▼ Background: Matrix metalloproteinase (MMPs) are proteins that have a unique role in immunity, tissue remodeling, and tumorigenesis. MMPs can break down extracellular components, release bioactive molecules, and act on pro-inflammatory cytokines, chemokines and other proteins to regulate varied aspects of inflammation and immunity. Nicotine (3-(1-methyl-2-pyrrolidinyl) pyridine), a key toxic component of tobacco, is thought to dysregulate matrix metalloproteinase secretion in immune cells. Aim: Examine the effect of nicotine on MMP9 secretion and activity in primary monocytes. Methods: Primary monocytes were stimulated with LPS (1 µg/ml) and Nicotine (100 ng/ml) and MMP9 secretion were studied. Gel zymography and ELISA techniques were used to study the secretion of MMP9 due to nicotine. Furthermore siRNA techniques were used to understand the role of the a7 nicotinic acetylcholine receptor (a7nAChR) characterize the MMP production. Conclusion: Nicotine decreased the MMP9 activity and secretion in primary monocytes. The a7nAChR receptor has an important role in the secretion and activity of MMP9 production in primary monocytes.

oral care is overlooked during the daily care of adults with intellectual and developmental disabilities (IDD). Due to the integration of adults with IDD from institutions into group homes and private homes, caregivers need to be taught to implement daily
oral hygiene protocols. The purpose of this pilot study, conducted by the University of Louisville and the Pacific Institute for Research and Evaluation, was to develop an
oral hygiene plan for adults with IDD residing in group homes in the Louisville, Kentucky area that would ultimately improve their
oral health and quality of life. This thesis focuses on the quality of implementation of the
oral health intervention strategy based on the results from the pilot study. Additionally, this thesis sheds light on the level of implementation quality as measured by dosage, fidelity, and caregiver reactions.

▼ P. gingivalis is a major periodontal disease causing pathogen. Gingival epithelial cells being the first line of defense, interact and internalize the bacteria evoking a cell signaling response that finally leads to the release of cytokines. IL-6 is a potent pro- inflammatory cytokine, which is one of the responsible factors for bone resorption, a clinical manifestation of periodontitis. The experimental model study hypothesizes that P. gingivalis infection increases the production of IL-6 in telomerase immortalized gingival keratinocytes (TIGKs) through up-regulation of c-Jun (AP-1 reporter). We observed the IL-6 production post-infection with P. gingivalis WT ATCC33277 and mutants ?0482, ?Ndk and ?luxS. The ndk gene in P. gingivalis controls ATP scavenging and persistence of the bacterium intracellularly. The mutant was therefore expected to be less efficient in the intra cellular environment. The luxS gene in the bacterium plays a role in maintaining several physiological functions like hemin uptake, quorum sensing etc. The mutant was hence expected to show reduced activation of cell signaling pathways and IL-6 production. The cells were infected with bacteria at 4h and 24h time points and concluded that IL-6 production is increased in infected samples, compared to controls. Also, western blots indicated that c-Jun phosphorylation was up regulated in infected samples, providing a possible evidence that P. gingivalis stimulates IL-6 production through phosphorylation of c-Jun in gingival epithelial cells, finally leading to IL-6 secretion.

▼ Objectives: Tooth extraction initiates a cascade of biological events leading to the reduction of alveolar ridge volume. Alveolar ridge preservation (ARP) is a surgical treatment which aims at minimizing hard and soft tissue changes following tooth extraction. Several techniques and materials have been studied and used clinically in ARP. The selection of the biomaterials used for this technique is determined by several factors, such as features of the extraction site, inherent biomaterial properties and handling preferences by the surgeon, among others. The purpose of this study was to evaluate the efficacy of alveolar ridge preservation via the application of Easy-graft CLASSIC® (Sunstar Americas Inc.), an alloplastic bone substitute with unique handling features, following flapless posterior single tooth extraction compared to a particulate freeze-dried bone allograft (FDBA) covered with a collagen wound dressing, which has been advocated as a predictable treatment modality. The primary outcome in this study was bone volumetric reduction of the alveolar ridge assessed using cone beam computed tomography (CBCT) scans obtained at baseline and 16 weeks after tooth extraction and ARP.
Methods: This study is part of a multicenter study in collaboration with the University of Maryland School of Dentistry. Seventeen healthy adults treatment planned for a single tooth implants in the area posterior to the canines, excluding third molars, were recruited on the basis of an eligibility criteria. Patients were randomly assigned to the control group or the experimental group. Minimally traumatic extraction of the tooth was completed and the presence of an intact buccal plate of bone was verified. The control group received FDBA and the site was stabilized with a collagen wound dressing and sutures. The experimental group received Easy-graft CLASSIC® with no attempt to approximate the marginal mucosa. Healing was assessed after 1, 2, 4, 8, and 16 weeks. DICOM data was used to assess the alveolar ridge volume and linear changes from baseline to 16 weeks after ARP. Clinical measurements of the buccal gingival thickness, buccal alveolar bone thickness, keratinized gingiva, and socket dimensions were made at the time of the extraction and were subsequently analyzed for possible influences on the observed volumetric and linear outcomes.
Results: The mean alveolar ridge volume reduction from baseline to 16 weeks post operatively for the control and the experimental group was 114.96 mm3 and 94.87 mm3, respectively. These values correspond to a reduction of 9.59% for the control group and 13.04% for the experimental group. This difference did not reach statistical significance. The average loss of ridge width was 1.10mm for the FDBA and 1.24mm for the Easy-graft CLASSIC® with no statically significant differences between the two groups. Pulpaskada uppstår vid blekning med 38 % väteperoxid. Koncentration och blekningstid har betydelse. Tandblekningsmedel med neutralt pH-värde är mer skonsamt och närvaro av saliv under behandlingen visar minskad skada i oral hård- och mjukvävnad. Konklusion: Kortare behandlingstid och lägre koncentration av väteperoxid eller karbamidperoxid i tandblekningsmedlet ger mindre skadlig påverkan på oral hård- och mjukvävnad.

