▼ This research was initiated due to the popularity brand community phenomenon is gaining among companies through last several years. Growing number of companies on the market and shortening of the life cycle of products, make companies work hard in order to retain existing customers and gain new ones. Brand communities may provide many benefits to companies such as ability to build customer loyalty, increase of the marketing efficiency and enhancement of a brand. We wish to explore brand communities from two perspectives: inside, factors that may influence brand community members in particular purchase of augmented products and word of mouth communication and outside, the perception of brand communities as a future value by prospective customers.Due to our philosophical considerations and non-geographical nature of brand communities, the research method that we have chosen is quantitative. In order to collect a large amount of primary raw data we have used a survey approach. Therefore the self-administered questionnaire was designed. Two sampling techniques were used in data collection process – convenience and snowball. The survey was distributed via e-mail and Internet. The predetermined respondents were asked to answer questionnaires and distribute them further. During the data collection period 258 respondents were surveyed.The findings of this research suggest that prospective customers, who have a favorite brand, see brand community as a future value, hence may have an incentive to buy products or services of a particular brand. A managerial implication in this case is that brand communities are a good opportunity for a company to build long term relations with existing customers by involving them in brand community activities. Brand community events such as meetings, festivals, and promotional activities also influence an incentive of consumers to buy augmented products or services of a company; hence giving a firm an opportunity to earn additional profits by selling more additional products, paid services or accessories. Word of mouth communication is another valuable asset that companies may use. In our research we have proved that word of mouth inside brand communities may influence its members to buy products or services of another brand under the influence of an opinion leader or the majority. So managers of companies that sell related or supplemented products may jointly organize events, and reach more consumers.

▼ Background: Although cows' milk is believed to be an important dietary component for supporting bone health, limited research has addressed whether cows' milk consumption in childhood can reduce bone fracture risk.
Methods: This was a prospective cohort study of healthy urban children aged 1-10 years. Modified Poisson regression was used to test the association between milk volume and milk fat consumed between 1 and 3 years of age and risk of fracture between 3 and 10 years of age.
Results: Among the 2466 children, 153 sustained at least one bone fracture. Participants drank on average 1.9 cups of cows' milk daily (2.5% average milk fat). Neither milk volume or milk fat consumed in early childhood were associated with fracture risk in later childhood.
Conclusion: Cows' milk consumption in early childhood was not protective against fracture in later childhood. These findings do not support increasing cows' milk consumption for fracture prevention.

▼ Introduction: Epigenetic alterations of microRNA (miRNA) processing genes are a potentially useful cancer biomarker as they may affect tumor suppressive processes via miRNA levels. This analysis focuses on five genes in the primary miRNA processing complex previously linked to cancer in many different
studies and 14 genes known to affect them. The objectives of this study are to examine associations between DNA methylation of these genes and: cancer incidence; cancer prevalence; intakes of green tea polyphenols and selenium; intakes and serum levels of folate and vitamin D; and levels of arsenic, cadmium, lead, and mercury.
Methods: Data were collected for the Department of Veterans’ Affairs Normative Aging Study every 3-5 years from 1999-2013 via demographic/lifestyle interviews, medical record reviews, blood draws, toenail clippings, and X-rays from 686 consenting participants. Blood samples underwent whole-epigenome profiling using the Illumina 450K BeadChip array. Cox models and generalized estimating equations were used to model cancer incidence and prevalence, respectively. Nutrient and heavy metal analyses were conducted via penalized Least Absolute Shrinkage and Selection Operator models and supplemented with regional enrichment tests.
Results: At three CpGs (DROSHA: cg23230564, TNRC6B: cg21034183, and TNRC6B: cg06751583), DNA methylation was associated with cancer development (inversely for cg23230564 and cg21034183, positively for cg06751583,). DNA methylation at one CpG (DROSHA: cg16131300) was positively associated with cancer prevalence while methylation at several hundred CpGs was associated with the micronutrients of interest. Patella and tibia lead were associated with methylation at relatively fewer (56 and six, respectively) CpGs. Other metals of interest were not associated with DNA methylation of miRNA processing genes. Regional analyses identified several nutrient- and metal-associated areas of methylation change.
Discussion: This study identifies two miRNA processing genes that may be useful markers of cancer, one of which may also be a useful marker of cancer progression, which should be validated in future
studies. The nutrients of interest and bone lead may also have substantial functional effects on miRNA processing, which may explain components of their health effects, and should also be explored in future
studies of these exposures.

▼ Background Serious liver disease in infancy causes significant morbidity. Up to 80% of children will eventually require transplantation. This study aims to investigate parent and family responses to the diagnosis of serious liver disease in infancy and to identify family factors that are predictive of the infants’ emotional and behavioural outcomes. Methods The study uses quantitative and qualitative methods. Parents of infants recently diagnosed with serious liver disease completed validated measures of parent stress, family function, impact of the illness on the family, and father engagement, as well as an interview about their experience of the infants’ illness. The measures were repeated after one year, with the addition of the Child Behavior Checklist (CBCL). Results Parents of 42 infants enrolled, and parents of 37 infants completed the study. Illness severity, liver diagnosis other than Biliary Atresia and parent perceptions of greater impact of the infants’ illness on the family predicted poorer infant outcomes. For mothers, the final best-fit model explained 32% of the variation in CBCL (P = .001). Fathers’ best-fit model explained 44% of the variation in CBCL (P < .001). Thematic analysis of the parent interviews revealed six major themes: uncertainty; awareness of the infant’s vulnerability; feelings of isolation; dealing with other aspects of life; the importance of shared experience; and adjustment. The integrated data analysis demonstrated that lack of extended family support, poor family adjustment to the illness, and financial stress are related to greater impact of the illness on the family. Conclusions The study identifies early risk factors for poor emotional and behavioural outcomes for infants with serious liver disease, providing an opportunity for early intervention. Parents who lack support from extended family, who have financial stress, or who report a high impact of the illness on the family, should be referred for psychosocial assessment.

▼ Accidental falls remain an important healthcare issue for older people. We report on three studies examining risk predictors for further falls, falls-related hospital attendances and mortality, and then test the effectiveness of a hospital-based falls prevention intervention. A prospective study of 498 older people who attended an Emergency Department (E.D.) with a fall showed that age 80 years and older was the greatest predictor of further falls, with a 2-fold increased adjusted risk by 5 years (HR 2.00; 95% C.I. 1.42 – 2.82). Mortality following an E.D. presentation with a fall was 19% at 1 year, increasing to 52% by 5 years. Increasing age and assistance with ADLs predicted both ED re-presentation and mortality. Being female and falls due to syncope were protective. The Concord Health and Ageing in Men (CHAMP), is a longitudinal study of 1705 men. Previous history of falls was the most significant predictor of future falls (IRR 3.12; 95% C.I. 2.49 – 3.91) and falls injury hospitalisations at 10 years (HR 1.48; 95% C.I. 1.09 – 1.99) in this cohort. Risk factors for falls included increasing age, disability in ADLs, being single, dementia, having 3 or more comorbidities, polypharmacy and reduced visual acuity. Dementia was associated with 2-fold increased risk of falls injury hospitalisation at 10 years (HR 2.67; 95% C.I. 1.69 – 4.22). Men born in a non-English-speaking country and men who were still working were less likely to be hospitalised die to a fall injury. A randomised controlled trial (n = 81) of a specialist-led CONFABs clinic versus enhanced G.P. coordinated care, showed an increased rate of falls (IRR 2.39; 95% C.I. 1.09 – 5.27) and risk of falls (RR 1.79; 95% C.I. 1.10 – 2.96) at 1 year with the CONFABS clinic intervention. There was no significant difference in the rate of injurious falls or in the number of fractures between the interventions. Compliance with recommendations was similar in both groups, although more falls prevention strategies were recommended to the falls clinic participants. There are shared risk factors for falls, fall hospitalisations and mortality, with increasing age, functional disability and dementia the most important to consider. Falls prevention strategies may be successfully provided in General Practice, supported by specialist risk assessment and recommendations.

