O pericárdio de mamíferos consiste de uma membrana serosa e um saco fibroso. A serosa cobre a porção externa do coração (pericárdio visceral) e margeia a parte interna do saco fibroso (pericárdio parietal). Funções como transporte de fluido e partículas, participação em reações inflamatórias, reparo tecidual, proteção contra microrganismos e disseminação tumoral têm sido atribuídas a esta estrutura. O espaço entre o pericárdio visceral e parietal contém o fluido pericárdico que serve como um lubrificante e desempenha muitas funções locais. A composição deste fluido é considerada como um ultrafiltrado do plasma, enriquecido em moléculas como prostaglandinas, fator natriurético atrial, fatores de crescimento e angiotensina II (Ang II) entre outras. A visão clássica da Ang II como uma molécula vasoativa que participa da regulação hemodinâmica sistêmica e local tem se expandido. Este peptídeo, hoje em dia, é considerado como uma citocina, desempenhando funções na patologia cardiovascular, com efeitos pró-fibróticos e pró-inflamatórios. Estudos têm demonstrado que além da enzima conversora de angiotensina (ECA), outras enzimas são capazes de gerar Ang II, dentre elas a quimase, cujo principal sítio de produção e estocagem são os mastócitos. Assim, os objetivos deste trabalho foram demonstrar a presença tanto de ECA quanto de quimase no fluido pericárdico bovino, quantificar os mastócitos e imunolocalizar ambas as enzimas na membrana pericárdica de boi, verificando assim seus possíveis sítios de síntese. Os resultados obtidos permitiram a identificação de uma isoforma de ECA no fluido pericárdico, com massa molecular de aproximadamente 146 kDa, após a imunorreação sobre membrana de nitrocelulose. Nas secções de membrana pericárdica, a ECA foi identificada ao longo da camada mesotelial, sugerindo ser esta a fonte de produção de parte da ECA existente no fluido pericárdico. Em relação à quimase, enzima pertencente às vias alternativas para produção de Ang II, não foi possível detectar a presença desta enzima em amostras deste fluido. Através das reações histoquímicas, os mastócitos foram identificados e quantificados na membrana pericárdica. A densidade média foi de 43,7714,73 mastócitos/mm por animal. Em relação à integridade destas células, a densidade de mastócitos degranulados foi maior que a densidade de mastócitos granulados, sendo esta diferença estatisticamente significante. Finalmente, a imunodetecção de quimase, através da microscopia de fluorescência, permitiu, pela primeira vez, a identificação de mastócitos quimase-positivos em secções de tecido bovino, embora em quantidade reduzida.

Acute myocardial infarction is one of the most common causes of death worldwide. It is defined as myocardial necrosis segment due to reduction or cessation of blood flow. The majority of cases with acute myocardial infarction are obligated to coronary artery disease or complications of atherosclerosis. In forensic science, except macroscopic and histopathological studies, there is a need to assess cardiac biomarkers in order to consolidate and to become more specific their usefulness in postmortem diagnosis of acute myocardial infarction. Specifically, the cardiac troponin I is detectable in pericardial fluid before becames detectable in serum, because she reaches the pericardial fluid through passive diffusion and ultrafiltration resulting from the pressure gradient. Furthermore, the levels of cTnI remain elevated for 6 days after the onset of clinical symptoms of acute myocardial infarction. The aim of this study is to compare the levels of post mortem concentration of cTnI in the pericardial fluid between groups of cases due to various causes of death, especially among acute myocardial infarction, drowning, cases with causes of death related to lung diseases and other causes of death, except the above cases. . The sample included medicolegal autopsy cases from the Department of Forensic Medicine and Toxicology, Medical Faculty of Athens University. For each case were recorded sex, age, postmortem interval and cardiac troponin I levels were measured by enzyme immunoassay technique (ELISA). For the statistical analysis of the data, the SPSS 11.0 program was used.In the group of cases with acute myocardial infarction it was observed statistically significant differences regarding the mean value of cTnI when compared to the other groups. There were no statistically significant correlations between genders, regardless of the cause of death. Also, no statistically significant differences were observed between mean value of cTnI and the postmortem interval. Furthermore, we observed that the cTnI values peak between the ages of 55 and 74 years. For ages over 75, cTnI mean value seems to be decreased but does not approximate the levels of cTnI in the ages under 55. In conclusion, cTnI is a cardiac marker of great importance in postmortem diagnosis of acute myocardial infarction and should be used assistantly macroscopic and histopathological studies.

