▼ Este estudio retrospectivo tiene como objetivo evaluar la prevalencia de mucositis periimplantar de los implantes colocados en los pacientes del Área de Periodoncia de la Clínica Especializada en Odontología de la Universidad de San Martín de Porres en un período comprendido entre los años 2001 y 2010. De 318 pacientes con un total de 955 implantes, se excluyeron 244 pacientes con 720 implantes de los cuales 697 implantes no tenían la información completa en las historias clínicas, 1 implante se retiró durante el procedimiento quirúrgico por no alcanzar estabilidad primaria, 18 implantes presentaron una pérdida temprana y los otros 4 implantes se perdieron de forma tardía. Por consiguiente, 212 implantes dentales colocados en un total de 74 pacientes fueron incluidos en la muestra de este estudio. Se utilizó la presencia de sangrado al sondaje (SAS) como parámetro de diagnóstico para la mucositis periimplantar.
The aim of this retrospective study was evaluated the prevalence of peri-implant mucositis of subjects treated with dental implants at the Clinic Specializing in Dentistry at the University of San Martin de Porres from 2001 to 2010. Materials and methods: 318 patients with a total of 955 implants, 244 patients with 720 implants were excluded, 697 implants didn’t have the complete information in medical records, 1 implant was removed during the surgical procedure because it didn´t achieve primary stability, 18 implants were earlyloss and 4 implants were late loss. Consequently, 212 dental implants placed in a total of 74 patients were included in this study. We used the presence of bleeding on probing (BOP) as a diagnostic parameter for periimplant mucositis.

▼ 5-fluorouracil (5-FU) is one of the most commonly prescribed anti-neoplastic drugs in modern cancer treatment. Although the drug is effective at destroying cancer cells, its administration is accompanied by serious, dose-limiting side effects, amongst the most prevalent of which is intestinal
mucositis. This disorder is characterised by ulceration and inflammation of the small intestine, and sufferers often experience severe abdominal pain, nausea and diarrhoea. Despite its predominance, there are currently no definitive treatments for intestinal
mucositis. Probiotics are defined as live bacteria which are able to exert beneficial physiological or therapeutic effects. Strains can be sourced from either food or the human microbiota, but must meet specific requirements prior to being officially recognised as probiotics. The mechanisms of probiotic action are highly species and strain specific, however, a number of strains have been shown to exert beneficial effects which may be suited to the treatment of intestinal
mucositis. These include; inhibition of pathogenic bacterial growth and inflammation, maintenance of cell cycling and strengthening of the intestinal barrier. While the majority of probiotic research has focused on the use of live bacteria, there has been a recent interest in bioactive factors that are secreted by the bacterial cells into the cell-free supernatant (SN). There are a range of benefits to using SNs in preference to live bacteria, such as reduced risk of sepsis and greater quality control during production. This thesis represents the first detailed examination into the efficacy of probiotic-based SNs in the treatment of 5-FU-induced intestinal
mucositis. Firstly, four different probiotic SNs were investigated in vitro for their ability to maintain cell growth following administration of 5-FU. The two strains deemed most effective where then assessed in an in vivo model of intestinal
mucositis. Rats were treated with SNs both before and after 5-FU administration. Improvement was reported in some indicators of intestinal damage in rats following SN administration. However, the overall effects were less pronounced than expected, given the extent of improvement reported in the in vitro model. These findings suggested that a different screening method was required prior to in vivo examination, and that the current in vivo treatment protocol required review. As
mucositis occurs only following chemotherapy administration, there is opportunity to administer therapeutic compounds prior to the onset of the disorder with the aim of preventing its development, rather than treating the damage. Two strains, Lactobacillus rhamnosus GG (LGG) and Escherichia coli Nissle 1917 (EcN), were examined for their ability to prevent 5-FU-induced reduction in intestinal barrier function and increased epithelial cell apoptosis in an in vitro model. ▼ IntroduÃÃo: Mucosite intestinal à um efeito adverso comum dos regimes anticÃncer de primeira linha para o tratamento do cÃncer colorretal. Dentre esses fÃrmacos estÃo o irinotecano e 5-FU utilizados em associaÃÃo. Muitos estudos tÃm sido realizados sobre a patogÃnese da MI. No entanto, novas investigaÃÃes sÃo necessarias, pois o curso da MI pode variar de acordo com o fÃrmaco e regime empregados. Objetivou-se entÃo desenvolver um novo modelo experimental de MI induzida pela combinaÃÃo de IRI e 5-FU em camundogos. MÃtodos e Resultados: camundongos C57BL/6 (20-25g, n=6) foram injetados com salina (100ÂL, i.p), IRI (30 ou 45 mg/kg, i.p), 5-FU (25, 37,5 ou 50 mg/kg, i.p) ou IRI+5-FU em doses combinadas por 4 dias. No 7 dia, diarreia, perda de peso e contagem de leucÃcitos foram registradas. ApÃs a eutanÃsia dos animais, amostras do Ãleo foram coletadas para anÃlise histopatolÃgica, atividade de mieloperoxidase (neutrÃfilo/mg proteÃna), nÃveis de TNF-α e IL-6 (pg/mg tecido). Teste Kaplan-Mayer log rank, Kruskal Wallis/Dunnâs ou teste ANOVA/Bonferroni foram usados para analise estatistica. p<0,05 foi aceito como significativo. A melhor combinaÃÃo de dose para induzir a MI sem mortalidade importante no 7 dia foi 5-FU (37,5 mg/kg) + IRI (45 mg/kg) (0% mortalidade), que foi entÃo usada para estudos subsequentes. IRI+5-FU induziu (p<0,001) diarreia (2[0-3]), perda de peso (86,7Â3,9g), e leucopenia (7,3 2,3x103) versus grupo salina (0[0-1]; 101,1Â0,6; 215,5 54,1, respectivamente) ou cada droga dada sozinha (5-FU: diarreia (0[0-1]), perda de peso [92,6Â2,7], e leucopenia [30,4 13,4]; IRI: diarreia (0[0-1]), perda de peso [94,8Â2,1], e leucopenia [49,2 5,5]). Adicionalmente, IRI+5-FU induziu infiltrado de cÃlulas inflamatÃrias, perda de vilos e arquitetura das criptas (4[4-4]), aumento na atividade de MPO (14641Â1598 neutrÃfilo/mgproteÃna), TNF-a (3,2Â0,9 pg/mg de tecido), IL-6 (1,4Â0,5 pg/mg de tecido) nÃveis teciduais versus grupo injetado com salina (0[0-1], 5747Â1155; 0,7Â0,2; 0,3Â0,1) ou com fÃrmacos injetados isoladamente (5-FU: dano intestinal (2.5[2-3]), MPO [3788Â1212], TNF-α [0,7Â0,2], IL-6 [0,2Â2,3]; IRI: dano intestinal (1[0-2]), MPO [3580Â1613], TNF-α [0,4Â0,2], IL-6 [0,07Â0,05]) (p<0,05). ConclusÃo: Desenvolvemos um novo modelo experimental de MI induzida pela combinaÃÃo de IRI+5-FU em camundongos, que abre perspectiva para um maior conhecimento sobre a patogÃnese da MI.