▼ Prevention of cardiovascular disease (CVD), a leading cause of death in men and women, is both a global and national public health priority. Prevention efforts have generally focused on well-known lifestyle (e.g., physical inactivity, unhealthy diet, smoking) and metabolic (e.g., overweight/obesity, hypertension, hyperlipidaemia) risk factors. It is also important for public health strategies to consider emerging risk factors, innovative approaches to risk factors, and evidence in middle-aged men and women, to develop effective prevention strategies. This thesis explored innovative aspects of cardiovascular risk in a large cohort of middle-aged and older Australian men and women (“the 45 and Up Study”) by examining: 1) emerging or lesser known risk factors such as raw vegetable intake (Chapter 3), sedentary behaviour (Chapter 4, Appendix 1) and psychological distress (Chapter 5); 2) the single versus joint influence of lifestyle risk factors on incident type 2 diabetes (Chapter 4, Appendix 1) and hypertension (Chapter 5); 3) potential gender differences (Chapters 3-5, Appendix 1), and female-specific behaviours such as breastfeeding (Chapters 6-7). Overall, findings support Australian recommendations for fruit and vegetable intake, physical activity, alcohol intake and infant breastfeeding. While the importance of reducing known risk factors for CVD prevention was evident, the role of raw vegetable intake, sedentary behaviour and psychological distress was inconclusive. Breastfeeding was associated with a lower maternal risk of CVD. Findings confirmed that adopting a cluster of healthy lifestyle behaviours can reduce CVD risk in the middle-aged and older population. Potential gender differences were explored and identified. This thesis contributes to the literature by exploring innovative aspects of cardiovascular risk that are relevant to middle-aged adults, particularly women, as well as informs health care providers, researchers and policy makers.

BACKGROUND: The POSSUM system is used to predict risk of complications following general surgical procedures. This 18-factor instrument has been challenging to apply to most surgical oncology patient populations. Our aim is to develop a simplified scoring system that is highly correlated with the incidence of major complications.

METHODS: We queried a single-institution IRB-approved prospective database from a surgical oncology population from January 2008 to December 2012. We identified patients undergoing complex abdominal or pelvic resections and factors associated with the development of major (Clavien-Dindo ≥ III) complications. Factors not included in the POSSUM system were incorporated into a new scoring system based on univariate correlation with complication rates (Chi-square). Optimal binning generated an ideal cut-off value associated with major complications. A composite scoring system (SOPI) was compared to standard POSSUM predictions using ROC analysis.
RESULTS: We identified 831 patients undergoing pancreatic (23%), hepatic (23%), colorectal (22%), esophagogastric (16%), retroperitoneal (4%), combined (3%), or other type (10%) of resection. Major complications occurred in 17% of patients. Two original POSSUM factors were included in the new SOPI model (cardiac history and EBL). Four factors improved correlation with complication rate: gender (female/male-1/3 points); operation type (retroperitoneal/ pancreatic or rectal/others-4/2/1 points); curative intent (curative/non-curative-1/2 points); and cancer (no cancer/cancer-1/2 points). ROC analysis generated a greater AUC for the simplified 6-factor system than standard 18-factor POSSUM (AUC: 0.676 vs 0.631). Increasing SOPI quartiles higher risk of major complications (5%, 15%, 20%, 29%; p-value < 0.001).
CONCLUSION: The efficacy of POSSUM or P-POSSUM for complex abdominal and pelvic resections remains unclear. The SOPI system, which is comprised of only 6 factors, was equivalent to P-POSSUM for predicting major complication rates (Clavien-Dindo score of at least III) for surgical oncology patients. Validation in a large, independent dataset is necessary before the system can be widely applied.

Data collection was performed by duly calibrated examiners, and a clinical chart consisting of demographic data on dental caries, oral hygiene, and gingival condition was prepared. A tactile-visual examination was conducted using a clinical mirror and periodontal probe. Data compilation and analysis were carried out using a SPSS software. In this analysis the chi-squared test was used for qualitative independent variables. To identify the net effect of treatment, multiple logistic analysis with forward stepwise model selection was performed. Results: The final sample was composed of 106 lesions in the 53 individuals, with mean age of 15.02 years, visible plaque index (VPI) of 23.34%, and gingival blood indices (GBI) of 25.92%. A statistically significant relationship (p = 0.003) was found between initial DMFS and prognosis of white enamel lesion. Conclusions: We observed that past caries experience and dental plaque were the main predictive factors for negative lesion outcomes, demonstrating the need for oral hygiene control through continuing preventive measures

▼ Après la Seconde Guerre mondiale, surtout à partir du début des années 1960, et jusqu’à la fin des années 1980, plusieurs projets de recherche sur le futur ont été menés par des équipes internationales avec pour objectif la promotion d’une « conscience globale », soit un sentiment d'appartenance à une communauté humaine unique partageant un destin commun. Cette thèse examine certains de ces projets, en particulier ceux qui sont menés au sein d’organisations internationales appartenant au système des Nations Unies. Elle démontre que la pratique de la prospective a été, pendant la guerre froide, source de nouvelles conceptions du monde.Les recherches menées se situent à la croisée de plusieurs champs historiographiques dont les trois principaux sont l’histoire du futur, l’histoire intellectuelle transnationale et l’histoire de la globalité. Elles s’articulent autour de la problématique suivante : comment les études sur le futur menées au sein du système onusien pendant la guerre froide ont-elles contribué à faire émerger et à défendre certaines conceptions du global, faisant du futur du monde à la fois une catégorie d’exploration scientifique et une catégorie d’action pour un ensemble d’acteurs internationaux ?
After the Second World War, in particular from the 1960s to the 1980s, several futures studies projects were conducted by international teams. Their goal was to promote the shaping of a « global consciousness » : the sense of belonging to a unique human community sharing a common destiny. This dissertation analyzes some of those projects, particularly those that were conducted within the united nations system. It shows that futurology created new ways of envisioning the world during the Cold War.

