Uvod: Th17 ćelije predstavljaju populaciju ćelija koje imaju ulogu u imunopatogenezi autoimunskih oboljenja centralnog nervnog sistema kao što su multipla skleroza i eksperimentalni autoimunski encefalomijelitis (EAE). Efekat glukokortikoida, lekova koji se primenjuju kao terapija inflamatornih i autoimunskih oboljenja, na IL-17 još uvek nije ispitan. Stoga je u ovoj studiji ispitivano dejstvo sintetskog glukokortikoida (GK), Metilprednizolona (MP) na ekspresiju i produkciju IL-17. Kako sam mehanizam i mesto dejstva GK nije precizno definisan, takođe je ispitan in vivo efekat MP na ekspresiju i produkciju IL-17 od strane mononuklearnih ćelija kičmene moždine (MNĆKM) tretiranih DA pacova tokom eksperimentalnog autoimunskog encefalomijelitisa (EAE). Materijal i metode: EAE je indukovan kod Dark Agouti (DA) pacova sa homogenatom kičmene moždine uz kompletni Frojndov adjuvans (CFA). Počevši od prvog dana bolesti, DA pacovi su tretirani sa MP i/ili antagonistom glukokortikoidnog receptora- RU486. Produkcija citokina od strane ćelija koje infiltriraju CNS kao i ekspresija gena je merena ELISA metodom i kvantitativnim PCR-om. Rezultati: MP inhibira produkciju IL-17 od strane mitogenom stimulisanih ćelija limfnog čvora (ĆLČ) DA pacova kao i antigen specifičnu produkciju od strane ćelija drenirajućeg limfnog čvora. Inhibicija produkcije IL-17 je dozno zavisna i nije uslovljena inhibicijom proliferacije tokom 48h inkubacije. Takođe, MP inhibira produkciju IL-17 od strane prećišćenih T limfocita, ali ne tako izraženo kao u slučaju ĆLČ DA pacova. Sa druge strane pokazano je da MP ublažava kliničko ispoljavanje EAE-a što je praćeno i inhibicijom ekspresije i produkcije IL-17 od strane mononuklearnih ćelija kičmene moždine tretiranih DA pacova. Zapažen efekat MP primenjenog in vivo nije posledica smanjenja procentualne zastupljenosti CD4+ T limfocita kao ni njihove apoptoze. Na kraju, antagonist glukokortikoidnog receptora Mifepriston- RU486 poništava inibitorno dejstvo MP na ekspresiju i produkciju IL-17 u in vitro i u in vivo uslovima, što ukazuje da su zapaženi efekti MP posredovani putem glukokortikoidnog receptora...

Os glucocorticoides são muito utilizados em Medicina Veterinária. São económicos, eficazes como anti-inflamatórios e imunossupressores, e atuam rapidamente em quase todas as células e sistemas do organismo. Porém, em tratamentos prolongados, podem provocar efeitos secundários que poderão debilitar o doente. Um destes efeitos é a hipocaliemia, habitualmente bem tolerada mas com potencial arritmogénico, que pode aumentar a morbilidade e a mortalidade em doentes cardíacos. O presente estudo teve por objetivo avaliar eventuais alterações da concentração plasmática de potássio em canídeos submetidos a corticoterapia com metilprednisolona em doses anti-inflamatórias. Foi realizado numa amostra de 21 indivíduos (n=21) da espécie Canis familiaris, avaliados no Centro de Medicina Veterinária Anjos de Assis (CMVAA) no Barreiro, e no Hospital Escolar da Faculdade de Medicina Veterinária da Universidade de Lisboa (FMV-UL), com indicação clínica para realizarem corticoterapia com metilprednisolona em doses anti-inflamatórias, por via oral, em regime ambulatório, por um período de 18 dias. Para cada doente foram realizadas colheitas em 3 tempos para quantificar os níveis plasmáticos de potássio. Valores de p inferiores a 0,05 foram considerados estatisticamente significativos. Os resultados demonstram que a metilprednisolona pode provocar diminuições estatisticamente significativas dos níveis plasmáticos de potássio e que estas poderão estar relacionadas com o sexo do doente.

ABSTRACT - PRELIMINAR STUDY OF KALEMIA EVALUATION IN DOGS THAT WERE SUBMITTED TO CORTICOTHERAPY AND CLINICAL SIGNIFICANCE - Glucocorticoids are widely used in Veterinary Medicine. They are cheap, they are effective as an anti-inflammatory and immunosuppressive drug and they act in almost every cell and system of the body. However, prolonged treatments can lead to adverse effects that might impair the patient. One of these effects is hypokalemia, that is often well tolerated, but it has arrhythmogenic potential and it might rise morbidity and mortality in cardiac patients. This study has by purpose to evaluate potential variations of the kalemia in dogs submitted to corticotherapy with methylprednisolone in anti-inflammatory dosages. It had a sample of 21 dogs (Canis familiaris) (n=21) assessed in Centro de Medicina Veterinária Anjos de Assis (CMVAA) in Barreiro, and in Hospital Escolar da Faculdade de Medicina Veterinária da Universidade de Lisboa (FMV-UL), between those outpatients with clinical indication to proceed to corticotherapy with methylprednisolone in anti-inflammatory dosages, by oral administration, for 18 days. Each patient was submitted to blood collection in 3 times to evaluate plasmatic potassium levels. P-values lower than 0,05 were considered as statistically significant. These results prove that methylprednisolone can reduce kalemia in a statistical significant way and that could be related to the patients gender.

