Pulsed-field gel electrophoresis (PFGE) is a laboratory technique in which Salmonella DNA banding patterns are used as molecular fingerprints for epidemiologic study for "PFGE clusters". State and national health departments (CDC) use PFGE to detect clusters of related cases and to discover common sources of bacteria in outbreaks. Objectives. Using Houston Department of Health and Human Services (HDHHS) data, the study sought: (1) to describe the epidemiology of Salmonella in Houston, with PFGE subtype as a variable; and (2) to determine whether PFGE patterns and clusters detected in Houston were local appearances of PFGE patterns or clusters that occurred statewide. Methods. During the years 2002 to 2005, the HDHHS collected and analyzed data from routine surveillance of Salmonella. We implemented a protocol, between May 1, 2007 and December 31, 2007, in which PFGE patterns from local cases were sent via e-mail to the Texas Department of State Health Services, to verify whether the local PFGE patterns were also part of statewide clusters. PFGE was performed from 106 patients providing a sample from which Salmonella was isolated in that time period. Local PFGE clusters were investigated, with the enhanced picture obtained by linking local PFGE patterns to PFGE patterns at the state and national level. Results. We found that, during the years 2002 to 2005, there were 66 PFGE clusters, ranging in size from 2 to 22 patients within each cluster. Between different serotypes, there were marked differences in the sizes of PFGE clusters. A common source or risk factor was found in fewer than 5 of the 66 PFGE clusters. With the revised protocol, we found that 19 of 66 local PFGE patterns were indistinguishable from PFGE patterns at Texas DSHS. During the eight months, we identified ten local PFGE clusters with a total of 42 patients. The PFGE pattern for eight of the ten clusters matched the PFGE patterns for cases reported to Texas DSHS from other geographic areas. Five of the ten PFGE patterns matched PFGE patterns for clusters under investigation at PulseNet at the national level. HDHHS epidemiologists identified a mode of transmission in two of the ten local clusters and a common risk factor in a third local cluster. Conclusion. In the extended-study protocol, Houston PFGE patterns were linked to patterns seen at the state and national level. The investigation of PFGE clusters was more efficacious in detecting a common transmission when local data were linked to state and national data.

▼ Objective. we examined the impact of whole grains, refined grains and total dietary fiber intake as determinants of the racial differential in the clinical aggressiveness of prostate cancer observed between African Americans (AA) and European Americans (EA).Methods. A population-based case-only study in which the case-control design was used to analyze data from the North Carolina-Louisiana Prostate Cancer Project, on 667 AA and 820 EA. Cases were highly aggressive cases of histologically confirmed adenocarcinoma of the prostate and controls were intermediate-aggressive and lowaggressive cases combined. Logistic regression was utilized to examine differences between cases and controls with respect to intake of refined grains, whole grains and dietary fiber, stratifying by race. Polytomous logistic regression was used to evaluate the associations between the clinical grade of prostate cancer as outcome variable, and nutrient intake including race*nutrient interaction terms. Results. There was a non-significant positive association between refined grains and risk of aggressive prostate cancer for AA (OR, 1.52; 95% CI, 0.82-2.82 for the 4^th quartile as compared to the first quartile of intake). ORs showed inverse associations between intake of whole grains and prostate cancer aggressiveness. ORs increased in magnitude with higher intake of whole grains for EA (ORs for the 3^rd and 4^th quartiles versus the 1^st were 0.82, and 0.52 respectively). ORs for AA for the 3^rd and 4^th quartiles versus the 1^st were 0.58 and 0.92. A similar trend as for whole grains was observed with total dietary fiber. Differential intakes of refined grains and whole grains among AA and EA partly explained the within stage higher prostate cancer grades that AA experience compared with EA. Conclusion. A reduction in the American diet of refined grain foods and an increase in the consumption of whole grain foods with a high amount of dietary fiber may reduce the clinical aggressiveness and consequently the mortality from prostate cancer. ▼ Background. Healthcare system in United States is faced with a unique challenge of catering to a population that is racially, culturally and linguistically diverse. Efforts to provide health care appropriate to each of such populations are a part of the national health care agenda. Yet, certain locally diverse minorities might be neglected due to their confined location in a part of the country. The Polish community is one such linguistically and culturally unique population in the Chicago area, within the county of Cook in the state of Illinois. The Cook County Health System provides health care for the un- and under insured population in the county, but specific language and cultural barriers exist for the uninsured Polish population. Objective. To improve access to cardiovascular preventive health-care for the uninsured Polish population in Chicago area by analyzing barriers and devising effective and sustainable solutions. Methods. Efforts were made to establish community collaboration in order to assess needs and devise effective solutions towards breaking the barriers to provide health care to the Polish population. Care was provided at a geographically proximate health center. Screening using a non-lab based cardiovascular risk assessment tool