pain intensity and quality during tooth
translation by 2 continuous stresses. Eight subjects (five males, 3 females) who required
maxillary first premolar extractions had maxillary canines retracted segmentally using 4 kPa
on one side and 78 kPa on the other. Subjects scored Modified McGill
Pain Questionnaire-
Short Forms (MMPQ-SF), Visual Analogue Scales (VAS), and Present
Pain Intensities (PPI)
for both sides at the beginning of 13 appointments during 4 phases: baseline, post-placement
of separators, early and later tooth-loading.
Pain intensity (MMPQ-SF, VAS, PPI) and generalized/emotional subscale scores
showed no significant differences between stresses. Localized subscale scores were higher
for 78 kPa compared to 4 kPa sides. Females tended to report higher VAS and PPI compared
to males. Significant differences were found between baseline and post-placement of
separators and between baseline and early tooth-loading using MMPQ-SF and localized
subscale scores.

▼ The focus of this study is the role of the Radiation Therapist (RTT) in the assessment of pain in
cancer patients. The study was carried out at a Radiotherapy Department of a large Teaching
Hospital in Ghana and addressed the following research questions; 1) What is the role of the
RTT in the assessment of pain in cancer patients, 2) Why should the RTTs’ role be extended to
include pain assessment, 3) What are the challenges for the RTT when taking on the role of
pain assessment in radiation oncology and 4) How can pain assessment become a routine role
for the RTT in a busy radiation oncology department? 5) How would this extended role of the
RTT assist management of patient?
This study was conducted because many cancer patients suffer pain and to many, it can be
more debilitating than the primary disease itself. The RTTs who are involved in the daily
management of cancer patients during their radiation treatment can find it stressful to witness
their patients going through such pain particularly when they do not have a role in the
management of pain. In Ghana, there are few radiation oncologists (ROs) and therefore an
extended scope for RTTs, that includes pain assessment and a meaningful contribution to the
management of their patients’ pain, would be advantageous to all.
A mixed method research approach was adopted for gathering quantitative and qualitative data.
This included data collection of; interview, observation and review of existing document. A
pain questionnaire SF-MPQ-2 by Melzack (2009) was adapted as a tool for assessing pain in
the study participants.

▼ BACKGROUND: Chronic
pain is a debilitating disease that affects millions of adults in the United States. In recent years, there has been a steady increase in the use of interdisciplinary interventions to treat chronic
pain. While the effectiveness of such programs is well-documented in adult populations, little is known about the efficacy of such methods for elderly populations.
OBJECTIVE: This study assessed the effectiveness of an interdisciplinary
pain management program in addressing chronic
pain and associated disability (e.g., loss of functioning, poor mood) in the elderly. Study aims also included a comparison of outcomes between elderly and non-elderly participants in an interdisciplinary
pain management program, as well as an examination of which variables may serve as predictors for completion of such programs.
METHOD: Participants included one hundred and thirteen chronic
pain patients receiving treatment in an interdisciplinary program at the Eugene McDermott Center for
Pain Management. Participants completed multiple measures pertaining to
pain and associated conditions at baseline, mid-way through the program (2 weeks post-enrollment) and upon completion of the program (4 weeks post-enrollment).
ANALYSIS: Repeated measures ANOVA analyses examined changes in participants’ scores over time on various self-report measures that assessed
pain rating, functionality and mood. Separate analyses were conducted to evaluate results from elderly participants only, and then to compare results between elderly and non-elderly participants. A binomial logistic regression was performed to determine the effects of age and mood on the likelihood that participants in an interdisciplinary
pain management program would complete treatment in the program.
RESULTS: Although elderly participants’ mean scores across a variety of measures showed signs of improvement, these changes did not meet statistical significance. However, when examining elderly and non-elderly participants together, both groups demonstrated significant improvement across all outcomes with the exception of depression. There were group differences on a select number of outcomes, including those that assessed
pain disability, anxiety and depression levels; on these measures, the mean scores for non-elderly participants were higher as compared to elderly participants. There were no interactions between the two groups for any of the variables assessed. The logistic regression model that examined the effects of age and mood on the likelihood that participants would complete treatment in the current study was not significant.
DISCUSSION: The current study demonstrates that non-elderly participants experience benefit from participation in interdisciplinary
pain management programs. Further research is warranted with a larger elderly population to assess how treatment may need to be tailored to the unique needs of the elderly.

▼ Introduction: Complex regional pain syndrome (CRPS) is a form of neuropathic
pain that sometimes develops after trauma or surgery. While diagnostic criteria
have been debated, there is agreement participation in rehabilitation should be
the primary management. However, there are gaps in the evidence guiding
assessment and treatment choices for individuals with CRPS. The purpose of
this thesis was to advance the rehabilitation of CRPS by 1) ongoing development
and refinement of evaluations for the specific symptoms of CRPS, and 2) to
investigate effectiveness of a new treatment (somatosensory rehabilitation)
posited to address allodynia associated with CRPS.
Methods: We conducted a series of 4 studies addressing various aspects of
CRPS assessment and the somatosensory rehabilitation method: a) a cognitive
debriefing study for content validation of the Patient-Reported Hamilton Inventory
for CRPS; b) English translation and cultural validation of the Radboud
Evaluation of Sensitivity; c) a retrospective cohort study of the effectiveness of
somatosensory rehabilitation for allodynia in the upper limb; and d) a pilot study
of the somatosensory rehabilitation method to consider the measurement
properties of the embedded evaluation tools of allodynography and the rainbow
pain scale, and to provide estimates for future controlled trials of effectiveness.
Results: The cognitive debriefing study identified potentially problematic items,
and constructs which needed enhancement in future versions of the PR-HICRPS
assessment. The second paper reported the translation and cultural
validation of the RES-E, finding support for test-retest reliability, internal
consistency, and preliminary evidence for construct validity and reproducibility.
The third paper presented preliminary evidence of a strong effect size for the
SRM in an uncontrolled consecutive cohort. Finally, the fourth paper provides an
interm analysis of the psychometric properties of allodynography and the rainbow
pain scale, and estimates large sample sizes will be required for future trials.
Discussion and Conclusion: None of the assessment tools described herein is ready for unrestricted use in clinical practice or research. Although the effect size estimates for somatosensory rehabilitation from the retrospective cohort are encouraging, the incomplete pilot data suggests large, multi-site trials and careful selection of the primary outcome measures will be required for future, rigorous trials of this method.

Midwives are responsible to assess pain before treating it; then they should reassess the pain to evaluate the effectiveness of the interventions and plan future therapy. Accurate and objective measures of labour pain continue to be scarce and the discrepancy in labour pain perceptions between parturients and health-care providers remains challenging. Various pain measurement tools are currently in use measuring chronic and acute pain, but many problems were encountered applying these methods to the woman in labour. The charts were detailed and required too much time to complete. The aim of the study was to develop a multidimensional labour pain assessment instrument to assist clinicians and midwives with labour pain control. Objectives for developing a valid and reliable instrument to accurately measure labour pain included refinement of the labour pain assessment instrument developed from literature, testing of the refined instrument on patients during labour, compilation of a final instrument and development of guidelines on how to implement the labour pain assessment instrument in nursing practice. A descriptive and exploratory approach was used to describe, evaluate, observe, explore and assemble new knowledge on assessment and measurement of pain during the first stage of labour. Development of the instrument was addressed in the literature chapter. The research was conducted in three phases, combining qualitative and quantitative research.During phase one, the instrument was refined in two stages. Focus group interviews were conducted with members regarded as knowledgeable in the field of normal labour who evaluated the face and content validity of the instrument. The most senior people teaching Midwifery at all South African universities evaluated the face and content validity of the instrument with an open-ended questionnaire thereafter, using the Delphi technique. The analysis of the first Delphi round was compared to the focus group analysis. The instrument was altered and submitted to the Delphi experts in a second open-ended questionnaire to confirm the alterations.

