▼ This research consisted of three projects. The first study identified 10 attributes of an effective
manager within the public health sector. While much has been written about the attributes of
effective managers, little is known about how these attributes fit in the public sector, or in the
public health sector more specifically. The participants in the first study were all NSW Health
employees. Participants were surveyed, and the results identified 10 attributes of an effective
manager from a list of over 630 possible attributes. They were encouraging, transforming,
efficient, (not) insecure, (not) evasive, courageous, (not) expedient, reasonable and
trustworthy. These 10 attributes were then used in the second study designed to ascertain the
prevalence of these attributes among staff at an exemplar health care facility (Canterbury
Hospital). Around 3% of staff were identified by other staff as displaying these characteristics
much more than the average person. Interestingly, these qualities were not associated with
whether or not the employee was already a manager of some type. These ratings were also
examined for possible bias. While staff could be overly kind or harsh in their ratings of others
(which could be corrected for with the methods developed in this research), no bias was
evident based on the gender or relative seniority of the person doing the rating or being rated.
The third study was a focus group, which consisted of senior executive staff of Sydney Local
Health District (SLHD). The participants of the focus group were asked to identify any barriers
that were associated with the recommendations from the second study and the potential use of the tool used in the second study. Barriers that were identified included concerns in relation
to rating other staff and issues that could affect the use of the tool. The focus group participants also suggested changes that could be made to the tool and identified potential
uses of the tool. These results can potentially be used to identify management talent of the
future, further develop existing management, and distinguish the management needs of the
public health sector from the general management literature.

▼ Aim: The aim of this study was to examine the perception of spirituality that was held by senior
managers in one area
health service in NSW. Method: Semi-structured interviews were conducted with 18 participants at the senior management level such as Executive Director and Director of Nursing level. Each interviewee was given a statement defining spirituality and a copy of 8 questions to review a number of viewpoints about delivery of spiritual/pastoral services in their facility. Each interview was audio recorded. Analysis: Thematic Analysis was utilised as the study began with no hypothesis to test.Results: 18 people were interviewed. Each participant acknowledged that Pastoral Services were delivered in their facility. No one felt that it was an unnecessary service. Most felt that Spirituality was a concept that needed to be introduced into the training of the clinical disciplines. All felt that it was not their facilitys responsibility to train people in Spirituality. Most felt that it was either the training institutions, such as universities, or the professional organisations which should do the training. There was a strong feeling that either NSW
Health or the Area
Health Service should set guidance but they wanted the ability to adapt to their particular site. Besides liturgy, there was little awareness of what chaplains actually did. Although internationally, and in states such as Victoria, there has been a move away from denominational chaplaincy to discipline specific chaplaincy, there was no awareness of this trend, but neither was there any opposition to it. ▼ This dissertation describes original research that explores, for the first time, the historiography and roles of Australia’s Health Information Managers (HIMs). The ecology of health information management was examined to show how key societal and political influences have shaped the profession, and to identify the implications for HIMs of evolving organisational arrangements. In interrogating the profession’s roles, a qualitative approach was used, incorporating face-to-face, in-depth interviews with an Australia-wide cohort of HIMs and ethnographic observation or in-depth interviews in three case studies.
Scaffolded by the Foucaultian centrality of the medical record to medicine and the modern hospital, the drivers and contestations that shape HIMs’ work hinge upon: standardisation; scientific medicine; bureaucratisation; technologisation; the hegemony of healthcare managers; the forces of the risk society exemplified in healthcare resourcing, performance, and quality of care accountabilities; health consumers’ interests; and a nascent commodification of health information.
The findings situate HIMs as a metaphorical keystone species in the health information ecology: they populate a critical interface between uncertainty and the indeterminacy of absence of information, and the clarity of knowledge and “truth”. They struggle constantly to impose order. Drawing upon Durkheim and Mauss, this is seen to be effected through the HIMs’ purifying and transforming the ‘mess’ of the raw data of the performance of healthcare into the elegance and sacredness of interpretable, mobile codes and facts. They constantly traverse boundaries, negotiate, and form alliances with human and non-human actors. They structure and legitimate bureaucratic relationships to facilitate health surveillance, and commodify patients and their diseases. Their infrastructuring and technologising work thusly translates, classifies and qualculates the messy and the unclassifiable into the workable and the portable.
The overarching problematic of the contemporary Australian health information management profession is three-fold. First, paradoxically and notwithstanding the centrality of their scientific work, the HIMs’ ordering across the spectrum of information in healthcare is largely invisible. Second, technology proves a temperamental ally that demands constant updating. Third, the chasm between their dichotomous worlds of disorder and order is ever-widening, amid: increasing systemic dependence on their information production; intensifying environmental risk; multiplying standards; and increasing volumes of data. It is concluded that the profession has emerged as an organising device central to healthcare managerial, research, and medical interests. Occupying a trustworthy role, the HIMs’ challenges lie in determining what is health information, governing its production systems, and (re-)constructing, reconciling, and disseminating critical informational outputs as the official facts of both patient and healthcare system.

▼ Background: Power relations among district health managers border on which district health manager has formal as well as any informal powers to do what, and to what extent, and the manifestation of power among these managers. A healthy relationship between health institutions at the district level is dependent on the clarity of formal and informal rules governing the arrangements of these healthcare institutions. These rules translate into the power structure for district health managers in the discharge of their duties. However, in practice, these formal power arrangements are likely to be distorted by informal powers.
Objective: The study explored how power relations between district directors and medical superintendents were likely to affect the delivery of healthcare services, such as supervision of health centre clinical care from the district hospital and the patient referral system, which are pivotal to the quality of clinical care delivery in rural districts of Ghana.
Methods: The study was conducted in three districts in Ghana; Bongo, Kintampo North and Juaboso, using the constructivist / interpretivist approach. Purposive sampling technique was used to select study sites and the study participants. The study used participant observation, conducted 11 interviews, 9 focus group discussions (with 61 participants) and documentary review for data collection across the three case study districts. Transcription of the voice-recordings was done verbatim, cleaned and imported into the Nvivo version 11 platform for analysis. Data was analysed using the inductive content analysis approach. Ethical clearance was granted by the ethics review committee of Ghana Health Service.
Findings: The study revealed that, the district directors derive legitimate powers from their positions as heads of the district health services. The medical superintendents also have legitimate powers to oversee and supervise district hospitals’ activities. However, the study revealed that informal sources of power contribute to how district directors relate to medical superintendents in practice. These informal power sources are broadly described as financial and medical dominance. The study revealed that health centres organise and deliver a wide range of services. It was observed that health centre services delivery are supported by a borrowing system between district health institutions, knowledge sharing between health centres and district hospitals, and use of multi-task approach by healthcare providers; to minimise staff inadequacy challenges in the health centres in order to make clinical care services accessible to the rural communities. The study also found that power relations between district directors and medical superintendents affect the pivotal role of district hospitals in the delivery of clinical care at the health centre levels. It was reported that health centre clinical care is inadequately supervised and health centres delay in referring patients to district hospitals for further management.