Extraction of a tooth leads to a series of healing events that are intimately associated with dimensional changes in the alveolar ridge that typically result in a net volume loss. Previous studies have evaluated the extent and pattern of those resorptive changes, however it remains challenging to predict the degree of change that will occur, as numerous local and systemic factors may play a role in the biologic events that follow tooth extraction. The purpose of this study was to assess the role that phenotypic characteristics of the periodontium play in the alveolar ridge remodeling processes that take place following single tooth extraction.
Methods:
Healthy patients in need of a single tooth extraction in the maxillary arch from second premolar to second premolar (inclusive) and who met a predefined eligibility criteria were enrolled in this study. An impression of the maxillary arch was made and a cone beam computed tomography (CBCT) scan of the maxilla was obtained immediately prior to tooth extraction at the baseline visit. At the time of the extraction, clinical measurements were made including probing depth, bone sounding, buccal keratinized mucosa width, buccal and palatal alveolar bone thickness, and buccal and palatal soft tissue thickness. Fourteen weeks following the baseline intervention, patients returned to the clinic for a second impression of the maxillary arch and a second CBCT of the maxilla. Linear and volumetric bone measurements were made using the data obtained from the CBCT scans. The casts obtained from the impressions were digitally scanned and volumetric measurements were made from the digitized data to assess volume changes of the residual ridge. The primary outcome of interest was the volumetric percent reduction of the alveolar ridge following single tooth extraction. Spearman correlations were utilized to evaluate relationships between variables and modeling was completed to predict the percentage of volumetric change in the hard and soft tissues using the clinical variables.
Results:
A total of 21 patients participated in the study, 19 patients are included in this analysis (one patient has yet to complete the study, one patient was later excluded due to lack of compliance). Of the 19 extraction sites included, 17 were maxillary premolar teeth. At baseline, the average buccal plate thickness was 1.09 mm. After 14 weeks, the average loss of alveolar bone width was 1.66 mm. The average loss of buccal bone height was 1.10 mm and mean loss of palatal bone height was 1.36 mm. The average percentage volumetric reduction of the bone as measured from a CBCT scan was 26.42% after 14 weeks of healing. Mean percentage volumetric reduction of the ridge, as measured from a digitized cast, was 18.89%. Oral Health Related Quality of Life of patients being treated with Conventional Complete Dentures, including: a) Level of expertise of the dental provider (third year dental student vs. fourth year dental student); b) Length of time edentulous; c) Number of previous prostheses; d) Race; e) Sex; f) Age; g) Systemic conditions; h) Condition of the bone ridge; i) Economic factor and insurance coverage, in patients at the University of Iowa College of Dentistry (D3 and D4 Clinics). Materials and methods: Eighty-one subjects who had been completely edentulous (49 men and 32 women; age range from 23 to 93 years old, mean of 58.9+13) for a minimum of 3 months were recruited. Out of the eighty-one, eleven subjects were not included in the final analysis because their prostheses were not delivered due to various reasons (time, death, discontinued treatment). Subjects were asked to complete the OHIP-EDENT (
Oral Health Impact Profile for Edentulous Patients) and the patient satisfaction questionnaire to record data at baseline, at 1-week post-delivery and at 1-month post-delivery of the new prostheses. Additional to these two instruments, the PDI Classification instrument (to assess bone/tissue condition) and the Kapur Index (to assess retention/stability of the new prostheses) were also used. Baseline comparisons of subjects seen at the D3 vs. the D4 clinic were carried out using chi-square tests of association for nominal ordinal variables, and the Wilcoxon-Mann-Whitney test for ordinal and quantitative outcomes. Bivariate analyses were conducted to assess potential relationships between covariates and the primary outcomes (total OHIP-EDENT and total Patient Satisfaction Questionnaire scores at one-week and one-month follow-ups). The Spearman rank correlation was used to identify increasing or decreasing relationships between primary outcomes and quantitative and ordinal variables; the Wilcoxon-Mann-Whitney test was used to assess relationships between primary outcomes and categorical covariates. Results: Overall, there was not enough statistical evidence to conclude that patient satisfaction and quality of life were significantly affected by most of the factors being studied. However, there was suggestive evidence of the presence of higher level of quality of life for patients seen in the D4 clinic (students with higher level of expertise). At the one-month follow-up, the comparison of the total OHIP-EDENT score between the D3 and D4 clinics was significant at the 0.05 level (p=0.018, exact Wilcoxon-Mann-Whitney test). The total OHIP-EDENT scores tended to be lower (indicating greater satisfaction) in the subjects seen in the D4 clinic. Two of the nineteen items from the patient satisfaction questionnaires also suggested a greater level of patient satisfaction for patients seen in the D4 clinic (students with higher level of expertise). ▼ Cette thèse porte sur les capacités de lecture des enfants souffrant de troubles spécifiques du langage oral (TSLO). Elle tente de préciser les processus à l’oeuvre lors de la lecture de mots isolés chez ces enfants, en analysant leurs compétences langagières écrites en lien avec leurs compétences orales. Dans un premier temps, nous présentons les principaux acquis de la littérature à ce propos. Puis nous exposons quatre études expérimentales réalisées auprès d’enfants avec un TSLO ainsi qu’auprès d’enfants au développement typique, sur les sujets suivants : (1) la reconnaissance de mots écrits isolés en lecture à voix haute, (2) la reconnaissance de mots écrits isolés en lecture silencieuse, (3) la compréhension de mots écrits, (4) et les facteurs prédictifs de la lecture. Les résultats les plus marquants indiquent qu’en reconnaissance de mots écrits, les enfants avec un TSLO présentent un retard d’un peu plus de trois ans, en lecture à voix haute comme en lecture silencieuse. En tant que groupe, leur procédure de lecture phonologique apparaît moins efficiente que leur procédure orthographique. L’hétérogénéité interindividuelle demeure toutefois importante. Les enfants qui souffrent des troubles phonologiques expressifs les plus sévères sont ceux dont la procédure phonologique est la plus altérée. Quant à la compréhension de mots écrits isolés, elle apparaît coûteuse en termes de ressources de traitement. Enfin, les facteurs prédictifs de la lecture des enfants avec un TSLO sont identiques à ceux des enfants au développement typique, à l’exception d’un facteur original : chez les enfants avec un TSLO, l’instabilité phonologique est prédictive des capacités de lecture, contrairement aux enfants contrôles. Ce facteur mériterait de plus amples recherches.
This thesis is concerned with the reading abilities in children with specific language impairment (SLI). It aims to precise the process involved when these children read isolated words, by analyzing their written language abilities in relation to their spoken language abilities. Firstly, the main findings of the literature on this topic are presented. Then, four experimental studies conducted both in children with SLI and typically developing children are described. These studies deal with: (1) identification of isolated written words in reading aloud, (2) identification of isolated written words in silent reading, (3) written word comprehension, (4) and predictive factors of reading. The striking results of these studies indicate that, in written word identification in reading aloud, as well as in silent reading, children with SLI show an average delay of about three years, compared to reading control children. As a group, their phonological procedure is less efficient than their orthographic procedure. However, interindividual heterogeneity remains high. ▼ Long-chain bases (sphingosine, dihydrosphingosine, and phytosphingosine) are present in the
oral cavity and have potent antimicrobial activity against
oral pathogens. However, little is known about their cytotoxicity for
oral cells, an important step in considering their potential as future antimicrobial agents for
oral infections. In this study, primary
oral keratinocytes, primary
oral fibroblasts, dendritic cells, and
oral squamous cell carcinoma cells were exposed to 10.0-640.0 µM long-chain bases and glycerol monolaurate (GML) in cell culture medium containing resazurin (e.g., Alamar Blue, Invitrogen Corp., Carlsbad, CA). Cell metabolism was assessed at 48 hours by the reduction of resazurin to resorufin. Percent cytotoxicity was defined as the median fluorescence intensity (MFI) of resazurin in cell culture media of cells treated with dilutions of long-chain bases/MFI of resazurin in cell culture media of untreated cells x 100, and the lethal dose 50 (LD50) was determined from the dose response curve where the 50 percent cytotoxicity intercepts with the long-chain base concentration on the x-axis. For all cells, the LD50 (mean µM + std err) of sphingosine, dihydrosphingosine, phytosphingosine, and GML were 69.7 (1.7), 29.2 (1.7), 20.6 (1.7), and 134.3 (1.7), respectively. Primary
oral keratinocytes were more resistant to long chain bases, whereas
oral fibroblasts, dendritic cells, and
oral squamous cell carcinoma cells were more susceptible. Overall, long chain bases have LD50 for
oral keratinocytes,
oral fibroblasts, dendritic cells, and
oral squamous cell carcinoma cells that are considerably higher than the minimal inhibitory concentrations for
oral pathogens, a finding important to their future potential as therapeutics for prevention or treatment of periodontal disease infections.