Introduction: Intestinal mucositis is a common side effect of anticancer regimens for first-line treatment of colorectal cancer. Among such drugs are irinotecan and 5-FU used in combination. ▼ O cÃncer colorretal (CCR) à um dos mais comuns tumores malignos atualmente. Regimes à base de irinotecano (IRI) tÃm sido utilizados no tratamento do CCR e do CCR metastÃtico. Contudo, tais regimes estÃo associados ao surgimento de mucosite intestinal (MI), muitas vezes limitante do tratamento. A MI e a diarrÃia grave sÃo efeitos colaterais frequentes que podem atingir de 15-25% dos pacientes em quimioterapia. No entanto, o manejo clÃnico destes efeitos secundÃrios ainda à parcialmente ineficaz. O estudo dos mecanismos e mediadores envolvidos na resposta imune no estado de pÃs sepse vem evidenciando que nesta condiÃÃo parece haver uma inibiÃÃo de eventos inflamatÃrios entre os quais a migraÃÃo de neutrÃfilos. Dessa forma, o estado de pÃs-sepse poderia interferir no curso da MI induzida por IRI. Pretende-se avaliar os efeitos da imunomodulaÃÃo em animais sobreviventes de sepse grave no desenvolvimento das alteraÃÃes inflamatÃrias da MI induzida pelo IRI. Camundongos C57BL/6 machos, 20-25g, foram divididos em 4 grupos (n=5-8), submetidos ao modelo de ligaÃÃo e perfuraÃÃo do ceco (CLP) e tratados com ertapenem 20mg/Kg i.p. 6h depois da CLP e de 12/12h por trÃs dias, para induzir aproximadamente 50% de sobrevida. Animais submetidos à cirurgia mas nÃo à perfuraÃÃo do ceco (SHAM) receberam soluÃÃo salina (Sal, 5 mL/Kg, i.p.). Quinze dias apÃs CLP, os animais sobreviventes e animais do grupo SHAM receberam soluÃÃo salina (Sal, 5 ml/Kg, i.p.) durante 4 dias ou IRI (75mg/kg, i.p.). A massa corpÃrea (g) foi mensurada diariamente. No 5 ou 7  dia, foi avaliada a contagem total de leucÃcitos (x103/ÂL) e a diarreia (escores). ApÃs a morte, o jejuno e o Ãleo foram recolhidos para a dosagem da mieloperoxidase (MPO), IL-1β e IL-10 (pg/mL) e anÃlise histopatolÃgica, ANOVA/Bonferroni ou Kruskal Wallis/Dunn foram usados como testes estatÃsticos. P <0,05 foi aceito. Protocolo CEPA 84̸ 2014. Animais pÃs-sÃpticos por CLP tratados com IRI apresentaram reduÃÃo dos escores de diarreia e atividade da MPO no 5 e 7 dia (diarreia 5D: [0(0-0)]; 7D: [0(0-2) / MPO do jejuno e Ãleo, 5D: 2,96Â1,01; 2,35Â0,76 e 7D:0,42Â0,14; 0,92Â0,19, respectivamente) quando comparados com os animais SHAM tratados com IRI (diarreia 5D: 1[0-1]; 7D:[2,5(1-3)] /MPO do jejuno e Ãleo, 5D: 8,16Â1,48; 7,96Â0,96; 7D: 2,51Â0,67; 2,87Â0,65, respectivamente). AlÃm disso, animais pÃs-sÃpticos tratados com IRI apresentaram reduÃÃo nos danos histopatolÃgicos no 5 e 7 dia (5D no jejuno e Ãleo: [1(1â3)] e [1(0-1)]; e 7D no Ãleo: [2(1-3)] em relaÃÃo a IRI (5D no jejuno e Ãleo: [4(2-4)] e [4(1-4)] / 7D: Ãleo [4(3-4)]) e aumento dos nÃveis da citocina IL-10 (5D no jejuno e Ãleo: 55,80 3,42; 34,45Â1,87; 7D: Ãleo: 133,00Â20,25) em comparaÃÃo com SHAM + IRI (5D no jejuno e Ãleo: 34,45Â1,87; 6,91 3,05; 7D no Ãleo: 63,67 9,08). ▼ Abstract: Chemotherapy is one of the treatments used to cure cancer, and have as a principle the use of drugs that alter the metabolism of neoplastic cells leading to cell death. Despite of the benefits of chemotherapy, the drugs also affect normal cells and cause side effects that directly affect the quality of life of patients. Such effects in the oral cavity can be classified as acute (
mucositis, hyposalivation, candidosis, herpes) occur during the treatment, and late (hyposalivation) occur after completion of treatment.
Mucositis is the most important acute side effect, and mainly causes pain and dysphagia. Besides the change in the amount of saliva, chemotherapy also alters the saliva composition, predisposing oral tissues to some collateral effects. Proteins SPLUNC (short palate, lung and nasal epithelium clone) were identified in salivary glands, saliva, fluid from the airways and other tissues, with a possible role in the innate immune system in these regions. The aim of this study was to evaluate the correlation between the expression of proteins SPLUNC1 and SPLUNC2A with the presence of side effects such as
mucositis and hyposalivation during chemotherapy, as well as the quantification of these proteins before, during and after treatment. Were performed three collections of whole saliva in 20 healthy subjects, 20 patients undergoing conventional chemotherapy and 15 patients undergoing conditioning regimens prior to autologous hematopoietic stem cell transplantation and proteins were evaluated by western blotting. We observed a significant change in the quality of saliva during chemotherapy, characterized by increased expression of SPLUNC1, but it was not observed any association with worsening of hyposalivation and

▼ Radiation is widely used for treatment of head and neck cancer. But the acute side effect of radiation, especially oral mucositis, is observed almost all patients. Several radio-protective agents are developed but there is no definite agent to prevent. We found novel synthetic compound 3-Amino-3-(4-fluoto-phenyl)-1H-quinoliune-2,4-dione (KR-22332) as an candidate of radio-protective agent and the aim of this study is to investigate the effectiveness of KR-22332 in vitro. The cell viability, apoptosis, reactive oxygen species generation, and mitochondrial membrane potential as well as changes in the signal pathway related to apoptosis were investigated. Then, the embryotoxicity of KR-22332 were explored in a zebrafish model. Radiation-induced apoptosis and altered mitochondrial membrane potential in primary human keratinocyte (HaCaT) was observed. KR-22332 inhibited radiation-induced apoptosis and intracellular reactive oxygen species generation. KR-22332 significantly reduced the expression of ATM, p53, and TNF-α compared to their significant increase after radiation treatment. The results of this study suggest that KR-22332 significantly inhibited radiation-induced apoptosis in human keratinocyte and may be a safe and effective candidate treatment for the prevention of radiation-induced mucositis.
방사선 치료는 두경부암에서 광범위하게 사용되고 있다. 하지만 방사선 치료의 급성 부작용인 구내염은 거의 모든 방사선 치료 환자에서 관찰되고 있으나, 이를 완벽하게 예방할 수 있는 약제는 아직 없는 현실이다. 본 실험에서는 합성물인3-Amino-3-(4-fluoto-phenyl)-1H-quinoliune-2,4-dione(KR-22332)의 in vitro에서 방사선 조사 성 구내염의 예방물질로의 가능성을 확인하였다. 방사선 조사에 의한 세포 사멸 및 미토콘드리아 막 전위의 변화가 사람 각질세포(HaCaT세포)에서 관찰되었는데, KR-22332는 방사선 조사에 의한 사람 각질세포의 사멸 및 세포 내 활성 산소의 생성을 억제하였다. 이러한 KR-22332의 효과는 ATM, p53, TNF- α 의 발현을 저하시켜서 나타나는 것으로 생각된다. 본 실험은 KR-22332가 사람 각질세포의 방사선 조사에 의한 세포사멸을 억제시키며, 방사선 조사에 의한 구내염을 예방하는 후보 물질로 향후 추가적인 연구가 필요하다는 것을 밝히는 것에 의의가 있다.