▼ Les exploitations de polyculture-élevage sont de plus en plus reconnues au plan international comme limitant les problèmes environnementaux tout en permettant une agriculture productive et économiquement viable. Les incitations à la spécialisation de la PAC et la diminution de la main d'oeuvre agricole les ont néanmoins marginalisées en Europe. Cette thèse vise à éclairer les conditions de maintien d'exploitations de polyculture-élevage durables. Un travail en partenariat avec des agriculteurs, maires et conseillers agricoles a été conduit sur un cas d'étude en zone défavorisée simple française, les Coteaux de Gascogne. Nos travaux ont montré que, malgré une forte variabilité entre exploitations, la polyculture-élevage locale était un bon compromis par rapport à la spécialisation pour une durabilité environnementale et économique. Une analyse des trajectoires passées des exploitations nous a permis d'éclairer quatre types de « chemins pour durer » en polyculture-élevage. Parmi ceux-ci, deux sont apparus pertinents pour le maintien de la polyculture-élevage en contexte incertain : « maximiser l'autonomie » et « diversifier les ateliers ». Sur la base de ces deux types de trajectoires, nous avons co-construit avec les partenaires deux scénarios techniques prospectifs. Pour le type « maximiser l'autonomie », implanter des intercultures fourragères basées sur des légumineuses permettrait de favoriser l'autonomie alimentaire du troupeau en maintenant la fertilité des sols. Pour le type « diversifier les ateliers », finir des génisses permettrait de les valoriser en circuit court. Ces scénarios ont été adaptés puis simulés sur des exploitations locales. Ce type de démarche a permis i) d'impliquer fortement les acteurs locaux via des réunions collectives et ii) une approche prospective originale fondée sur une étude rétrospective intégrant le temps long.
Mixed crop-livestock farms are again attracting worldwide interest, as they are considered to be a good way to limit environmental problems while allowing a productive and economically viable agriculture. The incentives of the Common Agricultural Policy and decreasing workforce availability nevertheless marginalized these farms in Europe. This thesis aims at understanding the conditions for a survival of sustainable mixed crop-livestock farms. A partnership process with farmers, mayors and technical advisers has been led in a French less favoured area, the Coteaux de Gascogne. Our work has shown that even if a wide variability existed between farms, local mixed crop-livestock farming was a good trade-off compared to farm specialization concerning an environmental and economic sustainability. An analysis of farm past trajectories allowed us to enlighten four “paths to last” in mixed crop-livestock farming. Uvod: Preoperativna procena anatomskih parametara i
kliničkih faktora je važna u otkrivanju pacijenata sa povišenim
rizikom za otežanu endotrahealnu intubaciju (OEI). Odnos
hiomentalne distance (Hyomental Distance Ratio - HMDR) je odnos
između hiomentalne distance – HMD (rastojanje od hioidne kosti do
vrha brade) u položaju maksimalne ekstenzije vrata (HMDe) i u
neutralnom položaju vrata (HMDn). Mandibulohioidna distanca (MHD)
je vertikalno rastojanje od gornje ivice hioidne kosti do ravni
donje ivice mandibule u neutralnom položaju vrata. U radu je
ispitivana prediktivna vrednost, senzitivnost i specifičnost
različitih prediktora OEI, sa posebnim osvrtom na HMDR, hiomentalnu
distancu i mandibulohioidnu distancu. Takođe, ispitivano je
postojanje eventualnih razlika u senzitivnosti i specifičnosti HMDR
i drugih prediktora OEI u zavisnosti od grane hirurgije. Osnovna
hipoteza istraživanja bila je da vrednost HMDR manja ili jednaka
1.2 i mandibulohioidna distanca predstavljaju značajne prediktore
otežane endotrahealne intubacije u svim granama hirurgije.
Metodologija: Istraživanje je obuhvatilo 262 pacijenta koji su
podvrgnuti elektivnim hirurškim intervencijama u Kliničkom Centru
Srbije (KCS) u periodu od aprila 2011. godine do aprila 2012.
godine, a operisani su na Klinikama za urologiju, abdominalnu
hirurgiju, ginekologiju, endokrinu hirurgiju i
otorinolaringologiju. Pored HMDR, MHD, HMDe i HMDn ispitivani su
Mallampati skor i Indeks telesne mase (Body Mass Index - BMI). Kao
pokazatelj OEI ispitivane su vrednosti HMDe<5.3 cm, HMDn≤5.5 cm,
MHD<2.0 cm, MHD>3.5 cm i HMDR≤1.2. Osnovni ishodi koji su
praćeni su: učestalost otežane vizuelizacije larinksa i OEI.
Procena postojanja otežane vizuelizacije larinksa i OEI je vršena
na osnovu klasifikacije Kormaka i Lihena (Cormack, Lehane) čiji
stepeni III i IV su prediktori OEI...

▼ Concomitant with the obesity epidemic, the consumption of sugar-sweetened beverages (SSBs) has increased over the same period. There is an increasing debate as to the potential association between SSB consumption and obesity. Recommendations to limit SSB consumption have been widely acknowledged. Finding suitable beverage alternatives for SSBs may be particularly beneficial in reducing SSB consumption and curbing its associated health outcomes. However, evidence on the long-term effects of replacing SSBs with other beverage types is sparse. By using an epidemiological approach, the current thesis aimed to examine the associations between SSB consumption, the substitution of SSBs by other beverage types, and long-term changes in obesity outcomes in cohorts of Danish and Australian children. Analyses were undertaken in three datasets: the European Youth Heart Study, the Childhood Asthma Prevention Study, and the Healthy Start Study. Multivariate linear regression models were used to assess associations between SSB consumption and changes in obesity outcomes. Substitution models were performed to evaluate the association between substitutions of SSBs with other beverage types on obesity outcomes. Positive associations of SSB consumption and long-term changes in obesity outcomes were consistently found in all three cohorts. Significant associations between substitutions of SSBs by water, milk, or artificially-sweetened beverages with obesity outcomes were also revealed. Results of the current thesis confirm a positive association between SSB consumption and increases in long-term obesity outcomes in prospective cohorts of Danish and Australian children. Moreover, water, milk or artificially-sweetened beverages were found to be better alternatives for SSBs in regard to obesity development. The findings of this thesis are particular relevant for health researchers and policy makers in delivering practical recommendations towards SSB consumption and obesity prevention.

