▼ Severe lower limb injuries are potentially devastating and pose some of the most difficult decisions in trauma surgery. The goal is to ensure survival and reconstruct the most functional limb possible. Ideally this is achieved by salvaging the injured limb. However, in certain situations amputation is the safest and most effective method of achieving an optimal outcome. Errors in these decisions may have profound consequences, yet they are frequently based on incomplete information and uncertain risks. Furthermore, most surgeons have limited experience making these decisions, and existing decision-support tools are unhelpful. The aim of this thesis was to improve the understanding of decision-making following severe lower limb trauma, and develop accurate prognostic models that can help identify those patients whose limb can be safely and effectively salvaged, and also identify those for whom attempts at limb salvage would be dangerous or fail. The rationale for amputation decisions was analysed in a cohort of severe lower limb injuries (n = 579). Two prognostic models were designed to support difficult aspects of these decisions. Both models were developed using Bayesian networks that combine existing knowledge with individual patient data. The first provides early and accurate identification (AUROC = 0.927) of patients at risk of Trauma-Induced Coagulopathy, the principal indication for damage-control intervention. The model's performance in new patients, and ability to handle missing predictor information, was prospectively validated. The second model accurately predicts the likely outcome, in terms of viability, of attempted limb salvage. This model outperformed the most widely used decision-support tool, the Mangled Extremity Severity Score (AUROC 0.932 versus 0.723; P < 0.0001). These Bayesian network tools accurately quantify critical risks that make rational judgement on the safety and effectiveness of interventions possible. This information enables individualised and evidence-based decisions, at a time when decision-making is most effective.

▼ OBJECTIVE: To evaluate the humoral and cellular immune response against bone associated antigens when delivered in a vaccine, and to evaluate the immunomodulation on the aforementioned immune response with the addition of adipose-derived mesenchymal stromal cells (AD-MSCs). ANIMALS: 68 C57BL/6 mice PROCEDURES: Femur fragments harvested from Balb/C or C57BL/6 mice were resuspended in PBS and cationic liposomal DNA complexes (CLDC) to create an allograft or autograft vaccine, respectively. A positive control vaccine was created utilizing bovine alkaline phosphatase (ALP) resuspended in PBS and CLDC. Twenty C57BL/6 mice were divided into four treatment groups: non-vaccinated (n=5), ALP vaccine recipients (n=5), allograft bone vaccine recipients (n=5), or autograft bone vaccine recipients (n=5). Forty-eight C57BL/6 mice were divided into the same 4 vaccine treatment groups (n=16), and received either intravenous AD-MSCs (n=8) or a subcutaneous injection of AD-MSCs (n=8). All mice received an initial vaccine on Day 1 and a booster vaccine on Day 7, followed by euthanasia on Day 21. Blood was collected on Day 1 and Day 7 just prior to vaccination, and on Day 21 just prior to euthanasia. Serum was subjected to an antibody detection ELISA to evaluate the humoral response. Spleens were collected and flow cytometry was used to evaluate T cell proliferation as an indicator of the cellular immune response. RESULTS: The bone vaccines did no elicit a detectable humoral immune response to the total bone antigen vaccine used in this model. The addition of AD-MSCs had no effect on the lack of a detectable humoral immune response. The T cell response towards a bone antigen was dampened in mice previously vaccinated with a bone vaccine. This effect was most pronounced when looking at the T cell response towards an allograft bone antigen in mice previously vaccinated with an allograft bone vaccine, particularly with the addition of AD-MSCs. CONCLUSIONS AND CLINICAL RELEVANCE: The bone vaccine model used in this study allowed evaluation of the humoral and cellular immune response towards bone associated antigens. The model suggests that recipients of an allograft bone vaccine will dampen the T cell proliferation seen upon second exposure to the bone antigens. This model could be used in future vaccine studies looking at the effect of vaccinating mice with a bone vaccine prior to undergoing a limb-salvage procedure. If efficacious, the bone vaccine model may provide a new treatment option for decreasing the risk of transplant rejection following massive limb reconstruction. ▼ Introduction: Sarcomas are among the very rare malignant tumors with over 70 different subtypes. The infiltration of vital and functionally important structures such as vessels and nerves are often a particular challenge in surgical treatment. The aim of this study was to examine operated soft tissue sarcomas on extremities at the Charité on overall survival. There was a separate analysis of the sarcoma, that received vascular replacement to ensure the limb perfusion. Methods: The study was carried out in retrospective study design with the evaluation of already recorded data sets. Included were all patients with sarcoma disease of the extremities, that were operated between 2006 and 2015 at the Charité. Excluded from the analysis were patients with osteosarcoma, chondrosarcoma and PNET / Ewing's sarcoma. 154 patients were included in the analysis. Six patients received vascular replacement. Statistical analysis for overall survival was performed as a univariate analysis using Kaplan-Meier curves. All values with a p ≤ 0.05 in the log-rank test were considered statistically significant. Statistically significant values of the univariate analysis were further investigated using the Cox regression analysis with back elimination for the multivariate analysis. Statistically significant were all values with p ≤ 0.05, that were associated with survival as influencing factors. Results: The 5-year survival rate was 64.4 %, the 5-year recurrence-free survival rate was 58.3 %. Factors influencing overall survival in the univariate analysis were ASA classification (p = 0.029), tumor size (p = 0.024), lymph node findings (p < 0.000), metastasis of the primary tumor (p = 0.015), grading of FNCLCC (p = 0.003), vascular replacement (p = 0.039), treatment of metastasis in the primary tumor (p < 0.000), intraoperative pRBC (p <0.000), intraoperative FFP (p <0.000), intraoperative radiotherapy (p = 0.001), postoperative complications (p = 0.017), Clavien-Dindo classification (p < 0.000). In the multivariate analysis, predictors of overall survival were ASA classification (p = 0.002, HR 1.5 [1.1 - 2.09]), grading of FNCLCC (p = 0.013, HR 1.8 [1.1 - 3.0]), treatment of metastases of the primary tumor (p < 0.000, HR 0.4 [0.24 - 0.64]), FFP administered intraoperatively (p = 0.004, HR 3.5 [1.4 - 8.3]), intraoperative radiotherapy (p = 0.003, HR 26 [2.1 - 217.6]). The hazard ratio (HR) was used as a descriptive measure to compare survival times. Conclusion: The surgical removal of soft tissue sarcomas of the extremities and patients with vascular replacement have a moderate survival probability. Vascular replacement compared to vascular preservation in the presence of vascular tumor involvement shows statistically significant higher 5-year survival. The result of vascular resection with vascular replacement is not significant in the multivariate analysis and thus the overall survival is multifactorial independent of vascular resection that has taken place. ▼ Traumatic injuries to the extremities are commonly observed in emergency room patients and military personnel in combat. Restoring high mobility and functionality is a primary goal post-injury, which may require the use of rehabilitative devices, surgical interventions, and rehabilitation therapies. The research detailed in this dissertation investigates specific elements of these approaches through the use of experimental study and modeling and simulation. In the first study, the influence of passive-dynamic ankle-foot orthosis bending axis on the gait performance of limb salvage subjects was investigated. Bending axis location was altered by fabricating customized orthosis components using additive manufacturing and was tested in a gait laboratory. Altering bending axis location did not result in large or consistent changes in gait measures, however subjects expressed strong preferences for bending axis condition and preference was strongly related to specific gait measures. This suggests that preference and comfort are important factors guiding the prescription of bending axis location. In the second study, musculoskeletal modeling was used to examine the influence of transfemoral amputation surgical techniques on muscle capacity to generate forces and moments about the hip. Muscle reattachment tension and stabilization were shown to be critical parameters for post-amputation capacity, which supports the use of myodesis stabilization (muscle is reattached directly to bone) in amputation procedures. In the third study, a forward dynamics simulation of transfemoral amputee gait was developed and used to examine individual muscle and prosthesis contributions to walking subtasks. The