▼ The workplace is an important avenue for supporting workforce
health. Employees spend the majority of their time in the workplace. Visible support for a healthy lifestyle from leadership, direct supervisors, and coworkers may positively impact employees’ perception of support for
health, as well as other work-related measures. The goal of this dissertation is to validate an instrument measuring employees’ perception of workplace support for
health, identify strategies that will engage
managers and supervisors to support employees’ participation in wellness programs, and explore the potential benefits of supporting employees’ wellness efforts in the workplace. In the first aim, we test the reliability and validity of a five-item scale measuring employees’ perception of workplace support for
health (from leadership, direct supervisor, and coworkers). This study uses survey data collected from small workplaces in King County, Washington at three time points from 2013 to 2017. We demonstrate that the instrument is reliable, detects change in employees’ perceptions over time, and is valid among a population of small workplace employees. When implementing workplace
health promotion interventions, researchers and practitioners can use the scale to assess employees’ perception of workplace support for a healthy lifestyle. In the second aim, we evaluate managers’ barriers and facilitators to supporting employee participation in the Washington State wellness program using an exploratory sequential mixed methods study design. We interviewed and surveyed state employees in management positions (executive, middle, and line), at four Washington State agencies located in the greater Olympia area. The results suggest that
managers support the wellness program, but they also face challenges with accommodating employees’ participation due to workload and scheduling. About half the
managers receive support from the manager above them, and most have not received training on the wellness program. We identified several strategies that may assist
managers in supporting their employees’ participation in wellness programs: the provision of training, formal expectations, and encouragement to provide support for employees’ participation. In the third aim, we explore the direct and indirect associations between perceived managers’ support for wellness (actions taken by
managers to support participation in wellness programs) and employees’ engagement at work. We conducted a path analysis using employee survey data from two Washington State agencies located in the greater Olympia area. Our findings suggest managers’ support for wellness has moderate indirect associations with employees’ engagement at work. We found the relationship between the perception of managers’ support for wellness and employees’ engagement is mediated by other variables, such as perceived respect and feedback, and agency support for
health. The perception of managers’ support for wellness has indirect benefits for employees’ engagement at work.

Health Background; The presence of high-quality, motivated
managers is a key aspect of
health system performance, but also one of the most difficult inputs to guarantee, particularly in resource-limited environments. Frequency of turnover of key managerial staff has also been associated with poor system performance. Methods; A descriptive retrospective longitudinal study was carried out of all
managers of the eighty nine (89) critical management posts in the Khartoum State Ministry of
Health (KSMOH) who were at their post or posted between January 1, 2005 and December 31, 2010, to assess the frequency and severity of senior
health manager turnover. Results; A total of 328 postings of
managers were made over the 6-year period in the 89 key
health management positions studied. 263(80%) were medical doctors, 28(9%) technicians (public
health officers, lab technicians, statisticians), and 37 (11%) were other cadre however only 33 (10%) of them were females. A total of 239
managers left their positions over the study period translating to an average turnover rate of 48% for all cadre. The average turnover rate by cadre over the 6 year period was 53% for medical doctors, 30% for
health technicians and 35% for other cadres. Among all
managers who left positions, 85 (35%) were at their post for 1 year or less. This group was overwhelmingly comprised of medical doctors (92%). 155 (65%) were in post for less than 2 years, 139 (90%) of them were medical doctors, 12 (8%) were other cadre while only 4 (3%) were
health technicians. The average employment period for all
managers was approximately 2 years. On average, doctors stayed for ~1 year less than technicians and other cadres included in this sample. Conclusion; A high and increasing turnover rate over the study period was observed among the key
health managers in (KSMOH). Doctors are the most likely
managers to leave their positions, therefore moving towards to public
health and management-oriented leadership style at
health departments may improve the stability and continuity of the national
health system in Sudan.

There is a growing demand for reliable small area health statistics by governments, researchers, health managers and programmes due to continuous surge in communicable and non-communicable diseases in developing countries. Countries’ routine health information systems data (facility-based data) have been identified as a key system building block of primary health care which produces information in greater detail to meet the high data demand. However, the huge amount of facility-based data collected daily, aggregated and reported monthly into the national health information systems have rarely been used for evidence-based decision-making. Instead, household surveys have become the primary source of data for local area public health decision-making. Household surveys have been designed to measure indicators at the regional and national levels only, and therefore are unable to produce reliable estimates for small areas due to the small sample sizes. Furthermore, household surveys are mostly conducted once every 5 years or more, therefore health managers and policy-makers must wait up to five years or above for Demographic and Health Surveys or Multiple Indicator Cluster Surveys to be conducted to enable them to assess what progress has been made or be able to monitor implementation of programmes and interventions which must be conducted on monthly, quarterly and yearly basis.
Poor health facility reporting, inadequacy of methods, inadequacy of population denominators for calculating population-based rates and lack of external consistency checks were among the key constraints identified to be responsible for the limited use of health facility-based data for public health decision-making.
Objective
The main objective of this study was to explore the usage of geospatial and statistical techniques to address issues associated with limited use of facility-based data and to evaluate its value for small area evidence-based decision-making.

Method
This study is both a descriptive cross-sectional and an exploratory one. It made use of a number of datasets including: health facility visits data for malaria and diarrhoea recorded by all health facilities offering outpatient services in Greater Accra Region (GAR) in 2014, population and housing census data, Ghana demographic and health survey data (GDHS), climate data, health facility locations and base map of GAR. Completeness of facility reporting rate of the region and the districts were determined. Multiple Imputation by Chain Equation was used to fill the data gap created by non reporting facilities. Health facilities locations, public and private, have been geo-coded and linked to the enumeration boundaries of 2010 population census data to provide the denominators for small area disease rates calculations. ▼ Por muito tempo, os estudos relativos aos gerentes foram interditados, em parte, devido à imagem de representantes e defensores dos interesses das organizações que os mesmos possuíam perante a sociedade. Atualmente, com a reestruturação produtiva, a categoria gerencial, antes preservada, passa a ser alvo da instabilidade e da intensificação do trabalho. Tal situação é particularmente evidente no caso dos gerentes do setor financeiro, que, devido à concorrência acirrada que caracteriza esse setor, são submetidos a uma pressão constante por resultados. Tais considerações motivaram o presente estudo, que tem como objetivo geral analisar a relação entre o trabalho e a saúde de gerentes de bancos públicos e privados, da cidade de João Pessoa (Pb) e como objetivos específicos identificar as formas de inserção e de formação profissional na carreira gerencial; investigar o conteúdo e as especificidades do trabalho gerencial; delinear as condições e a organização do trabalho dos gerentes de banco; identificar as fontes de sofrimento/ prazer no trabalho, bem como a dinâmica do reconhecimento; analisar os modos de gestão adotados pelos gerentes na relação trabalho e vida privada; e descrever os processos saúde doença dos gerentes, relacionando-os com sua atividade. O aporte teórico desse estudo reúne contribuições oriundas da Ergologia; Psicodinâmica do Trabalho; Ergonomia da Atividade e Clínica da Atividade. Quanto ao Método, privilegiou-se a entrevista semi-estruturada, tendo em vista o interesse em acessar as vivências, singularidades e perspectivas dos gerentes de banco em relação ao seu trabalho. Participaram desta pesquisa 16 gerentes de bancos públicos e privados da cidade de João Pessoa, que ocupavam gerências gerais e intermediárias. A análise dos dados foi conduzida através da análise de conteúdo temática. No que se refere aos Resultados, identificamos que a gestão por metas regula a atividade dos gerentes diariamente e, para alguns deles, esse monitoramento de prazos e resultados chega a ser sufocante. Os gerentes apontaram também como fontes de sofrimento a falta de reconhecimento por parte dos superiores e da sociedade; o relacionamento ruim com os pares e/ ou equipe; as demissões e aspectos decorrentes da Organização de Trabalho (jornada de trabalho, pressão por resultados, decisões sobre as quais não se tem autonomia e falhas no sistema do banco). Como principais fontes de prazer no trabalho gerencial, eles ressaltaram o reconhecimento por parte dos pares, dos clientes e da sociedade; e o salário. Nesse contexto de dificuldades, contradições e imprevistos, os pares surgem como instrumento valioso de aprendizado e tutoria, por meio da troca de seus saberes práticos. ▼ The objective of this research is to evaluate Human
Resources managerial/development effectiveness (HRM/DE) of
frontline
managers from the perspectives of
managers themselves and
stakeholders in the public
health sector (PHS) of Ghana. The study
did this through the development of a conceptual framework which
combined the use of integrated organisational and management
theoretical perspectives and contextual variables. The study
employed the mixed methods research methodology which combined both
empiricism and post post-positivists’ views with critical realism
as the underpinning philosophy. A total of 18 district directors of
health, from two regions were purposively sampled and interviewed
utilising an in-depth open ended questionnaire through the
discussion. Additionally, key policy makers were interviewed and
focus group discussions held and a structured questionnaire
completed by another group of employees, who assessed managers’
capabilities. Discourse analysis was used for the analysis with the
aid of Nvivo 7 for the qualitative material whilst quantitative
data were analysed using simple descriptive statistics. Findings
were triangulated using Marquart and Zercher’s (2000) cross-over
track analysis framework.Findings show research questions were
answered. Majority of district directors lack managerial
competencies; are less interested in HRM/D activities, less
confident of their human resource skills and less sure of the
political and representational skills required of
managers. In
particular, it is necessary to take account of the political
structure of the PHS of Ghana; significant differences exist in
power, individual or group interests, values, assumptions and
expectations. However, most district directors have tried to
indigenise HRM/D practices as a way of motivating and retaining
staff. Core Human resources managerial competencies from the
perspectives of the three sampled groups have been compiled. It is
the first time such a study has been conducted in the PHS of Ghana
and which has therefore made inroads in the existing literature and
has contributed to HRM/D literature information in Africa
particularly Ghana. It also paves the way for understanding
management in the African context and perspective and specifically
in
health care settings. This study has gone beyond the two groups
of respondents and proved that the use of multiple respondents
generates rich findings and unveiled what would normally have not
been possible if single respondents were used.