▼ Objectives: The aim of the study was to evaluate in vitro the 3D and 2D marginal fit and
2D internal fit of CAD and Press all-ceramic crowns made from digital and conventional
impressions.
Methods: A dentoform replica tooth (#30) was prepared for an all-ceramic crown (Master
Die). 30 impressions were made with PVS material; 30 definitive casts were poured in
type IV gypsum. 30 Lava
TM C.O.S. impressions were made; 30 resin
models were produced. 30 crowns were waxed and pressed in lithium disilicate (IPS e.max
Press) (15 from each impression technique) and 30 crowns were milled from lithium
disilicate blocks (IPS e.max CAD) (15 from each impression technique) utilizing the E4D
scanner and milling engine. The Master Die and the intaglio of the 60 crowns were
digitized using a 3D laser coordinate measurement machine (CMM). For each specimen a
separate data set was created for the Qualify 2012 software. The two data sets, digital
master die and digital intaglio of the crown, were merged using Best-Fit alignment. An
area above the cavosurface margin with 0.75mm occlusal-gingival width circumferentially
was defined. The 3D marginal fit of each specimen was an average of all 3D measurement
values on that specified area for all the crowns, and it was used for the statistical
analysis. For the 2D measurements, two sections, one facial-lingual and one
mesial-distal, were made through the grooves on the standardized metal base of the
tooth. The distance between the die and the intaglio surface of the crown were measured
at 7 standardize points (2 on the margins, 2 at 0.75mm above the margin, 2 on the axial
walls and 1 on the occlusal surface). For the 3D measurements one-way ANOVA with
post-hoc Tukey's HSD test was used to determine whether there were significant
differences in mean marginal fit values among four experimental groups (alpha=0.05). For
the 2D data, a two-way ANOVA was performed to detect a significant interaction between
the type of impressions and the type of crowns on the marginal and internal fit
(alpha=0.05).
Results: One-way ANOVA revealed that the 3D mean marginal fit for Group A (0.048mm±SD
0.009) was significantly lower than those obtained from other three experimental groups
Group B (0.088mm±SD 0.024), Group C (0.089mm±SD 0.020) and Group D (0.084mm±SD 0.021),
while no significant differences were found among Group B, C and D. Similar results were
found regarding the 2D marginal fit Group A (0.040mm±SD 0.008), Group B (0.076mm±SD