residual hip muscles, and intact ankle, knee, and hip muscles worked synergistically to provide body support, anteroposterior propulsion, mediolateral control, and leg swing. Increased contributions of contralateral muscles to ipsilateral subtasks as well as increased duration of specific muscle contributions were observed in comparison to non-amputee and transtibial amputee walking. These findings can be used to help develop targeted rehabilitation therapies and improve transfemoral amputee locomotion. Through elucidating the influence of PD-AFO bending axis on gait performance as well as the influence of transfemoral amputation surgical techniques on muscle capacity and function, this research provides a foundation for improved rehabilitation outcomes, and thus mobility for individuals who have experienced traumatic lower-limb injuries. ▼ Cardiovascular diseases are the underlying cause for majority (>30%) of deaths worldwide. They include coronary heart disease and peripheral artery disease, conditions characterized by limited blood flow and inadequate oxygenation. Treatment strategies include management of symptoms and risk factors, reduction of oxygen demand and surgical revascularization to increase circulation. Therapeutic revascularization is a potential alternative for patients who are poor candidates for these interventions due to advance disease or co-morbidities. Revascularization involves the genetic or pharmacologic stimulation of vascular growth processes to facilitate tissue perfusion and promote functional recovery. Adaptive vascular growth occurs via capillary vessel growth (angiogenesis) and growth or remodeling of collateral arteries (arteriogenesis). Delivery of angiogenic factors such as fibroblast growth factor 2 (FGF2) to ischemia tissues can stimulate blood vessel growth. FGF2 consists of two classes of protein isoforms generated from alternative translation of the <i>Fgf2 </i>gene, high molecular weight (HMW) FGF2, and low molecular weight (LMW) FGF2. Proof-of-concept studies in animal models of chronic ischemia provided evidence for the therapeutic potential of exogenous LMW FGF2. This promise, however, did not translate into successful clinical use. Currently, the functions of the endogenous FGF2 isoforms in ischemia-induced revascularization are not well understood. Elucidating the function(s) of the FGF2 isoforms in vascular growth is of great clinical importance and may lead to the development of novel pharmacological therapies for ischemic diseases.Mice with a targeted deletion of all FGF2 isoforms (Fgf2-/-), HMW FGF2 (FGF2 LMW-only) and LMW FGF2 (FGF2 HMW-only) were employed to identify the distinct role(s) of the FGF2 isoforms in chronic ischemia. Revascularization was evaluated in mice subjected to chronic hindlimb ischemia using measures of limb function, tissue viability, and vessel growth. FGF2 HMW-only mice had a faster recovery of limb function compared to wildtypes. <i>Fgf2-/-</i> and FGF2 LMW-only mice, however, had a significantly slower recovery of function. Additionally, no limb necrosis was detected in FGF2 HMW-only mice while Fgf2-/- and FGF2 LMW-only mice had significant necrosis of their ischemic limbs. The early recovery of limb function in FGF2 HMW-only limbs was preceded by improved revascularization (angiogenesis and arteriogenesis). Vessel growth was significantly decreased in FGF2 LMW-only muscles and not different from wildtypes in Fgf2-/- mice, indicating that the presence of LMW FGF2 inhibited vascular growth. The effect of the HMW FGF2 isoforms on revascularization and protection from ischemic injury was not associated with activation of FGF receptor (FGFR). Angiogenesis-related proteins including IGFBP-3, IGFBP-10 and CX3CL1 were increased in HMW FGF2-only muscles and are likely involved in the protective effect of HMW FGF2. ▼ Hand dominance is the preferential use of one hand over the other for motor tasks. 90% of people are right-hand dominant, and the majority of injuries (acute and cumulative trauma) occur to the dominant limb, creating a double-impact injury whereby a person is left in a functional state of single-handedness and must rely on the less-dexterous, non-dominant hand. When loss of dominant hand function is permanent, a forced shift of dominance is termed injury-induced hand dominance transfer (I-IHDT). Military service members injured in combat operation may face I-IHDT following mutilating injuries (crush, avulsion, burn and blast wounds) that result in dominant limb amputation or limb salvage. Military occupational therapy practitioners utilize an intervention called Handwriting For Heroes to facilitate hand dominance transfer. This