▼ The integration of biological and genetic data has established that diverse biological processes, involving multiple effectors, influence circulating levels of triglyceride and cholesterol. This diversity may underlie the genetic complexity of human dyslipidemias, including the common and highly atherogenic condition, Familial Combined Hyperlipidemia (FCHL). The aetiologies of FCHL are currently undetermined. In this thesis, a multi-pronged approach was employed to identify genes/variants contributing to the linkage observed between the chromosome 21q22.2-22.3 interval and lipid traits, in white-British FCHL families. Additionally GPIHBP1, which encodes glycosylphosphatidylinositol-anchored high-density lipoprotein-binding protein 1, was studied. GPIHBP1 represents a strong FCHL candidate gene due to its role in the lipolytic processing of triglyceride-rich lipoproteins. Combined genetic and gene expression analyses, focussed upon a refined 3.8Mb interval on chromosome 21q22.3 that was linked to lipid abnormalities in subsets of FCHL families, identified two genes (COL18A1 and PKNOX1 ) that warrant further investigation with regard to their contribution to FCHL. Promising results were also obtained for C21orf57, which resides just outside the 3.8Mb interval. Genetic association analyses in 1725 members of 239 FCHL families identified nominal association (P=0.0009) between a TSPEAR variant, rs34163868, and plasma triglyceride levels. Furthermore, transcript levels of CBS and TRPM2 were significantly altered by treatment with the PPAR-agonist bezafibrate in a rat hepatoma cell line, thus implicating these genes in triglyceride/fatty acid metabolism. In combined analysis of five independent cohorts, the minor allele of the GPIHBP1 variant, rs11538388 was protective against hypertriglyceridemia (P=2.98x10-4). The same allele was associated with decreased risk of coronary heart disease in the prospective Northwick Park Heart Study II (hazard ratio for carriers=0.76, P=0.0480) and delayed age of onset in the Southampton Atherosclerosis Study (odds ratio=0.76, P=0.0146). Collectively, these data demonstrate that the rs11538388 minor allele, or variant in linkage disequilibrium, is associated with more favourable processing of atherogenic lipoproteins.

The combined in vitro and in vivo results presented in the thesis can be summarized as follows: Nigella sativa (NS) seed oil and it s methanol (NSF) or volatile oil (VO) extracts contained significant amounts of natural antioxidants, which directly react with and quench stable DPPH, cation ABTS and NO radicals and also possessed an effective OH radical scavenging activity in both the assay for the deoxyribose test and nonenzymatic lipid peroxidation in phospholipid liposomes. The presence of good quantity of phenolic compounds in NS seed oil, NSF and VO extracts may contribute to overall antioxidant activities. In addition, a significant chelating activity against Fe+2 in the oil and it s extracts suggest an in vivo preventive role in the free radical mediated oxidative damage. The data revealed that all the antioxidants, present in the thymoquinone (TQ) rich VO extract of NS seed oil, responsible for DPPH scavenging activity, were fully fractionated in NSF extract of oil. The comparison of scavenging activities of NS seed oil and it s NSF and VO extracts with pure TQ and pure limonene (LMN), two constituents of VO, against DPPH , OH, ABTS + and NO revealed a two-fold lower IC50 value for NSF, compared to VO, in the order NSF gt pure TQ gt NS oil gt pure LMN. Consistent with these results, TQ rich NSF extract exhibited a 2-fold higher inhibition of nonenzymatic lipid peroxidation in phospholipid liposomes by reducing the formation of MDA, which is mediated by a significantly stronger OH radical-scavenging activity compared to TQ rich VO extract of NS seed oil. Taken together, these findings demonstrate that 100 % higher quenching efficiencies of NSF extract against the above four free radicals are apparently due to the presence of all the radical-scavenging agents associated with VO extract plus good amount of mixture of phytosterols, known to exhibit strong antioxidant activity, are also extracted out in methanol as NSF from fixed oil portion of NS seed oil.