▼ Syftet med denna studie var att beskriva erfarenheter av socialt stöd bland mellanchefer i hälso- och sjukvården. Bakgrund Mellanchefer i hälso- och sjukvården har ett betydelsefullt och krävande uppdrag. Stöd i olika former kan reducera risken för ohälsa orsakad av stress i arbetet och organisationens stöd är viktigt. Genom att stärka mellancheferna erhålls en positiv effekt för både patienter och medarbetare. Metod Studien baserades på semi-strukturerade intervjuer med nio mellanchefer, vilka arbetade inom slutenvården vid två sjukhus i norra Sverige. Intervjuerna spelades in, skrevs ut ordagrant och analyserades med kvalitativ innehållsanalys. Resultat Analysen resulterade i tre huvudkategorier; Erfarenheter av fungerande stöd inom organisationen, erfarenheter av bristande stöd inom organisationen och andra faktorer av betydelse för erfarenheter av stöd. Sammanfattning De flesta mellanchefer hade goda erfarenheter av stöd i organisationen, men påtalade behovet av en tidig och mer strukturerad introduktion när de påbörjade sitt uppdrag. För att förebygga ohälsa föreslås förbättrad introduktion och tidiga utbildningsinsatser, förbättrad feedback från överordnad, samt utvecklat mentorskap. Implikationer Studiens resultat kan användas i syfte att sträva efter att bibehålla det stöd som fungerar och förbättra bristande stöd för mellanchefer i hälso- och sjukvården.

▼ Biståndshandläggning är ett högst aktuellt ämne som diskuteras världen över och som forskas mycket om i Sverige. Syftet med studien var att undersöka hur biståndshandläggare upplever sin egen hälsa av att fatta beslut. Åtta biståndshandläggare intervjuades, varav två var män och sex kvinnor med ett åldersspann på 32–42 år. Sex av dessa hade en socionomexamen och två av de en beteendevetenskaplig kandidatexamen på högskolenivå. I analys av resultatet identifierades fyra grundkategorier: negativa upplevelser, stressupplevelser, positiva upplevelser och förhållningssätt. Resultatet visade att de biståndshandläggare som hade låg arbetserfarenhet upplevde stress genom handläggningsprocessen medan de som hade flera års erfarenhet inte påverkades i lika hög grad. Studien ligger till grund för framtida studier där framtida forskning kan breddas med hjälp av ett större urval för att kunna få en insyn i bredare perspektiv. Undersökningens slutsats var att individens hälsa påverkas genom beslutsprocessen både positivt som negativt.

▼ The aim of this study was to examine rural nurse
managers' experiences with mentoring once assuming their new management role. This research study used qualitative methods to examine mentoring experiences in relatively new and inexperienced nurse
managers in the rural setting. Social Learning Theory was used as the theoretical framework, and the conceptual framework consisted of Stewart and Krueger's (1996) concept analysis ofmentoring in nursing. Stewart and Krueger's concept analysis identified the following six essential attributes of mentoring in nursing: a teaching-learning process, a reciprocal role, a career development relationship, a knowledge or competence differential between participants, a duration of several years, and a resonating phenomenon. Ten nurse
managers working in six different rural hospitals in southcentral Kentucky were interviewed for this study. While mentoring had occurred for participants, it occurred at varying degrees. Six of the participants reported mentoring relationships consistent with Vance and Davidhizar's (1996) definition of mentoring used for this study. Three participants reported being mentored once assuming their new role; however, further discussion with the nurse manager revealed a relationship more consistent with receiving training or having a temporary preceptor. One participant understood the meaning of mentoring, but she was unable to identify a mentor since assuming her role as nurse manager. Data analysis of this study's participant responses revealed several common themes: difficult transition to management role; perceptions of having a mentor; desirable traits of a mentor; investing time in people and training; and discovering individual leadership style on the job. Responses showed the importance of mentoring and that it was valued by new
managers. Further research in the needs of new
managers could help hospitals do more to help the new manager feel supported, which would contribute to the manager's longevity in the position.