identified individuals at high risk of adverse cardiovascular outcomes. Such screening was offered in health fairs at community gatherings (a Polish cultural festival and a local community church). Individuals with no access to health care providers were given follow-up appointments at a nearby county health clinic. As needed, all these individuals received continuity care with primary care physician and/or a cardiologist. Results. Strong collaboration was established between the health care facility and a community organization named Polish American Association. Apart from providing insight into the barriers faced by the Polish population in accessing health care facilities, this organization identified effective venues for health screening, created necessary publicity and also facilitated Polish translation services at the local health clinic, the Logan square health center. A total of 125 patients were screened. 63 of them were identified as high risk and not having access to a provider. All these individuals were given appointments for a clinic visit at the Logan square health center, a geographically proximate health clinic. 54% of those screened had >20% risk of a 5 year adverse cardiovascular outcome. 34 of the 68 have been seen in follow-up clinic till date. 56% of them had a new medication prescribed in their first clinic visit while 38% had an investigation ordered. All were provided with a primary care physician while 38% needed follow-up with a cardiologist. Discussion. ▼ Purpose. The purpose of this study is to determine if being prescribed medication for an index condition can be used as a predictor of comorbid awareness in HIV-positive individuals. Background. Advances in antiretroviral therapy have increased survival and life expectancy of HIV-positive individuals but mortality remains high. There is a growing population of older HIV-infected patients who are developing non-HIV related comorbidities. These comorbidities have been associated with decreased quality of life and increased risk of mortality. Methods. Patient responses were sorted to determine frequencies of awareness for the category: "Has it and did not know they had the condition." Descriptive analyses were performed. A cross tabulation was done to produce the group that responded with the following: "I take medication for it," "I do not take medication for it." Frequency analyses were then done for the following response: "I don't have the condition," and "I am not sure I have the condition," but are documented to have the index condition and be on medication. Predictors of awareness were assessed using statistical tests such as the Analysis of Variance (ANOVA), T-tests, and Pearson correlations. Results. Out of the 15 conditions assessed: high blood pressure (58.3%), arthritis (30.8%), cardiovascular disease (28.6%), stroke (16.7%), and high cholesterol (12.9%) had the highest percentage of patients who were on medication but were not aware they had the condition. There were very few patients who were on medication and were not aware they had comorbid conditions, however, a positive association between oral treatment and the index condition being treated could not be established. Black non-Hispanics were found to have the highest sensitivity score (42%) as well as the group to have the most chart documented comorbidities (43%). Conclusion. Further studies are needed to identify risk factors for patient awareness in the HIV-infected population; specifically education level, literacy, and health literacy.

▼ This research examines weather and road conditions relation to traffic crashes on Bureau of Land Management (BLM) and U.S. Forest Service (FS) land in three states: Idaho, Oregon and California. Crash data for Idaho and Oregon were supplied by the state transportation departments while the California data were obtained from the Federal Highway Administration’s Highway Safety Administration (FHWA). The results are mixed, probably because of the different methods of data collection: Idaho seems to have particularly severe crashes during bad weather on these public lands when all roads on the public lands are compared with other rural Idaho state and federal highways. Oregon's comparable weather-related crashes do not show such severe crashes in poor weather or road conditions, but Oregon on these federal lands crashes in good weather are very severe as are the “non-weather” crashes on lightly traveled rural highways in the State. California’s FHWA Highway Safety Information System data offered a much more objective test of crashes on the public domain, based on federal and state roadways versus private, rural land roads during "weather" and “non-weather” conditions. In the aggregate, weather-related crash differences appear non-significant for California’s public and private lands. The salient finding in California is that on average, "non-weather" crashes on BLM and USFS land are significantly more severe than on comparable rural roadways in the State. Using FHWA projections of crash costs, the BLM and FS crashes produce about 30 percent greater losses.) The latter finding may be a result of more speed with good weather conditions, adverse roadside environments and the increased time required for emergency response to public land crashes. Future deployments of Intelligent Weather technology for rural California roadways could benefit from the database assembled for this research, especially the weather-related crash analysis for roadway/county/federal or rural land contingencies in Appendix A. Dramatic differences in local crash costs were observed in the limited fine-scale analysis done in this study. Providing weather and location crash cost in a Geographical Information System would further assist management and policy-makers in efforts to reduce rural crash risk.

