Uvod: Mamografija je metoda izbora – zlatni
standard - za ranu detekciju karcinoma dojke i dobro prihvaćen
skrining metod za ţene određene ţivotne dobi. Koristeći
mamografiju, moguće je otkriti tumor u stadijumu kada je lečenje
najefektivnije, ali samo u slučaju da je obezbeđena produkcija
snimaka konstantnog kvaliteta u smislu kontrasta i rezolucije. Sa
druge strane, upotreba jonizujućeg zračenja je rizičan deo
mamografske procedure. Iz tog razloga, kontrola doze i rizik za
visoko radiosenzitivno glandularno tkivo dojke su podjednako vaţni
i svi mamografski pregledi moraju biti opravdani u smislu koristi i
rizika. Cilj prve faze studije je bio identifikacija slabih tačaka
mamografske prakse u Srbiji, u smislu tehničkih parametara
mamografije, te definisanje predloga korektivnih mera. Cilj druge
faze je evaluacija analogne i digitalne mamografije, u smislu
kvalteta slike i primljene doze zračenja, kao prvi korak
optimizacije mamografskog procesa te uvođenja digitalne mamografije
u rutinsku kliničku praksu u Srbiji. Materijal i metode: U prvoj
fazi analizirano je 20 reprezentativnih mamografskih jedinica
(engl. Mammography unit - MU), korišćenjem Protokola kontrole
kvaliteta za analognu mamografsku opremu. Proučavani su rentgenska
cev, generator, obrada filma, kvalitet snimka i primljena doza
zračenja. Druga faza studije je uključila 387 asimptomatskih
pacijentkinja, distribuiranih na pregled na analognim i digitalnim
mamografskim aparatima (109 i 287 pacijentkinja, respektivno), uz
naknadno računanje srednje glandularne doze, te procenu kvaliteta
slike...

▼ In 2010, 240,000 women were diagnosed with breast cancer and 16,000 women were diagnosed with cervical cancer (www.seer.cancer.gov). In spite of technological advances and increases in education regarding cancer screening, healthcare costs are still on the rise and disagreement still exists between physicians who create guidelines and policy makers (Woloshin, 2000). This project examines cancer screening and examines socio-economic and demographic predictors for usage of preventative screening measures. This project also examines disparities in the current screening process. Data from the National Health Interview Survey (NHIS) from 1987 and 2010 were used to compare differences across time. The results indicate that utilization of mammograms and Pap smears overall has increased, but many disparities still exist amongst groups who actually receive the preventative screenings.

▼ In order to reduce the high mortality rate of breast cancer, most research in mammogram image analysis aims to develop a CAD (Computer Assisted Detection) system that can assist the clinician in the difficult task of "early" diagnosis. The comparison of temporal pairs of mammograms is believed by radiologists to be often crucial for diagnosing cancer, especially as breast tissue is highly variable across the population making it difficult to perform diagnosis reliably from a single mammogram. However, changes in the appearance of mammograms due to differences in compression and imaging conditions can limit the effectiveness of temporal comparison. In this thesis, a registration method that aligns temporal (or bilateral) mammograms is presented. This technique has the potential to assist the clinician to detect changes more efficiently (e.g. interval cancers in mammogram sequences, architectural distortions or microcalcifications in bilateral mammograms). In addition, the application of this technique to mammograms of Hormone Replacement Therapy (HRT) users is investigated. Since long-term use of HRT can increase the risk of breast cancer (as a side effect of glandular tissue regeneration), quantification of temporal tissue density changes (in addition to registration) is needed for assessing density changes locally. In order to derive quantitative measures of breast tissue change, the mammogram pairs are processed using the hint representation of interesting tissue. Since such a measure depends on the image information context, we examine the problems that arise from combining image registration and quantification, aiming to develop a robust framework for temporal tissue density change assessment.

Introduction: Between 6-10% of women attending breast screening are recalled to investigate an unclear area on the mammogram. Image quality is known to affect image interpretation and it has been suggested that the number of recalls could be reduced with improved image quality. Aim: This study aimed to investigate the effect image quality has on recall rates, to assess reader consistency using the PGMI classification system and to establish factors leading to recall. Materials and Methods: A six member panel assessed 904 sets of images (698 recalled; 206 non-recalled) through a BreastScreening Program during three separate phases (pilot, main and non-recall). The pilot study was conducted without additional training in PGMI. Levels of agreement and Kappa statistics were calculated to assess intra- and inter-consistency. The percentage of and reasons for inadequate images was calculated; while a case-control study was conducted to establish factors increasing the likelihood of a client being recalled. Results: The level of agreement between panel members significantly increased from the pilot to the main study (45.5% to 57.7%) before decreasing slightly for the non-recall (57.7% to 52.2%). Overall, 3.3% of the 904 sets of images were classed as inadequate; the most common PGMI reason was exposure (31%); the left MLO was considered the most common inadequate projection (30%), with more privately produced (66%) images considered inadequate compared to public images (34%). Inadequate image quality did not hinder the cancer detection rates. The case-control component demonstrated current and previous HRT use, increased breast density, better image quality and images being taken at a public site all contributed to a client being recalled. Conclusion: The results of this study demonstrated that inadequate image quality was not a major factor leading to recall; although twice the number of recalled images were considered inadequate compared to the non-recalled images. The use of the PGMI classification system is highly subjective, with low levels of agreement amongst users. The use of HRT, breast density, imaging site and image quality all contribute to a client being recalled.

▼ This research work was aimed at assessing the quality management system and radiation protection of patients of four mammography facilities in Ethiopia. A questionnaire was designed to collect data to assess the status of the quality management systems at the facilities. Quality control measurements were done to verify whether the mammography machines were performing self- consistently compatible with National and International standards. The status of patient protection was evaluated by assessing the mean glandular dose with ACR Phantom in conjunction with image quality assessment using ACR accredited phantom and Leeds test object. From the analysis of the questionnaires administered the facilities did not meet the quality management system criteria. The mammographic machines performed self –consistently with respect to all the parameters tested expect with facility D which recorded a high deviation of ± 57.21% at 30 kV outside the acceptance criterion. The beam qualities were acceptable except one facility which could not meet the criteria with a HVL value of 0.29 mmAl. The image quality assessments of the four facilities were within the acceptance criteria for detectability of microcalcification, masses, fibers and spatial resolution. Estimated mean glandular dose (MGD) of values 1.0 mGy, 1.03 mGy, and 0.96 mGy at 28 kV for the selected facilities were less than the 2 mGy and 3 mGy recommendations by International Atomic Energy Agency (IAEA) and American College of Radiology (ACR) respectively. Management of the mammography facilities must establish an efficient and effective quality management system which includes plan and do (quality assurance), check (quality control) and take action (quality improvement) to govern the imaging process ensuring optimum radiation protection of the patient.

