ENGLISH ABSTRACT: This study was designed to retrospectively evaluate the established embryo vitrification/warming programme currently implemented at Drs Aevitas Institute for Reproductive Medicine and to look at factors that might play a role in optimizing the pregnancy outcomes thereof. Vitrification is the achievement of a “state of suspended animation” wherein molecular translational motions are arrested without structural reorganization of the liquid. In embryo vitrification it involves placement of the embryo in a very small volume of vitrification medium that must be cooled at extremely high cooling rates. The vitrification medium contains cryoprotectants to prevent any cryoinjury from occurring to the embryo. This process was initially proposed to effectively manage supernumerary embryos, but it has also provided a viable method of reducing costs for additional embryo transfers as well as the reduction of the incidence of multiple births. Patients who are at risk of ovarian hyper stimulation syndrome (OHSS) can also have all of their embryos vitrified in advance to reduce the likelihood of adverse clinical symptoms if a pregnancy is established. Throughout the period in which vitrification has been in practice, there have been advances in technology as well as continual research being conducted to establish whether newly suggested techniques do, in fact, optimize the outcomes of vitrification. Focus has subsequently been applied to the carrier device used for vitrification, the day on which the embryos are vitrified and stored, as well as the number of embryos transferred in each respective cycle, all to ensure the most favourable outcome. This retrospective study confirmed the use of the Cryotop® as the most viable carrier device for successful survival and pregnancy outcomes. Transfer of day 5 vitrified embryos resulted in significantly higher pregnancy rates compared to day 6 vitrified embryos. Results also indicated that the number of embryos transferred does indeed have a significant effect on the pregnancy outcome and consequently we can possibly argue against the implementation of single embryo transfer in the vitrification programme. Investigation into the effect of female age, specifically oocyte age, on each of these categories indicated that reduced age can be associated with optimal outcomes; however this could not be proven statistically in this cohort of patients. To further look at optimization of the vitrification/warming programme, a Literature Survey was conducted to ascertain the results after Assisted Hatching in frozen/warmed human embryos. Assisted Hatching has been proposed as a solution to Zona Pellucida hardening, which has been found to occur during vitrification. The need for further studies and a meta-analysis of the literature is confidently proposed, as well as a Prospective Study to evaluate the effect of Laser Assisted Hatching in the human blastocyst vitrification/warming programme at Drs Aevitas Institute for Reproductive Medicine.

▼ The aim of this research was to evaluate the effectiveness of Kazinva Rural Health Center, in the provision of Reproductive Health information to the women of Chikondano settlement in Lusaka west. Its objectives were to determine women's knowledge on reproductive health, to establish women's awareness of the existence of reproductive health information at Kazinva, to find out the different methods being used by the Clinic to disseminate reproductive health information, to investigate the attitude of women towards reproductive health information and also to establish the factors leading to the reproductive health problems in Chikondano settlement. The research methodology used is a non experimental design, which is a methodology used when a research is carried out in an uncontrolled and natural setting. The study was descriptive, meaning it involved a systematic collection and presentation of data to give a clear picture of a particular situation. The sampling method used in selecting the study sample or the respondents was the Simple Random Sampling method and the sample size of 40 households was randomly picked from the total population of about 400 households in the settlement. An interview method type of data collection was used for data collection, since the population in consideration is semi or illiterate and due to the fact that the topic under study is very sensitive.The key findings of this research were that most of the women do not know what reproductive health is all about. It was also discovered that despite the fact that most of these women knows that Kazinva health center provides reproductive health information a good number of them do not use it to access RHI. According to the findings, most of these women use family and friends as well as the media and press as their source of reproductive health information. The study revealed that drama groups and peer educators are the most effective media in the dissemination of reproductive health information but unfortunately clinic talks were discovered to be the most used mode of RHI dissemination. It was also found out that these talks takes long and that most of the women felt that the information being provided by the center is not adequate. From these findings it was concluded that Kazinva health center is not effective in their provision of reproductive health information to the people of Chikondano and there is need for them to improve, thus it was recommended that the provision of reproductive health information be extended to males so that they can be at par with females when making reproductive health decisions, that