▼ Introdução: A inflamação e o estresse oxidativo estão associados à progressão do infarto agudo do miocárdio (IAM) para a insuficiência cardíaca (IC). Logo, o tratamento com metilprednisolona, um conhecido glicocorticoide com propriedades anti-inflamatórias e antioxidantes, tem o potencial de mitigar a piora da função do ventrículo esquerdo após o IAM. Entretanto, a utilização de glicocorticoides após o IAM tem apresentado resultados contraditórios que podem ser decorrentes da maneira como são aplicados. Objetivo: Verificar a influência do período de administração de acetato de metilprednisolona sobre a função cardíaca de ratos após o infarto agudo do miocárdio. Materiais e Métodos: No experimento 1, foram utilizados 29 ratos Wistar machos divididos em 3 grupos: Sham (n=14), infartado tratado com salina logo após o IAM (IAM0 n=7) e infartado tratado 7 dias pós-IAM (IAM7 n=8). A função cardíaca foi avaliada 2 e 56 dias pós-IAM pelo exame de ecocardiografia. No experimento 2 foram utilizados 45 ratos Wistar machos divididos em 4 grupos: Sham (Sham, n=13); infartado (IAM, n=14); infartado e tratado com metilprednisolona no dia da indução do infarto (IAM+M0, n=7); infartado e tratado com metilprednisolona no 7º dia após a cirurgia (IAM+M7, n=7). A cirurgia do IAM foi realizada pela oclusão da artéria coronária descendente anterior esquerda. Uma dose única de acetato de metilprednisolona (40mg/kg,i.m.) foi aplicada nos animais dos grupos IAM+M. A função cardíaca foi avaliada pela ecocardiografia e pelo cateterismo ventricular 56 dias após a indução do infarto. Dados morfométricos e de estresse oxidativo do coração foram avaliados No experimento 3, foram utilizados 16 ratos Wistar machos, sendo divididos em 2 grupos: grupo controle tratado com salina no dia 0 (C, n=5) e grupo tratado com acetato de metilprednisolona (40mg/kg. i.m.) no dia 0. A coleta de sangue ocorreu uma vez por semana durante oito semanas, para avaliação de parâmetros do soro. A função cardíaca foi avaliada pelo cateterismo ventricular 56 dias o início da administração farmacológica. Dados morfométricos e de estresse oxidativo do coração foram avaliados. Parecer CEUA/UFRGS: 30797. Resultados: Os grupos IAM e IAM+M0 e IAM+M7 apresentaram área de infarto de 50,4%, 54,8% e 55,1%, respectivamente. A fração de encurtamento e a mudança de área fracional do ventrículo esquerdo diminuíram significativamente (37% e 39%) no grupo IAM em relação ao Sham. O tratamento iniciado no 7º dia promoveu um declínio adicional na mudança da área fracional (66%) quando comparado ao IAM. Tais parâmetros foram semelhantes entre os grupos IAM e IAM+M0. Observamos um aumento de 69% na pressão diastólica final do ventrículo esquerdo (PDFVE) no grupo IAM quando comparado ao grupo Sham. O tratamento com acetato de metilprednisolona, independentemente do tempo de administração, foi capaz de atenuar a PDFVE apenas os grupos IAM e IAM+M7 apresentaram congestão pulmonar. ▼ Introdução: bronquiolite obliterante (BO) é uma forma rara de doença pulmonar crônica que ocorre após uma injúria grave ao trato respiratório inferior e resulta em estreitamento ou obliteração total das pequenas vias aéreas. Há várias etiologias possíveis, mas em crianças a doença é geralmente pós-infecciosa. O tratamento básico é de suporte, pois não há medidas terapêuticas comprovadamente efetivas. Há evidências, baseadas em estudos animais, de que o uso de corticóides na fase inicial da doença poderia modificar seu curso, revertendo a atividade inflamatória, especialmente a deposição de fibroblastos. A pulsoterapia é utilizada para reduzir os efeitos colaterais da administração sistêmica prolongada de corticóide oral, e é uma alternativa para os pacientes com quadros mais graves. Objetivos: avaliar parâmetros clínicos, laboratoriais, radiológicos e de função pulmonar nos pacientes pediátricos com bronquiolite obliterante submetidos a pulsoterapia com metilprednisolona, antes e após o procedimento, e demonstrar que ocorre melhora clínica, considerando-se o grau de hipoxemia, número de agudizações pulmonares, função pulmonar, número de internações, uso prolongado de corticóides orais e suas complicações. Avaliar também os efeitos colaterais e complicações do tratamento. Métodos: foram avaliadas 40 crianças na faixa etária de até 18 anos, de ambos os sexos, em seguimento na Divisão de Pneumologia Pediátrica do Instituto da Criança do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, com diagnóstico de bronquiolite obliterante por dados clínicos e tomográficos e/ou de biópsia pulmonar, e que foram submetidas à pulsoterapia com metilprednisolona endovenosa pelo período mínimo de seis meses. A análise estatística foi feita usando testes não paramétricos para comparação de medidas repetidas (Friedman, Wilcoxon), e para comparação múltiplas foi realizado o pós-teste de Dunn com nível de significância = 5%. Resultados: houve diminuição no número de agudizações pulmonares comparando o período de seis meses antes da pulsoterapia e 24 meses após o início da pulsoterapia (p=0,0042). Houve diminuição no número de internações hospitalares comparando o período de seis meses antes da pulsoterapia e 18 meses após (p<0,0001). Houve melhora na saturação de oxigênio comparando o período de seis meses antes e no primeiro (p=0,0002) e segundo ano após (p=0,0005). Houve aumento na estatura e peso das crianças comparando o início e o final da xii pulsoterapia: escore Z aumentou de -1,08 para -0,63 (p=0,015) e de -0,91 para -0,59 (p=0,039) respectivamente. Dos 23 pacientes em corticoterapia oral contínua, foi possível suspensão em 20 (87%). Dos 20 pacientes em oxigenoterapia domiciliar, foi possível a suspensão completa em seis pacientes. Conclusões: a pulsoterapia com metilprednisolona pode ser eficaz no tratamento de pacientes com bronquiolite obliterante, resultando em diminuição das agudizações pulmonares e internações hospitalares, com retirada da corticoterapia oral contínua e da oxigenoterapia domiciliar… Advisors/Committee Members: Rodrigues, Joaquim Carlos.