intervention trains the injured military member how to write again using the previously non-dominant hand. Efficacy and clinical effectiveness studies were needed to validate the use of this intervention. This dissertation contains three studies related to I-IHDT. One study measured handwriting performance in adults who previously (greater than 2 years ago) lost function of their dominant hands. Results verified that handwriting performance, when measured on two separate occasions (six-weeks apart) was similar (stable). A second study examined the efficacy of Handwriting For Heroes in non-impaired participants. Results demonstrated a positive effect on the variables that measured the written product: legibility, writing speed (letters-per-minute); as well as a positive effect on the variables that measured the writing process: kinematic and kinetic parameters. The final study examined the clinical effectiveness of Handwriting For Heroes in an injured military population. Results did not show as positive results as the efficacy study, despite similar compliance with the intervention. Specifically, non-impaired participants started with faster writing speeds in their non-dominant hands (higher letters-per-minute) and made more gains (wider ranges). The non-impaired participants also started with faster dexterity (betters scores on the Grooved Pegboard) but they made fewer gains than the injured service members (smaller ranges). Nevertheless, injured participants clearly made gains in all dependent variables thereby demonstrating clinical effectiveness of the intervention

▼ The use of chemotherapy has increased survival in patients with Ewing’s sarcoma (ES), with the consequent development of local surgery techniques as limb salvage (LS). Survival and adverse events have been widely studied in general ES series,as well as local complications in LS series. However, there is not specific series of ES cases treated by LS, despite of being the most used local technique. Survival, oncological events and local complications in ES locally treated by LS were studied in this study. MATERIAL AND METHODS: ES patients treated at Hospital de la Santa Creu i Sant Pau in Barcelona between 1984 and 2008 were included in this study. Cases treated by multimodal therapy and locally by LS were selected. Patient’s characteristics, type of treatment and adverse events were analyzed. RESULTS: Ninety patients were studied. Fifty of them were treated by multimodal therapy and LS. Mean age was 20 years. Overall survival was 68.8% and event free survival was 60.6%. Response to chemotherapy, pelvic location and age were found as prognostic factors. Metastasis developed in 36% of the patients. Response to chemotherapy, pelvic location and local recurrence were independent risk factors. Local recurrence was found in 14% of cases. Age, resection margins and axial location were associated with worse local control, but the differences were not significant in multivariate analysis. 26.3% of reconstructed cases developed mechanical complications. Tumor size and type of reconstruction were found as independent risk factors for mechanical failure. Postoperative infection was found in 7.9% of reconstructed cases. Secondary amputation was made in 6%, finding no independent risk factors. DISCUSSION: Survival and oncological adverse events in ES treated by LS are similar to previous data and studies with general treatment and other local technics. However, dissemination at diagnosis is not a prognostic factor because of positive selection in ES treated by LS. Metastatic spread after LS has a similar frequency as in previous general studies. Local recurrence is a risk factor for metastasis appearance, and other predictive risk factors are similar to prognostic factors in previous general ES studies. Local recurrence in ES treated byLS is similar to ES treated by any surgical technique. Independent predictive risk factors were not found for local recurrence. Mechanical failure in ES treated byLSis similar to previous series of LS with different types of tumor. Osteoarticular allograft reconstructions and tumor size bigger than 10 cm have an increase risk of mechanical failure. Infection in ES treated by LS is similar to previous LS studies, being more frequent in prosthetic reconstructions, tibial location and patients treated with radiotherapy before surgery. Secondary amputation is more frequent in tibial tumors and is associated with local infection, without significant differences in multivariate analysis. Peripheral arterial disease is caused by plaque buildup in the peripheral arteries. Standard treatments are available when the blockage