a bigger structure be built to be able to meet the demand. The presenters of health talks should make their presentation short and interesting, should increase the number of peer educators so that they can reach out to every corner of the community, the Government of Zambia should employ more health workers at Kazinva health center so that the provision of information can be effective by alternating the health workers. ▼ ENGLISH ABSTRACT: Animal welfare has recently gained significant attention in commercial livestock industries worldwide. Specifically, several studies involving husbandry practices with positive human-animal interactions have shown a favourable link between improved animal welfare and production. However, limited research is currently available on optimal husbandry practices for the ostrich industry, which is still plagued by low fertility, high embryo and chick mortality, as well as variable growth rates. The poor ostrich production performance observed could thus reflect the difficulties of the birds to adapt to the commercial farming systems, and/or a failure of commercial practices to provide the basic requirements for this newly domesticated species. Hence, this study examined the effect of different husbandry practices varying in the intensity of human presence and interactions with ostrich chicks from day-old to 3 months of age on: weight gain, survival, immune competence, short- and long-term stress responses, social behaviour, docility, fear responses, meat quality and skin damage. The reproduction performance when the birds reached sexual maturity also was recorded. The study showed that exposure to additional human contact (as compared to standard husbandry practices where human contact was limited to the provision of food and water), resulted in improved early growth, survival and immune competence, and even more so when chicks were exposed to gentle physical handling interactions. Furthermore, chicks exposed to such human presence and interactions expressed lower short-term stress responses when exposed to a potentially stressful event (i.e. feather harvesting and feather clipping), and lower long-term stress responses (as measured in corticosterone levels in the floss feathers), compared to chicks exposed to limited human presence. These results suggest an improved ability of the former to adapt to routine ostrich farm management practices. Furthermore, while social behaviour, docility and fear responses to humans when the birds reached the juvenile stage did not vary between the different husbandry practices, the birds habituated to human presence at an early age were more likely to associate with a familiar rather than an unfamiliar human. This suggests that ostriches can not only discriminate between people, but also adjust their behaviour accordingly. Husbandry practices did not affect meat pH, meat colour, carcass attributes or skin damage. Females that received additional human presence along with physical gentle handling produced a higher number of eggs in their first year of reproduction as 2-year-olds, compared to females that were exposed to limited human presence. The difference in egg production was not observed during the second year of breeding. Hence, additional human presence combined with gentle physical handling seemed to improve early egg production and adaptation of females to the breeding environment. Over the course of the 1980s and the 1990s, West Germany and then Germany developed the most restrictive law in the advanced industrialized world to regulate reproductive and genetic technologies. Under the Embryo Protection Act of 1990. the government criminalized egg and embryo donations, sex selection, alterations to the human germ line, and any use of reproductive technologies leading to "divided motherhood." In the early 2000s, after intense public controversy, the German parliament decided to maintain a ban on preimplantation genetic diagnosis (PGD). My dissertation asks why Germans were so concerned about these technologies. To the extent that this question has been posed, scholars have mainly attributed German anxieties to the specter of the Nazi past. But although the Nazi past certainly mattered, this dissertation argues that Germans worried as much if not more about the pathologies of liberalism, especially the development of neoliberalism from the 1970s onward. Critiques of reproduction were therefore a locus for broad political and economic critiques in late-twentieth-century Germany. This dissertation explicates those critiques by tracing the changing meanings of two concepts in particular, human dignity and eugenics. These concepts structured debates on reproduction but were also constituted by them. Analyzing the concepts of human dignity and eugenics shows that in the 1980s German critics feared the technologization of reproduction, through which scientists and doctors would pursue a perfectionist eugenics. In so doing, such actors would supersede God's role in creation and destroy the Christian subject, in particular one's ability to grow out of imperfection toward a closer union with God. By the end of the century, critics grew suspicious of individual decisionmaking. They came to fear the marketization of reproduction - namely that the logic of the market dictated the everyday reproductive decisionmaking of individual parents. Critics now reconceptualized both dignity and eugenics. They invoked dignity in a Kantian more than Christian register, and for the first time they spoke of eugenics as inherently unacceptable, whether orchestrated from above or arising from individual choices below.