▼ Heat stress is a common physical agent associated with many occupations. The most commonly used method of assessing heat stress exposure is an empirical method using the Wet Bulb Globe Temperature Index but his method is limited in its ability to parse out individual contributors to the heat stress. The International Organization for Standardization (ISO) published a rational model called Predicted Heat Strain (PHS) in 2004, and rational methods have the advantage of separating out the individual pathways for heat exchange. The objective of this research was a performance assessment of the current PHS model. This experimental design consisted of 15 trials (3 clothing ensembles and 5 heat stress levels) involving 12 men and women. The clothing ensembles were work clothes, NexGen® (microporous) coveralls, and Tychem® QC (vaporbarrier) coveralls. The heat stress levels were 1.0, 2.0, 3.5, 5.5 and 9.0 °C-WBGT above the average critical environment for each ensemble determined in prior studies. The metabolic rate was 190 W/m². The two outcomes of each trial were an exposure time when core temperature reached 38 °C (ET38) and a Safe Exposure Time (SET) defined as the amount of time required to reach either a core temperature (T_re) = 38.5 ºC, a heart rate of 85% age-estimated maximum, or fatigue. Trial data for environment, metabolic rate and clothing were inputs to the (PHS) model to determine a predicted amount of time for the participants to reach a T_re = 38 ºC, which was the limiting condition in PHS for acute exposures. The first consideration was predictive validity for which PHS-Time was compared to ET38. The expectation would be that PHS-Time would predict the mean ET response. Results for predictive validity indicated a moderate agreement between ET38 and PHS-Time (r² of 0.34 and Intraclass Correlation Coefficient at 0.33). When the method for accounting for clothing was changed to that recommended by ISO, the PHS predicted times moved systematically toward a shorter exposure time and modest agreement (r² of 0.39 and Intraclass Correlation Coefficient at 0.31). Protective validity was the ability of the PHS-Time to predict an exposure time that would be safe for most people. In this case, PHS-Time was compared to SET. The PHS was protective for 73% of the cases. When it was modified to account for clothing following the ISO method, the protective outcomes were 98%. In addition, the PHS model examined with respect to starting core temperature and fixed height and weight. Using the actual core temperature improved the outcomes somewhat, but changing from 36.8 to 37.0 would be sufficient. There is a strong tendency to over-predict PHS-Time for individuals with a low body surface area, usually short and lower than average weight.

▼ The purpose of this three-part dissertation is to understand the importance of informal social support, specifically of the male partner, on breastfeeding intentions and behaviors. It explores the impact male partner perceptions and attitudes may have on breastfeeding and implications for improving breastfeeding outcomes with intervention strategies focusing on male partners. Chapter 1 outlines the benefits of breastfeeding, summarizes data on breastfeeding rates in Hawai‘i and the US, describes the influence that informal support can have on a woman’s breastfeeding behaviors, and presents the purpose and conceptual framework of the dissertation research. Chapter 2 reports the results of a systematic literature review on fathers’ influence on and perceptions of breastfeeding and father-focused interventions to increase breastfeeding and discusses implications for future intervention development. Chapter 3 presents findings from a qualitative study in which male partners of pregnant women and new moms were interviewed regarding their attitudes, knowledge, and feelings about breastfeeding. Chapter 4 investigates the infant-feeding attitudes of WIC mom and partner pairs using a quantitative approach to explore associations between mom and partner attitudes and infant feeding intentions and whether the partner’s infant feeding attitudes influence infant feeding intentions over and above the mom’s attitude. Participants for Chapters 3 and 4 were recruited within the Hawai‘i US Department of Agriculture’s Women, Infants, and Children (WIC) program. Chapter 5 presents the main conclusions of the dissertation research, implications for practice and policy, future research directions, and contributions the research makes to literature. Consistent with recent national and international research, results suggest that the male partners of the Hawai‘i WIC population are an important source of support and influence in the initiation and continuation of breastfeeding. Male partners want to be iv more involved in the infant feeding process, but do not feel they have the tools and knowledge to support a breastfeeding mom. To increase breastfeeding rates and change cultural norms related to breastfeeding, it is important to improve male partners’ attitudes toward and perceptions of breastfeeding. Findings suggest that an intervention based in Social Cognitive Theory may have the best chance of improving the male partner’s knowledge and attitudes about breastfeeding.