▼ An exciting alternative to the current methods for bone regeneration is osseous tissue engineering. One such method focuses on enhancement of osteoblast differentiation through rotary cell culture techniques. The response of osteoblast aggregates to periodontal microorganisms and their by-products will ultimately be important in their success as a method of bone regeneration. In this study, I hypothesize that human embryonic palatal mesenchymal (HEPM, ATCC 1486) pre-osteoblast cells produce different cytokine responses depending upon whether they are grown in a 2-dimensional tissue culture flask or a 3-dimensional tissue culture vessel and whether they are exposed to an un-inoculated, sterile Porphyromonas gingivalis growth medium or exposed to a 24 hour, sterile, P. gingivalis culture supernatant. Objectives: My objectives were first to determine and compare the cytokine response of HEPM, ATCC 1486 pre-osteoblast cells depending upon whether they are grown in a 2-dimensional tissue culture flask or a 3-dimensional tissue culture vessel and whether they are exposed to an un-inoculated, sterile P. gingivalis growth medium or exposed to a 24 hour, sterile, P. gingivalis culture supernatant. Methods: In 3 experiments, 5 X 106 HEPM, ATCC 1486 cells were grown in a 2-dimensional tissue culture flask or a 3-dimensional tissue culture vessel and exposed to an un-inoculated, sterile P. gingivalis growth medium or exposed to a 24 hour, sterile, P. gingivalis culture supernatant and incubated for 72 hours in 5% CO2 at 37 °C. Media was removed from the tissue culture flasks or rotary vessels at 0, 1, 2, 4, 8, 12, 24, 36, 48, 60, and 72 hours to determine cytokine concentrations in the Luminex 100 IS Instrument (Luminex®, Austin, TX). HEPM, ATCC 1486 pre-osteoblast cell morphology was assessed by light and scanning electron microscopy at 96 hours. Results: In experiment 1, there were increases in IL-6 and IL-8. The IL-6 response of cells grown in a 2-dimensional tissue culture flask was higher than that of cells grown in a 3-dimensional tissue culture vessel. The IL-8 responses of the cells grown in 2-dimensional, 3-dimensional tissue culture were nearly identical. In light and scanning electron microscopy cells appeared normal and HEPM, ATCC 1486 pre-osteoblast cell aggregates were similar to that previously reported. In experiment 2, there were also increases in IL-6 and IL-8. The IL-6 and IL-8 responses of HEPM, ATCC 1486 pre-osteoblast cells grown in a 3-dimensional tissue culture vessel exposed to a 24-hour, sterile, P. gingivalis culture supernatant were higher than cells exposed to un-inoculated, sterile P. gingivalis growth media. ▼ Kartiokeilatietokonetomografiakuvaus (KKTT-kuvaus) on erityisesti hammaslääketieteeseen soveltuva 3D-kuvausmenetelmä. Kuvausindikaatioina ovat tyypillisesti kirurgiset, endodontiset ja implantologiset ongelmat. Tutkimuksessa selvitettiin säteilyannosten ja lähetetietojen perusteella Helsingin yliopiston suu- ja leukasairauksien osastolla tehtyjen KKTT-kuvausten oikeutusperiaatteen toteutumista SEDENTEXCT:n ohjeistukseen verrattuna.
Aineisto koostui 1.1 – 31.3.2015 suoritetuista 190 KKTT-kuvauksesta ja lähetteistä. Laskennallisista säteilyannoksista laskettiin kuvausindikaatiokohtaiset keskiarvot. Lähetetiedoista kirjattiin potilaan ikä kuvaushetkellä, sukupuoli, kuvausindikaatio ja lähettäneen hammaslääkärin koulutusaste. Tulokset käsiteltiin taulukkolaskentaohjelman avulla.
Yleisimmät kuvausindikaatiot aikuisilla olivat endodontiset ongelmat ja preoperatiivinen kuvantaminen sekä kirurgiassa että implantologiassa. Lapsilla ortodontiset ongelmat olivat yleisin kuvausindikaatio. DAP-keskiarvo lasten kuvauksissa (n=30) oli 247,6 mGyxcm2 ja aikuisilla (n=160) 335,8 mGyxcm2. ▼ Tutkimuksen tarkoituksena on arvioida panoraamatomografia (PTG) -kuvan käyttökelpoisuutta implanttihoidon preoperatiivisessa suunnittelussa kartiokeilatietokonetomografia (KKTT) -kuvaan verrattuna. Implanttifikstuuran eli hampaan keinojuuren asettamista varten alveoliharjanteen korkeuden ja leveyden tulee olla riittävä. PTG-kuvasta ei voida arvioida alveoliharjanteen leveyttä luotettavasti. Tutkimuksessa tarkasteltiin 30 implantointipotilaan PTG- ja KKTT-kuvia. Implantointi oli suunnitteilla alaleuan molaari- tai premolaarialueille. PTG-kuvasta sekä vastaavasta KKTT-kuvasta mitattiin implantoitavan alueen alveoliharjanteen kokonaiskorkeus ja lisäksi KKTT-kuvasta mitattiin myös alle viiden mm paksuisen alveoliharjanteen korkeus kyseisellä alueella. Mittaustuloksia vertailtiin kuvausmenetelmien välillä sekä laskettiin alle viiden mm leveän alveoliharjanteen korkeuksien keskiarvo, vaihteluväli ja keskihajonta käyttäen Excel-taulukkolaskentaohjelmaa.
Alveoliharjanteen korkeuden lineaariset mittaukset olivat kuvausmenetelmien välillä yhteneviä 56,7 %:ssa ja KKTT-kuvista mitattuna suurempia 43,3 %:ssa kuin PTG-kuvissa. Kolmasosassa tapauksista luuharjanne oli kauttaaltaan riittävän leveä ja lopuissa tapauksista alle viisi mm leveän harjanteen korkeuden osuus alveoliharjanteen kokonaiskorkeudesta vaihteli suuresti. Tulosten perusteella PTG-kuvasta voidaan turvallisesti mitata implantoitavan alueen luun kokonaiskorkeus, mutta alveoliharjanteen riittävän leveyden varmistamiseksi tarvitaan KKTT-kuva.

▼ Introduction: This study compared the clinical efficacy of panoramic imaging and Cone Beam Computed Tomography (CBCT) in the optimal placement of virtual dental implants in the posterior edentulous bounded regions of the jaws. Material and methods: From a retrospective audit of patient records, fifty-two subjects were recruited with sixty-one dental implant sites in the maxilla and mandible. Physical measurements of the residual alveolar ridge were performed and consensus decisions of optimal implant length and bone graft necessity were obtained using reformatted panoramic alone (RP) or cross-sectional imaging (XS). Results: Horizontal restorative space measurements greater for RP (p=0.001). Shorter implants were chosen more often using CS than RP. Use of XS allowed planning that reduced the need for bone graft procedures by 50%. Conclusion: The use of cross-sectional imaging provides supplemental information that significantly influences virtual implant position and the need for bone grafting.

Melanoma is one of the deadliest tumors. Despite
the advances in understanding the melanoma cell biology and drug
discoveries, treatment resistance remains a challenge. Chondroitin
sulfate proteoglycan (CSPG4) is expressed on the surface of
melanoma cells, and its role in metastatic progression has been
established. One known mechanism responsible for this activity is
modulation of the activity of ERK1,2. PI3k/mTOR pathway is also
frequently deregulated in melanomas . We therefore investigated if
CSPG4 has an impact on PI3K/mTOR pathway activity. Our results
indicate that CSPG4 causes resistance to growth inhibition and
apoptosis in early stage melanoma cell lines. The knockdown of
CSPG4 sensitized the advanced disease stage cell lines to growth
inhibition. CSPG4 also induced resistance at various levels of mTOR
pathway in different cell lines, and resulted in inhibition of
motility and invasion in response to treatment with PF-05212384, a
potent PI3K/mTOR inhibitor. Melanoma cell lines derived from all
stages of melanoma growth showed resistance to inhibition of 4ebp1.
Our results indicate that CSPG4 play a role in decreased
sensitivity of melanoma cells to mTOR pathway
inhibition.