▼ Tutkimuksen tarkoituksena oli kuvata ja analysoida, mitä on sosiaali- ja terveystoimessa työskentelevien keski- ja lähijohtoon kuuluvien esimiesten kokema keskinäinen yhteisöllisyys.
Tutkimus toteutettiin kolmessa vaiheessa. Tutkimuksen ensimmäisessä vaiheessa suoritettiin systemaattinen kirjallisuuskatsaus, jonka avulla selvitettiin, miten yhteisöllisyyden ilmenemistä työelämässä on tutkittu ja miten työelämän yhteisöllisyys näyttäytyy olemassa olevan tutkimuksen valossa. Tutkimuksen toisessa vaiheessa kehitettiin tutkimuksessa käytetty kyselylomake, jonka laadinnassa hyödynnettiin aikaisempaa kirjallisuutta ja aikaisempien kyselylomakkeiden kysymyksiä suoraan tai osittain muunneltuina. Tutkimusta varten kehitetyn kyselylomakkeen osa-alueet pohjautuivat kirjallisuuskatsauksessa suoritetun sisällönanalyysin tuloksena muodostuneeseen käsitykseen työelämän yhteisöllisyyttä selittävistä tekijöistä ja yhteisöllisyyden tunteen seurauksista. Työelämän yhteisöllisyyttä selittäviksi tekijöiksi sisällönanalyysin tuloksena pelkistyivät yksilö-, yhteistyöja vuorovaikutus- sekä kulttuuritekijät. Yhteisöllisyyden tunteen seuraukset työyhteisössä ilmenevät työntekijän sitoutumisena, työtyytyväisyytenä ja työhyvinvointina sekä parempana työn laatuna. Tutkimuksen kolmannessa vaiheessa tutkittiin työelämän yhteisöllisyyttä selittävien tekijöiden yhteyttä työtyytyväisyyteen ja työhyvinvointiin sekä organisaatioon sitoutumiseen ja palvelun laatuun esimiesten kokemana. Kyselylomakkeen sisältämien avointen kysymysten avulla selvitettiin yhteenkuuluvuuden tunnetta edistäviä ja estäviä tekijöitä esimiesyhteisössä esimiesten kokemana. Kyselylomakkeen palautti 136 (n = 241) sosiaali- ja terveystoimessa työskentelevää johtajaa. Vastausprosentti oli 56.
Tulosten mukaan yhteisöllisyyttä selittävistä tekijöistä työn kokeminen merkityksellisenä, avoin tiedonkulku organisaatiossa ja omalta esimieheltä saatu arvostus ovat yhteydessä työtyytyväisyyteen ja työhyvinvointiin sekä palvelun laatuun esimiesten kokemana. Vastaavasti työn kokeminen merkityksellisenä ja avoin tiedonkulku organisaatiossa kuten myös kollegoiden keskinäinen yhteenkuuluvuuden tunne, luottamus ja arvostus sekä avoin vuorovaikutus työhön liittyvissä asioissa ovat puolestaan yhteydessä organisaatioon sitoutumisen kanssa. Esimiesyhteisössä yhteenkuuluvuuden tunnetta edistävinä ja estävinä tekijöinä ovat kollegoiden välisen vuorovaikutuksen määrä ja laatu, kollegiaalinen tuki ja kannustus, organisaatiorakenne ja yhteiset käytännöt sekä johtamiseen liittyvät tekijät. Saatujen tulosten mukaan avoimuuteen, keskinäiseen luottamukseen, kunnioitukseen ja arvostukseen perustuva vuorovaikutus edistää yhteenkuuluvuuden tunnetta ja siten myös yhteisöllisyyttä esimiesyhteisössä. Sen sijaan epäluottamus, kilpailu, kateus ja puutteellinen arvostaminen ovat yhteenkuuluvuuden tunnetta ja yhteisöllisyyttä estäviä tekijöitä. generalizada de que esta temática é fundamental para o sucesso das organizações.
Hoje em dia, ao enfermeiro chefe não basta ser gestor, deve ser um líder que procura implementar
mudanças e inovação, mantendo a satisfação e a motivação dos seus subordinados. É genericamente
assumido que a liderança em contexto organizacional procura, sobretudo, o desenvolver as capacidades
e a confiança dos colaboradores.
Esta dissertação aborda o conceito de liderança focando-se nas perspetivas das chefias de enfermagem,
de uma organização de saúde, sobre os estilos de liderança desenvolvidos. O tema assume particular
interesse pela constante necessidade de inovação e desenvolvimento na área da saúde associada a uma
contenção de custos que se vive, pelo que uma liderança eficaz se torna crucial.
O presente estudo adota uma metodologia qualitativa, assente num estudo de caso, atendendo ao perfil
exploratório da investigação. O método de colheita de dados privilegiado foi a entrevista semiestruturada,
aplicada a 16 enfermeiros em funções de chefia da mesma organização de saúde hospitalar.
As conclusões deste estudo oferecem um desafio às chefias de enfermagem, no sentido de colocarem
de parte visões tradicionalistas de liderança e de, em contrapartida, promoverem mudanças nos seus
comportamentos e atitudes e de adquirirem competências necessárias a uma liderança eficaz.

▼ Tuberculosis is a health threat, globally, in Africa, South Africa as well as in the North West Province. Although a number of positive interventions have been implemented, like the introduction of direct observation treatment strategy, still tuberculosis remains a threat. This may be due to the fact that while interventions to fight tuberculosis have been formulated and implemented, the most important resource in the Department of Health, which are nurses. Nurses working in the tuberculosis programme who play a vital role in the implementation of the health strategy are left behind and not given the proper support that they need to ensure the implementation of the health strategy. Nurses need to receive physical, emotional and social support from management so that they can give quality care to their patients.
The purpose of this study was to explore the support from management to nurses working in the tuberculosis programme in the primary health care facilities at the Matlosana sub-district so as to make recommendations to management with the aim of improving the nurses’ work life and consequently rendering quality care to the tuberculosis patients.
The research was conducted in the Matlosana sub-district in the North West Province of South Africa. A qualitative research design was used to explore and describe the support by management to nurses working in the tuberculosis programme in the primary health care facilities. A purposive voluntary sampling method was used to select participants who met the set criteria. In depth Semi structured interviews were conducted. Data was captured on an audio recorder, and transcribed verbatim. The researcher and the co-coder analysed the data after data saturation was reached. A consensus was reached on the categories that emerged.
The results showed that most facility managers lack knowledge about tuberculosis making it difficult for them to support nurses working in the tuberculosis programme. The lack of support resulted in the arousal of feelings such as frustration, feeling undermined, feeling unnoticed and unappreciated. It also resulted in resistant behaviours such as underperformance, loss of interest in their work, wanting to leave to where they will be supported and reluctance to take annual leave due to fear of piling work. However, few participants reported supportive experiences from both their facility managers and from the tuberculosis coordinator. Recommendations were made for the field of nursing education, community health nursing practice and nursing research with the aim of improving the nurses’ work life and consequently rendering quality care to the tuberculosis patients.