Les infections à Papillomavirus Humain (HPV) sont des infections sexuellement transmissibles très fréquentes. La persistance de ces infections est un facteur causal du cancer du col de l’utérus et est aussi à l’origine d’autres cancers de la zone ano-génitale et de verrues génitales chez les femmes et chez les hommes. Depuis l’introduction de deux vaccins bivalent et quadrivalent permettant de prévenir certains types d’HPV, de nombreux modèles mathématiques ont été développés afin d’estimer l’impact potentiel de différentes stratégies de vaccination. L’objectif de ce travail de thèse a été d’estimer l’impact potentiel de la vaccination en France sur l’incidence de certains cancers liés à l’HPV, notamment le cancer du col de l’utérus et le cancer anal chez les femmes françaises ; ainsi que sur la prévalence des infections à HPV 6/11/16/18. Différents modèles dynamiques de type déterministe ont été développés. Ils sont représentés par des systèmes d’équations différentielles ordinaires. Une étude théorique du comportement asymptotique d’un premier modèle comportant peu de strates a été réalisée. Le nombre de reproduction de base R0 et le nombre de reproduction avec vaccination Rv ont été estimés. Des modèles plus complexes ont intégré une structure d’âge et de comportement sexuel. Les modélisations réalisées permettent de conclure à l’impact important de la vaccination sur la prévalence des infections à HPV et sur l’incidence des cancers du col de l’utérus et de la zone anale chez les femmes françaises dans un délai de quelques décennies, si l’on prend en compte les taux de vaccination observés en France au début de la campagne de vaccination

Human Papillomavirus infection (HPV) is the most frequent sexually transmitted disease. Epidemiological studies have established a causal relationship between HPV infections and occurence of cervical cancer. These infections have also been incriminated in anogenital cancers and anogenital warts among women and men. Since the introduction of bivalent and quadrivalent vaccines which offer protection against some HPV genotypes, many mathematical models have been developed in order to assess the potential impact of vaccine strategies. The aim of this thesis work was to assess the potential impact of HPV vaccination in France on the incidence of some cancers linked with HPV, particularly cervical cancer and anal cancer in French women, and on the prevalence of HPV 6/11/16/18 infections. Different deterministic dynamic models have been developped. They are represented by systems of ordinary differential equations. A theoretical analysis of the asymptotic behavior for a first model with few strata is realized. The basic reproduction number R0 and the vaccinated reproduction number Rv are assessed. More complex models taking into account age and sexual behavior have been developed. Using vaccination rates observed in France at the launch of the vaccination campaign, our modeling shows the large impact of vaccination on HPV prevalences, on cervical cancer and anal cancer incidences…

▼ This study was conducted in order to understand Grade 10 Life Science teachers’ perceptions and experiences of teaching human reproduction topics in the Ohangwena political region in the northern part of Namibia. It also explored how the culture of the majority of the inhabitants of Ohangwena region who are Oshiwambo speaking impacts the teaching of human reproduction topics. The study was prompted by the fact that the Grade 10 Life Science syllabus requires learners to learn details of human reproduction although in Oshiwambo culture such things are not discussed openly. And this section has proven to be one of the poorly answered sections in the Grade 10 national examinations. A total of thirty five (35) Grade 10 Life Science teachers participated in this study. It is a qualitative case study underpinned by an interpretive paradigm. The unit of analysis for this study was teachers’ perceptions and experiences of teaching the topic of human reproduction. Data were generated through document analysis, questionnaires and tape recorded semi-structured interviews. Such data were analysed inductively by sorting them into categories and then into emerging themes. Categories included teachers’ views and experiences of being taught and teaching human reproduction, how culture influenced the manner in which teaching and learning of human reproduction takes place as stated in the socio-cultural theory, and how teachers of different gender perceived and experienced the teaching of human reproduction according to the feminist standpoint theory. The emerging themes were colour coded and then developed into analytical statements. Data triangulation, member checking, and follow up interviews ensured data validity and trustworthiness. This study revealed that the cultural belief of the Owambo people that prohibits adults to talk to children about things related to sex because it encourages them to practice sex is still strong among individuals in the community including teachers themselves. As a result, teachers are not comfortable with teaching human reproduction topics. From the findings, it is recommended that teachers should be empowered to teach Life Science, specifically human reproduction topics, with confidence. Furthermore, parents should be made aware of the importance of including human