jos išplitimą, renkantis optimalią chirurginio gydymo metodiką.
Darbo uždaviniai: 1. Nustatyti neinvazinio ultragarsinio tyrimo
sąsajas su morfologinio tyrimo rezultatais, vertinant odos
melanomos storį, svarbų chirurginio gydymo taktikai. 2. Įvertinti
dviejų invazinių diagnostikos metodų taikymo tikslingumą, surandant
sarginį (-ius) limfmazgius, kai aplink naviką arba pooperacinį
randą suleidžiamas audinių dažas ir RF. 3. Palyginti sarginių
limfmazgių skaičių ir mikrometastazių dažnį dviejose sergančių odos
melanoma pacientų grupėse: operuotųjų vieno etapo operacija –
radikali naviko ekscizija ir sarginių limfmazgių biopsija („V“
tiriamųjų grupė) ir operuotųjų dviem etapais – pirminė diagnostinė
naviko ekscizija 0,5 cm atstumu nuo naviko kraštų, po to radikali
pooperacinio rando reekscizija su sarginių limfmazgių biopsija („D“
tiriamųjų grupė).
The aim The aim of this study was to
evaluate the potential and value of non-invasive and invasive tests
in diagnosing cutaneous melanoma and assessment of its
dissemination, and to justify optimal technique for surgical
management. Objectives 1. To determine correlation between
non-invasive ultrasound investigation and results of morphologic
examination in assessment of cutaneous melanoma thickness
influencing surgical treatment strategy. 2. To evaluate the
expedience of using two invasive diagnostic methods (tissue stain
and RA injection at the site of the tumour) in detection of
sentinel lymph node(s). 3. To compare the count of sentinel lymph
nodes and the rate of micrometastases between two groups of
patients with cutaneous melanoma: patients who underwent one-stage
surgery – radical excision of the tumour and sentinel lymph node
biopsy (group “V”), and patients who underwent two-stage surgery –
primary diagnostic excision of the tumour (0.5 cm from borders of
the lesion) followed by a radical re-excision of the postoperative
scar and sentinel lymph node biopsy (group
“D”).

▼ Introduction There are many personal and behavioral factors that can decrease the effectiveness of condoms, even when one is used every time. Clinical condom failure (the condom breaking, tearing, or completely slipping off the penis) contributes to less than perfect effectiveness rates and can be at least partially attributed to these personal and behavioral factors. Although specific use and storage behaviors can be reasons for a condom to break or slip, there are often other factors at play. Negative perceptions of condoms, self-efficacy when using condoms, type of sex act, and history of STI’s can all impact clinical failure rates.
Objective The purpose of this study was to assess four major factors: negative perceptions of condoms, self-efficacy, history of STI’s, and type of sex act, and their relationship to condom breakage and slippage in a sample of 381 condom users in Atlanta, GA.
Methods Data were drawn from the baseline survey of the C-PLEASURE Study. Bivariate analyses were conducted to determine the relationship between each independent variable and the two outcome variables: experiencing condom breakage in the last 6 months and experiencing condom slippage in the last 6 months. Then two logistic regression models, one for experience of any condom breaks in the last six months and one for experience of any condom slips in the last six months, were used to further determine these relationships.
Results The only significant relationship that was found was between negative perceptions of condoms and condom slips (AOR=1.08 95% CI= [1.02, 1.14], p=0.01). History of STI’s, type of sex act, lubricant use, lubricant type, consistent condom use, and self-efficacy did not return any significant relationships with experience of condom breaks or slips in the last 6 months, although self-efficacy did approach a significant relationship with condom slips (p=.07).
Conclusions Since negative perceptions are correlated with condom failure, attitudes and perceptions of condoms need to be addressed for greater efficacy. Further studies on self-efficacy and condom failure should be pursued to determine how strong the protective qualities of self-efficacy can be when it comes to effective use of condoms and prevention of condom failure.

of diagnosed sexually transmitted infection (
STI) and are at high
risk of human immunodeficiency virus (HIV) acquisition.
STI and HIV
testing services are free and readily available to most university
students in Canada, however, only some individuals choose to access
them. Using data from a cross-sectional survey of eight Maritime
Canadian universities, we determined that 62% of vaginally sexually
active undergraduate students have never had an
STI test and 85%
have never been tested for HIV. Across groups stratified by
biological sex and risk of
STI acquisition, younger students with
less sexual health knowledge were significantly less likely to be
tested for
STI and younger, heterosexual students were
significantly less likely to be tested for HIV. Health promotion
may be most effective at increasing overall lifetime
STI and HIV
testing when targeting younger students with campaigns designed to
improve sexual health knowledge.

▼ This dissertation examines the socioeconomic and health profiles of sexual minorities, especially with respect to how they are impacted by public policy. In the first chapter I explore the relationship between legal same-sex marriage (SSM) and rates of common sexually transmitted infections. Results indicate that legal SSM is associated with a significant decrease in syphilis rates, an especially strong proxy for risky sex between men. In the second chapter, which is joint work with Christopher S. Carpenter, we make use of new information on sexual orientation to first reproduce the well-documented finding of an earnings premium for self-identified lesbians. However, these data also show—for the first time in the literature—that self-identified gay men also earn significantly more than comparable heterosexual men. In the third chapter, which is joint work with Christopher S. Carpenter and Gilbert Gonzales, we provide the first large-scale evidence on transgender status, gender identity, and socioeconomic outcomes in the United States using representative data from 31 states. Individuals who identify as transgender have significantly lower employment rates, lower household incomes, higher poverty rates, and worse self-rated health than otherwise similar men who are not transgender.