▼ Objectives: The ZEB family of transcription factors (ZEB1 and ZEB2) have been demonstrated to play a role in metastatic progression of several cancers, and may also influence the initial transformation and generation of cancer stem cells. However, the expression pattern of ZEB proteins in the development of
oral squamous cell carcinoma has not been investigated. The purpose of this study was to define changes in expression and subcellular localization of the ZEB family in both precancerous lesions and different grades of OSCC. Materials and Methods: Seventy-nine tongue biopsies were subjected to immunohistochemistry and immunofluorescence to determine the expression and subcellular location of ZEB1 and ZEB2 across six histological grades of precancerous and cancerous lesions. Results: Surprisingly, Both ZEB1 and ZEB2 exhibit changes in subcellular location between healthy tissue, dysplasia, carcinoma in situ, and well differentiated, moderately differentiated, and poorly differentiated carcinoma. ZEB1 expression in healthy tissue is mainly nuclear. As carcinoma progresses, subsets of patients show either primarily cytosolic or primarily nuclear ZEB1 in tumors. In histologically normal tissue, ZEB2 is expressed in the cytosol of a band of suprabasal cells. In early grades of carcinoma it remains cytosolic, but in more advanced carcinoma becomes a mix of cells with either nuclear or cytosolic ZEB2. Conclusions: Changes of subcellular distribution of ZEB1 and ZEB2 occur during development of
oral squamous cell carcinoma. Cytosolic localization of either ZEB1 or ZEB2 likely disrupts their ability to regulate transcription. The presence of patients with differences in ZEB1 or ZEB2 localization suggest that there may be different clinical outcomes related to different patterns of expression.

▼ A procura de resultados previsíveis a longo prazo tem despertado diversas questões acerca dos materiais e técnicas utilizadas em Implantologia. Uma destas questões está relacionada com o tipo de conexão entre o implante e a reabilitação suprajacente. No caso da Prostodontia Fixa Implanto-Suportada, a principal decisão será entre o aparafusamento ou a cimentação das restaurações.
Vão ser analisados vários fatores essenciais para o sucesso a longo prazo de uma prótese sobre implantes para ambos os métodos de fixação. Estes fatores incluem: estética, reversibilidade, adaptação passiva, oclusão, resistência à fratura retenção protética, colocação do implante, função imediata, restaurações provisórias, colocação da prótese, reabilitação de casos unitários e conexão interna e externa.
Assim sendo, o objetivo do presente estudo visa estabelecer uma comparação entre a prostodontia implanto-suportada aparafusada e cimentada focando as principais diferenças e vantagens de cada uma das opções.
A amostra para análise teve como base a consulta de processos clínicos de pacientes que procederam à reabilitação
oral fixa implanto-suportada realizada pelos alunos do Mestrado de reabilitação
oral do ISCS-N no período compreendido entre 2008 a 2012.
Conclui-se que ambas as técnicas apresentam vantagens e desvantagens, pelo que é importante fazer uma opção ponderada e fundamentada na reabilitação de cada paciente.
Não devemos pensar que uma retenção é melhor ou pior que a outra mas sim basear-nos nas vantagens e desvantagens, indicações e contra-indicações para selecionar a que melhor se adequa a cada caso.

Es un hecho visible en nuestra sociedad que las personas que sobrevivieron a la dictadura franquista española o van agotando su vida, o sus recuerdos y vivencias se desvanecen en el tiempo. Se suele dejar como herencia a sus descendientes una transmisión de testimonios que normalmente no se suele recoger y que pierde valor por las modificaciones que pueda sufrir a raíz de las interpretaciones de cada uno. Por lo que al final, la historia
oral de cada zona se termina por perder irreversiblemente. Con este estudio se pretende aportar un grano de arena más a esa corriente de recuperación de la memoria
oral de España, en este caso centrado en la ciudad de Santander, para que esos testimonios recuperen su valor patrimonial, para que el ciudadano español recupere su protagonismo en su propia historia y para que, sobre todo, las futuras generaciones no olviden lo que vivieron y cómo lo vivieron sus antepasados.

▼ RESUMEN El presente trabajo pretende constituir una exploración de los elementos del
imaginario popular de la cultura
oral del noroeste peninsular a través de la cual poder obtener
información acerca de un pasado, siempre indeterminado, de las creencias o identidades de
dicho territorio. Con tal fin, se parte de un análisis exhaustivo de dos tipos de fuentes: la
literatura
oral contenida en recopilaciones hechas a partir de fuentes directas y, por otro lado,
la literatura escrita que hace mención a esa “cultura de las supersticiones” pero con una
perspectiva más ideológica que cultural y que iría desde los Precursores del siglo XVIII hasta
los ensayos etnográficos y antropológicos del siglo XX. El objetivo último es tratar de
utilizar, en la medida de lo posible, la cultura de tradición
oral como fuente de información de
una realidad histórica lejana, a partir del convencimiento de que esta vía no ha sido
suficientemente explorada hasta la actualidad.

El propósito de este trabajo es hacer una revisión del estado de la enseñanza del componente
oral del español en la isla caribeña de Martinica para guiar a futuros auxiliares de conversación en el desempeño de su labor. Para ello, se define primero el concepto de expresión
oral, detallando los factores inherentes y las dificultades resultantes de su desarrollo en el aula y se describe el contexto de estudio desde una perspectiva sociocultural.
Con el fin de justificar y describir la figura del auxiliar de conversación dentro de este sistema educativo, se reexamina el plan curricular de la asignatura de español dentro del sistema educativo de Martinica y se realiza un análisis de manuales para identificar las carencias que puede suplir el auxiliar de conversación. Por último, se presenta una propuesta didáctica diseñada acorde a los parámetros descritos en el marco teórico y a las carencias identificadas en la enseñanza en Martinica.

▼ The purpose of this research was to explore the various features of a natural tooth's inherent structural design of a natural tooth that enable it to perform its mechanical functions effectively, and then to apply the principles and techniques inspired by these investigations to the design of prostheses for replacing a single missing tooth. The biomechanical analysis of the natural tooth was performed on both a macroscopic (overall shape as determined by its anatomy) and a microscopic (effect of microstructures on bulk material properties and their distribution) scale. The nature-inspired design guidelines were then implemented within a FE framework for optimizing dental prostheses designs. Finally, the mechanical performances of the optimal designs derived from the numerical models were validated with in vitro tests.