▼ Breast cancer is the most common form of
cancer among women globally and the second leading cause of
cancer-related deaths among women in the United States.
Microcalcifications (µcals) are the most commonly detected
abnormality in the diagnosis of breast cancer. However, the
detection of µcals and other breast lesions can be obscured by
radiographically dense breast tissue. The radiographic density of
breast tissue varies between women depending on the relative
amounts of low-density adipose tissue and high-density
fibroglandular tissue. Therefore, the overall objective of this
study was to develop an anatomic breast phantom with tunable levels
of radiographic tissue density for investigating contrast-enhanced
detection of µcals. A phantom with tunable levels of fatty acid and
albumin-agarose hydrogels was developed to mimic low-attenuating
and high-attenuating fibroglandular tissues in the breast.
PEGylated bisphosphonate-functionalized gold nanoparticles
(BP-PEG-Au NPs) enabled contrast-enhanced detection of µcals as
small as 0.5 mm in hydrogels mimicking dense breast tissue using
laboratory and preclinical micro-computed tomography. Importantly
BP-PEG-Au NPs also enabled contrast-enhanced detection of µcals as
small as 0.1 mm in anatomic phantoms mimicking dense breast tissue
using clinical
mammography. Contrast enhancement decreased with
increasing breast density and decreasing µcal size. The anatomic
breast phantom developed in the study is expected to fill a gap in
the translation of new imaging probes and methods from the
laboratory to the clinic.

▼ Aims: 1) to report participation rates in breast self-examination (BSE), clinical breast examination (CBE) and mammography among women living in Ras Al Khaimah (RAK) and to examine their beliefs, attitudes and knowledge regarding breast cancer and early detection practices, 2) to report mammographic density patterns and their associations with demographic and reproductive factors in RAK and Riyadh. Methods: In the first study, 102 women were interviewed and a total of 366 and 792 mammographs were collected for the second and third studies, respectively. The first and second studies were carried out in RAK whereas the third study was conducted in Riyadh. The American College of Radiology, Breast Imaging Reporting and Data System (ACR, BI-RADS) 5th version and the Laboratory for Individualized Breast Radiodensity Assessment (LIBRA) software were used to estimate mammographic density. Results: In the first study BSE was practiced and CBE and mammography were performed on 51, 40 and 38 women, respectively. Only 4% were aware about the appropriate age to start mammography screening and 84.3% did not know the correct screening interval. Reasons for not planning to undergo CBE or mammography screenings were fear of cancer, pain during the examination and absence of doctor’s recommendation. In the second study, 22%, 43%, 24% and 11% women were allocated to BI-RADS a, b, c and d, respectively. Emirati women were significantly less likely to have increased mammographic density than Western women. In the third study, a mean percent density of 10.3cm2 and dense breast area of 19.1cm2 were reported. BMI, menopausal status and age at menarche remained significant predictors for mammographic density. Conclusions: This thesis has two main outcomes. First, establishing culturally sensitive public awareness and education programs is a priority in RAK. Second, mammography would be considered a modality of choice for breast cancer screening among Emirati and Saudi women.

▼ Having a high recall rate may increase the probability of cancer being detected earlier, however it also has been related to increased false positive decisions, causing significant psychological and economical costs for both screened women and the mammography screening service. Therefore, the purpose of this thesis is to explore the impact of various recall rates on breast radiologists’ performance in a laboratory setting. Methods This study was designed to encompass two aspects 1) the effect of setting varying recall rates on the performance of breast radiologists in screening mammography 2) types of mammographic appearances of breast cancer are more likely to be missed at different recall rates. Five Australian breast radiologists were recruited to read one single test set of 200 mammographic cases (180 normal and 20 abnormal cases) over three different recall rate conditions: free recall, 15% and 10%. These radiologists were tasked with marking the location of suspicious lesions and providing a confidence. Results A significant decrease in radiologists’ performance was observed when reading at lower recall rates, with lower sensitivity (P=0.002), case location sensitivity (P=0.002) and ROC AUC (P=0.003). Reading at a lower recall rate had a significant increase in specificity (P=0.002). The second study of this thesis showed that breast radiologists demonstrated lower sensitivity and receiver ROC AUC for non-specific density (NSD) (P=0.04 and P=0.03 respectively) and mixed features (P=0.01 and P=0.04 respectively) when reading at 15% and 10% recall rates. No significant change was observed on cancer characterized with stellate masses (P=0.18 and P=0.54 respectively) and architectural distortion (P=1.00 and P=0.37 respectively). Conclusion Reducing the number of recalled cases to 10% significantly reduced breast radiologists’ performance. Stellate masses were likely to be recalled (90.0%) while NSDs were likely to be missed (45.6%) at reduced recall rates.

▼ Aims: This thesis aims to investigate the level of consistency in radiologists’ mammographic density (MD) reporting and to investigate the reasons that lead to reporting variability, and then to develop novel, affordable phantom images to validate imaging parameters linked with MD assessment. Methods: Project one: 17 radiologists were asked to report the BI-RADS density category and to manually segment the areas they regarded as MD for 40 clinical images. The consistency in the BI-RADS density reporting was quantified, and the impact of MD segmentation was investigated. For radiologists’ MD segmentation, the variation was quantified and the impact of image appearance was explored. Project two: A set of 100 phantom images were generated. Phantoms were manufactured using various masses and distributions of fine steel-wool set within a gelatine base to simulate the glandular and adipose tissue. A clustered lumpy background noise was added to the phantom images to simulate more complex images. The correlations between the simulated fibroglandular tissue mass and a series of imaging parameters were explored. Results: For the BI-RADS density reporting, k=0.477 for the 17 participants. In 450 out of 656 paired cases (approx. 69%), the BI-RADS categories matched the MD segmentation categories. The range of MD segmentation areas varied from 7% to 73% across the 40 images. The mean gray level value for 8 bit images being 146±19 vs. 99±14 for common and variation areas respectively. For the phantom project, very strong correlation was found for the standard deviation of mean intensity for the whole phantom image (r = 0.83, p<0.01). For the selected dense regions, very strong were shown for percentage density (r = 0.91, p<0.01), and integrated density (r = 0.93, p<0.01). Very strong and significant correlations remained after the lumpy noise was added. Conclusion: There were large variations for both MD reporting and MD identification between radiologists. In addition to subjectivity in reporting a category, the variation in radiologist MD identification is another major reason that leads to inconsistency in density reporting. Using our novel and affordable phantom, in addition to percentage density, the integrated density and the standard deviation of mean intensity may be used to facilitate MD quantification.

