical difference from placebo controls and a robust response to placebo is often cited as a reason for the failure. Retrospective studies of baseline variables in clinical trials have identified characteristics that were associated with an increased likelihood of responding to placebo. Such information is valuable for the design of future clinical trials and no such studies have been conducted in FXS. This study was a post-hoc analysis of data from Seaside Therapeutics' Phase 3 clinical trials in FXS. Baseline variables for subjects receiving placebo were pooled for analysis. To determine if a subject responded to placebo, the parent-rated ABC-SA and the ABC-IR were used. Clinician-rated assessments, including the CGI-S and CGI-I, were examined as well. Two-sample t-testing, one-way ANOVA testing, and correlation coefficients were calculated to compare the responses of subjects with different baseline characteristics. General linear regression modeling was used to determine if there were multiple baseline variables that could predict placebo response. Logistic regression modeling was used to determine if the baseline variables could predict whether a subject had a higher chance of being a treatment responder. A total of 287 subjects were randomized and completed the Phase 3 studies. Analyses for this study were conducted in a subgroup containing 106 subjects who received placebo. 76% improved during the study on the ABC-SA, indicating that there was a strong placebo effect on the study. None of the dichotomous baseline variables were associated with statistically significant differences in ABC-SA, ABC-IR, CGI-S, or CGI-I scores. Placebo-treated subjects in the 209FX302 study who were taking antipsychotics improved less on the CGI-S than those not on those medications. A similar pattern was observed on the ABC-IR and ABC-SA. Other categorical baseline variables were tested and there was no difference in the mean changes. The CGI-S score at baseline appeared to predict a statistically significant difference in the ABC-IR as more severe subjects were more likely to show a larger change in the ABC-IR. Similar, although not statistically significant results were seen with ABC-SA, CGI-I, and CGI-S changes, in that more severe subjects had greater responses to placebo. ▼ Despite the available literature addressing the placebo effect regarding performance, there is a paucity of research addressing the possibility of a placebo effect both within and between bouts of repeated sprint performance. Therefore, the purpose of this study was to determine if the administration of a placebo influences recovery during and between sessions of intermittent sprinting. Ten sprint-trained participants performed a series of repeated sprint tests under two different conditions in a counterbalanced order; one where they were administered a control beverage (i.e., water) and another with an experimental beverage (i.e., placebo). Each testing session consisted of three repeated anaerobic sprint tests (RAST) on a non-motorized treadmill with seven minutes of recovery between sets. Participants were then given 24-hours of recovery whereupon the repeated sprint test protocol was repeated. Ratings of perceived exertion were recorded within five seconds after each sprint. Following each RAST, participants were asked to rate their pain using a visual analog scale which was followed by a blood lactate sample drawn. Participants were then asked to rate their recovery using the perceived recovery status scale 30 seconds before beginning each subsequent RAST. Power was recorded throughout each session from a non-motorized treadmill in order to analyze changes in sprinting performance. Approximately 20 minutes following the final sprint, participants provided a session rating of perceived exertion to rate the global difficulty of the session. A 2x3 repeated measures ANOVA was used to determine significant differences in peak power, mean power, decrement score, recovery score, perceived recovery status, rating of perceived exertion, pain, and blood lactate between RASTs and conditions. Results from the analyses revealed no significant differences between conditions during the day one testing session. However, results showed the experimental condition produced significantly higher peak power (p < 0.001) and mean power (p = 0.002) versus the control condition in RAST3 of the day two session. These findings were observed in absence of any other significant difference in metabolic or perceptual strain (p > 0.05). In conclusion, it appears the administration of a placebo can attenuate the decline in performance as fatigue increases during repeated sprinting bouts. ▼ The current study was designed to examine the effects of uridine on depressive symptoms and neuropsychological test performance in depressed youth with bipolar