▼ Background: Interleukin-17 (IL-17)-producing cells are increasingly considered to be the major pathogenic population in various autoimmune disorders. The effects of glucocorticoids, widely used as therapeutics for inflammatory and autoimmune disorders, on IL-17 generation have not been thoroughly investigated so far. Therefore, we studied the effects of a synthetic glucocorticoid methylprednisolone (MP) on IL-17 expression and production in rat lymphocytes. Since the mechanisms and the site of glucocorticoids' actions are still not completely defined we also investigated the in vivo effect of the synthetic glucocorticoid MP on the expression and production on IL-17 by cells infiltrating CNS tissue during experimental autoimmune encephalomyelitis (EAE). Methods: EAEwas induced in Dark Agouti (DA) rats by immunization with rat spinal cord homogenate (SCH) mixed with adjuvant. Commencing on the day when the first EAE signs appeared, DA rats were injected daily for 3 days with MP and/or RU486, an antagonist of glucocorticoid receptor. Cytokine production and gene expression in CNS infiltrating cells and lymph node cells were measured using ELISA and real time PCR, respectively. Results: Production of IL-17 in mitogen-stimulated lymph node cells (LNC) from nontreated rats, as well as in myelin basic protein (MBP)-stimulated draining LNC from rats immunized with SCH + adjuvant was significantly reduced by MP. The reduction was dose-dependent, sustained through the follow-up period of 48 hours, and was not achieved through anti-proliferative effect. Additionally, MP inhibited IL-17 production in purified T cells as well, but to less extent than in LNC. On the other hand treatment of rats with MP ameliorated EAE, and the animals recovered without relapses. Further, MP inhibited IL-17 expression and production in cells isolated from the CNS of DA rats with EAE after the last injection of MP. The observed effect of MP in vivo treatment was not mediated through depletion of CD4+ T cells among CNS infiltrating cells, or through induction of their apoptosis within the CNS. Finally, the glucocorticoid receptor-antagonist RU486 prevented the inhibitory effect of MP on IL-17 production both in vitro and in vivo, thus indicating that the observed effects of MP were mediated through glucocorticoid receptor-dependent mechanisms. Conclusion: These results show that MP potently inhibits IL-17 expression and production and suggest that the clinical efficacy of MP in various T-cell-mediated inflammatory reactions is at least partly due to the inhibition of Th17 lymphocytes. AIM: The spinal cord injury is a major problem in every modern, developing society, with an important social and financial impact. An immediate and effective treatment of these injuries is a constant challenge for every doctor. A key role to this goal has the early treatment of the inflammatory process, which is induced after mechanical injury of the spinal cord. Locally developed inflammation disturbs normal neural tissue fuction, which can be seen clinically. In this experimental study we evaluate anti-inflammatory and neuroprotective action of methylprednisolone after spinal cord injury, with a comparison between intraperitoneal and intrathecal administration.METHOD: Adult male Wistar rats were used for this experimental study. Under general anaesthesia, two-level laminectomy was performed in thoracolumbar junction. After dividing the dural sac, reproducible injuries of spinal cord were created using a specific for this purpose developed device. After injury, methylprednisolone was administrated in doses according to Bracken et al developed protocol, applying either intrathecal, or intraperitoneal administration. Neurological evaluation of rats was performed regularly, using BBB scale.RESULTS: Neurological evaluation of laboratory animals revealed that there was a clear improvement in rats that underwent intrathecal administration of methylprednisolone, in comparison to control group, as well as to those rats that methylprednisolone was administered intraperitoneal. This experimental study revealed no neurological improvement after intraperitoneal administration of high dose of methylprednisolone.CONCLUSION: Analyzing the results from this experimental study, we have concluded that intrathecal administration of high-dose methylprednisolone after traumatic spinal cord injury helps improving neurological outcome. Administration of high doses of corticosteroids in patients that suffered a traumatic spinal cord injury remains controversial, mostly due to the observed side effects. Neverthenless, it remains an important treatment option which can improve clinical oiutcome, if it is applied immediately and if measures are taken, in order to avoid or to eliminate unwanted side effects.

A esclerose múltipla (EM) é considerada doença rara no país, porém tem um grande impacto econômico na sociedade, pois é uma das causas mais importantes de incapacidade em adultos jovens. A doença manifesta-se através de surtos, ou seja, pioras neurológicas agudas, com sintomas instalando-se em horas ou poucos dias. O tratamento do surto é altamente eficaz, podendo reverter o déficit neurológico estabelecido. A única forma regulamentada de tratamento é através da internação em hospital, porém poderia ser realizado em regime de hospital-dia que, contudo, não é previsto pelo Sistema Único de Saúde. Este é um estudo de custo-minimização, desenvolvido com o objetivo de comparar os custos do tratamento do surto de EM à base de corticóide, em regime de internação e regime de hospital-dia, com o objetivo de confirmar a economia deste último. Verificaram-se todos os materiais, medicamentos e profissionais necessários ao tratamento de pulsoterapia com metilprednisolona em pacientes com EM. Esta lista foi orçada tomando-se como referência os valores praticados pelo Hospital de Clínicas de Porto Alegre em sua última compra. Além disto, os valores salariais foram baseados no piso salarial das categorias de profissionais envolvidos no tratamento. Idealizou-se um hospital-dia com 12 leitos, respeitando todas as recomendações do Ministério da Saúde. Os custos do tratamento completo em regime de internação e em hospital-dia foram respectivamente R 564,23 e R 172,41 - uma redução absoluta de R 391,82 e relativa de 69,44%. O levantamento do número de Autorizações de Internação Hospitalar (AIHs) para tratamento de surto de esclerose múltipla em todo o país foi de 10.157 no período de 2002 a 2006. Considerando todo este período, o número de diárias hospitalares (pacientes-dia) que poderiam ter estado disponíveis para ocupação por outras patologias foi de 93.816. Concluiu-se que o tratamento do surto da esclerose múltipla em hospital-dia é bem mais econômico que o regime de internação, promovendo uma potencial relevante economia à sociedade, permitindo melhor utilização dos leitos hospitalares.

Multiple sclerosis is considered a rare disease in Brazil; however has a great economical impact in our society, and is known as one of the most important causes of disability among young adults. Multiple sclerosis is characterized by neurological relapses, occurring in a period of hours or a few days. The treatment of relapses with corticosteroids in high doses is highly effective in relieving the neurological deficit. It can be administered as inpatient or outpatient basis, the last not authorized by the Brazilian Public Health System. This is a cost-minimization analysis in order to compare the costs of this treatment as inpatient to outpatient. INTRODUÇÃO: O transplante pulmonar é a única opção terapêutica para determinados pacientes com doença pulmonar em estágio terminal. A maioria dos transplantes de pulmão é realizada a partir de doadores em morte encefálica, condição que per si pode comprometer o sucesso do procedimento. A fisiopatologia da morte encefálica é complexa e envolve mecanismos hemodinâmicos, simpáticos e inflamatórios. A administração de metilprednisolona 60 minutos após indução da morte encefálica reduz a atividade inflamatória em pulmões de ratos. OBJETIVO: Avaliar os efeitos do tratamento com metilprednisolona em pulmões de ratos transplantados a partir de ratos doadores induzidos à morte encefálica quando administrada 60 minutos após indução da mesma. MÉTODOS: doze ratos Wistar foram randomizados e anestesiados em 2 grupos (n=6): controle: indução de morte encefálica e administração de solução salina; metilprednisolona (MET): indução de morte encefálica e administração de metilprednisolona 60 minutos após. Os animais foram observados e ventilados por 120 minutos e depois submetidos a transplante pulmonar. Foram avaliados parâmetros hemodinâmicos e gasométricos, escore histológico, dosagem de substâncias reativas ao ácido tiobarbitúrico (TBARS), superóxido dismutase (SOD), TNF-α e interleucina-1β (IL-1β). RESULTADOS: houve redução significativa dos níveis de TNF-α e IL-1β no grupo que recebeu metilprednisolona (p=0,0084; p=0,0155, respectivamente). Não houve diferença significativa na atividade do TBARS e SOD entre os grupos controle e MET (p=0,2644; p=0,7461, respectivamente). Não houve diferença significativa na gasometria, nos parâmetros hemodinâmicos, e nas alterações histológicas entre os grupos controle e MET. CONCLUSÃO: a utilização de metilprednisolona após a morte encefálica em ratos doadores reduz a atividade inflamatória em pulmões transplantados, mas não tem influencia sobre o estresse oxidativo.