reproduction topics in the school curriculum so that they can motivate their children to study it with an open mind. A ideia que versa essa dissertação é do esplêndido que a Reprodução Humana Assistida (RHA), traz a possibilidade de fazer um filho em um casal isento de fertilidade. Diante do que se percebe em termos de avanços da tecnociência do crescente número de casais em situação de infertilidade o foco desta pesquisa é a Doação de embriões criopreservados. Mormente existam em outros países leis específicas que regulem a RHA, o Brasil dispõe apenas de uma Lei da engenharia genética - Lei de Biossegurança, n. 11.105, 24/03/2005, o que faz com que profissionais da área se deparem com diversos desafios éticos. Há de se ressaltar, que polêmicas quanto à valoração do embrião em toda a sociedade brasileira permeiam está seara, mais especificamente, a questão da pesquisa com célulastronco embrionárias. É ainda ressaltante dizer da escassez de pesquisas e publicações nesta área, em especial, no que diz respeito às implicações emocionais da Doação de embriões. Para tanto, os objetivos desta pesquisa perfazem a compreensão dos aspectos bioéticos envolvidos na problemática da Reprodução Humana Assistida, bem como o estudo dos aspectos éticos vividos pela Equipe Técnico-administrativa de uma Clínica de Reprodução Humana do Estado de São Paulo, responsável pelo tratamento de Reprodução Humana Assistida de casais que tiveram embriões excedentários após serem submetidos ao tratamento de Fertilização in vitro e a reflexão sobre a dimensão subjetiva vivida pela Equipe Técnico-administrativa de uma Clínica de Reprodução Humana do Estado de São Paulo, responsável pelo tratamento de Reprodução Humana Assistida de casais que tiveram embriões excedentários após serem submetidos ao tratamento de Fertilização in vitro. Sendo a metodologia utilizada neste estudo a abordagem qualitativa, descritiva, bibliográfica, documental, exploratória e de campo, tendo como método o hermenêutico-dialético e a pesquisa de campo com a Equipe Técnico-administrativa da Clínica supracitada. Ao que se chega ao final deste estudo nada mais são que (in)conclusões, pois por certo muitos questionamentos éticos se abrem ao se desvelar o universo pesquisado: a Doação de embriões. O vínculo estabelecido ente o casal com os embriões excedentes criopreservados permeia o processo de decisão do casal quanto ao destino destes, marcado por incertezas, temores e sentimentos de solidariedade, aliado a isto está o fato de que a contemporaneidade representa um modo de viver em que a fluidez de valores é ressaltada, a fragilidade e transitoriedade dos laços afetivos são preponderantes. É forçoso ainda dizer que a importância do pensamento foi apreciada durante todo o percurso e no desfecho deste estudo optou-se por enfatizar quão poderoso ele é. Haja vista que as proposições ou sugestões vislumbram, antes de mais nada, “o Pensar” no sentido da elaboração como saída para quem vivencia a angústia da tomada de Decisão do que fazer com os seus embriões.

O presente estudo teve como objetivo identificar as expectativas, os sentimentos, medos e desejos de mulheres em relação à maternidade, às Técnicas de Reprodução Humana Assistida e à infertilidade. Para tanto, foram analisados três blogs que versavam sobre o tema da infertilidade. Cada blog foi considerado como caso de análise, caracterizando um estudo de casos múltiplos, de abordagem qualitativa. As autoras tinham entre 26 e 29 anos de idade e estavam casadas há mais de um ano. Os achados foram discutidos a partir de dois eixos temáticos: desejo de gravidez e maternidade e sentimentos em relação à infertilidade e formas para lidar com essa condição e com o tratamento. Evidenciou-se o desejo de maternidade entre as participantes, a partir da necessidade de engravidar a qualquer custo físico e psíquico, na busca de uma completude narcísica. Foi possível perceber ainda que o tratamento com técnicas de reprodução humana assistida muitas vezes exacerbou os seus sentimentos de frustração e desvalia. Sugere-se que novos estudos ampliem as investigações sobre o tema, considerando aspectos tais como as eventuais repercussões no vínculo conjugal a partir da vivência da infertilidade, além da relação dessas mulheres com suas mães.

This study aimed to identify womens expectations, feelings, fears and desires on motherhood, human assisted reproduction technologies and infertility as well. Three blogs about infertility were analyzed. Each blog was considered as a case, characterizing a qualitative multiple cases study desing. The blogs authors were women (26 to 29 years old) that were married for more than a year. The findings were analyzed according two thematic axes: 1. the pregnancy and motherhood desire, and 2. feelings related to infertility and manners of handling with this condition and its treatment. The motherhood desire was manifested by participants, because the need to getting pregnant at any physical and psychic effort, in search of a narcissistic fulfillment. It was possible to note that human assisted reproduction treatment has increased their frustration and depreciation feelings. New studies could broaden the investigation about this theme, considering aspects such as the eventual repercussions of infertility experience in marital relationship and the relationship that women establish with their own mothers.