disorder. Participants included 20 adolescents diagnosed with bipolar depression between the ages of 13 and 21. Additionally, this study sought to compare depressed bipolar youth to an age- and gender-matched unaffected comparison group. Therefore, 10 unaffected comparison adolescents also participated in the study. Recruitment of participants took place between 2014-2015 at a university-based psychiatric hospital. All youth and their families provided consent to participate in the study and were administered a diagnostic clinical interview. Mood was assessed weekly throughout the study using rating scales and neuropsychological tests were administered pre- and posttreatment. Bipolar participants were randomized to treatment with 500mg twice daily of uridine or placebo. Three participants were removed from the data analyses (2 in the uridine group and 1 in the placebo group) due to noncompliance. Results of the study demonstrated that uridine was tolerated and there were no serious adverse events. Three participants (33%) in the uridine vs. four participants (50%) in the placebo group showed reduced depression symptoms. However, there were no significant differences between the placebo and uridine groups on depression ratings from pre- to posttreament. Additionally, there were no differences in neurocognitive performance between the uridine or placebo groups from pre- to posttreatment. Importantly, treatment with uridine did not have any negative effects on neurocognitive performance; however, these results did not suggest any neuropsychological benefits either. Bipolar participants showed more impairment in attention, executive functioning, and processing speed than unaffected comparison participants at baseline. Academic experiences for bipolar participants were different from comparison participants and included more academic difficulties, discontinuations, and transfers to new schools and programs. Participants who reported academic difficulties or received special education services performed lower on attention, working memory, executive functioning, and processing speed tasks. Without more subjects, no firm conclusions can be drawn from the current findings; however, these findings suggest a need for further research into uridine treatment. Treatment of adolescent bipolar disorder is complex and treatments specifically designed for youth are needed.

▼ Mechanisms through which physiotherapy influence musculoskeletal pain include both the specific ingredient of an intervention as well as contextual factors inherent to clinical encounters including the therapist, patient and setting. These contextual factors are often termed “non-specific” effects and are associated with the placebo effect. Although well documented in other areas, the impact of contextual factors in treatment of low back pain (LBP) is unknown. In addition, the contributors to the physiotherapy placebo response in LBP have not been elucidated. This project investigated the effect of contextual factors in patients with LBP receiving either active or sham interferential current therapy (IFC). Determinants of a favorable response to the placebo response were also explored. A sample of 117 chronic LBP participants were randomly assigned into 4 groups: active limited (AL) included the application of active IFC in a limited therapeutic encounter (i.e. limited patient-practitioner interaction), sham limited (SL) included sham IFC in a limited therapeutic encounter, active enhanced (AE) included active IFC in an enhanced therapeutic encounter (i.e. supportive patient-practitioner relationship, encouragement), and sham enhanced (SE) included sham IFC in an enhanced therapeutic encounter. Outcomes included pain intensity (PI-NRS) and muscle pain sensitivity (PPT). Analysis included MANOVA, and logistic regression. Also, clinical significance was determined. There were statistically significant differences between groups on PPT’s and PI-NRS (baseline and after treatment). Mean differences in PI-NRS were 18.3 mm, 10.0 mm, 31.4 mm, and 22.2 mm, for the groups AL, SL, AE, and SE respectively. Clinically important effect sizes were found. Mean differences in PPT’s were 1.2 kg, 0.3 kg, 2.0 kg, and 1.7 kg for the group AL, SL, AE, and SE respectively. Again, clinically important effect sizes were found. The level of therapeutic alliance and pain duration were the factors associated with the placebo response. Results highlight the important role of contextual factors in the treatment of patients with chronic LBP. Enhanced therapeutic relationship was associated with meaningful clinical improvement. Also, perceived therapeutic alliance was associated with placebo response. Factors other than the specific ingredient of a treatment may have a large role in achieving positive clinical outcomes, and exploring them is central to physiotherapy practice.

