▼ A Maloclusão é um problema de saúde pública com uma alta prevalência em diferentes populações e com implicações físicas e psicológicas que influenciam a saúde oral e a qualidade de vida do indivíduo. As interações sociais são também condicionadas pela maloclusão afetando o modo como as pessoas se auto percecionam bem e aos indivíduos à sua volta. Assim sendo, a auto-percepção da saúde oral tem um importante papel no entendimento da influência da maloclusão na qualidade de vida. Enquanto o ortodontista prioriza a função e a oclusão na consulta, o paciente pode perceber que outros fatores são igualmente importantes para iniciar o tratamento. Objectivos: Responder às questões: 1-Qual a maloclusão com maior e menor necessidade de tratamento ortodôntico para os 4 grupos de estudo; 2-Se a profissão influencia a procura de tratamento ortodôntico e a preocupação com os cuidados orais nomeadamente a ida à consulta dentária; 3-Qual dos 2 sexos procura mais o tratamento ortodôntico; 4-Se a idade está relacionada com a procura do tratamento ortodôntico e qual a sua perceção; Metodologia: Para o presente trabalho de campo foi elaborado um inquérito e distribuído pelo público alvo do tema de modo a satisfazer os objetivos propostos. Além disso foi realizada uma pesquisa bibliográfica usando o motor de busca PubMed, com as palavras chave “malocclusion”, “orthodontic treatment needed” e “perception”. Posteriormente foi necessário fazer uma segunda pesquisa no mesmo motor de busca mas com as palavras “demand”, “orthodontic treatment” e “perception”. De ambas as pesquisas foram utilizados apenas os artigos com interesse ao tema. Foi também feito o recurso ao livro Ortodontia Contemporânea de William Proffit, 5ª edição. Discussão: Não foram encontrados estudos que apresentassem os mesmos objectivos propostos no presente trabalho. No entanto, é possível ter um termo comparativo não entre os grupos de estudo neste trabalho avaliados, mas sim entre as características influenciantes e gerais da população. A bibliografia refere que a estética dentofacial é o principal factor determinante na demanda por tratamento ortodôntico. Conclusão: Os Médicos Dentistas Generalistas e os indivíduos não profissionais da área (leigos) consideram que o Apinhamento Dentário é maloclusão com maior necessidade de tratamento ortodôntico. Já os Ortodontistas e os Estudantes de Medicina Dentária consideraram a Mordida Cruzada Anterior. Quanto à maloclusão de menor necessidade de tratamento ortodôntico a maioria, dentro dos 4 grupos de estudo, consideraram ser a presença de Dentes Conóides. Verificou-se que a profissão influencia também o cuidado que o indivíduo tem com a sua saúde oral (tanto a frequência com que vai à consulta dentária como o facto de se ter submetido a tratamento ortodôntico). No que diz respeito à relação do género com o facto de se submeter ao tratamento ortodôntico, o género feminino realiza mais este tipo de tratamento do que o género masculino. ▼ ABSTRACTReevaluating Our Orthodontic Treatment Goals: The Impact of Ideal Torque on Alveolar Bone SupportStephanie Purnomo, DDSOBJECTIVE: The purpose of this investigation was to: 1) develop a protocol to measure molar torque, taking into consideration both crown and root positions using 3D visualization software (InVivo5), test the reliability of using this analysis on a sample of adult patients; 2) Utilize this method to evaluate molar torque changes in relation to skeletal transverse jaw relationships and buccal bone coverage in patients before and after orthodontic treatmentMETHODS: 32 adult patients were retrospectively selected from the UCSF orthodontic clinic. All were over 16 years old, with normal ANB angle between zero and five degrees, final CBCT records taken by August 2016, and initial records taken after August 2012. Initial and Final Dicom files were landmarked with 41 points, generating 16 different measurements utilized to measure molar torque, transverse jaw relationship, and the quality of bone surrounding the roots of maxillary first molars at 4 separate sites: the mesiobuccal and distobuccal root apex and furcation. RESULTS: Intra- and inter- rater reliability of maxillary first molar torque values were excellent (r=0.98, 0.91) for right and left sides respectively (p<0.05). All other measurements demonstrated good intra and inter-rater reliability, with the exception of bone thickness measured at MBRF (mesiobuccal root furcation) and DBRF (distobuccal root furcation) on the left and right sides. There was a weak negative correlation between maxillary molar torque and the transverse maxillary deficiency prior to treatment (r=-0.03), indicating greater initial buccal crown torque when the maxillary width is narrow. There was a weak negative correlation between transverse deficiency and average change in torque throughout treatment (r=-0.17), indicating greater compensatory buccal crown torque after treatment when the maxillary width is narrow. Patients that experienced large molar torque changes during treatment demonstrated the largest changes in buccal root alveolar bone thickness (r=0.24). The percentage of root exposures were most pronounced in the mesiobuccal roots of both right and left first molars, at the level of the furcation. CONCLUSIONS: This custom analysis is a repeatable and a reliable method for measuring whole tooth torque/ buccolingual inclinations. Orthodontic treatment in adults should be carefully evaluated to reduce risk of losing alveolar bone support in posterior teeth. In patients with larger skeletal maxillary transverse deficiency, a greater change in torque is observed, which is weakly correlated to changes in buccal bone thickness surrounding the roots of maxillary first molars.