▼ Natural killer T cells (NKT) are T lymphocytes with a reactivity directed against glycolipid antigens presented by MHC-like CD1d molecules expressed by antigen presenting cells (APC). A large proportion of these cells (invariant NKT cells (iNKT)) possess strong immunomodulatory properties which impose a profound impact on the development and course of chronic diseases. The main aim of this project was to characterize iNKT cells and CD4+ iNKT subset numerically and functionally in the context of familial hypercholesterolemia (FH) and in murine models of dyslipidemia. Additionally, using an in vitro PBMC proliferation essay in the presence of iNKT cell antigen (α-GalCer) and IL-2, we investigated how lipoproteins affect the ability of iNKT cells to proliferate following α-GalCer stimulation. We report that FH patients exhibit a significantly decreased percentage of CD4 positive iNKT compared to age and sex matched controls (Control: 40.8% ± 4.2% (±SEM), N=31; FH patients: 19.2% ± 3.5% of total iNKT (±SEM); N=26; *P<0.01), while the total numbers of iNKT cells are similar between FH patients and control individuals. iNKT cell function measured as the ability of iNKT cells to proliferate in vitro upon α-GalCer stimulation was decreased in FH patients (~ 2 fold). iNKT cells levels are decreased in the liver of LDLR -/-, ApoE-/- and ApoA-I-/- mice, but increased in the blood of LDLR -/- and ApoE-/- mice (compared to WT mice). Additionally we report that iNKT cells are enriched in cardiac lymph nodes. In vitro experiments showed that the absence of lipoproteins abrogated iNKT cell proliferation in PBMC proliferation essay in presence of α-GalCer. Finally, our data indicate that HDL is the major lipoprotein class which supports α-GalCer exposure to APCs and subsequent iNKT cell presentation under physiological conditions. LDLr-/ - mice are spontaneously hyperlipidemic and resistant to the development of neointimal lesions. This study determined the influence of plasma levels of high density lipoprotein on cardiovascular inflammation, insulin resistance, and blood cell count in LDL (LDLr-/ -) receptor gene knockout mice. Three groups of 3-month-old male mice were used: Group WT, wild-type mice, Group S, LDLr-/ - mice fed a standard diet, Group HL, LDLr-/ - mice fed a hyperlipidic diet. After 15 days, blood was collected for analysis of plasma lipids, glucose, insulin, and hematological assays. The HOMA index was calculated to determine insulin resistance. The heart and aorta were removed and histologically processed. Heart sections were immunohistochemically processed with the anti-CD40L antibody to evaluate the inflammatory process. Artery sections were stained with hematoxylin-eosin and picrosirius red to assess morphological and morphometric changes. The S mice were resistant to inflammatory, had a low immunoreactivity to CD40L, high HDL plasma levels, and showed no insulin resistance, even with moderate hyperlipidemia in relation to WT. HL mice exhibited severe hyperlipidemia, increased immunoreactivity to CD40L, marked morphological alterations in the aorta wall, and insulin resistance, all associated with a decrease in HDL plasma levels in relation to S. The results showed a negative association between the plasma levels of high density lipoprotein and the total and differential leukocyte and platelet counts in the LDL receptor gene knockout mice. This ratio showed the important influence of the high density lipoprotein on the modulation of the immune and inflammatory response in dyslipidemias. Therefore, the evaluation of the blood cell count results, routinely correlated with the lipid plasma levels, can be promising in the prevention and prognosis of the severity of pathological conditions involving immune responses in dyslipidemias. The high HDL plasma level is a protective factor against the development of cardiovascular inflammation and insulin resistance in LDLr-/- mice, thus preventing the incidence of neointimal lesions.

Pharmacology

Endothelial cell (EC) activation is a change of the endothelium from a quiescent state to one that is involved in immune reactions. Activation of ECs is associated with the inception of atherosclerosis. Atherosclerosis is a chronic inflammatory disease that involves adaptive and innate immunity. There are many pro-inflammatory stimuli which activate the endothelium. The pro-inflammatory cytokine interleukin-17 (IL-17) has been shown to activate lung microvascular ECs. Enhanced expression of the IL-17 receptor by synovial ECs is associated with rheumatoid arthritis. These studies suggest that IL-17 plays an important role in EC biology. Nevertheless, the role of IL-17 in EC activation and endothelial dysfunction in the context of