▼ El efecto placebo ha sido ampliamente estudiado en el área médica dónde ha sido definido como una aparente mejora en la salud que no se debe a ningún tratamiento en particular, sino a la creencia del paciente de que mejorará o será curado (Borsook & Becerra, 2005). Este efecto ha sido utilizado en diversos ámbitos del área de la salud, como en la curación de heridos de guerra, la determinación de la efectividad de ciertos narcóticos, y como una manera legítima de tratar ciertas condiciones como el asma, cáncer, alergias, insomnio, entre otras (Crow, 2006). El efecto placebo ha sido parte del espectro de estudios e investigaciones del Marketing. Shiv, Carmon y Ariely (2005) demuestran que el precio que los consumidores pagan por una bebida energética no sólo afecta el beneficio percibido del producto, sino que también afecta la eficacia de éste. En otras palabras, aumentando el precio del producto, dejando todo lo demás constante, podría incrementar la eficacia del mismo. Es interesante notar la diferencia del efecto placebo típicamente estudiado, el cual tiene como frontera de análisis el área perceptual de los individuos, (Ej: Vino más caro, es percibido de mejor calidad que uno más barato), con el presente caso, donde se analiza su impacto en el comportamiento de los individuos. La variable dependiente del estudio es una medida de desempeño cognitivo, la cual actúa como medición de la eficacia de las bebidas energéticas según el “claim” transversal en toda la categoría, el cual se asocia con la mejora del desempeño mental, cognitivo, estado de alerta, vigilia, entre otros. En la presente investigación, se replica el experimento de Ariely y colegas (2005) con un nivel adicional de precios y se estudia la mediación de variables como la actitud hacia la marca y el rol de las expectativas de respuesta en relación al desempeño del producto utilizado. El autor del estudio encuentra evidencia empírica para un cumplimiento parcial de un efecto placebo negativo o no deseado en una de las condiciones particulares del diseño experimental factorial. La investigación adiciona datos empíricos acerca del efecto placebo en una muestra altamente familiarizada con el producto y que corresponde a un segmento importante en las estrategias de marketing de las compañías involucradas en esta categoría de producto. Se discuten además, implicancias estratégicas y éticas para el marketing que aportan al debate de las decisiones de precio en una industria que utiliza esta variable como señal de calidad y reflejo de status por parte de sus productos comercializados.

▼ The purpose of the project was to better understand sources of learning that lead to placebo nausea, with the aim of developing interventions with clinical utility. After introducing theory regarding placebo effects and nausea in Chapter 1, Chapter 2 systematically reviewed studies that had previously attempted to manipulate nausea via the placebo effect. This review revealed that there were some features of interventions associated with success, but that results were often conflicting and a better paradigm to model placebo nausea in healthy participants was required. Chapter 3 introduced such a paradigm, illustrating how Galvanic Vestibular Stimulation (GVS) could be used to administer both placebo and nauseating stimulation under the guise of an experiment looking at the effects of GVS on spatial ability. Chapter 4 presented a study that used this paradigm to explore the development of placebo-induced relief from nausea through instruction, conditioning, and their combination. Both sources of learning reduced nausea on test relative to controls. Chapter 5 refocused on the problem of nausea that is incidentally conditioned in the treatment context. A model of conditioned nausea was tested, and it appeared that conditioning had developed and had also generalized perfectly well to a new test context. This finding was taken as evidence that the GVS device had overshadowed any context-nausea learning, and was used to inform the development of a new latent inhibition paradigm tested in Chapters 6 and 7. Chapter 6 pre-exposed these GVS-related reactive cues in the form of placebo stimulation in an attempt to retard conditioning through latent inhibition. Here, pre-exposure reduced conditioned nausea to the placebo on test relative to a group who did not receive pre-exposure, and had reduced nausea to the level of a control group who received no conditioning. Given that the applicability of any such intervention to applied settings rests on its ethicality, in a novel extension, Chapter 7 tested whether this latent inhibition effect required deception. This study replicated the effect of pre-exposure observed in Chapter 6, as well as finding that latent inhibition occurred to the same extent in a group fully informed as to the nature of the pre-exposure phase. Together, these findings suggest that placebo effects can occur after both instruction and conditioning, and that taking advantage of placebo effects in the clinic could offer an important method of intervening to reduce nausea.