▼ Introduction: The effect of cone-beam computed tomography (CBCT) imaging on orthodontic diagnosis and treatment planning has not been fully elucidated despite the increasing use of 3-dimensional (3-D) radiography in dentistry. The application of CBCT imaging in orthodontics has been shown to provide information that aids in the localization of impacted canines1, 2, 3 , placement of miniscrews4, 5, 6, identification of pathology7, 8, and assists in the assessment of alveolar bone levels9, 10, orthodontic tooth movements11, 12, the temporomandibular joint (TMJ)12, 13, 14, sinuses15, 16, and airway.15, 17, 18 In addition, CBCT is useful for the evaluation of root resorption11 and planning of orthodontic treatment19, 20. While the clinical value of CBCT is mostly empirical, there are no specific studies so far to evaluate the differences between the diagnosis and treatment plans of orthodontic patients when using conventional records and CBCT images.; Purpose: Our hypothesis is that CBCT imaging provides additional information that will influence orthodontic diagnosis and treatment planning.; Methods:  Four orthodontic examiners diagnosed and created treatment plans for 38 consecutively selected orthodontic patients using conventional orthodontic records (photos, panoramic and lateral cephalometric radiographs) and CBCT images. Examiners completed written questionnaires to indicate their diagnosis and preferred plan of treatment for each case at three different points in time, each one month apart using conventional orthodontic records for the first two analyses (T1, T2) and CBCT data in addition to traditional records for the third (T3). The level of experience with CBCT imaging of the examiners was also scored. The diagnoses and treatment plans at the three points in time were compared.; Results: Statistically significant differences (p<.05) were found using conventional records and CBCT data when comparing T1 and T2 to T3 for all examiners. No significant differences (p<.05) were present among the 4 raters when comparing the diagnoses (p=.629) or treatment plans (p=.137) made using conventional radiographs at T1 and T2. Examiners with greater CBCT experience showed a higher correlation and statistically significant changes in diagnosis upon addition of the CBCT data when compared to the less experienced examiners.; Conclusion: CBCT imaging in orthodontics results in significant changes in diagnosis but not the treatment planning. A sufficient level of training in CBCT is necessary to take advantage of the differences in diagnosis using 2-D and 3-D imaging. When compared to conventional 2-D imaging, CBCT provides additional diagnostic information to the orthodontist, however the impact that these diagnostic changes have on treatment planning needs to be further analyzed. Future studies with greater number of cases and examiners are needed to adequately evaluate the role of CBCT in routine orthodontic care. Advisors/Committee Members: Paine, Michael L. The patients were treated in three centres in secondary care hospitals Tayside NHS, United Kingdom. The levelling and alignment stage of treatment was assessed from the start of treatment until the ligation of the working archwire for each bracket slot system (0.016x0.022 stainless steel for the 0.018-inch group and 0.019x0.025 stainless steel for the 0.022-inch group). Periapical radiographs were taken before the start of treatment and after 9 months in treatment for the maxillary central incisors to assess orthodontically-induced inflammatory root resorption (OIIRR). The “Smiles better” questionnaire was completed by the participants at 6 months from the start of treatment. Primary outcome measures: The duration of the levelling and alignment stage of orthodontic treatment in the maxillary and mandibular arches. Secondary outcome measures: The number of scheduled appointments for the levelling and alignment stage of orthodontic treatment in the maxillary and mandibular arches, OIIRR at 9 