▼ Mammography is dependent on optimal breast positioning by the radiographer. A literature review identified the historical origins of the positioning criteria used by breast screening programmes quality assurance standards, by which mammographic images are evaluated. Uncertainty that the metrics described for the craniocaudal view could support objective evaluation and reproducibility was confirmed. A mixed method explanatory sequential design was developed with two research phases. A survey of Australian and New Zealand radiographers, and radiologists assessed knowledge and preferences for image quality positioning criteria. Discordance between radiographers’ and radiologists’ knowledge and differences in criteria preferences for the craniocaudal view was demonstrated. A novel qualitative Think-Aloud study explored Australian and New Zealand radiographers’ clinical reasoning processes and strategies during image evaluation. The Think-Aloud study demonstrated three radiographer decision-making stages: segmental, environmental, and holistic and a dual process of intuitive and analytical decision-making was revealed. Integrated results indicate that the complexity of image quality evaluation is affected by the emotive nature of the imaging examination despite the use of practice guidelines. Radiographer concerns for radiation dose to radio-sensitive breast tissue and the woman’s decision to return to future screening contributed to final image quality decisions. Interprofessional communication should be strengthened to better appreciate differences in image quality decisions, leading towards a shared responsibility for provision of optimal mammographic images.

knowledge of radiological aspects as well as the clinical exam and pathophysiology of
breast diseases. In this work a
mammography phantom was developed to be used for
training in the operation of mammographic x-ray equipment, image quality evaluation,
self-examination and clinical examination of palpation. Polyurethane was used for the
production of the phantoms for its physical and chemical properties and because it is
one of the components normally used in prostheses. According to the range of flexibility
of the polyurethane, it was possible to simulate breasts with higher or lower amount of
adipose tissue. Pathologies such as areolar necrosis and tissue rejection due to surgery
reconstruction after partial mastectomy were also simulated. Calcifications and nodules
were simulated using the following materials: polyethylene, poly (methyl methacrylate),
polyamide, polyurethane and poly (dimethyl silicone). Among these, polyethylene was
able to simulate characteristics of calcification as well as breast nodules. The results
from mammographic techniques used in this paper for the evaluation of the phantoms
are in agreement with data found in the literature. The image analyses of four phantoms
indicated significant similarities with the human skin texture and the female breast
parenchyma. It was possible to detect in the radiographic images produced regions
of high and low radiographic optical density, which are characteristic of breasts with
regions of different amount of adipose tissue. The stiffnesses of breast phantoms were
adjusted according to the formulation of the polyurethane which enabled the production
of phantoms with distinct radiographic features and texture similar to human female
breast parenchyma. Clinical palpation exam of the phantoms developed in this work
indicated characteristics similar to human breast in skin texture, areolar region and
parenchyma

▼ Background: It is generally known that
mammography screening improves survival and reduces mortality from breast cancer by detecting the disease at an early stage when treatment is more effective. Despite improvements in uptakes of
mammography screening, however, not all breast cancers are detected by mammograms. Some women become aware of their breast cancer through symptoms. We conducted a population-based study of the risk factors for symptomatic awareness of breast cancer among women with primary in situ or invasive breast cancer. Patients were eligible if they self-reported having had a mammogram within two years prior to either the symptom or imaging that led to the diagnosis of breast cancer. Information on patient demographics, tumor and biological characteristics, and healthcare system characteristics were collected using computer-assisted interviews. The final sample for analysis was 737 patients.
Results: Among women with a recent negative
mammography screen, non-Hispanic (nH) blacks and Hispanics, younger women, women with negative estrogen receptor (ER) and progesterone receptor (PR) markers, and patients with dense breasts are more likely to report symptomatic detection than nH whites, older patients, women with one or both positive ER and PR markers, and patients with fatty breasts. Tumor and biological factors (i.e., hormone receptor status and histologic grade) and breast density are strongly associated with symptomatic detection.
Conclusion: Our results suggest that minorities are at risk for higher burden of disease due to late stage diagnosis (which is a consequence of the method of detection). Abnormal or suspicious mammograms in high-risk groups (e.g., being a minority, or having highly dense breasts, etc.) should warrant immediate and closer follow-up because of the likelihood of these cancers turning out to be symptomatic and consequently more advanced and aggressive. Monitoring detection patterns can serve as useful indicators of the efficacy of intervention programs designed to reduce mortality by early disease detection. Finally, although the impact was not statistically significant, improving the quality of the services available in facilities that predominantly serve minorities who come from relative disadvantage might also help alleviate the disparity.

▼ Objective.To examine the relation between continuity of care, demographics variables, healthcare resource distribution, and preventive healthcare
Data Sources/Study Design.Administrative data are based on the Taiwan's National Health Insurance (NHI) research databases. The ages of the objects in this study are divided into two groups. One group is above 30 years old while the other are between 40-70 years old. The objects were eligible for cervical cancer screening and
mammography from 2010 to 2012.
Methods.Logistic regression is conducted to examine the relation between continuity of care that has ever used preventive healthcare before, and regular use preventive healthcare. Meanwhile multiple linear regression analysis is conducted to examine the relation between continuity of care and preventive healthcare utilization.
Results.Patients with lower continuity of care have higher frequency to use preventive services, regular preventive service and preventive service utilization.
Conclusions.Health policy and doctorsâ habits may affect the relation between continuity of care, and health care utilization. The results of this study may provide a reference to improve continuity of care and preventive healthcare.