mucositis incidence and oral care outcomes, and to explore possible risk factors for oral
mucositis among inpatient children receiving cancer treatment at the Women’s and Children Hospital, Adelaide, Australia. The objectives were to investigate the evidence on oral
mucositis prevention, assess and validate the combined use of the Children’s International
Mucositis Evaluation Scale (ChIMES) and the World Health Organization (WHO) oral
mucositis scale in recording oral
mucositis incidence, develop and implement a standardized hospital oral care protocol, and to record prospectively oral
mucositis incidence, oral health outcomes, and possible risk factors. Methods: A systematic review was conducted to assess the current evidence on oral
mucositis prevention among children. Results of a previous retrospective study were used to design a prospective pilot study. The pilot study was carried out for seven months during which the new oral care protocol was implemented and the ChIMES and the WHO oral
mucositis scale were validated through daily recording of oral
mucositis in the oncology ward. Measures of reliability and compliance were assessed among nurses and dental staff involved in recording oral
mucositis and oral health status. The pilot study was followed by a prospective clinical observational study and recorded measures of oral
mucositis (12 months) and oral health status (24 months). Measures of oral health outcomes were assessed initially and then every three months through clinical examination to record dental caries and oral hygiene while measures of oral
mucositis was recorded daily during the hospital stay of recruited children. The incidence of oral
mucositis, oral health outcomes, and dental treatment utilization were then analyzed to explore possible risk associations. Results: The systematic review supported the benefit of implementing a standardized oral care protocol to prevent oral
mucositis among children. Thirty-eight children were conveniently sampled during the pilot study during which high levels of reliability and compliance (87%) in using ChIMES and WHO oral
mucositis scales were achieved. Dental referrals increased from 53% to 100% after adopting the comprehensive oral care protocol. Sixty-seven children were recruited during the prospective part with oral
mucositis incidence similar to that of the pilot study (33% versus 34%). Dental caries prevalence was 28% with absence of new carious lesions throughout the 24 months follow up. Regular dental reviews were significantly related to shorter duration of oral

mucositis (GIM) has become increasingly recognised as a major toxicity of cancer treatment. The efficacy and safe use of irinotecan (a topoisomerase I inhibitor chemotherapeutic drug) is compromised because of GIM. Severe GIM often necessitates dose reduction or treatment discontinuation thus compromising patient survival. In rat studies, irinotecan has been shown to cause apoptosis, histological damage, inflammation and activation of signalling pathways. These signalling pathways include nuclear factor kappa B (NF-κB), tumour necrosis factor (TNF)/stress and toll-like receptors (TLR). The stimulation of TLRs can lead to early or late NF-κB activation, which up-regulates many genes involved in the development of GIM, including pro-inflammatory cytokines. Despite extensive research in this area, there is currently no clinically therapeutic intervention to prevent GIM development following irinotecan administration. Therefore, research focusing on designing therapeutic strategies targeted to specific pathological pathways is greatly needed. In the light of newly emerging roles in inflammatory diseases for TLR2 and TLR4, these receptors have gained significant attention in the development of GIM. TLRs play a role in NF-κB regulation, pro-inflammatory cytokine activation, intestinal inflammation, and regulation of proliferation and apoptosis. Given that these cellular events are key characteristics of GIM, it is suggested that they may be central mediators of the injury process. Recently, it was reported that TLR2 and TLR9 are involved in doxorubicin-induced GIM, and TLR4 is involved in 5-fluorouracil- and methotrexate-induced GIM, providing further evidence as targets for intervention. As such, the overarching objective of this PhD project was to investigate the involvement of TLR2, TLR4 and pro-inflammatory cytokines in irinotecan-induced GIM and the effect of their inhibition, by the antidepressant drug amitriptyline (AMI), on the development of GIM. The first study of this research project investigated the involvement of TLR2, TLR4 and pro-inflammatory cytokines in irinotecan-induced diarrhoea. TLR2, TLR4, interlukin-1 beta (IL-1β), TNF and interleukin-6 (IL-6) mRNA and protein expression was investigated in the colon and jejunum of Dark Agouti rats treated with irinotecan (200 mg/kg intraperitoneally). The expression of each marker (at 72 and 96 h) was compared between rats that developed diarrhoea and rats that did not develop diarrhoea following treatment. These two time points have shown to present maximum damage severity and diarrhoea occurrence, respectively, following irinotecan administration in our rat model. Results showed that mRNA expression of TLR2, TLR4, IL-1β and TNF increased significantly in the colon of rats that developed diarrhoea at 96 h compared to rats that did not develop diarrhoea. ▼ Purpose: To determine whether Livionex Dental Gel affects oral hygiene, bacterial counts, and incidence of mucositis and bloodstream infections (BSI) in pediatric Human Stem Cell Transplant (HSCT) recipients. Methods: This was a prospective, double-blinded, randomized-controlled trial pilot study. Subjects were randomized to Livionex or control (PreviDent 5000 Plus for children ≥6 years or Tom's and Maine Children's Toothpaste for children <6 years) groups in a 2:1 ratio. Gingival index (GI), plaque index (PI), relative amounts of mutans streptococci (MS), total streptococci (TS), Lactobacilli (LB), and total viable count (TVC), mucositis and BSI were recorded and compared using linear mixed effect models across timepoints. Brushing compliance was assessed with questionnaire responses and were compared using the Wilcoxon rank sum test. Results: Twenty-six subjects (19 in Livionex and 7 in control) participated. MS counts decreased significantly in both groups. BSI incidence was not significantly different. There were no significant differences in GI, PI, incidence of mucositis, and questionnaire responses. Conclusions: There is possible benefit from using Livionex in this population. Studies with larger sample-sizes are needed to further confirm the effect of Livionex on bacterial counts and BSI occurrences in HSCT patients.

▼ Mucositis of the alimentary tract (AT) or alimentary
mucositis is a debilitating adverse effect of cancer therapies in a
high percentage of patients receiving chemotherapy, bone marrow
transplantation, and high-dose radiation to the head and neck. This
common oral complication bears a high impact on health, quality of
life, a patient’s ability to adhere with the prescribed therapies,
as well as the overall economic outcome. Relatively little is known
of the pathophysiology of
mucositis and current therapeutic
interventions are ineffective. Recently a new population of
mesenchymal stem cells from human gingiva (hGMSC) have been
discovered (Zhang et al., 2009), which exhibit not only multipotent
differentiation and self-renewal capacities, but also possess
distinct immunomodulatory functions.; We hypothesized that
treatment with hGMSC reduces epithelial injuries and ameliorates
mucositis of the alimentary tract (AT) associated with cancer
therapies. We explored the feasibility of using hGMSC to reduce the
incidence and severity of
mucositis induced by cancer therapies. We
observed a significant reduction of AT
mucositis and recovery of
body weight in nearly 100% of mice undergoing one-dose treatment
with hGMSC. Treatment with hGMSC was capable of reducing the
incidence and severity of mucosal ulceration, promoting
proliferation of mucosal epithelial cells and the regeneration of
damaged crypts at the histological level. Our findings suggest that
hGMSCs can ameliorate
mucositis by promoting the regeneration of
chemotherapy and/or radiotherapy induced mucosal damage. Thus,
hGMSC cell therapy may be a potential agent in the prevention and
treatment of AT
mucositis.

▼ Introducción: Los avances en el tratamiento del cáncer infantil muestran que la
radioterapia en conjunto con cirugía y quimioterapia ha producido un aumento
significativo en la curación de muchas enfermedades malignas pediátricas en
cabeza y cuello. Sin embargo estos tratamientos pueden producir consecuencias
adversas a nivel oral entre las que se encuentran alteraciones de los tejidos
blandos, duros y estructuras anexas. El objetivo de esta investigación fue
determinar la presencia de alteraciones dentarias, flujo salival e historia de caries
dental, en pacientes pediátricos oncológicos en relación a esquemas
terapéuticos basados en quimio-radioterapia o sólo quimioterapia.
Material y métodos: Se realizó un muestreo por conveniencia reclutándose 50
pacientes pediátricos a partir de la población consultante del Hospital Dr.
Roberto del Río. Se registró información a partir de la ficha clínica respecto de
diagnóstico oncológico, evolución, tipo de tratamiento aplicado y momento en
que este se realizó. Se realizó un examen clínico oral en el que se registró el
índice COPD y ceod, flujo salival, junto con alteraciones dentarias de estructura
(opacidades e hipoplasias), de forma, de tamaño (microdoncias) y de número
(agenesias).
Resultados: La muestra final estuvo constituida por 50 pacientes, 23 hombres y
27 mujeres con un promedio de 10,8 años de edad. 22 pacientes pediátricos
oncológicos estuvieron sometidos a esquema de quimio más radioterapia y 28
sólo con quimioterapia. La presencia de caries, microdoncia y agenesia fue más
frecuente en los casos expuestos a radioterapia. No se observó diferencias para
COPD ni ceod como tampoco para flujo salival entre los pacientes sometidos a
los distintos esquemas. No obstante la gran mayoría de los pacientes
conservaban un flujo salival (promedio 0,79 ml/min) dentro de parámetros de
normalidad y el daño por caries (promedio COPD=3.06, ceod=2,56 ) está dentro
de lo esperado para la población infantil chilena sana.