months from the start of treatment using periapical radiographs and patient perception of wearing orthodontic appliances. Results: The data from 92 patients (mean age 19.55 years) were analysed after the completion of their levelling and alignment stage of orthodontic treatment. An ANOVA test showed no statistically significant difference in the duration or number of scheduled appointments for the levelling and alignment stage in the maxillary and mandibular arches between the two appliance groups. Non-parametric statistical test showed no statistically significant difference in the severity of OIIRR and patient perception of wearing orthodontic appliances between the two study groups except for the soreness of teeth, where more patients in the 0.022-inch group experienced significant teeth soreness than the 0.018-inch group. Multiple regression analysis determined that 49.6% of the variance in the duration of levelling and alignment duration for the maxillary arch can be explained by five factors: alignment of ectopic tooth, scheduled appointment intervals, gender, bracket slot size system and the number of failed scheduled visits. For the mandibular arch, 50.8% of the variance in the levelling and duration of alignment can be explained by three factors: scheduled appointment intervals, arch irregularity and the number of debonded brackets. Darbo tikslas - ištirti ortodontinių anomalijų paplitimą, gydymo reikalingumą bei gydymo poreikį tarp Lietuvos moksleivių. Tikslui pasiekti iškelti šie uždaviniai: 1) nustatyti ortodontinių anomalijų paplitimą tarp 10–11 m. ir 14–15 m. amžiaus Lietuvos moksleivių; 2) įvertinti ir palyginti ortodontinių anomalijų gydymo reikalingumą tarp 10–11 m. ir 14–15 m. Lietuvos moksleivių; 3) ištirti Lietuvos moksleivių požiūrį į ortodontinių anomalijų gydymo poreikį; 4) nustatyti moksleivių ortodontinių anomalijų gydymo poreikio sąsajas su demografiniais ir socialiniais veiksniais. Tyrimas atliktas dešimtyje Lietuvos apskričių, 41 atsitiktinai pasirinktoje mokykloje. Tyrimo metu nustatytas dantų susigrūdimas, tarpai tarp dantų, gilus ir kryžminis sąkandis, pirmųjų krūminių dantų tarpusavio santykis pagal Angle klasifikaciją, gydymo reikalingumui nustatyti naudotas ICON indeksas, o gydymo poreikiui bei sąsajoms su socialinias veiksniais - klausimynas. Ortodontinės anomalijos nustatytos 77 proc. 10–11 m. amžiaus ir 61,7 proc. 14–15 m. amžiaus tirtųjų moksleivių. Ortodontinis gydymas reikalingas beveik pusei jaunesniojo amžiaus ir kas trečiam vyresniojo amžiaus moksleiviui, o ortodontinio gydymo poreikis buvo panašus abiejose amžiaus grupėse.

The aim of this study was to assess the malocclusion prevalence, orthodontic treatment need and demand among Lithuanian schoolchildren. The objectives of the study: 1) to describe the malocclusion prevalence among Lithuanian schoolchildren in 10–11 and 14–15-year old age groups; 2) to evaluate the need for orthodontic treatment among 10–11, 14–15 year-old schoolchildren in Lithuania using the ICON index; 3) to investigate the association between normative (objective) and self-perceived (subjective) orthodontic treatment need; 4) to assess the associations of age, gender, and socioeconomic factors with dental appearance satisfaction and demand for orthodontic treatment. The survey was conducted in 41 randomly selected schools in 10 counties of Lithuania. The study recorded crowding, spacing, overbite, crossbite, relationship of the first upper and lower molars according to Angle’s classification, the orthodontic treatment need was assessed using the ICON index and the children were also invited to complete a questionnaire about treatment need and their appearance. The results showed that 77% of 10–11-year-old and 61.7% of 14–15-year-old schoolchildren had malocclusion. The treatment need among younger schoolchildren was higher than in older group. The subjective demand for orthodontic treatment among 10–11-year-old schoolchildren and 14–15-year-old schoolchildren was similar.