▼ Ionizing radiation is used to screen for, diagnose, and treat breast cancer. Noncancerous breast cells are exposed to a range of doses as a result of these procedures in addition to the low levels of natural background radiation that they are always exposed to. With the consistent increase in medical radiation procedures, and the climbing rates of breast cancer in most populations, understanding the effects of these exposures is of interest and importance. Radiation exposure results in DNA double strand breaks which can trigger various cellular responses including cell cycle arrest, and cell death. Apoptosis is a form of highly regulated cell death that is controlled by the Bcl-2 family of proteins. Bcl-2 and Bcl-XL are anti-apoptotic proteins that are commonly overexpressed in human cancers including breast cancer. This thesis was focused on investigating the effects of Bcl-2 and Bcl-XL expression on the radiation response of human mammary epithelial cells. Bcl-2 and Bcl-XL were exogenously expressed in MCF-10A cells, and Bcl-XL expression was knocked down. Cytogenetic techniques were used to characterize the MCF-10A cells used. Automated image based assays were used to validate the anti-apoptotic effects of the exogenous proteins against known apoptotic stimuli, and to probe the effects of radiation exposure. ▼ The purpose of this study was to understand knowledge, attitudes, and behaviors towards
mammography in women and to explore relationship among knowledge, attitudes, and behaviors towards
mammography in women.
Our research used a self-constructed structured questionnaire. A total of 450 questionnaires were distributed with 387 valid ones returned. The recovery rate was 86%. SPSS 21.0 statistical software was used for questionnaire coding, data statistics, and analysis. Conclusions were summarized as follows:

Remarkable differences in knowledge towards
mammography can be found in individual background variables such as age, education level, and any formal job; among them, women 50-54 years old showed better knowledge than women 65-69 years old; those who with education level of graduate school showed better knowledge towards
mammography than the illiterate women; furthermore, those who had formal job showed better knowledge than the unemployed.
2. Attitudes towards
Mammography
Remarkable differences in attitudes towards
mammography can be only found in education level, among them, those who with education level of college and university had better positive evaluation than the illiterate women.

▼ Οι μικροαποτιτανώσεις είναι από τις πιο σημαντικές ενδείξεις παθήσεων του μαστού και μπορεί να αποτελέσουν πρώιμη ένδειξη καρκίνου του μαστού. Πρόκειται για εναποθέσεις ασβεστίου στο μαστό με τη διάμετρός τους να κυμαίνεται από 0.1 έως 1 mm και εμφανίζονται είτε μόνες είτε σε ομάδες. Η ακριβής τμηματοποίηση (segmentation) των μικροαποτιτανώσεων στη μαστογραφία συνεισφέρει στην εξαγωγή αξιόπιστων χαρακτηριστικών μορφολογίας, που χρησιμοποιούνται στην αυτόματη διάγνωση με υπολογιστή (Computer-aided Diagnosis, CADx).
Στα πλαίσια της παρούσης διδακτορικής διατριβής προτείνεται μία νέα μέθοδος τμηματοποίησης μικροαποτιτανώσεων, η οποία αρχικά εντοπίζει σημεία του περιγράμματος αυτών. Αυτό επιτυγχάνεται με την εφαρμογή της μεθόδου ενεργών ακτίνων (Active Rays), πολικός μετασχηματισμός ενεργών περιγραμμάτων (Active Contours), σε 8 διευθύνσεις και σε δύο κλίμακες του μετασχηματισμού κυματίων (wavelet transform) με φίλτρα Β-spline. Ακολούθως, χρησιμοποιείται μέθοδος επέκτασης περιοχής (region growing) για τον ακριβή προσδιορισμό του περιγράμματος της μικροαποτιτάνωσης. Ως κριτήριο για την αύξηση της περιοχής χρησιμοποιήθηκαν τα σημεία στο περίγραμμα της μικροαποτιτάνωσης, όπως αυτά προσδιορίσθηκαν από τη μέθοδο των ενεργών ακτίνων. Επίσης, υλοποιήθηκε μέθοδος ακτινικής βάθμωσης, η οποία έχει πρόσφατα προταθεί στη βιβλιογραφία για την τμηματοποίηση μικροαποτιτανώσεων, και χρησιμοποιήθηκε για συγκριτική αξιολόγηση.
Οι δύο μέθοδοι τμηματοποίησης εφαρμόστηκαν σε 149 ομάδες μικροαποτιτανώσεων, κυρίως πλειόμορφων, που αντλήθηκαν από 130 μαστογραφικές εικόνες από τη βάση DDSM (Digital Database for Screening
Mammography). Η ακρίβεια τμηματοποίησης των δύο μεθόδων αξιολογήθηκε από τρεις ακτινολόγους με χρήση 5-βάθμιας κλίμακας. Η ακρίβεια τμηματοποίησης της προτεινόμενης μεθόδου βρέθηκε ίση με 3.96±0.77, 3.97±0.80 και 3.83±0.89, όπως αξιολογήθηκε από κάθε ακτινολόγο, και 2.91±0.86, 2.10±0.94 και 2.56±0.76 για την συγκρινόμενη μέθοδο. Οι διαφορές στην ακρίβεια τμηματοποίησης των δύο μεθόδων ήταν στατιστικώς σημαντικές (Wilcoxon signed-ranks test, p<0.05).
Επίσης, μελετήθηκε η επίδραση των δύο μεθόδων τμηματοποίησης στην απόδοση μεθόδου αυτόματης διάγνωσης (χαρακτηρισμό) ομάδων μικροαποτιτανώσεων με υπολογιστή. Η μέθοδος αυτόματης διάγνωσης στηρίζεται σε επιβλεπόμενη ταξινόμηση προτύπων χαρακτηριστικών σχήματος ομάδας αποτιτανώσεων. Συγκεκριμένα, χρησιμοποιήθηκε ταξινομητής ελαχίστων τετραγώνων – ελάχιστης απόστασης και εξήχθησαν χαρακτηριστικά ομοιότητας και διαφοροποίησης (variability) ομάδας μικροαποτιτανώσεων, τα οποία περιγράφουν τη μορφολογία μεμονωμένων αποτιτανώσεων (εμβαδόν, μέγιστη διάμετρος, σχετική αντίθεση). Η απόδοση ταξινόμησης αποτιμήθηκε μέσω εμβαδού καμπύλης παρατηρητών (ROC). Τα χαρακτηριστικά Εμβαδού και μέγιστης Διαμέτρου επέδειξαν σημαντικά υψηλή απόδοση ταξινόμησης (Mann-Whitney U-test, p<0.05) όταν εξήχθησαν από μικροαποτιτανώσεις τμηματοποιημένες με την προτεινόμενη μέθοδο ενεργών ακτίνων (0.82±0.06 και 0.86±0.05, αντίστοιχα). ▼ Mammography screening is effective in reducing breast cancer (BC) mortality; however there are widespread concerns that it may also lead to over-diagnosis, i.e. the detection of BC that would not have emerged clinically in a woman’s lifetime had she not participated in screening. The extent of over-diagnosis due to
mammography is contested, with published estimates varying from 0% to 54%. The principal aim of this research is to quantify the level of over-diagnosis of BC associated with population-based
mammography screening in South Australia (SA). The following questions are addressed: (1) Have BC incidence rates increased following the introduction of screening in SA and is the increase greater than expected based on projections of pre-screening trends?, (2) Has the prevalence of key breast cancer risk factors also increased?, (3) To what extent does hormone replacement therapy (HRT) use affect breast cancer risk and screening outcomes?, (4) Are there any differences in the underlying risk of BC among screening participants and non-participants?, and the central question (5) What is the level of over-diagnosis due to organised
mammography screening in SA? Questions 2-4 relate to the potential for estimates of over-diagnosis to be confounded by risk factor differences/temporal changes. A review of previous studies of over-diagnosis due to
mammography screening is included, which highlights methodological complexities relating to measurement of over-diagnosis and offers some explanations for why published estimates vary to such a great extent. The first two questions were answered through descriptive analyses of BC incidence trends in SA from 1977-2009, as well as trends in the prevalence several key breast cancer risk factors collected via the SA Health Omnibus Surveys during 1991-2009 (alcohol use, body weight, HRT use) and Australian Bureau of Statistics (fertility rates and age at first birth). The effect of HRT on various screening outcomes (e.g. screen-detection rates, interval cancer rates, recall to assessment) was examined though multivariable Poisson regression modelling using individual person level data from BreastScreen SA, which included self-reported HRT use at the time of each screening episode. Differences in underlying risk of BC between screening participants and non- participants were investigated using 2012 South Australian Health Omnibus Survey data. Two different methods were used to quantify over-diagnosis. Method 1 used a case-control design to compare screening histories for women with and without BC. Odds ratios (OR) were determined across different time intervals after screening to allow for lead time effects and applied to background reference rates based on pre-screening incidence trends. Over-diagnosis estimates were obtained by comparing cumulative incidence with and without screening. se primenjuje klinički pregled, radiološka dijagnostika, uglavnom
mamografija i ultrasonografija i patohistološka dijagnostika.
Ograničenja palpacije, mamografije i ultrazvuka i potreba da se ne
previdi rani karcinom dojke, doveli su do povećanja broja biopsija,
koje se najčešće izvode zbog benignih lezija. Stoga je razvijena