hyperlipidemia-induced atherosclerosis has not been studied. In the current study, we investigated the role of IL-17 in EC activation in vitro with mouse aortic ECs and human aortic ECs. In addition, we used the IL-17/ApoE double knock-out mouse to determine the role of IL-17 in vessel function and atherosclerosis development. First, we found that hyperlipidemia increased the number of IL-17-producing cells in the spleens from wild type mice and ApoE-/- mice that were fed a Western diet when compared to their respective normal chow diet controls. We also found that after treatment with the pro-atherogenic factor, oxidized LDL, there was an increase in the expression of IL-17 receptor by ECs. Using an EC specific array, we found that IL-17 induced significant up-regulation of four genes that are associated with EC activation in mouse aortic ECs. The four genes induced in IL-17-treated mouse aortic ECs were Cxcl1, Cxcl2, Il6, and Csf2. Moreover, we also found that IL-17 induced these four genes in human aortic ECs, and we showed that enhanced monocyte adhesion to ECs was dependent on these four genes. It was previously observed that a Western diet induced vessel dysfunction in the aortas of ApoE-/- mice. Thus, we sought to determine whether IL-17-deficiency rescues impaired endothelium-dependent relaxation in ApoE-/- mice that were fed a Western diet with the Wire Myograph System. We found that ApoE-/- mice on a 3-week Western diet had impaired endothelium-dependent relaxation when compared to IL-17-/-ApoE-/- mice. Endothelium-independent relaxation in response to sodium nitroprusside (SNP) and contraction responses induced by potassium chloride (KCl) and phenylephrine (PE) were not different in ApoE-/- mice and IL-17-/-ApoE-/- mice. Since our in vitro studies and vessel function assay pointed to a pro-atherogenic role for IL-17, we investigated lesion formation in ApoE-/- mice and IL-17-/-ApoE-/- mice. Lesion formation was assessed with Sudan IV staining of the whole aorta and Oil red O staining of aortic sinus cross sections. IL-17 deficiency in ApoE-/- mice did not affect atherosclerotic lesion formation in our study. THE INCREASE OF BLOOD LIPID LEVELS IS ONE OF THE MAIN FACTORS IN PRODUCING ATHEROMATOSIS. THE PURPOSE OF THIS STUDY WAS TO INVESTIGATE WHETHER THE PRESURGICAL STRESS PRODUCES CHANGES OF BLOOD LIPID LEVELS. WE STUDIED 18 MALE PATIENTSAGED 30-50 YEARS WHO WERE SUBMITTED TO SURGERY OF MODERATE SEVERITIES. IN ALLPATIENTS BLOOD SAMPLES WERE TAKEN THE FOLLOWING DAY AFTER ADMISSION, IN THE MORNING OF THE OPERATION AND IMMEDIATELY BEFORE ANAESTHESIA. WE MEASURED THE FOLLOWING PARAMETERS: 1) TOTAL CHOLESTEROL, 2) TOTAL LIPIDS, 3) TRIGLYCERIDES, 4) HDL, 5) LDL, 6) APO-A1, 7) APO-B, 8) CORTIZOL, 9) INSULIN, 10) GROWTH HORMONE, 11) PROLACTINE, 12) OESTRADIOL. FROM THE HORMONES THAT WERE MEASURED ONLY CORTIZOL SHOWED INCREASE (P < 0,05). ALL THE LIPID FACTORS SHOWED SIGNIFICANT GRADUAL INCREASE. FROM THESE RESULTS WE CONCLUDE THAT FACTORS WHICH CAUSE ANXIETY AND STRESS HAVE BEEN THE CAUSE OF THE INCREASE OF BLOOD LIPIDS. THE EVALUATION OF THESE FINDINGS MUST BE DONE IN COMPARISON WITH THE INCREASED FREQUENCYAND DURATION OF STRESS CAUSING FACTORS IN THE POPULATION OF CIVILIZED COUNTRIES.

▼ ENGLISH ABSTRACT: Coronary heart disease (CHD), common among South African whites, is increasingly manifesting itself in the coloured population of South Africa. More than 70% of coloureds participating in the CRISIC I study, a study undertaken to investigate CHD risk factors in the coloureds of the Cape Peninsula, had serum cholesterol levels imparting CHD risk. Increased levels of plasma low-density lipoprotein cholesterol (LDL-C) are the hallmark of the disease familial hypercholesterolemia (FH). The underlying molecular defect of FH consists of mutations in the low-density lipoprotein (LDL) receptor gene. The coloured study population was screened for the presence of the three founder related point mutations in the LDL receptor gene that account for more than 90% of FH cases in the Afrikaner population of South Africa. The prevalence of the FH Afrikaner mutations was determined in 31 FH heterozygotes and 39 hyperlipidemic coloureds. The 39 hyperlipidemic coloureds did not meet the criteria for heterozygous FH. The FH Afrikaner-1 (Asp₂₀₆→Glu , FH-1) en FH-Afrikaner-2 (Val₄₀₈→Met FH-2) mutations had the same prevalence, 12.9% (4 individuals), in the FH group. The FH Afrikaner-3 (Asp₁₅₄→Asn, FH-3) mutation was absent in the FH group. The three FH Afrikaner mutations were not detected in the hyperlipidemic group of 39 individuals. Haplotype analysis in the families with the FH-1 mutation showed that the chromosomal background of the mutation was compatible with that described in the Afrikaner