INTRODUCTION: Lung transplantation is the only therapeutic option for a subset of patients with end-stage lung disease. Most lung transplantations are performed using brain-dead donors, a condition that compromises the success of the procedure. The physiopathology of brain death is complex and involves haemodynamics, the sympathetic nervous system and inflammatory mechanisms. Administering methylprednisolone 60 minutes after inducing brain death in rats has been shown to modulate pulmonary inflammatory activity. Abstract The aim of this work was to describe the clinical features and clinical course of Henoch-Schönlein purpura (HSP) in a prospective setting, to compare the efficacy of cyclosporine A (CyA) and methylprednisolone (MP) pulses for the treatment of severe HSP nephritis (HSN) and to study the effect of prophylactic prednisone treatment given at disease onset on the long-term outcome. A total of 223 children with newly diagnosed HSP were followed up prospectively for 6 months. Patients with severe HSN also had extrarenal symptoms more frequently during this time. Protein loss via the intestine was more common than previously described, occurring in 3% of the patients. HSN developed in the early course of the disease. The results suggest that weekly urine dipstick tests are indicated for 2 months after HSP onset and individually for over 6 months in cases of HSN or HSP recurrences. Prednisone did not affect the frequency or timing of the appearance of HSN. The efficacy of CyA and MP treatments was evaluated in a trial with a mean follow-up time of 6 years involving 24 paediatric patients (11 CyA, 13 MP), 15 of whom were randomized and 9 were treated according to the given protocol without randomization. Oral CyA was not inferior to intravenous MP pulses and proved to be an efficient, safe steroid-sparing treatment for severe HSN. All the CyA-treated patients achieved remission of nephrotic-range proteinuria within 3 months, while remission was achieved more slowly in the MP group and only in 6/13 (46%) with the initial treatment. There was no difference in the renal biopsy findings two years after initiation of the therapy. The 8-year outcome of HSP was assessed by means of a health questionnaire in 160 (94%) of the 171 former patients in the randomized placebo-controlled prednisone trial and in 138 (81%) with urine analysis and measurement of blood pressure. HSP carried a good prognosis, although skin relapses occurred up to a decade after the initial onset and could be accompanied by late-onset nephritis. Hypertension and/or renal abnormalities were recorded in 13% of the patients, being more frequent in those with an initial occurrence of HSN (OR 4.3, p=0.009, 95% CI 1.4–14.0) and warranting long-term follow-up of HSN patients. Early prednisone treatment did not affect the long-term outcome of HSP and should not be routinely used.

Tiivistelmä Väitöskirjan tarkoituksena oli kuvata Henoch-Schönleinin purppuran (HSP) oireita ja taudinkulkua, verrata siklosporiini A:n (CyA) ja metyyliprednisolonipulssihoidon (MP-pulssihoidon) tehoa vaikean HSP-nefriitin (HSN) hoidossa ja selvittää taudin alussa annetun prednisonihoidon vaikutusta pitkäaikaisennusteeseen. Hoje em dia, o uso abusivo de corticosteroide nos vários tipos de choque(hipovolémico,distributivo e séptico) tem levado a debates polémicos. Ao comparar, os resultados dos inúmeros autores que abordaram este tema, não é possível tirar uma conclusão unânime. A opinião de cada um diverge, havendo autores que apoiam e outros que refutam a sua utilização no choque. O objetivo deste estudo consiste em verificar se o uso metilprednisolona vai surtir algum benefício na reversão e sobrevivência do choque, nos animais, em que a terapêutica inicial de abordagem ao choque não tenha surtido efeito. O presente estudo foi realizado no Hospital Veterinário da Arrábida (HVA), em Azeitão, ao longo de um período de 7 meses, decorrido entre 1 de Setembro de 2011 até 31 Março de 2012. Durante esse tempo, procedeu-se ao acompanhamento de 57 animais em choque hipovolémico, distributivo ou séptico. Nas primeiras horas após ser instituído a fluidoterapia de choque e vasopressores, existiram animais em estudo que não conseguiu estabilizar a pressão arterial média (PAM) dentro dos valores normais de referência. Nestes casos foi administrado um tratamento adjuvante com metilprednisolona. Através da análise descritiva da população em estudo verificou-se que a taxa de mortalidade foi menor no grupo de canídeos e felídeos em que não se efetuou o tratamento adjuvante com metilprednisolona.

Today, the abusive use of corticosteroids in various types of shock (hypovolemic, distributive and septic) has led to controversial debates. By comparing the results of several authors who have addressed this topic, it is not possible to draw a unanimous conclusion. The opinion of each differs, with authors who support and others refute its use in shock. The aim of this study is to determine if the use methylprednisolone will be benefit in survival and reversal of shock. In the animals that therapeutic approach to initial shock has not been effective. This study was conducted at the Veterinary Hospital of Arrábida (HVA), in Azeitão, over a period of 7 months between September the 1st, 2011 until March 31th, 2012. During this time, we proceeded to the accompaniment of 57 animals in hypovolemic, distributive or septic shock. In the first hours following the set up of shock fluid and vasopressors, there were animals in this study that not stabilize the mean arterial pressure within the normal reference values. These cases was administered an adjuvant treatment with methylprednisolone. Through the statistical analysis of the studied animals and by comparing the results between the two groups, it was found that the mortality rate was lower in the group of dogs and cats which have not been administered with adjunctive therapy with methylprednisolone.