▼ Orofacial pain disorders are challenging to treat because of poor targeting to the trigeminal, orofacial and brain structures involved. Intranasal administration targets these structures while minimizing systemic exposure and side effects. Lidocaine was intranasally administered to rats, and the trigeminal nerve and trigeminally-innervated structures (teeth, temporomandibular joint (TMJ), and masseter muscle) accumulated up to 20-fold higher tissue concentrations of lidocaine than the brain and blood as measured by ELISA. Intranasally administered infrared dye reached the trigeminal nerve and brain within 10 minutes. Intranasal administration distributed the dye to the trigeminal nerve through three regions with high drug concentrations in the nasal cavity: the middle concha, the maxillary sinus and the choana. These findings indicate that intranasal delivery has the potential to target and treat trigeminal pain disorders with fewer side effects. Such disorders include: tooth pain, TMJ disorder, trigeminal neuralgia, headache, and even certain brain diseases. Although intranasal delivery may aid in targeting approved medications, better novel treatments need to be developed for orofacial pain disorders, specifically trigeminal neuralgia.
A new testing device (TrigeminAir) was developed to assist in the screening of trigeminal neuralgia models and treatments. The TrigeminAir device assesses orofacial sensitivity in rats over time by measuring the sip rate of sweetened condensed milk in the presence of a normally non-painful air puff stimulus on the whisker pad. This device detected, 5 to 12 hours after infraorbital nerve injection, that carrageenan (2% or 4%) reduced sip rate by inflaming the whisker pad. During the same time period, intranasal 10% lidocaine (8mg) reversed this reduced sip rate by anesthetizing the inflamed whisker pad. In the first four hours after 4% carrageenan injection, intranasal lidocaine (4% and 10%) decreased neuronal activity in nerves innervating the maxillary teeth, palate, whisker pad and snout. Sensory and motor activity was unaffected after intranasal lidocaine administration, except for anesthesia of the whiskers affecting balance. In conclusion, these results suggest the TrigeminAir device is an efficient and reliable screening method of preclinical models and treatments for orofacial pain and demonstrate that intranasal lidocaine effectively reduces tactile allodynia in rodents. Intranasal delivery and the TrigeminAir device could accelerate the development of new orofacial pain treatments by improving targeting and screening of treatments.

▼ Melanoma is one of the deadliest tumors. Despite the advances in understanding the
melanoma cell biology and drug discoveries, treatment resistance remains a challenge.
Chondroitin sulfate proteoglycan (CSPG4) is expressed on the surface of melanoma
cells, and its role in metastatic progression has been established. One known mechanism
responsible for this activity is modulation of the activity of ERK1,2. PI3k/mTOR
pathway is also frequently deregulated in melanomas . We therefore investigated if
CSPG4 has an impact on PI3K/mTOR pathway activity. Our results indicate that
CSPG4 causes resistance to growth inhibition and apoptosis in early stage melanoma
cell lines. The knockdown of CSPG4 sensitized the advanced disease stage cell lines to
growth inhibition. CSPG4 also induced resistance at various levels of mTOR pathway
in different cell lines, and resulted in inhibition of motility and invasion in response to
treatment with PF-05212384, a potent PI3K/mTOR inhibitor. Melanoma cell lines
derived from all stages of melanoma growth showed resistance to inhibition of 4ebp1.
Our results indicate that CSPG4 play a role in decreased sensitivity of melanoma cells
to mTOR pathway inhibition.

This history of Likiep Atoll in the northern Ratak region of the Marshall Islands explores the cultural, epistemological, and historical context of Paramount Chief Jortōkā's sale of Likiep to A. Capelle & Co. employee and partner José Anton deBrum of Portugal in 1877 and deBrum's transfer of ownership to his employer ten months later. The investigation applies an eclectic ethnographic approach to reveal historical and cultural dynamics not reflected in surviving documents but that likely played a key role in the momentous transaction. Factors considered include the physical condition of the land; chiefly rivalries and the prevalence of land sales and leases as an alliance building strategy; the pervasiveness of violence and epidemic disease; genealogies and genealogical connections; and the flexible application of indigenous philosophies to land use and tenure practices. Also featured are counternarratives employed following the sale by particular sectors of Likiep society as part of a strategy to maintain their place within the atoll's cultural, historical, and genealogical landscape by calling the sale into question and challenging the truth of history in the process.
The dissertation's focused methodology and use of diverse cultural and historical resources demonstrates the important contributions ethnography can make not just to local interpretations of history, but also to ongoing academic discussions of translocal themes such as colonialism and imperialism, islander agency, accommodation and resistance, Christian conversions, indigenous knowledge and epistemology, land and sovereignty, and the practice and construction of history itself. The narrative in turn challenges the effectiveness of sweeping regional histories that, while significant for the larger trends they elucidate, do not capture the multiplicity of situated events, experiences, and interpretations that make Oceania so diverse for its many pasts, presents, and futures. Throughout, the project demonstrates that localized histories and historiographies are key to understanding the vast and expanding region of Oceania and to the ongoing dehegemonization of the discipline of Pacific History and Pacific studies more generally.

▼ RESUMEN A pesar de los esfuerzos de los distintos gobiernos, el aprendizaje de una segunda lengua extranjera sigue siendo una de las asignaturas pendientes de nuestro sistema educativo actual. De entre todas las destrezas, la de la expresión
oral es la gran damnificada. La inseguridad y ansiedad que produce tener que hablar en una lengua que no es la nativa es una de las razones por las que el nivel de competencia comunicativa
oral de los estudiantes es bastante deficiente. En este trabajo plantearemos una propuesta para desarrollar esta destreza productiva utilizando algunas de las técnicas más características de uno de los métodos concebidos expresamente para proporcionar seguridad al estudiante: el método de enseñanza en comunidad. Para ello nos serviremos de la herramienta digital Flipgrid a fin de sacar un mayor provecho de las actividades comunicativas del libro de texto de 1º de ESO y favorecer el desarrollo de aspectos como la pronunciación, la fluidez y la corrección.