patients. In the FH-2 heterozygous patients the mutation was found on a haplotype that differed from the Afrikaner haplotype at polymorphic sites at both sides of the defect in exon 9. These results indicate that identical LDL-receptor gene mutations originated in two different South African population groups due to independent events at a potential CpG dinucleotide "hotspot". The clinical effects of the FH-1 and FH-2 mutations were the same as described in the Afrikaners. Heteroduplex analysis detected a 3 bp deletion that causes deletion of amino acid Gly₁₉₇ (FH Lithuania) in one of 23 FH heterozygous coloured patients (4.3 %) and in one indiviaual of 90 hyperlipidemic coloureds (1.1%) already screened for the FH Afrikaner mutations , by means of heteroduplex analysis. A hyperlipidemic group of 66 coloureds, screened for the first time for mutations in exon 4 of the LDL-receptor gene by means of heteroduplex analysis, showed one individual (1.5%) with the FH Lithuania mutation. In all cases, the haplotype of the mutant allele was compatible with that described in Ashkenazi Jews. The heteroduplex screening method was sensitive enough to detect the FH-1 mutation, as 5 individuals of the group of 66 hyperlipidemic coloureds were found to have the FH-1 mutation. Atherosclerosis is a major factor influencing morbidity and mortality worldwide. The pathogenesis of atherosclerosis has been extensively investigated however treatment modalities have not changed much over the past decade. Prevention of atherosclerosis and its complications, both primary and secondary, are based mainly on controlling the various cardiovascular risk factors. Treating combined hyperlipidemia, and in particular reducing LDL-cholesterol and triglyceride levels, is established as a highly efficacious means of reducing both morbidity and mortality from cardiovascular disease. With the increased emphasis on various lipoprotein sub fractions, many patients need to consider combining treatments to achieve recommendations. Although statins can be an effective treatment for hyperlipidemia, they may not be sufficient to achieve the recommended LDL-cholesterol and triglyceride goals as set out by national governing bodies. This thesis examines the lipid, inflammatory and cardiovascular esponse to concomitant supplementation with phytosterols and omega-3 fatty acids in combined hyperlipidemia. Phytosterols and omega-3 fatty acids are functional ingredients with potential cardiovascular benefits. Phytosterols inhibit cholesterol absorption, thereby reducing total-cholesterol and LDL-cholesterol. The consumption of 1.5-2.0g/day of phytosterols can result in a 10-15% reduction in LDL-cholesterol within a three week period, in hyperlipidemic populations. The added benefit of phytosterol supplementation has been demonstrated in individuals already taking statin medications. Omega-3 fatty acid supplementation has strong hypotriglyceridemic properties, and provides benefits in other risk factors associated with cardiovascular disease, such as anti-thrombotic and antiinflammatory function. Given the propensity of hyperlipidemia to manifest in high risk individuals and populations alike, there is a plausible role for combining phytosterols and omega-3 fatty acids supplementation. A series of clinical trials were undertaken to explore the plasma lipid, inflammatory and overall cardiovascular response to combined supplementation with phytosterols and omega-3 fatty acids rich in either EPA or DHA. This particular dietary combination of functional ingredients was designed to optimise improvements in plasma lipid profile in individuals with combined hyperlipidemia. Findings from this thesis show that the combined supplementation of phytosterols and various omega-3 fatty acids reduces total-cholesterol, LDL-cholesterol, triglycerides and increases HDL-cholesterol, greater than the supplementation of either function ingredient alone. Furthermore, a number of circulating inflammatory mediators were analysed showing significant reductions in response to the combined dietary treatment. ▼ Diplomamunkámban a hypertonia és dyslipidemia/hyperlipidemia kognitív funkciókra kifejtett hatásait vizsgáltam néhány tanulmány alapján. A kognícióhoz tartozik a megismerés, megértés, memória és a kivitelezés. A kognitív funkciók romlása lényegében a mentális státusz hanyatlását jelenti, mely különböző súlyosságban fordulhat elő. A kogníció csökkenése időskorban természetes jelenség, azonban egy súlyossági szintet elérve már az egyén mindennapjait is zavaró deficitként van jelen, mely demenciában is végződhet. Az enyhe kognitív zavar (MCI) a demencia preklinikai fázisa, az esetek megközelítőleg 10%-ban Alzheimer demenciába torkollik. A mentális