▼ To protect immature organ systems during corrective cardiac surgery, patients are cooled to a minimal body temperature of 17°C. However, prolonged inflammation and “capillary leakage” are common unwanted effects after corrective cardiac surgery in newborns and infants. The aim of this study was to investigate the effect of deep hypothermia and rewarming on human endothelial cells, as well as the treatment with Methylprednisolone and/or Tacrolimus. Stimulated and unstimulated human umbilical vein endothelial cells (HUVEC) were exposed to a dynamic temperature protocol analogous to clinical settings. Furthermore, endothelial cells were pretreated with Methylprednisolone and/or Tacrolimus as well as with Mitogen-Activated Protein Kinase-inhibitors (U0126, SB203580 and SP600125). Morphological changes, cell viability, expression of the pro-inflammatory cytokine IL-6 and the chemokines IL-8 and MCP-1 and ERK 1/2, SAPK/JNK and p38 activation were investigated. Hypothermia led to intercellular gap formation and elongated cell shapes in unstimulated endothelial cells. Upon rewarming, endothelial cell integrity was restored. Only the combined treatment with MP and TAC inhibited these hypothermia-induced changes. Stimulated endothelial cells secreted significantly higher IL-6 protein two hours after rewarming. The combined pretreatment with MP and TAC served to inhibit the increased IL-6 secretion. SB203580 as specific p38 inhibitor most effectively down regulated the unwanted IL-6 release after cooling and rewarming. These results suggest that the combined treatment of MP and TAC, as well as the inhibition of p38 or components of the p38 pathway could be a promising and selective therapeutic target to prevent endothelial dysfunction and prolonged inflammation after deep hypothermia and rewarming. Introdução: Os corticosteroides intra-articulares (IA) são amplamente utilizados no tratamento da osteoartrite (OA) de joelho, porém é desconhecido qual dentre estes agentes é o mais eficaz. Objetivo: O objetivo do presente estudo foi comparar a eficácia das infiltrações IA de triancinolona hexacetonida (TH) e de acetato de metilprednisolona (AM) na OA de joelho. Pacientes e Métodos: Pacientes com OA sintomática de joelho, graus II ou III de Kellgren-Lawrence, foram randomizados para receber uma única infiltração IA com 40mg de TH ou AM. As avaliações clínicas foram realizadas nas semanas 4, 12 e 24. O desfecho primário do estudo foi a melhora da dor do joelho pelo paciente por escala visual analógica (EVA) da visita basal à semana 4. Os desfechos secundários incluíram a avaliação global da doença pelo paciente e pelo médico, o questionário de osteoartrite Western Ontario and McMaster Universities (WOMAC), o índice de Lequesne e o critério de resposta Outcome Measures in Rheumatology and Osteoarthritis Research Society International (OMERACT-OARSI). Na análise estatística, foram empregadas equações de estimativa generalizada, com estatística de Wald para contrastes do tipo 3 e ajustes de Tukey-Kramer para comparações múltiplas. Resultados: Cem pacientes foram incluídos na população com intenção de tratar, 50 em cada braço do estudo. Uma melhora significativa na dor pela EVA foi observada na semana 4 para ambos os grupos (P < 0,0001), não havendo diferença entre eles (P=0,352). Esta melhora foi sustentada até a semana 24. Uma melhora significativa em relação à avaliação basal foi observada na avaliação global da doença pelo paciente e pelo médico, no questionário WOMAC e no índice de Lequesne, não havendo diferença entre os grupos. A melhora nos desfechos secundários de avaliação foi sustentada durante o estudo, exceto para a avaliação global da doença pelo paciente. O critério de resposta OMERACT-OARSI foi alcançado por 74% e 72% dos pacientes dos grupos TH e AM, respectivamente. Conclusão: TH e AM são igualmente eficazes na OA de joelho, e a melhora na dor e na função física pode ser sustentada por até 24 semanas

Introduction: Intra-articular (IA) corticosteroid injections are broadly used in the treatment of knee osteoarthritis (OA), but it is unknown which of these agents is the most effective. Objective: The aim of the present study was to compare the efficacy of triamcinolone hexacetonide (TH) and methylprednisolone acetate (MA) IA injections in knee OA. Patients and Methods: Patients with symptomatic knee OA, Kellgren-Lawrence grades II or III, were randomized to receive a single IA injection with 40mg of TH or MA. HIGH CONCENTRATIONS OF STEROIDS, HYDROCORTISONE OR METHYLPREDNISOLONE, IN VITRO INHIBIT THE ADHERENCE OF THE POLYMORPHONUCLEAR LEUKOCYTES (PMNLS), THAT HAVEBEEN ACTIVATED THROUGH THE COMPLEMENT. DEXAMETHASONE DID NOT INHIBIT THE ABOVE MENTIONED ADHERENCE. THE ACTIVITY OF METHYLPREDNISOLONE AND DEXAMETHASONE WAS STUDIED IN VIVO IN THE PRESENT STUDY. BOTH DRUGS WERE GIVEN IN HIGH EQUIVALENT DOSES OF 30 MG/KGR FOR MP AND 6 MG/KGR FOR DEX BOLUS IN VIVO IN TWO GROUPS OF PATIENTS RESPECTIVELY. BLOOD SERUM FROM THE TWO GROUPS OF PATIENTS WAS TAKEN BEFORE AND ON 4, 8, 12 AND 16 HOURS AFTER THE BOLUS ADMINISTRATION OF THE DRUGS. ADHERENCE OF PLMNS TO NYLON WAS ASSAYED BY THE METHOD OF MAC GREGOR. WHENTHE PMNLS WERE SUSPENDED IN SERA FROM PATIENTS OBTAINED AT 4 AND 8 H AFTER THE ADMINISTRATION OF MP, PMN ADHERENCE WAS MARKEDLY INHIBITED. NO INHIBITION OF PMN ADHERENCE WAS NOTED WHEN THE PMNLS WERE SUSPENDED IN SERA OBTAINED AFTER THE ADMINISTRATION OF DEX. GIVEN THE FACT THAT PMN ADHERENCE TO NYLON FIBERSIS A PROCESS SIMILAR TO THE PMN ADHERENCE TO THE ENDOTHELIAL CELLS DURING THEEARLY PHASE OF MIGRATION INTO INFLAMMATORY FOCI, ONE COULD DEDUCE THAT MP IS MORE EFFECTIVE THAN DEX IN THE TREATMENT OF SEPTIC SHOCK.

O uso dos corticosteroides, na redução do edema, dor e trismo, associados às cirurgias de terceiros molares tem sido investigado nas últimas décadas. Este estudo revê os trabalhos publicados na última década, que relataram os efeitos da dexametasona e/ou metilprednisolona, no controlo dessas sequelas pós-operatórias. Foram incluídos 29 artigos, destes, 19 avaliaram a dexametasona, 8 a metilprednisolona e 2 compararam a eficácia destes dois fármacos. Da sua análise conclui-se que os corticosteroides são úteis na redução dos sintomas pós-operatórios, com redução significativa do edema e trismo, embora o seu papel no controlo da dor não esteja bem estabelecido. São usadas doses, vias de administração e posologias diferentes, tendo a administração única pré-operativa sido considerada mais eficaz e a administração submucosa uma via alternativa de eficácia comparável às outras vias e com vantagens como a fácil execução, não necessitando de habilidade extra, e de não depender da adesão do paciente.

The use of corticosteroids, in reducing edema, pain and trismus, in third molar surgeries has been researched in recent decades. This study reviews the published papers in the last decade, reporting the effects of dexamethasone and / or methylprednisolone on the control of post-operative sequelae. Were included 29 articles, of which 19 evaluated dexamethasone, 8 analyzed methyl-prednisolone and two compared the effectiveness of the two drugs. of their analysis it was concluded that corticosteroids are useful in reducing post-operative symptoms, with a significant reduction of edema and trismus, although their role in pain control is not well established. Different doses, routes of administration and dosages are used, with the single preoperative administration being considered more effective and submucosal administration an alternative route of efficacy comparable to other routes and with advantages such as easy execution, requiring no extra skill, and not depend on patient compliance.