▼ This dissertation explores the association between chronic maternal psychosocial stress and neonate body size, size for gestational age and gestation length in Samoan women living on the Pacific island of Tutuila, American Samoa. Two interrelated studies were conducted to address this goal. First, the medical charts of all women of Samoan descent giving birth on island (n=1052) were reviewed to determine whether their marital status impacted the amount of prenatal care they accessed, the types and amounts of surgical interventions they received during pregnancy and their neonates' size and gestation length. Results indicated that married women accessed prenatal care earlier and more frequently and were less likely to have an episiotomy or operative vaginal delivery than unmarried women. However, women's marital status was not associated with any other medical interventions used during delivery. Although marital status was significantly related to some neonate outcomes, these differences are unlikely to be biologically significant and more likely to be an artifact of the large sample size. The second study recruited 151 women from their initial prenatal care appointments. Multiple measures of psychosocial stress were conducted throughout their pregnancies and their delivery and neonate outcomes were collected post-partum. A physiological response to psychosocial stress was measured using a minimally invasive indirect biomarker: Epstein-Barr Virus antibody levels (EBV). EBV was compared to three self-reported stress measures (status congruence, Perceived Stress Scale, Monthly Stress Questionnaire) in 78 pregnant women to determine if any of these self-reported scales accurately reflect psychosocial stress in Samoan women. There was no relationship between EBV and any of the self-reported measures of stress. Finally, the relationship between psychosocial stress (as measured by EBV), maternal characteristics and neonate outcomes was investigated. No relationship was found. Taken together, this study provides no evidence that psychosocial stress impacts pregnancy outcomes in Samoan women giving birth in American Samoa. These results were somewhat surprising in light of previous research conducted in the United States and Europe that has found a relationship between psychosocial stress and neonate outcomes. ▼ A wealth of research has been amassed and continues to grow through efforts to understand the complex nature of the relationship between the colonization and development of the human gut microbiota, its influence on the development of the immune system, and its role in both health and disease. Since previous research has demonstrated early life conditions can influence the colonization and development of the human gut microbiota, it is critical to understand how circumstances around the birthing process affect long-term outcomes beginning at this crucial stage in our development. Using the 1970 British Cohort Study, this thesis examines the relationship between birthing conditions and the outcomes of overall health, mental health, and reproduction using the evolutionary framework of life history theory through backwards stepwise regression analyses. Results indicated being born at home or use of maternal pain relief during labor resulted in a lower occurrence of infections and childhood diseases. As well, being born at home, having an assisted vaginal delivery, or use of maternal pain relief during labor resulted in a decrease in the likelihood of respiratory issues. Contrary to expectation, elective cesarean delivery predicted a lower likelihood of developing respiratory issues in this birth cohort and requires future research. Use of maternal anesthetics during labor resulted in a greater occurrence of digestive issues. Interestingly, being born at home with a medical practitioner and being born in a medical facility with a medical practitioner both correlated with an earlier age of menarche. Being born at home was also found to correlate with a greater likelihood of a cohort member having a greater number of offspring themselves. Surprisingly, birthing conditions did not predict the likelihood of a cohort member developing depression in this study. From the results, there appears to be a connection between conditions that present an early life stress and negative health outcomes as well as an earlier age of menarche. These results are consistent with predictions from life history theory. The results also suggest that a reduction of stress for the birthing mother could present a reduction in early life stress for a fetus leading to a lower occurrence of immune dysfunction that translates to a decreased likelihood of respiratory issues and infections and childhood diseases. Additionally, the results for the reproduction outcome suggest that conditions representative of early life stress or extrinsic risk, whether that risk be increased exposure to pathogens or due to a more difficult birth, lead to an earlier age of menarche. While other research has concluded that cesarean section is detrimental for long-term health outcomes of the fetus, my findings demonstrate complex and multi-faceted relationships between early life conditions and long-term outcomes. ▼ As the number and age of human couples turning to assisted reproductive technology (ART) continues to increase, it is essential for clinicians to understand infertility threats related to both female and male patients. The objective of this study was to evaluate the association between age, environment, and reproductive success in male patients having participated in assisted reproductive technology. In corresponding experiments, male infertility variables such as; age, lifestyle exposures, body mass index (BMI), and infertility length with current partner (ILCP) were investigated. A retrospective collection of clinical male patient data from 2011 to 2014 was evaluated. Thirty-five variables were collected from an original sample of 132 patients and correlated for relationships related to male fertility. A negative relationship was observed between pregnancy and male age, IVF pregnancy and male age, male age and semen volume, and male age and semen progressive motility. A negative correlation was also revealed among alcohol usage and semen volume and alcohol usage and total motile sperm/specimen. Additionally, a positive correlation was observed between ILCP and percent normal semen. The goal of the following study, the clinician survey, was to evaluate and compare differences in opinions. Questions pertained to male infertility factors and fertility clinic practices. Clinicians responded with the following opinion rates; 67.9% felt semen analysis was an effective predictor, 32.7% reported no idea if DNA fragmentation was a predictor, 58.5% were in agreement that male age had somewhat significance, 80.1% responded that genetics and/or epigenetics displayed somewhat or significant influence (41.5% and 39.6%), 58.5% believed male exposure/environmental factors displayed significance, 53.9% felt access to more male information would enable better care. The most commonly seen descriptive variable clinicians reported was ILCP (70.8%), the most important semen characteristic was sperm count (84.6%), the most commonly seen urological variable was vasectomy (77.8%), smoking was the most commonly seen environmental exposure (74.5%), and medication use was the most commonly seen medical variable (84.8%). Clinicians described that 39.1% of patient charts were <25.0% completed and 63.0% of clinicians acknowledged that the industry was not providing adequate male reproductive information to infertility patients.