O cancro oral é a forma de cancro mais comum da cabeça e pescoço, e nos últimos anos, tem-se observado ao aumento da sua incidência, bem como, aumento da sua mortalidade.
Numa fase inicial do carcinoma espinocelular oral, a forma mais prevalente de cancro oral, as manifestações clínicas e os sintomas são mínimos, sendo por isso importante o conhecimento por parte do médico dentista desta patologia, para o seu diagnóstico atempado e tratamento precoce, devido ao seu grande impacto na saúde de um paciente.
Sabe-se que, praticamente todos os cancros orais são precedidos por alterações clínicas visíveis na mucosa oral, denominadas lesões potencialmente malignas. Uma lesão pré-maligna consiste numa doença, síndrome ou um achado clínico, que se não tratado pode conduzir a cancro. Na cavidade oral, existem inúmeras lesões visíveis que podem alertar o médico dentista para uma possível perturbação potencialmente maligna. O diagnóstico precoce das lesões orais, é um assunto importante devido às complicações sérias que podem ocorrer quando o tratamento é atrasado, particularmente tratando-se de lesões potencialmente malignas. O seu diagnóstico e tratamento precoce, permite portanto a deteção ou prevenção do cancro, diminuindo a sua incidência e melhorando a sobrevida daqueles com carcinoma espinocelular oral.
As lesões orais potencialmente malignas são um problema de saúde grave, com uma prevalência considerável, onde a deteção precoce influencia grandemente o prognóstico destes pacientes. Este trabalho tem como objetivo uma abordagem das lesões potencialmente malignas, proporcionando ao médico dentista informação atualizada e relevante, tendo como intuito promover um diagnóstico mais precoce, sensibilizar para um diagnóstico diferencial, bem como as várias modalidades de tratamento e orientação destes pacientes. Serão abordadas as lesões pré-malignas mais prevalentes, que de acordo com os estudos são a leucoplasia, a eritroplasia, o líquen plano e a fibrose submucosa. Deste modo, foi realizada uma revisão da literatura, tendo em consideração as publicações científicas nos últimos 5 anos. Foi conduzida uma pesquisa nas bases de dados da Pubmed e ScienceDirect, com os termos “oral premalignant lesions”, “oral leukoplakia”, “oral erythroplasia”, “oral submucosal fibrosis” e “oral lichen planus”. Oral cancer is the most common form of head and neck cancer, and in recent years, it has been observed that it’s incidence as been increaseing, as well as its mortality.
At the early stage of oral squamous cell carcinoma, the most prevalent form of oral cancer, the clinical manifestations and symptoms are minimal, so it is important to the dentists, to know this pathology, for its timely diagnosis and early treatment due to its great impact on the health of a patient.

Objective: The present preliminary study examined how oral feeding improves oral health status in dysphagic inpatients.Background: Research suggests that the oral health condition of dysphagic patients is affected by not only oral health care but also oral intake ecause oral feeding is expected to facilitate salivary secretion and orofacial motor action.Materials and methods: Twenty-one dysphagic patients participated in the present study. Clinical data including feeding status, oral health condition, and ingestion-related motor function were collected every week until the patients were discharged from the hospital. All recorded scores were compared between the first and last examination. The correlations among the items were investigated.Results: Feeding status, including intake level and dietary form, was significantly improved. Oral hygiene and tongue coating were also significantly improved, while ingestion-related function was less changed. Oral hygiene, tongue coating, and tongue moisture were significantly improved with improvement in feeding status. A few parameters of the oral health condition were significantly correlated with those of ingestion-related function.Conclusion: Oral resumption may be important to maintain or improve oral health in 2 dysphagic patients, although oral intake does not improve or affect the whole ingestion function in the short term.

Varhaislapsuuden karies on merkittävä kansanterveysongelma varsinkin lapsirikkaissa maissa ja väestöissä. Karieksen hoitaminen vie paljon voimavaroja ja aiheuttaa mittavia taloudellisia seuraamuksia. Karies voi ilmaantua lapselle jo vauvaikäisenä, pian ensimmäisten maitohampaiden puhjettua suuhun. Alle 3-vuotiaiden karieksesta on kuitenkin niukasti tilastotietoja. Maailman terveysjärjestökin suosittaa tietojen keräämistä vasta 3-vuotiaiden ikäryhmästä. Heistä kariesta sairastaa Suomessa 16 %, Yhdysvalloissa 25 %, Englannissa 30 %, Iranissa 46 % ja Saudi-Arabiassa 61 %.
Tämä väitöstutkimus selvitti karieksen esiintymistä ja sen vaaratekijöitä 1─3-vuotiailla Teheranissa. Lisäksi tutkimus arvioi perusterveydenhuoltoon sisällytetyn karieksen ehkäisyn tuloksellisuutta. Tutkimuskohteiksi arvottiin Teheranista 18 neuvolaa. Jokaisessa oltiin 4 päivää, jolloin kaikkia rokotuksiin tulleita 1─3-vuotiaita äiteineen pyydettiin osallistumaan tutkimukseen. Kahta lukuun ottamatta kaikki äidit suostuivat, ja aineistoon tuli kaikkiaan 504 lasta äiteineen. Kaikki 1-vuotiaat, 242 lasta äiteineen, valittiin karieksen ehkäisykokeiluun. Sitä varten neuvolat jaettiin kolmeen ryhmään, joista kaksi (A ja B) oli koeryhmiä ja yksi (C) oli vertailuryhmä.
Tutkimus alkoi äidin haastattelulla. Siinä selvitettiin perheen koulutus- ja tulotaso sekä lapsen ruokinnasta imetyksen kesto, yösyötöt ja päiväaikaan nautitut makeat. Vielä kysyttiin lapsen ja äidin suuhygieniatavoista ja äidin kokemuksista lapsen suun puhdistamisessa. Sitten hammaslääkäri tutki lapsen suun ja kirjasi karieksen ja hammasplakin esiintymät.
Suun tutkimuksen jälkeen äiti ja lapsi siirtyivät rokotushuoneeseen. Koeryhmissä (A ja B) äidit saivat terveydenhoitajalta suunterveyttä koskevan esitteen ja kehotuksen lukea se huolellisesti. Lisäksi ryhmässä A terveydenhoitaja kertoi suun ja hampaiden terveydenhoidosta saman esitteen avulla, ja neuvolan henkilökunta muistutti suunhoidon tärkeydestä puhelimitse kahdesti seuraavan puolen vuoden kuluessa. Vertailuryhmässä äideille ei annettu suunhoidon ohjeita. Kaikissa ryhmissä äitejä muistutettiin seuraavan rokotuskerran ajankohdasta, muttei mainittu tulevaa toista hammastarkastusta.
Varhaislapsuuden kariesta sairasti ikäryhmästä riippuen 3─26 % tutkituista 1─3-vuotiaista, ja 65─76 %:lla oli hammasplakkia. Äideistä 68 % harjasi hampaansa päivittäin ja 39 % puhdisti lapsensa suun päivittäin. Mitä useammin äiti harjasi omat hampaansa, sitä paremmin hän huolehti lapsen suun puhtaudesta. Rintaruokinta oli yleistä eikä lisännyt kariesvaaraa. Yöllä pullomaitoa saavilla karies oli 5 kertaa yleisempää kuin muilla. Neuvolassa saatu ohjeistus ehkäisi selvästi karieksen syntyä puolen vuoden kokeessa.