The aim of this study was to compare the subjective perceptions of the occlusal appearance of 11 to 12 year-old schoolchildren of Nairobi with the modified Aesthetic Component (AC) scale of the Index of Orthodontic Treatment Need (IOTN). The objectives were to assess the children's perception of their occlusal appearance, categorise the occlusal appearance using the AC scale, by both the children and researcher; and to compare the children's' perception and the AC of the IOTN. Satisfaction with appearance of teeth and occlusion as well as peer comparison was investigated. The treatment need and demand was assessed using the modified AC photographs of the IOTN. Altogether, 488 school children (249 girls and 239 boys) assessed their perception of the anterior teeth using a Verbal Descriptor Scale (VDS) and a Visual Analog Scale (VAS) questionnaire. The researcher examined the anterior teeth of the participants and categorised them using the AC of the IOTN. The following results were reported: about two-thirds of the respondents were satisfied with the appearance of their teeth, with the gender difference being statistically significant. About 35% were dissatisfied or very dissatisfied, but reasons for it varied. More than half of the respondents rated their teeth as better than those of their peers and three-quarters were satisfied with their occlusion, the two responses elicited no gender difference. Treatment need was assessed by use of the AC indicated that there was a moderate agreement between researcher and respondents’ in treatment need assessment. The researcher found 36.3% needed treatment against self-perceived need of 30.9%. One-third of the respondents determined by the researcher to 'need treatment', felt 'no need'. There was no statistical gender difference in perceived need for treatment. The selection of ranked photographs of the AC by both researcher and respondents demonstrated skewed distributions towards the 'low ranked' or attractive end of the ranking order irrespective of the state of occlusion. Statistical comparison of the two methods used, namely the VDS and VAS, indicated that the responses were significantly different. Therefore it is suggested that any treatment priority assessment should take perceptions of occlusal appearance. into consideration. ▼ Influence of orthodontic caregiver behaviour on the perceived satisfaction of patients during orthodontic treatment. Dr. F.J. du Raan M.Sc. (Orthodontics) thesis, Department of Orthodontics, University of the Western Cape. In this thesis I interviewed patients that are busy with orthodontic treatment, as well as those that have undergone orthodontic treatment at the department of Orthodontics at the University of the Western Cape, with the aid of four questionnaires. Patients completed questionnaires to provide general and demographic information, quantify their satisfaction with the orthodontic treatment process, their perception of the orthodontic clinician's behavioural traits and lastly they completed the NEO-FFI personality questionnaire to determine their own personality profile. All these questionnaires were used in previous studies, or they were slightly modified to be applicable to orthodontics. The information gained was used to determine if there are correlations between the patient’s perceived satisfaction of the treatment process with patient specific treatment variables (as acquired from the General information and Demographics Questionnaire ), demographic factors, clinician's behavioural traits and patient specific personality traits and any combination of the above mentioned. We wanted to determine which behavioural traits of the orthodontic caregiver influences the perceived satisfaction with the treatment to the greatest extent. Furthermore, we wanted to determine if certain personality traits of the patients would influence their perceived satisfaction with the treatment process or their perception of the clinician's behavioural traits. The only aspect from the General Information and Demographics Questionnaire that had any correlation to satisfaction with the treatment process or the perception of the clinician's behavioural traits, was whether the patient was treated by a single registrar or multiple registrars. Patients treated by multiple clinicians had a lower average score for satisfaction and orthodontist behaviour. Results from the study shows that all the clinician's behavioural traits do have statistically significant influence on the perceived satisfaction with the treatment process, but certain behavioural traits have a greater influence. Result showed Empathy and Care to have the strongest influence on perceived satisfaction, whereas Motivation has the lowest influence. The NEO-FFI personality questionnaire was used to register each patient's personality profile. Scoring for the following personality traits created the personality profile: Neuroticism, Extraversion, and Openness to Experience, Conscientiousness and Agreeableness. Patient personality profiles were shown to have no significant influence on the patient's perceived satisfaction with the treatment process.Neuroticism was shown to have a weak negative correlation with the Professionalism sub-category of the Orthodontic Clinician Behaviour Questionnaire. ▼ Introduction: Improvement of aesthetics is often the reason patients seek orthodontic treatment. The ability to accurately assess aesthetic treatment need from the viewpoint of the patient is necessary. The threshold for orthodontic treatment is not constant in all countries