▼ Vulvar vestibulitis syndrome is a highly prevalent and under-investigated pain syndrome. It is believed to be the most common form of dyspareunia, or painful intercourse, in pre-menopausal women. Despite the fact that pain is its defining characteristic, its conceptualization as a sexual dysfunction remains the predominant view. The first chapter of this thesis reviews support for a re-conceptualization of vulvar vestibulitis as a pain disorder, and contends that the pain component must be measured as it is in chronic pain patients, both objectively and subjectively. This paper is followed by a study in which tactile and pain thresholds in genital and non-genital body areas of 13 women with vulvar vestibulitis syndrome and 13 non-affected women were measured, while data about the intensity and unpleasantness of these sensations were collected. Results indicated that women with vulvar vestibulitis have decreased tactile and pain thresholds in both genital and non-genital body areas, as well as higher unpleasantness ratings in response to painful stimuli than controls, replicating data from the chronic pain literature. The third paper presents a pain conceptualization of vulvar vestibulitis according to classification criteria used by the International Association for the Study of Pain, and introduces the development of a vulvalgesiometer, a standardized method of genital pain measurement. Data collected with the vulvalgesiometer are presented in the fourth paper, a study in which 14 women with vulvar vestibulitis and 14 control women participated. Women with vulvar vestibulitis had significantly lower pain thresholds, higher pain unpleasantness ratings, and used adjectives similar to those reported during intercourse in response to painful stimulation. The vulvalgesiometer was used to elicit genital tactile and pain sensation in the two groups while brain activation patterns were measured via functional magnetic rdsonance imaging, the focus of the fifth paper. Results demonstrated that women with vulvar vestibulitis have augmentations of brain activity in the processing of both painful and non-painful genital stimulation as compared with non-affected women. Overall, findings from these studies suggest that the pain of vulvar vestibulitis can be reliably measured and that the pain perspective has important implications for both research and treatment of urogenital pain problems.

▼ O objetivo geral deste estudo foi avaliar a dor crônica advinda de diferentes tipos de cânceres. Os objetivos especÃficos foram descrever as caracterÃsticas sociais dos participantes, caracterizar a dor percebida por meio de indicadores especÃficos e identificar os 10 descritores de dor crônica de maior e menor atribuição conforme a percepção da amostra. Participaram do estudo 45 mulheres com câncer de mama, 45 homens com câncer de próstata e 60 indivÃduos de ambos os sexos com câncer de sistema digestivo. Para a mensuração da dor foram utilizados os 50 descritores de dor crônica que compõem a Escala Multidimensional de Avaliação de Dor (EMADOR), utilizando-se o método psicofÃsico de estimação de categorias. Os resultados da caracterização social das 3 amostras foram: a média de idade foi de 60 anos, 54% eram do gênero masculino, 58,7% casados, 65,3% pertencentes à religião católica e 50,6% tinham ensino fundamental completo. Na caracterização da dor foi evidenciado que, referente ao sÃtio primário do tumor, 30% estavam localizados na mama, 30% na próstata e 14,7% no estômago. O inÃcio da queixa dolorosa ocorreu em 40,7% dos participantes antes do diagnóstico da doença, 52,7% relataram somente 1 sÃtio de dor e 55,4% relataram que a mesma era intermitente. A intensidade da dor atribuÃda nas 3 amostras, considerando a média aritmética, demonstrou que os participantes com tumores de sistema digestivo reportaram os maiores escores (7,58 ± 2,59) e 26,6% desta amostra relataram escores acima de 8 para todos os descritores apresentados. Na avaliação dos descritores 10 descritores de maior atribuição foi observado que, 5 estavam presentes nas 3 amostras: -dolorosa?, -desconfortável?, -chata?, -desagradável? e o descritor -incômoda?, o qual foi o mais atribuÃdo tanto pelos participantes com câncer de sistema digestivo quanto pelos participantes com câncer de próstata, demonstrando a similaridade da linguagem da dor nos diferentes grupos. Sobre a dimensão dos descritores de maior atribuição nas 3 amostras houve predomÃnio de descritores de dor crônica que caracterizaram a dimensão afetiva (50%), seguido pelos de dimensão cognitiva (26,6%) e pelos de dimensão sensitiva (23,3%). ConcluÃmos que, mesmo sendo a dor oncológica um fenômeno considerado individual, multidimensional com comprometimento em diversos domÃnios da vida, a sua linguagem apresenta semelhança independente do sÃtio do tumor e do gênero e a dimensão afetiva da dor deve ser melhor explorada nas avaliações clÃnicas.