direktno otkriva elastične karakteristike tkiva dojke, koja se
razlikuju po tvrdoći, pa invazivni karcinom ima najnižu
elastičnost, a potom neinvazivni karcinom, fibrozno tkivo dojke,
normalno glandularno tkivo, potom masno tkivo dojke i to ovim
redom. Elastografska evaluacija se vrši kvalitativno (Tsukuba
elastični skor, TES) i semikvantitativno (odnosom kompresibilnosti
masti i lezije, FLR, SR). CILJ RADA: Cilj istraživanja je bio da se
proceni da li elastografijom, kao novom metodom, mogu da se
postignu bolji rezultati u detekciji i razlikovanju benignih i
malignih lezija, u odnosu na konvencionalni ultrazvuk i mamografiju
i da li kombinacija elastografije i ultrazvuka ili elastografije,
ultrazvuka i mamografije može da poveća pouzdanost dijagnostike
lezija u dojci. Takođe je cilj bio i da se utvrdi da li postoje
neke kliničke osobine bolesnica koji mogu da utiču na ove
dijagnostičke testove. MATERIJAL I METODE: Istraživanje je
obavljeno u KBC „Bežanijska kosa“, u Beogradu kao jednogodišnja
prospektivna studija preseka. Studija je obuhvatila ukupno 128
lezija kod 122 bolesnice (prosečne starosti 54 godine, raspona
21-84 godine) koje su dijagnostikovane kao benigne ili maligne,
mamografijom i ultrazvukom i dalje analizirane elastografijom.
Dijagnostički algoritam je uključivao i klinički pregled, kao prvi
test. Mamografija i ultrazvuk su analizirani i kodirani po BI RADS
klasifikaciji. Elastografija je određena Tsukuba elastičnim skorom
(TES), sistemom od 5 kategorija i vrednošću odnosa kompresibilnosti
masnog tkiva i lezije (SR, FLR). Dijagnostički rezultati su
poređeni sa histopatološkim nalazom, kao standardom. Izračunati su
senzitivnost, specifičnost, tačnost, pozitivna i negativna
prediktivna vrednost i stepen lažno-pozitivnih i lažno-negativnih
nalaza sa svaki modalitet. Primenili smo ROC analizu u proceni
uloge elastografije i drugih modaliteta i njihovih kombinacija u
dijagnosti lezija u dojkama...

▼ This dissertation contributes to the field of health economics, which, in the past couple of decades, has substantially increased our understanding of the determinants of human health, health-related behavior, and health care choices.
A large body of literature has documented the influence of peer group behavior on individual choices. The purpose of my research is to examine the extent of such a phenomenon in breast cancer preventive behavior. Using Behavioral Risk Factors Surveillance System (BRFSS) surveys from 1993-2008, I measured the effect of other female screening behavior on an individual's decision to have a routine breast cancer screening by calculating the size of a so called social multiplier in mammography.
I estimated a vector of social multipliers in the use of annual mammograms by taking the ratio of group-level effects of exogenous explanatory variables to individual-level effects of the same variables. Peer groups are defined as same-aged women living in the same geographical area: county or state. Several econometric methods were used to analyze the effect of social interactions on decision to undergo mammography, including ordinary least squares, fixed effects, the split sample instrumental variable approach, and a falsification test.
My results supported the hypothesis that social interactions have an impact on the decision to have a mammogram. For all women over age 40, I found strong evidence of social interactions being associated with individual's education and ethnicity. In addition, the decision for women ages 40-49 to have a screening was subject to peer influence through their place of employment and ownership of health insurance. Finally, for women age 75 and older, being married and aging were the most important channels through which peer group influenced the decision to have a mammogram.
This research has important policy implications in the presence of current health care reform that reimburses breast cancer screening at 100%, while rates of mammography receipt remain below the policy goal.
Furthermore, I examined the effect of the 2009 United States Preventive Services Task Force change in screening recommendations on screening behavior. I demonstrated an immediate reduction in the receipt of mammography among women of all age groups following the revision of screening guidelines. I found that in 2010, the twelve month mammography receipt decreased by 1.97 (women ages 40-49), 2.20 (ages 50-74), and 3.61 (age 75 and older) percentage points, and the twenty-four months mammography receipt decreased by 1.47 (women ages 40-49), 1.05 (ages 50-74), and 1.92 (age 75 and older) percentage points. Analysis using a two-year follow up period after the revision of screening recommendations provided further support to this conclusion.

