目的:代謝性酸塩基異常のステップワイズ解析では, AG による全体診断は出来ず, 混合性の判定は難解のため新法を考案した.方法:AG,重炭酸ギャップBG と二酸化炭素ギャップCG を評価に用い,ABC 順に算出する基準値を0に揃え(ΔAG を使用), 夫々の正常/増加/減少3区分の組み合せによる解析をABC-Gap法とした. ΔAG を横軸, BG を縦軸に取り, 各々の正常上下限値に線を引いた直交座標上に3区分を重ね合わせて『BG はAG とは独立した元々のHCO3-の増減変化量』を見出し, 数理論理学による「代謝性病態区分図」を作成, この図に患者のΔAG とBG を座標としてplotし検証した.結果・結語:臨床診断と座標点が属する区分病態との一致が確認され, 理論的正当性と臨床的有用性が証明された. 呼吸性代償等の評価CG は代謝性に加味されるので, ABC-Gap法は手順が覚え易く結果判定が容易で, 3者の組み合せで代謝性酸塩基異常の全体診断ができる有用な診療手段と考える.