▼ In the modern times, embedded communication networks are being used in increased number of embedded systems to provide more reliability and cost effectiveness. Designers are forced to limit and minimize the size, weight, power consumption, costs and also the design time of their products. However, network controller chips are also expensive and hence moving functionality from hardware to software cuts down the costs and also makes custom fit protocols easier to implement.
Traditional methods of sharing a processor are not adequate for implementing communication protocol controllers in software because of the processing required during each bit. The available idle time is fine grain compared to the bit time and is usually small for even the fast context switching techniques (e.g. co-routines) to run any other thread. Without some scheme to recover this fine-grain idle time, no other work in the system would make any progress.
Software Thread Integration (
STI) provides low cost concurrency on general-purpose microprocessors by interleaving multiple threads of control (having real-time constraints) into one. This thesis introduces new methods for implementing communication protocols in software using statically scheduled co-routines and software thread integration. With co-routines, switching from primary to secondary threads and vice versa can be done without incurring a penalty as severe as "context - switching". This technique will be been demonstrated on the SAE J1850 communication standard used in off- and on-road land-based vehicles. These methods also minimize the number of co-routine calls needed to share the processor thereby enabling finer-grain idle time to be recovered for use by the secondary thread. Thus, now more secondary thread work can be done and also the minimum clock speed required of the processor is reduced. These factors enable the embedded system designers to use processors more efficiently and also with less development effort.

▼ Back ground: Trichoomonas vaginalis is a flagellated, single cell protozoan parasite. T.
vaginalis carries the distinction of being the only truly sexually transmitted parasitic infection in
humans. It is highly adapted to the human urogenital tract and majority of infections are
asymptomatic. Symptomatic trichomoniasis presents with an offensive vaginal discharge and
vulval itching in women. T. vaginalis infection has been shown to be associated with adverse
pregnancy outcomes, particularly premature rupture of membranes, pre-term delivery and low
birth weight.
Objective: the aim of this study was to determine the prevalence of T. vaginalis among
STI
compliant and (or) pregnant women attending Merawi Health center. Simultaneous comparison
of the laboratory (wet mount microscopy) result with the syndromic management results was
performed.
Result: 222 women have participated in this study. 198 were pregnant and 24 were
STI
complaint. Majority of them were under the age group of 15-25 years (48.6%, 108). 73.9% (164)
were illiterate, 94.1% (209) were married, and 86.9% (193) didn’t have awareness to any of
STIs. Syndromic management approach has identified 4 women (1.8%) who had self-reported
symptoms related to trichomoniasis. However; the laboratory wet mount diagnosis has identified
14 women who were positive for the trophozoites of T. vaginalis. As a result, the prevalence of
trichomoniasis using the laboratory wet mount result was increased to 6.3% (14/222). The
sensitivity of syndromic management approach with respect to the wet microscopy was 21.4%.
Conclusion: As a result of high rates of asymptomatic infections, syndromic management
approach should not be the first choice to screen out trichomoniasis. So, this study supports the
need for improved diagnostic parameters and disease control activities to reduce adverse
trichomoniasis associated reproductive health outcomes such as ectopic pregnancy, low infant
birth weight and preterm labor.

Idiopathic urethritis (IU) comprises up to 50% of symptomatic cases of male urethritis in clinical settings. The syndrome is of an unknown etiology but may be due to an as yet unidentified bacterial pathogen(s). We were interested in identifying pathogens that could cause IU using multiple methods. Shotgun metagenomic sequencing or 16S rRNA sequencing methods can provide rich datasets but are limited by the completeness of the corresponding sequence reference databases. We generated metagenomic and 16S datasets from DNA extracted from urethral swabs of men with IU to determine the composition of their urethral microbiome. In order to enrich the corresponding reference databases used to identify the reads in the sequence datasets, I cultivated bacteria from the first void urine (FVU) of men with IU. My goal was to grow and whole genome sequence bacterial isolates that are not currently represented in the reference databases.
Of the 216 men we enrolled at the Bell Flower STD clinic in Indianapolis, IN, 59 men had IU. I grew a total of 802 isolates from the FVU of the IU patients and identified those isolates using colony-based 16S rRNA PCR. Based on % sequence similarity to the nearest type strain, I sorted the 16S alleles into four categories: Species (≥98 % identity) (N=264), Genus (≥95 % identity) (N=407), Closest Match (<95 % identity) (N=95), and No Hit (0 % identity) (N=22). There were 24 genera represented in the isolate collection. Of these, the six most abundant genera were Streptococcus, Staphylococcus, Corynebacterium, Haemophilus, Gardnerella, and Prevotella. These six genera composed nearly 80% of all IU-associated isolates. All sequences below 98% sequence similarity represent potentially novel strains of bacteria. We will proceed with whole genome sequencing of bacterial isolates with the goal of improving genome database coverage of bacterial diversity in the male urethra.

STI risk behavior, but research has been limited. The present study examined the association between discounting and
STI risk behavior, including drug use, in youth (N = 155), ages 14-21 years. Further the study included a psychometric examination of the Sexual Discounting Task (SDT) with regard to
STI risk behavior. The SDT should be considered a clinically meaningful assessment of
STI risk behavior in youth. Sexual discounting had robust associations with
STI risk and differences were found across gender. Further, differences in gender, sexual discounting, and
STI risk were found across drug users. In addition,
STI risk differences were by gender and across drug users. The results highlight discounting as a critical and fairly unrecognized variable for understanding
STI engagement, especially in females and drug users, which may benefit current
STI intervention and prevention strategies.

