▼ Background: Ibuprofen, diclofenac and aspirin belong to the same class of drugs called NSAIDs, but are used interchangeably at Tonga Hospital. The problem with this approach is that it may lead to preventable misguided and increased spending on pharmaceuticals. Aim: To investigate the pharmacoeconomic implications of interchangeable use of oral NSAIDs for pain management at a district hospital. Setting: This study was conducted at a district hospital in the Tonga village in the Nkomazi municipality, Mpumalanga Province, South Africa. Methods: A quantitative retrospective descriptive study, using existing patient records as well as medicine stock control records, was conducted to investigate the cost-effectiveness of oral NSAIDs when used interchangeably in the management of pain in adult patients at a district hospital. Results: The total number of patients included in this study was 211 in a split of 104 in 2013 and 107 in 2014. The mean ages of all the patients who participated in our study in 2013 and 2014 were 36 and 35 years respectively and there were more females than males. Most patients who presented at Tonga Hospital for pain management were suffering from minor bodily/joint pains (36.0%, n=76), whereas the least number of patients were suffering from bone fractures (10.9%, n=23). Our study found that most patients (31.3%; n=66) treated with ibuprofen were suffering from minor bodily/joint pains, whereas the least number of patients (1.4%; n=3) treated with diclofenac were suffering from inflammatory conditions. Females were the largest users of NSAIDs (both ibuprofen and diclofenac) in Tonga Hospital when compared with males. Patients between the ages of 19-35 years were the most prevalent (28.4%, n=60) who were treated with ibuprofen when compared with patients 18 years and below (9.5%, n=20). Also, patients who were 18 years and younger and treated with diclofenac were the least number of patients (1.4%, n=3). The highest total NSAID stock volumes issued from July to December of 2013 and 2014 combined was ibuprofen (36978 packs) when compared with diclofenac (11127 packs). The stock volumes for both ibuprofen and diclofenac were higher in July, with 8170 for Ibuprofen and 2099 for diclofenac. Diclofenac stock volumes fell to their lowest (1583) in September, whereas Ibuprofen stock volumes fell to their lowest (4478) in December. The fall in stock volumes issued might be attributable to many factors including but not limited to non-delivery by the supplier or non-ordering by the pharmacy staff. In all instances ibuprofen stock volumes issued were higher than that of diclofenac. The acquisition cost of ibuprofen when calculated as mean price per tablet during 2013 to 2014 was consistently lower, with an average price of (0.285 ZAR) when compared with diclofenac (0.995 ZAR). 0.3% of the population of Nkomazi east, on average, gets treatment of ibuprofen daily whereas 0.01% of the population gets treatment of diclofenac daily. ▼ Substrate-selective Inhibition of Cyclooxygenase-2: Molecular Determinants, Probe Development, and In Vivo Effects. Daniel John Hermanson Dissertation under the direction of Professor Lawrence J. Marnett Cyclooxygenase-2 (COX-2) oxygenates arachidonic acid (AA) to form prostaglandins (PGs), but also inactivates endocannabinoids (eCBs) in vitro. We have identified and characterized a series of novel probes termed “substrate-selective” COX-2 inhibitors (SSCIs) that selectively inhibit eCB oxygenation with no impact on PG production. Using X-ray crystallography and site-directed mutagenesis we have elucidated the binding determinants of a series of SSCIs. The phenomenon of substrate-selective COX-2 inhibition arises from the fact that although COX-2 is a structural homodimer, it is a functional heterodimer. Kinetic analyses have revealed that substrate-selective COX-2 inhibitors bind in the allosteric COX-2 subunit and act as non-competitive inhibitors of eCB oxygenation in the catalytic subunit through a conformational change involving Leu-531. We have explored the biological impact of SSCIs in multiple settings including stimulated dorsal root ganglion cells and the brain. These studies established that SSCIs act in a COX-2-dependent manner to decrease the production of eCB-derived PGs and increase the levels of eCBs while having no impact on PG production or AA levels. Importantly, the biochemical effects of SSCIs are unique relative to other modes of COX inhibition. Augmenting eCBs in the brain with SSCIs leads to cannabinoid receptor 1 activation and anxiolytic effects in multiple pre-clinical models of anxiety. Thus, COX-2 is a critical regulator of eCB signaling in vivo. We have identified AA as a non-competitive inhibitor of eCB oxygenation by COX-2 in vitro. Inhibition of AA production in simulated macrophages through pharmacological or genetic inhibition of cytosolic phospholipase A2 (cPLA2) leads to a robust decrease in AA and PGs with increased eCB-derived PGs. Lipidomic analyses identified that this is due to enrichment of AA-containing phosphatidylinositols, diacylglycerols, and 2-arachidonoyl glycerol upon pharmacological or genetic inhibition of cPLA2. These studies classify the determinants of the oxygenation of AA and eCBs by COX-2 in stimulated macrophages. Advisors/Committee Members: H. Prostaglandins (PGs) are important signalling factors for bone mechanotransduction. The inhibition of cyclooxygenase, responsible