▼ Introduction: This thesis reports a research project with a mixed methods research design and a pragmatist worldview. It documents, analyses and reflects upon the evolution of a project, that over nine years, examined the challenges of designing and implementing a clinical trial design that could capture the effect of using Ovarium compositum to support the outcomes of women undergoing infertility treatment at Jersey General Hospital. Aims: The initial aim of the research project was to design and conduct a randomised clinical trial of a homeopathic product, Ovarium compositum, as an additional treatment for women undergoing infertility treatment at Jersey General Hospital. When the trial did not meet it's recruitment target the focus of the project switched to an investigation of the reasons for trial failure. The need for a pragmatist philosophical worldview to bring structure and understanding to the process of writing and rewriting the thesis became increasingly clear as the project evolved. The aims are divided into two categories, methodological and outcomes. Methodological aims include: • To explore the appropriate methodology for designing a clinical trial of a homeopathic product. • To understand the role of a pragmatist worldview in clinical trial design and in rewriting a thesis. • To understand the use of Mixed Methods Research in healthcare research. Outcome aims include: To understand the conceptualisation of infertility and its treatment by homotoxicologists in the UK.

▼ This study aimed to explore the experiences of midwives who were also homeopaths as they attempted to straddle the different philosophical and practice paradigms they encountered in each domain. It also explored the implications of their experience on their practice. Over recent decades the National Health Service (NHS) has moved towards a scientific-bureaucratic perspective, in which the emphasis is on the use of evidence-based frameworks. It has been argued that this development has moved the focus in healthcare away from ‘caring’. In parallel, there has been an increase in the demand for complementary and alternative medicines in the United Kingdom (UK), and elsewhere. In responding to this call a number of midwives have taken up training opportunities in massage, aromatherapy, hypnotherapy, acupuncture and homeopathy, amongst others. There are no studies however, that have examined the impact of training as a homeopath on midwives and their practice. After a comprehensive analysis of existing literature, this study used an interpretive hermeneutic framework to explore the experience of midwives who trained as homeopaths. In-depth interviews were conducted with seven midwife homeopaths. The findings were analysed using three different lenses. The first of these conceptualised and explored the midwives narratives as personal and professional metamorphoses, as they changed from midwife to homeopath or midwife homeopath. Secondly, the data were framed using a Heideggerian lens, which illuminated a process of transformation into being authentic practitioners. This demonstrates how authenticity allowed the participants to (re) engage with, and further value the therapeutic relationship as a fundamental element of their clinical practice. The final lens explored the impact engendered of being authentic on the therapeutic relationship that developed between midwife homeopath and the women in their care. The conclusion was drawn that the therapeutic relationship developed by an authentic practitioner via a homeopathic consultation is transferable to other healthcare practitioners, and offers a challenge to practice based in notions of scientism and bureaucracy. Further exploration of these phenomenon’s could help to deliver on the drive to deliver compassionate personalised care across the NHS as a whole.

▼ The apparent success of homeopathy is often attributed to a collaborative, holistic and empathic consultation and to the practitioner patient relationship. However most research into the homeopathic consultation has focussed on patients’ experiences although the practitioner is a crucial component of the therapeutic context and may have an important part to play in optimising health outcomes in homeopathy. Therefore the aim of this research was to gain an in depth understanding of homeopathic practitioners’ perceptions and experiences of the consultation. An additional aim was to generate a theoretical model to explain the processes that underpin the homeopathic consultation. A qualitative approach was taken using constructivist grounded theory methodology employing a process of two phases of data collection. Medical and non medical homeopaths were sampled from the registers of the Faculty of Homeopathy and the Society of Homeopaths. Phase 1 involved face to face in-depth interviews with homeopaths. From these interviews a theoretical model of the homeopathic consultation was developed. Phase 2 of data collection involved observations of homeopathic consultations and the use of solicited practitioner reflective diaries in order to confirm, refute or enlarge on the model which developed from phase 1. The emergent theoretical model entitled “a theoretical model of a UK classical homeopathic consultation” describes how homeopaths view and enact the consultation process. The findings indicate that the consultation consists of processes which are interlinked and dependent on each other. I have labelled these processes exploring the journey together, finding the level, responding therapeutically and understanding self. Central to these processes is the core category which I have labelled connecting. This study has highlighted that the whole process of the homeopathic consultation is important and aspects that are unique to homeopathy such as the process of identifying and matching the homeopathic remedy cannot be separated from the rest of the consultation and must be considered as part of the whole process. Additionally the ritualistic and symbolic aspects of the consultation that are embedded in this process are of paramount importance and provide meaning, highlighting the power of the consultation to promote beneficial effects for the patient. This research has implications for all stakeholders and signals many future avenues for research.

▼ A síndrome da ardência bucal (SAB) é definida como uma sensação de queimação principalmente na língua, palato e ou gengiva ou em outra região da mucosa oral, na ausência de lesão oral específica. Caracterizada por uma contínua, espontânea, e intensa sensação de queimação como se a boca ou a língua tivessem sido escaldadas. A etiologia é pobremente compreendida embora nova evidências como neurológicas, emocionais e alterações hormonais estejam relacionadas com a SAB. É mais comum entre as mulheres na pós-menopausa e causa intenso desconforto e sofrimento. Antidepressivos tricíclicos, benzodiazepínicos e drogas antipsicóticas são as opções mais indicadas no tratamento e mostraram resultados variáveis. O objetivo desse estudo foi investigar a ação do tratamento homeopático na sintomatologia da ardência bucal em duas fases: estudo duplo-cego placebo-controlado e estudo aberto. Associados à SAB, níveis de ansiedade e depressão também foram considerados. Esse estudo foi composto por 31 pacientes diagnosticados com SAB na Clinica do Departamento de Estomatologia da FOUSP, entre julho de 2011 a setembro de 2012. Estes (25 mulheres, e 6 homens média de idade de 58,93 anos, variação 34-85 anos), foram randomizados em dois grupos G1(Arsenicum album) e G2 (Placebo), durante 90 dias, com washout de 30 dias (FASE1). O sintoma ardor bucal e a resposta terapêutica foram avaliadas utilizando uma escala visual de sintomatologia (EVS) a cada visita. Para se avaliar o Efeito global percebido (EGP), utilizou-se uma escala de 5 pontos ao término da FASE 1. O nível de redução sobre o sintoma ardor avaliado pela EVS e o efeito Global percebido (EGP) foram estatisticamente significantes para o grupo G1(Arsenicum album) com redução de sintoma de 44,50% em relação ao placebo (12,65%). Na FASE 2 (estud0 aberto), 28 pacientes que participaram da FASE 1, receberam medicamento homeopático individualizado durante 7 meses, com visitas mensais e Washout de 60 dias. O sintoma ardor e resposta terapêutica foram avaliados utilizando (EVS) inicial obtido ao final da FASE 1, e no final da FASE 2. Houve redução de sintomas no final do tratamento de 64,60%. A análise dos dados nos permitiu concluir que ambos os tratamentos FASE 1 e FASE 2 tiveram efeito terapêutico com significância estatística. Houve efetividade do tratamento homeopático sobre o placebo na sintomatologia da ardência bucal. A FASE 2 apresentou melhores resultados quando comparados aos resultados obtidos na FASE 1. O Grupo Medicamento da FASE 2 foi mais eficaz quando comparado aos Grupos, Medicamento e Placebo da FASE 1, com significância estatística no intervalo de confiança estabelecido. Os melhores resultados foram observados nos pacientes com sintomas entre 6 - 36 meses.