▼ Objetivo: O presente estudo tem como objetivo avaliar a situação atual de lesões brancas de cárie, bem como a influencia dos fatores preditores no prognóstico dessas lesões, diagnosticadas há 6 anos, sem intervenção clínica. Métodos: Um estudo do tipo prospectivo avaliou 53 estudantes da rede pública da cidade de Natal/Brasil, entre 13 e 18 anos, de ambos os sexos. A coleta de dados foi realizada por um examinador devidamente calibrado, com o auxílio de uma ficha clínica composta por dados demográficos, como também de cárie dentária e higiene oral. O exame executado foi do tipo tátil-visual, com a utilização de espelho clínico e sonda periodontal, sendo registrado para a cárie dentária ( CPO-S,Ncárie e CPOCEO) e para a higiene oral (IPV e ISG). A tabulação e a análise dos dados foram realizadas no software SPSS. Nesta análise foi utilizado o teste de associação Qui-Quadrado para as variáveis independentes qualitativas. Para a identificação do efeito líquido, foi realizada a análise de regressão logística múltipla com uma modelagem do tipo stepwise forward. Resultados: A amostra final foi composta por 106 lesões de 53 indivíduos, com média de idade 15,02 anos, com índices elevados de placa visível (IPV 23,34%) e de sangramento gengival (ISG 25,92%). Foi verificada uma relação estatisticamente significativa (p = 0,003) entre as variáveis CPO-S2 inicial e o prognóstico das lesões brancas. Conclusão: Foi observado que uma maior experiência passada de cárie somada a presença de biofilme dentário são os principais fatores preditores para o desfecho negativo das lesões, demonstrando a necessidade de um controle contínuo da higiene oral e da aplicação constante de medidas preventivas para o sucesso clínico, que seria a inativação das lesões brancas de cárie
Objective: The aim of the present study is to assess the current situation of white enamel lesions on vestibular surfaces of permanent upper incisors, diagnosed 6 years ago, without clinical intervention. Methods: A prospective study reassessed 53 students of both sexes, aged between 13 and 18 years old, all attending the public school system in Natal, Brazil. Data collection was performed by duly calibrated examiners, and a clinical chart consisting of demographic data on dental caries, oral hygiene, and gingival condition was prepared. A tactile-visual examination was conducted using a clinical mirror and periodontal probe. Data compilation and analysis were carried out using a SPSS software. In this analysis the chi-squared test was used for qualitative independent variables. To identify the net effect of treatment, multiple logistic analysis with forward stepwise model selection was performed. Results: The final sample was composed of 106 lesions in the 53 individuals, with mean age of 15.02 years, visible plaque index (VPI) of 23.34%, and gingival blood indices (GBI) of 25.92%. ▼ Nesta pesquisa foi estudada a prática de prospecção do futuro (estudos do futuro) como suporte à busca de informações para a decisão empresarial. Dentre os objetivos da pesquisa se encontram a busca de associações entre os métodos de prospecção e as diferentes categorias de informação, a avaliação da importância atribuída à atividade de prospecção, a identificação da frequência de uso e dos horizontes de tempo para os quais os métodos são utilizados, bem como a obtenção do grau de satisfação percebida com os resultados obtidos. A pesquisa é realizada por metodologia mista, quantitativa e qualitativa. Foram feitos levantamento de dados quantitativos e estudos de casos, e os resultados foram analisados de forma complementar. Na amostra de respondentes foi identificado que há associação entre a categoria da informação que está sendo prospectada e os métodos de estudos do futuro. Determinados métodos são mais utilizados para algumas categorias de informação, a exemplo do método de Cenários para buscar informações sobre Política e Sociedade. Por outro lado, determinadas combinações apresentaram menor uso, como o método Roadmap Tecnológico para buscar informação sobre Tecnologia. É atribuída elevada importância para a prospecção do futuro. Tal avaliação depende do tipo de decisão ser estratégico ou tático, e este último fator também influencia a frequência em que a atividade é praticada. O horizonte de tempo da prospecção também está associado aos métodos, sendo que Cenários apresenta maior uso para horizontes de mais de cinco anos. Há métodos que apresentam maior satisfação dos respondentes da amostra, sendo eles: Cenários, Previsões e Projeções de Executivos e Pesquisas de Mercado. Os métodos mais praticados são os que apresentam a possibilidade de interatividade pessoal, como Cenários e Previsões e Projeções de Executivos. Por outro lado, os métodos mais objetivos, como os que utilizam modelos matemáticos e os que possuem metodologia mais estruturada, não figuraram entre os mais praticados. Nos casos estudados foi identificado que os métodos mais praticados são baseados em opinião e julgamento e utilizam a interatividade pessoal como principal forma de condução. Houve também preocupação manifestada sobre a necessidade de estruturação da atividade nas empresas e sobre a prática da mesma seguindo recomendações metodológicas.
The use of future studies methodologies to provide information for decision making has been studied in this research. The objectives of the research are to analyze the association of methods to different information categories, to evaluate the importance of future studies, to identify the frequency of use of future studies and the time horizon in which the methods are used in searching information, and to gather users perceived satisfaction with the results. ▼ INTRODUÇÃO: O dispositivo endoscópico temporário de exclusão duodenojejunal (DETEDJ) promove exclusão funcional do duodeno e da porção inicial do jejuno, impedindo seu contato com o quimo. Em estudos prévios, mostrou-se eficaz na perda de peso e apresentou resultados promissores no controle do diabetes mellitus tipo 2 (DMT2). O objetivo deste estudo foi avaliar a eficácia e a segurança do DETEDJ, mantido por 12 meses, na perda de peso e controle do DMT2. MÉTODOS: realizou-se estudo de coorte prospectivo, aberto, incluindo 22 pacientes com obesidade (IMC 35 kg/m²) e DMT2, candidatos a cirurgia bariátrica, os quais foram submetidos a colocação do DETEDJ, mantido por 12 meses, e com seguimento por mais 6 meses após sua retirada. RESULTADOS: o DETEDJ foi implantado com sucesso em todos os pacientes. Treze indivíduos completaram 12 meses com o dispositivo e 12 retornaram 6 meses após a retirada. Na análise por intenção de tratamento foram estudados todos os pacientes com o dispositivo implantado (n = 22). Comparando o momento inicial com a última aferição antes da retirada do dispositivo, houve redução significativa do peso (119,4 ± 23 kg vs. 102,1 ± 18,7 kg p < 0,001), do IMC (45,3 ± 7,1 kg/m2 vs. 38,7 ± 5,7 kg/m2 p < 0,001), da glicemia de jejum (179,4 ± 68,8 vs. 139 ± 58,1 mg/dL p < 0,001) e da HbA1c (8,9 ± 1,7% vs. 6,9 ± 1,2% - p < 0,001). Dezesseis pacientes apresentavam HbA1c controlada ( 7%) no momento da retirada, contra apenas um no início do estudo. Na análise por protocolo, foram estudados os 12 pacientes que completaram o seguimento de 6 meses após a retirada. A perda de peso e o controle glicêmico obtidos com o DETEDJ estavam mantidos no final do acompanhamento (peso, IMC e HbA1c p < 0,001 comparado com tempo basal; glicemia de jejum p = 0,001 comparado com tempo basal). Houve redução significativa dos níveis de triglicerídeos, colesterol, insulina e peptídeo C ao término do tratamento com o DETEDJ (análise por intenção de tratamento). CONCLUSÃO: o DETEDJ mantido por um ano é seguro e eficaz para perda de peso e controle do DMT2. O seguimento por um período de seis meses após sua retirada demonstra manutenção dos benefícios obtidos
BACKGROUND: endoscopic duodeno-jejunal bypass liner (DJBL) promotes functional exclusion of duodenum and initial jejunum, preventing mucosal contact with chyme. Previous studies have shown effective weight loss and promising results on type 2 diabetes mellitus (T2D) improvement. The aim of this study was to evaluate efficacy and safety of DJBL, maintained for 12 months on weight loss and T2D control. METHODS: open-label, cohort, prospective study, including 22 patients with obesity (BMI 35 kg/m²) and T2D, candidates for bariatric surgery, submitted to DJBL implant maintained for 12 months, and with 6-months follow-up after removal. Métodos: Estudo clínico prospectivo transversal, que avaliou o efeito da ação tópica do colírio de fenilefrina a 10% e da manobra de levantamento manual da pálpebra no posicionamento palpebral de indivíduos portadores de ptose palpebral involucional unilateral ou bilateral. Os pacientes foram filmados antes e 5, 10 e 15 minutos após a instilação do colírio. A distância margem reflexo (DMR1) foi medida com a utilização do software Image J e os resultados foram analisados com o modelo linear de efeitos mistos. Resultados: Na pesquisa foram incluídos 70 indivíduos entre 44-86 anos, 64 destes do sexo feminino (91.43%), divididos em três grupos: indivíduos com ptose unilateral; com ptose bilateral e controles. O olho submetido à instilação do colírio de fenilefrina apresentou aumento da DMR1 nos primeiros 10 minutos: de 1,33±0,66 mm para 2,06±0,89 mm (grupo ptose unilateral), de 1,26 ± 0.63 mm para 2,29±0,86 mm (grupo ptose bilateral) e de 3,12 ± 0,68 mm para 4,06 ± 0,92 mm (grupo controle). A DMR1 diminuiu nos olhos contralaterais dos pacientes portadores de ptose, mais significativamente após a instilação do colírio de fenilefrina: teste de fenilefrina vs. levantamento manual da pálpebra = 18.9% vs. 17.2% de redução no grupo com ptose unilateral, e 13.6% vs. 10.7% de redução no grupo com ptose bilateral. Os fatores: dominância ocular, uni ou bilateralidade da ptose e a sua intensidade não influenciaram na resposta aos testes. Conclusões: O aumento da DMR1 ocorreu nos olhos testados de todos os grupos. Tanto o levantamento manual da pálpebra como a instilação do colírio de fenilefrina 10% agiram na pálpebra superior do olho contralateral, porém a resposta foi mais significativa após o teste da fenilefrina e nos grupos com ptose, unilateral e bilateral
Purpose: The primary aim of this study is quantify and compare the effect of 10% phenylephrine instillation and manual elevation (ME) on the upper eyelid position of the tested eye and the contralateral eye in patients with involutional blepharoptosis (IB). Gestational diabetes (GDM) indicates increased risk for diabetes and the metabolic syndrome in women. Research on prenatal exposure to GDM as a risk factor for metabolic diseases is conflicting. Overweight (body mass index ≥ 25 kg/m2) is a strong risk factor for GDM and metabolic diseases; however, there are few published previous studies distinguishing the separate effects of overweight and GDM on the later risk for metabolic diseases in women and their children.
The present study evaluated pre-pregnancy overweight and GDM as determinants of long-term risk for diabetes and hypertension in women, and the metabolic consequences of prenatal exposures to maternal pre-pregnancy overweight and different types of maternal diabetes in children. The results are based on prospective, clinical data from Oulu University Hospital (n = 63 mothers and their children), and the Northern Finland Birth Cohort 1986 (NFBC 1986, n = 9,362 mothers and their 9,479 children).
Compared to normal-weight mothers with normal glucose tolerance in pregnancy, the NFBC 1986 mothers with simultaneous pre-pregnancy overweight and GDM had strikingly high risks for developing diabetes (hazard ratio, HR 47.2; 95% confidence interval 25.5–87.4) and hypertension (HR 9.2 [6.1–13.9]) twenty years after delivery. The risks for these diseases were elevated in mothers with pre-pregnancy overweight even when they had normal glucose tolerance during pregnancy (HR diabetes 12.6 [7.4–21.6], HR hypertension 2.9 [2.1–3.9]). GDM per se indicated increased risk only for diabetes (HR 10.6 [4.2–27.0]).
In the cohort from Oulu University Hospital, increased fasting insulin concentration (P = 0.04), first phase insulin response (P = 0.03), and HOMA-B (P = 0.008) were already observed at pre-school age in the offspring of mothers with Type 1 diabetes compared with offspring of mothers with GDM.
In the NFBC 1986 offspring, the prevalence of metabolic syndrome was 2.4% at age 16 years, using the International Diabetes Federation pediatric definition. Abdominal obesity, a waist girth over half one’s length, defined approximately 85% of the adolescents with metabolic syndrome. The risks for overweight and abdominal obesity were high in those with prenatal exposure to both maternal pre-pregnancy overweight and GDM (odds ratio for overweight 4.1 [1.9–8.6], for abdominal obesity 3.8 [1.7–8.8]). In children of normal-weight women, prenatal exposure to GDM was not associated with increased risk of these outcomes.
Based on this study, preventing and reducing overweight in fertile age seems to be a key target for preventing metabolic diseases in women and their children.