Tämän väitöskirjan tavoitteena oli kuvata suusyövän yleisyyttä ja siihen vaikuttavia tekijöitä Teheranissa, Iranissa tutkimalla suusyöpäpotilaita, suusyöpäkasvainten ominaisuuksia, potilaille tehtyjä diagnooseja ja niiden viivästymistä sekä heidän selviytymistä sairaudestaan.
Suusyöpäkasvainten tietoja kerättiin 1042 suusyöpäpotilaalta. Nämä tiedot kerättiin 30 suurimman Teheranilaissairaalan potilaskortistoista vuosien 1993-2003 ajalta. Eloonjäämisanalyysiä varten tiedot kerättiin vuosien 1996-2003 arkistoista 470 suusyöpä- ja 82 huulisyöpäpotilaan osalta ja heitä seurattiin vuoden 2005 loppuun.
Potilaan kokemien ensioireiden ja lopullisen syöpädiagnoosin välistä viivettä varten kerättiin tiedot Teheranilaisista sairaaloista 100 peräkkäisen suusyöpäpotilaan tiedoista vuosien 2004-2006 välillä. Ns diagnostinen viive jaettiin kahteen osaan: 1) ensioireiden ja ensimmäisen sitä seuranneen lääkärikäynnin väli ja 2) ensimmäisen lääkärikäynnin ja lopullisen diagnoosin välinen ero.
Useimmat suusyövät olivat pitkälle edenneitä diagnoosin tekemisen hetkellä, kasvain oli siis yli 4 senttimetriä halkaisijaltaan ja/tai kaulan alueen imusolmukkeissa oli jo etäpesäkkeitä. Eloonjäämistodennäköisyys viiden vuoden aikavälillä oli suusyöpäpotilaille 30% ja huulisyöpäpotilaille 62%, mitkä olivat merkittävästi alempia kuin yleisesti länsimaissa vastaavat luvut. Tämä tutkimus osoitti, että keskimääräinen diagnostinen viive oli korkea (7,2 kk, SD 7,5), erityisesti kun niitä verrataan kehittyneimpien terveydenhuoltojärjestelmien vastaaviin tietoihin. Yleensä potilaasta aiheutuva viive oli huomattavan suuri ensioireiden ja lopullisen diagnoosin välisestä ajasta.
Tässä tutkimuksessa tehtyjen havaintojen pohjalta on perusteltua esittää kehitettäväksi ennaltaehkäisevä tiedotusohjelma, jossa kansalaiset voisivat saada enemmän tietoa suusyövästä, sen ensioireista jotta he hakeutuisivat aikaisemmin hoitoon. Lisäksi terveydenhoitohenkilöstöä, erityisesti hammaslääkärejä ja suuhygienistejä tulisi kouluttaa varhaisen diagnoosin tekemiseksi, jotta Iranissa tehtävien suusyöpähoitojen lopputulokset paranisivat.
Oral Cancer in Tehran, Iran: An approach for understanding disease burden.
The present study investigated the burden of oral cancer in Tehran, Iran in terms of patient and tumour characteristics, survival rate, and delay in diagnosis, with the main focus on oral cavity cancers.
For exploring the characteristics of malignant oral tumours, data were obtained from records of 1042 patients diagnosed with invasive oral cancers during 1993-2003 in 30 major hospitals in Tehran, and analysed in three groups: tumours of the lips, oral cavity, and major salivary glands. ▼ This work aims at investigating the characteristics of oral presentations made in Spanish classes and verifying: how the students in the audience, the researcherteacher and the presenter act during these activities; the affective aspects revealed by the students and the evaluation they make of such activities; as well as observing the linguistic problems revealed at the oral presentations. The set of participants consists of five students taking the last year of a Language course in Portuguese and Spanish and their respective literatures, and the teacher of the group, who is also the researcher. The research took place in a private college in the State of São Paulo inland. The data were collected in Spanish language classes in 2005. The instruments for data collecting were: two questionnaires, guided oral interviews, audio recordings, assessment cards, conversation based on the assessment cards, and field notes. This study is based on theories of: oral presentation in several contexts (Goffman,1981, Ander-Egg &Aguilar ,1981, among others) and in the educacional context (Graham,1994, Underhill, 1987, among others); the communicative approach (Almeida Filho, 1993, Widdowson, 1991, Scott, 1981, Breen &Candlin,1980); communicative competence (Savignon, 1983, Canale &Swain, 1980, among others); Brazilians speaking Spanish (Ferreira, 1997, Kulikowski &González, 1999, Alcaraz, 2005, Almeida Filho, 1995); and finally, the concept of transference (Selinker, 1972, Lado, 1957, Kellerman,1978, Cruz , 2004). The results reveal the students consciousness raising of: their linguistic problems, their roles as students and their responsabilities as future teachers, the procedures which can reduce affective factors and the importance of oral presentations as a pedagogical activity. Este trabalho tem por objetivos investigar as características da apresentação oral desenvolvida nas aulas de língua espanhola e verificar a atuação das alunas da platéia, da professora-pesquisadora e da apresentadora durante essas atividades; os aspectos afetivos revelados pelas alunas e a avaliação que fazem dessa atividade; e observar os problemas lingüísticos revelados nas apresentações orais. O quadro de participantes é constituído por cinco alunas cursando o último ano do curso de Licenciatura em Letras, com habilitação em português e espanhol e suas respectivas literaturas, e a professora do grupo, que também é pesquisadora. Os dados desta pesquisa foram coletados em uma faculdade particular do interior de São Paulo, durante as aulas de língua espanhola, em 2005.