▼ Homoeopathic practitioners in the United Kingdom can be divided into two groups,those with medical qualifications and those without, professional homoeopaths. This study examines these two groups to discover how they practise homoeopathy and why. Also examined are any tensions that may exist, both between the two groups and within the groups. Collecting qualitative and quantitative data using questionnaires and interviews, a randomly selected sample of homoeopaths was studied. All subjects were members of either the Faculty of Homoeopathy or the Society of Homoeopaths. The study starts by examining the development of homoeopathy over its almost 200 year history. Following this section data regarding the practice of homoeopathy and the opinions of homoeopaths on this practice are discussed. The homoeopath's opinions regarding their opposite numbers are also discussed, that is professional homoeopath's opinions of medically qualified homoeopaths and vice versa. The data highlighted a number of tensions that exist between medically qualified homoeopaths and professional homoeopaths. Medically qualified homoeopaths questioned the wisdom of allowing non-medically qualified people to practice homoeopathy and the professional homoeopaths questioned the validity of the homoeopathic methods used by medically qualified homoeopaths. Tensions within professional homoeopathy were also identified between pro and anti professionalisation and registration subgroups. Another tension identified was between those professional homoeopaths who claim to use the original, 'classical' formulation of homoeopathy and those using a more eclectic therapeutic regime with changes to the original method incorporated into their practices. Finally, the utility of the concept of heresy when describing both medically qualified homoeopaths and professional homoeopaths in the United Kingdom was addressed. Although the labelling of heretics is properly reserved for those members of the orthodoxy, not for researchers, a small number of homoeopaths were identified as holding potentially heretical ideas. On the whole the medically qualified homoeopaths and the majority of professional homoeopaths could not be regarded as heretics or dissenters in any way. In the light of the tensions that were identified, and the policies being promoted by the professional bodies, the conclusion examines the possible future of homoeopathy in the United Kingdom in the first years of the new millennium. A thoroughly modernist medicine in a possibly postmodern era.

▼ This thesis examines from a medical anthropological viewpoint how the practice of the newly imported complementary and alternative medicine (CAM) has been transplanted, received and transformed in Japan. More specifically, I focus on homeopathy, which was introduced into Japan in the late 1990s. To address the research question, I focus on the practice of homeopathy from the anthropological viewpoint. The adoption of any new form of medicine is influenced by the prevailing medical, social and cultural context. So, how and why was homeopathy introduced into Japan the late 1990s? I explore this question by focusing on three aspects of the reception of homeopathy in Japan: (1) the institutionalisation of the homeopathy, including the formation of associations of practitioners and homeopathic colleges; (2) the translation of the theory and practice of homeopathy by the practitioners into a culturally acceptable form; (3) the utilisation and consumption of homeopathy by the patients, their families and self-prescribers. Over eighteen months of fieldwork in Japan led me to focus on these three elements of homeopathic practice. Regarding the theoretical framework, this mainly explores medical pluralism and the health care system in Japan from an anthropological perspective, and the globalisation and transmission of medicine. I argue that the success of homeopathy in Japan was largely thanks to the transmission strategies set by the founders of the colleges for lay homeopaths. Mothers in particular, concerned by worries over family health care, were drawn by this approach. Furthermore I also argue that this group not only be' self-help groups, creating thereby a strong tie with the lay homeopaths. I argue that mothers gained a sense of the empowerment through homeopathy. Within the Japanese health care system it was the popular sector that received and developed homeopathy.

▼ Temos presenciado um crescimento expressivo da ovinocultura de corte nos últimos anos. Algumas doenças de grande importância econômica tem sido motivo de estudos, não apenas pelo impacto econômico que representam, mas também para a saúde do animal e do homem. Com a crescente preocupação com resíduos de medicamentos pesticidas nos produtos de origem animal, perda da qualidade do leite e conseqüente baixo ganho de peso dos borregos de corte, os sistemas orgânicos de produção vem ganhando espaço. Sabe-se que a mastite subclínica é uma das responsáveis pelo baixo rendimento de carcaça na ovinocultura de corte. Neste experimento, optou-se por realizar tratamento com medicamento homeopático (Phytolaca decandra) das ovelhas com tetos diagnosticados com CMT 2+ e 3+, sem sinais de mastite clínica. Dois lotes de ovelhas com mastite subclínica foram usados. Um lote controle que recebeu placebo e o lote tratado que recebeu remédio homeopático duas vezes ao dia junto ao concentrado a partir da quarta semana do parto (de lactação). Foram colhidas duas amostras de cada tetos com mastite subclínica no início do experimento (30 dias do parto) e a cada 15 dias até o desmame quando os borregos tinham aproximadamente 60 - 65 dias de vida. Das amostras colhidas, uma seguiu para identificação do agente microbiológico e outra para a contagem de células somáticas (CCS). Ao final do estudo não houve diferença estatisticamente significativa entre as CCSs da secreção láctea quando comparadas antes e depois do tratamento homeopático no mesmo grupo, bem como quando comparou-se ambos os grupos (tratados e placebo). Porém observou- se que o grupo tratados apresentou redução significativa (P<0,05) nos isolados das amostras com Staphylococcus spp. e aumento significativo de amostras sem crescimento bacterianos (negativas) e ganho de peso estatisticamente significante (P<0,05) quando comparado ao grupo placebo.
We have seen an expressive growth of lamb production in the last years. Some diseases of economic importance have been studied not just by its economic impact but also for the animal and human health. With worry about pesticides and drugsŕemaining (left over) in animal origin products, the loss of milk quality and consequent lamb\'s low gain weight, the organic production systems are growing up. It\'s known that subclinical mastitis is one of the most dangerous and one of those held responsible for the decrease of weight-gain in lamb breeders. ▼ Escherichia coli é o agente etiológico mais importante das diarréias neonatais em suínos, sendo a Enterotoxigênica (ETEC) a mais comumente isolada. Foram descritos cinco tipos principais de fímbrias: F4 (K88), F5 (K99), F6 (987P), F18 e F41 e enterotoxinas termolábeis (LT), termoestáveis (ST), e a toxina shiga-like ou verotoxina (Stx2) em isolados de origem suína envolvidas no processo de diarréia. A exigência do consumidor em busca de carnes sem resíduos químicos e a proibição do uso de antibióticos e quimioterápicos na produção de suínos, bem como o custo de tratamentos alopáticos, tem levado a suinocultura a procurar alternativas. O presente estudo foi desenvolvido em granjas de suínos no Estado de São Paulo e Mato Grosso, Brasil. Foram avaliados 157 leitões, e realizados 243 coletas de fezes. Coletas de fezes de 3 e 8 leitões com diarréia foram realizadas em cada granja, visando a pesquisa da presença de Escherichia coli para a preparação do medicamento Bioterápico. Concomitantemente uma anamnese detalhada foi feita para a escolha do medicamento homeopático ideal para cada granja (China off 30 CH, Phosphorus 30 CH e Pulsatilla 30 CH respectivamente). Formados 4 grupos de 12 leitões e suas respectivas mães (primíparas), os tratamentos foram simultâneos: controle (antimicrobiano empregado na granja), medicamento homeopático, Bioterápico de E.coli e medicamento homeopático associado com o respectivo Bioterápico. Os medicamentos foram feitos segundo a farmacotécnica homeopática brasileira. Foi realizada a pesquisa de fatores de virulência e enterotoxinas em 154 colônias de. Escherichia coli através da Reação em Cadeia da Polimerase (PCR). No tratamento da diarréia os grupos tratados com medicamentos homeopáticos passaram de 58,3% de animais com diarréia para 5,5% ao final do tratamento, uma diferença estatisticamente significante (p< 0,05), e os grupos de medicamento e Bioterápico 30 CH associados também apresentaram significante redução de animais doentes (p< 0,05), pois passaram de 47,0% de animais com diarréia no inicio para 20,0% ao final. No ganho de peso os mesmos grupos obtiveram maiores ganhos, com resultados de forma estatisticamente significante (p< 0,05) em relação aos outros grupos. Nos animais com E.coli foram encontrados apenas o fator de virulência F41 em 10,4% dos animais e a enterotoxina Stb em 1,9% dos animais. Concluí-se que os medicamentos homeopáticos são uma alternativa eficiente para o uso em distúrbios entéricos em suínos, assim como para aumentar o ganho de peso, além de ser mais econômico.