▼ Bakgrund: Studier visar att sjuksköterskor ofta underskattar barns smärta. Vid omvÃ¥rdnad av barn med smärta bör självskattning vara förstahandsvalet vid smärtskattning. Smärtskattning och dokumentation är sjuksköterskans ansvar, därför behöver sjuksköterskor utbildning och riktlinjer för att förbättra smärtbehandlingen för barn och ungdomar. Syfte: Syftet var att kartlägga sjuksköterskors attityder och kunskaper gällande smärta hos barn och ungdomar pÃ¥ en pediatrisk akutvÃ¥rdsavdelning. Ytterligare ett syfte var att se om en intervention kan pÃ¥verka sjuksköterskornas attityder och kunskaper samt smärtskattningsfrekvens. Metod/design: En kvantitativ kvasiexperimentell studie utfördes. En enkät besvarades av 30 sjuksköterskor före och efter en intervention, vilken innebar ett deltagande i en utbildning om barns smärta och användning av smärtskattningsinstrument. Vid utbildningen utformades även riktlinjer gällande smärtskattning pÃ¥ avdelningen. Resultat: När användandet av smärtskattning värderades i en femgradig betygsskala fick sjuksköterskorna högre medelbetyg efter interventionen jämfört med innan (1,69 vs 2,5; p=0,049). Ingen signifikant förändring kunde pÃ¥visas gällande sjuksköterskornas attityder och kunskaper. Slutsats: Smärtskattningsfrekvensen blev signifikant högre efter en intervention med evidensbaserat utbildningsmaterial och utformning av riktlinjer för smärtskattning. Ingen skillnad hittades mellan bakgrundsfaktorer och attityd- och kunskapsformuläret. Sjuksköterskorna pÃ¥ avdelningen behöver fortsätta arbeta för att riktlinjerna kring smärtskattning följs och uppdateras utifrÃ¥n forskning och kliniska erfarenheter.
Background: Studies show that nurses often underestimate children´s pain. Self-report should be the standard when caring for a child with pain. Pain measurement and documentation is the nurse´s responsibility, therefore pediatric nurses need education and established guidelines to improve pediatric pain management. Aim: The aim was to survey nurse`s knowledge and attitudes regarding pain in children and adolescents in an pediatric ward. Another aim was to investigate if an intervention could affect nurse’s knowledge and attitudes towards children`s pain, as well as the frequency of pain measurement. Method: A quantitative and quasi-experimental study was conducted. A questionnaire was handed out to 30 nurses before and after an intervention regarding pain in children. During the intervention the nurses learned how to use validated pain measurement-tools and guidelines regarding pain measurement was formed. Results: The use of pain measurement-tools were evaluated in a five-point rating scale, the nurses got higher average points after the intervention than before (1.69 vs 2.5; p=0,049). No significant change was found regarding nurses' attitudes and knowledge. ▼ BAKGRUND: En av vÃ¥ra snabbast växande folksjukdomar i dagens samhälle är demens. Individer med demenssjukdom besitter olika hinder därav bl.a. kognitiv pÃ¥verkan eller kognitiv svikt, vilket i sig medför svÃ¥righeter av kommunikationsart. Detta medför vidare problematik kring identifieringen av smärtsymtom. SYFTE: Att undersöka hur sjuksköterskor bedömer smärta hos äldre personer med en diagnostiserad demenssjukdom. METOD: Litteraturstudien utformades som en litteraturöversikt över förekommande forskning där syfte och frÃ¥geställningar besvaras utifrÃ¥n resultat av tidigare vetenskapliga studier. Artikelsökningarna inriktades pÃ¥ de bibliografiska referensdatabaserna CINAHL samt PubMed där bÃ¥de kvantitativa och kvalitativa studier har granskats. RESULTAT: Tre huvudteman identifierades: Observation som instrument, självskattningsskalor som instrument samt sjuksköterskan som instrument. Observationsinstrumenten ADD, PACSLAC-D, PAINAD, DisDAT, NOPPAIN samt STI visade sig vara mest användbara för patienter med svÃ¥r demens. För de individer med mild till mÃ¥ttlig demens lämpar sig självskattningsinstrument som VAS, VRS och FPS. Utöver instrumenten krävs även att sjuksköterskan har kunskap om sÃ¥väl demens som individen bakom demensdiagnosen. Även riktlinjer, ett gott intraprofessionellt samarbete och anhörigas kunskap om individen är komponenter som har betydelse för att lindra smärta hos individer med demens. SLUTSATS: Konklusionen av resultatet visar att sjuksköterskans kunskap om demenssjukdom samt kunskap om individen bakom diagnosen utgör tillsammans med anhöriga en grund för att identifiera smärta.
BACKGROUND: One of our fastest growing endemic diseases in today's society is dementia. Individuals with dementia possess different series of obstacles including cognitive loss or cognitive impairment, which raises difficulties of communication. This causes further concerns relating to the relief of painful symptoms. OBJECTIVE: To investigate nurses' pain assessment in older people with diagnosed dementia. METHOD: The study was designed as a literature overview, research where the aim and questions answered on the basis of results of previous scientific studies. Searches for articles were focused on the bibliographic reference databases CINAHL and PubMed where both quantitative and qualitative studies have been reviewed. RESULTS: Three main themes were identified: Nurses knowledge as instruments, observation as an instrument and self- rating scales as instruments. Observation instruments ADD, PACSLAC -D, PAINAD, DisDAT, NOPPAIN and STI proved to be most useful for patients with severe dementia. For those individuals with mild to moderate dementia were self -report instrument such as VAS, VRS and FPS suitable. In addition to the instruments also requires that the nurse has knowledge of both dementia as the individual behind the dementia diagnosis. ▼ Det är känt att ambulanspersonalens omhändertagande av barns smärta kan bli bättre. Akut smärta hos barn är en vanlig orsak till att barn söker ambulanssjukvÃ¥rd. Barn i för- skoleÃ¥lder utgör en utmaning pÃ¥ grund av bristerna i den sprÃ¥kliga och kognitiva ut- vecklingen, som gör det svÃ¥rt för dem att kommunicera kring sin egen smärta. Att ge- nomföra en adekvat smärtskattning hos barn är ett sätt att mäta deras smärta och därige- nom utföra en lämplig behandling, samt kunna utvärdera effekten av genomförd be- handling. Idag är VAS det vanligaste smärtskattningsinstrument som används i ambu- lansen, det är endast validerat frÃ¥n barn > 8 Ã¥r. Syfte: Att undersöka hur smärtskattning av barn i Ã¥ldrarna tre till sex Ã¥r inom prehospital akutsjukvÃ¥rd genomförs och vilka för- utsättningar som finns för att genomföra smärtskattning. Studien är en integrativ littera- turstudie. Tio artiklar ingÃ¥r i studien, Ã¥tta kvantitativa och tvÃ¥ kvalitativa. Sökning efter vetenskapliga artiklar skedde i databaserna Pubmed, Cinahl och Embase. Artiklarna granskades med SBU:s granskningsmall. Resultatet visar pÃ¥ olika sätt hur ambulansper- sonal försöker förstÃ¥ barns smärta, men att det i dagsläget finns mer att önska. Smärt- skattning kan utföras oftare men kunskap och erfarenhet saknas om hur och varför man ska smärtskatta. Behandlingsriktlinjer räcker inte för att öka frekvensen samt förbättra utförandet av smärtskattning. I ambulansmiljö saknas det validerade instrument till barn i  förskoleÃ¥ldern. Förutsättningar för att använda validerade smärtskattningsinstrument visade sig vara kunskap och erfarenhet. BÃ¥de personalen och verksamheten har ett an- svar för att skapa förutsättningar till smärtskattnings. Det för att öka chansen till en hÃ¥llbar vÃ¥rdrelation.

▼ Bakgrund: Smärta är ett komplext och omfattande problem inom sjukvÃ¥rden. Utan adekvat behandling kan smärta leda till sämre livskvalitet för patienten. Sjuksköterskan har en viktig och betydelsefull roll i att identifiera och bedöma patientens smärta dÃ¥ en optimalt utförd smärtbedömning är en förutsättning för adekvat smärtlindring. Att se människan bakom patienten är en viktig aspekt för att identifiera smärta ur ett helhetsperspektiv. Syfte: Syftet var att undersöka sjuksköterskors bedömning av patienters smärta. Metod: En litteraturöversikt baserad pÃ¥ elva vetenskapliga artiklar, med bÃ¥de kvantitativ och kvalitativ design. Resultat: Resultatet presenteras i fyra olika huvudteman. Verbal kommunikation, Observation av patienters smärta med subteman: smärtbeteenden och fysiologiska tecken, Hjälpmedel för smärtbedömning och Sjuksköterskans förförstÃ¥else och tidigare erfarenheter av smärta. Inga evidensbaserade rutiner eller riktlinjer följdes. Diskussion: Resultatet diskuterades med anknytning till personcentrerad vÃ¥rd och konsensus begreppet människa. Smärtbedömningen utfördes oftast utifrÃ¥n sjuksköterskans egen tolkning och var inte alltid anpassad efter den enskilda individen. Patienter som har smärta är i behov av att bli sedda och smärtbedömda utifrÃ¥n deras individuella behov. Sjuksköterskor har därför en central roll i att individanpassa bedömningen av smärta.
Background: Pain is a complex and major problem in health care. Without adequate treatment, pain can lead to decreased quality of life for the patient. The nurse has an important and significant role in identifying and assessing the patient's pain when an optimally performed pain assessment is a prerequisite for adequate pain relief. To see the person behind the patient is a key aspect of identifying pain from a holistic perspective. Aim: The aim was to explore the nurses' assessment of pain in patients. Method: A literature review based on eleven scientific articles, with both quantitative and qualitative design. Results: The results are presented in four main themes. Verbal communication, Observation of patients' pain subthemes: behaviors related to pain and physiological signs, Pain assessment tools and Nurse's preunderstanding and past experiences of pain. No evidence-based rutines or guidelines were followed. Discussion: The results were discussed related to person-centered care and the concept of human. Pain assessment was usually carried out on the basis of the nurse's own interpretation and were not always tailored to the individual. Patients who have pain need to be seen and pain assessed on their individual needs. Nurses have therefore a major role to individualize the assessment of pain.