▼ A computer-aided diagnostic (CAD) system that uses a unique shape-based classification scheme, the Ellipse-Closed Curve Fitting (ECCF) algorithm, is developed for digital mammogram image analysis. The system is developed to work as a post-processing extension to a previously developed CAD system that locates and segments mass lesions, or tumors, found in digital mammograms into separate images. The ECCF system is implemented in the MATLAB mathematical scripting language and is thus capable of running on multiple platforms. The ECCF algorithm detects edges in tumor images and casts them into closed curve functions. Parameters for an ellipse of best fit for a closed curve function are computed in a way analogous to that in linear regression, where a line of best fit is determined to fit a set of data points. In addition to the shape-fitting algorithm, the ECCF system comprises several other independently functioning components, including auxiliary algorithms and techniques that perform image cropping and edge detection, employed initially to prepare the images for efficient processing, and self-test tools that calculate R2, area matching ratios, and a "shape conformity value" to determine the "goodness of fit". Output generated by the ECCF system for sufficiently large image sets may contain correlations between malignant tumors and their shape that may be captured with data mining techniques, the implementation of which may result in an improved integrated CAD system.

▼ PURPOSE: Breast conserving surgery (BCS) requires the establishment of adequate margins to reduce local recurrence. The purpose of this study was to determine whether intraoperative specimen mammography is effective and efficient as margin assessment method in Asian women.
METHOD: A total of 182 patients who had breast cancer treated with BCS between October 2015 and September 2017 were evaluated. After wide excision, intraoperative specimen mammography was used to assess margin adequacy. Whenever the margins were close to the lesions, further resection was performed. And 4 direction frozen section analysis were done in 84 patients who were treated with BCS between January 2012 and December 2014. We compared two groups of different margin assessment methods. The margins were histologically assessed and correlated with the specimen mammographic findings.
RESULTS: 61.6% patients have dense breasts (mammographic density > 50%) and 85.7% of dense breasts could margin assess by intraoperative specimen mammography. Incomplete intraoperative excision detected by specimen mammography led to immediate re-excision in 24 (13.2%) cases which were more than 13 (15.5%) cases in group of frozen section analysis (p = 0.47). Significant correlations were found between the radiological and histological margins (R2 = 0.222, p < 0.05). The operation time of frozen section analysis group were statistically significant longer than specimen mammography group (mean 108.4 and 85.2 minutes, respectively).
CONCLUSION: Our results show that breast lesions intraoperative specimen mammography for BCS is useful to identify margin clearance regardless of the mammographic density.

▼ INTRODUÇÃO: O objetivo deste estudo é determinar a correlação anatomopatológica das calcificações amorfas agrupadas diagnosticadas na mamografia digital de campo total, ou seja, das calcificações suspeitas mais tênues, uma vez que houve aumento da caracterização de calcificações na mamografia digital. MÉTODOS: Estudo retrospectivo baseado nos laudos mamográficos classificados como categoria BI-RADS 4 no primeiro ano de introdução da técnica digital, com análise dos diagnósticos anatomopatológicos das microcalcificações amorfas agrupadas submetidas à biópsia de fragmento assistida à vácuo no serviço. Calculou-se: os achados anatomopatológicos que se associaram ao achado radiológico de microcalcificações amorfas agrupadas e o valor preditivo positivo destas calcificações biopsiadas. RESULTADOS: Dos 219 achados por microcalcificações amorfas agrupadas, 78 foram submetidos à biópsia de fragmento assistida à vácuo com seguimento conhecido ou cirurgia subsequente. O diagnóstico anatomopatológico correspondeu a oito (10%) casos malignos, 36 (46%) casos benignos, e 34 (44%) diagnósticos de lesões de risco, das quais oito (10%) do subgrupo cicatriz radiada/lesões papilíferas (sete cicatrizes radiadas e um papiloma) e 26 (33%) do subgrupo atipias/ neoplasias lobulares, correspondendo a 14 (18%) hiperplasias ductais atípicas, quatro (5%) neoplasias lobulares e oito (10%) lesões de células colunares com atipia (atipia epitelial plana). A ampliação cirúrgica foi recomendada para as lesões com potencial incerto de malignidade à biópsia e realizada em 65% do subgrupo atipias/neoplasias lobulares, com taxa de subestimação nula (0/18). O tempo médio de seguimento das pacientes com diagnóstico benigno ou de lesão de risco foi 22 meses. CONCLUSÕES: Um terço das microcalcificações amorfas agrupadas em mamografia digital de campo total corresponderam a lesões precursoras representadas pelas atipias (ductais e colunares) e neoplasias lobulares. Essas lesões se associaram às calcificações amorfas agrupadas numa proporção de cerca de 3:1 em relação às lesões malignas, cujo VPP 3 foi 10
PURPOSE: To determine the anatomopathological correlation of grouped amorphous calcifications (the most tenuous of suspicious calcifications) disclosed on full-field digital mammography, given the enhanced characterization of calcifications provided by digital mammography. METHODS: A retrospective study of mammographic reports classified as BI-RADS® category 4 at a private diagnostic service specialized in breast imaging was carried out on exams performed during the first year of introducing the digital technique. ▼ OBJETIVOS: Avaliar a certeza de qualidade dos resultados mamográficos em uma instituição de referência para diagnóstico e tratamento do câncer de mama na Região Sul do Brasil, considerando as recomendações da quinta edição do BIRADS (“Breast Imaging Reporting and Data System”) para propósitos de auditoria. MATERIAL E MÉTODOS: Foi realizado um estudo de coorte observacional retrospectivo e transversal com 4911 pacientes que foram submetidas a pelo menos uma mamografia durante o ano de 2013 em um hospital público regional, ligado a uma universidade pública federal na Região Sul do Brasil. Essas pacientes foram acompanhadas até 31 de dezembro de 2014, para checar as taxas de desempenho dos resultados mamográficos na instituição em estudo, incluindo verdadeirospositivos (VP), verdadeiros-negativos (VN), falso-positivos (FP), falso-negativos (FN), valores preditivos positivos (VPP), sensibilidade e especificidade com um intervalo de confiança de 95%. RESULTADOS: O estudo demonstrou alta certeza de qualidade nos resultados das mamografias digitais diagnósticas, especialmente em relação à sensibilidade (90,21%) e especificidade (98,97%). O valor preditivo positivo (VPP) geral foi estimado em 65,3%. A taxa de interpretação anormal (ou reconvocação) foi de 12,26% e a razão de verossimilhança (“likelihood ratio”) das mamografias diagnósticas obtida foi 86,68%. CONCLUSÕES: Conclui-se que os resultados de mamografias digitais diagnósticas foram apropriados e similares aos valores pospostos pelo BIRADS. Além disso, o estudo proporcionou auto-reflexão e auto-avaliação da prática radiológica no serviço, o que é essencial para o melhoramento e processo de colaboração mais efetivo entre todos os profissionais envolvidos com o diagnóstico e tratamento do câncer de mama. Esses resultados podem incentivar outras instituições a atingirem altas taxas de qualidade de desempenho nos resultados mamográficos, apesar de possíveis limitações estruturais quando comparadas a instituições de países desenvolvidos.
OBJECTIVES: To evaluate the quality assurance of the mammography results in a reference institution for breast cancer diagnosis and treatment in southern Brazil, considering the BIRADS (Breast Imaging Reporting and Data System) 5th edition recommendations for auditing purposes. MATERIAL AND METHODS: We performed a retrospective cohort observational and cross-sectional study with 4911 patients who underwent at least one mammography throughout the year of 2013 in a regional public hospital, linked to a federal public university in southern Brazil. ▼ Rationale and Objectives. The objectiv of the present study was to evaluate a
near-infrared (NIR) laser breast imaging system using the example of the
computed-tomography-laser-
mammography-system [CTLM]) as an adjunct or an
alternative to
mammography. The near-infrared-technique used in this study is
based on the absorption of near-infrared-light by haemoglobin and desoxy-
haemoglobin. Therefore carcinomas of the female breast can be detected
indirectly by imaging their neovascularisation. Materials and Methods. 276
patients were examined by four well-versed radiologist using the information
of CTLM,
mammography and the combination of both techniques. 124 of the 276
patients underwent biopsies (52 malignant cases and 72 benign cases), and 152
patients had a 2-year follow up. Three-dimensional scans were acquired with a
near-infrared-laser-computed-tomographic-scanner (CTLM) at a slice thickness
of 4 mm. Mammograms were analyzed alone and in combination with CTLM. Results.
For the combination of
mammography and CTLM, the area under the ROC curve was
larger than for
mammography alone. But the difference was not statistically
significant. In addition the direct comparison between the systems showed an
evident superiority of the
mammography according to the CTLM. With a
sensitivity of 0,38 comparing to 0,73 for the
mammography, CTLM was clearly in
an inferior position, in spite of better specificity with 0,84 for CTLM versus
0,82 for the
mammography. Furthermore the CTLM was less sensitive in the
examination of the subpopulation of patients with dense parenchyma
(sensitivity 0,32 versus 0,62 for the
mammography). The area under the ROC
curve for the CTLM as a single diagnostic method was significant smaller than
the area under the ROC curve for the
mammography (0.6841 versus 0.8311)
independent of the density of the tissue of the breasts. (p < 0,0001 and/or
0.0072). In addition the weakness of the CTLM was shown in the poor
reliability of its results. With inter-rater-reliability-coefficients of 0,21
and/or 0,26 evident deviations between the statements of the four readers in
their assessments were shown. In opposite the mammographic diagnoses of the
four readers matched in a larger scale which is shown in inter-rater-
reliability-coefficients of 0,64 and/or 0,67. Conclusion. As an adjunct the
CTLM can give additive information in the diagnostics of the carcinoma of the
female breast. But comparing CTLM with
mammography the CTLM is in an inferior
position. At the present time CTLM achieves less accurate information
comparing to
mammography