▼ O elevado custo dos tratamentos tradicionais da mastite bovina, associado à redução de produção e inviabilidade de tratamento das mastites subclínicas durante a lactação, bem como a exigência cada vez mais rigorosa da ausência de resíduos de antimicrobianos por parte de instituições nacionais e internacionais, impulsiona o desenvolvimento de novas alternativas terapêuticas que visem minimizar o impacto das medidas tradicionais de tratamento. A homeopatia surge como importante alternativa, sendo aceita nacional e internacionalmente. O objetivo do trabalho foi avaliar o tratamento homeopático com Phytolacca decandra 30CH durante a lactação de vacas com mastite subclínica, utilizando parâmetros de qualidade do leite como Califórnia Mastitis Test (CMT), contagem de células somáticas (CCS) totais, porcentagens de polimorfonucleares (PMN) e de mononucleares (MN), teores de proteína, lactose, gordura, sólidos totais (ST) e extrato seco desengordurado (ESD), além da mensuração da produção leiteira. Foram selecionadas 26 vacas, CMT 2+ e 3+ sem sinais de mastite clínica, entre o terceiro e sexto mês de lactação, pluríparas e divididas em dois grupos, um com tratamentos medicamentosos mensais e outro com tratamentos quinzenais. Mensalmente foram colhidas duas amostras de leite de cada glândula mamária (teto) que apresentava mastite subclínica. Uma das amostras foi utilizada para CCS e análise dos componentes do leite, e a outra para identificação microbiológica. Nos grupos tratados, foi administrado o medicamento homeopático Phytolaca decandra 30CH diluído em água e aspergido nas mucosas oro-nasais e vaginais, enquanto que nos grupos denominados controle foi administrado placebo da mesma maneira. A pesagem da produção láctea de ambos os grupos foi realizada quinzenalmente até o final do experimento. Verificou-se então que não houve diferença significativa entre a produção láctea, CCS e a presença de microrganismos na secreção láctea das glândulas quando comparadas antes e depois do tratamento homeopático, dentro de cada grupo, bem como quando comparados os grupos entre si. As medianas de CMT e porcentagens de PMN foram compatíveis com infecções mamárias agudas, embora os animais tenham sido diagnosticados como portadores de mastites crônicas e em nenhum momento desenvolveram sinais clínicos.

▼ Life expectancy has steadily increased over the last two centuries due to improvements in living standards, education, science and medicine. However, instead of basing health decisions on the best scientific evidence available, millions of people are currently using
unsupported complementary and alternative medicines (CAM). The use of CAM is rising worldwide despite the lack of robust scientific evidence for their safety or efficacy. The central concern around the use of CAM is that users might delay or forgo the use of
conventional medicine potentially leading to severe health deterioration.
Homeopathy, one of the most widely used CAM, is particularly interesting as its proported mode of action is deemed impossible by current scientific knowledge. Moreover, multiple
systematic reviews have come to the conclusion that
homeopathy is no more effective than a placebo.
Homeopathy proponents, however, claim that homeopathic remedies are an effective medical treatment, which could lead to misinformed consent of potential
homeopathy users.
This research project investigates how
homeopathy users perceive
homeopathy, especially with regards to its scientific basis. The central question addressed in this thesis is: do
homeopathy users in New Zealand use homeopathic remedies because they believe
the remedies to be scientifically proven to work? A sequential explanatory mixed methodology was used to answer this question, starting with collection and analysis of quantitative data via a survey. The survey results informed the design of follow-up semi-structured interviews with self-identified
homeopathy users.
According to the survey results, more than half of the respondents were
homeopathy users. Of those respondents who were
homeopathy users, 78% believed that homeopathic remedies were scientifically proven to work. This is in contrast to non-users who mostly did not believe
homeopathy had a scientific basis. Interview results challenged initial findings and showed that most
homeopathy users were aware of the lack of scientific evidence behind
homeopathy, but used the remedies regardless. Rather than scientific evidence, it was revealed that participants valued personal, anecdotal, and traditional evidence when deciding to use
homeopathy. The discussion argues that current communication efforts focused on the lack of scientific support for
homeopathy fail to reach users. In dismissing users’ experiences and perceptions, science communicators risk increasing distrust in scientific evidence and potentially strengthening users’ beliefs by compounding an adversarial relationship. In order to limit risks and complications from avoiding conventional medicine, future research into health communication must be developed with users’ experiences and perceptions in mind.

▼ The subject of this thesis is the lived experience of training in non-orthodox health care professions1 or CAM (complementary and alternative medicine) as it is commonly known. The thesis focuses both on the nature of the knowledge and skills acquired during training (knowledge of the body) and the changing embodiment of the students (embodying knowledge). It is based on ethnographic research, conducted over one academic year, at two case-study sites: the anonymized Colleges of Homeopathy and Osteopathy. The data presented in this thesis offers four distinct contributions. Methodologically, it offers insights into the embodied experience of conducting sociological research and the deep impact that this experience has on the researcher, further supporting the argument that reflexivity is a vital component of valid and reliable research. Empirically, it contributes to our understanding of an under-researched area, the ?Tactice of CAM therapies generally, and the training of practitioners particularly. Theoretically, the explicit focus of both the participants in the study .and myself, as researcher, on 'bodies' makes it a worthwhile topic of study to contribute to the growing discipline of embodied sociology. Finally, from a social policy perspective, the explosion of interest in CAM in recent years, and particularly the growing pressure on practitioners to regulate their professions, makes research into the nature of professional knowledge and practice very timely. The thesis concludes that it is of critical importance to consider embodiment in any understanding of healthcare knowledge or practice. In particular, an embodied sociological perspective permits recognition of the depth and nature of the knowledge and skills that healthcare practitioners learn to deploy on a day-to-day basis.