▼ Bakgrund: Studier visar att barns smärta underskattas av sjuksköterskor samtidigt som smärtskattningsskalor används i för liten utsträckning inom barnsjukvÃ¥rden.Syfte: Att undersöka hur smärtbedömning skedde pÃ¥ en postoperativ barnavdelning och hur smärtskattningsskalor användes samt barnens erfarenheter av smärtlindringen. Utöver det var syftet att undersöka hur barnens Ã¥lder pÃ¥verkade dessa faktorer. Metod: Studien är en kvantitativ tvärsnittsstudie baserad pÃ¥ ett enkätunderlag och ett konsekutivt urval. Enkäter delades ut postoperativt till 30 barn i Ã¥ldrarna 5-17 Ã¥r som genomgÃ¥tt olika typer av operationer. Barnen svarade pÃ¥ enkäterna själva eller med stöd av sina vÃ¥rdnadshavare. Resultat: Smärtskattningsskalorna Visuell Analog Skala (VAS) och Faces Pain Scale-revised (FPS-r) användes i mycket liten utsträckning (3% vardera) medan Numerisk Skala (NRS) användes i hög utsträckning (78%). Endast 20% av barnen fick information om nÃ¥gon smärtskattningsskala preoperativt. Barnen var generellt nöjda med hur ofta de hade blivit tillfrÃ¥gade om sin smärta (73%) och majoriteten (80%) tyckte att de alltid (43%) eller oftast (37%) fÃ¥tt tillräckligt med hjälp mot smärtan. Inga signifikanta skillnader kunde ses utifrÃ¥n Ã¥lder. Slutsats: Studiens resultat visar att NRS användes i hög utsträckning samt att barnen generellt var nöjda med hur ofta de blivit tillfrÃ¥gade om sin smärta och med hjälpen de fÃ¥tt mot smärtan. Resultatet visar ocksÃ¥ omrÃ¥den där förbättringar kan göras sÃ¥som den preoperativa informationen. Studien belyser det viktiga ämnet barns smärta ur barns perspektiv och kan fungera som ett underlag för att förbättra bedömning och behandling av barns smärta.
Background: Studies show that in pediatric care, nurses underestimate children's pain and perform pain assessments too seldom.Aim: To investigate how pain measurements were performed in a postoperative pediatric care unit, how pain assessment scales were employed, and how children experienced the pain relief. Furthermore the study aimed to investigate how children's age influenced the above mentioned factors. Method: A quantitative cross-sectional study based on a survey and a consecutive sample. 30 children, undergoing surgery, aged 5-17 answered questionnaires postoperatively. The children answered the questionnaires themselves or with support from their caretakers. Results: The pain assessment scales Visual Analoge Scale (VAS) and Faces Pain Scale-revised (FPS-r) were rarely used (3% each), while Numeric Rate Scale (NRS) was frequently used (78%). ▼ A Disease Itself: The Transformation of Pain After 1945By Stephen Beitler A Disease Itself examines a fundamental shift in the understanding and clinical status of pain in the United States after 1945. Pain was transformed from a simple, rote, well understood, and largely treatable event into a clinical syndrome defined by variability, chronicity, and complexity. Pain became subjective, rooted in individual experience, beliefs, and circumstances as much as in organic illness and injury. An insurgent model of pain refashioned the primary clinical challenge as chronic pain, asserted dense interconnections among pain’s psychological and physical dimensions, and advocated multidisciplinary treatment. Together, these changes refashioned how people experienced pain, how clinicians treated it, legislative actions, and the fortunes of dozens of pharmaceutical companies. They reflected as well substantial cultural shifts in ideas and practices of identity, selfhood, authority, and autonomy. A Disease Itself explores the history of how pain has been measured, managed, and organized professionally in order to track the origins and contours of this fundamental change. The dissertation begins with a historical review of the development of scientific, medical, and cultural understandings of pain from 1800 to 1945. It then examines the measurement, management, and professionalization of pain through three in-depth case studies. The first case study examines how a new model of pain was operationalized clinically in an assessment instrument, the McGill Pain Questionnaire (MPQ), published in 1975. The second explores how wider debates over risk and reward in treatment were reflected in enduring clinical ambiguities surrounding a highly successful pain reliever, Darvon, between its debut in 1957 and its removal from the market by the Food and Drug Administration (FDA) beginning in 2010. The third case study depicts how a new pain paradigm was central to the launch of International Association for the Study of Pain (IASP) after 1973. A Disease Itself draws on an extensive array of research studies on the MPQ, government and industry documents on Darvon, and the published works of early leaders of the IASP in its portrayal of a fundamental shift in the understanding and clinical status of pain.

▼ Bakgrund: svÃ¥righeter uppstÃ¥r med kommunikationen mellan patient och sjuksköterskan vid insjuknande i Alzheimers sjukdom vilket leder till hinder i smärtbedömning av personer med Alzheimers sjukdom jämfört med bedömningen avpersoner utan kognitiv nedsättning. Syfte: syftet med litteraturstudien var att belysa vikten av korrekt smärtbedömning hos patienter med Alzheimers sjukdom. Metod: studien var en litteraturstudie, 12 artiklar samtliga kvantitativa som svarade mot studiens syfte har granskats och analyserats. Resultat och konklusion: bedömning av smärta hos patienter med Alzheimers sjukdom är komplex, slutsatsen är tvetydig dÃ¥ resultatet visar dels att personer med Alzheimers sjukdom har mer smärta respektive mindre smärta än personer utan kognitiv nedsättning. Bedömningsinstrumenten som undersöktes i studien visade sig vara relevanta men ansvar och kunskap frÃ¥n sjukvÃ¥rdspersonalen som använder instrumentet var ett krav för att kunna utesluta felkällor av tecken pÃ¥ smärta sÃ¥ som onormala rörelser, beteende och ljud. Implikation: vidare forskning bör göras där fokus ligger pÃ¥ de enskilda diagnoserna istället för gruppen demenssjukdomar samt ökad kunskap hos sjukvÃ¥rdspersonal pÃ¥ smärta i relation till Alzheimers sjukdom.
Background: difficulties with communication is shown in the progress of Alzheimer’s disease which leads to a difference in assessment of pain in persons with Alzheimer’s disease compared to persons without a cognitive impairment. Aim of the study: the aim of the study was to illuminate the importance of correct pain assessment in patients with Alzheimer’s disease. Method: the study was a literature review, 12 quantitative articles which met the study’s purpose was reviewed and analyzed. Results and conclusions: pain assessment in patients with Alzheimer’s disease is complex, the conclusion is ambiguous since the results show that people with Alzheimer’s disease has more in some studies and less pain in others in comparison with people without a cognitive impairment. The pain assessment tools that were examined in this study were shown to be relevant, but it was important that the staff using the instruments had to be able to rule out confounders of pain such as unusual movements, behaviour and sounds. Implication: further research should focus on the the separate diagnoses that are included in the dementia genre. Also an increased knowledge and awareness in the staff about pain in correlation with Alzheimer’s disease.