Background: Today, adolescents are recognized as groups with special health-related vulnerabilities .They are exposed to various risks such as unprotected sex, early marriage, early pregnancy, sexually transmitted infections (STIs) and HIV/AIDS. Many adolescents are sexually active. These sexual activities put adolescents at risk of various reproductive health problems including
STI and HIV/AIDS.STIs have occurred mostly among young people, with the highest reported rates found among those aged 15-24 years.
Objective: The major objective of this study is to assess the sexual behavior and the knowledge of
STI and HIV/AIDS among high school adolescents.
Method: A cross-sectional study was conducted on 562 adolescents using a pre tested questionnaires from March to April 2014, among two high schools, in Jardega Jarte woreda, Horo Guduru Wollega zone, west Oromia regional state.
Result: From the total respondents, 38.6 % ( 53% of male and 47% of female) of adolescents had experienced sexual activity. Among those, only 46.6% of them had used condom the first time they had sexual intercourse. The mean age at first sexual intercourse was 16.24 (16.24 ± 0.682) for male and 15.40 (15.40±0.796) for females. From all sexually active adolescents, 92.2% (97.2% of boys and 86.7% of the girls) had their first sexual intercourse between the ages of 15 and17 years.
The reasons cited for the initiation of their first sexual intercourse were fall in love (37.9%), had desire, 74 (36%), peer pressure, 30 (14.5%), were drunk, 12 (5.8%), to get money or gifts, 5 (2.4%), rape, 4(1.9%), and got married, 2 (0.9%).
Among all adolescents, 45.7% of students have heard about sexual intercourse other than vaginal while 54.3% of students have not heard.
98.9% of the respondents claimed to have ever heard about sexually transmitted diseases (STIs), of which, HIV/AIDS (96.6%), gonorrhea (75.8%), Syphilis, (74.3%) and Chancroids (40.1%), were the most commonly known types of
STI.

The most commonly cited sources of information for HIV/AIDS were media, such as radio and TV, (78.3%), school clubs,(74.5%) and followed by school teachers, (62.9%).
The majority of the adolescents mentioned the modes of HIV/AIDS transmission to be through unprotected sexual intercourse (96.4%), sharing contaminated instruments (81.8%) followed by from mother to child (68.4).
Conclusions and recommendations: from this study, one can conclude that substantial proportion of the adolescents ever had sexual intercourse was higher. Moreover adolescents are practicing risky sexual behaviors such as unprotected sex, multiple sexual partners, and sex at early age. Therefore, an integrated effort needs to be initiated to address such adolescents’ sexual and reproductive health problems through establishing and strengthening school anti AIDS clubs, providing in service training for teachers on adolescent reproductive health and establishing youth centers.

Gonorrhoeae is a major public health concern due to its capacity to develop antibiotic resistance and its potential complications including pelvic inflammatory disease, epididymitis, infertility, and disseminated infection. In order to keep up with resistance trends, the treatment recommendations for gonorrhea have changed frequently. In other areas of medicine, guideline adherence has been shown to be limited, even without frequent guideline changes. In the case of gonorrhea, inappropriate treatment can have serious individual and public health implications, making the quality of and compliance with clinical guidelines critical. This thesis is a three-part mixed methods research project assessing the development and uptake of gonorrhea treatment guidelines. We conducted a systematic review of current gonorrhea treatment guidelines and used the AGREE II tool to assess the quality of guideline development (Chapter 2). We observed that guideline quality varied greatly with many guidelines having weaknesses in their use of existing evidence to develop recommendations, their reporting of potential conflicts of interest and how they were addressed, and their consideration of barriers to the implementation of their recommendations. We then assessed physician adherence to first-line treatment recommendations in Ontario by conducting a segmented time series analysis of Ontario gonorrhea treatment data from iPHIS, the province’s reportable disease database (Chapter 3). Following the introduction of new guidelines that recommended substantial changes from current practice, we found very dramatic drops in guidelines adherence that then improved slowly over time. We then explored the use of process mapping as a tool to look at the local management of cases in the City of Ottawa by following them across the various possible treatment pathways (Chapter 4). Here, we noted differences in practice between the management of cases at Ottawa Public Health’s Sexual Health Clinic and the management of cases elsewhere in the community.