▼ Introdução: Além de problemas a curto prazo para a mãe e o recém-nascido, o sobrepeso e a obesidade na gestação levam a inúmeros problemas da saúde materna de longo prazo e no desenvolvimento da criança. O não tratamento de gestantes com transtorno mental comum e sobrepeso pode aumentar a frequência de depressão perinatal com riscos diretos ao concepto. Tem sido observado na literatura o uso da homeopatia na obesidade, na gestação e nos transtornos mentais comuns. Objetivo: Avaliar a eficácia do tratamento homeopático em gestantes com sobrepeso ou obesidade I ou II, sem comorbidades, suspeitas de transtorno mental comum, na prevenção do ganho excessivo de massa corporal durante a gestação. Método: Foram estudadas gestantes com sobrepeso ou obesidade I/II e suspeitas de transtorno mental comum, sem doenças concomitantes. No grupo teste (62) receberam tratamento homeopático e no controle (72) placebo. Foram avaliados o ganho de peso na gestação por meio da diferença entre os índices inicial e final de massa corporal corrigida pela idade gestacional, APGAR, peso do recém-nascido, auto percepção de saúde materna, complicações maternas, entre outras condições. Resultados: A média da diferença entre o índice de massa corporal inicial e final corrigidos pela idade gestacional foi de 4,95 kg/ m2 no grupo homeopatia e de 5,05 kg/m2 no grupo controle. A diferença entre as médias não foi significante com valor de p = 0,815 e ICdif 95% (-0,916 a 0,722). O índice de APGAR no quinto minuto foi estatísticamente significante com valor de p = 0,040. Não se observaram diferenças significativas nos demais desfechos. Conclusão: A homeopatia não contribuiu para a prevenção do ganho excessivo de massa corporal em gestantes com sobrepeso ou obesidade. A homeopatia desempenhou um papel regulador melhorando a vitalidade dos RN no quinto minuto de vida.
Introduction: Aside from short-term problems related to the mother and newborn, overweight and obesity in gestation leads to countless long-term health problems for the mother\'s and childs development. The lack of treatment for pregnant women whom are overweight and have a common mental disorder may increase the frequency of prenatal depression with direct risks to conception. It has been observed in literature the use of homeopathy in cases of obesity, gestation, and common mental disorders. ▼ This study analyzes the complex and changing historical meanings of returning to nature during the Weimar years through a set of case studies of back-to-nature organizations. The most popular of these organized endeavors were the life-reform (Lebensreform) movement, devoted to vegetarianism,
homeopathy, and nudism; the conservationist movement; and an array of independent and adult-sponsored youth movements determined to improve the minds and bodies of adolescents through organized hiking. Naturist rhetoric and practice addressed issues of general contemporary concern: the ambiguous relations of class, gender, and generation; the burdens of the lost war; and the uncertain fate of the German people. Each organization claimed to have embarked on a more natural path toward a brighter future. Two common tendencies can be seen in the processes by which the naturists gave nature its higher purpose. First, each organization established official rules and practices intended to guide their constituents in the most correct way of returning to nature. Second, the concept of nature itself underwent change due to the multivalence of contemporary nature visions. Naturist rhetoric was shaped and contested around three contradictory incarnations of nature: (1) an awestruck, neo-Romantic concept of nature as a realm of individuality, emotion, and mystery; (2) human nature, usually perceived as dangerously irrational and sexual; and (3) nature as a source of immutable laws. As the Weimar years wore on, this latter rationalistic vision became increasingly dominant in back-to-nature organizations. By the early 1930s, nature had been rhetorically honed into a more orderly and cultured nature – a clean nature. In basing their worldviews and plans of improvement on natural law, the naturists transformed nature into the utopian model for a more stable Germany. The study sheds light on cultural responses to the trends that constituted the modernity specific to the Weimar years, ranging from the rise of new mass cultural forms, to changing relations between the sexes, to the social, economic, and political legacies of war and revolution. Moreover, the study of nature concepts brings us closer to an understanding of the cultural continuities between Weimar and the Third Reich. For the Nazis appropriated the dominant rationalistic concept of nature as clean, ultimately using it to legitimate their murderous purification of the national community.