▼ Background: Analysis of and intervention for movement dysfunction, especially walking dysfunction, is the central expertise of physical therapists (PTs). PTs are called to establish a diagnosis prior to making patient management decisions yet no valid classification system for walking exists. Objectives: The purpose of this study was to determine if locomotor experts could arrive at consensus on the validity, mutual exclusivity and understandability of diagnostic classifications for bipedal locomotion. Methods: An electronic mail Delphi survey methodology was utilized. Experts were recruited purposefully through research and clinical leaders in the PT profession. Identified experts were then asked for their recommendations in the snowball phase of recruitment. In the first survey, the panelists were asked if any of the 15 original classifications should be added removed, reworded, or merged. In Rounds 2 and 3, following a summary of responses from the previous round, panelists were asked to rate the validity, mutual exclusiveness and understandability of each original and modified classification using a 4 point Likert Scale as well as to comment on them. Constant comparative analysis was used to analyze qualitative data and non-parametric statistics was used for ordinal data (p < 0.05). Consensus was defined as: 1) greater than 75% of participants agree or strongly agree that a classification is valid, mutually exclusive and understandable, 2) no difference between Rounds 2 and 3 responses, 3) Kappa coefficients are > .60 and 4) there is a reduction in the percentage of panelists who comment as well as a convergence of themes between Rounds 1 and 3. Results: A total of 287 participants were invited initially and 58 experts participated in all rounds. Full consensus was reached for 5 of the modified diagnostic classifications and partial consensus for 6. There were no significant differences between Rounds 2 and 3 ratings and there was a decrease in the percentage of panelists who made global comments between Rounds 1 and 3. Other measures of consensus did not reflect full consensus. Conclusions: This study provides initial validation for several bipedal locomotor classifications and provides a model for development of diagnostic classification systems for physical therapist practice.

▼ Basal cell carcinoma (BCC) is a skin cancer of particular importance to the Australian community. Its rate of occurrence is highest in Queensland, where 1% to 2% of people are newly affected annually. This is an order of magnitude higher than corresponding incidence estimates in European and North American populations. Individuals with a sun-sensitive complexion are particularly susceptible because sun exposure is the single most important causative agent, as shown by the anatomic distribution of BCC which is in general consistent with the levels of sun exposure across body sites. A distinguishing feature of BCC is the occurrence of multiple primary tumours within individuals, synchronously or over time, and their diagnosis and treatment costs contribute substantially to the major public health burden caused by BCC. A primary knowledge gap about BCC pathogenesis however was an understanding of the true frequency of multiple BCC occurrences and their body distribution, and why a proportion of people do develop more than one BCC in their life.
This research project sought to address this gap under an overarching research aim to better understand the detailed epidemiology of BCC with the ultimate goal of reducing the burden of this skin cancer through prevention. The particular aim was to document prospectively the rate of BCC occurrence and its associations with constitutional and environmental (solar) factors, all the while paying special attention to persons affected by more than one BCC. The study built on previous findings and recent developments in the field but set out to confirm and extend these and propose more adequate theories about the complex epidemiology of this cancer.
Addressing these goals required a new approach to researching basal cell carcinoma, due to the need to account for the phenomenon of multiple incident BCCs per person. This was enabled by a 20 year community-based study of skin cancer in Australians that provided the methodological foundation for this thesis. Study participants were originally randomly selected in 1986 from the electoral register of all adult residents of the subtropical township of Nambour in Queensland, Australia. On various occasions during the study, participants were fully examined by dermatologists who documented cumulative photodamage as well as skin cancers. Participants completed standard questionnaires about skin cancer-related factors, and consented to have any diagnosed skin cancers notified to the investigators by regional pathology laboratories in Queensland. These methods allowed 100% ascertainment of histologically confirmed BCCs in this study population. 1339 participants had complete follow-up to the end of 2007. Statistical analyses in this thesis were carried out using SAS and SUDAAN statistical software packages. Modelling methods, including multivariate logistic regressions, allowed for repeated measures in terms of multiple BCCs per person. ▼ BACKGROUND: Although alcohol use disorders (AUDs) have enormous public health
consequences, the rate of diagnosis of AUDs remains unsatisfactorily low. The
primary aim of this study was to compare the detection of AUDs by
anesthesiologists in a large preoperative assessment clinic to that by
computerized self-assessment of the Alcohol Use Disorder Identification Test.
Secondary outcome measures were to compare the actions taken by
anesthesiologists upon a finding of an AUD. METHODS: One thousand five hundred
fifty-six patients were included. Before preoperative assessment, patients
were asked to complete the Alcohol Use Disorder Identification Test (positive
scores: men > or = 8, women > or = 5) using a computer. The authors performed
a retrospective chart analysis of the anesthesiologists' actions upon a
finding of an AUD. The anesthesiologists were blinded to the results of the
computer-based assessment and to the subsequent chart analysis. RESULTS: The
prevalence rate of AUDs determined by the anesthesiologists was 6.9% (107 of
1,556), whereas the proportion of patients positive for an AUD using the
computerised Alcohol Use Disorder Identification Test was 18.1% (282 of 1,556)
(P < 0.001). The detection rate by the anesthesiologists of AUDs among men was
significantly higher than among women (P < 0.001) as well as in the elderly
compared with younger patients (P < 0.001). Action taken by anesthesiologists
was mainly based on evaluating quantity of alcohol consumption. CONCLUSION:
The computer-based self-assessment increases detection rates of AUDs in busy
settings such as a preoperative assessment clinic. Prevalence rates of AUDs
are underestimated. Best-practice guidelines for detection of AUDs are not
implemented in the daily clinical routine. Barrier analysis is urgently
required.