▼ To examine the association between religiosity and breast cancer screening methods, 6,541 women aged 60 and older from the Health, Well-Being and Aging in Latin America and the Caribbean Study (SABE) were studied. The outcomes were reporting a mammogram, a clinical breast examination (CBE) or breast self-examination (BSE) within the last 2 years. Women who reported religion being very important were more likely to have a mammogram (OR=1.90, 95% CI 1.53-2.35), a CBE (OR= 1.70, 95% CI 1.44-2.00) and a BSE (OR= 1.44, 95% CI 1.23-1.68) compared with women who reported no religious affiliation. This suggests that religiosity may facilitate breast cancer screening behaviors among older women.

▼ A mamografia é o método de diagnóstico mais eficaz na detecção precoce de doenças
mamárias. Os sistemas digitais permitem obter radiografias com melhor qualidade de
imagem, mas nem sempre estão associadas a protocolos de aquisição otimizados, resultando
em doses absorvidas mais elevadas nas glândulas mamárias. Neste sentido, tornam-se
pertinentes levantamentos dosimétricos e avaliações da proteção radiológica nas instituições
que realizam mamografias. O presente trabalho tem como objetivo estimar a dose glandular
média (DGM) em pacientes que realizaram mamografias em sistemas digitais de Recife,
Brasil e avaliar a qualidade das imagens obtidas. Foram avaliados: um mamógrafo Siemens,
modelo Mammomat 3000 Nova e leitora CR Carestream, modelo Kodak DirectView; dois
mamógrafos digitais DR Lorad, modelo Hologic Selenia e um equipamento analógico (telafilme)
Lorad M-IV com uma processadora de filmes Kodak, modelo X-Omat 2000. As DGMs
foram estimadas a partir da metodologia descrita por Dance e colaboradores, em pacientes
com idades entre 40 e 64 anos, com espessuras de mama comprimida que variaram entre 2 e
9 cm. Foram levadas em conta apenas as incidências de rotina (CCD, CCE, MLOD, MLOE).
Ao todo, foram coletados os seguintes parâmetros de irradiação de 5200 mamografias: tensão
(kV); carga (mAs); combinação anodo/filtro, bem como, a espessura da mama comprimida,
força de compressão utilizada e idade da paciente. Em relação à qualidade da imagem, foram
analisadas a função da transferência de modulação (MTF), a razão sinal-ruído (SNR) e a razão
contraste-ruído (CNR) para diferentes espessuras de simuladores de mama, de acordo com o
documento Humam Health Series 17, da Agência Internacional de Energia Atômica.
Paralelamente, utilizando os mesmos critérios de avaliação, médicos radiologistas analisaram
50 imagens mamográficas. Os resultados mostraram que os valores das DGMs foram de:
2,70±0,50 mGy para o equipamento tela-filme, 3,91±0,72 mGy para o equipamento CR,
2,86±0,53 mGy para o DR1 e 3,83±0,70 mGy para o DR2. As espessuras médias das mamas
foram, respectivamente: 4,82 cm; 6,73 cm; 6,02 cm; 6,11 cm. A força de compressão foi
menor do que o indicado para os equipamentos digitais e maior para o tela-filme. Os valores
de DGM obtidos com o simulador de mama foi menor que os obtidos para paciente com
espessura de mama equivalente. O parâmetro SNR foi considerado adequado para todos os
equipamentos. A razão contraste-ruído não foi satisfatória para mamas mais espessas, porém a
DGM se mostrou bem abaixo do limite recomendado para este tipo de mama, fazendo-se
necessário o ajuste no controle automático de exposição. A MTF foi satisfatória tanto para o
equipamento CR quanto para o DR1. A avaliação das imagens clínicas identificou falhas no
posicionamento, indicando a necessidade de treinamento para os operadores dos