▼ Face à crescente procura por outras modalidades terapêuticas que abordam o ser humano de forma holística e a introdução das mesmas no SUS, torna-se muito importante a avaliação da efetividade e segurança dessas formas de cuidado. A Homeopatia faz parte deste conjunto de terapêuticas e, para sua avaliação, pode existir a necessidade de se valer de múltiplos instrumentos para abarcar os vários aspectos de uma resposta integral ao tratamento. Este trabalho teve como objetivo identificar e elaborar categorias de análises e instrumentos que permitam avaliar e mensurar a efetividade deste tratamento, bem como testá-los, considerando-se as
características desta racionalidade. Foram levantados, na literatura nacional e internacional, trabalhos sobre efetividade do tratamento homeopático, em busca da definição do estado da arte mas também dos principais problemas, limitações e
possibilidades dessas avaliações tendo em vista seu resultado integral. Finda esta etapa, a pesquisa destinou-se a elaboração, proposição e testagem de uma metodologia considerada mais adequada a avaliar o tratamento homeopático nesta
perspectiva. Um estudo observacional foi realizado em serviço público homeopático no município de Juiz de Fora, com tratamento individualizado, no qual foi utilizada uma estratégia de avaliação composta por três componentes: (1) avaliação de
qualidade de vida pelo instrumento SF-36; (2) análises em busca de objetivar e quantificar queixas clínicas e outros atributos de natureza subjetiva (sensação de bem-estar, sono, estado cognitivo e memória, vida sexual, sensação de felicidade)
por meio da utilização de uma escala visual analógica (EVA), na mensuração da intensidade e de opções fechadas, a exemplo do SF-36, na estimativa da frequência desses aspectos e (3) entrevistas qualitativas por intermédio de questionário
semiestruturado, com a finalidade de abordar questões relacionadas a biopatografia e mudança da atitude vital (como pacientes enfrentam os problemas do cotidiano, fatores deflagradores das queixas, como se sentem e como reagem, além de indagar seus projetos de vida e felicidade). A aplicação do questionário SF-36 apresentou algumas dificuldades de compreensão pelos participantes, talvez devido à baixa escolaridade dos entrevistados, mas mostrou-se útil à pesquisa, embora demonstre limitações na avaliação do aspecto integral do resultado da terapêutica analisada. O acompanhamento das queixas clínicas, sensação de bem-estar, sono e estado cognitivo e memória foram captados e mensurados de forma satisfatória tanto pela EVA (intensidade dos sintomas) quanto pelas respostas fechadas para medir a frequência. Situações como as avaliações da biopatografia e da sexualidade foram insuficientes para serem adequadamente avaliadas pelo pesquisador e o paciente somente. ▼ Ce travail est le résultat d’un essai d’extraction d’un possible fondement logique à la médecine homéopathique.Aucune véritable théorie sur l’homéopathie n’a donc vu le jour lors de sa découverte par S. Hahnemann et c'est cette lacune que nous nous proposons ici d'interroger.Dans cette recherche, nous avons donc essayé d’identifier un possible fondement philosophique à même d’expliquer l’effectivité thérapeutique de la médecine homéopathique, en nous appuyant sur la philosophie hégélienne en raison de sa systématicité. Puisqu’il n’y a plus de substance (donc de principe actif) d’un point de vue moléculaire dans la plupart des médicaments homéopathiques, comment expliquer l’effectivité de cette substance ? Nous formulons ici et essayons de démontrer l’hypothèse selon laquelle il existerait une rationalité différente de celle sur laquelle repose l’allopathie. Une rationalité qui serait précisément à même de fonder logiquement l’effectivité thérapeutique de l’homéopathie.Trois questions ont guidé notre réflexion : - Comment concevoir la notion de substance homéopathique dans la philosophie de la Nature de Hegel ?- Comment élever le système de la santé à une postulation ontologique ? - Comment rattacher la pathologie à une logique de la liberté ? Pour notre démonstration, nous avons, dans notre première partie, ouvert sur l’histoire de la philosophie, qui nous conduit à penser la substance chez Hegel. Nous avons poursuivi, dans un deuxième temps, notre réflexion par une analyse de la Science de la logique telle que Hegel la développe dans l’Encyclopédie, de manière à poser les bases d’une proposition de rationalité pour l’homéopathie. Proposition que nous avons développée dans la troisième partie, dans laquelle nous posons enfin la question de la liberté de l’individu face à la pathologie.
This work results from attempts at a possible unraveling of the logical grounds of homeopathic medicine. No genuine epistemology of homeopathy has been proposed by S. Hahnemann upon its discovery. Adressing such caveat has been the purpose of our work.To this aim, this research has been dedicated to the identification of a possible philosophical ground capable of explaining the effectiveness of homeopathy. This goal has been pursued considering GWF Hegel’s works as guiding principles and roadmaps because of the systematic rigor of this author’s theoretical productions. Since active molecules are deemed absent from most homeopathic preparations, how can one explain their therapeutic activity?Our hypothesis aimed at providing an answer to this question necessitates the introduction of another reasoning mode, not shaped by conventional pharmacology, i.e. differing from allopathy’s. ▼ Background & Aim: Osteoarthritis (OA) sufferers frequently turn to complementary and alternative medicine (CAM), of which the most common are acupuncture and homeopathy, to improve manageability of their condition. However, there is little extant evidence of effectiveness for these treatments for OA, particularly for homeopathy. One criticism of homeopathic studies is that treatment protocols do not reflect true homeopathy. The nature of true homeopathy is not documented in extant literature. In the current study a mixed methods approach was used to investigate the use of homeopathy for osteoarthritis using a survey, conducted with a parallel acupuncture survey for comparison, follow up interviews with homeopaths and a patient-centred study in a homeopathic department offering treatment on the NHS, in order to inform future studies. Method: The current study involved three phases; (1) A descriptive survey conducted on n=362 medical and non-medical homeopaths and acupuncturists, was used to , investigate practice of the therapies (2) Follow up interview of n=28 of the homeopathic practitioners. (3) A patient-centred study of n=11 patients with OA receiving homeopathy in the primary care setting. Results: (1) Most commonly encountered conditions were chronic diseases. Medical and non-medical acupuncturists practised very different forms of acupuncture particularly in terms of diagnostic techniques used and theoretical underpinning. Homeopathic practitioners used individualised treatments, abiding by classical homeopathy. Differences between medical and non-medical homeopaths included time spent in the consultation (p= 0.01), strength of confidence in homeopathy for asthma (p=0.01), musculo-skeletal (p=0.046) and acute conditions (p=0.01)., and confidence in conventional medicine (p=0.01). There was a belief amongst acupuncturists and homeopaths that the treatments may work on electrodynamic fields in the body. (2) A similar approach was taken by participants during a detailed initial consultation. However, irrespective of medical status, varied approaches were used to identify the remedy, potency, and remedy f6mi, and the source of remedy also varied. Main themes regarding the modus operandi of homeopathy included stimulation of self healing mechanisms and identifying in detail events at the point where the initial health imbalance occurred. Identification of this point together with the patient was considered a potential trigger for the healing process to begin, adding a particular importance to the role of the consultation. (3) OA patients in the primary care setting identified pain or stiffness as the most common primary complaint., with an emotional factor such as anxiety and limitations caused by their condition as a secondary complaint. A desire to reduce their medication or to improve the manageability of their condition was a common theme for interest in receiving homeopathy, with access to NHS homeopathic treatment and perceived safety of receiving treatment from medical doctors being important factors.…

▼ Ultradiluição (UHD) é o efeito de uma solução, diluída acima do número de Avogrado, que na dependência da sua dinamização (diluição com sucussão) induz um efeito celular supressivo ou estimulante, com conseqüente obtenção de uma curva dose-efeito oscilatória. Por outro lado, a 3,3,5 Triiodo-L-Tironina (T3) é o hormônio mais importante na indução e manutenção das mudanças metamórficas dos girinos, nelas incluídas a absorção da cauda. O presente estudo, cego e randomizado, tem como objetivo comprovar que o T3 5.10-24M (10ª cH) altera a apoptose induzida pelo T3 100 nM na cauda de girinos de Rana catesbeiana, in vitro. Foram distribuídos 60 explantes em três grupos: Grupo A: sem o estímulo do T3 em dose farmacológica e em UHD; Grupo B (teste): sob a ação de T3 100 nM e T3 10ª cH (5.10-24 M); Grupo C (controle): sob a ação do T3 100 nM e etanol 70% sem sucussão. A análise estatística da área dos explantes, no primeiro e ultimo dia do experimento, e do índice apoptótico foi realizado através do teste t Student e foi considerado estatisticamente significante quando p<0,05. Embora sem diferenças significativas na área dos explantes do grupo teste e no grupo controle, um maior e significante índice apoptótico foi identificado nos explantes do grupo teste. Este resultado confirma que o T3 na 10ª cH altera a ação do T3 em dose farmacológica. Futuros experimentos serão realizados, com diferentes dinamizações, com o objetivo da parametrização da curva dose-efeito
Ultra High Dilution (UHD) is the effect of a solution, beyond the Avogadro limits, that in the dependence of the applied dinamization (dilution with succussion) elicits a suppressive or a stimulant effect on a living cell, with a consequent generation of an oscillatory dose-effect curve. The entire process of anuran amphibian metamorphosis is under thyroid hormones control, included the complete resorption of the tadpole tail. A random and blind study was performed, with the intent to prove that T3 5.10-24 M (10ª cH) modifies the apoptosis induction of T3 100 nM in Rana catesbeiana tadpoles tail tips, in vitro. 60 Explants were distributed in three ways: Group A: without T3 action, at pharmacological and UHD dose; Group B (test): under the action of T3 100 nM and treated with T3 10ª cH (UHD); Group C (control): under the action of T3 100 nM and treated with ethanol 70% unsuccussed. In order to identify significant differences in the area of the remainder explants, at the first and final day of the experiment, and in the apoptotic index we used a student t-test. Although we didnt find statistical difference in macroscopic tadpoles tail tips area from test and control groups, a high and significant (p<0,05) index of apoptosis in histology was found in explants of test group. This data confirms that T3 10 cH modifies the effect of T3 at pharmacological dose. More studies will be necessary, using different dinamizations, to the parameterization of the dose-effect curve proceeding from these experiments