for the synthesis of PGs, with non-steroidal anti-inflammatory drugs (NSAIDs) has been shown to influence bone formation induced by mechanical stimulation. The purpose of this study was to examine the timing effects of NSAID administration on: 1) bone formation induced by multiple mechanical loading events in a rat model and 2) the PGE2 response of MLO-Y4 osteocyte like cells stimulated by fluid shear stress. The rat forelimb compression model was used to induce bone formation in male and female rats using a 1-month loading protocol (12 loading sessions). The right forelimbs were loaded and the left forelimbs served as non-loaded controls. NSAIDs were administered orally either before or after loading. Fluorochrome labels were administered to the rats to determine mineral apposition rate (MAR). The NSAIDs examined (indomethacin, NS-398 and ibuprofen) did not significantly affect periosteal MAR, administered either before or after loading, suggesting NSAIDs do not affect bone adaptation to multiple mechanical loading events. To examine in vitro effects of NSAIDs on PGE2 production, an orbital shaker was used to apply fluid shear stress to MLO-Y4 cells seeded in 6-well culture plates. Indomethacin was added to the culture media either before or after loading and media PGE2 concentrations were determined at various time points by enzyme immunoassay. Fluid shear stress increased PGE2 production of MLO-Y4 cells and indomethacin administration inhibited that response when administered both before and after fluid flow. However, PGE2 production was influenced by the media changes that occurred in the in vitro experiments, making it difficult to differentiate between indomethacin effects and media change effects. The in vitro experiments revealed the difficulties of modeling the timing effects of NSAID administration on MLO-Y4 PGE2 production in response to fluid flow.

Bone is a dynamic tissue that can adapt to mechanical loading. Prostaglandins (PGs) are important signalling factors produced by osteocytes, the bone mechanosensing cells, that help to activate various cells and cell processes leading to changes in bone structure. Blocking PG signalling with non-steroidal anti-inflammatory drugs (NSAIDs) has been shown to influence bone formation induced by mechanical stimulation in animals and humans. The purpose of this study was to examine the timing effects of NSAID administration on: 1) bone formation induced by multiple mechanical loading events in rats and 2) the PG production of osteocyte like cells in response to fluid flow stimulation. The results of this study suggest that NSAIDs, administered either before or after loading, do not affect bone responses to multiple mechanical loading events. Further investigation is needed to determine the translatability of these findings to NSAID use around the time of exercise in humans.

▼ Det finns många läkemedel som höjer blodtrycket eller interagerar med antihypertensiva läkemedel vilket motverkar en blodtryckssänkning. Den vanligaste interaktionen är en försämring av de antihypertensiva läkemedlens effekt på grund av intag av non steroidal anti-inflammatory drugs, NSAIDs (7). Syftet med denna litteraturstudie är att försöka besvara: Hur påverkas olika grupper av antihypertensiva läkemedel? Hur påverkar traditionella NSAID preparat respektive COX-2 hämmare effekten av antihypertensiva läkemedel? Hur allvarliga är interaktionerna? Vilken dos behövs för att en interaktion mellan NSAIDs och antihypertensiva läkemedel ska uppstå? Vilka typer av patienter utgör riskgrupper? Nio randomiserade kontrollerade kliniska studier inkluderades i detta examensarbete. Studierna undersökte sju olika NSAIDs; diklofenak, ibuprofen, indometacin, piroxikam, sulindac, celecoxib och rofecoxib, och representerade flertalet olika typer av antihypertensiva läkemedelsbehandlingar. Ingen signifikant påverkan på blodtrycket sågs då NSAIDs kombinerades med kalciumantagonister. En signifikant ökning av blodtrycket sågs för ACE-hämmare, ARBs, betablockare och diuretika. Ingen av studierna visade någon signifikant skillnad på interaktionskänslighet mellan ACE-hämmare, ARBs, betablockare eller diuretika. Blodtrycksökningar sågs för diklofenak, ibuprofen, indometacin, piroxikam, och rofecoxib. Celecoxib undersöktes i tre studier och påverkade inte blodtrycket i någon av dem. Slutsatser: Den individuella kardiovaskulära risken för olika NSAIDs beror på flera olika faktorer förutom COX-2 specificitet. Risken är svårbedömd och ännu kan inga NSAIDs klassas som säkra vad gäller kardiovaskulär risk. Dock tycks celecoxib sakna blodtryckhöjande effekter och skulle kunna vara ett bra alternativ för de hypertoniker som behöver samtidig behandling med NSAID preparat. Trots detta kan inga slutsatser dras om celecoxibs kardiovaskulära risk då preparatet skulle kunna påverka andra riskfaktorer förutom blodtryck. Interaktionsmekanismen mellan NSAIDs och antihypertensiva läkemedel är fortfarande oklar och fler studier på detta område behövs. Men troliga teorier är att NSAIDs hämning prostaglandinsyntesen ger en ökad perifer resistens samt en ökad volymretention. Båda dessa effekter ger ett ökat blodtryck. Interaktionen ger relativt små blodtrycksökningar men dessa är kliniskt relevanta då de kvarstår en längre tid. Hos hypertonipatienter där samtidig behandling med NSAIDs är nödvändigt bör blodtrycket och njurfunktion stå under noggrann övervakning.