Conclusiones: Los distintos esquemas terapéuticos no se relacionan con mayor
frecuencia de alteraciones dentarias, caries o hiposialia. Sin embargo la
exposición a radioterapia pareciera constituir un evento que predispone a mayor
riesgo para microdoncia, agenesia y caries. Tampoco se observó disminución del
flujo salival ni mayor daño por caries respecto a lo esperado para la población
chilena de niños.

No entanto, um importante efeito colateral associado ao irinotecano, a mucosite intestinal, tem impactado negativamente na qualidade de vida dos pacientes e no sucesso terapÃutico. Trabalhos anteriores demonstraram que na patogÃnese da mucosite intestinal hà a participaÃÃo de mediadores prÃ-inflamatÃrios, como IL-1, IL-18, IL-33, Ãxido nÃtrico dentre outros, cuja modulaÃÃo leva à reduÃÃo do infiltrado neutrofÃlico no intestino e melhora do dano tecidual. Entretanto, o papel de receptores de quimiocinas, como o CXCR2, importantes no recrutamento de neutrÃfilos, ainda nÃo foram investigados no contexto da mucosite. Objetivos: Avaliar o papel de receptores CXCR2 na mucosite intestinal induzida pelo Irinotecano. MÃtodos: Camundongos C57BL/6 machos, 18-25g, foram divididos em grupos (n=6), administrados por 4 dias com salina (5mL/kg, i.p) ou com irinotecano (75, 90, 105 ou 120 mg/kg, i.p). A dose de 120 mg/kg foi a que melhor reproduziu o quadro inflamatÃrio caracterÃstico da mucosite, sendo entÃo utilizada nos ensaios posteriores em associaÃÃo ao SB225002, um antagonista de receptores CXCR2. Os animais foram analisados quanto ao peso corpÃreo, escores de diarreia, contagem de leucÃcitos. ApÃs a eutanÃsia, uma amostra de intestino foi coletada para anÃlise histopatolÃgica e morfomÃtrica, dosagem de mieloperoxidase e nÃveis de IL-1β, IFN-γ e KC. AlÃm disso, o comprimento do intestino delgado foi mensurado, bem como o peso do conteÃdo sÃlido. Avaliou-se tambÃm a bacteremia. Adicionalmente, realizou-se a quantificaÃÃo dos receptores CXCR2 e CCR2, alÃm do ensaio de quimiotaxia in vitro de neutrÃfilos isolados de camundongos tratados com IRI ou com IRI+SB225002. (Protocolo CEPA 58/14). Resultados: O IRI em todas as doses avaliadas promoveu uma significativa (P<0,05) perda ponderal e leucopenia. Sendo que, apenas as doses de 105 e 120 mg/kg foram capazes de reduzir de forma significativa (P<0,05) a razÃo vilo/cripta e de aumentar (P<0,05) o infiltrado neutrofÃlico (ensaio de MPO). Nenhuma das doses avaliadas promoveu diarreia nos camundongos desse experimento. A dose de 120 mg/kg foi que a melhor reproduziu o dano histopatolÃgico tÃpico da mucosite intestinal, sendo a dose escolhida para as demais anÃlises. O uso do antagonista dos receptores CXCR2, o SB225002, associado ao IRI nÃo protegeu os animais da perda de peso, da leucopenia, do dano histopatolÃgico (mensurado pela razÃo vilo/cripta), da reduÃÃo do comprimento do intestino delgado nem da reduÃÃo do peso do conteÃdo do delgado. Todos esses parÃmetros apresentaram-se de forma semelhante nos animais tratados apenas com IRI ou em associaÃÃo IRI+SB225002. Quanto ao infiltrado neutrofÃlico, observamos que o uso do SB225002 no D2, 24h apÃs a 1 administraÃÃo do IRI, foi capaz de reduzir a atividade da MPO (P<0,05). ▼ IntroduÃÃo: MGI à o termo que descreve os efeitos da quimioterapia antineoplÃsica nas mucosas, podendo acometer o trato alimentar de maneira global ou localizada. 15 a 40% dos pacientes em quimioterapia apresentam algum grau de mucosite. Muito da fisiopatologia da MGI permanece desconhecido. A IL-18 à uma citocina pleiotrÃpica, com funÃÃo regulatÃria sobre o sistema imune e envolvimento nas fases iniciais da inflamaÃÃo. Objetivo: Avaliar o envolvimento de IL-18 na patogÃnese da MGI induzida por CPT-11. Materiais e MÃtodos: camundongos Balb/C IL-18Wt ou IL-18KO, machos, foram tratados durante quatro dias consecutivos com CPT-11 (60mg/Kg, i.p.) ou veÃculo (0,5 mL, i.p.). Um grupo de animais IL-18Wt recebeu, alÃm do CPT-11, a proteÃna ligante de IL-18 (IL-18Bp; 200 Âg, i.p.). Os seguintes parÃmetros foram avaliados: diarrÃia, leucograma, sobrevida, anÃlise histopatolÃgica, atividade de mieloperoxidade (MPO), dosagem de citocinas (TNF-α, IL-1β ) por ELISA e imunoistoquÃmica para TNF-α e IL-1β nas mucosas ileais. Resultados: CPT-11 induziu diarrÃia significante e promoveu alteraÃÃes intestinais exuberantes (encurtamento de vilos, achatamento e vacuolizaÃÃo de enterÃcitos, necrose em criptas e infiltrado inflamatÃrio de leucÃcitos polimorfonucleares e cÃlulas mononucleares) Observou-se aumento da atividade de MPO e dos nÃveis tissulares de TNF-α e IL-1β dosados por ELISA e intensa imunomarcaÃÃo com anticorpos anti-TNFα e anti-IL-1β nesses animais. AlÃm disso, tais animais apresentaram resposta contrÃtil intestinal exacerbada ao estÃmulo com colinÃrgicos (acetilcolina e betanecol). Os animais IL-18KO tratados com CPT-11 apresentaram diarrÃia estatisticamente menos severa e menor intensidade de alteraÃÃes morfomÃtricas e histolÃgicas. NÃo houve aumento de TNF-α, mas houve de IL-1β , a atividade de MPO nÃo diferiu da observada nos animais que receberam veÃculo e nÃo houve aumento de resposta contrÃtil ao estÃmulo colinÃrgico. Os animais que receberam CPT-11 e IL-18Bp apresentaram diarrÃia estatisticamente menos intensa que aqueles que receberam apenas CPT-11, resposta contrÃtil estatisticamente menos acentuada e menor atividade de MPO. Entretanto, as alteraÃÃes morfomÃtricas e histolÃgicas nÃo diferiram das encontradas nos animais tratados sà com CPT-11. ConclusÃo: IL-18 està envolvida na patogÃnese da MGI induzida por CPT-11. IL-18Bp atenua os eventos envolvidos na MGI induzida por CPT-11 e à um possÃvel candidato a modulador farmacolÃgico desse processo. Mucosite à um termo clÃnico que descreve uma sÃndrome caracterizada por reaÃÃo inflamatÃria da mucosa de todo o trato digestivo com ulceraÃÃes. A mucosite intestinal por antineoplÃsicos desencadeia alteraÃÃes da motilidade digestiva. A dexametasona à um glicocorticÃide cujo principal efeito farmacolÃgico decorre de sua aÃÃo antiinflamatÃria e imunossupressora Objetivos: Investigar a dismotilidade gastrintestinal associada à mucosite intestinal induzida por irinotecano em camundongos, bem como avaliar o efeito da modulaÃÃo farmacolÃgica da resposta inflamatÃria sobre estas alteraÃÃes motoras. MÃtodos: Camundongos Swiss machos (20â25g) foram tratados do D1-D4 com irinotecano (75 mg/Kg, i.p.) ou com salina (0,1ml, i.p.), o sacrificio foi realizado no sÃtimo dia experimental. Afim de estudar alteraÃÃes relacionadas à mucosite intestinal, amostras do duodeno, jejuno e Ãleo, foram removidas para avaliar a injÃria epitelial por morfometria, escores histolÃgicos, e atividade de MPO, tendo a diarrÃia sido avaliada antes do sacrifÃcio. Para avaliaÃÃo de citocinas amostras de duodeno foram retiradas e pelo mÃtodo de ELISA foi determinada a concentraÃÃo de TNF-, IL-1, KC e IL-10. Para avaliar a motilidade digestiva, os animais foram deixados em jejum de 18 horas do D6 para o D7. No D7, foram administrados 350ÂL da soluÃÃo glicosada (5%) contendo vermelho de fenol (VF) a 0,75 mg/ml