▼ INTRODUÇÃO: A rinite alérgica é uma condição clínica comum que apresenta sintomas diversos num significante número de pacientes, deteriorando a qualidade de vida daqueles refratários aos tratamentos usuais (anti-histamínicos e corticosteróides nasais tópicos). Apresentando princípios curativos similares, a imunoterapia sublingual e a homeopatia podem reduzir os sintomas e a necessidade de medicamentos na rinite alérgica, embora a eficácia e a efetividade de ambas terapêuticas não sejam ainda suficientemente conhecidas. OBJETIVOS: O objetivo deste estudo foi avaliar a efetividade clínica do tratamento homeopático individualizado prolongado, comparativamente ao placebo, em adultos portadores de rinite alérgica perene. MÉTODOS: Um total de 41 pacientes com rinite alérgica perene foi alocado numa primeira fase duplo-cego e placebo-controlada durante seis meses, sendo tratada com doses sublinguais semanais de medicamentos homeopáticos individualizados ou placebo. Após esta fase inicial fechada, todos os pacientes foram convidados a participar de uma segunda fase controlada aberta, em que receberiam tratamento homeopático pelo período máximo de 36 meses, e os resultados foram comparados com a melhora da fase inicial. O escore dos sinais e sintomas, a necessidade de medicamentos de resgate e a qualidade de vida foram mensurados por questionários e avaliações clínicas pessoais, aplicadas por um mesmo avaliador independente, antes e após cada fase. As doses dos medicamentos homeopáticos e de resgate utilizados, assim como os efeitos colaterais, foram documentados num diário pessoal. Os desfechos clínicos primário e secundários foram, respectivamente, os escores dos sinais e sintomas alérgicos específicos e gerais. Títulos da IgE total foram mensurados antes e após cada fase. RESULTADOS: Após os seis meses da fase placebo-controlada inicial, na análise por protocolo de todos os pacientes incluídos no estudo, não foram observadas diferenças significativas entre os grupos ativo e placebo nos escores clínicos, na utilização de drogas de resgate, na qualidade de vida e nos títulos da IgE total. Entretanto, as análises dos subgrupos da segunda fase mostraram uma crescente e significativa melhora nos desfechos clínicos primário e secundários após 12 meses de tratamento homeopático individualizado, comparativamente à variação de melhora dos mesmos pacientes na fase inicial fechada. Diferença significativa na qualidade de vida foi observada apenas após o segundo ano de tratamento homeopático. CONCLUSÃO: Neste estudo, o tratamento homeopático foi acompanhado de um significante efeito placebo. A efetividade da homeopatia pôde ser observada após 12 meses da terapêutica, apresentando efeito preventivo de longa duração após 36 meses de tratamento homeopático individualizado.

▼ O Objetivo deste trabalho é tratar da Homeopatia e da Medicina Científica, e de sua convivência institucional, considerando principalmente o cenário posterior à década de 1980, quando de sua assimilação pela Medicina Oficial brasileira. O que se pretende demonstrar, fundamentalmente, é que a Medicina Científica e a Homeopatia são essencialmente diferentes, e acima de tudo demonstrar que este caso particular de relações institucionais deve ser entendido sob pelo menos duas perspectivas, de uma epistemológica, e de outra sociológica. Essas duas perspectivas são essenciais para entender a moderna relação institucional entre Homeopatia e Medicina Científica, que acontece num contexto de assimilação institucional da Homeopatia pela Medicina Oficial. Ainda que essa integração nunca possa vir à ser concretizada completamente, essa assimilação é resultado de uma grande mudança no universo simbólico das culturas ocidentais, principalmente dos conceitos e valores ligados ao imaginário da saúde. Neste novo contexto simbólico, a ciência como instituição é ressignificada e e devidamente recontextualizada, em termos de cultura popular e senso comum (universo simbólico da vida cotidiana), de modo que passa a poder acomodar os mais diversos tipos de conhecimentos em seu universo. É sob a perspectiva do processo de racionalização e desencantamento do mundo que essas instituições são analisadas e a compreensão dos universos simbólicos mantidos por cada grupo é o objetivo central desse trabalho.
This study deals with Homeopathy and Scientific Medicine and focus its institutional relationship, especially considering the latter scenario to the 1980s decade, after its assimilation by brazilian Official Medicine. It intends to demonstrate that Scientific Medicine and Homeopathy are fundamentally and essentially different things and, above all, demonstrate that this particular case of institutional relations should be understood in, at least, two perspectives: epistemological, and sociological. These perspectives are essential to understand the modern relationship between Homeopathy and Official Medicine - which takes place in a context of institutional assimilation of homeopathy by the Official Medicine (although this integration could never come to be fully realized). This assimilation is the result of a change in the symbolic universe of western cultures, especially the concepts and values linked to the health imaginary. In this new symbolic context, science, as an institution, is resignified and recontextualized in terms of popular culture and common sense (the symbolic universe of everyday life), allowing to accommodate all different types of knowledge in this new resigninified universe of science. Its from the process of rationalization and disenchantment of the worlds perspective that these institutions are analyzed, and the understanding of the symbolic universes maintained by each group is the central objective of this work.