▼ Introduction: This study aimed to calculate the prevalence of three bacterial sexually transmitted infections (STIs): Chlamydia trachomatis (CT), Neisseria gonorrhoeae (NG), and Mycoplasma genitalium (MG), in asymptomatic HIV-negative men-who-have-sex-with-men (MSM) attending Western Sydney Sexual Health Centre (WSSHC). The associations between patient-related factors (demographic, behavioural and clinical factors) and the presence/absence of bacterial STI pathogens were investigated. Methods: This study will calculate the prevalence and risk factors using a retrospective cross-sectional approach relying on pre-collected patient data and laboratory testing results. A multivariable logistic regression model was used to analyse the correlations of STIs diagnosis and specific risk factors. Participants were asymptomatic MSM aged 18 years old and over attending the WSSHC Express Clinic and undergoing onsite STIs screening between 1 February and 31 May 2017. Results: A total of 307 asymptomatic MSM were involved in this study where 82 (26.7%) participants were found to have at least one pathogen at one anatomical site. The three most prevalent diagnoses identified were ano-rectal CT, 9.8% (95% CI: 6.5 to 13.0), ano-rectal MG, 9.4% (95% CI: 6.2 to 12.7), and oro-pharyngeal NG, 5.5% (95% CI: 2.9 to 8.1). Asymptomatic STI cases was relatively low in oro-pharyngeal CT, 1.0% (95% CI: 0.04 to 2.3) and urethral NG, 0.3% (95% CI: 0.04 to 2.3). MG was not detected in oro-pharyngeal samples. Co-infections occurred in 15/307 MSM (4.9%), most frequently at ano-rectal sites. A multivariate model demonstrated that MSM who practised more condomless sex (adjusted odds ratio/AOR=2.39, 95% CI: 1.05 to 5.51, p=0.04) and had past STI episodes (AOR=1.92, 95% CI: 1.01 to 3.62, p=0.04) were more likely to have STI diagnosis. Conclusion: Bacterial STIs were common among asymptomatic HIV-negative MSM attending the Express Clinic in WSSHC. Infections affecting ano-rectal sites remained concerning within this population, as high-risk sexual behaviours were still frequently reported. This study also found a high prevalence of MG infection which was previously not considered as a major infection. These results highlight the importance of STI screening strategies for MSM.

▼ Introduction Human Immuno-deficiency Virus (HIV) infection and Sexually Transmitted Infections (STIs) are important transmittable infections in the world. They are transmitted mostly through sexual activity and contaminated blood exchange (needle). Key populations are the most affected groups in the world from these infections, one of the important key population is female sex worker (FSW). Although FSW is the most vulnerable group, but stigma, discrimination, poverty and less evidence based policy have been the reasons this group cannot get access to health care. Endocervical infections or also called cervicitis is common among FSW. This infection is commonly caused by Neissheria gonorrhoea and Chlamydia trachomatis, although infection of Mycoplasma genitalium in endo cervix also quite common. This study investigated the sensitivity and specificity of current test, the correlation between prevalence of endocervical infections with behavior and other routine test result among women working in entertainment venues in Jayapura. Aims of this study is to evaluate current STI-HIV screening test and to generate recommendation for stakeholders. Material and methods This study was using questionnaire which was adapted from several studies about FSW behaviour, and one of this study was conducted on national scope, the Integrated Biological and Behavioural Survey (IBBS) in 2011. The laboratory result was from routine screening test in the reproduction health centre (PKR) and additional PCR test for N. gonorrhoeae and C. trachomatis on endocervical sample. Participants of this study are women working in entertainment venues in Jayapura-Papua, Indonesia for at least three months. Results Among 176 participants who got tested for any endocercival test of N. gonorrhoea and C. trachomatis, 24.4 percent (n = 43) of participant had at least 1 endocervical infection. Among all participants, the percentage of STIs was 26.5 percent (n=50). On bivariate analysis; Age, marriage status, last education, number of clients in the last week, have standard education, have regular sexual male partner in the last 1 year, have history of vaginal discharge in the last 5 year, have regular HIV check, use hotel and or rent room/ working place as a place of sexual transaction were associated with any endocervical infection. On multiple logistic regression analysis age, marriage status and a past history of vaginal discharge are significantly related with any endocervical infection among participants.

▼ Aboriginal people living in remote communities experience a high burden of sexually transmissible infections (STIs). To interrupt the transmission of STIs and prevent complications, regular testing and timely treatment are required. The aim of this thesis is to provide new knowledge about the extent of regular testing, and barriers and facilitators to offering testing and timely treatment from the perspective of health staff working in these areas. Four studies were undertaken. The first, a longitudinal analysis, showed that among individuals attending 68 remote Aboriginal health services, 20.3% of those who had an initial chlamydia/gonorrhoea test were re-tested at 12 (+/-3) months, with rates higher in young people and females. Among both males and females, re-testing was more likely for those who attended a health service which saw predominantly Aboriginal clients. For females, re-testing was also more likely if they attended services which used electronic medical records, and for males, re-testing was more likely if they attended a service which employed Aboriginal health workers and more male staff. Studies 2 and 3, involved qualitative interviews with 36 staff working at 22 remote Aboriginal health services. Barriers for staff offering
STI tests included; cultural norms requiring the separation of genders, competing clinical demands, concerns about client confidentiality and limited staff capacity. For timely treatment, barriers included; the large distances between the health centre and laboratories, delays in checking and actioning test results, under-utilisation of recall systems and difficulties in locating clients following receipt of a positive result. The fourth study systematically reviewed the international literature to assess the reach, uptake and outcomes of outreach as a strategy to increase testing coverage. The review revealed that despite the number of people tested being relatively small, the yield of infections was high. The proportion of the target population tested was higher in venues (community centres) but lower in street/public areas. In conclusion, the research in this thesis has identified client, cultural, health centre and systematic factors which impact on regular testing and timely treatment. Many of the barriers identified are modifiable, while others may require innovative strategies to overcome; such as outreach programs, point-of-care testing, and testing modalities outside the clinic.