▼ Vårt syfte med denna studie är att belysa de verktyg som kan erfordras för att enhetschefer ska kunna förmedla sitt ledarskap till sina medarbetare. Detta på grund av att medarbetarna inom kommunen inte är helt belåtna med ledarskapet från kommun eller sina chefer, vilket framgår i den medarbetarundersökning som genomfördes 2014 av kommunen. I samma medarbetarundersökning framkom det likaså att sjukskrivningar bland medarbetarna i kommunen kan beror på ledarskapet. Därför har vi valt att undersöka en del av kommunens enhetschefer och hur de ser på sitt eget ledarskap samt vad de kan göra för att få sina medarbetare att må bättre och förebygga sjukskrivningar. Då tiden inte fanns till att undersöka alla enhetschefer inom den valda kommunen, valdes vård och omsorgsförvaltningens enhetschefer ut för vår egen enkätundersökning. Resultatet av vår enkätundersökning utföll sig på så sätt att enhetscheferna erfordrar mer tid att ägna sig åt sina medarbetare och att administrativt stöd fordras för att underlätta enhetschefernas arbetsmiljö. Om enhetscheferna erhåller mer tid till att spendera med sina medarbetare kan det skapa ett starkare psykologiskt band emellan dem, vilket gör att tilliten från medarbetarna till enhetscheferna stärks. När tilliten stärks vågar medarbetarna vända sig till sina enhetschefer för att berätta hur de mår. Genom att plocka bort de administrativa uppgifterna och i stället ge dem till en assistent får enhetscheferna mer tid till att spendera med sin personal och utifrån detta kan de eventuellt få en översikt av hur verksamheten fungerar. Översikten gör att de kan fatta bättre strategiska beslut för deras verksamhet och medarbetare.
Our purpose with this study is to highlight the tools that may be required for the unit managers that they should be able to convey their leadership to their employees. This is due to the employees of the municipality are not completely satisfied with the leadership of the municipality or its managers, as evidenced in the employee survey conducted in 2014 by the municipality. In the same employee survey revealed also that the sick leave among employees in the municipality can depend on leadership. Therefore we have chosen to examine a part of the municipal unit managers and how they see their own leadership, and what they can do to get their employees to feel better and prevent sickness. When the time was to examine all unit managers within the chosen municipality, was elected the health and care administration unit managers out for our own survey. The results of our survey precipitated itself so that unit managers require more time to devote themselves to its employees and administrative support necessary to facilitate the unit managers. If the unit managers will receive more time to spend with their employees, it can create strong psychological ties between them, allowing the trust from employees to the unit managers will be strengthened. ▼ Tutkimuksen tarkoituksena oli kehittää validi ja reliaabeli arvostavan johtamisen mittari sekä kuvata, mitä arvostava johtaminen on, miten se toteutuu terveydenhuollon organisaatioissa ja mitä yhteyttä sillä on työhön sitoutumiseen, työstä lähtemiseen, ammatista lähtemiseen, urakehitykseen ja joustavuuteen työssä.
Tutkimus oli kaksivaiheinen. Ensimmäisessä vaiheessa kehitettiin arvostavan johtamisen mittari käsiteanalyysin, kirjallisuuskatsauksen ja delfoi-tutkimuksen avulla. Mittari pilotoitiin ja esitestattiin empiirisesti. Käsiteanalyysin (n = 14) avulla kuvattiin arvostavan johtamisen käsitettä. Kirjallisuuskatsauksen (n = 20) avulla tarkennettiin arvostavan johtamisen kuvausta siitä, mitä arvostavalla johtamisella tarkoitetaan ja miten arvostavaa johtamista on tutkittu. Delfoi-tutkimuksen ensimmäisessä vaiheessa nuoret aikuiset (n = 68 opiskelijaa ja n = 8 nuorta hoitajaa) kirjoittivat kehyskertomuksellisia esseitä (n = 76), joissa he kuvasivat näkemyksiään unelmiensa työstä ja johtamisesta. Näin pyrittiin löytämään täydentävä näkökulma arvostavaan johtamiseen aikaisemman kirjallisuuden lisäksi. Käsiteanalyysin, kirjallisuuskatsauksen ja esseeaineisto analysoitiin sisällön analyysilla. Aikaisempaan tutkimukseen perustuvan kirjallisuuskatsauksen mukaan arvostava johtaminen on ammattitaidon arvostamista, vuorovaikutusta ja yhteistyötä henkilöstön kanssa. Tämän tutkimuksen esseeaineiston mukaan arvostava johtaminen on suunnitelmallista johtamista, tasa-arvoisuutta, osaamisen arvostamista ja työssä jaksamisen edistämistä. Delfoi-tutkimuksen toisessa vaiheessa nuorten aikuisten kirjoittamia esseitä hyödynnettiin poimimalla niistä väittämiä mittariin. Esseitä kirjoittaneet arvioivat kaksi iteraatiokierrosta käsittävällä likert-asteikollisella tutkimuksella, vastasivatko väittämät heidän käsityksiään arvostavasta johtamisesta. Väittämät pilotoitiin hoitotieteellisessä asiantuntijaryhmässä (n = 8) ja esitestattiin kahdessa terveydenhuollon organisaatiossa (n = 229 henkilökuntavastaajaa, 95 johtajaa).
Kehitetyssä mittarissa (AMS 1,0) on 5-portainen Likert-asteikko. Mittarin väittämät (83) jakautuvat suunnitelmalliseen johtamiseen (21), tasa-arvoisuuteen (16), osaamisen arvostamiseen ja työssä jaksamisen edistämiseen (23). Henkilöstön ja johtajien kysymykset olivat sisällöltään samoja, mutta johtajakyselyssä väittämät muutettiin johtajalähtöisiksi. Sitoutumista tutkittiin Allenin ja Mayerin sitoutumismittarin tunneperäistä sitoutumista mittaavilla väittämillä (8). Kyselylomakkeessa oli myös vastaajaa itseään ja hänen nykyistä työtään koskevat taustakysymykset, henkilökunnalle 21 ja johtajille 28, mukaan lukien ammatista lähtemistä (3), työstä lähtemistä (2x13), urakehitystä (7) ja joustavuutta työssä (10) koskevat kysymykset.
Toisessa tutkimusvaiheessa kuvattiin arvostavan johtamisen toteutumista kyselyn avulla (n = 2671 henkilökuntavastaajaa, 426 johtajaa).Aineistosta muodostettiin keskiarvosummamuuttujia pääluokka- ja alaluokkakohtaisesti. Henkilökunnan ja johtajien vastausten riippuvuutta tutkittiin…

health services. A qualitative descriptive, exploratory and contextual research design was followed. The data collection was conducted through two focus group discussions comprising of pharmaceutical
managers and district
health services
managers respectively. The study found that medicine was not consistently available in the various districts, especially in community
health centres and primary
health clinics. The factors contributing to the non-availability of medicines include challenges with deliveries from Medical Depots, poor medicine stock management, shortage of pharmacists and pharmacist’s assistants in the facilities, lack of the electronic medicine management systems and the separate existence of Pharmaceutical Services and Medical Depot within the province. The study made recommendations on how to improve medicine availability within the district
health services.

▼ Fieldworkers are an important cadre of research staff due to the critical roles they play in
health research not just in Africa but globally. In Africa, most international research centres employ fieldworkers to provide support in seeking informed consent; collecting basic samples and maintaining good relationships with communities involved in research.
Seeking informed consent is an especially important role. It does not only ensure the promotion of autonomy for those involved in the research but has an important implication on the quality and reliability of data collected from research participants. According to most international ethical guidelines, ensuring potential research subjects voluntarily participate in research after fully understanding all key elements of the research they are involved in is essential. Although research in most international research centres is overseen by highly qualified and renowned researchers and scientists, the influence of fieldworkers, who are the cadre of
health research staff involved in the day to day operationalization and ground application of this important ethical principle, has received little focus and continues to be understudied. Understanding how fieldworkers conceptualize the informed consent process can provide important insight on fieldworkers’ ethical practices and how ethical guidelines are operationalized on the ground.
We conducted four Focus Group Discussions (FGDs) with
health research fieldworkers and eight In-depth Individual Interviews with field
managers/supervisors at the Kenya Medical Research Institute (KEMRI-Wellcome Trust Programme (KWTRP) to develop an in-depth understanding of how this cadre of staff conceptualize the process of informed consent. Data were captured using digital recorders and transcribed before being analyzed thematically.
The results indicate a good understanding of
health research for both fieldworkers and field
managers. The relationship between KWTRP and the community was described to be of mutual benefit sharing, with the community benefiting from new interventions and improved
health care while KWTRP got research subjects from the community. Informed consent was described as an important part of getting people to participate in research voluntarily. There was, however, some mixed attitude and perceptions regarding the value and key elements of the informed consent process. Several factors were said to influence informed consent process, including the environment where consenting was done, recruitment targets, support and supervision and the training that was given to fieldworkers. Fieldworkers perceived their work as critical in
health research. However, lack of recognition, capacity building and clear approaches for professional development affected their motivation and attitude about their roles. The findings support the need for further research to identify specific mechanisms for supporting fieldworkers to undertake their roles more effectively.