em cada animal antes do sacrifÃcio. ApÃs os tempos de 10, 20 e 30 min, os animais foram sacrificados e submetidos a uma laparotomia mediana, sendo o estÃmago retirado completamente e o intestino delgado divido em 3 partes iguais: proximal, medial e distal. A seguir o esvaziamento gÃstrico, trÃnsito intestinal e transito gastrintestinal (centro geomÃtrico) foram determinados por espectofotometria. A seguir, em outro grupo experimental, foi avaliado se a modulaÃÃo farmacolÃgica do processo inflamatÃrio poderia reverter Ãs alteraÃÃes da motilidade digestiva associadas à mucosite por irinotecano. Para tanto, dexametasona (2,5mg/kgm s.c.) ou metoclopramida (10mg/kg i.p.) do D1-D7, foram administradas 30 minutos antes da administraÃÃo de irinotecano. A seguir foram avaliados todos os parÃmetros descritos acima. Resultados: O tratamento com irinotecano foi capaz de induzir uma lesÃo intestinal com um importante comprometimento da barreira epitelial funcional com a presenÃa das seguintes alteraÃÃes: diarrÃia, leucopenia, encurtamento acentuado das vilosidades intestinais, necrose parcial de criptas, aumento da atividade de MPO, aumento na concentraÃÃo de KC e IL-1 e retarde do esvaziamento gÃstrico e transito gastrontestinal e aceleraÃÃo do transito intestinal. ▼ IntroduÃÃo Mucosite intestinal (MI) à um efeito colateral bastante recorrente em pacientes submetidos ao tratamento com irinotecano, fÃrmaco utilizado em esquemas terapÃuticos de primeira linha no tratamento do cÃncer colorretal. Nas Ãltimas dÃcadas muitos aspectos relativos à patogÃnese da MI tÃm sido elucidados, entretanto, pouco se sabe sobre o perfil de cÃlulas linfocitÃrias e o papel destas no seu desenvolvimento. O objetivo deste trabalho Ã, portanto, investigar o papel de linfÃcitos na patogÃnese da MI atravÃs da imunossupressÃo com abatacept. Metodologia Camundongos swiss machos, 20-25g, foram divididos em grupos (n=8) e tratados durante 4 dias com salina (5 mL/kg, i.p.), irinotecano (75 ou 45 mg/kg, i.p.), abatacept (10 mg/kg, i.p. 1h antes do irinotecano no primeiro dia) ou somente abatacept (10 mg/kg, i.p. no primeiro dia). Os animais foram avaliados diariamente quanto ao peso, escore de diarreia e sobrevida. A eutanÃsia ocorreu no dia 5 no primeiro protocolo e no dia 7 no segundo. Coletou-se sangue para a contagem total de leucÃcitos e apÃs a eutanÃsia amostras de Ãleo foram extraÃdas para realizaÃÃo de ensaio de mieloperoxidase, anÃlise histopatolÃgica e dosagem de KC no Ãleo dos animais. Para anÃlise estatÃstica foram utilizados os testes ANOVA/Bonferroni, Kruskal-Wallis/Dunnâs ou Log-rank (Mantel-Cox), com p<0,05 aceito como significativo. Resultados Abatacept reduziu (p<0,05) a sobrevida dos animais no primeiro protocolo, fazendo do dia 5 o ideal para eutanÃsia. Irinotecano causou perda ponderal significativa (p<0,05) em relaÃÃo ao grupo salina, entretanto abatacept nÃo agravou a perda. O prÃ-tratamento com abatacept antecipou o aparecimento de diarreia de grau moderado para o quarto dia. Abatacept induziu reduÃÃo na razÃo vilo/cripta, altura dos vilos e profundidade das criptas de forma significativa em relaÃÃo ao grupo tratado somente com irinotecano (p<0,05), alÃm de aumentar o infiltrado de neutrÃfilos no Ãleo dos animais. Irinotecano aumentou o nÃvel de KC no Ãleo dos animais em relaÃÃo ao grupo salina, entretanto abatacept nÃo modificou o nÃvel desta citocina de forma significativa. No segundo protocolo, a subdose de irinotecano nÃo provocou dano intestinal intenso, porÃm o tratamento com abatacept comprovou seu efeito deletÃrio induzindo o aparecimento de diarrÃia no sexto dia (p<0,05) em relaÃÃo ao grupo salina, alÃm de agravar o dano à arquitetura intestinal. Abatacept elevou a infiltraÃÃo de neutrÃfilos no intestino dos animais tratados com irinotecano, em comparaÃÃo ao grupo injetado apenas com esse fÃrmaco (p<0,05), o que foi associado com os nÃveis elevados de KC no intestino desses animais. Irinotecano promoveu mielotoxicidade em todas as doses, porÃm abatacept nÃo agravou essa toxicidade. Abatacept isoladamente nÃo causou alteraÃÃo em nenhum parÃmetro avaliado. ConclusÃo Abatacept agravou a mucosite intestinal induzida por irinotecano em camundongos.

▼ A radioterapia tem importante aplicação clínica no tratamento das neoplasias malignas de cabeça e pescoço. A mucosite oral causada pelo efeito das radiações ionizantes, nos tecidos normais, manifesta-se como processo inflamatório reativo, com alta prevalência e diferentes graus de envolvimento tecidual. Os avanços alcançados com os laseres de baixa potência, assim como o uso de alguns fármacos, indicam a possibilidade de adoção de uma estratégia eficaz na terapia da mucosite oral. O presente estudo teve como objetivo avaliar a aplicação do laser de baixa potência associado ou não ao uso de antimicrobiano tópico (clorexidina gel 0,2%) no tratamento da mucosite oral. Quatorze pacientes portadores de neoplasia maligna na região de cabeça e pescoço, com indicação de tratamento radio e ou radioquimioterápico foram alocados em dois grupos: Grupo I: laser e Grupo II: laser e clorexidina. Os pacientes foram submetidos a tratamento profilático e terapêutico durante oito semanas. O grau de severidade da mucosite foi avaliado pelo método da OMS e a dor pela escala visual analógica (EVA). Os resultados mostraram que não houve diferença significativa entre a aplicação do laser isolado ou associado ao uso do antimicrobiano tópico. A laserterapia promoveu redução da dor e do grau de severidade da mucosite oral induzida pela radioterapia. Novos estudos devem ser realizados com a finalidade de serem estabelecidos protocolos padronizados e efetivos na abordagem terapêutica da mucosite oral.
Radiotherapy has an important clinical role in the treatment of the malignant tumors of head and neck. Oral mucositis induced by radiotherapy is a reactive inflammatory process of the oral mucosae with high prevalence and different degrees of tissue involvement. The advances reached with low power lasers devices, as well as the use of some medicines, indicate the possibility of adoption of an efficient strategy in handling with the oral mucositis. The aim of the present study was to evaluate the application of the low power laser associated or not with topical antimicrobials use in oral mucositis treatment. Fourteen patients with head and neck cancer undergoing radiotherapy, with or not associated chemotherapy, were located in two groups: Group I: laser and Group II: laser and clorexidine. All the patients were submitted to prophylactic and therapeutical treatment during eight weeks. The grade and severity of mucositis was evaluated using the WHO scale and pain was measured by visual analogue scale (VAS). The study showed that there is not significant difference between the use of laser application only or associated with topical antimicrobials. Lasertherapy promoted decrease of the pain and the severity grade of oral mucositis induced by radiotherapy. New studies must be developed in order to establish standardized and effective protocols in oral mucositis management.