▼ Despite the progress made in reducing morbidity, dyspepsia is one of the most common GI diseases, resulting in impaired quality of life and work loss. The purpose of this study was to assess the management of dyspepsia in health centers of Addis Ababa city administration. A cross sectional study was conducted from September 1, 2016 to January 30, 2017 by using previously tested data extraction tool for extract relevant information, which then entered to Epi data and SPSS 20 was used for data analysis. A logistic regression was employed to examine the association between variables. A total of 850 patient with dyspepsia were enrolled, from those study participant 29% had got inappropriate managements. Of all dyspepsia patient who attend health centers 34.2% and 65.8 % were received one drug prescription and triple drug prescription respectively. Regarding appropriate management based on the cause of dyspepsia, patients with NSAIDS are less likely to have appropriate dyspepsia management (AOR= .718(.441, 1.168); P=0.001) than H. Pylori (AOR= 1; P= 0.001) and Stress cause dyspepsia (AOR= 1.642(.893, 3.021). Patients’ age group from 16 to 30 account 46.4%, which is a higher prevalence of dyspepsia compare to other age group in the study, also women 58.1% have a higher prevalence of dyspepsia. Based on the finding of this study, it can be concluded that health professionals are mostly follow the guidelines but not always with respect to the use of single and triple treatment in management of dyspepsia which depend on the cause. Proton pump inhibitors drugs have high prescription rate than histamine 2 antagonist and antacid regardless to the cause. Almost half of patients with dyspepsia use NSAIDS. Patient who need additional antibiotic that was not received were higher than patient who received antibiotic that was not needed so it is important that intervention is needed to educate clinicians about the danger of irrational drugs use and to promote the recommended guide lines in the management of dyspepsia. ▼ Background: Acute postoperative pain can develop into chronic pain if not managed well. Nurse anesthetists consider many factors when developing an anesthetic plan to provide optimal postoperative pain management. Multimodal pain management is recommended for managing pain in the perioperative period and this may include administering medications such as intravenous (IV) acetaminophen and IV non-steroidal anti-inflammatory drugs (NSAIDs) to the patients if not contraindicated. Even though these are valuable and daily considerations for nurse anesthetists, there is not a standard of care for addressing postoperative pain management in adult abdominal surgical procedures. Objective: The purpose of the project is to determine the perspectives of nurse anesthetists for developing postoperative pain management in adult patients having abdominal surgical procedures. Design: This is a descriptive study designed to determine if current nurse anesthetist practices are being guided by evidence-based practices and if the findings can be used to develop a standard of care. Participants: Nurse anesthetists from a local Tucson, AZ health care facility Measurements: Nurse anesthetists were interviewed in-person. They were asked six semi-structured questions and the answers were recorded and transcribed into the program NVivo. Each answer was then coded and compared for emerging common themes. Results: Five out of eleven nurse anesthetists participated in this project. Thirty-five themes were discovered and three main categories developed: 1. Nurse anesthetists consider multimodal pain management an integral part of the anesthetic plan. IV acetaminophen and IV NSAIDs are considered for every surgical patient if they are not contraindicated; 2. IV acetaminophen is used more frequently for abdominal procedures compared to IV NSAIDs because of risks for bleeding associated with IV NSAIDs; and 3. Each anesthetic plan is individualized to safely address both the patient and surgical factors. Conclusion: A multi-center study should be considered for a future project to determine if these common themes would be found consistently across health care facilities. Eventually, this information could be used to develop a standard of care for managing postoperative pain in adults having abdominal surgery. Other methods for addressing multimodal pain management such as regional blocks should be considered for future studies as well. ▼ Nonsteroidal anti-inflammatory drugs (NSAIDs) are one of the most frequently prescribed classes of drugs worldwide. Due to their excellent effects for analgesic, anti-inflammatory and antipyretic, most elder population used frequently for osteoarthritis. Gastrointestinal symptoms and ulceration associated with NSAIDs are common. Such ulcers may cause pain, bleeding, or perforation. It leads to other medical problems. The aim of this study was to examine the utilizations