▼ Sequential mammograms contain important information, such as changes of the breast or developments of the masses, for diagnosis of disease. Comparison of sequential mammograms plays an important part for radiologists in identifying malignant masses. However, currently computer-aided detection (CAD) programs can not use such information eciently. The diculties lie in the registration of sequential mammograms.
Most of current methods register sequential mammograms based on control points and image transformations. For these methods to work, extraction and correspondence of the control points is essential. This thesis presents a new approach in registering mammograms. The proposed method registers mammograms by associating mass-like objects in sequential mammograms directly. The mass-like objects appear in the images of normal breasts as well as images of breast with cancer. When the mass-like objects in sequential mammograms are accurately associated, measurements of changes in mass-like objects over time become possible.
This is an important way to distinguish mass-like objects associated with cancer from cysts or other benign objects.
The proposed method is based on graph matching. It uses the internal structure of the breast represented by the spatial relation between the mass-like objects to establish a correspondence between the sequential mammograms. In this method, the mammogram is firstly segmented into separate components using an adaptive pyramid (AP) segmentation algorithm. A series of filters, based on the features of components, is then applied to the components to remove the undesired ones. The remaining components, the mass-like objects, are represented by a complete graph. The spatial relations between the remaining mass-like objects are expressed by fuzzy spatial relation representation and are associated to the edges of the graph as weights. Association of the mass-like objects of two sequential mammograms is realized by finding a common subgraph of the corresponding two graphs using the backtrack algorithm.
The segmentation methods developed in the course of this work were tested on a separate problem in computer-aided detection of breast cancer, namely the automatic extraction of the pectoral muscle.
The graph matching method was tested independently of the segmentation method on artificially distorted mammograms and the full process, including the segmentation and the graph matching, was evaluated on 95 temporal mammogram pairs. The present implementation indicates only a small improvement in cancer detection rates but also presents opportunities for a substantial development of the basic method in the future.

▼ Rintasyöpä on naisten tavallisin syöpätyyppi ja yleisin tai toiseksi yleisin syövästä johtuva kuolinsyy länsimaissa. Taudin nopeaan kasvuun on monia syitä, jotka heijastavat sosioekonomista kehitystä, ikääntymistä ja väestönkasvua. Mahdollisista riskitekijöistä on saatavilla paljon tutkimustietoa, ja väestöpohjaisen mammografiaseulonnan on todettu olevan tärkeä keino taudin havaitsemiseksi varhaisessa vaiheessa. Tärkeitä riskitekijöitä ovat naisten seulontakäynneillä ilmoittamat ”rintaoireet”, joita ei kuitenkaan aina arvioida riittävän hyvin tai joita koskevia tietoja ei laisinkaan kerätä seulontaohjelmassa. Tämä väitöstutkimus pyrkii esittämään uutta näyttöä näiden naisten rintasyöpiä koskevista tuloksista.
Työ koostuu neljästä osatutkimuksesta, joissa on hyödynnetty erilaisia rekisteriperäisiä yksilötasoisia tietoja. Tutkimuksen ensimmäisessä osassa selvitimme mammografiaohjelman toimivuutta kuvaavien mittareiden suhdetta rintaoireisiin. Totesimme, että naiset, joilla oli oireita, heidät kutsuttiin useammin jatkotutkimuksiin ja heillä havaittiin useammin syöpä kuin oireettomilla naisilla. Kyhmyjen herkkyys ja positiivinen ennustearvo (PPV) olivat suurempia kuin vetäymien ja nännieritteiden. Mammografiatutkimuksen herkkyys ja PPV-arvot olivat suurempia, kun käyntiin liittyi kyhmy tai vetäymä, mutta ei silloin kun kyseessä oli nännierite.
Toisessa kohorttitutkimuksessa arvioitiin väärän positiivisen tuloksen ja väärän positiivisen lähetteen kumulatiivista todennäköisyyttä ja verrattiin oireisia ja oireettomia käyntejä. Naisilla, joilla oli todettu kyhmy vähintään kerran, väärän positiivisen testituloksen kumulatiivinen riski kymmenen seulontakäynnin jälkeen oli 45 %, ja vastaava arvio oli 25 % vetäymälle ja 35 % nännieritteelle. Vastaava luku oireettomille käynneille oli noin 18 %. Tämän perusteella totesimme, että väärän positiivisen lähetteen kumulatiivinen riski oli suurempi kaikissa oiretyypeissä verrattuna vastaavaan oireettomaan käyntiin.
Kolmannessa vaiheessa arvioimme seulonnassa havaittujen ja seulontakertojen välillä havaittujen rintasyöpien riskiä suhteessa rintaoireisiin. Seulonnassa havaitun rintasyövän ikävakioitu riski käynneillä, joihin liittyi kyhmy, vetäymä tai nännierite oli vastaavasti 8 kertaa, 2,4 kertaa ja 1,7 kertaa suurempi kuin käynneillä, joihin ei liittynyt oireita. Riski oli 22 kertaa suurempi, jos samalla käynnillä todettiin sekä kyhmy että vetäymä. Seulontakertojen välillä ilmenevien syöpien riskiä arvioitiin vertaamalla oireisia ja oireettomia käyntejä taustamuuttujien avulla. Oireiden ja seulontakertojen välisen rintasyövän riskin välillä oli vahva positiivinen yhteys. Välisyöpien kumulatiivinen ilmaantuvuus lisääntyi seulonnan jälkeen nopeasti, kun seulontakäynnnilä oli todettu kyhmy.
Lopuksi arvioimme rintasyöpäkuolleisuuden ja kaikista syistä johtuvan kuolleisuuden riskiä oiretyyppien mukaan ja vertasimme niitä oireettomiin käynteihin. Rintasyövästä johtuva kuolleisuusaste oli hyvin korkea viiden vuoden sisällä seulontakäynnistä, johon liittyi kyhmy tai vetäymä.…