▼ Durante ou após o tratamento, quimio ou radioterápico, o cirurgião-dentista deve atuar na prevenção e no tratamento das sequelas e lesões bucais que possam ocorrer. A laserterapia possui um papel fundamental e inovador considerando-se algumas repercussões bucais decorrentes da radioterapia (RT) e da quimioterapia (QT) especialmente quanto a mucosite bucal e a xerostomia. Foi realizado um levantamento dos prontuários de pacientes sob tratamento de neoplasias na região da cabeça e pescoço que foram submetidos à laserterapia de baixa potência no Laboratório Especial de Laser em Odontologia (LELO) da FOUSP, com objetivo de verificar a adequação e efetividade do protocolo terapêutico, estabelecido para os pacientes na prevenção e no tratamento da mucosite oral pós radio e/ou quimioterapia. Examinados cem prontuários de pacientes submetidos a protocolo de laserterapia de baixa potência (LBP) diodo InGaAlP com 660 nm, 40 mW, 6 J/cm2 em um tempo de 6 segundos por ponto em 49 pontos para o tratamento e/ou prevenção das principais manifestações bucais relacionadas a RT (concomitante ou não com cirurgia e/ou QT), atendidos no período de 2007 a 2010. Não foram incluídos os prontuários que não descreviam a periodicidade das aplicações para prevenção e/ou tratamento com LBP, a evolução clínica do paciente frente à laserterapia, dados daqueles pacientes que estavam em tratamento e relacionados a neoplasias outras que não na região de cabeça e pescoço. Foram incluídos na análise os seguintes dados para o levantamento: gênero do paciente, idade, hábitos nocivos (tabagismo, etilismo), frequência e hábitos de higiene bucal, diagnóstico e região da neoplasia, estadiamento TNM, dose total da RT recebida, outros tratamentos relacionados (cirurgia e/ou quimioterapia), queixa principal odontológica/estomatológica. O diagnóstico da mucosite seguiu os Critérios de Toxicidade Comum do Instituto Nacional de Câncer para escala de mucosite oral radio-induzida. Foram colhidos dados quanto ao protocolo de laserterapia de baixa potência utilizados para os casos de mucosite quanto ao período do tratamento, número total de sessões de laserterapia, a frequência das aplicações e desfecho clínico: expresso pelo paciente e/ou avaliação pelo clínico assistente. Os dados obtidos foram apresentados de forma descritiva, em percentuais e médias. Foram selecionados sessenta e três prontuários referentes a neoplasias na região da cabeça e pescoço. O total de prontuários válidos foi dividido em dois grupos: prevenção (n=21) e tratamento (n=42). No grupo de prevenção foram considerados aqueles relativos às aplicações antes da primeira manifestação da mucosite oral. No grupo tratamento consideraram-se aqueles com descrição de aplicações de laser em lesões de mucosite oral. O protocolo utilizado foi capaz de reduzir as lesões e diminuir as manifestações clínicas da mucosite oral em todos os pacientes tratados. ▼ A mucosite oral constitui efeito colateral dose-limitante da quimioterapia, relativamente freqüente, desconfortável e potencialmente letal. O controle terapêutico é sintomático e ainda inexiste procedimento estabelecido para prevenir a sua manifestação. O objetivo deste estudo foi verificar a eficácia do laser de baixa potência na prevenção da mucosite oral e as influências de variáveis demográficas (idade e sexo), subjetivas (xerostomia e paladar) e objetivas (história médica), em pacientes portadores de adenocarcinoma de cólon, submetidos à quimioterapia com fluorouracil (5-FU) e ácido folínico (Leucovorin®). Quarenta e oito pacientes foram incluídos no estudo. 18 homens e 30 mulheres, na faixa etária dos 37 aos 78 anos com média de idade de 62,5 anos. Os pacientes foram endereçados, seqüencial e alternadamente, para dois grupos. Vinte e cinco pacientes compuseram o grupo submetido à aplicação preventiva de laser (grupo LP) e 23 pacientes receberam aplicação placebo (grupo C). A aplicação do laser, preventivo ou placebo, foi realizada na semana de infusão da quimioterapia, uma única vez, e os pacientes foram reavaliados após sete dias. O laser utilizado foi o AlGaInP, 660nm, fluência de 3J/cm2. Os graus de mucosite foram mensurados pela escala WHO, e a xerostomia e a dor por escala visual analógica (VAS). Os pacientes que desenvolveram mucosite receberam aplicação terapêutica de laser, independentemente a que grupo pertencesse. Vinte e um pacientes cumpriram os seis ciclos programados de quimioterapia (10 C e 11 LP), e 27 pacientes completaram apenas parte do protocolo (13 C e 14 LP). Houve ocorrência similar de mucosite oral entre os grupos, cuja freqüência variou em torno de 50% em cada ciclo, com a seguinte distribuição média: 22% de mucosite grau 1, 18% de grau 2, 10% de grau 3 e nenhum grau 4. Foram registrados 95 episódios de mucosite nos 188 ciclos de quimioterapia dos 48 pacientes incluídos no estudo. Nos 126 ciclos dos 21 pacientes que cumpriram todo o protocolo ocorreram 63 episódios de mucosite. A análise estatística não mostrou diferença significativa quanto à ocorrência de mucosite entre os grupos. Os pacientes do sexo feminino, pacientes acima de 60 anos de idade e a queixa de xerostomia prévia à quimioterapia, embora tenham mostrado tendência a diferença em alguns ciclos, não mantiveram essa característica na análise integral dos resultados. Pacientes com queixa de alteração de paladar prévio à quimioterapia e história de diabetes mellitus e gastrite manifestaram quadros mais graves de mucosite que a média geral da população estudada. O efeito benéfico do laser terapêutico foi altamente significante para os pacientes que desenvolveram mucosite oral. Concluiu-se que uma única aplicação de laser de baixa potência não foi efetiva para prevenir a mucosite oral na população estudada. Pacientes portadores de diabetes, gastrite, queixa de alteração de paladar e aqueles que manifestam graus mais severos de mucosite nos ciclos iniciais da quimioterapia merecem maior atenção no seguimento em função do pior…

▼ Abstract: Radiotherapy causes alteration in saliva composition and PLUNC proteins participate in innate immunity of the oral cavity. The aim of this study was to verify if radiotherapy is able to modify the salivary PLUNC expression and if they are associated with radiotherapy side-effects. MATERIALS AND METHODS: Unstimulated whole-mouth saliva of 65 voluntaries (45 cancer patients and 20 controls) was collected. In the study group the collection was performed one week before the beginning of radiotherapy, in the middle of the treatment and one week after finishing. SPLUNC1 and SPLUNC2A expression were detected by western blotting and was analyzed with clinicopathological data and radiotherapy side-effects. RESULTS: Reduction of salivary flow rates was observed during and after conclusion of radiotherapy, being more accentuated in patients who underwent radiotherapy involving the facial region. Facial radiation field was correlated with collateral effects, mainly with the presence (p=0.0110) and severity (p=0.0143) of
mucositis. SPLUNC1 and SPLUNC2A were detected in saliva of patients without treatment in variable concentrations. The study group showed levels of SPLUNC2A significantly higher than the control group, while SPLUNC1 did not show differences. Concentration of PLUNC was modified by radiotherapy, observing decreasing of glycosilated form of SPLUNC2A levels (p<.0001) and increasing of SPLUNC1 levels (p=0.0081) in the second and third collections. The only association between collateral effects of radiotherapy and PLUNC was the presence of
mucositis (p=0.0363) and its severity (p=0.0500) with higher levels of SPLUNC1. CONCLUSIONS: The present study reported that levels of SPLUNC1 and glycosilated SPLUNC2A are affected by the radiotherapy, suggesting that these proteins may have importance in the oral microenvironment of irradiated head and neck cancer patients