of medical resources associated with inpatients with NSAID related peptic ulcer disease and compared to non-NSAID related peptic ulcer disease. The study used the database from a teaching hospital in southern Taiwan. Inpatients who were identified ICD-9 CM codes as peptic ulcer diseases from January 1st of 2008 to December 31 of 2008 were included in this study. We also examined the indications for usage of NSAIDs, the date of prescription before the index date. The results showed 17.6% of inpatients with peptic ulcer disease related to NSAIDs. Inpatients with NSAIDs related peptic ulcer disease compared with inpatients with non-NSAIDs related peptic ulcer disease had significant difference in age and comorbidity. Although rapid urease test positive rate for Helicobacter pylori was higher in group of non-NSAID related peptic ulcer disease, it seemed underestimate because of the test number of patients was low. The average total direct medical cost of inpatients with NSAID related peptic ulcer was NT 36,491 and non-NSAID related peptic ulcer was NT 37,266.1. There were no significant difference in medical costs of standard care , intensive care , blood products,endoscopy, endoscopic hemostasis, surgery, CT scan , ultrasound, laboratory tests,medications, doctorâs service between the two groups. Diagnostic and therapeutic procedures were no statistically significant difference, including blood transfusion,CT scan, endoscopic hemostasis, surgery, symptoms presentations, and intensive care. In this study, the duration for using NSAID was within 30 days for inpatients associated with NSAID related peptic ulcer, which had a substantial excess numbers of ulcer hospitalization. Since the common disease for using NSAID is osteoarthritis,which is very popular in elderly, therefore, we suggested that the policy makers of the National Health Insurance should be aware that preventive usage of proton pump inhibitors for the elder population who need frequent use of NSAIDs might decrease NSAID related ulcer complications. Nonsteroidal anti-inflammatory drugs (NSAIDs) are a widely used class of drugs, due in part to the effective anti-inflammatory and analgesic properties they exhibit. Unfortunately, NSAIDs also exhibit substantial gastrointestinal (GI) toxicity. The mechanisms underlying the ability of NSAIDs to cause ulceration in the stomach and proximal duodenum are well understood, and this injury can largely be prevented through the suppression of gastric acid secretion by proton pump inhibitors (PPIs) or histamine H2 receptor antagonists (H2RAs). In contrast, the pathogenesis of small intestinal injury induced by NSAIDs (i.e., NSAID-enteropathy) is poorly understood, and there are no proven-effective therapies. This is a major clinical concern as NSAID-induced enteropathy and bleeding occur more frequently than NSAID-induced gastropathy, and is associated with significantly higher rates of morbidity and mortality. There is clear evidence that indicates important contributions to NSAID-enteropathy by bile, enteric bacteria, and the enterohepatic circulation of NSAIDs. However, it is not clear which of these mechanisms is/are the primary driver(s) of intestinal damage and injury. There is also evidence that hydrogen sulfide (H2S) can protect the GI mucosa from ulceration and reduce the severity of NSAID-induced GI damage, although the mechanisms of H2S-induced intestinal protection remain to be determined. Therefore, the central aim of this thesis was to evaluate the roles of bile, enteric bacteria, and the enterohepatic circulation of NSAIDs in the pathogenesis of NSAID-enteropathy, and to investigate the ability of H2S to protect the small intestine from NSAID-induced damage. Chapter 1 is an introduction to the relevant literature and Chapter 2 is an outline of the thesis scope and objectives. In Chapter 3, I demonstrated that the co-administration of an H2S-releasing agent protected rats from NSAID-induced enteropathy, in part by preventing NSAID-induced dysbiosis and bile cytotoxicity. In Chapter 4 and 5, I established that the co-administration of PPIs and H2RAs exacerbated NSAID-enteropathy in part by causing intestinal dysbiosis and enhanced bile cytotoxicity. Lastly, I demonstrated that the small intestine-sparing effects of an H2S-releasing NSAID, ATB-346, are partly attributable to the reduced enterohepatic circulation of ATB-346 or the naproxen liberated from this drug (Chapter 5). In summary, the work presented in this thesis provided novel understanding of the complicated pathogenesis of NSAID-enteropathy by confirming that the nature of the bile, the enterohepatic circulation of NSAIDs, and the nature of the intestinal microbiota are of paramount importance. In addition, the results also demonstrated that hydrogen sulfide represents an effective preventative therapy for NSAID-enteropathy and that H2S-releasing NSAIDs, such as ATB-346, have remarkable preclinical safety.