funkciókért felelős agyi régiók, magvak organikus elváltozásai, illetve keringési rendellenességei vezethetnek a deficit kialakulásához. Ezek alapján felvetődött, hogy a cardiovascularis rizikótényezők (például a hypertonia és hyperlipidemia) hatással lehetnek a mentális státuszra. A pathomechanizmus alapját a hypertonia talaján kialakuló endothel károsodás, az apró thrombusok, továbbá a hyperlipidemiaval együtt az atherosclerosis adja. Az érszűkítő folyamat részleges, teljes illetve fluktuáló elzáródást okozhat. A microinfarktusok kialakulása nem mindig okoz klinikai tüneteket, azonban egy idő és bizonyos mennyiségű infarktus után a kognitív deficit kialakulhat - ha a kognícióért felelős területek is érintettek, például a nucleus basalis, nucleus caudatus, thalamus, limbikus rendszer, cortex stb. Először a magasvérnyomás hatásait vizsgáltam meg. Az elolvasott cikkek szinte kivétel nélkül kapcsolatot találtak az említett tényező és a mentális státusz között. Nincs kritikus határérték, mely fölött a kognitív deficit kialakul, de a kutatások alapján úgy tűnik, hogy a magasvérnyomás hosszú idejű fennállása vezet a mentális hanyatláshoz. Az antihipertenzív készítmények mentális funkciókra kifejtett hatását is áttekintettem. A jelenlegi hypertonia kezelési protokoll szerint elsősorban angiotenzin konvertáló enzim gátlók adása illetve kombinálása kalcium-csatorna gátlókkal vagy diuretikumokkal a leggyakrabban alkalmazott kezelési stratégia. Az ACE-gátlókat angiotenzin receptor blokkolókkal is helyettesíteni lehet, a β-blokkolók adása is szóba jön. Az ACE-gátlót, kalcium-csatorna blokkolót, illetve ARB-t szedők körében a kognitív teljesítmény jobb volt a kontroll csoportokhoz képest, a kognitív deficit kialakulása is kisebb arányban fordult elő. Feltehető, hogy e készítmények közvetetten protektív hatásúak a kognitív funkciókra, a vérnyomás csökkentése és kedvezőbb hemodinamikai viszonyok teremtése által. Ugyanakkor, a diuretikumokat illetve béta blokkolókat szedők körében a kognícióra való pozitív hatást nem tudták kimutatni. A diplomamunkám második felében a dyslipidemia hatásait részleteztem. A legfontosabb szövődmény az atherosclerosis kialakulása. Az atherosclerosis érszűkítő hatása a vérellátás romlásához vezet. ▼ BACKGROUND: Homeless individuals have higher rates of cardiovascular disease when compared to the general population. Educational curricula in such communities have the potential to decrease the morbidity and mortality associated with hypertension and hypercholesterolemia. However, lifestyle changes from these educational sessions can be difficult for homeless individuals to implement because of competing priorities. OBJECTIVES: The objectives of the “Corazones Sanos” community project are to increase participant knowledge of hypertension and hypercholesterolemia and to counsel individuals on barriers to implementation of therapeutic lifestyle changes in the homeless population. Additional objectives include providing reinforcing support for implementation of lifestyle changes and creation of educational materials related to cardiovascular risk and lifestyle changes for the homeless population. METHODS: This community health education project was implemented with 66 patients from an urban transitional housing facility for homeless individuals recovering from substance abuse. Spanish-speaking patients >18 years old with unstable or lack of housing were eligible to participate in this study. Fifteen minute educational sessions were led by the project coordinator with the end knowledge goal of normal blood pressure and cholesterol values, daily exercise requirement, importance of medication compliance, and healthy diet practices. A reflective approach was utilized to assess participant quotations and themes of barriers to heart healthy living in the homeless population. RESULTS: Participants gained a better knowledge of cardiovascular disease risk factors after participating in education sessions. The main barriers to therapeutic lifestyle changes for prevention of cardiovascular disease included decreased access to healthy foods and fitness opportunities, mental health challenges, lack of social support, and recovering from addiction. About 30% of participants completed a follow-up individual education session. CONCLUSIONS: Casa Esperanza community members gained a better understanding of normal blood pressure and cholesterol ranges, healthy dietary practices, daily exercise requirement, and the importance of medication compliance through this education program. The findings from this project should assist healthcare clinicians with providing effective education for the homeless population to overcome the barriers identified and implement therapeutic lifestyle changes.