▼ Chlamydia trachomatis (CT) and Neisseria gonorrhoea (NG) are curable sexually transmissible infections (STIs) which represent a significant public health burden, particularly in young Australia Aboriginal people in remote communities. Prompt testing and treatment is fundamental to
STI control yet distance to urban laboratories is a significant barrier. CT/NG point-of-care (POC) testing offers an ideal solution, but until recently had been unavailable. This thesis aims to identify settings where POC testing could be beneficial, benefits and barriers to implementation and health service staff acceptability.The thesis was based on two sets of qualitative interviews. The first occurred with 18 Australian key informants with remote, sexual health and laboratory expertise, and generated three discrete studies. The second occurred with 16 trained nurses and Aboriginal health workers (AHWs) from the first 12 remote primary care services internationally to use GeneXpert CT/NG POC testing and resulted in study four. Study one focused on settings where the technology would have greatest benefit, with remote Aboriginal communities most commonly identified, as well as juvenile justice and outreach services to highly mobile or marginalised populations. In study two, informants identified the benefits of POC use for clinical practice including improved management of STIs- more timely and targeted treatment, earlier commencement of partner notification, and reduced effort associated with client recall, but noted it will result in changes to the
STI management pathway, and policy and clinical guidelines may need to be altered. Study three focused on the public health implications; with the key perceived benefit being
STI control, and barriers including the potential to negatively impact on disease notification and NG antibiotic sensitivity surveillance. In study four, most nurses and AHWs found the test easy to use and useful, and reported improved management of STIs consistent with the key informant’s perceived benefits. In conclusion, this thesis has provided information to inform implementation of CT/NG POC testing, including selection of appropriate settings, and the need to review clinical guidelines and establish systems to avoid adverse impact on public health surveillance. Importantly the research demonstrated the new technology was highly acceptable to staff working in remote primary care.

An evaluation of the drivers of epidemics in South East Asia and Australia is conducted along with projection of epidemic trajectories according to various scenarios. Economic implications of public health and clinical decisions in HIV and
STI control and care are also assessed.In this thesis mathematical modelling is used in a number of different ways. Modelling was used to measure how effective the implementation of a public health policy was in mitigating the HIV epidemic in Cambodia in the past (Chapter 1). We then used modelling to project possible epidemic trajectories and calculate risk in two very different settings: Cambodia, a low income country where the majority of infections occur due to heterosexual intercourse and Australia, a high-income country with an epidemic predominantly based in the MSM population (Chapters 1, 2, 3 & 6). What can be learned from both settings is: the importance of ensuring that any intervention put into place is contextually appropriate; sentinel and behavioural surveillance are conducted routinely, are maintained and any deficiencies identified and addressed; and educating those whom are at risk about how they are at risk and how to minimise that risk through behaviour change is important.

development and use of an original methodology to measure inflation
credibility, as well as the first results of such measurement in
terms of an inflation credibility barometer. The barometer is an
instrument measuring the degree of acceptance of the accuracy of
historic inflation figures. Despite the lack of knowledge about
inflation and the low inflation credibility recorded by this first
calculation of an inflation credibility barometer for South Africa,
valuable information about inflation is unveiled to the
authorities. The research results serve as a benchmark, but cannot
be compared to earlier research, as this study represents the first
systematic measurement of inflation credibility in South Africa.
The barometer yields better results than the limited current
international measurement of perceptions of the accuracy of
historic inflation figures. The barometer (i) reports the
credibility of inflation figures as a figure between zero and 100;
(ii) will highlight changes in credibility over time with repeated
use; (iii) can be explained easily to the general public; (iv)
provides for international comparison between countries; and (v)
can be used by all countries. The use of inflation credibility
barometers and changes in barometer readings over time can also
serve as an early warning system for changes in inflation
perceptions that might feed through to inflation expectations.
Sampling results used to calculate a South African inflation
credibility barometer show little public understanding of the rate
of inflation. Owing to an increased focus on inflation figures in
countries using an inflation-targeting monetary policy, central
banks entrusted with such a policy should adopt a communication
strategy highlighting the calculation and measurement of the rate
of inflation. This study shows that no generally accepted
international benchmarks for successful central-bank communication
strategies have been developed, but the use of the methodology
developed in this study will assist in the assessment of the
effectiveness of communication strategies. This study makes three
further contributions of significance to available literature on
inflation in South Africa. The first is an analysis of prices
increases and inflation over a period of 85 years (1921 to 2006)
and a selected comparison of salaries and remuneration over a
period of 78 years (1929 to 2006). To this end data sets were
developed for comparative purposes, thereby distinguishing between
perception and reality about the accuracy of inflation figures over
time. As this comparison has not been done before, a methodology
was developed that can be used in future research. Based on these

further contributions of significance to available literature on
inflation in South Africa. The first is an analysis of prices
increases and inflation over a period of 85 years (1921 to 2006)
and a selected comparison of salaries and remuneration over a
period of 78 years (1929 to 2006). To this end data sets were
developed for comparative purposes, thereby distinguishing between
perception and reality about the accuracy of inflation figures over
time. As this comparison has not been done before, a methodology