▼ Selenium (Se) is an essential micronutrient with anti-inflammatory properties that are fundamental to human health. A vital component of many metabolic pathways, an inverse causal relationship of Se with inflammatory diseases, including cancers, and infections has been suggested. While deficiency is frequently the center of discussion, some studies have emphasized the importance of different Se levels in disease regulation, establishing the true physiological significance of this element. The metabolic action of Se is seen through selenoproteins, which are synthesized by a complex mechanism. With over 25 identified selenoproteins, the health implications of these proteins are seen in their ability to decrease oxidative stress and inflammation. The study presented here aims to understand the anti-inflammatory effects of Se. Using a combination of ex-vivo and in-vitro macrophage models, the anti-inflammatory benefits of Se were investigated using a bacterial endotoxin, LPS, and Th-2 cytokine, IL-4, treated murine bone marrow derived macrophages (BMDM). Previous studies have shown that Se supplementation of macrophages led to a decrease in LPS-induced expression of inducible nitric oxide synthase (iNOS), a prototypical marker of classical (M1) macrophage activation. In the current study, IL-4 treated macrophages supplemented with 100 nM Se significantly increased the expression of alternatively activated macrophage (M2) markers, Arg-I, Ym1, and Fizz1. Se treatment also increased the enzymatic activity of Arg-I. Conversely, expression of classically activated macrophage (M1) markers, TNFα, iNOS, and IL1β, were significantly decreased in LPS treated macrophages cultured in 100 nM Se and IL-4, suggesting a synergistic effect of Se and IL-4. Interestingly, studies utilizing Trspfl/flCreLysM KO mice macrophages, characterized by a total loss of selenoprotein expression, or those lacking GPX1, a highly expressed selenoprotein, showed a complete abrogation of Arg-I activity, even under high Se (250nM) conditions, demonstrating the importance of selenoproteins for alternative macrophage activation, leading to attenuated inflammation. To effectively translate these findings into an in-vivo model, a gastrointestinal helminthic parasite model was used. The helminthic parasite Nippostrongylus brasiliensis (Nb) is a short-lived murine parasite that migrates from the skin via the lungs to the small intestines, before clearance 14 days after infection. Mice infected with Nb and supplemented with Se significantly increased the expression of the M2 markers Arg-I, Ym1, and Fizz1 in the small intestines, while decreasing the presence of intestinal worms and fecal eggs. To implicate macrophage-specific selenoproteins in resolution of the infection, Se supplemented Trspfl/flCreWT and Trspfl/flCreLysM mice were infected and compared. ▼ Pharmaceutical drugs are used extensively by humans and domestic animals. The detection of compounds such as non-steroidal anti-inflammatory drugs (NSAIDs) in effluents of sewage treatment plants and surface waters has raised concerns about whether these drugs have the potential to impact aquatic organisms. However, little is known about either the mechanism of action of NSAIDs or their impact on aquatic organisms. A key indicator of animal stress performance is the elevation in plasma cortisol levels, the primary circulating corticosteroid in teleosts, and the associated metabolic changes in response to stressor exposure. The secretion of cortisol is under the control of the hypothalamus-pituitary-interrenal (HPI) axis with the terminal step involving the activation of interrenal steroidogenesis by adrenocorticotropic hormone (ACTH) from the pituitary gland. Cortisol, predominantly via glucocorticoid receptor (GR) activation, is involved in a wide array of animal functions, including growth and metabolism, osmo- and iono-regulation, stress and immune function and reproduction, all of which play a role in regaining homeostasis after stressor insult. The overall objective of this thesis was to investigate the role of NSAIDs in impacting the evolutionarily-conserved adaptive stress response in a model teleost fish, the rainbow trout (Oncorhynchus mykiss). Specifically, the impact of NSAIDs on stress coping mechanisms was investigated by examining: i) interrenal steroid biosynthetic capacity and cortisol production, ii) target tissue GR function, iii) cellular heat shock protein response, iv) tissue-specific metabolic response to stressors, and iv) ionoregulatory performance in seawater. The experimental approach involved a series of whole animal in vivo and in vitro studies, using rainbow trout interrenal cell preparations, with two NSAIDs, salicylate and ibuprofen, commonly detected in our surface waters. Fish were subjected to stressors of varying intensity and duration, including handling disturbance, heat shock and salinity exposures, to identify targets impacted by NSAIDs in fish. NSAIDs did not affect resting plasma cortisol levels but disrupted the acute ACTH-stimulated corticosteroidogenesis in vitro