Hyperlipidemia [high lipids plasma levels such as triglycerides(TG), totalcholesterol (TC) and low-density lipoprotein cholesterol (LDL)] may be associated with parotid enlargement and fatty degeneration of parotid parenchyma.The aim of this study was to investigate the role of experimental hyperlipidaemia and ischemia in the parotid gland’s microstructure in Wistarrats and the influence of statin treatment. Forty Wistar rats (111.06±3.36g) were divided into 5 groups (A1, A2controls-B1, B2 experimental). Control animals were fed with normal cereal rodents pellets while the experimental groups took an hyperlipidaemic mixture(50% cereals, 20% butter, 10,5% sucrose, 10% casein, 2,5% cholesterol,1% vitamins, 0,15% propylhiouracil, 0,1% choline). The animals of group A2and B2 were treated with simvastatin (Zocor) 40mg/kg/daily p.o. for 3months. On the other hand, the rats of group B3 were treated with hyperlipidemic diet for 3 months and afterwards were given Zocor40mg/kg/daily p.o. for the following 3 months and normal cereal rodents pellets. In the sixth group (group C) experimental ischemia was performed and after one month observation the rats were sacrifed in order to perform a histological examination of the parotid gland. Statistical analysis was performed by Kruskal-Wallis Test and Mann-Whitney Test. Total serum cholosterol (TC), triglyceridia (TG), high density lipoproteins (HDL) and low density lipoproteins (LDL) were increased ingroups B1, B2 while the parotid weight was decreased. The histological investigation demonstrated changes in the parotid gland morphology of the hyperlipidaemic groups, including the presence of chronic inflammation, fibrosis, lipocytes, foci of lymphocytic infiltration, nucleiabnormalities, diversity of vessel wall thickness. The influence of statin tendedto potentate the chronic inflammatory infiltrate while the lipocytes were decreased and remodelling of the parotid gland’s structure occurred. On the other hand, minor alteretions of the parotid gland of rats in group C were demonstrated. Conclusively, hyperlipidaemic diet lead to an elevated serum lipid profile that may be responsible for parotid gland swelling with lipid infiltration, nuclei abnormalities, lymphocytic infiltration, fibrosis and parotid duct obstruction while statin’s treatment seemed to repair some of these injuries.

U okviru disertacije je ispitan hemijski sastav odabranih cijanobakterijskih sojeva roda Anabaena, Nostoc i Spirulina, kao i uticaj azota na sastav rodova Anabaena i Nostoc. U ispitivanim sojevima određen je sadržaj lipida, proteina, mineralnih materija, sastav i sadržaj lakoisparljivih jedinjenja, aminokiselina i masnih kiselina. U heksanskim i metanolno/vodenim ekstraktima sojeva određen je sadržaj ukupnih fenola, sastav i sadržaj karotenoida i proteinskih pigmenata, kao i antioksidativni potencijal ekstrakata prema DPPH radikalu. Takođe, ispitan je antihiperlipidemijski potencijal roda Spirulina S2 u biološkom ogledu, pri čemu je određen nivo ukupnog holesterola, LDL i HDL čestica, izračunati su relevantni biohemijski markeri hiperlipidemije, kao i koncentracija ALS i AST u plazmi eksperimentalnih životinja. Određen je hemijski sastav fecesa eksperimentalnih životinja, kao i sadržaj holesterola i sastav i sadržaj žučnih kiselina u fecesu. Na bazi hemijskog sastava ispitivanih sojeva cijanobakterija: visokog sadržaja proteina, izbalansiranog aminokiselinskog i masnokiselinskog sastava i visokog sadržaja karotenoida, može se zaključiti da oni imaju potencijal kao sirovine za unapređenje nutritivnog sastava prehrambenih proizvoda. Dokazana antihiperlipidemijska aktivnost soja S2 dodatno ga čini atraktivnim za razvoj proizvoda sa dodatom vrednošću namenjenih osobama sa poremećajem u metabolizmu lipida. Rezultati naših ispitivanja su pokazali i da se na hemijskih sastav pojedinih sojeva može uticati putem promene sastave hranljive podloge što svakako ostavlja prostora za buduća istraživanja.