Atualmente existem diversos alvos terapÃuticos no tratamento do cÃncer, um deles reside no fato de inibir a replicaÃÃo do DNA impossibilitando a cÃlula tumoral de se duplicar como à o caso do 5-fluoruracil (5-FU).O objetivo do trabalho foi avaliar o efeito antiinflamatÃrio do peptÃdeo mimÃtico da ApoE (COG 133) em camundongos Swiss desafiados pela mucosite intestinal induzida pelo 5-FU. Utilizamos camundongos Swiss machos com peso de 30g, sendo desafiados com injeÃÃo (dose Ãnica) intraperitoneal de 5-FU (450 mg/kg).Alguns animais foram tratados com diferentes doses (0,3, 1,0 e 3,0 ÂM) do peptÃdeo mimÃtico da apolipoproteÃna E (ApoE) COG133 por via intra-peritoneal, apÃs o desafio com 5-FU. Animais tratados com PBS foram usados como controles. Os camundongos foram sacrificados com soluÃÃo de deslocamento cervical 3 dias apÃs o desafio com 5-FU.Algumas amostras de intestino foram congeladas imediatamente em nitrogÃnio lÃquido e em seguida armazenadas em freezer a -80ÂC para biologia molecular. Outras amostras foram fixadas em formaldeÃdo para processamento histolÃgico.Monitoramos o peso corporal dos animais e fizemos leucometria a partir de coleta retrorbital de sangue para avaliaÃÃo do efeito citotÃxico mielosupressor do 5-FU nos grupos experimentais.Avaliamosos parÃmetros morfomÃtricos de altura de vilo e profundidade de cripta em segmentos do duodeno. Para a detecÃÃo de proteÃnas de interesse, utilizamos o Western blot com a utilizaÃÃo dos seguintes anticorpos IL-1β, TNF-α e iNOS. Para o RT-PCR, avaliamos os seguintes primers IL-1β, TNF-α e iNOS. No ensaio de citocinas por ELISA, utilizamos os seguintes anticorpos IL-1β, IL-10, TNF-α. Nas anÃlises morfomÃtricas de duodeno, encontramos uma grande reduÃÃo na altura de vilos e profundidade de criptas nosanimais tratados com 5-FU, que foi reduzida com com a utilizaÃÃo do peptÃdeo COG133 na dose 3,0 ÂM (p<0,001).Nas anÃlises histopatolÃgicas, observamos um intensoinfiltrado inflamatÃrio nos animais do grupo que foi injetadocomo 5-FU, que foi parcialmente revertido pela administraÃÃo do peptÃdeo COG133.Os dados de ELISA evidenciaram um aumento da quantidade de citocinas prÃ-inflamatÃrias IL-1β (p<0,05), TNF-α (p<0,05) e uma reduÃÃo da citocina antiinflamatÃria IL-10 (p<0,05), quando comparados ao controle salina, em duodenos de animais que foram injetados com 5-FU. A administraÃÃo do peptÃdeo COG 133 reduziu os nÃveis de IL-1β de maneira estatisticamente significante (p<0,05 1μM; p<0,001 3μM), TNF-α (p<0,001 na dose de 1,0 μM); p<0,001 na dose de 3μM) e para IL-10 (p<0,001 na dose de 3μM).Dados de Western blot evidenciam que o peptÃdeo na dose de 3μM reduziu de maneira estatisticamente significante a expressÃo de citocinas como IL-1β (p<0,001), TNF-α (p<0,05) e do mediador inflamatÃrio iNOS (p<0,05) no duodeno. ▼ Background: Colorectal cancer is one of the most deadly forms of cancer in humans. Current chemotherapeutic agents have limited efficacies for colon cancer treatment, and concurrently increase the risk of developing intestinal
mucositis.
Mucositis is the term for painful inflammation and ulcerating lesions of the mucous membranes lining the digestive tract. Intestinal
mucositis is a common side-effect of cancer chemotherapy. To date, there are no satisfactory clinical treatments available to combat
mucositis and colorectal cancer. Probiotics are live bacteria, some of which have been demonstrated to secrete bioactive factors capable of exerting beneficial effects; especially in the digestive system. The potential for probiotic-derived factors to combat intestinal
mucositis and colon cancer has yet to be thoroughly investigated. In particular, the effects of factors derived from the same probiotic strain and cultured in different growth media have not been investigated in either setting. Objective: The project described in this thesis aimed to identify therapeutically effective bioactive formulations (probiotic-derived factors) capable of protecting the digestive tract from the devastating effects of
mucositis and their potential for colon cancer management. The capacity for supernatants (SNs) from Escherichia coli Nissle 1917 (EcN) and Faecalibacterium prausnitzii (Fp) cultured under different growth conditions in the settings of
mucositis and colorectal cancer models in vitro and in vivo were evaluated. Materials and Methods: EcN was grown in Luria-Bertani (LB) broth, tryptone soya broth (TSB), de Man Rogosa Sharpe (MRS) broth, and M17 broth supplemented with 10% (v/v) lactose solution (M17). Fp was grown anaerobically in M2 medium supplemented with 30% rumen fluid. Rat epithelial IEC-6, human colonic adenocarcinoma Caco-2 and T-84 cells were treated with DMEM (control), growth media alone (without factors) or SNs, in the presence and absence of 5-FU. Cell viability (MTT), reactive oxygen species (ROS), cell apoptosis and cell monolayer permeability were determined. Optimal SNs from the in vitro studies were further investigated in female dark agouti rats (101-146g). Animals received a daily oral gavage of EcN or Fp SNs (1mL) respectively from day 0 to 8. At day 5, rats were injected with either saline or 5- Fluorouracil (5-FU; 150 mg/kg). ▼ This study characterized maxillary sinus mucositis (SIMS) adjacent to teeth with apical periodontitis (AP), and assessed its resolution three months after endodontic treatment. 29 subjects who maxillary posterior teeth with AP were imaged with cone-beam computed tomography (CBCT). Resolution of SIMS was assessed with CBCT three months after treatment, and periapical healing was assessed using the PAI after six months. Four non-compliant subjects were discontinued and SIMS was identified in 14/25 subjects (56%). Three months post-treatment, SIMS was resolved fully in 3/10 subjects (30%), and partially in 3/10 subjects (30%). Six months post-treatment, 6/10 subjects (60%) were classified as healed or healing. CBCT revealed a lower-than-expected prevalence of SIMS adjacent to teeth with AP. In specific cases SIMS might linger beyond three months after the elimination of the endodontic infection. Due to the low statistical power, association between the resolution of SIMS and periapical healing could not be explored.

mucositis is defined as damage of oral mucosa caused by unwanted detrimental effects of the cytotoxic chemotherapy. Oral
mucositis presents as widespread painful ulcerations and erythematous eruptions and occurs in between 40-100% of all patients undergoing chemotherapy. Currently, there are no standard treatments available to prevent oral
mucositis and the consequences on health care systems remain extensive. Research into the pathogenesis of oral
mucositis has shown a complex underlying process, involving several overlapping biological events in the epithelium and submucosal layers. However, this pathogenesis is still not fully understood. A group of proteolytic enzymes called matrix metalloproteinases (MMPs) have been recently postulated to play a part in mediating the damaging process seen in oral
mucositis. It is well established that MMPs and their naturally existing inhibitors (tissue inhibitors of metalloproteinases; TIMPs) maintain a balanced level in normal physiological status of oral mucosa. Their dysregulated balance underlies some pathophysiological aspects of several diseases including some diseases of the oral and gastrointestinal mucosae. These mucosal diseases include ulcerative colitis, Crohn‟s disease, recurrent aphthous stomatitis and oral lichen planus. However, as MMPs and TIMPs have not been well studied in oral
mucositis, this formed the basis of this thesis. Hypothesis and Aims of the thesis: If MMPs and TIMPs are involved in the pathogenesis of oral
mucositis, their tissue levels will change following the administration of cytotoxic chemotherapy. This may correlate to any histopathological changes in the oral mucosa. This thesis aimed to investigate the morphological changes and the tissue expression levels of MMP-2, -3 -9 and TIMP-1 within the oral cavity in a well-established pre-clinical animal model of chemotherapy-induced oral
mucositis. Results and Discussion: The findings presented in this thesis demonstrate epithelium thickness reduces without obvious ulceration in the oral mucosa very early after chemotherapy administration. Maximum atrophy is observed 60 min following chemotherapy in both dorsal and ventral surfaces of the tongue. This reduction in epithelial thickness is associated with significant up-regulation of MMP-2, -3 and -9 and down-regulation of TIMP-1 in all layers of the oral mucosa. MMP-9 is also up-regulated at later time point. These findings support previous evidence that oral
mucositis involves changes in the submucosa before it is clinically evident. The early reduction in epithelial thickness confirms similar findings reported in earlier studies of oral
mucositis in rat buccal mucosa. The up-regulation of MMP-2, -3, -9 and down-regulation of TIMP-1 coincided with the previously described early up-regulation of transcription factors and pro-inflammatory cytokines in oral mucosa, suggesting a relationship between their up-regulation/down-regulation and the release of these factors and cytokines. Introdução: A peri-implantite é definida como um processo inflamatório que afeta os tecidos moles e duros adjacentes ao implante, interferindo no processo de osteointegração e, consequentemente, no sucesso do tratamento.