and stressor-induced plasma cortisol response in vivo. The mode of action of NSAIDs in disrupting cortisol production involves inhibition of the key rate-limiting step, the steroidogenic acute regulatory protein (StAR), in steroidogenesis. Also, tissue (brain, liver and gill) GR protein content is a target for endocrine disruption by NSAIDs leading to abnormal negative feedback regulation of plasma cortisol levels and reduced target tissue responsiveness to cortisol after stressor exposure. The drugs also clearly affected the cellular stress response in rainbow trout by perturbing the expression of heat shock protein 70 (hsp70), a highly conserved stress coping mechanism. ▼ This thesis has addressedth e issue of primary care prescribing in different European countries. The first hypothesis investigated was: that the planned implementation of a multinational consensus-basedE uropean Formulary in primary care will result in more rational prescribing. A controlled trial involved 235 GPs from eight European countries with half the GPs participating in an educational intervention. This comprised dissemination of the Formulary and discussion of antibiotic and NSAID prescribing. Details of 101,544 doctor-patient consultations were collated and prescribing was compared and contrasted, before (Phase 1) and after the intervention (Phase 11), using performance indicators. This included measurement of the prescribing concordance with drugs recommended in the Formulary which increased by 2.9% (SEM 0.7) between Phases I to 11 in the intervention group and decreased by 1.3% (SEM 0.6) in the control group. This difference was found to be highly significant (p <0.001). Although some changes in clinical practice occurred, more notable differences were found in prescribing patterns between countries. A second hypothesis followed: that identification of the main influences on the participating GPs'prescribing will assist in the explanation of the varying effects of the Formulary in the different countries. A two-stage Delphi questionnaire study asked the GPs to identify the factors which they perceived to influence their prescribing and to rate their importance. The most important influences were drug related characteristics in six countries, followed by education/information and then patient factors. Pharmaceutical industrial factors were considered the least important influence in six countries, which followed regulatory factors in five countries. More influential factors appeared to be in the GPs' control, rather than ones imposed by national health care systems, regulation and government. The results show that the extent of Formulary adoption varied in different European countries. There remains a continuing place for the promotion of rational prescribing principally through education and information, including prescribing guidelines. Future initiatives may be more appropriate within countries but require adequate and sustained professional and government support.

▼ Photoallergic contact dermatitis (PACD) is a clinical problem that has often been poorly understood and neglected by dermatologists over recent years. This can be partly attributed to its investigation by photopatch testing (PPT) falling between the expertise of photobiologists and contact dermatitis clinicians. One result of this situation was that no European Baseline PPT series had been agreed on. To redress this, the European multi centre photopatch test study (EMCPPTS) was conceived to provide up to date information on which photoallergens are of greatest clinical relevance. Its conduct and results form the core research project of this thesis. To enable the EMCPPTS to proceed and its results to be viewed in a wider context, the other Chapters of this thesis explore important related aspects of PACD and PPT in Europe. The introduction examines the nature of PACD and PPT and reviews current photoallergens. Then, the investigation of the two photoallergens carprofen and chlorproethazine by PPT is recounted. These studies highlight deficiencies within the current European regulatory system for preventing photoallergens from reaching the marketplace, as well as providing templates for the investigation of new photoallergens in the human environment. This is followed by a pilot PPT study which provides new information on the optimum non-irritating concentration of the 19 ultraviolet sunscreen absorbers to be used in the EMCPPTS. The issue of attempting to determine the photoallergenic potential of the EMCPPTS agents with respect to exposure patterns is addressed by conducting a sunscreen survey in the UK. The EMCPPTS itself is then detailed, as well as the difficulties that can be encountered when conducting a large clinical study of this nature. The results from the EMCPPTS and other presented studies were shown to be of importance in deciding upon a new European Baseline PPT series. The process involved in deciding this series, as well as its content are described before overall conclusions and possible future studies in the domain of PACD and PPT are discussed.