▼ The majority of research on the placebo effect has focused on beneficial effects in patients or participants told to expect an active treatment, but who are actually given a placebo. Two important and relatively understudied aspects of the placebo effect are the extent to which expectancies influence outcomes in double-blind randomised placebo-controlled trials (RCTs) and whether the placebo effect contributes to treatment side effects. The current project investigated these two issues in both clinical and experimental settings. The first study involved reanalysing a double-blind RCT of naltrexone and acamprosate for alcohol dependence based on whether participants believed they had been allocated to receive active treatment or placebo (perceived treatment). The second study extended on this by developing an experimental model for these effects using dummy (placebo only) double-blind RCTs for cognitive performance. This allowed for the manipulation of observable changes in the form of false feedback. The third study investigated whether warning participants about side effects increases their occurrence, frequency, and/or severity in three dummy trials for sleep difficulty in healthy volunteers. The final study complemented this by examining whether first time chemotherapy patients’ expectancies for nausea were associated with their post-chemotherapy nausea. The studies on perceived treatment in double-blind RCTs indicated that participants’ beliefs about their treatment allocation can influence their actual treatment outcomes via the placebo effect and that these beliefs are affected by the feedback they receive about their performance. The studies on placebo-induced side effects indicated that the placebo effect may contribute to treatment side effects but that this effect is generally likely to be small. These findings confirm that the placebo effect can influence treatment outcomes and emphasise the importance of considering patient expectancies when delivering medical treatment. They also highlight some general limitations associated with research on the placebo effect, which include, whether conveying uncertainty undermines the placebo effect and whether measuring or manipulating expectancies is the best way to evaluate the placebo effect.

▼ Background: Tennis elbow (lateral epicondylitis) is a common condition with a community prevalence of 1-3%, resulting in pain at the elbow and weakness extending the wrist. It is associated with overuse and if it progresses to a chronic stage, tennis elbow shows both macroscopic and microscopic degeneration at the origin of the extensor carpi radialis brevis (ECRB). While there is no universally effective management for chronic tennis elbow, a common surgical technique (Nirschl & Pettrone. J Bone Joint Surg Am, 61(6A): 832-839) involves cutting out the degenerated portion of the ECRB. The results of this technique have been reported as excellent, yet no surgical procedure for tennis elbow has been compared with placebo surgery.Methods: This study was a prospective, randomised, double-blinded, placebo controlled clinical trial investigating the Nirschl technique (surgical excision of the macroscopically degenerated portion of ECRB; n=11) compared with a sham operation (skin incision and exposure of ECRB alone; n=11) to treat chronic tennis elbow. The primary outcome measure was defined as patient rated elbow pain with activity at 6 months post-surgery. Secondary outcome measures included other patient rated pain and functional outcomes, elbow stiffness and range of motion, epicondyle tenderness and strength measurements.Results: The two groups were matched for age, gender and duration of symptoms. Both the Nirschl and sham procedures improved patient rated pain frequency and severity, elbow stiffness, difficulty with picking up objects and twisting motions and grip strength over 6 months (p<0.01). The only difference observed between the groups was that patients who underwent the Nirschl procedure for tennis elbow had significantly more pain with activity at 2 weeks, when compared with sham surgery alone (p<0.05). No side effects or complications were reported.Conclusion: This pilot study indicates that, in the short term, surgical excision of the degenerative portion of ECRB confers no additional benefits to patients with chronic tennis elbow over and above a skin incision alone. ▼ In the current climate of patient-centered care, an increasing amount of responsibility is being placed on patients to make health decisions that optimize their outcomes. Patients may vary on the amount of control they prefer to exert in a decision context. Factors such as locus of control and cultural background may influence this desire for control over treatment options. Individuals from Western, independent cultural contexts may be more amenable to the idea of making medical decisions