▼ Social capital is a concept explored by researchers and scholars from a variety of disciplines, including sociology, management, economics, political science, and health. As with other forms of capital (e.g. financial capital, human capital), social capital is a resource drawn upon to aid productivity, and has been linked to community and economic development, good education, democracy, and health (Cook, 2000; Hawe & Shiell, 2000; Veenstra, 2000; Kawachi, Kennedy & Glass, 1999; Lomas, 1998; Kawachi, Kennedy, Lochner & Prothrow-Stith, 1997; Fukuyama, 1995; Putnam, 1993, 2000). Similar to concepts like sense of community and community identity, social capital has been studied in relation to mortality, risk behavior, depression, and self-reported health status. These studies have concluded that higher social capital is positively correlated with good health and healthy behavior and negatively correlated with risky behavior, a variety of diseases, and depression. If social capital proves to be an area that interventions can target for improvement, this may hold significant implications for health planning and programming in communities. This research has capitalized on the availability of social capital data collected from a diverse population in Texas in 2002 and 2006 to explore the quality of the measures used how social capital is affected by rural/urban status. Recognizing what influences the variations in social capital from community to community could enhance our understanding of how to better measure social capital and apply our findings to health improvement strategies. Although a relatively young concept, social capital has emerged as a significant community characteristic to be considered in the study of population health. If what Putnam (2000) and Fukuyama (2005) argue is true and the U.S. is experiencing an ongoing decline in social capital, continuing to build our understanding of how social capital is produced and its effects may be of increasing importance in efforts to promote and maintain population health.

▼ Research has linked several factors of one's neighborhood environment to an individual's likelihood of being physically active; however, the bulk of this research has focused on, or is based on previous research from, more urban environments. As such, the measures and findings of this research may not apply to more rural settings. Further, rural settings present a special challenge due to their relative geographic expanse and lower population density, compared to more urban areas. Thus, research is needed to determine the applicability of urban-based measures of neighborhood environment to rural settings. Research is needed into examining new potential methods for evaluating the neighborhood environment in rural areas that can account for the geographic size and sparseness of these areas. This dissertation demonstrated that urban measures may not apply to rural settings in Central Texas, when measured at both the individual-level and the neighborhood-level. At the individual level, neighborhood environment, measured using variables commonly applied in urban studies, was not associated with an increased or decreased odds of meeting recommended physical activity levels. At the neighborhood level, certain factors within the neighborhood environment was associated with a change in odds of meeting physical activity recommendations; however, there were differences between rural and urban settings in which factors were associated with a change in odds of meeting recommended physical activity recommendations. Further, this dissertation examined the utility of a spatial interpolation technique, inverse distance weighting, in predicting neighborhood environments across a spatial plane based on a sample of neighborhood residents in urban and rural settings. Preliminary findings suggest that this methodology may be of some utility, but the methodology was prone to predicting false negatives in a number of instances. Further examination is needed before results are definitive. This work demonstrates the need for further research into factors related to being physically active in rural areas. Commonly studied dimensions of urban environments may not measure similar constructs within rural settings. Future research should seek to better understand the factors unique to rural environments that influence physical activity among rural residents.