▼ The main goals of this research were to synthesize nitrogen oxide releasing diazeniumdiolates and their prodrugs and to evaluate their pharmacological effects. The different projects and their results are described below. i. Comparison of HNO and NO donating properties of cyclic amine diazeniumdiolates Diazeniumdiolates are an attractive class of donor compounds as they can be tuned to release NO or both NO and HNO depending upon the amine backbone. Isopropylamine (IPA/NO) and cyclohexylamine (CHA/NO) diazeniumdiolates are currently the only examples of primary amine based diazeniumdiolates. A series of structurally related cyclic amine based diazeniumdiolates were synthesized and characterized. An acetoxymethyl derivative was also synthesized to facilitate cellular uptake and to achieve higher HNO levels in cells. ii. Nitrogen oxide releasing diazeiumdiolate based adducts of N-des-methyl-tamoxifen Nitrogen oxide (NO/HNO) donating diazeniumdiolate adducts of N-desmethyltamoxifen (a key metabolite of the breast cancer drug tamoxifen) were synthesized. DEA/NO-AcOM, an NO donor was also synthesized to monitor the effect of NO on breast cancer cell survival. Derivatives of N-desmethyltamoxifen were found to be effective towards estrogen receptor positive (ER+) cells only. DEA/NO-AcOM was found to be cytotoxic towards estrogen-dependent and independent cell lines, in combination with tamoxifen, or by itself. iii. Synthesis and characterization of nitrogen oxide adducts with non-steroidal anti-inflammatory drugs (NSAIDs) Our group has shown HNO releasing diazeniumdiolate derivatized aspirin to be comparably effective in preventing gastric ulceration to NO-releasing diazeniumdiolate based aspirin analogues. Series of such NSAID adducts were further extended by synthesizing such derivatives of indomethacin and niflumic acid. NO/HNO releasing analogues of aspirin and indomethacin were cytotoxic towards two different breast cancer cell lines, irrespective of estrogen dependence.iv. Chlorambucil analogue of PABA/NOChlorambucil, an alkylating agent is used in leukemia treatment. Tumor cells resistant to alkylating agents often have increased glutathione levels and increased activity of glutathione-S-transferase (GST). PABA/NO is an NO donor with a promising anticancer profile. The chlorambucil analogue of PABA/NO was synthesized to utilize GST for releasing NO and to potentially overcome cellular resistance. Les analgésiques tel que le paracétamol, ou acétaminophène (ACE), et les anti-inflammatoires non stéroïdiens (AINS) comme l’aspirine et l’ibuprofène, sont largement utilisés dans le monde pour soulager douleur, fièvre ou inflammation. Ces médicaments, vendus sans ordonnance et souvent pris en automédication, sont de plus en plus utilisés par les femmes enceintes depuis les années 1990, notamment aux États-Unis et en Europe. De nombreuses études de cohorte ont mis en avant l’association entre la prise de ces molécules par les femmes durant le premier et deuxième trimestre de grossesse, et l’apparition de malformations congénitales des organes reproducteurs des garçons nouveau-nés, comme la cryptorchidie ou l’hypospadias. Ces analgésiques inhibent l’activité enzymatique des cyclooxygénases (Cox), qui catalysent l’étape clef de la voie de biosynthèse des prostaglandines (PGs), dont la prostaglandine D2 (PGD2), impliquée dans la détermination sexuelle des lignées somatique et germinale de la gonade mâle chez la souris. Afin d’étudier si la gonade embryonnaire peut constituer une cible de ces molécules, l’objectif principal de cette thèse a été d’analyser l’impact de l’exposition in utero à l’ACE, l’aspirine et l’ibuprofène seuls ou en combinaison, pendant la période de détermination sexuelle (de 10,5 à 13,5 jours post coitum (jpc) chez la souris). Des doses « thérapeutiques » similaires à celles utilisées chez l’Homme ont été administrées par voie orale. Mon travail a permis de démontrer que l’exposition in utero à la combinaison des deux drogues ACE et ibuprofène induisait, dans le testicule embryonnaire, une accélération de la différenciation de la lignée germinale du testicule embryonnaire, précurseurs des gamètes à l’âge adulte, par le biais d’une reprogrammation épigénétique avancée, ainsi qu’une augmentation du stockage du glycogène dans les cordons testiculaires, via l’activation de l’expression des gènes de la matrice extracellulaire. De plus, ce projet a permis d’identifier pour la première fois le profil d’expression des prostaglandines dans le testicule embryonnaire de souris. Dans un deuxième temps, l’impact de l’exposition in utero à ces drogues sur la stéroïdogenèse a été étudié, en parallèle avec l’étude sur le rôle de la PGD2 dans les cellules stéroïdogènes (cellules de Leydig) dans le testicule fœtal de souris à 17,5 jpc. Ces résultats pourraient conduire à modifier l’utilisation de ces médicaments chez la femme, au cours des deux premiers trimestres de grossesse.