O presente trabalho centra-se no estudo de administrações intrassinoviais em cavalos. O objetivo dos tratamentos intrassinoviais são a diminuição da dor e interrupção do processo inflamatório, limitando o processo degenerativo a eles associado. Neste estudo estão descritos 11 casos clínicos, os quais foram submetidos a tratamento intrassinovial. Foram recolhidos os seguintes dados, em todos os cavalos: população, raça, idade, desporto, indicação para tratamento, local de lesão, grau de claudicação, molécula utilizada, resultados a curto prazo e a médio prazo. Os cavalos estudados eram Puro-Sangue Lusitano ou Português de Desporto, em atividade desportiva, com uma média de idades de 14anos, com doença degenerativa articular, principalmente na tarsometatársica, navicular e metacarpofalângica, tratados essencialmente com metilprednisolona. A maioria dos cavalos teve uma melhoria superior a 75% do grau de claudicação a curto prazo e uma a melhoria completa a médio prazo. Como esperado, os corticosteroides foram os fármacos mais utilizados, e o seu uso adequado conduziu a melhoria, sem complicação. Apesar dos resultados positivos obtidos na população estudada, é importante a realizar mais estudos sobre os tratamentos intrassinoviais, nomeadamente sobre a sua real eficácia e potenciais riscos, assim como estudo de alternativas aos corticosteroides.

The present paper focuses on joint administration, also known as intra-synovial administration. The purpose of this procedure is to decrease the pain and intra-synovial inflamation, limiting the associated degenerative process. In this study 11 clinical cases that have been submited to treatment were addressed. The following data have been collected: population, breed, age, sport, treatment indication, OA site, degrre of lameness, molecule used, results in short and med-term. The horse study were Puro-Sangue Lusitano and Portugês de Desporto, in sport career, with an avarege of 14 years old, with degenerative joint disease, mostely in tarsometatarsal joint, navicular and metacarpophalangica, treated especially with methylprednisolone. Most of the horses had an improvement of over 75% of the lameness degree in a short-term and a complet response in mid-term As expected the corticosteroids were the most used drug and the correct use has led to improvement with out complications. Despite the positive results obtained it can be conclude that more studies should be done about intra-synovial treatments, especially about its real effectiveness and potential risks, as well as the study of alternatives to corticosteroids.

Les pathologies vésicales sont nombreuses et nécessitent la plupart du temps un traitement médicamenteux. Il peut alors être administré par voie intravésicale, augmentant son efficience et limitant les effets indésirables systémiques. Nous nous sommes intéressés à quatre thérapeutiques : l’alun de potassium, le chlorhydrate de lidocaïne, l’hemisuccinate de méthylprednisolone, la mitomycine C et le bacille de Calmette et Guérin (BCG) en application locale directement dans la vessie. La perméabilité de l’aluminium à travers la vessie a été étudiée au travers d’un cas clinique et des expériences ex vivo ont été réalisées pour définir les paramètres de perméabilité et proposer un algorithme de prédiction de la quantité d’aluminium absorbée dans l’organisme après irrigation intravésicale à l’alun de potassium. Cet algorithme dépend du poids du patient, de la durée de l’irrigation et du volume de la solution utilisée en irrigation vésicale. Des études de perméation ont aussi été réalisées pour le chlorhydrate de lidocaïne, l’hemisuccinate de méthylprednisolone, la mitomycine C et le BCG afin de sécuriser l’emploi de ces thérapeutiques par voie intravésicale. Les études réalisées permettent de prédire, selon les caractères physico-chimiques des molécules, la pénétration dans la membrane vésicale. Une nouvelle formulation galénique réalisée à base d’un gel thermosensible a été étudiée pour optimiser les thérapeutiques intravésicales. Des études de perméation ont été faites avec la nouvelle formulation galénique. Pour finir, une culture de cellules cancéreuses urothéliales a été mise au point et un test de viabilité a été réalisé pour la mitomycine C et le BCG

Bladder diseases are numerous and mostly require medication. Drugs can be administered by intravesical route, thereby increasing efficiency and reducing systemic side effects. We are interested in four drugs: potassium alum, lidocaine hydrochloride, methylprednisolone hemisuccinate, mitomycin C and bacillus Calmette- Guérin (BCG). Mathematical modelling of drug transport through bladder wall is proposed considering scarce literature on this route of administration. The permeability of aluminum through bladder wall was studied through a clinical case and ex vivo experiments were performed to propose a simplified algorithm for the calculation of aluminium dose absorbed in patient with impaired renal function as function of volume of 1% alum solution in the bladder, duration of intravesical irrigation and patient body weight. Permeation studies were also conducted for lidocaine hydrochloride, methylprednisolone hemisuccinate, mitomycin C and BCG to secure intravesical administration of these drugs. Practical outcome of this study could drive compounding optimisation towards improvement of safety and efficacy in patient undergoing intravesical administration. A metilprednisolona (MP) e o gangliosídeo GM-1 são drogas de uso clínico estabelecido para o tratamento da lesão medular em humanos, embora sua eficácia e seus mecanismos de ação ainda não sejam totalmente entendidos. O objetivo do presente trabalho foi avaliar os resultados da recuperação da função locomotora e comparar com as alterações histomorfométricas da medula de ratos com lesão medular medicados com MP; GM-1 e sua associação. A lesão medular foi produzida pelo sistema New York University® em 24 ratos Wistar, divididos em quatro grupos: controle (n=6), MP (n=6), GM1 (n=6) e MP+GM1 (n=6). A avaliação da recuperação da função locomotora dos ratos foi realizada utilizando-se a escala de BBB no 2º, 7º e 14º dias após lesão medular e sacrificados no 14º dia para análise histológica e morfométrica de área total, área preservada e percentual de área preservada. Concluímos que a MP e sua associação com o GM-1 mostraram-se eficazes na recuperação da função locomotora e que todos os ratos medicados demonstraram melhora no percentual de área preservada superior ao grupo controle. Os Grupos MP e GM1 foram superiores na preservação de substância branca e o GM-1 demonstrou efeitos benéficos na preservação de substância cinzenta no centro da lesão. A substância cinzenta demonstrou ser mais suscetível à lesão que a substância branca e não houve correlação entre os achados histológicos e a recuperação da função locomotora.