In this thesis, the chemical composition of the selected strains of cyanobacteria belonging to Anabaena, Nostoc and Spirulina genus was determined. Influence of nitrogen addition on the chemical composition of Anabaena and Nostoc strains has also been investigated. Content of lipids, crude protein, and minerals, as well as composition and content of volatile organic compounds, amino acids, fatty acids was determined in the investigated strains. Total phenolic content and composition and content of carotenoids, protein pigments and antioxidative action toward DPPH radical was determined in the hexane and methanol/water extracts of the investigated strains. Antihyperlipidemic potential of the Spirulina S2 strain was investigated in a biological assay, and content of total cholesterol, LDL and HDL was determined in plasma of the experimental animals, which allowed calculation of relevant biochemical parameters for hyperlipidemia. ALT and AST concentrations were also determined in the plasma of the experimental animals. Chemical composition, as well as content of cholesterol and composition and content of bile acids in the feces of the experimental animals was also determined. ▼ The relationship between cerebrovascular disease (CVD) risk factors and cognitive impairment or dementia has been widely studied with significant variability in findings between groups. We hypothesized that chronic small vessel injury in the form of arteriolar sclerosis, measured quantitatively using MRI to measure total white matter hyperintensity (WMH) volumes, would identify specific association of CVD risk factors and patterns of cognitive decline, associated with mild cognitive impairment of the cerebrovascular type, that represent the core features of vascular cognitive impairment in our cohort. A Cross-sectional analysis of clinical and quantitative MRI data on 114 subjects with normal cognitive function (n=52) and mild cognitive impairment (MCI; n=62) was performed. Quantitative total WMH volumes were examined in relation to potentially causative CVD risk factors and resultant test scores across cognitive domains using linear regression models adjusted for age, gender, and education. Among CVD risk factors analyzed, age (p< 0.001), education (p= 0.003), hypertension (p= 0.012), and hyperlipidemia (p= 0.008) demonstrated the strongest associations with WMH volumes. Conversely, diabetes, smoking, history of heart attacks, atrial fibrillation, and history of stroke that have shown associations with CVD pathology on imaging in other studies were not statistically associated with increased WMH in this cohort. WMH volumes were associated with decrease performance on the Trial Making Test type A & B and long delayed free recall on the California Verbal Learning Test. Our findings suggest similarities and yet differences in comparison to other studies. Hypertension and hyperlipidemia appear to represent common shared risks across geographically disparate groups. Our findings, like others, suggest CVD pathology impact processing speed and executive function and provide further evidence for CVD effects on short-term memory in those at risk for cognitive decline and the future development of dementia in our cohort.

A hiperlipemia causa sérias complicações aos equinos hospitalizados, principalmente quando associada ao estresse, doenças e traumas. Tratamentos convencionais com soluções de heparina, glicose a 5%, insulina, entre outras, têm sido utilizados em equídeos predispostos, mas são questionados quanto ao seu real efeito. O interesse e as pesquisas em relação à utilização da nutrição clínica no equino, em especial a nutrição parenteral, vêm crescendo. Sua ação visa evitar a doença e o catabolismo através da regulação do balanço energético negativo, inclusive servindo de adjuvante às terapias já implementadas no tratamento da doença primária, fornecendo o requerimento básico nutricional para a manutenção e recuperação do organismo. Para avaliar a eficácia das soluções de nutrição parenteral em equinos que desenvolveram hiperlipidemia durante a internação, foram avaliados 14 equinos atendidos com afecções diversas e que apresentaram triglicérides séricos (TG) acima de 150mg/dl. Os mesmo foram divididos em dois grupos: grupo controle (G1), sem interferência do manejo e grupo tratamento (G2), que recebeu nutrição parenteral parcial sem lipídeos. Tais soluções eram compostas de glicose a 50%, aminoácido a 10%, oligoelementos, complexo vitamínico e eletrólitos e foram infundidas até a resolução da hiperlipidemia. Os animais dos dois grupos foram monitorados diariamente em relação aos valores de glicemia, triglicérides e colesterol. A média do tempo de redução dos TG no G1 foi de 209,2 ± 131,9 horas e no G2 foi de 34,9 ± 41,8 horas, com diferença estatística entre eles. Os valores de colesterol não acompanharam a elevação dos TG. Em relação à perda de peso e escore corporal, os dois grupos apresentaram redução durante a internação, sem diferença estatística entre eles. Nenhum animal do G2 desenvolveu hiperlipemia, ao contrário do G1, em que cinco equinos apresentaram, em algum momento da internação, triglicérides acima de 500mg/dl. Embora outros critérios devam ser utilizados na escolha dos pacientes que necessitam deste tipo de tratamento devido principalmente aos custos e necessidade de constante monitorização, conclui-se que a nutrição parenteral é um método rápido e seguro para a prevenção da hiperlipemia em equinos com hiperlipidemia durante a internação por outras enfermidades.