▼ Objetivo: validar a Children’s and Infants’ Postoperative Pain Scale (CHIPPS) em crianças brasileiras de zero a 60 de meses idade. Pacientes e métodos: 100 crianças saudáveis de zero a 60 meses recebendo vacinação por via parenteral foram estudadas. A escala foi traduzida e testada para confiabilidade interobservador, consistência interna, validades de conteúdo, construção e critério concorrente. O comportamento das crianças foi gravado em vÃdeo antes, durante e após a vacinação na unidade básica de saúde. Três observadores independentes analisaram o comportamento através dos filmes. Resultados: a escala mostrou excelente confiabilidade interobservador (coeficiente de correlação intraclasse 0,89) e muito boa consistência interna com alfa de Cronbach de 0,86. A correlação positiva entre a CHIPPS e a MBPS (Modified Behavioral Pain Scale) confirma a validade de critério concorrente (coeficiente de Spearman 0,70 antes e 0,81 após a vacinação). A validade de construção foi determinada pela comparação dos escores de cada criança antes da vacinação (sem dor) e após o procedimento (com dor), tendo a diferença nos escores de dor sido estatisticamente significativa (teste de Wilcoxon, p<0,001). Os especialistas que revisaram a escala concordaram com a validade de conteúdo. Conclusão: Com base nos dados obtidos através do presente estudo, podemos inferir que a CHIPPS é uma escala válida e confiável para ser usada em crianças brasileiras pré-verbais.
Objective: This study was designed to validate the Children’s and Infants’ Postoperative Pain Scale (CHIPPS) in Brazilian children aged zero to 60 months. Methods: 100 healthy children receiving parenteral vaccination were studied. The scale was translated and tested for inter-rater reliability, internal consistency, and construct, content, and concurrent validity. The children’s behavior was videotaped before, during and after the vaccination in primary care unit. Three observers then rated pain behavior from videotapes. Results: The scale had showed excellent inter-rater reliability (intraclass correlation coefficient 0.89) and a very good internal consistency, with Cronbach’s alpha of 0.86.The positive correlation between CHIPPS and MBPS (Modified Behavior Pain Scale) supports concurrent (criterion) validity (Spearman coefficient 0.70 before and 0.81 after vaccinations). The construct validity was determined by comparing the scores of each child before vaccination (without pain) and during the procedure (experiencing pain), and the difference in pain scores was statistically significant (Wilcoxon signed rank test; p<0.001). Content validity (by expert review) was very good. Conclusions: Based on the results obtained, we can infer that CHIPPS is a valid and reliable tool for Brazilian children aged zero to five years old.

▼ Estudo exploratório descritivo, com abordagem quantitativa, desenvolvido entre fevereiro e maio de 2010, com o objetivo de avaliar a dor dos pacientes submetidos a cirurgias abdominais em um Hospital Universitário, em Natal/RN; identificar a localização e a intensidade dolorosa mensurada pela Escala de Estimativa Numérica; analisar a dor nas dimensões sensorial-discriminativa, motivacional-afetiva e a cognitivo-avaliativa, utilizando o Questionário para dor McGill; relacionar o processo álgico com a idade, sexo, religião e tipo de abordagem cirúrgica; identificar a efetividade da medicação utilizada no alÃvio da dor pós-operatória. A amostra constou de 253 pacientes submetidos a cirurgias abdominais. Os resultados mostraram que 63,63% eram mulheres, entre 38 e 47 anos (21,34%); não alfabetizados (21,73%), casadas (64,03%), procedentes da grande Natal (67,97%) e católicas (74,30%). Na primeira avaliação, 84,19% apresentaram dor pós-operatória; 18,97% apresentaram dor leve; 21,74% moderada e 43,48% severa. A média do número de descritores escolhidos através do Questionário de Dor McGill foi 10,78 (DP= 6,09) e o Ãndice de dor 23,65 (DP= 15,93). Os descritores escolhidos com maior frequência foram: dor enjoada (69,01%), cansativa (65,25%), fina (62,44%), aborrecida (58,69%), ardor (46,48%), pontada (38,50%) e cólica (35,21%). Na segunda avaliação, 57,71% pacientes não referiram dor pós-operatória e 42,29% continuaram com queixas de dor. Apenas 41,90% dos pacientes tiveram sua dor totalmente aliviada após administração do analgésico. Os grupos farmacológicos mais utilizados foram os analgésicos simples 37,86%, opióides fracos 32,98%, AINES 19,85% e opióides fortes 9,31%. Não encontramos relação significativa entre a variável dor pós-operatória com o sexo, religião e tipo de abordagem cirúrgica. ConcluÃmos que houve uma alta prevalência de dor pós-operatória, principalmente a de intensidade severa. A dor foi efetivamente aliviada em menos da metade dos pacientes. Dessa forma, percebemos que não existia coerência entre a intensidade da dor do paciente e o analgésico administrado para o seu alÃvio. A educação permanente dos profissionais de saúde deverá ser uma das ações mais eficazes para evitar ou minimizar a ocorrência desse evento