▼ The physiology of the menstrual cycle has been reviewed and the suitability of the ovarian steroid urinary metabolites estrone glucuronide and pregnanediol glucuronide as markers of fertility and as utilised by the home Ovarian Monitor fertility assay was discussed. The biomaterials for the homogeneous enzymeimmunoassay which forms the basis of the Ovarian Monitor home fertility assay were prepared. One of the major difficulties in preparing a signal generator for use in home tests is the separation of the unconjugated enzyme material from the desired signal generators. In this thesis a new procedure was developed for the purification and isolation of the complex range of signal generators formed during acylation of hen egg white lysozyme with estrone glucuronide. A cation exchange column in the presence of 7 M urea allowed the separation to be carried out in the absence of the hydrophobic effects which complicate other schemes. Even under these conditions complex behaviour was seen which could be rationalised in terms of the tertiary structures of the conjugates and their electrostatic fields. A second step involving hydrophobic interaction chromatography gave two pure mono conjugated estrone glucuronide lysozyme conjugates in good yield the activities of which were highly inhibited (>90%) by anti-estrone glucuronide antibodies. The availability of the pure conjugates allowed the effect of tertiary structure on the immunoassays to be evaluated. The results showed that both mono acylated hen egg white lysozymes could be used to give good standard curves for use in monitoring menstrual cycles for the naturally occurring periods of fertility and infertility. Since the specific activity of human lysozyme is three times that of hen egg white lysozyme, in an attempt to provide a more rapid test for fertility human lysozyme estrone glucuronide conjugates were synthesised with estrone glucuronide for the first time. However, despite the fact that these two enzymes had very similar tertiary structures they behaved completely differently in the protein chemistry and immunological experiments reported in this thesis. The human enzyme was more easily acylated to give pure mono acylated conjugates in high yield and the conjugates were more easily purified to give highly inhibitable conjugates (>95%). The differences in behaviour could be accounted for in terms of the sequence differences between the two lysozymes and the relative exposure of the lysine residues. A fast assay (1-2 minutes) was developed for urinary estrone glucuronide using the three new signal generators. However, the sensitivity of the assays was less than half that of the hen egg white conjugates making them unsuitable for use in home assays for fertility. The assays could be useful for women using fertility drugs such as clomiphene. The lack of sensitivity of the assays and other binding behaviour indicated a much tighter binding to the antibody than with the hen egg white conjugates. ▼ Spermatogenesis is a complex and coordinated process of formation of sperms from the precursor spermatogonia, occurring inside the unique environment existing in the seminiferous epithelium. This process of development, characterized by concomitant changes in the cellular morphology, metabolism and differential gene expression, can be divided into 3 distinct phases: i) proliferation of the spermatogonia through mitosis; ii) meiosis or reduction division, which commences with transformation of the type B spermatogonia into primary spermatocytes and their subsequent entry into the meiotic prophase I. These primary spermatocytes, divide to form secondary spermatocytes, and then divide again to form haploid round spermatids; (iii) spermiogenesis or differentiation and maturation of the round spermatids without further division to form the unique spermatozoa (Kerr and De Kretser, 2006, Clermont, 1966, Heller and Clermont, 1964). This complex process of division and differentiation is regulated at three distinct levels: i) The extrinsic level where the gonadotropins and testosterone regulate gene expression in the germ cells sustaining their survival and differentiation (French, 2012); ii) The interactive regulation that involves interactions between the somatic cells such as the Sertoli cells and the germ cells; iii) The intrinsic gene expression associated with each step of development of the germ cells (Eddy, 2002) wherein each stage of differentiation is accompanied by precise stage-specific differential gene expression. (Kleene, 1996, Kierszenbaum et al., 2003, Sassone-Corsi, 2002, Kleene, 2001, Sassone-Corsi, 1997). Any alterations in this gene expression pattern leads to disruption and/or arrest of spermatogenesis at various stages, causing male infertility (Zorrilla and Yatsenko, 2013, Krausz et al., 2015). Many studies have been focused on investigating the underlying molecular mechanisms governing the process of germ cell development such as self-renewal, meiotic recombination and differentiation (Hecht, 