【目的】パノラマX 線写真による骨粗鬆症スクリーニング指標は腰椎や大腿骨頚部などの骨密度、あるいは骨代謝マーカーと密接に関係することが世界中で多数報告されている。一方、骨粗鬆症のアウトカムである骨折との関係については、海外の2 つの報告では「関係あり」、国外の1 例および国内の1 例では「なし」とされており、未だ真偽が明確になってはいない。本研究では、病院を受診してパノラマX 線写真を撮影した患者をベースとした病院ベース多目的コホートを作成し、パノラマX 線写真による骨粗鬆症スクリーニング指標と骨粗鬆症診断歴(未骨折者)および骨粗鬆症性骨折歴との関係について検討を行った。【対象および方法】松本歯科大学病院を2007~2013 年に受診し、歯科治療のためパノラマX線写真を撮影した40 歳以上の患者2186 名に対して、骨粗鬆症を初めとする種々の全身疾患や栄養摂取状況、生活習慣に関する質問表と質問に関する同意書を送付し、回答が得られた1021 名(男性371 名、女性650 名) を分析した。本研究は松本歯科大学倫理委員会の承認(0152 号) を受けて行われた。書類を送付した全ての患者のパノラマX 線写真について、経験年数24年の歯科放射線専門医が骨粗鬆症スクリーニングの皮質骨形態指標をこれまでの報告に従い、3 型( 1 型:正常、2 型:軽度~中等度粗鬆、3 型:高度粗鬆) に分類した。骨粗鬆症診断歴( 未骨折) および骨粗鬆症性骨折歴を各々独立変数として、近年WHO… (more)