▼ Introdução: O câncer de mama é a neoplasia mais comum entre as mulheres e a manifestação de dor é um evento frequente, podendo ocorrer em todas as fases da doença, bem como ser decorrente de várias abordagens diagnósticas e terapêuticas. Objetivos: Descrever se a dor está ou não relacionada com o câncer de mama, identificar a ocorrência de dor nas últimas 24 horas, verificar a intensidade da dor relacionada ou não ao câncer e descrever os principais sÃtios anatômicos de dor. CasuÃstica e método: Estudo descritivo envolvendo mulheres com diagnóstico de câncer de mama e em tratamento ambulatorial, em qualquer fase da doença ou tratamento, no perÃodo de novembro de 2011 a abril de 2012. As entrevistas foram realizadas no ambulatório de oncologia do Hospital Governador João Alves Filho, utilizando um formulário elaborado pelos pesquisadores e o Inventário Breve da Dor (IBD). Resultados: Noventa mulheres com idade entre 27 a 80 anos foram entrevistadas, 59 (66%) não relacionaram a dor com o câncer de mama e 73 (81%) não relataram a presença de dor antes da doença. Apenas 34 mulheres (38%) referiram dor nas últimas 24 horas interferindo negativamente na habilidade de caminhar, no sono e no humor. A média de dor em uma escala de 0 a 10 foi de 4,8 e os principais sÃtios anatômicos de dor foram os membros superiores (23%), membros inferiores (21%) e coluna vertebral (15%). Conclusão: Em 59 (66%) mulheres a dor não esteve relacionada com o câncer de mama e os MMSS foram apontados como os principais sÃtios anatômicos de dor. A dor esteve presente, nas últimas 24 horas, em 38% das mulheres, de intensidade moderada e interferindo na qualidade de vida.
Introduction: Breast cancer is the most common cancer among women and the manifestation of pain is frequent and can occur at all stages of the disease, as well as being a consequence of various diagnostics and therapeutics approaches. Objectives: To describe if the pain is related or unrelated to breast cancer, identifying the occurrence of pain in the last 24 hours, check the intensity related or unrelated to breast cancer and report the main anatomical sites of pain. Methods: A descriptive study involving outpatient women diagnosed with breast cancer at any stage of the disease or treatment, from November 2011 to April 2012. The interviews were conducted with the oncology outpatients from Hospital João Alves Filho using a screening questionnaire elaborated by the researchers and the Brief Pain Inventory (BPI). Results: Ninety women aged 27-80 years old were interviewed, 59 (66%) did not relate the pain with breast cancer and 73 (81%) reported no pain before the illness. Only 34 women (38%) reported pain in the last 24 hours negatively affect the ability to walk, sleep and mood. The average pain on a scale from 0 to 10 was 4.8. The major anatomical sites of pain were the upper extremities (23%), lower limbs (21%) and spine (15%). In four prospective clinical trials, 114 parturients used intravenous patient-controlled remifentanil analgesia during the 1st stage of labour. The median effective dose per bolus was ascertained to be 0.4 μg/kg and the pain scores were reduced with this by a median of 2 on a numerical scale (0–10). Compared with nitrous oxide, 15 parturients included in a cross-over study reported a larger reduction in pain scores during remifentanil analgesia (1.5 vs. 0.5, p =  0.001) and better pain relief scores (2.5 vs. 0.5 on a ranked five point scale 0–4, p <  0.001). In a parallel study including 45 parturients, epidural analgesia (EDA, 20 ml bupivacaine 0.625 mg/ml and fentanyl 2 μg/ml) was associated with lower pain scores (5.2 vs. 7.3 with remifentanil, p =  0.004) but variables related to satisfaction with analgesia (pain relief score, proportion of mothers with desire to continue with the given medication and termination of the study due to inadequate pain relief) were similar. A comparison of two methods for timing the remifentanil bolus during the uterine contraction cycle suggested that delaying the bolus does not improve analgesia. A period effect was noted in the cross-over trial with higher pain scores and increased drug consumption during the second study period suggesting acute hyperalgesia.
Side effects of remifentanil analgesia included respiratory depression warranting oxygen supplementation in 33% of parturients. Sedation was experienced by the parturients using remifentanil and this was scored as stronger than sedation during nitrous oxide and EDA. The number of parturients with nausea did not increase during remifentanil analgesia. Other maternal side effects included dizziness, a difficulty in visual focusing and itching. Foetal heart rate tracing abnormalities were noted. The incidence of abnormal tracings and decreased UapH were not different, however, from that observed during nitrous oxide or EDA. Apgar scores at 1 and 5 minute indicated no neonatal depression.

pain (69.01%), tired (65.25%), thin (62.44%), bored (58.69%), ardor (46.48%), pointed (38.50%) and colic (35.21%). In their second assessment, 57.71% of patients didn t relate any postoperative
pain and 42.29% were still complaining about the
pain. After taking analgesic medication, just 41.90% of patients who had complete
pain relief. The Pharmacological groups most used were: simple analgesic (37.86%), weak opioids (32.98%), AINES (19.85%) and strong opioid (9.31%). It was not found a significant postoperative
pain variation related to the sexes, religion and kind of surgery. It was concluded there were a high level in the number of patients with postoperative
pain, mainly in a severe scale. Less than half of patients had the
pain relief. Then, it was observed there was not coherence between the
pain intensity and the analgesic it was used. To solve or relieve this kind of problems is necessary a permanent education to the health professionals who works in this area

▼ The aim of the present study was to evaluate an array of psychometric tests administered to a heterogeneous group of chronic
pain patients at pre- and post- treatment to determine the effect sizes of the measures. The sample included patients (N=312) who participated in an interdisciplinary treatment program, which included medical, psychological, psychiatric, and physical therapy components. This sample was narrowed to include only those who completed treatment (n=262). Subjects were evaluated on a variety of physical/functional, psychosocial, and coping measures, including the Visual Analog Scale (VAS), Million Visual Analog Scale (MVAS), Oswestry Low Back
Pain Disability Questionnaire (OSW),
Pain Medication Questionnaire (PMQ), Medical Outcomes Survey 36-Item Short Form Health Survey (SF-36), Beck Depression Inventory-II (BDI-II), and Multidimensional
Pain Inventory (MPI). Paired sample t-tests were conducted to evaluate each measure for pre- to post-treatment change. These measures were further analyzed using Cohen's d (1992) to obtain the effect size. Results indicated that the instruments showing the greatest effect size were the VAS (d= 1.27) and the MVAS (d=0.94), both within the large effect size range. The OSW (d=0.67) showed a medium effect size, while the SF-36/PCS (d=0.19) had the lowest effect size of the physical measures. Results indicated a medium effect size for psychosocial measures. The PMQ (d=0.79) BDI-II (d=0.72) and the SF-36/MCS (d=0.62). The MPI exhibited an extremely low effect size (d=0.03). The heterogeneous population was also broken down into three categories of
pain diagnoses including musculoskeletal, all other single
pain diagnoses (e.g. headache, neuropathy, reflex sympathetic dystrophy, firbomyalgia), and multiple diagnoses (more than one type of
pain). Overall, this study offers information on the effect sizes of different measures in order to facilitate the decision making process when selecting assessment tools to use with chronic
pain populations, and supports the use of multiple assessment measures.

▼ The purpose of this study was to build a new profile classification system for the Multidimensional
Pain Inventory (MPI). According to some clinical researchers, the current profiles of the MPI do not fully portray how chronic
pain patients evaluate and manage their
pain because of the great variability in each subgroup. This study tried to revise the current profile classifications based on clinical observations, which may then lead to facilitate improved patient assessment, professional communication, and treatment planning. Participants, who completed pre- and post- treatment MPI measures, were randomly selected from the interdisciplinary program at the Eugene McDermott Center for
Pain Management at UT Southwestern Medical Center at Dallas. Two hundred and eighty patients were then assigned to three different groups depending upon the re-coded scores from their pre-treatment MPI. Patients were grouped according to their MPI subscale scores. In order to determine if the hypotheses were supported or not, paired t-test were completed on six different psychosocial and functional outcome measures. Analyses were also conducted to check for differences among the nine different groups. As postulated, a number of significant relationships were identified. Paired t-test analyses demonstrated the significance of the relationship between certain MPI subscales. When
Pain Severity (PS) and Interference (I) were below average (as determined by recoded T scores), patients had a good prognosis. When Life Control (LC) equaled Affective Distress (AD), participants were seen to have a good prognosis as well. Associations between the other MPI subscales were also assessed, but data did not support those hypotheses. The majority of the outcomes did not meet expectation, because of several limitations with the study design.