▼ 본 연구는 국내에서는 최초로 성폭력 피해 여성들을 종단적으로 추적하여 성폭력 피해 이후의 심리적 후유증 변화 양상과 심리적 증상을 매개하는 심리적 변인들을 탐색하였다. 이를 위해 아주대학교병원의 「여성, 학교 폭력 피해자 One-Stop 지원센터」에 방문한 성인 성폭력 피해자들 중 정신건강의학과에 내원한 자들을 대상으로 동의하에 연구를 실시하였으며, 총 세 번의 평가가 이루어졌다. 70% 이상의 피험자가 사건발생 1개월 이내에 평가가 시작되었다. 1차 평가에서는 개인적 정보 및 사건 특성 정보를 수집하고, 사건 후의 심리적 증상, 죄책감/수치심 경향성, 외상에 대한 인지적 요인, 사회적지지, 전반적인 인지기능을 측정하였다. 2차 평가에서는 자기보고 형식의 질문지 및 임상가 면접을 통해 외상 후 스트레스 장애 증상 및 우울 증상의 변화를 평가하였고, 3차 평가에서는 임상가 면접을 통한 임상가를 위한 외상 후 스트레스 장애 진단 척도를 시행하였다.
본 연구에는 총 44명의 성폭력 피해자가 참여하였으며, 9명이 중도 탈락하여 최종 35명이 분석에 포함되었다. 이 중 2차, 3차 평가에서 한 번이라도 PTSD 진단 기준을 만족하는 경우 Persistent-PTSD(P-PTSD) 집단으로 분류되었고, 2차, 3차 평가에서 모두 PTSD 진단 기준을 만족하지 않았을 경우 Recovered-PTSD(R-PTSD) 집단으로 분류되었다. 최종 분석에 포함된 35명 중 21명은 P-PTSD 집단, 14명은 R-PTSD 집단으로 분류되었다. P-PTSD 집단과 R-PTSD 집단의 1차 평가에서의 심리적 증상을 비교해 본 결과, 전반적인 우울, 불안, PTSD 증상이 P-PTSD 집단에서 더 높은 것으로 나타났다. 또한 사건에 대한 귀인 양식 또한 전반적으로 부정적인 것으로 나타났다.
1차 평가와 2차 평가를 모두 시행한 24명의 연구 대상자 중 1차 평가 이후 1개월 뒤에 시행된 2차에서 실시한 임상가를 위한 외상 후 스트레스 장애 진단 척도(CAPS)에서 외상 후 스트레스 장애(PTSD) 진단 기준을 만족한 경우 PTSD 집단으로 분류하였고, 그렇지 않은 경우는 Non-PTSD 집단으로 분류하였다. 17명은 PTSD 집단에 해당되었고, 7명은 Non-PTSD 집단에 해당되었다. 외상 이후 초기 1개월 간의 PTSD 증상의 변화 양상을 살펴본 결과, 두 집단에서 대부분의 PTSD 증상의 회복이 유의미한 수준으로 일어나는 것으로 나타났다. 한편, 회피 증상에서만 집단과 시간 간의 상호작용 효과가 나타났고, 이는 Non-PTSD 집단이 PTSD 집단에 비해 회피 증상의 회복이 초기에 더 빠른 속도로 이루어짐을 시사한다. 또한 외상 초기 평가에서는 성폭력 피해자의 PTSD 증상과 세상과 자신에 대한 부정적 인지 양식, 죄책감/수치심 경향성이 대부분 유의미한 상관이 있는 것으로 나타났으나, 초기 평가에서 측정한 세상에 대한 부정적 인지 양식만이 1개월 이후의 PTSD 증상과 유의미한 상황이 있는 것으로 나타났다.
사건 직후의 초기 평가에서 측정한 심리적 증상, 인지 요인, 인지 기능과 사건 발생 5-6개월 이후의 PTSD 증상과의 관련성을 살펴본 결과, 초기 평가에서의 PTSD 증상과 외상에 대한 부정적인 귀인 양식, 청각기억 검사(KAVLT)의 지연 회상 점수가 사건 발생 5-6개월 이후의 PTSD 증상 심각도와 유의미한 상관관계가 있는 것으로 나타났다. 단계적 중다회귀분석 결과, PTSD 증상의 회피 증상과 청각기억 검사(KAVLT)의 지연 회상 점수가 결합된 모형이 50.3%의 설명력으로 사건 발생 5-6개월 이후의 PTSD 증상의 심각도를 가장 잘 예측하는 것으로 나타났다. 이는 외상 발생 초기에 회피 증상을 높게 보이고, 청각기억 검사에서의 수행 수준이 낮을수록 5-6개월 이후의 PTSD 증상 심각도가 높음을 의미한다.
결론적으로, PTSD 증상이 장기간 지속되는 집단이 단기간에 회복을 보이는 집단에 비해 외상 초기에서부터 우울, 불안, PTSD 증상의 심각도가 높은 것으로 나타났다. 대부분의 성폭력 피해자가 1개월 간 심리적 증상이 회복되는 양상을 보였으며, 그 중에서도 PTSD 증상이 단기간에 회복되는 집단의 경우에는 장기간 지속되는 집단에 비해 회피 증상의 회복이 더 빠른 속도로 초기에 일어났다. 또한 이러한 PTSD 증상의 회복 과정에서 외상과 관련된 인지 양식 중에서도 세상에 대한 부정적인 인지 양식의 영향력이 높은 것으로 나타났다.