and benefit more from this control while those from Eastern, interdependent contexts may not benefit as much from pressure to select a treatment. To aid in the decision-making process, people across cultures often turn to others for advice. However, the benefit of this advice can vary depending on a myriad of factors including the interpersonal closeness between decision-maker and their advisor. For example, an individual consulting their best friend or significant other may find themselves with better treatment outcomes than an individual seeking information from impersonal webpages or online support groups. Further, individuals from disparate cultural contexts may find more or less benefit from consultations with close or distant others, perhaps due to differing social and familial orientations and goals. The goal of the current study was to better understand how interpersonal closeness and culture interact to affect an individual’s experience of treatment decision-making and ultimately their treatment outcomes. Results demonstrate moderate support for the hypotheses; individuals who made a treatment selection after discussing their options with a close other showed better treatment outcomes than distant or no advisor groups. Additionally, the Japanese participants showed a mediation effect such that individuals who experienced the decision process negatively (e.g., rated making a treatment selection as “unpleasant”) subsequently had worse treatment outcomes. Unexpectedly, the influence of advisor condition did not differ across culture and the aforementioned effects of advisor condition were not moderated by variables anticipated to assess the desire for decisional control. Explanations and implications for these results are discussed. These findings shed light on the treatment decision-making process and how factors of the decision environment may impact treatment outcomes. Les antidépresseurs sont les traitements pharmacologiques de choix de la dépression. Pourtant devant un nombre non négligeable d’études « négatives » et l’absence d’efficacité dans les dépressions peu sévères, la question de leur utilité reste controversée. Cette controverse présuppose que l’efficacité des antidépresseurs se mesure facilement. Pourtant, comme dans toute expérience, le dispositif expérimental peut influencer le résultat mis en évidence. L’objectif de cette thèse est justement d’explorer l’incidence de la procédure expérimentale sur l'effet mesuré des pharmacothérapies antidépressives. Est-ce que leur efficacité mesurée dans les études randomisées reflète leur effectivité (dans la « vraie vie ») ? Pour répondre à cette question, nous avons conduit une méta-analyse d’un large panel d’études de méthodologies très différentes. Une méta-régression a été réalisée pour modéliser la réponse au traitement en fonction des caractéristiques des études. Elle a permis de mettre en évidence 1/ une réponse supérieure dans les essais randomisés par rapport aux études observationnelles (possiblement plus représentatives de la « vraie vie »); 2/ une variation de la réponse en fonction de la durée de l’étude, du nombre de visites de suivi, de la population de patients inclus, du type d’analyse choisie, de la possibilité d’avoir un placebo et du design en aveugle. Une revue de la littérature apporte ensuite une perspective qualitative à la question posée. L’objectif était de décrire les dispositifs expérimentaux actuellement utilisés ou envisageables dans l’évaluation des antidépresseurs. La revue de la littérature 1/ a permis de préciser les limites des études actuelles en termes de validité interne (mesure des critères de jugement, attrition, levée de l’aveugle importance de la réponse placebo) et externe (difficultés majeures de généralisations des études); 2/ conduit à proposer une évaluation multimodale des antidépresseurs dans laquelle les études observationnelles auraient toute leur place. La question de la mesure est ensuite abordée à travers des simulations. Après avoir exploré l’impact du dispositif expérimental au niveau de l’essai en soi, le phénomène est analysé à un niveau plus global. Le choix a été fait d’un raisonnement par l’absurde, sur le terrain de la « comparative effectiveness ». L’objectif était de comparer les placebos d’antidépresseurs grâce à une méta-analyse en réseau. Ces comparaisons indirectes n’ont pas permis de mettre en évidence de différence entre placebos d’antidépresseurs. Un biais de publication a pu être mis en évidence et empêche de conclure formellement sur un résultat pourtant évident. Ce résultat ouvre une discussion sur le degré de confiance que l’on peut apporter à ce que l’on considère généralement comme des preuves scientifiques de l’effectivité de nos pratiques. Nos résultats impliquent un nouvel éclairage à la controverse sur les antidépresseurs en replaçant le débat sur le terrain épistémologique.