▼ The objective of this study was to evaluate the clinical, cerebrospinal fluid (CSF), and histopathologic effects of an epidurally administered NSAID (ketorolac) in dogs. This was performed as a blinded, placebo controlled study using twenty-two adult mixed breed dogs with 16 treatment and 6 control dogs. Dogs were anesthetized and epidural catheters were placed at the lumbosacral space. Catheter placement was evaluated fluoroscopically. Ketorolac (0.4 mg/kg) or placebo (5% ethanol) was administered epidurally over a 52 hour period, with 5 injections given at 12 hour intervals. At 1, 2, 4, or 8 hours after the first and last injection of ketorolac, dogs were anesthetized and CSF was obtained. Control dogs had CSF sampled 1 hour after the first and last ethanol injection. Neurologic function and pain response was evaluated before and during the study. Selected dogs were then euthanized and necropsies performed. None of the dogs exhibited any clinical or neurological abnormalities during the study. No statistical difference was noted in pain response or CSF analysis between treatment and control dogs. Gross necropsy revealed gastrointestinal ulceration of varying degrees in all treatment dogs. Histopathologic analysis of the spinal cord and meninges revealed minimal focal leptomeningeal phlebitis in 2 of 8 treatment dogs and minor subdural inflammation in one control dog. No changes to the neural structures were noted in any dogs. Epidural administration of ketorolac did not cause clinical signs, alteration in CSF values, or pathologic changes to the spinal cord when used for short duration. Gastrointestinal ulceration was common when ketorolac was administered epidurally at 0.4 mg/kg every 12 hours for 5 treatments. ▼ Summary of Background Data: Trends of increasing mortality with prescription opiate use have been reported in multiple publications. In populations with CNMP, adjuvant medications are commonly combined with opiates to decrease symptoms and/or side effects. Studies evaluating ACM of these combinations have not been reported in a workers’ compensation population.Objective: To determine the relative risk and predictors of all-cause mortality (ACM) among workers’ compensation patients prescribed opiates for chronic non-malignant pain (CNMP) alone and in combination with commonly utilized adjuvant medications compared to those taking NSAIDs only.Methods: A historical cohort study of workers’ compensation population in Ohio from 01/01/2000 to 06/10/2011 was conducted. NSAIDs only patients (controls) are compared to cases who are prescribed short acting opiates only (SA), short and long acting opiates (SLO), short and long acting opiates and/or anxiolytic and/or sedative and/or hypnotic medication (SLO + ASH), and short and long acting opiates and/or any muscle relaxers and/or antidepressants (SLO + MR-AD). Outcome of interest is ACM. Relative risk and adjusted risk factors of ACM are presented for each medication group.Results: When comparing to NSAIDs only subjects, patients taking SLO + ASH have the highest RR for death, 3.22 (95 percent CI, 1.60-6.94). Age, sex, dose and duration on medications are not significant predictors of mortality. Patients who are prescribed SLO + MR-AD also have increased risks of mortality, RR 1.73 (95 percent CI, 1.15-2.59). Age, sex and the number of emergency room visits are significant predictors of ACM.SLO and SA opiates only groups have ACM RR of 1.57 (95 percent CI, 0.92-2.71) and 1.23 (95 percent CI, 0.89-1.69) respectively. Age and total medications prescribed are significant predictors of ACM in both of these groups.Conclusion: The use of any anxiolytic, sedative, and/or hypnotic medications with short and long acting opiates in combination poses a significant increased risk of ACM. A lesser, but still highly significant risk of death is noted when combining SLO and MR-AD medications. Health care providers should be aware of these risks to provide safe medical care for the injured workers. Advisors/Committee Members: Haynes, Erin (Committee Chair).