▼ Introduction: Prior in vitro work has shown that implants restored with aftermarket (AM) abutments are more prone to leakage at the implant-abutment interface (
IAI) than implants seated with their original equipment manufacturer (OEM) abutments. The goal of the current study was to examine the relationship between bacterial colonisation at the
IAI and implant parameters using OEM or AM abutments on identical implants. It was hypothesised that microbial colonisation at the
IAI may increase peri-implant inflammatory cell accumulation and lead to increased bone and soft tissue loss around the AM abutments.
Method: Sixty ø4/3 mm x 10 mm implants were placed into healed mandibular post-extraction ridges in ten mature ewes. Abutments were placed immediately or at 2nd stage surgery after 2 months. Six groups (n=10) were evaluated as follows: Delayed aftermarket abutment (A), delayed OEM abutment (B), immediate aftermarket abutment (C and D), immediate OEM abutment (E and F). Radiographs and microbial sampling were taken at baseline, 2 months, and 4 months. Identification and relative quantification of DNA for oral microbial species was performed using Polymerisation Chain Reaction (PCR) (
IAI Padotest, Switzerland). Mean percent bone- to-implant contact (%BIC) and distance to first bone contact were calculated from two images per implant using Image J software. Linear mixed models were used to look for between group differences and mixed logistic regression models used for the binary outcomes. Analyses were conducted using Stata 14.1 and p<0.05 was considered statistically significant.
Results: While there were more failures of the implants restored with aftermarket abutments (14 vs. 8) this difference failed to reach statistical significance (p=0.17). There was no statistical difference between OEM vs. aftermarket for %BIC, first bone contact, radiographic bone changes, height of soft tissue, bone density, or microbial colonisation. There was no correlation between microbial leakage and implant outcomes.
Conclusion: This animal study found no difference in microbial leakage between OEM and AM abutments. There was no apparent relationship between increased microbial leakage and implant outcomes. Statistical analysis was complicated by the higher-than-expected number of implant failures. While some intriguing trends were noticed, a larger human study is now recommended to further explore this relationship.

▼ Today there are many different Cad-applications on the market for engineers. Because of big building projects it has become more and more important to work with the same application or to have access to a neutral file type to be able to transfer files between different ones. This report studies the single objects and how they react when transferring them between ADT and ArchiCAD via the neutral type IFC. We have also been looking at problems during the transfer between and if there are some connections between these problems. Afterwards we created a neutral object model of a small country house to see if the objects appeared different. The result shows no differences. But looking at the problems that appeared during the transmissions they felt small and no logical. Today it is not possible to save in a neutral type without facing small problems. But with small corrections it would become possible to use IFC without any difficulties. If we get IFC to work properly it would save a lot of time, effort and the possibility to store your projects for the future.

▼ The present study investigated how cognitive determinants and emotional determinants, in particular fears and worries, relate to
STI testing behaviour. A survey was conducted online among 906 participants aged 16 to 26 years who completed a digital questionnaire about sexuality, sexual health,
STI testing behaviours and cognitive and emotional factors potentially influencing testing behaviours. The cognitive factors were specified by an extended health belief model (HBM) and encompassed perceived risk and severity of STIs, perceived pros and cons of
STI testing, and perceived behavioural control over
STI testing behaviour. Emotional factors referred to fears and worries about STIs and
STI testing. All cognitive variables of the extended HBM, as well as fears and worries, were found to be univariately associated with
STI testing behaviour. In multivariate analysis, perceived behavioural control was found to add to HBM variables. As predicted the relation between fears and worries and
STI testing was mediated by cognitive variables, but only the combination of extended HBM variables reduced the association between fears and worries and
STI testing behaviour to non-significance; individual cognitive variables did not significantly mediate this association. Results of this study suggest that emotions influence cognitions in explaining
STI testing behaviour. Fears and worries affect threat perception as well as evaluation of the behaviour, suggesting that threat appraisal and coping appraisal are intricately related and may not work in parallel as suggested by classic social cognitive models. Limitations and implications of the study are discussed.

▼ Introduction: Sexually transmitted infections have caused an immense global burden, of disease
predominantly in developing countries. The impact is felt in reproductive and child health and lately
in the transmission ofHIV. The syndromic flow charts were developed to improve the management
of STI in resource poor settings. They are practical, inexpensive and easy to implement and are
specific for the local epidemiological pattern of micro organisms causing sexually transmitted
infections.
Objective: To determine whether the current flow charts in use for the syndromic management of
STI in Nairobi are within an acceptable level of efficacy.
Methodology: The study was carried out in three City Council clinics in Nairobi, which offer STI
treatment using the syndromic flow charts during the months of November and December 2002.
Patients treated using the syndromic flow charts thereafter had their diagnosis validated against
laboratory tests. Efficacy of the flow charts was assessed using repeat laboratory tests for all-patients
who present with initial positive results. All the data collected were analyzed using SPSS.
Results: Sensitivities of 90, 53.6, 20, 20 and 40, percent were derived for the flow charts that treat
urethritis, vaginitis, PID, GUD and cervicitis respectively. Specificities of27.3, 47.1, 72.5, 87.5 and
72.5% were again calculated for the above syndromic flow charts respectively. The PPV were low at
36, 35.7, 4.3, 5.9 and 8.3%. The most commonly used drugs, norfloxacin and spectinomycin,
presented with a sensitivity of 96.2% each against gonococcal isolates. An overall cure rate of70.4%
was achieved.
Conclusion: The urethral discharge flow chart had a high sensitivity whereas all others had low
sensitivity; it however, had a very low specificity in diagnosis and treatment of STI. Generally, the
chart is not a perfect diagnostic tool for STI and has to be augmented by other methods. The cure
rate achieved was below the acceptable level; though the

Performing business or personal financial
auditing, the State Tax Inspectorate's staff analyzes data,
performs screening and afterwords plans auditing. Therefore,
inspection plans are being made up. This paper analyzes the work
particularity of the Auditing Screening Division of the Šiauliai
County State Tax Inspectorate including challengers of the
non-computerized formation of the auditing plans. Having done the
comparative analysis of products existing on the market, it was
decided to develop a new system. The system is designed,
implemented, tested and the quality evaluation has been carried
out. The information system facilitates the verification planning
process: clearance of data, reporting and data
searches.