▼ This study aimed to examine predictors of completion and success in interdisciplinary treatment for chronic
pain and to provide clinicians with relevant information in determining the appropriate treatment intensity for patients. A total of 1,062 patients were examined who participated in one of three different levels of treatment intensity: a 120 hour treatment program (n=699), a 72 hour treatment program (n=61), and a 24 hour treatment program (n=302). Results indicated that higher levels of anxiety and greater number of visits to the emergency room in the twelve months prior to treatment were predictive of premature termination of treatment in the more intensive program. For less intensive programs, number of hours resting per day was predictive of early termination of treatment. Predictors of success were then examined utilizing five definitions of success. No single measure was found to be a significant predictor across all five domains. However, lower levels of health care utilization, higher levels of affective distress, greater perceived interference from
pain, and lower levels of perceived control were predictive of successful outcomes. Overall, the findings indicate that individuals with greater dysfunction at treatment entry will benefit more from treatment. When comparing three programs of different intensities, the most intensive program produced a higher proportion of successful outcomes. Pre-treatment variables were explored as a means of identifying relevant clinical variables that could be utilized by clinicians to identify the most appropriate treatment program for patients. Greater number of hours resting and higher levels of
pain at pre-treatment were found to be useful variables. Namely, those resting more than seven hours per day or reporting
pain levels higher than six out of twelve indicated a need for the most intensive program.

▼ Muitos profissionais de saúde assumem intrinsecamente que é natural a pessoa estar deprimida quando se vê confrontada com a iminência da morte. Mas a depressão é uma condição tratável e cabe aos profissionais estarem capacitados para a sua identificação e tratamento precoce. Em cuidados paliativos a depressão e a dor são duas entidades frequentemente presentes e a sua relação de reciprocidade é controversa. Contudo, ninguém duvida do impacto negativo que causa na qualidade de vida e no sofrimento dos doentes e famÃlia.
O presente estudo teve como objectivo identificar qual a influencia da dor na depressão nos doentes em cuidados paliativos.
Estudo de cariz transversal, descritivo-correlacional, com colheita de dados por formulário, recorrendo-se à Escala Visual Analógica para avaliação da intensidade da dor e ao Inventário de Depressão de Beck para determinar a depressão. Foi critério de exclusão os doentes com alterações neurológicas incapacitantes de responder às questões.
Amostra não probabilÃstica, constituÃda por 50 doentes internados nas Unidades de Cuidados Paliativos de Cantanhede e Seia, no perÃodo de 24 Janeiro a 30 Junho de 2011, sendo 26 mulheres e 24 homens com idades compreendidas entre os 41 e os 95 anos. Os dados obtidos permitiram traçar o perfil sócio-demográfico médio dos doentes estudados, caracterizado como indivÃduo de 72 anos, reformado, a residir com familiares em meio rural, casado ou a viver em união de facto e com o ensino básico.
A maioria dos doentes apresentou sintomatologia depressiva (92.0%). No momento da recolha de dados 60% dos doentes revelou ter dor e destes 76,7% classificou-a de moderada.
Assim, podemos inferir que a dor e a depressão estão intimamente relacionadas. A presença de dor é preditiva da depressão, no entanto os dados obtidos foram insuficientes para comprovar a relação entre a intensidade da dor e a depressão.

ABSTRACT
Many health professionals intrinsically assume that is natural to be depressed when someone is confronted with the imminence of death. However it’s important to know that it is a treatable condition and it’s up to professionals to be able for their identification and early treatment. In palliative care, depression and pain are two entities often present and their mutual relationship is controversial. But no one doubts about the negative impact on life quality and in patient and family’s suffering.
The present study as objective identifies the influence of pain on depression in patients with palliative care.
Cross-sectional study, descriptive and correlational, performed a data collection through a form, where were used the Visual Analog Scale to assess pain intensity and the Beck Depression Inventory to determine depression. Exclusion criteria were patients who presented disabling neurological disorders to answer the questions.

▼ Aim The thesis set out to examine validations of three observational pain assessment tools and establish nurses’ expectations of them and the factors that might influence them within intensive care unit (ICU) settings. Background The guidelines to pain assessment specific to ICU patients have been of great interest to health professionals over the last 20 years. Pain assessment remains a challenge for most ICU patients due to the difficulty of assessing pain with any precision. Evidence suggests that the Behavioural Pain Scale (BPS) and Critical- Care Pain Observation Tool (CPOT) have demonstrated sound psychometric properties. A review of the relevant literature highlighted the fact that no such studies have yet been conducted with a similar homogenous group in Asia. The Wong-Baker Face Pain Rating Scale (FPRS) is currently widely used for nonverbally communicating patients (NVCPs) with pain in ICU settings, and is even recommended for use with children. Valid assessment tools are required for effective pain assessment in ICU settings, particularly in patients who are experiencing communication difficulties. Design An embedded mixed methods design was employed to: 1) translate Chinese versions of BPS and CPOT, 2) test their validity and reliability of in comparison with FPRS, and 3) establish the nurses’ expectations about the three study scales when undertaking pain assessment by using semi-structured focus group interviews. Methods This thesis initially reviews the literature available to select the most appropriate scales for assessing pain in critically-ill NVCPs. The selected scales were then translated into a Traditional-Chinese version using established procedures for the Taiwanese context. Evaluations of the three pain scales were gathered using quantitative measures of pain scores in NVCPs experiencing painless/painful interventions. These were further compared with a few focus groups to establish the feasibility and utility of the three pain scales. The psychometric properties of the pain scales were assessed for reliability by using internal consistency and inter-rater agreement) and for validity by using content validity, concurrent validity, discriminant validity, and responsiveness. The validity was evaluated using ANOVA to compare the changes between the different procedures. The significance level was set at 0.05. As for the analysis of the qualitative data, this study typically follows the path of aggregating the words into themes of information and presenting the diversity of ideas gathered during the data collection. Results For the 2068 observations in 237 patients, there were no statistical differences between the characteristics of the BPS, CPOT, and FPRS groups. Validity was demonstrated by changes from baseline in the scores of the three groups, which were significantly higher during suction (p < 0.001). Measuring pain in infants is important but challenging for researchers and health care professionals. The measurement of skin conductance (SC) is considered as a measure of stress and a surrogate indicator of pain.
Purpose
This dissertation provides insight on the validity of SC for pain measurement in infants and consists of two studies: 1) a scoping review synthesizing the methods and findings of previous studies on validating or using SC for measuring pain in infants; 2) a primary study evaluating the validity of SC for measuring pain in mechanically ventilated infants.
Methods
Arksey and O’Malley’s framework informed the methods of the scoping review. Nine electronic databases were searched. Data were analyzed and presented descriptively. The primary study used a prospective cross-sectional observational design. Eligible infants were those up to 12 months of age, hospitalized in intensive care units, who were mechanically ventilated, and required painful and non-painful procedures.
Results
Scoping review: Twenty-eight studies with 1061 infants were included, including 23 cross-sectional observation studies and five interventional studies. The validity evidence of SC was tested in relation to referent pain measures (13 variables), stimuli (13 variables), age (2 variables) and other contextual variables (11 variables). Fifteen studies evaluated the validity evidence in relation to phase of painful procedure, and SC increased significantly during painful procedures in most studies (n=14/15). However, inconsistent findings on other validity evidence and wide variation in methods existed across studies.
Primary study: SC showed good validity in relation to the category of procedure, the phase of procedure and the referent pain measures in critically ill mechanically ventilated infants. The findings from diagnostic test accuracy showed that SC had good capacity of detecting moderate to severe pain. However, the values of SC need to be used with caution, due to the imperfect correlations with the referent pain measures and imperfect positive predictive value.
Conclusions
SC is a promising approach to measuring pain in critically ill infants. Further research testing the validity of SC in relation to pain treatments and advancing the technology of measuring and analyzing SC is needed before it can be recommended for clinical use.