Το οξύ έμφραγμα του μυοκαρδίου αποτελεί μία από τις συχνότερες αιτίες θανάτου σε παγκόσμια κλίμακα. Ορίζεται ως νέκρωση τμήματος του μυοκαρδίου λόγω μείωσης ή διακοπής της αιμάτωσής του. Η πλειοψηφία των οξέων εμφραγμάτων του μυοκαρδίου οφείλεται στη στεφανιαία νόσο ή σε επιπλοκές της αθηρωμάτωσης.Στην ιατροδικαστική επιστήμη, πλην της μακροσκοπικής και της ιστοπαθολογικής μελέτης, καθίσταται αναγκαία η αξιολόγηση καρδιακών βιοχημικών δεικτών, προκειμένου να εδραιωθεί και να γίνει πιο συγκεκριμένη η χρησιμότητά τους στη μεταθανάτια διάγνωση του οξέος εμφράγματος του μυοκαρδίου. The investigation of drowning constitutes one of the biggest problems in forensic practice. Elevated cardiac troponin I (cTnI) levels in biological fluids have been associated with myocardial damage, whereas increased Mg2+ and Ca2+ levels were found in cases of saltwater drowning. The aim of this study was to examine the diagnostic utility of postmortem determination of cTnI, Mg2+ and Ca2+ in the pericardial fluid, in differentiating between cases of saltwater drowning related to myocardial injury and those brought about by other causes. This study included 76 cases selected during a 2-year period from medicolegal autopsies. The cases were divided into three groups, according to the cause of death established based on macroscopic and microscopic evidence. The groups were: 1) saltwater drowning without myocardial infarction (n = 23), 2) saltwater drowning with histological evidence of myocardial infarction (n = 28), and 3) myocardial infarction unrelated to drowning (n = 25). cTnI was determined with an enzyme immunoassay; Mg2+ and Ca2+ with standard colorimetric assays. Pericardial cTnI levels were significantly lower in group 1 compared to groups 2 and 3. In contrast, pericardial Mg2+ and Ca2+ levels were both significantly higher in groups 1 and 2 compared to group 3. Our results suggest that the postmortem determination of pericardial cTnI levels may be useful in detecting previous myocardial damage as a contributory factor in death from saltwater drowning and provide independent confirmation of the usefulness of pericardial Mg2+ and Ca2+ levels for differentiating between saltwater drowning and fatal acute myocardial injury unrelated to the former.

Η ιατροδικαστική διερεύνηση του πνιγμού αποτελεί ένα από τα μεγαλύτερα προβλήματα στην ιατροδικαστική πράξη. Τα αυξημένα επίπεδα της καρδιακής τροπονίνης Ι (cTnI) στα βιολογικά υγρά έχουν συσχετισθεί με μυοκαρδιακή βλάβη, ενώ τα επίπεδα μαγνησίου (Mg2+) και ασβεστίου (Ca2+) βρέθηκαν αυξημένα σε περιπτώσεις πνιγμού σε θαλάσσιο ύδωρ. Ο σκοπός της παρούσας μελέτης ήταν να εξετάσει τη διαγνωστική χρησιμότητα των επιπέδων cTnI, Mg2+ και Ca2+ που προσδιορίζονται μεταθανάτια στο περικαρδιακό υγρό, για τη διαφοροποίηση μεταξύ των περιπτώσεων πνιγμού σε θαλάσσιο ύδωρ που σχετίζονται με μυοκαρδιακή βλάβη και εκείνων που προκαλούνται από άλλες αιτίες. Η μελέτη περιέκλειε 76 περιστατικά που συλλέχθηκαν κατά τη διάρκεια μιας περιόδου δύο ετών και εξετάστηκαν στο Εργαστήριο Ιατροδικαστικής και Τοξικολογίας της Ιατρικής Σχολής του Αριστοτελείου Πανεπιστημίου Θεσσαλονίκης. Τα περιστατικά χωρίστηκαν σε τρεις ομάδες, με βάση την αιτία θανάτου όπως αυτή διαπιστώθηκε μακροσκοπικά και μικροσκοπικά.