and no previous attempts to determine this threshold has been made in South Africa. By ensuring that patient’s perceptions of treatment need are incorporated into the index chosen to assess perceived need, accurate data can be obtained. Determining perceived need from the patient’s viewpoint is important, and understanding its relationship with perceived need according to the professional, as well as normative need can facilitate better patient communication and management of expectations. Aims: There were three main aims of this study. First, South African children’s perceptions of treatment need according to the Aesthetic Component (AC) of the Index of Orthodontic Treatment Need (IOTN) was assessed to determine if they were similar to those of the dentists who established the threshold, or if the threshold of the grading system should be altered to better suit laypeople’s opinions. Second, the newly established threshold could then be used to determine the perceived needs for orthodontic treatment, and compare them to the normative need of the population. Finally, factors that may influence the perceived needs of the patients such as gender, ethnicity and socio-economic position could be investigated. Methodology: A cross-sectional study on 317 children aged between 11 and 14 years was done. 43.8% were male and 56.2 % were female. The sample was chosen from five schools in the Lekwa District of Mpumalanga using a multi-stage sampling technique. The study population comprised of four groups based on ethnicity; Asian (3%), black (74%), coloured (6%) and white (17%). The socio-economic position (SEP) was determined by Principal Component Analysis of household assets. Societal perceived, subjective perceived and self-perceived needs were assessed using a questionnaire and the child-rated AC of the IOTN. An intra-oral examination was conducted using the AC of the IOTN to assess objective perceived need and the modified Dental Health Component (DHC) of the IOTN to assess normative treatment need. Results: Treatment threshold was determined to be grade 3 of the AC of the IOTN according to societal perceived need of the group of South African children, and was confirmed by self-perceived need. Subjective perceived need for treatment was assessed using the AC grade participants felt best reflected their aesthetic impairment, and was found to be 20.2%, compared to actual self-perceived need of 38.5%. The latter was deduced by comparing the child-rated AC (subjective perceived need) to their perception of treatment need of that grade (societal perceived need). ▼ Objetivo: Comparar os resultados cefalométricos obtidos com tratamentos compensatórios e tratamentos orto-cirúrgicos para pacientes que apresentam Classe III esquelética. Metodologia: Foram selecionadas de forma retrospectiva telerradiografias iniciais e finais de 97 pacientes Classe III esquelética. Os critérios de inclusão foram: dentição permanente completa até primeiros molares, relação molar de Classe III, ausência de extrações dentárias durante o tratamento, ausência de problemas periodontais severos, Wits menor que -2mm, telerradiografia em normalateral iniciais e finais, fotografias intra e extra-bucais iniciais e modelos de gesso iniciais. Os pacientes foram divididos em 3 grupos. Grupo C: pacientes que foram submetidos ao tratamento ortodôntico associado à cirurgia ortognática (uni ou bimaxilar) totalizando 30 pacientes (idade média inicial 25,07 [22,90-25,56]). Grupo R pacientes que foram submetidos ao tratamento ortodôntico compensatório usando a prescrição Roth totalizando 30 pacientes (idade média inicial 16,22 [15,68-23,90]). Grupo B pacientes que foram submetidos ao tratamento ortodôntico compensatório usando a prescrição Biofuncional para Classe III totalizando 37 pacientes (idade média inicial 19,97 [18,53–24,68]). A severidade da Classe III foi determinada pela relação posterior e dividida em ¼, ½, ¾ e completa. Todas as telerradiografias foram digitalizadas e seus traçados foram realizados pelo mesmo examinador no software Dolphin (Dolphin Imaging Versão 11.9). As variáveis estudadas foram divididas em grupos: Alterações Maxilares, Mandibulares, Maxilo-Mandibulares, Convexidade Facial, Padrão Facial, Posição dos dentes superiores, Posição dos dentes inferiores e Perfil Tegumentar. A análise estatística foi realizada utilizando o software SPSS, versão 18.0. [SPSS Inc. lançado 2009. PASW Statistics for Windows, Versão 18.0. Chicago: SPSS Inc.]. As variáveis contínuas simétricas foram expressas como média e erro padrão da média (± SEM) ou mediana e intervalo de confiança de 95% ([IC 95%]), definido pelo teste de Shapiro-Wilk. As variáveis categóricas foram descritas por frequências absolutas (n) e relativas (n%). Para comparação de meios entre grupos independentes, foi aplicada uma Análise de Variância Unidireccional (ANOVA) com teste post hoc de Tukey ou teste de Kruskal-Wallis com teste post hoc Dunn. Além disso, o teste de classificação assinado por Wilcoxon foi usado para dados emparelhados (por exemplo, medições pré e pós-tratamento). Por outro lado, as variáveis categóricas foram comparadas intragrupo pelo teste do Qui-Quadrado com análise residual ajustada padronizada. Todos os dados foram avaliados usando o SPSS, versão 18.0. O nível de significância foi fixado em 5%.Resultados: O grupo C apresentou uma severidade maior do 8 que os demais grupos tendo 73,3% dos pacientes com uma Classe III severa (3/4 e completa). Houve um avanço do ponto A (projeção da maxila) nos Grupos B e C e um recuo do mesmo no Grupo R.