Kuitenkin kipu on edelleen alihoidettua, koska sitä ei arvioida tai sen voimakkuuden arviointi ei ole järjestelmällistä, eikä kirjaaminen ole systemaattista. Hoitamattomalla kivulla on todettu olevan toipumista hidastavia tai ehkäiseviä vaikutuksia. Se on toimintakykyyn ja elämänlaatuun vaikuttava oire. Kivun hoidosta on julkaistu sekä kansainvälisiä, että kansallisia suosituksia. Kirjallisuuden perusteella terveydenhuoltohenkilöstöltä puuttuu edelleen tietoa ja taitoa kivun arvioimiseksi sekä asenteet kivun arviointiin ovat edelleen negatiiviset tai välinpitämättömät. Lisäksi terveydenhuollon organisaatioilta puuttuu yhtenäisiä toimintatapoja kivun arviointiin.
Tämän tutkimuksen tarkoituksena oli kuvata henkilökunnan käsityksiä kipumittarin käyttöön vaikuttavista tekijöistä iho- ja keuhkosairauksien sekä sisätautien vuodeosastoilla henkilökunnan näkökulmasta. Lisäksi tarkoituksena oli kuvata henkilökunnan käsityksiä kivun arvioinnin kehittämistä edistävistä tekijöistä. Tavoitteena oli tuottaa kivun arvioinnin nykytilanteesta tietoa, jota voidaan käyttää kivun arvioinnin kehittämisessä.
Aineistonkeruumenetelmänä käytettiin tutkimuskohteena olevien vuodeosastojen hoitajille (n=79) ja lääkäreille (n=12) suunnattua kyselyä, jonka avointen kysymysten vastaukset analysoitiin induktiivisella sisällön analyysilla. Avoimissa kysymyksissä kysyttiin syitä kipumittarin käyttämättömyyteen tai satunnaiseen käyttöön, sekä henkilökunnan arviota käytössä olevan kipumittarin soveltuvuudesta työyksikön potilaiden kivun arviointiin. Lisäksi kysyttiin, mitä kivun mittaamisen kirjaamisesta on sovittu työyksikössä, sekä mitä muuta vastaaja halusi tuoda esiin kivun mittaamiseen liittyen. Strukturoiduilla kysymyksillä kysyttiin osallistujien taustatietoja, kipumittarin käytön yleisyyttä vuodeosastoilla ja kivun arviointiin liittyvää ohjeistusta sekä kivun kirjaamisen systemaattisuutta.
Lähes 75% tutkimuskohteena olevilla osastoilla oli jokin kipumittari käytettävissä. Useimmiten se oli VAS-mittari. Kuitenkin vain noin joka viidennen kipupotilaan kohdalla käytettiin kipumittaria kivun arviointiin. Yli 70% vastaajista kuvasi, ettei työyksikössä ole sovittu kivun kirjaamisesta tai siitä ei ole tietoa. Henkilökunnan käsitysten mukaan kipumittarin käyttöön vaikuttivat kivun luonne, potilaan sairaus ja yhteistyökyky sekä henkilökunnan asennoituminen kivun arviointiin.
Henkilökunnan käsitysten mukaan kivun arvioinnin kehittämistä edistäisi kivun arvioiminen päivittäisessä potilastyössä, kivun arvioinnin vakiinnuttaminen ja potilastietojärjestelmän hyödyntäminen kivun arvioinnissa.
Tämän tutkimuksen perusteella potilaan saama kivun hoito ei ole tasalaatuista tai suosituksiin pohjautuvaa tutkimuskohteena olevassa organisaatiossa. Suuressa osassa yksiköitä on jokin kipumittari käytössä, mutta kipua ei arvioida jokaiselta potilaalta ja kivun arvioinnin kirjaaminen on puutteellista.

pain measurement device. The redesign will focus on improving the original Computer Controlled Pressure Algometer, designed by Zimkowski in 2010.
Pain measurement is very difficult to accurately accomplish, and current methods are limited to subjective techniques such as
pain rating scales and handheld algometry. These subjective methods of
pain measurement have many shortcomings that render them inadequate at reliably assessing an individual's
pain. Large variability has been shown with these testing methods, which can be prone to error due to test administrator, environmental and psychological conditions, and individual patient factors. This study hypothesizes that by creating an automated device, these sources of variability may be minimized. The improved automated
pain measurement device enhances usability, functionality, and aesthetics compared to the original device - these improvements will enable more practical clinical studies. Furthermore, the addition of biometrics could introduce an objective measure of
pain. By creating an automated
pain measurement device with integrated biometrics, both subjective and objective
pain response data could be collected for each patient, eventually establishing a personalized
pain scale for each patient. These personalized
pain scales could then be used to better assess, diagnose, and treat ailments which cause chronic and acute
pain.

▼ Patienter med nedsatt kommunikationsförmÃ¥ga har begränsade möjligheter till att förmedla sin smärtupplevelse. Sjuksköterskan har till uppgift att bedöma patientens smärta vilket kan vara oerhört komplext. Syftet med denna litteraturstudie var att belysa bedömning av smärta hos vuxna patienter med nedsatt kommunikationsförmÃ¥ga. Litteraturstudiens resultat grundades pÃ¥ 15 kvalitativa och kvantitativa vetenskapliga artiklar som kvalitetsgranskades och bearbetades. Analysen av dessa artiklar resulterade i tvÃ¥ teman: smärtbedömning av vuxna patienter med nedsatt kommunikationsförmÃ¥ga och smärtuttryck hos vuxna patienter med nedsatt kommunikationsförmÃ¥ga. Smärtan är flerdimensionell och bör ses som en helhet, dÃ¥ den bottnar i bÃ¥de fysiska och psykiska faktorer. Smärtan uttrycker sig pÃ¥ flera sätt, detta bör därför lyftas fram och diskuteras mer i sjuksköterskeutbildningen och pÃ¥ klinik. Tidigare forskning har visat att kvinnor upplever och uttrycker mer smärta än män. Kommande forskning bör inriktas mot hur vida smärtuttrycken skiljer sig Ã¥t och om smärtbedömningsinstrument är anpassade för de olika könen var för sig. Genom bearbetning av resultatet skapades en checklista (LARV) för att bedöma olika smärtupplevelser hos patienter med nedsatt kommunikationsförmÃ¥ga. LARV är tänkt som en tillgÃ¥ng och ett komplement till sjuksköterskans smärtbedömning utifrÃ¥n kompetens och erfarenhet. LARV bör prövas inom vÃ¥rden för att utvecklas.
Patients with impaired communication skills has limited ability to                   communicate their pain experience. The nurse is responsible for                   assessing the patient's pain, which can be extremely complex. The                   purpose of this study was to illustrate the assessment of pain in adult                patients with impaired communication skills. The result were based on                15 qualitative and quantitative research articles that are peer reviewed                and processed. The analysis of these articles resulted in two themes:                pain assessment of adult patients with impaired communication skills                and pain expression in adult patients with impaired communication                skills The pain is multidimensional and should be considered as a                   wholeness and contains both physical and psychological factors. The                pain is expressed in many ways, this should be highlighted and                   discussed more in nursing education and in practice.                Previous research has shown that women experience and express more                pain than men.