is proximal and focal, however when distal and diffuse the same type of the treatment options are not beneficial due to the diseased locations. Restoration of blood flow and further salvaging of the limb in these patients can occur in a retrograde manner through the venous system, called retroperfusion or arteriovenous reversal. Retroperfusion has been explored over the last century, where early side to side artery to venous connections had issues with valve competency prohibiting distal flows, edema buildup, and heart failure. However, more recent clinical studies create a bypass to a foot vein to ensure distal flows, and though the results have been promising, it requires a lengthy invasive procedure. It is our belief that the concerns of both retroperfusion approaches can be overcome in a minimally invasive/catheter based approach in which the catheter is engineered to a specific resistance that avoids edema and the perfusion location allows for valves to be passable and flow to reach distally. In this approach, the pressure flow relations were characterized in the retroperfused venous system in ex-vivo canine legs to locate the optimal perfusion location followed by in-vivo validation of canines. Six canines were acutely injured for 1-3 hours by surgical ligation of the terminal aorta and both external iliac arteries. Retroperfusion was successfully performed on five of the dogs at the venous popliteal bifurcation for approximately one hour, where flow rates at peak pressures reached near half of forward flow (37±3 vs. 84±27ml/min) and from which the slope of the P/F curves displayed a retro venous vasculature resistance that was used to calculate the optimal catheter resistance. To assess differences in regional perfusion, microspheres were passed during retroperfusion and compared to baseline microspheres passed arterially prior to occlusion in which the ratio of retroperfusion and forward perfusion levels were near the ratio of reversed and forward venous flow (0.44) throughout the limb. Decreases in critical metabolites during injury trended towards normal levels post-retroperfusion. By identifying the popliteal bifurication as a perfusion site to restore blood flow in the entirety of the distal ischemic limb, showing reversal of injury, and knowing what catheter resistances to target for further chronic studies, steps towards controlled retroperfusion and thus more efficient treatment options can be made for severe PAD patients.

Context. One of the most controversial subjects in anaesthesia today is whether or not neuraxial anaesthesia is more efficient to general anaesthesia in high-risk patients undergoing noncardiac surgery. The cumulative results showed that the incidence of postoperative cardiovascular morbidity and mortality is similar, regardless of type of the anaesthesia. So, is relevant to answer the search question: what is the efficiency of the neuraxial anaesthesia compared with general anaesthesia for lower-limbs revascularization in elderly? Objective. It is to determine the efficiency of the neuraxial anaestheisa versus general anaesthesia for lower-limbs revascularization in elderly. Hypothesis. The hypothesis is that the neuraxial anaestesia is more efficient (OR 0.67) than general anaesthesia for lower-limbs revascularization in elderly. Design. Systematic review with meta-analyse of the original articles of the randomized controlled trials. Setting. Federal University of Alagoas, Maceió, AL. Sample. Original articles of the randomized controlled trials that compared two anaesthetic technique (neuraxial anaesthesia vs. general anaesthesia) in elderly submitted to lower-limbs revascularization surgery. The information was accessed from EMBASE, LILACS, MEDLINE, CINHAL and ISI WEB OF SCIENCE. Main outcomes. Primary outcomes: Mortality, cerebral infarction, myocardial infarction, paralysis and postoperative lower limb amputation rate. Secondary outcomes: Duration of hospital stay, postoperative cognitive dysfunction, postoperative wound infection, other postoperative infections, neuraxial haematoma and complications in the anaesthetic recovery room. Complementary data: internal validity, external validity and statistical analyze. Statistical methods. For data analysis the odds ratio were used in the randon effect model with corresponding 95% confidence interval.

Contexto. A controvérsia atual é saber se a anestesia neuroaxial é mais eficiente do que a anestesia geral em pacientes de alto risco submetidos à cirurgia não cardíaca. Os resultados acumulados mostram que a incidência pós-operatória de mortalidade e morbidade cardiovascular é similar independentemente da técnica anestésica.