The methylprednisolone and the GM-1 ganglioside are drugs with established clinical usage for the treatment of spinal cord injury in human; however its efficiency and its active mechanisms are not completely understood yet. The objective of the present paper has been to evaluate the results from the neurological function recovering and to compare these with the histomorphometric alterations in rats with spinal cord injury, prescribed with methylprednisolone; GM-1 and its association. The spinal cord injury has been done by the New York University system® in 24 Wistar rats which were assigned to one of four groups: control (n=6), MP (n=6), GM1 (n=6) and MP+GM1 (n=6). The evaluation of the neurological function outcome has been carried out using BBB locomotor rating scale on the second, seventh and fourteenth days after the injury and sacrificed on the fourteenth day for histological and morphometric analyses of total cross-sectional area, spared area and percentage of spared area. We concluded that the methylprednisolone and its association with the GM-1 revealed themselves effective concerning to the locomotor function recover and that every medicated rat demonstrated an improvement in the preserved area percentage superior to the control group. The MP and GM1 Groups were superior in the white matter preservation and the GM-1 demonstrated beneficial effects regarding the gray matter preservation at the injury epicenter. The gray matter has been more sensitive for damaged than the white matter and there has not been correlation between the histological findings and the locomotor function recovering.

Administration of steroids systematically (IV or per mouth) is considered to be the treatment of choice for idiopathic SSHL worldwide, although their specific action remains unknown. Setting aside the controversy concerning dosage and duration of treatment, the efficacy of steroids remains the subject of many clinical trials. However, systemic administration of steroids may have serious side effects and such treatment may be contraindicated in patients with peptic ulcer, glaucoma, diabetes, hypertension, osteoporosis, and other diseases, as well as those with pregnancy. IT administration of corticosteroids as an alternative treatment modality for idiopathic SSHL appears attractive as it avoids the side effects of systemic steroids and obtains higher inner ear concentrations. The results of the present study revealed that nearly half of the patients in the treatment group showed significant hearing improvement with IT administration of methylprednisolone after unsuccessful IV steroid administration, whereas none in the matched control group had similar improvement. With regard to potential predictors of the final PTA outcome, we found that IT treatment; better post-IV PTA and absence of vertigo were good predictors of the outcome. In contrast, sex, age, affected ear, days to admission and shape of the initial audiogram, did not show any significant influence on the outcome. The present study suggests that IT administration of methylprednizolone in the middle ear is effective and safe treatment for patients with idiopathic SSHL who have failed traditional systemic steroid administration. Although it may also be effective as 1st-line treatment, we can only recommend it for the patients who have failed traditional systemic steroid administration, because of the design of the present study. Current literature indicates the need for evidence-based, prospective randomized controlled clinical trials to test the efficacy of IT steroids as 1st line treatment.