▼ Esta pesquisa tematiza o conceito saúde na perspectiva dos modelos médicos vitalistas, situando-se no eixo da dimensão doutrina médica das Racionalidades Médicas, e tem como objeto de estudo os vitalismos de Hahnemann e Nietzsche. A partir do levantamento e análise bibliográfica de textos e da abordagem disciplinar histórica e filosófica, teve como objetivos analisar os conceitos de vida, saúde, doença e cura presentes nos pensamentos desses autores, traçar correspondências e explicitar as diferenças dos pensamentos envolvidos. Como apoios teóricos os trabalhos de Canguilhem, Luz e Foucault. Partindo da ênfase na atitude vital do sujeito em seu processo de saúde-doença-convalescença-cura, que ambos pensadores destacam, buscou-se avaliar as hipóteses de o vitalismo hahnemanniano se assemelhar ao nietzscheano e se seria possível afirmar que a busca da grande saúde equivaleria à meta do tratamento homeopático ao contemplar a liberdade do espírito na conquista da ampliação da normatividade vital. Concluiu-se que os vitalismos de Hahnemann e Nietzsche são semelhantes na medida em que as bases de seus pensamentos ressaltam a vida enquanto um jogo de forças e luta, onde enfatizam a irredutibilidade dos fenômenos dos vivos às propriedades físico-químicas; a concepção dos seres humanos como totalidades únicas e singulares nas quais há um jogo de forças atuantes, promovendo diferentes saúdes no mesmo indivíduo, de acordo com as variadas fases da vida; e as hierarquias existentes entre as forças, resultando em análises diagnósticas, possibilidades de intervenção terapêutica e acompanhamento do processo saúde-doença. Correspondem a formas de olhar a vida humana de modo dinâmico, valorizando todos os aspectos físicos, mentais, emocionais e as interações/relações com o meio em que vive. A grande saúde para um espírito que se torna o que é amplia o ideal de cura homeopático ao contribuir para a ressignificação do conceito de saúde como expansão da normatividade vital e da vida como criação de valor, promovendo deslocamentos de perspectivas individual e coletiva, a fim da conquista de uma saúde mais alegre e vital e afirmadora do espírito livre. Ambos os pensamentos podem promover importantes reavaliações do conceito de vida e saúde na sociedade e na medicina contemporâneas, centradas nos valores estatisticamente determinados, generalizantes e normalizadores do paradigma normal/patológico
This research discusses the health concept from the perspective of vitalistic medical models, ranging in the axis of Medical Rationale medical doctrine, having as study object the vitalist studies by Hahnemann and Nietzsche. ▼ Ces travaux ont pour but d’identifier des métabolites dans des teintures-mères (TMs) utilisées en homéopathie et en phytothérapie pour mieux contrôler la qualité des TMs et de mieux comprendre leur mode d’action sur la santé humaine. Nous avons étudié, par une approche de métabolomique globale, 19 TMs et un produit leader : le L52, fabriqués par les Laboratoires Lehning. Premièrement, nous avons utilisé des approches non-ciblées en construisant des banques de données GC-MS et UPLC-MS/MS, afin d’identifier un maximum de molécules dans chaque extrait. Puis, nous avons fractionné certaines TMs et purifié des métabolites inconnus pour une identification fine en HRMS. Nous avons identifié de nombreuses molécules dans chaque TM, montrant que ces dernières sont très riches en molécules pouvant être utilisées pour le contrôle-qualité des TMs et valorisées pour la santé humaine.
Tinctures defined as hydro-alcoholic extracts have been used from centuries in homeopathy and phytotherapy, but their chemical compositions remain still unknown. During my PhD, metabolomics analyses of nineteen tinctures and one leader product, L52, made by Laboratoires Lehning, were conducted using untargeted metabolomic approach. We build GC-MS and UPLC-MS/MS databases to identify a large amount of metabolites. Then, we used semi-preparative HPLC with both UV and mass detection to isolate some compounds from tinctures. We used UPLC-HRMS to obtain chemical formula, a prerequisite for metabolites identification. Finally, we identified a broad range of different metabolites in each tincture, highlighting the metabolic complexity of the TMs. These molecules can now be used for quality-control and valued for a better understanding of these products on human health.

▼ Vaihtoehtolääkinnän suosion kasvu on yleistynyt 70-luvulta alkaen kaikkialla läntisissä yhteiskunnissa. Tutkimuksissa on pääsääntöisesti keskitytty selvittämään vaihtoehtohoitojen käytön yleisyyttä ja käytön syitä. Eri tutkimusten mukaan 20-60 % Keski-Euroopan väestöstä käyttää jonkinlaista vaihtoehtohoitoa. Suomessa vuonna 1992 tehdyssä tutkimuksessa käyttäjien määräksi arvioitiin 46 % väestöstä.
Yhtenä keskeisenä vaihtoehtolääkinnän suosiota selittävänä tekijänä on pidetty vastaanottojen ja hoitosuhteen laadullista erilaisuutta verrattuna lääkärin vastaanottoihin. Vaihtoehtolääkintä on nähty alueena, jossa potilaskeskeiset työskentelytavat toteutuvat. Potilaat raportoivat kokevansa hoitosuhteen osallistavana ja tasa-arvoisena ja potilas nähdään hoitosuhteessa yleensä aktiivisena toimijana. Tutkittua tietoa vaihtoehtolääkinnän vastaanottojen vuorovaikutuksesta ei ole kuitenkaan ollut juurikaan saatavilla.
Tämä tutkimus on ensimmäinen suomalainen väitöstason tutkimus, joka analysoi yhden vaihtoehtolääkinnän muodon homeopaattisen vastaanoton vuorovaikutusta ja keskustelukäytäntöjä. Tutkimuksessa kuvataan minkälaisista toiminnoista ja vaiheista homeopaattinen vastaanotto koostuu. Toiseksi analysoidaan yksityiskohtaisesti hoidosta päättämistä: sitä, millä eri tavoilla hoitopäätöksistä kerrotaan ja vastaavasti sitä, miten potilaat vastaanottavat hoitoaan koskevat päätökset eli millä tavoin he osallistuvat päätöksentekoon. Lisäksi kuvataan miten ja mistä erimielisyys päätöksentekotilanteessa syntyy ja millä tavoilla syntynyttä ristiriitaa ratkaistaan. Punaisena lankana analyysissa on kysymys siitä, miten homeopaattinen hoitoajatus muokkaa vuorovaikutusta.
Tutkimuksen pääaineisto koostuu yhteensä 40 videoidusta vastaanotosta viideltä eri homeopaatilta ja se on kerätty vuosina 2000 ja 2001. Aineiston homeopaattiset vastaanotot ovat kestoltaan keskimäärin 45 minuuttia: vaihteluväli on 15 minuutista kahteen tuntiin. Potilaista naisia on 23, miehiä on neljä ja lopuissa vastaanotolla potilaana on vanhempi lapsiensa kanssa. Potilaiden ikäjakauma vaihtelee muutaman viikon ikäisestä vauvasta vaariin. Yleisimpiä vaivat ovat erityyppiset allergiat ja ihottumat sekä psyykkisen hyvinvointiin ja mielialaan liittyvät seikat. Vertailuaineistona tutkimuksessa on käytetty 20 yleislääkärin vastaanottoa sekä soveltuvaa aiempaa vuorovaikutustutkimusta. Tutkimusmenetelmänä on käytetty keskustelunanalyysia ja tutkimus on osa institutionaalisen vuorovaikutustutkimuksen perinnettä. Keskustelunanalyysi tarjoaa välineitä sekä eritellä vuorovaikutustilannetta puheenvuoro puheenvuorolta etenevänä toimintana ja osallistujien yhteisenä aikaansaannoksena että sitä miten institutionaalinen tilanne ja siihen liittyvät tehtävät muokkaavat keskustelun kulkua.
Homeopaattisen vastaanoton rakenne muistuttaa pitkälti yleislääkärin vastaanoton rakennetta: aloitus, käynnin syyn kuvaaminen, tutkiminen ja haastattelu, diagnoosi, hoidosta päättäminen ja lopetus. Yksi keskeinen ero on se, että erillistä diagnoosivaihetta ei homeopatiassa esiinny. Selitys…