▼ Background: Despite having high incidence and prevalence of STIs, the identification and treatment of infection in men who have sex with men (MSM) is difficult in certain areas of the world. Conservative sexual values, criminal punishment for sex workers, and difficulty of engaging MSM in homophobic societies inhibit individuals from seeking treatment or accurately reporting risky behavior. In settings without laboratories, diagnosis is made clinically based on the presence of symptoms, known as syndromic management. If the
STI epidemic is going to be curbed, improvement of
STI surveillance and monitoring is needed due to problems with access, utilization, and sensitivity of
STI reporting.
Aim 1: to calculate the sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of syndromic management of chlamydia, gonorrhea, and syphilis in gay and bisexual MSM (GBMSM) participating in the Anza Mapema study in Kisumu, Kenya.
Aim 2: to estimate the time between specimen collection and treatment of MSM with laboratory and clinically diagnosed sexually transmitted infections at Anza Mapema in Kisumu, Kenya.
Aim 3: to estimate the rate of men who test positive for a curable sexually transmitted infection, but never receive treatment. The rate will be described as failure-to-treat.
Methods: This study is nested within a parent cohort study called Anza Mapema which took place between 2015 and 2017. The sensitivity, specificity, PPV, and NPV were calculated for syndromic treatment. Time-to-treat was calculated as median time in days. Failure-to-treat rates were estimated as a percentage.
Results: The sensitivity of syndromic management at Anza Mapema was estimated to be 8.4% (4.8, 13.5), specificity was 99.3% (98.8, 99.7), PPV was 57.7% (36.9, 76.7) and NPV was 90.7% (89.3, 92.1). Median time-to-treat for syndromic treatment was 0 days (IQR of 0-0) and for laboratory treatment was 130 days (IQR of 92-173). For laboratory and syndromic diagnoses STIs, failure-to-treat rates were 70.3% and 3.1%.
Conclusions: In resource limited settings, syndromic diagnosis is a more effective method for treating STIs than laboratory diagnosis. The process from specimen collection to proper treatment of the infection can possibly lead to a high failure-to-treat rate. Without properly run systems for management where nearly 100% of diagnosed individuals receive proper treatment, laboratory diagnosis may not be very beneficial to treatment.

▼ The prevalence of sexually transmitted infections (STI) is high in the adult populations world-wide but varies between populations and time periods. Since a high proportion of infected individuals are asymptomatic, diagnostic approaches to reduce further transmission and complications are essential.
The three main topics of this thesis are (1) the prevalence of the herpes viruses: herpes simplex type 1 (HSV-1) and type 2 (HSV-2), Epstein-Barr virus (EBV) and cytomegalovirus (CMV) in different populations; (2) the clinical spectrum of genital infection with HSV-2; and (3) the connection between different criteria of cervicitis and female urethritis and a positive test for Chlamydia trachomatis.
Herpes simplex virus type 2 infections, diagnosed by type-specific serology, were common in both STI patients and pregnant women. Of the pregnant women 10% were seropositive for HSV-2, and of female and male STI patients 23% and 12% had HSV-2 antibodies, respectively. Infection with HSV-2 was often asymptomatic and only 41% of seropositive patients had a history of genital herpes. Atypical manifestations, so-called unrecognised infections, were common and are of clinical importance.
Among 112 male patients with urethritis no cases of herpes simplex virus were found. Instead, Epstein-Barr virus was detected by PCR in urethral samples in a significantly higher proportion of the subjects than in the controls (21% vs. 6%). EBV was also detected in 10.5% of cervical samples from young women attending for Cervical Cancer Screening. In a similar proportion of these women, 11.5%, cytomegalovirus was found in the cervical specimens.
In female STI patients a significant correlation with a positive C. trachomatis test was shown for mucopurulent discharge in the cervical portio, for easily induced bleeding from the same locus, and for more polymorph nuclear leucocytes (PMNL) than epithelial cells in the vaginal wet smear. However, no correlation was demonstrated between microscopical cervicitis or urethritis and C. trachomatis.
In conclusion, diagnostic tests for HSV-2 should be performed generously in patients with recurring genital symptoms of unknown cause. The detection of EBV in urethral samples from men with urethritis and the demonstration of EBV and CMV in the cervix of young women support genital transmission of these viruses. Epstein-Barr virus was significantly correlated to male urethritis, which has not been demonstrated previously. However, further studies are needed to elucidate a possible causality between EBV and urethritis. Since an elevated number of PMNL in stained smears from the cervix or the urethra was not correlated with a positive test for C. trachomatis, routine sampling for microscopy from these loci in unselected female STI patients is questionable.