▼ Le concept d’anthropologie prospective élaboré dans les années 1950 par Gaston Berger, fondateur de la prospective en France, n’a pas eu la postérité qu’il mérite. Il a été contourné par les prospectivistes, qui ont conduit la prospective dans les parages du marketing, et oublié par les philosophes, qui font trop rarement de l’avenir un véritable sujet de réflexion. Or au début du XXIe siècle, après la prise de conscience de l’entrée de l’humanité dans l’ère de l’anthropocène, ce concept représente peut-être le meilleur fil conducteur pour l’élaboration de la toute nouvelle responsabilité prospective qui pourrait faire contrepoids aux stratégies suicidaires des principaux acteurs de la civilisation occidentale – stratégies exclusivement guidées par les objectifs de puissance technologique ou militaire, de croissance économique ou de recherche de profit à court terme – stratégies qui sacrifient ostensiblement l’avenir de l’humanité au bénéfice du présent ou du court terme. La présente thèse cherche à dessiner l’infrastructure conceptuelle de cette responsabilité prospective qui reste à imaginer et à structurer. La Table des Situations Stratégiques esquissée ici représente un premier pas dans cette direction. Elle ambitionne d’aboutir à une grille d’analyse et d’évaluation des stratégies déployées par les principaux acteurs de la civilisation occidentale sur la surface du globe. Elle vise à consolider l’idée d’un « permis d’engager l’avenir de l’humanité », c’est-à-dire l’idée d’un contrôle démocratique de la pertinence des stratégies des entreprises transnationales ou des gouvernements, sur tous les sujets qui concernent l’avenir à long terme de l’humanité.
The anthropological concept of perspective futures developed in the 1950’s by Gaston Berger (the founder of future studies in France) was not accorded the posterity it deserves. It was overlooked by prospectivists that led future studies into marketing gimmicks and forgotten by philosophers that rarely engage in true reflection on the subject. Or during the beginning of the 21st century, after becoming aware of the human’s entry into Anthropocene era this concept represent possibly the best guide in preparing all the new prospective responsibility that could counter dangerous strategies of major actors of western civilization-strategies that are exclusively guided by military force or technological advancement, economic growth or the pursuit of short term profit-strategies that sacrifices ostensible the future of humanity or the benefit of the present or of the short term. The present thesis attempts to design conceptual framework of this prospective responsibility that rest on imagination. The foundation of the strategic situation initially sketched out here represents the first step in that direction. ▼ Face aux enjeux pour le futur posés par l’ampleur des dérèglements écologiques (changement climatique, extinction des espèces, pressions sur les ressources en eau…), les discours sur l’avenir se multiplient, qu’ils promettent une résolution des problèmes par la technologie, des modèles de société utopiques ou considèrent l’effondrement comme inéluctable. Ces discours sur l’avenir ont-ils une influence sur les situations concrètes de gestion de l’environnement ? Peuvent-ils soutenir ou contraindre l’action des acteurs cherchant à améliorer l’état des écosystèmes ? Cette thèse traite ces questions en s’intéressant aux démarches prospectives et à leur contribution à des stratégies d’acteurs défendant une prise en charge efficace des enjeux environnementaux. Grâce à huit études de cas, elle analyse comment des concepteurs d’interventions prospectives conçoivent ces démarches pour qu’elles fournissent des prises pour l’action d’acteurs d’environnement (agences de l’eau, gestionnaires d’espaces protégés, associations…). Les démarches prospectives influencent la « fabrique des futurs », c’est-à-dire la compétition entre différentes représentations de l’avenir des systèmes sociotechniques qui impose ou exclut certaines trajectoires de changement. Faire exister dans la fabrique des futurs des discours sur l’avenir crédibilisant une transformation de ces systèmes favorable aux enjeux écologiques fournit une ressource susceptible de renforcer le pouvoir d’acteurs d’environnement. Les concepteurs de prospectives travaillent à des reconnexions entre les discours sur l’avenir produits lors de leurs démarches et les processus de gestion de l’environnement. Ces reconnexions peuvent fournir, non sans difficultés, des prises sur les jeux d’acteurs, l’élaboration d’instruments de gestion de l’environnement et le fonctionnement des organisations impliquées. La thèse propose un cadre d’analyse pour accompagner la réflexion des concepteurs prospectifs dans la conception et la mise en œuvre de leurs démarches. Plus largement, elle ouvre des pistes de réflexion sur les enjeux stratégiques de la production de discours sur l’avenir pour participer aux transitions des systèmes sociotechniques.

▼ Atherosclerosis is the underlying cause of myocardial
infarction (MI) and numerous environmental and genetic factors
contribute to its development. Despite advances in our
understanding of atherogenesis, many uncertainties on the
relationship between diet and cardiovascular disease (CVD) risk
remain. Chapter 1 reviews the biologic mechanisms for the effects
of dietary fats on CVD risk, including effects on serum lipids,
vascular function, chronic inflammation, and thrombosis.; Chapters
2 and 3 examine the effects of sex, ethnicity, and genetic
variation on the associations between specific types of dietary fat
and CVD risk and atherosclerosis. Among 53,120 Latino and
African-American men and women in the Multiethnic Cohort Study,
greater intakes of monounsaturated fat and marine-derived
long-chain n-3 polyunsaturated fatty acids were associated with
lower risk of MI. Greater intakes of saturated fat and n-6
polyunsaturated fat were associated with higher risk of MI among
Latinos. Among 1,827 healthy subjects who participated in one of
four randomized controlled atherosclerosis trials, atherosclerosis
measured using carotid intima-media thickness (CIMT) was not
associated with promoter polymorphisms for 5-lipoxygenase, an
inflammatory gene in the overall sample. However, there were
significant gene-environment interactions such that homozygosity
for shorter 5-lipoxygenase promoter alleles was atherogenic among
users of lipid-lowering medication or individuals with low dietary
intake of linoleic acid, the intermediate-chain n-6 polyunsaturated
fatty acid that comprised the majority of energy intake from
polyunsaturated fats. Taken together, these findings suggest that
nutritional recommendations for prevention of cardiovascular
disease may need to account for differences in the relationship
between diet and cardiovascular disease across population
subgroups. Further investigation is needed to understand the
interactions between diet, inflammation, and other factors
underlying atherosclerosis and cardiovascular disease.; Chapter 4
validates Multiethnic Cohort Study self-reports of
physician-diagnosed MI and other conditions through comparison with
a statewide database of California hospital discharge diagnoses.
The sensitivity of self-reports ranged from 59 to 93% and was
higher for chronic conditions (such as hypertension, diabetes, and
cancers) than for acute events (such as MI, stroke, hip fracture,
and cholecystectomy). Sensitivity of self-reports also varied by
sex, ethnicity, age, education, length of the hospital stay, and
illness severity.

▼ Neonates classified as high and low risk for developmental complications were examined in a hospital clinic at six months of age. Among several measures of infant development, weight was selected for a study of dropout effects. At the six month clinic visit, 85 of 193 high risk and 75 of the 194 low risk neonates selected were still in the study. The overall dropout rate was 59%. In order to assess the effect of dropouts upon six month weight comparisons between high and low risk infants, the following three techniques were used to estimate missing weight values: adding specified constants to birthweight, adding random normal numbers to birthweight, and predicting weight by regression equations. Regardless of the estimating technique used, the mean weight of the high risk group was significantly lower than that of the low risk group. Dropouts did not differ significantly from nodropouts in prenatal, intrapartum, or neonatal characteristics. The study results suggested that further comparisons between high and low risk infant groups at six months of age could proceed under the assumption that dropouts would not affect any conclusions drawn from the study.

▼ In the Antelope study availability, use and diagnostic accuracy of spiral CT in patients clinically suspected of PE was investigated. This study was divided in two separate phases, starting with a prospective evaluation of available diagnostic techniques part of the diagnostic consensus strategy in The Netherlands in a specifically designed algorithm. Phase I showed that in a direct comparison with the gold standard sensitivity and specificity of spiral CT is too low to endorse its role as a single test to exclude pulmonary embolism. Also spiral CT had a limited added value as a second procedure following ventilation-perfusion scintigraphy. The accuracy of CT was significantly better in cases with interobserver agreement and good image quality.
The available data of phase I was used for a cost-effectiveness analysis. Combined with recent literature data two new strategies were proposed and studied independently. Phase II showed in a prospective clinical management study that spiral CT can be used safely as a first line test to rule out pulmonary embolism in clinically suspected patients. It is also possible to reliably determine an alternative diagnosis in 25% of all patients. A new questionnaire showed that since 1997 the use of spiral CT has increased considerably.