Objetivo: Realizar uma revisão narrativa sobre a peri-implantite, abordando os possíveis fatores de risco, os parâmetros de diagnóstico e as diferentes modalidades terapêuticas.
Metodologia: Realizou-se uma pesquisa bibliográfica recorrendo às bases de dados da “MEDLINE/Pubmed”, “SciELO”, “EBSCO HOST” e “Science Direct”, com as seguintes palavras-chave: “Peri-implantitis”, “Mucositis”, “Peri-implant infection”, “Diagnosis”, “Treatment”, “Implant”, “Risk factors”.
Conclusão: Para cada caso clínico de peri-implantite devem ser utilizados diversos parâmetros de diagnóstico, como o índice de placa, profundidade e sangramento à sondagem, supuração e a evidência de perda óssea radiográfica.
Aquando da reabilitação oral com implantes, o paciente deverá obedecer a um programa de manutenção eficaz, com o objetivo de manter a integridade dos tecidos peri-implantares.

Značaj multidimenzionalnog uticaja
maksilofacijalnih tumora je doveo do povećanog interesovanja za
kvalitet života pacijenata. Studije QOL(Quality of Life) daju
kliničke informacije o efektima bolesti, terapiji i neželjenim
efektima. Prednosti za pacijenta u ovim studijama su te da mogu da
izraze svoje mišljenje o negativnim aspektima bolesti i doprinesu u
donošenju terapijskih odluka jer vreme preživljavanja nije direktno
povezano sa poboljšanjem „kvaliteta života“. Cilj ovog istraživanja
bio je da se ispita efekat različitih terapijskih modaliteta na
radijacioni mukozitis u pacijenata sa postresekcionim defektima
orofacijalne regije. Metodologija: U ovom istraživanju, je
uključeno 30 pacijenata sa tumorima orofacijalne regije koji su
posle hirurške terapije protetski rehabilitovani i podvrgnuti
zračnoj terapiji.Pacijenti su podeljeni u tri grupe po deset
ispitanika. Metodom slučajnog izbora, jednoj grupi ispitanika je
dat standarni protokol u terapiji radijacionih mukozitisa, koja će
predstavljati kontrolnu grupu. Drugoj i trećoj grupi ispitanika su
pored standardne terapije uključeni preparati „Stomatovis“ i
„Gelclair“ i predstavljaju eksperimentane grupe. BMS upitnici (0. i
kontrolni ) su korišćeni u vizuelnoj kvalifikaciji simptomatologije
pacijenta. Procena podataka kvaliteta života je obrađena u upitniku
EORTC-QLQ-C30 verzija 3.0 i njegovom dodatku H&N35, na srpskom
jeziku. Upitnik OHIP-20 (The Oral Health Impact Profile) je
korišćen u proceni kvaliteta protetske terapije pacijenata.
Mikrobiološka obrada uzoraka materijala, kao i identifikacija
izolovanih vrsta mikroorganizama, vršeni su u laboratoriji predmeta
Mikrobiologija i imunologija stomatološkog fakulteta u Beogradu.
Postupak je podrazumevao standardnu mikroskopsku, kulturalnu i
biohemijsku identifikaciju. Identifikacija je predstavljala
standardne bakteriološke metode, a kvantifikacija je obavljana
brojanjem izniklih kolonija na zasejanim hranljivim podlogama. Pri
tome su rezultati izraženi kao ukupan broj CFU (Colony Forming
Unit) izniklih kolonija po uzorku materijala. Rezultati:
Simptomatologija pacijenata se kroz faze ispitivanja pogoršavala.
Visoke vrednosti kserostomije, poremećaja ukusa i bola, a veoma
niske vrednosti salivarnog protoka BMS -kontrolnog upitnika ukazuju
na sporedni štetni efekat zračne terapije. Za razliku od kontrolne
i stomatovis grupe, kod gelclair grupe ispitanika intenzitet
simptoma se postepeno povećavao kroz sve tri faze istraživanja.
Ispitanici gelclair grupe su imali znatno manje tegoba u gutanju
hrane tokom svih faza zračne terapije i značajno manje bolova,
tokom 2. i 3. faze zračenja. Imali su manje problema u uzimanju
obroka pred drugim ljudima, manje bolnih senzacija i manje bolnih
mesta u ustima, bili manje uznemireni zbog problema sa protezama,
tolerantniji prema ukućanima i imali manji subjektivni osećaj
stanja bolesti u odnosu na kontrolnu i stomatovis grupu.

▼ La restauración de los dientes ausentes mediante implantes dentales constituye hoy en día un tratamiento rutinario y muy extendido. Varios estudios longitudinales han demostrado altas tasas de supervivencia de los implantes en función, que van desde el 90 % al 95 % en un periodo de hasta 20 años de seguimiento.
Las complicaciones biológicas alrededor de los implantes dentales se clasifican en tempranas y tardías. Los fracasos tempranos son los que tienen lugar en el tiempo hasta la conexión del pilar al implante y son más fáciles de diagnosticar debido a la falta de osteointegración. Los fracasos tardíos son el resultado de la incapacidad de mantener la oseointegración, siendo la complicación más importante la peri-implantitis.
Objetivos: En 54 pacientes portadores de 344 implantes dentales con al menos una arcada dentaria desdentada total, rehabilitada totalmente con implantes dentales, se tomaron en 146 cuadrantes dentales los parámetros clínicos y microbiológicos con los siguientes objetivos:
1. En pacientes con tejidos periimplantarios sanos, estudiar si hay diferencias clínicas y microbiológicas del tejido periimplantario entre fumadores y no fumadores.
2. En pacientes con algún implante dental con
mucositis periimplantaria, determinar si existen diferencias a nivel clínico y microbiológico entre implantes con
mucositis y con los tejidos periimplantarios sanos.
3. En pacientes con algún implante dental con peri-implantitis, determinar si existen diferencias a nivel clínico y microbiológico entre implantes con peri-implantitis y con los tejidos periimplantarios sanos.
Material y Método: Se realizó un estudio de tipo prospectivo y transversal en la Unidad de Cirugía Bucal del Departamento de Estomatología de la Facultad de Medicina y Odontología de la Universitat de València. Durante el periodo comprendido entre enero de 2009 y junio de 2010, se seleccionaron consecutivamente 78 pacientes con al menos una arcada dentaria edéntula rehabilitada totalmente con implantes dentales.
Todos los pacientes eran portadores de implantes TSA® Treatment Surface Avantblast (Phibo Dental Solutions, Sentmenat, Barcelona, España). A todos los pacientes intervenidos para la colocación de implantes dentales se les explicó el procedimiento y se les informó de los riesgos de la intervención. Se recogió la historia clínica completa de cada paciente y todos firmaron el documento de consentimiento informado.
Conclusiones: Tras estudiar en 54 pacientes portadores de implantes dentales con tejidos periimplantarios sanos,
mucositis periimplantaria y peri-implantitis, y teniendo en cuenta las limitaciones del presente estudio, puede concluirse:
1. El tabaco no influye en los parámetros clínicos ni microbiológicos en pacientes con tejidos periimplantarios sanos.