1998, Grootegoed et al., 2000, Robles et al., 2017). Analysis of differential gene expression in isolated and purified populations of different germ cells have been very useful in the understanding of the genetic regulation of human spermatogenesis by providing information about the cell type-specific gene expression and regulation. (Meistrich et al., 1973, Bellvé, 1993, Meistrich et al., 1981, Chalmel et al., 2007). However, these methods are limited by the large amounts of tissue required, which is difficult to obtain in the case of humans (Schultz et al., 2003). Large-scale gene expression studies and the “omics revolution” have also helped in identifying some of the regulators of spermatogenesis (Carrell et al., 2016). In spite of advances in the current understanding of the regulation of spermatogenesis, the exact molecular mechanisms of how the genetic and epigenetic alterations affect human spermatogenesis are still unclear (Neto et al., 2016). ▼ In mice and other mammalian species, deposition of semen into the female reproductive tract elicits a local inflammatory response. Whether a comparable response occurs within the human cervix has not previously been studied. The experiments described in this thesis demonstrate, using cervical tissue biopsies taken before and after intercourse, that exposure to semen elicits an infiltration of leukocytes into the cervical tissue of peri-ovulatory women. Immunohistochemical analysis identified macrophages and dendritic cells as the predominant leukocytes recruited into the cervical epithelium and stroma following intercourse. Cytotoxic / suppressor T lymphocytes and memory T cells were also increased. Comparable responses were not detected following condom-protected intercourse. Quantitative real-time PCR was performed on duplicate tissue biopsies to investigate the molecular regulation of this response. Expression of GM-CSF, a potent stimulator of myeloid cell recruitment, was found to increase by 2.5-fold following unprotected intercourse. Trends towards increased IL-6 and IL-8 mRNA were also observed. Condom-protected intercourse did not activate cytokine expression, further suggesting that exposure to semen, as opposed to mechanical trauma, provides the inflammatory stimulus. In an in vitro model using the immortalised Ect-1 cell line, TGFβ was identified as a candidate active seminal factor. All three TGFβ isoforms were capable of mimicking the stimulatory ability of seminal plasma in Ect-1 cells and were comparable in their capacity to stimulate both GM-CSF and IL-6 expression in a dose-responsive manner. The addition of TGFβ isoform-specific neutralising antibodies inhibited seminal plasma-induced increases in these cytokines. However TGFβ was unable to stimulate IL-8 production. Addition of IFNƴ was found to strongly inhibit TGFβ-stimulated GM-CSF production, and 19-0H PGE₁ was found to increase IL-6 and IL-8, but not GM-CSF production. Responses to seminal plasma constituents were almost exactly replicated in primary cultures of human ectocervical cells. These results identify TGFβ as the major active constituent in human seminal plasma and indicate that other seminal agents, 19-0H PGE₁ and IFNƴ, interact with TGFβ to differentially regulate cervical cytokine expression. Finally, whether human seminal plasma cytokine content was associated with fertility in men was examined. No relationship between seminal plasma TGFβ₁, TGFβ₂, TGFβ₃, IL-8 or bacterial endotoxin content and fertility status was observed. However, there was an increased likelihood of high IFNƴ content in the male partners of couples experiencing infertility, most notable in recurrent miscarriage. The discriminating value of IFNƴ was increased when evaluated as a ratio of total TGFβ content. Inflammatory changes after exposure of the female reproductive tract to seminal plasma are implicated in 'conditioning' the maternal immune response, to facilitate successful embryo implantation and pregnancy. ▼ Notre travail s’inscrit dans le champ de la didactique de la Biologie en général et celui de la reproduction humaine en particulier. Nous nous intéressons spécialement à une « tentative » d’autonomisation des apprenants - plus exactement des élèves - tunisiens en classe de terminale scientifique par le biais de l’autocorrection/auto-évaluation dans l’apprentissage de la reproduction humaine et l’éducation à la sexualité. Le choix de notre sujet de recherche plonge ses racines dans les méthodes et procédés utilisés dans les apprentissages scolaires en rapport avec l’appartenance des apprenants, tout comme les enseignants, à une société arabo-musulmane, où, parler de reproduction reviendrait à parler de sexualité qui est un terme « sale », « vulgaire », « honteux » et l’évoquer pourrait bien être contre la morale, donc une attaque à la pudeur. Mais, cette conception qui empêcherait les élèves, jeunes adolescents, de parler ouvertement de sexualité et de reproduction, est-elle d’ordre religieux ? Ou qu’elle n’est qu’une construction sociale ? La