Aim: Is to examine and describe how opportunities to share knowledge with other organisational members experienced by managers with occupational health and safety responsibilities. Method: A qualitative case study with semi-structured interviews from N=10 informants. Main results: Managers' experience are perceived to be the possession of a superior power and that they need to bring in non-relational resources to dissemination knowledge. Conclusion: Theory suggests that by creating relationships by using communication and interaction to access trust and reciprocity, there are great opportunities for knowledge transfer. We have taken part of the informants' experiences, we have come to the conclusion it's not the easy to implement relationships in the work environment because it does not always precondition for this. Because of cultural differences, experience-and competence differences and organisational characteristics.
Problemformulering: I arbetet förekommer det stora risker för ohälsa och olyckor, för att kunna minimera dessa risker måste det ske en kunskapsspridning inom organisationen. Att sprida kunskap gällande arbetsmiljö är en viktig faktor i alla arbetsområden i samhället, dock är spridningen inte det enklaste. För att kunna sprida kunskap behöver vi undersöka hur chefer som ansvarar för arbetsmiljö upplever sina förutsättningar för kunskapsspridning inom organisationer. För att sedan ta del av detta och skapa förutsättningar som främjar spridningen av kunskap. Syfte: Att undersöka och beskriva hur förutsättningar att dela med sig av kunskap till andra organisationsmedlemmar upplevs av chefer med arbetsmiljöansvar Metod: En kvalitativ fallstudie med semistrukturerade intervjuer från N=10 informanter. Innehållsanalys har används för att analysera data. Huvudresultat: Skapandet av relationer är inte det enklaste tillvägagångsättet för att sprida kunskap. Chefernas förutsättningar upplevs vara att inneha en överordnad position och att de behöver/vill ta in icke-relationella resurser för att kunskapen enklare skall spridas. Slutsats: Teorin menar att nära relationer som byggs upp med hjälp av kommunikation och interaktion för att komma åt tillit och ömsesidighet medför stora förutsättningar för kunskapsspridning. Denna studie visar att tänket att skapa värdefulla relationer finns hos cheferna. Men på något vis är det inte det enklaste att genomföra, eftersom det inte alltid finns förutsättningar för detta. Det vill säga förutsättningar som, kulturella skillnader, erfarenhet- och kompetenskillnader och organisatoriska egenskaper.

▼ Syftet med studien var att belysa hållbarheten med aktivitetsbaserat arbetssätt (ABW) utifrån chefers egen situation med fokus på hur chefer erfar ABW samt hur chefer uppfattar hälsa och välbefinnande på aktivitetsbaserade arbetsplatser. En kvalitativ forskningsdesign med inspiration från fenomenografisk analys tillämpades med semistrukturerade intervjuer som datainsamling baserat på ett strategiskt urval. Resultatet från studien visade att fyra beskrivningskategorier kunde urskiljas för hur chefer erfar ABW. De olika kategorierna speglade chefernas uppfattningar om hur möjligheter och utmaningar med att arbeta aktivitetsbaserat påverkade arbetsvillkor och hållbarhet. Chefers främsta fokus gällande hälsa och välbefinnande utgick från medarbetarnas perspektiv som behövde balanseras mot chefers egna behov. Den flexibilitet som ABW erbjuder uppskattas av chefer och medför även en beredskap för oväntade situationer. Det tar tid att vänja sig vid ABW och passar inte för alla individer eller för alla yrkeskategorier. För att ABW ska vara hälsofrämjande och hållbart behöver arbetssättet utvärderas mer och anpassas till den egna organisationen.
The aim of the study was to elucidate sustainability with activity-based working (ABW) based on the manager's own situation with a focus on how managers experience ABW and how managers perceive health and well-being in activity-based workplaces. A qualitative research design with semi-structured interviews as data collection with inspiration from phenomenographic analysis was used in the study. The results of the study showed that four description categories could be distinguished for how managers experience ABW. The different categories reflected managers' perceptions of how opportunities and challenges in working activity-based affect working conditions and sustainability. The managers' main focus on health and well-being was based on the employees' perspectives that need to be balanced against managers' own needs. The flexibility that ABW offers was appreciated by managers and also led to preparedness for unexpected situations. It takes time to get used to ABW and it is not suitable for all individuals or for all occupational categories. In order for ABW to be health-promoting and sustainable, the way of working needs to be evaluated more and adapted to its own organization.

▼ O presente trabalho propôs a estudar as articulações entre saúde mental e trabalho, mais especificamente a dinâmica prazer-sofrimento bancário dos trabalhadores que ocupam cargo de gestores intermediários em um banco estatal, localizado no estado do Rio Grande do Sul, em agências diversas, de diferentes cidades. Fundamenta-se na importância de estudar as experiências relacionadas ao prazer e o sofrimento no trabalho, assim como, no significado do trabalho de gestão, além de interessar-se em investigar se gestores com tempos diferentes no cargo percebem de forma diferente o prazer e o sofrimento relacionado ao trabalho. Isto porque temos poucos estudos que visam aprofundar conhecimentos acerca destes gestores intermediários. Nossa pesquisa fundamenta-se nos pressupostos teóricos e metodológicos da Clínica em Psicodinâmica do Trabalho. No que diz respeito ao sofrimento, este foi relacionado a excessiva cobrança de metas, consideradas inatingíveis, intensificado pela existência de avaliações individuais, e estas, sendo objetivas, desconsideram esforço empregado para realizar uma tarefa, o que causa frustração. O volume de serviço excessivo também é mencionado como causador de desconforto, além do sofrimento ético, que coloca o trabalhador em um dilema moral, ao não saber se atende aos desígnios da empresa ou as necessidades dos clientes. Esses fatores que provocam sofrimento, muitas vezes, podem culminar no adoecimento do trabalhador. Os fatores identificados ora como fontes de sofrimento, ora como fontes de prazer, foram: o reconhecimento, a sensação de estabilidade e a quantofrenia. No que diz respeito as estratégias defensivas utilizadas pelos trabalhadores para manterem-se trabalhando, as possíveis de serem detectadas foram: a negação, a racionalização, repensar a escolha e não realizar a reflexão. Todas elas objetivam proteger a saúde mental do trabalhador contra a descompensação, porém não modificam a organização do trabalho. O eixo significado do trabalho de gestão nos mostrou que, os gestores entrevistados têm vigorosamente interiorizadas as diretrizes da organização, por conta disto, seu trabalho é essencialmente centrado na tarefa. Há casos mais raros, nos quais o sentido do trabalho está ligado aos valores relativos à construção de identidade, reconhecimento e relacionamento interpessoal, além daqueles de dupla racionalidade, entretanto, entendemos que a racionalidade instrumental ainda se sobrepõe. Portanto, não causa espanto o aparecimento de inúmeras vivências relacionadas ao sofrimento no trabalho, visto a satisfação estar mais intimamente relacionada ao caráter subjetivo daquele. ▼ Whether the community is looking for “scapegoats” to blame, or seeking more radical and deeper causes, health care managers are in the firing line whenever there are woes in the health care sector. The public has a right to question whether ethics have much influence on the everyday decision making of health care managers. This thesis explores, through a series of published papers, the influence of ethics and other factors on the decision making of health care managers in Australia. Critical review of over 40 years of research on ethical decision making has revealed a large number of influencing factors, but there is a demonstrable lack of a multidimensional approach that measures the combined influences of these factors on managers. This thesis has developed an instrument, the Managerial Ethical Profile (MEP) scale, based on a multidimensional model combining a large number of influencing factors. The MEP scale measures the range of influences on individual managers, and describes the major tendencies by developing a number of empirical profiles derived from a hierarchical cluster analysis. The instrument was developed and refined through a process of pilot studies on academics and students (n=41) and small-business managers (n=41), and then was administered to the larger sample of health care managers (n=441). Results from this study indicate that Australian health care managers draw on a range of ethical frameworks in their everyday decision making, forming the basis of five MEPs (Knights, Guardian Angels, Duty Followers, Defenders, and Chameleons). Results from the study also indicate that the range of individual, organisational, and external factors that influence decision making can be grouped into three major clusters or functions. Cross referencing these functions and other demographic data to the MEPs provides analytical insight into the characteristics of the MEPs. These five profiles summarise existing strengths and weaknesses in managerial ethical decision making. Therefore identifying these profiles not only can contribute to increasing organisational knowledge and self-awareness, but also has clear implications for the design and implementation of ethics education and training in large scale organisations in the health care industry.