Η χορήγηση στεροειδών συστηματικά (ενδοφλεβίως ή από του στόματος) θεωρείται σήμερα η θεραπεία εκλογής για την αντιμετώπιση της ιδιοπαθούς ΑΝΒ, παρότι ο μηχανισμός δράσης τους παραμένει άγνωστος. Η αποτελεσματικότητα τους αποτελεί το αντικείμενο πολλών κλινικών μελετών παρά τις διαφοροποιήσεις αναφορικά με την χρησιμοποιούμενη δοσολογία και την διάρκεια θεραπείας. Η συστηματική τους χορήγηση παρόλα αυτά ενέχει κινδύνους, και αντενδύκνειται σε περιπτώσεις ασθενών με ιστορικό παθήσεων όπως πεπτικό έλκος, γλαύκωμα, σακχαρώδη διαβήτη, υπέρταση, οστεοπόρωση, και εγκυμοσύνη. Η ενδοτυμπανική χορήγηση στεροειδών παρουσιάζει το πλεονέκτημα της επίτευξης υψηλότερων συγκεντρώσεων στα υγρά του κοχλία, όπως και της αποφυγής της εμφάνισης συστηματικών παρενεργειών. ▼ Lichen planus is an inflammatory, immunologically mediated disease with manifestation on the skin and mucosa. In the present study, the topical application of the glucocorticoid methylprednisolone aceponate is described for the first time in patients with oral lichen planus, and the therapeutic effects are examined by using clinical, immunohistological, and microbiological criteria. The main objective of the study was to evaluate the efficacy of the glucocorticoid methylprednisolone aceponate (verum 1) in comparison with the reference glucocorticoid triamcinolone acetonide (verum 2) and with placebo. The primary target criterion of the study was the change in the »total size of all lesions«, as assessed by using a size score. Additional criteria for the evaluation of the therapy outcome (secondary target criteria) were: number of diseased regions, lesion size differentiated according to clinical form, intensity of erythema, whitish intensity, and intensity of complaints. The results of the follow-up examinations served the critical evaluation of the therapy outcome after 3 and 6 months. When comparing the efficacy of the two glucocorticoids, significantly better results were obtained for methylprednisolone aceponate than for triamcinolone acetonide, both with regard to the primary target criterion »total size of all lesions« and with regard to additional secondary target criteria. The therapy outcomes, differentiated according to clinical forms, revealed form-specific differences, with the therapy using methylprednisolone aceponate being particularly effective for the ulcerative form, i.e. the clinical form with the highest degree of severity. To reduce the risk of candidiasis, an antimycotic prophylaxis and add-on treatment is recommended. The results of the follow-up examinations of the study at 3 and 6 months after the end of therapy showed that the therapeutic efficacy had diminished both from the clinical and the patient’s view. On the one hand, the again increased values of the follow-up examinations suggest to put into perspective the results obtained, on the other hand, the values continued to be significantly lower as compared with the start of therapy, thus indicating the improved condition in the medium-term view of the therapeutic outcome. The most important immunohistological result of the study concerned the antimicrobial protein calprotectin and showed a reduction of the calprotectin expression in basal keratinocytes and a decreasing number of calprotectin-positive macrophages in the inflammatory infiltrate after glucocorticoid therapy. The healing process was associated with a quantitative cellular change in the CD163 macrophage population: the therapy led to a shift in the composition of the infiltrate cells towards the anti-inflammatory CD163 macrophage phenotype. The study result regarding the 25F9 expression of the epithelium gives a first indication that the 25F9 antigen represents a glucocorticoid-inducible epithelial tissue antigen in oral lichen planus. ▼ Organ injury after deep hypothermic circulatory arrest is still a matter of concern. Altered renal function with necessity of dialysis is still one of the main postoperative morbidities and may influence the prognosis. Pretreatment with steroids during corrective surgery of congenital heart defects is a routine method to the prevention of potential brain injury. We evaluated the effect of deep hypothermia on the regional perfusion and the effect of pretreatment with Methylprednisolone, systemic and intrathecal applies, on the regional cerebral perfusion after deep hypothermic circulatory arrest (DHCA). Method: 24 neonatal piglets (age < 10 days, weight 2,2 ± 0,5 kg) were randomly assigned to three groups: seven animals were pretreated with high-dose (30 mg/kg) methylprednisolone systemically 6 hours before surgery; Five animals were pre-treated with the same dose (30 mg/kg) methylprednisolone intrathecally; Twelve animals without pharmacological treatement served as control group. Full flow CPB (200ml/kg/min) was initiated for homogeneous systemic cooling. DHCA was induced at a temperature of 15 °C for 120 min. After rewarming (50 min) the animals were weaned from CPB. Fluorescent microspheres were used to determine regional perfusion. After the end of the experiment tissue samples were collected from kidney, myocard, spleen, liver, skeletal muscle, frontal lobe, parietal lobe, occipital lobe, cerebellum and brainstem. Results: Peripheral organs: The regional blood flow was significantly decreased in all investigated organs after deep hypothermic perfusion and increased following reperfusion. However, the most significant decrease was found in the kidney (P=0.02). CNS: Significant higher regional cerebral perfusion was registered in the intrathecal group. Conclusion: Hypothermia induced a significant decrease of regional perfusion. Intrathecal steroid treatment could reduce neuronal cell death significant. ▼ Postoperative infections, sepsis and multi organ failure are directly associated with a hyperinflammatory reaction and mean a problem in hemihepatectomies and other extended abdominal operations. This reaction is characterized by a disproportion of pro- and anti-inflammatory cytokines. Purpose of this study was to examine the influence of preoperative high-dose methylprednisolone on postoperative immunofunction. Main criterion was the postoperative IL-6-plasma level. The pro-inflammatory cytokines IL-6, IL-8 and TNF-a, the anti-inflammatory cytokine IL-10, HLA-DR expression on monocytes and TNF-a-synthesis after stimulation with LPS were measured. The determination of ALT, AST, albumin and TPZ- INR as well as t-bilirubin, AP and ?-GT reflected postoperative liver function, synthesis disorders or cholestasis. Also measured were the inflammation parameters CRP and leucocytes as well as creatinine as a marker for postoperative kidney function. The postoperative clinical outcome was assessed by postoperative complications, reconvalescence and duration of postoperative hospital stay. In this study 20 patients were included and they were double-blind randomized distributed in a ratio of 10:10 into a steroid and a control group. The patients of the steroid group received a preoperative dose of methylprednisolone (30 mg/kg body weight) and the patients of the control group got saline. All hemihepatectomies were done without pringle-maneuver. The results of this study showed significant lower levels of IL-6 and IL-8 in the steroid group as well as low suppressed TNF-a- and IL-10- concentrations compared to the control group. The TNF-a- concentration after LPS-stimulation was 4 h postoperative significant lower in the steroid group. The HLA-DR expression on monocytes showed no significant differences. CRP was significant lower from the 1st to the 4th postoperative day in the steroid group compared to the control group. ALT, AST, albumin and TPZ-INR were not influenced by preoperative high-dose methylprednisolone; t-bilirubin was significant lower in the steroid group on the 6th postoperative day. The duration of hospital stay was significant shorter in the steroid group, but this result was influenced by a prolonged stay of one patient in the control group because of postoperative complications. There was no significant difference between both groups relating to postoperative complications and reconvalescence. The results of this study show that a hyperinflammatory reaction is effectively inhibited by preoperative high-dose methylprednisolone and that the cellular immunity is not impaired. IN THIS EXPERIMENTAL WORK WE STUDIED THE EFFECT OF CORTICOSTEROIDS IN THE LUNGSOF NINE MODEL ANIMALS (MONGREL DOGS) SUFFERING FROM ACUTE PANCREATITIS. WE INDUCED AP BY INFUSING ACTIVATED BILE SALTS (BILE MIXED WITH BLOOD FROM THE AORTA) RAPIDLY INSIDE THE PANCREATIC DUCT. THE ANIMALS WERE DIVIDED IN THREE GROUPS. IN THE FIRST GROUP WE GAVE THE CURRENT THERAPY AND HIGH DOSE CORTICOSTEROIDS (METHLYLPREDNISOLONE 30 MGR/KGR OF BODY WEIGHT), IN THE SECOND ONLY THE CURRENT THERAPY AND IN THE THIRD GROUP ONLY DEXTROSE 5% IV SERUM. TO ALL GROUPS WE STUDIED BIOCHEMICAL PARAMETERS, THE ACTIVATION OF COMPLEMENT, ARTERIAL PRESSURE AND CVP AND THE HISTOLOGICAL SPECIMENS OF THE PANCREAS, THE LUNGS AND THEKIDNEYS. THERE WERE SIGNIFICANT DIFFERENCES BETWEEN THE THREE GROUPS IN COMPLEMENT ACTIVATION, BIOCHEMICAL DISORDERS AND HISTOLOGICAL FINDINGS. OUR RESULTSSHOW THAT, IF HIGH, DOSE CORTICOSTEROIDS ARE USED IN THE EARLY STAGES OF AP, THE RESULT IS SIGNIFICANT LIMITATION, IF NOT INHIBITION, OF THE CATASTROPHICALEVOLUTION OF ACUTE PANCREATITIS.

▼ Abstract: Purpose: To compare the efficacy of 20 mg subconjunctival triamcinolone in association with topical prednisolone 1% to 500 mg intravenous methylprednisolone in association with topical prednisolone 1% in the treatment of cornea endothelial graft rejection. Methods: Case-control study carried out at State University of Campinas Hospital. Patients submitted to penetrating keratoplasty that presented first episode of corneal endothelial rejection within 15 days of symptoms onset between November 2005 and October 2006 were treated with 20 mg subconjunctival injection of triamcinolone acetate in association with topical prednisolone acetate 1%. These patients were matched for age and diagnosis to patients that were submitted to a single 500 mg intravenous injection of methylprednisolone sodium succinate in association with topical prednisolone acetate 1% and analyzed regarding the reversion of the rejection episode, intraocular pressure at day 30 and visual acuity at the end of 1 year. Results: 16 patients were treated with 20 mg subconjunctival triamcinolone and topical prednisolone 1% during the period of recruitment and were matched to 16 patients treated with 500 mg intravenous methylprednisolone and topical prednisolone 1%. At the end of 1 year, the group treated with triamcinolone had a better outcome than the group treated with methylprednisolone (p=0.025), having 15 patients with clear grafts as the group treated with methylprednisolone had 10 patients. 3 patients from the group treated with triamcinolone had new rejection episodes during follow-up and were retreated successfully as in the group treated with methylprednisolone 4 patients had a new rejection episode, with 2 progressing to failure and 2 to success with retreatment. Intraocular pressure rose in both groups (p=0.002) at day 30 but there were no statistically significant differences between the groups (p=0.433). Visual acuity improved after 1 year in both groups (p=0.049) and the group treated with triamcinolone had better visual acuities (p=0.002). Conclusions: 20 mg subconjunctival injection of triamcinolone acetonide associated with topical prednisolone acetate 1% showed to be more effective than 500 mg intravenous methylprednisolone associated with prednisolone acetate 1% in this case-control study.