▼ Dans ces dernières années, les Systèmes de Transport Intelligents (STI) ont été considérés comme l'un des domaines de recherche les plus émergents en raison de leur rôle prometteur dans l'amélioration de la gestion du trafic et de la sécurité routière. Les applications coopératives de sécurité, étant les plus cruciales, ont gagné beaucoup d'intérêt. L'efficacité de ces applications dépend largement de l'échange efficace de deux principaux types d'informations. L'information de localisation périodique correspondant à l'information de localisation du voisinage et l'information événementielle qui est transmise en multi-sauts et générée lors de la détection d'une situation d'urgence. En raison de la caractéristique à grande échelle des STI, cette information fait l'objet du problème de congestion dans le réseau. L'objectif de cette thèse est d'assurer un contrôle fiable et robuste des informations de sécurité permettant de réduire la congestion du canal tout en tenant en compte des exigences des applications de sécurité. Nous examinons la diffusion de l'information événementielle en proposant une approche a multi-sauts qui a montré une amélioration de la réception de l'information. Cependant, cette approche reste très sensible à la charge de canal résultant de transmissions de l'information de localisation périodiques. D'autre part, la transmission efficace de l'information événementielle repose essentiellement sur la détection précise des événements de sécurité et en conséquence sur la précision de l'information de localisation. Ainsi, nous proposons un mécanisme de contrôle de l'information de localisation afin de fournir une meilleure précision et limiter la charge du canal. Les approches proposées dans cette thèse ont profondément étudié le compromis entre le respect des exigences des applications de sécurité et la gestion efficace de la congestion dans le réseau véhiculaire.
In the last decades, Intelligent Transportation Systems (ITS) have been considered as one of the most emerging research area due to their promising role in promoting traffic efficiency and enhancing road safety. ITS cooperative safety applications, being the most vital and critical, have gained a lot of attention. The effectiveness of these applications depends widely on the efficient exchange of two main types of information. The periodic awareness corresponding to the one-hop location information of surrounding environment and the multi-hop event-driven information generated at the detection of a safety situation. Due to the large scale characteristic of ITS, this information is expected to be subject to severe congestion which might impact its reliable reception. ▼ Genital inflammation is a key determinant of human immunodeficiency virus (HIV) transmission and may increase the availability of HIV-susceptible target cells at sites of exposure. The African, Carribean, and other black (ACB) community is at increased risk of HIV acquisition in Toronto and several genital conditions that increase HIV risk are also more prevalent, including bacterial vaginosis, herpes simplex virus type-2 (HSV-2) and human papillomavirus (HPV) infection. Therefore, we assessed the impact of these three genital conditions on mucosal immune cells and cytokines in the female genital tract of ACB women. Endocervical cells and cervicovaginal secretions were collected by cytobrush and Instead Softcup, respectively. T cells and dendritic cells were assessed by flow cytometry, proinflammatory cytokines and chemokines by multiplex enzyme-linked immunosorbent assay (ELISA), and the microbiota by 16S ribosomal ribonucleic acid gene sequencing. HSV-2 infection was associated with elevated levels of endocervical HIV target cells (CCR5+ CD4 T cells), Lactobacillus-deficient microbiota was associated with elevated proinflammatory cytokines, prevalent HPV infection was associated with Lactobacillus-deficient microbiota, and HPV clearance was associated with higher numbers of Langerhans cells in the female genital tract. These results suggest multiple ways in which genital conditions are associated with factors that have been shown to influence HIV susceptibility and provides novel targets for future HIV prevention strategies.

▼ Objective: Testing for STIs has been prioritized as part of a comprehensive HIV/AIDS prevention plan. Internet-based studies of STI testing among men who have sex with men (MSM) are efficient methods of recruiting non-clinic samples from diverse geographic areas. However, online survey methods raise unique concerns regarding threats to the validity of study samples and unknown measurement properties. Thus, this dissertation had two aims. The first was to examine methods related to online survey research by evaluating a protocol to detect invalid survey entries and determining the test-retest reliability of online measures of sexual behavior and STI testing. The second aim was to use the validated sample and reliable measures to examine correlates of STI testing in the year prior to the survey. Methods: In Manuscript 1, survey submissions were classified as valid and invalid according to a de-duplication and cross-validation protocol. Logistic regression models were used to determine associations between invalidity and key demographic and behavioral variables. In Manuscript 2, test-retest reliability over one week was evaluated using intraclass correlation coefficients (ICCs) and kappa statistics for measures of sexual behavior, HIV status, HIV testing, and STI diagnoses. Finally, in Manuscript 3, the valid sample from Manuscript 1 and measures that were evaluated in Manuscript 2 were used to examine the clustering and correlates of STI testing behaviors. Results: In Manuscript 1, three components of the protocol for detecting invalid submissions were responsible for identifying the most invalid survey submissions: duplicate IP address, changed eligibility responses, and duplicate payment name. A total of 146 (11.6%) of the submissions were identified as invalid. Invalid submissions had lower odds of reporting HIV testing in the past year. Hispanic/Latino identity, age, and HIV status were also significantly associated with invalidity. In Manuscript 2, counts of sexual partners (three months), HIV status, HIV testing, and STI diagnoses were found to have substantial (0.61-0.80) to almost perfect (0.81-1.00) seven-day test-retest reliabilities, according to commonly used cutpoints. Partner-specific data, however, were only fairly or moderately reliable (0.21-0.60). Finally, in Manuscript 3, a latent class analysis indicated five STI testing classes: no STIs, all STIs, bacterial STIs and hepatitis, bacterial STIs only, and hepatitis only. The largest class was no STIs, indicating that 45.8% of the validated sample had not been tested for STIs in the past year. Predictors of being in a testing class versus no STI testing included age, education, outness about having sex with men, HIV status, and having a sexual partner in the last three months.