health care costs place increased burden on patients,
health care personnel, administrators and policymakers. Decisions in
health care are influenced by data which can be transferred into valuable information and knowledge. Data sets that facilitate data collection, information management and knowledge building are needed by nurse
managers to support administrative decision- making. The Nursing Management Minimum Data Set (NMMDS,,¦) offers a standardized method to capture core data that can be collected in information systems, shared and reused for multiple purposes to support safe and cost-effective care.
The purpose of this descriptive study was to adapt to Iceland and clinically test the NMMDS-ICE in all adult inpatient care units in the country of Iceland (excluding psychiatry). The aims of the study were to 1) translate the NMMDS from source language (English) to target language (Icelandic); 2) to validate the translated instrument; and 3) to describe the environment, nursing care resources, and financial resources across acute adult inpatient care units in Iceland.
Instrument development consisted of translation, expert validation, and psychometric testing. The target population was all adult acute care units in hospitals in Iceland, and the nurse
managers (n=38) representing these units. Data collection included a mailed survey. The sample equaled the population. Furthermore, 134 staff nurses on these units (excluding staff nurses at Landspitali) completed a job satisfaction survey. Return rate was 74% for nurse
managers and 71% for staff nurses.
Semantic and content equivalence of the NMMDS-ICE was established. Five of seven subscales of the instrument received Cronbach¡¦s alpha score of 0.70 or higher. Results indicated that it was feasible to collect the NMMDS-ICE in hospitals in Iceland, albeit, there was an issue with time commitment to do so. The specialty services that best described the patient population were medical-, surgical services, birthing, and geriatrics. Furthermore, nurse
managers seem to perceive good control on their units, and both nurse
managers and staff nurses are satisfied with their job. A positive correlation was found between autonomy and satisfaction with nursing management, nursing administration, and own level of autonomy. Statistical differences were identified in environmental and staffing resources between hospitals.

En organisations ledarskap tillskrivs ofta den stödjande funktionen i krav, kontroll, och stödteorin. Genom teorin tilldelas ledaren ett ansvar för såväl anställdas hälsa som välbefinnande i arbetslivet. Att inneha en chefsroll i en organisation kan utifrån detta betraktas som utmanande. Ledarskapsforskningen innehåller en teori om hur ledaren kan anpassa nivån av stöd i relation till motstridiga krav i arbetslivet. Andra studier menar att ledarskap är ett resultat av den kontext ledarskapet utövas i. Första linjens chefer beskrivs genom forskning som en chefsposition utsatt för höga krav i kombination med begränsade kontrollutrymmen. Särskilt utsatta anses första linjens chefer inom hälso-, och sjukvården vara. Studier inom ämnet fokuserar främst på första linjens chefer med en vårdrelaterad yrkesroll där patientkontakt ingår, som verkar inom den offentliga hälso-, och sjukvården. Då den privata vårdsektorn växer sig allt större kan forskning med inriktning mot privat hälso-, och sjukvård ses som intressant att studera. Studiens syfte är att undersöka och beskriva första linjens chefers upplevelser av krav och kontroll inom den privata psykiatri-, och missbruksvården. Studiens datamaterial har samlats in genom semistrukturerade intervjuer och sammanställts genom en fenomenografisk analysansats. Resultatet av studien visar att första linjens chefers upplevelser av krav och kontroll är ett relaterade till chefsrollens ansvarområden, kontext och möjlighet att utöva ledarskap. Första linjen chefernas möjlighet att utöva ledarskap är i sin tur ett resultat av omgivande kontext. Att addera kontext som en faktor i den befintliga krav, kontroll stödteorin kan utökas förståelsen för chefers arbetsmiljö, möjlighet att utöva ledarskap och möjlighet att ge underordnade anställda stöd.
Aim: Examine and describe first line managers’ perceptions of demand and control within the private psychiatric, and treatment care. Background: First line managers’ (FLM) work in health care contains high demands and limited control space. It is claimed through leadership research that leaders can adapt the amount of support and control to a given work situation. Other leadership studies claims that a certain work situation, context, can affect the leadership performance. Due to the demand, control, support model, work situations containing high demands and limited control space effects the individuals’ experience of work in a negative way that can affect health and wellbeing. ▼ Good conditions in the workplace, fairness in the workplace and the social differences contribute to a general improvement in public health. A health-promoting leader is working to create a workplace that promotes health, supports its employees and contributes to their wellbeing. The purpose of this study was to investigate the first-line managers in the city of Västerås experience and deal with their conditions and obstacles in the health-promoting leadership. A qualitative approach was chosen and the selection was six first-line managers working in three different areas of the City of Västerås. Managers were interviewed by an interviewguide and the material was analyzed with a manifest content analysis, with an inductive approach. The results showed that managers had knowledge of health-promoting leadership and that they were working with health promotion strategies, both deliberately and unconsciously. Conditions that affect the health promoting leadership proved knowledge, manager’s personality, requirements, collaboration and support. Obstacles in health leadership were considered for some time, ignorance and to many employees to be responsible for. There was a need for more knowledge and support in health leadership. The conclusions were that the conditions for health promotion leadership should be improved and there is a need for training in the subject and the support of the head of health promotion within the City of Västerås.