▼ A evolução do conhecimento acerca das funções dos meniscos e do tratamento das suas lesões, impulsionada pelo advento da cirurgia artroscópica, consagrou e popularizou a meniscectomia por esta via, por ser uma técnica menos invasiva, com menor morbidade e menores custos hospitalares, a ponto de torná-la, atualmente, a cirurgia ortopédica mais frequentemente realizada no mundo. Embora a maior parte dos pacientes submetidos a esta intervenção cirúrgica tenha resultados favoráveis e resolução rápida dos sintomas, percebe-se que parte considerável dos pacientes, especialmente os mais idosos, não apresenta uma evolução póscirúrgica satisfatória, apresentando piora dos sintomas e, eventualmente, necessitando de nova cirurgia. Partindo da hipótese que em determinados pacientes a meniscectomia, em vez de tratar, precipita e acentua um desequilíbrio biomecânico do joelho, o autor realizou estudo observacional prospectivo não-controlado com 86 pacientes de ambos os gêneros, com idade superior a 50 anos (média de 60,2 ± 7,1 anos), submetidos à meniscectomia artroscópica para tratamento de lesão do menisco medial de natureza degenerativa, com o objetivo de determinar variáveis demográficas, clínicas, anatômicas e cirúrgicas relacionadas aos resultados desfavoráveis. As avaliações funcionais pré e pós-operatórias foram realizadas utilizandose o Índice do KOOS (Knee Injury and Osteoarthritis Outcome Score), aplicado a todos os pacientes antes da cirurgia e 60 meses depois. Após análise estatística dos resultados obtidos, o autor verificou que 10 fatores podem ser considerados preditivos das meniscectomias mediais artroscópicas neste grupo etário, sendo quatro fatores fortemente associados aos resultados desfavoráveis: lesão da raiz posterior do menisco medial, dor pré-operatória intensa, claudicação antes da cirurgia e tempo decorrido entre o início dos sintomas e a cirurgia; dois fatores foram considerados moderadamente associados aos resultados desfavoráveis: presença de edema de medula óssea na ressonância magnética (RM) préoperatória e duração da cirurgia; quatro fatores foram considerados associados de modo fraco aos resultados desfavoráveis: Índice de Massa Corporal (IMC) >= 30 kg/m2 , varismo do joelho, presença de cisto poplíteo na RM pré-operatória e extensão da ressecção do menisco medial

▼ O setor cosmético brasileiro gerou mais de 5,6 milhões de oportunidades de trabalho por todo o Brasil e faturou US 43 bilhões em vendas ao consumidor no ano 2013. Além disso, os cuidados cosméticos, especialmente aqueles destinados à pele, promovem o bem-estar e uma melhor qualidade de vida entre os usuários, oferecendo benefícios psicossociais e também para a saúde, como a proteção solar. No Brasil, a maioria das empresas no setor cosmético é de micro, pequeno ou médio porte, com capacidade inovadora baixa e investimentos em pesquisa e desenvolvimento incipientes, de modo que seu posicionamento futuro é bastante incerto diante da alta competitividade. Neste trabalho, buscou-se produzir material científico destinado a orientar os empreendedores e gestores das micro, pequenas e médias empresas (MPMEs) brasileiras do segmento de cuidados com a pele a traçar suas estratégias de diferenciação tecnológica, frente aos concorrentes de grande porte, até o ano 2025. A pesquisa, predominantemente qualitativa e preditiva, foi fundamentada na metodologia dos estudos prospectivos e foi estruturada nas seis fases seguintes: A) análise do macroambiente do setor cosmético de cuidados com a pele; B) identificação das oportunidades de inovação tecnológica em cuidados com a pele; C) análise de pedidos de patente relacionados aos cuidados com a pele no Brasil; D) identificação das tecnologias-chave para os cuidados com a pele até o ano 2025; E) desenvolvimento de cenários tecnológicos para as MPMEs do segmento de cuidados com a pele com projeções até o ano 2025 e F) proposição de um método de desenvolvimento de cenários orientados pela identidade organizacional para empresas com gestão centralizada no empreendedor. A fase A elucidou a dinâmica do setor cosmético brasileiro de cuidados com a pele, identificando convergências entre diferentes indústrias de bens de consumo. As fases B e C resultaram na elaboração de um novo sistema de classificação composto por dez categorias de oportunidades de inovação tecnológica em produtos para os cuidados com a pele, o qual foi útil para identificar as oportunidades tecnológicas mais exploradas pelo setor. Na fase D, evidenciaram-se sete grupos tecnológicos de maior potencial de mercado e de maior viabilidade tecnológica para o segmento de cuidados com a pele no Brasil, até o ano 2025. A fase E resultou em quatro cenários prospectivos para o desenvolvimento e diferenciação tecnológica das MPMEs cosméticas brasileiras, a citar: \'Cosméticos am-pm\'; \'Selo verde e amarelo\'; \'Beleza para os durõesé \'Cabeça nas nuvens\'. Enfim, na fase F, propôs-se um método inédito de construção de cenários prospectivos orientados pela identidade organizacional, adaptado às empresas com poucos colaboradores. Este estudo fornece projeções e inspirações que podem orientar as MPMEs cosméticas no Brasil a definir visões de futuro e estratégias tecnológicas que possibilitem sua diferenciação frente aos grandes concorrentes, bem como a afirmação da identidade que desejam disseminar.

▼ Introdução: O objetivo deste estudo foi avaliar o efeito de uma estratégia restritiva de transfusão de hemácias comparada a uma estratégia liberal na evolução clínica de pacientes submetidos à cirurgia cardíaca com circulação extracorpórea. Métodos: Estudo prospectivo, randomizado, e controlado. Foram randomizados 512 pacientes para uma estratégia liberal de transfusão (hematócrito 30%) ou restritiva (hematócrito 24%). Resultados: Os pacientes apresentaram idade média de 60,7 ± 12,5 anos no grupo liberal e 58,6 ± 12,5 anos no grupo restritivo. Em ambos, houve predomínio de pacientes com fração de ejeção normal, mas 13% dos pacientes do grupo liberal e 15% do grupo restritivo apresentavam fração de ejeção abaixo de 40%. A mortalidade ou a morbidade grave em 30 dias foi semelhante nos dois grupos (10% na estratégia liberal e 11% na estratégia restritiva, P=0,518). Não houve diferença entre os grupos em relação às taxas de complicações secundárias. A média da concentração de hemoglobina foi 10,5 0,9 g/dL no grupo liberal e 9,1 1,2 gdL no grupo restritivo (P<0,001). No grupo liberal, 198 pacientes (78%) receberam transfusão de hemácias, e no grupo restritivo 118 pacientes (47%). Independente da estratégia utilizada, o número de transfusão de hemácias foi fator preditor independente para a ocorrência de complicações clínicas graves ou morte em 30 dias (OR=1,21; IC 95%=1,1-1,4, P= 0,002). Conclusão: A estratégia restritiva de transfusão foi tão segura quanto à estratégia liberal em pacientes submetidos à cirurgia cardíaca com circulação extracorpórea. Introduction: The aim of this study was to evaluate whether a restrictive strategy of red blood cells transfusion is as safe as a liberal one with respect to clinical outcomes. Methods: In this prospective, randomized, controlled clinical trial we randomly assigned 512 patients after cardiac surgery to a liberal strategy of transfusion (to maintain a hematocrit at least at 30%) or to a restrictive one (to maintain a hematocrit at least at 24%). Results: The mean age of patients was 60.7 ± 12.5 year-old in liberal group vs. 58.6 ± 12.5 year-old in restrictive group. In both groups, most patients had normal left ventricular function, but 13% of patients in the liberal group and 15% in the restrictive one presented ejection fraction of less than 40%. The primary end-point - a composite endpoint of 30 day all cause mortality or severe morbidity was similar between groups (10% in the liberal-strategy group vs. 11% in the restrictive-strategy group) (P=0.518). Also, there were no differences between groups with respect to the rates of other complications. Hemoglobin concentrations were maintained at a level of 10.5 ± 0.9 g/dL in the liberal group and 9.1 ± 1.2 g/dL in the restrictive group (P<0.001).