▼ The influence of hyperlipidemia on the pharmacodynamic and pharmacokinetic aspects of lipophilic drugs was explored. The antiarrhythmic, amiodarone, and the antifungal, (±)-ketoconazole, were used as model drugs. Experimental hyperlipidemia was induced in rat using poloxamer 407 and two sensitive novel HPLC assays were developed. In a multiple dosing study, hyperlipidemia increased amiodarone plasma concentrations, heart concentrations and electrocardiographic changes. The amiodarone heart uptake could not be totally attributed to its unbound fraction, where the cardiac very low density lipoprotein receptors seemed to play a role in the uptake of bound drug. Amiodarone liver metabolism in presence and absence of hyperlipidemia was studied using isolated primary rat liver hepatocytes. The metabolism of amiodarone was lower in hepatocytes isolated from hyperlipidemic than those from normolipidemic rats. Hyperlipidemic serum resulted in a decrease in amiodarone metabolism and when coincubated, the expected decrease in unbound fraction seemed to resulted in greater inhibition of metabolism. (±)-Ketoconazole showed stereoselectivity in its pharmacokinetics in rat with (+)-ketoconazole showing higher plasma concentrations than its antipode. This was attributed to its higher protein binding. There was no difference in the total bioavailability of the two enantiomers. Ketoconazole enantiomers exhibited nonlinear pharmacokinetics. In normolipidemic rat plasma ketoconazole enantiomers were more than 95% bound to lipoprotein deficient fraction. Hyperlipidemia resulted in shifting both enantiomers 20% to very low density and low density lipoprotein fractions. In a pharmacokinetic assessment, hyperlipidemia was found to increase ketoconazole enantiomer volume of distribution. Moreover, the stereoselectivity ratios of most pharmacokinetic parameters were changed. After oral dosing, the uptake of (-)-ketoconazole was significantly decreased. Since ketoconazole is used as a potent CYP3A inhibitor, alteration in liver concentrations of (-)-ketoconazole, the more potent inhibitory enantiomer, could decrease its CYP inhibitory potential. Hyperlipidemia potentiated the CYP-mediated interaction between ketoconazole and midazolam with significantly higher midazolam AUC and lower clearance. This was attributed to the inhibitory action of ketoconazole and the effect of hyperlipidemia on the binding of midazolam. Hyperlipidemia was found to unexpectedly decrease midazolam unbound fraction in plasma. In conclusion, the findings could explain some unexpected dose versus effect outcomes in hyperlipidemic patients receiving amiodarone or ketoconazole.

▼ The aim of this PhD work was the characterization of the intestinal microbial ecosystem through bioinformatic and statistical analyses of the microbiomics data originated from three studies carried out on different human populations: healthy (non-diseased) adults, hyperlipidemic and normo-lipidemic children and adolescents, and subjects with diagnosed irritable bowel syndrome (IBS). Specifically, the three studies presented in this PhD theses were as follows: • Probiotic crossover intervention study: A randomized, double-blind, crossover, placebo-controlled intervention study was carried out to determine the impact of a Bifidobacterium bifidum strain on the IME of adult healthy volunteers. • Children’s dyslipidemia single arm intervention study: In this trial, the IME of children and adolescents with primary hyperlipidemia (a risk factor for cardiovascular diseases) was compared with the IME of control (normo-lipidemic) group. In addition, the modulatory effect of the regular intake of hazelnuts on hyperlipidemic subjects’s IME was examines in order to evaluate if the modulation of IME by hazelnuts ameliorates the lipid profile. • Irritable bowel syndrome (IBS) observational study: The aim of this trial was the characterization of the IME in human subjects affected by IBS. The characterization was performed through the analysis of the bacterial communities present in the faecal samples collected during a multicentre intervention trial (Cremon et al., 2017). Irritable bowel syndrome is the most common gastrointestinal disorder in western countries. IBS is conventionally classified in four different subtypes based on bowel clinical symptoms: constipation, diarrhoea, alternating constipation and diarrhoea, and unsubtyped IBS. Clinical and immunological data collected during the trial were used to investigate the potential correlations between the IME characteristic for a specific IBS subtypes and the physiological and clinical parameters of the host, including bowel symptoms, faecal levels of IgA and cytokines, and depression/anxiety scores. The analysis of the data collected during the three studies presented in this PhD thesis revealed significant correlations between the intestinal microbial ecosystem and specific physiological parameters, characteristic for the physiological conditions under study. Therefore, these findings suggest that the differential abundance of specific OTUs could be used as a biomarker for a specific host condition. Conclusion of the probiotic B. bifidum Bb crossover intervention trial The intervention trial based on the administration of B. bifidum strain Bb has shown that the probiotic treatment modified the relative abundances of bacterial taxa that have often been associated with healthy conditions. The treatment modulated the faecal levels of butyrate, a microbial metabolite exerting multiple effects on gut health.