pratique religieuse fait-elle obstacle à l’évocation des questions se rapportant à la sexualité dans la mesure où celle-ci n’est aucunement abordée au sein des familles où les parents sont musulmans pratiquants ? L’Islam comme religion incitant à la pudeur et la chasteté, fait-il de la sexualité un tabou ? Grace à des questionnaires nous avons tenté d’apporter de l’éclairage à tous ces questionnements. De par notre ancienne expérience professionnelle d’enseignante de Sciences de la Vie et de la Terre, nous avons remarqué chez les apprenants une réticence manifeste, qui s’exprime par un manque, voire une absence de participation lors de la construction des savoirs allant jusqu’à s’abstenir de poser des questions. Rien qu’un silence assourdissant qui règne dans la salle de classe lors de l’apprentissage de la reproduction humaine. Nous entendions une voix muette mais opprimée me disant : « Allez, faites vite ! Et pourvu de ça finisse ! ». Cette passivité jouait, éventuellement en faveur d’un paradigme éducatif purement transmissif. Notre idée première était de contraindre, d’une manière ou d’une autre, les apprenants à communiquer entre eux autour de ce thème « problématique » qu’est la reproduction humaine. Cette initiative était aussi bien dans le but de tenter de briser des obstacles éventuels qui pourraient s’interposer d’une part entre les apprenants des deux sexes et l’enseignant et, d’autre part, entre les apprenants et le savoir, que de laisser aux apprenants l’occasion de corriger eux-mêmes leurs propres erreurs, peut-être ne les reprennent-ils plus. Tant de questionnements nous sont venus à l’esprit : bien que non habitués aux pratiques autocorrectives/auto-évaluatives, les élèves tunisiens sont-ils en mesure de porter des jugements corrects à leurs propres travaux ? Si, travaillant en petits groupes dans l’élaboration d’un cours en vue de co-construire un savoir, seront-ils plus performants dans leurs auto-évaluations quant à ce même savoir ? Esta dissertação tem como objetivo discutir o princípio do anonimato de doadores de gametas na reprodução assistida. Inspiramo-nos no termo cunhado pela antropóloga norte-americana Charis Thompson, coreografia ontológica – termo que visa explicitar os diferentes modos pelos quais os aspectos técnicos, científicos, legais, políticos e financeiros do parentesco são coordenados nas clínicas de RA. Para melhor entender os diferentes elementos da questão, escolhemos três agentes centrais que, ao mesmo tempo, revelam e influenciam essa “coreografia”. São eles: a divulgação jornalística, os especialistas da área e a legislação. Entendemos que ainda que a ciência impressa nessas tecnologias tenha pretensões universais, os modos de conhecer tais procedimentos são dados a partir de “epistemologias cívicas” e operacionalizados a partir de “estratégias nacionais de regulamentação”. Por essa razão acreditamos que o método comparativo ilumina a especificidade dos diferentes casos e faz com que esses objetos produzam comentários recíprocos um sobre o outro. Além de conduzir entrevistas entre médicos e embriologistas especializados na reprodução assistida, nessa dissertação nos comparamos a divulgação jornalística de quatro países e a legislação de outros cinco. Tais comparações nos permitem visualizar o modo como a reprodução assistida e mais especificamente o anonimato de doadores tem sido configurados no jogo de semelhanças e diferenças; as lógicas que conduzem da prática a proibição; as razões que explicam por que a busca por irmãos é mais aceitável do que a busca por pais genéticos, e os pesos relativos da adoção e da doação de gametas.

In this dissertation, we discuss the principle of anonymity concerning the donors of gametes in the process of assisted reproduction. Our analysis is inspired in the term ontological choreography, coined by North American anthropologist Charis Thompson to describe the technical, scientific, legal, political and financial aspects of kinship that are coordinated in the clinics of assisted maternity. In order to better understand the different elements in question, we have chosen three central agents that simultaneously reveal and influence this “choreography”: journalistic articles, specialists in the area and legislation. We consider that, although the science present in these technologies has universal pretensions, the ways in which we come to know these procedures are conditioned by “civic epistemologies” and operationalized according to “national styles of regulamentation”. For this reason, we believe that the comparative method illuminates the specificity of different cases, causing each context to shed light on the others. Aside from conducting interviews among Brazilian doctors and embryologists specialized in assisted reproduction, we compare in this dissertation the journalistic divulgation of four countries and the legislation of another five. Such comparisons permit us to visualize the way in which assisted reproduction, and specifically donor anonymity, take shape in the play…