homeopathy and allopathic medicine shaped the culture in which William Faulkner grew up and wrote. Early 20th century America was daily subjected to a variety of conflicting approaches to maintaining or recovering physical, psychological, or spiritual health. The culture was discussing the role of vitalism for good health; the use and dosage of medicine to treat the individual or to treat the disease instead; the interaction of the mind, body, and spirit; the tendency of personality to emerge from inherent biology or acquired traits; the varied explanations for illness; and the legitimacy of doctors, their philosophies, and their remedies.
These competing definitions of psycho-biological health informed Faulkner’s character conceptions and portrayals. In their psycho-biological traits, some of his characters represent concurrently published homeopathic descriptions of constitutions quite accurately. Faulkner’s own life may have offered him opportunities to learn about alternative medicine and generated an interest – along with other medical dissidents – in opposing the newly-garnered authority of modern scientific medicine. It is also likely that Faulkner’s own beliefs about a divinity present in humans and the human capacity to neglect their spiritual essence would have instead supported the older, more romanticized, homeopathic ideas based on mind-body typology to balance an invisible vitalism.
Medicine and literature has recently established itself as an engaging and complementary-paired field in the humanities. This study contributes to the maturing interdisciplinary field by contemplating a famous author and some of his character portrayals from a medical or health perspective. This study of the writer and his fictional people suggests that the unorthodox ideology of
homeopathy continued to play a role in the culture through literature, even as it lacked legitimate authority from the newly established medical community.

▼ Esse estudo tem como objetivo conhecer as características da relação entre a medicina Homepática e a Biomédica enqua nto partícipes de um campo institucional comum, identificando o movimento de aproximação e afastamento entre homeopatas e médicos da biomedicina e identificando os elementos de caráter ideológico, cultural e técnico-cientifico que fazem parte desse processo, segundo o ponto de vista dos profissionais não homeopatas. Por meio de levantamento da atual situação da Homeopatia no campo da saúde no Brasil, identificam-se os municípios com serviços de homeopatia na rede SUS e as faculdades de medicina que desenvolvem atividades relacionadas à homeopatia. Foram selecionados para a pesquisa os municípios com maior produção ambulatorial de consultas homeopáticas de janeiro a novembro de 2003 (Datasus) e entre as faculdades aquelas de reconhecida relevância na formação médica. Foram entrevistados 48 profissionais de saúde (apenas dois não médicos), sendo 20 docentes/pesquisadores de 11 faculdades de medicina, 16 gestores e 12 médicos que trabalham na rede publica em 6 municípios de cinco estados e do Distrito Federal. As análises foram feitas tendo como categorias de referência as concepções de campo social e cientifico de Bourdieu, as concepções de racionalidades médicas de Madel Luz e os conceitos de modelos ou arranjos tecnológicos do trabalho em saúde de Mendes-Gonçalves, de formação de identidade profissional de médico e ideologia ocupacional de Donnangelo e Schraiber. Foram descritos e analisados através dos depoimentos dos entrevistados: os aspectos considerados facilitadores da aproximação entre as duas medicinas, os que dificultam a ampliação da presença da Homeopatia no campo da saúde, as diferentes formas de apresentação das resistências e as suas concepções sobre a Homeopatia. Entre outros, os resultados apontaram que a 6 legitimação profissional, a construção do SUS e a crise da biomedicina são condições favorecedoras da presença da Homeopatia nas instituições. Mas essas condições não garantem espaços de ensino ou assistência, e as iniciativas dos homeopatas ainda dependem de simpatias locais. Os entrevistados valorizam aspectos da pratica homeopática que recuperam a ideologia ocupacional associada ao ideal de boa prática médica: abordagem integral do paciente, a recuperação da dimensão humanística da medicina e os resultados que obtém ao resolver agravos para os quais os recursos da biomedicina são inadequados ou insuficientes. Facilitam a aproximação com a Academia o interesse no desafio cientifico que representa encontrar novos modelos de pesquisa para explicar, comprovar ou medir a ação da homeopatia e a inclusão de ambientes extrahospitalares como locais de ensino. Dificultam essa aproximação as resistências ao desconhecido e as dificuldades de compreender, com a visão da racionalidade cientifica moderna, a lógica em que a homeopatia opera. ▼ Au Mexique et plus spécifiquement dans l’Etat du Morelos, les sages-femmes traditionnelles constituent un groupe hétérogène quant à l’origine culturelle et ethnique, aux pratiques thérapeutiques, à la personnalité, à la formation reçue, aux honoraires perçus…. Personnages à la croisée des savoirs, les sages-femmes puisent leurs connaissances dans les traditions médicales náhuatl ; elles ont également reçu une formation académique de la part de la médecine occidentale.Elles pratiquent un panachage de techniques appartenant à des traditions médicales savantes ou populaires aux systèmes de référence a priori opposés. Elles démontrent des capacités d’intégration de connaissances issues de ces différentes traditions. Leur intérêt pour les thérapies dans la mouvance actuelle des médecines dites alternatives dénote aussi leur curiosité d’esprit et la souplesse de leurs pratiques. Au plan thérapeutique, les sages-femmes occupent un espace bien spécifique avec ses caractéristiques propres dans l’accompagnement de la femme au cours du processus de la maternité,de la conception à la période puerpérale. Les bains, les massages, l’alimentation et le remède végétal constituent une de leurs principales ressources thérapeutiques; l’importance de leur pharmacopée varie d’une dizaine à une centaine de plantes. Le système thérapeutique des sages-femmes est fondé sur un invariant : le concept du chaud et du froid.
In Mexico, more specifically in the state of Morelos, traditional midwives compose a heterogeneous group with regards to their cultural and ethnic origins, their therapeutic praxis, their individual personalities, their training, their fees, etc. At crossroads of knowledge sources, midwives draw their competence from Náhuatl medical traditions and benefited from an academic training from occidental medicine as well. They resort to a number of diverse techniques belonging to wise and popular medical traditions which often refer to a priori conflicting systems. Midwives from Morelos demonstrate their capacity to integrate the lore from those different traditions. Their interest towards the therapies relating to the sphere of the present so-called alternative medicines also indicates their openness and the flexibility of their praxis. From a therapeutic point of view, the midwives assume aspecific role with clear characteristics in accompanying women all along the maternity process, from conception to puerperal period. Baths, massage, nutrition and herbal remedies constitute their main therapeutical resources; the range of their pharmacopoeia varies from a dozen to about a hundredplants. The therapeutic system of the midwives is based on the invariant dichotomy of hot and cold.