(AML) för att säkerställa arbetsmiljön för medarbetare, samtidigt som de ska
säkerställa att insatser enligt lag 1993:387, om stöd och service för vissa
funktionshindrade (LSS) är av god kvalité och möter stödanvändares behov.
Detta, tillsammans med förekomst av utmanande beteende hos de boende, antyder
att första linjechefer har en komplex arbetssituation. Syftet med studien var därför
att undersöka hur första linjechefer inom LSS-bostäder, där det förekommer
utmanande beteende, upplever relationen mellan arbetsmiljöåtgärder och det
pedagogiska arbetet. Studiens fokus var att identifiera faktorer som verkar
främjande respektive hindrande i arbetsmiljöarbete och pedagogiskt arbete.
Empiri för studien samlades in med hjälp av semistrukturerade intervjuer med sju
första linjechefer. Genom tematisk analys har två huvudteman identifierats:
upplevelser av yrkesrollen och upplevelsen av relationen mellan arbetsmiljö och
pedagogik. Resultatet har analyserat utifrån Michael Lipsky teori, Street-level
Bureaucrats. Resultatet visar att praktiknära ledarskap upplevs centralt för att
upprätthålla god kvalité i det pedagogiska arbetet, som i sin tur bidrog till bättre
arbetsmiljö. Studien identifierar komplexitet och utmaningar i relationen mellan
arbetsmiljöarbetet och det pedagogiska arbetet inom tre kategorier: (1)
organisatoriska faktorer; (2) första linjechefens yrkesroll; och (3) bristande
kunskap hos personalen. Vidare forskning kan därför behövas om hur praktiknära
ledarskap påverkar kunskapsnivån i verksamheten och bemötandet av
stödanvändare, samt hur det påverkar både arbetsmiljön och första linjechefers
yrkesroll och arbetsuppgifter.

managers. The purpose of the study was therefore to investigate how frontline
managers for LSS-housing, where challenging behavior occur, experience the
relation between work environment and the pedagogical work. The focus of the
study was to identify factors that appear to promote and obstruct the efforts to
ensure adequate work environment and pedagogical work. The empirical data for
the study were collected using semi-structured interviews with seven frontline
managers. Through thematic analysis, two main themes have been identified:

▼ The aim of this study was to construct a theory for women's managerial career development that explained how women in health care services and health science faculties achieved senior management positions and developed their careers. It sought to discover the main barriers to career progress and achievement of senior level positions by women in health related organisations and to identify how women managers dealt with obstacles. In-depth interviews with 35 women managers in senior positions in 19 different organisations from three different cultural regions formed the major data source; observations, field notes, personal and operational journals, documents, and literature supplemented this data.This research was conducted in two phases. In phase one a descriptive approach was used to develop propositions about women managers and their careers. These propositions formed the guidelines for phase two. The second phase used grounded theory methods, incorporating feminist and interpretative perspectives to identify the previously inarticulated core problem shared by participants. The barriers that women encountered were the contradictory, inconsistent and incompatible assumptions about their potential to have long term careers and ability to move into senior level management positions.These assumptions had been received during their life and educational experiences, as well as from their organisations. The gendered context of health care organisations and university educational institutions contributed to the limited career aspirations and career progress of women with health professional qualifications. By applying grounded theory strategies for analysis of the data, it was discovered that the women managers dealt with this problem through a core process, labelled metamorphosis, a four stage process for overcoming assumptions. This core variable was the way these women managers moved from managing without confidence to managing with confidence and assurance.This process occurred over time having four stages, each involving different activities and strategies. The progressive spiral stages were: being in a quandary (struggling with incompatible and contradictory assumptions); observing, examining and reflecting (on the impact of internal and external assumptions on their behaviour in organisational contexts, then realising that opportunities existed); learning and reframing (the managerial skills in order to re-frame their assumptions about the traditional characteristics of a manager); and finally change and transformation into being confident managers, so developing women's presence in management.The findings generated a theory which proposed a managerial career development model for enabling women to manage with confidence and assurance. ▼ Cost-sharing in health centers was introduced in order to mitigate state expenditures, to supplement health system’s resources and make patients cost-conscious.The aim of this research is to investigate the aspects of users and professionals of health centers as far as cost sharing is concerned. This will result to the evaluation of the measure and the expression of proposals for improvement.For this reason three different questionnaires were developed and distributed to administrative personnel, to managers and to users of health centers. 193 administrative employees (response rate 96%) and 188 managers (response rate 93%) were participated, out of 202 health centers in Greece. Moreover, 626 users from 18 health centers of 6 Health Districts of Greece, sampling participated.The questionnaires were completed by telephone, by fax, via email, or through live interview. The collection of questionnaires from health professionals was accomplished during June 2011 to May 2012 and the questionnaires from users were accomplished during May 2012 to April 2013.Statistical analysis was performed by statistical package SPSS using the Pearson x2 test, the Kolmogorov-Smirnov test, the t-test and the Mann-Whitney test at p<0.05 level of significance.According to 92% of administrative personnel, patients complied with the payment of fees and according to 41.5% their visits to health centers have been reduced.Moreover, 64% of health centers do not employ only administrative personnel for the collection of medical fees but also nurses, physicians and technicians. At 85% of health centers information technology systems are not used for the collection of medical fees.According to 73% of the managers the introduction of cost-sharing had a positive impact on patients’ compliance and according to 66% the financing mechanism has contributed to cost consciousness from both patients and personnel. Although, 81.5% said that the revenues from medical fees do not cover the running costs of health centers and 80% said that they do not promote a more effective delivery of services. In addition, 61% of managers characterise cost-sharing as a positive step and 80% of them support the economic autonomy of health centers.According to users’ results 54% expressed much and too much satisfaction from health centers, 59% from whom do not pay fees. Moreover, 75% of participants do not agree with cost sharing as a way to improve the quality of health services, 53% from whom expressed from moderate to no satisfaction from health centers. Although, 98% of respondents said that they will visit again health centers if there is a reason.In conclusion, cost sharing has contributed to the rational use of health centers’ services, due to cost-consciousness from patients and professionals, as well as in the financing of the primary health care. ▼ This research tested a number of hypotheses relating
to variables in case
managers' work environment. All nine of these
hypotheses were found to be supported at the significance level of
p <.001. The study also tested a model of turnover intentions
that included the variables of role strain, job satisfaction,
organizational commitment, burnout, and turnover intentions. As a
result of the findings of path analysis, the model was revised and
one of the variables, organizational commitment, was excluded
because of the fact that it was found to not explain a significant
amount of variance in turnover intentions, the dependent variable,
over that which was explained by the other independent variables in
the model. Role strain was found to significantly affect a number
of variables in the model, above that which was initially
hypothesized, as it had an indirect effect, as well as a direct
effect on several of the variables in the model. Role strain was
found to have both a direct and an indirect effect on job
satisfaction. In addition, role strain was found to affect burnout
indirectly through three different paths. Role strain was also
found to have a total of five indirect paths to turnover
intentions. This research also found job satisfaction to be of
considerable importance. It had the largest direct effect on
turnover intentions. The findings of this research did not support
the position that had been advanced that case
managers have high
turnover intentions. Despite the high actual turnover among case
managers in the past, only moderate turnover intentions were found
among the case
managers in this sample. The case
managers in this
sample expressed a number of common concerns and dissatisfactions.
Among these were low pay, excessive paperwork, lack of resources in
the community, especially housing, with which to link up their
clients, and unrealistic role demands. In spite of these
dissatisfactions, case mangers report that they like working with
their clients and helping them to improve their lives. Case
managers also report considerable dissatisfaction with the pay that
they receive, and, to a lesser extent, dissatisfaction with
opportunities for promotion.