Each year thousands of people participate in mass health screenings for diabetes and hypertension, but little is known about whether or not those who receive higher than normal screening results obtain the recommended follow-up medical care, or what barriers they perceive to doing so. Methods. Study participants were recruited from attendees at three health fairs in low-income neighborhoods in Houston, Texas Potential participants had higher than normal blood pressure (> 90/140 mgHg) or blood glucose readings (100 mm/dL fasting or 140 mm/dL random). Study participants were called at one, two, and three months and asked if they had obtained follow-up medical care; those who had not yet obtained follow-up care were asked to identify barriers. Using a modified Aday-Andersen model of health service access, the independent variables were individual and community characteristics and self-perceived need. The dependent variable was obtaining follow-up care, with barriers to care a secondary outcome. Results. Eighty-two study participants completed the initial questionnaire and 59 participants completed the study protocol. Forty-eight participants (59% under an intent to treat analysis, 81% of those completing the study protocol) obtained follow-up care. Those who completed the initial questionnaire and who reported a regular source of care were significantly more likely to obtain follow-up care. For those who completed the study protocol the relationship between having a regular source of care and obtaining follow-up care approached but did not reach significance. For those who completed the initial questionnaire, self-described health status, when examined as a binary variable (good, very good, excellent, or poor, fair, not sure) was associated with obtaining follow-up care for those who rated their health as poor, fair, or not sure. While the group who completed the study protocol did not reach statistical significance, the same relationship between self-described health status of poor, fair, or not sure and obtaining follow-up care was present. The participants who completed the study protocol and described their blood pressure as OK or a little high were statistically more likely to get follow-up care than those who described it as high or very high. All those on oral medications for hypertension (12/12) and diabetes (4/4) who were told to obtain follow-up care did so; however, the small sample size allows this correlation to be of statistical significance only for those treating hypertension. The variables significantly associated with obtaining follow-up care were having a regular source of care, self-described health status of poor, fair, or not sure, self-described blood pressure of OK or a little high, and taking medication for blood pressure. ▼ Background. Social stability is an understudied construct with promise for understanding the extent and types of vulnerability experienced among urban populations. Social stability also offers a useful framework to understand the synergy of social vulnerabilities that form the context for and contribute to HIV transmission risk among urban populations. This study: (1) synthesized the existing literature on social stability; (2) investigated prevalence and patterns of co-occurrence among a set of social stability characteristics; (3) evaluated the possibility of underlying sub-groups of social stability; and (4) examined the extent to which indicators of social stability and two measures of multidimensional social stability were associated with HIV-related risk variables among a sample of primarily African-American, low-income women at risk for HIV. Methods. Data were from comprehensive baseline interviews with women and their female social network members (n=635) enrolled in an HIV prevention intervention in Baltimore, MD. Analysis included exploratory statistics, logistic regression, latent class analysis, and latent class regression accounting for clustered data using Stata and Mplus software. Stata version 10 and Mplus version 5 were used for analysis. Results. Literature review identified a set of six common domains of social stability: housing, residential mobility, employment, partner relationship, income, and incarceration. Analytic results supported the premise that this set of characteristics represents a single construct with identifiable underlying sub-groups. Individual indicators varied in their association with HIV-related outcomes and supported investigation of the cumulative impact of stability across domains. Both a cumulative and latent class measure of multidimensional social stability was associated with HIV-related risks, with differing implications. Conclusions. Results reinforce that instability characteristics tend to co-occur, but with heterogeneity. Findings also indicate that the co-occurrence of instability contributes to HIV risk. Future research should investigate socio-demographic and health characteristics associated with social stability. These findings add to the calls for attention to structural health determinants and provide a framework for evaluating social and contextual factors in social service and HIV prevention programs.

▼ Low physical activity is a problem for girls, and particularly African American female adolescents. Low physical activity has been associated with increased risks of cardiovascular disease, hypertension, diabetes, cigarette smoking, premature mortality, and overweight and obesity. Families and communities play an important role in influencing adolescent health; however, few studies have examined how these factors influence adolescent physical activity outcomes. Adopting an ecological theoretical framework, the purpose of this study was to examine the relationship between family factors (maternal control, maternal support, mother-child communication, and family cohesion), a community factor (religiosity, defined as attendance at religious services and religious youth activities) and physical activity among African American and White female adolescents. Wave I data from the National Longitudinal Study of Adolescent Health was analyzed. The sample includes 736 African American and 1,692 White female adolescents in grades seven through 12. Descriptive statistics were computed for all family and community measures. Logit models were used to evaluate family and community influences on female adolescents achieving five or more bouts of moderate to vigorous physical activity per week (MVPA); and whether the relationship between maternal control, maternal support, mother-child communication, family cohesion, religiosity, and MVPA was moderated by race. Maternal control was a significant predictor of MVPA for the total sample and the White female sample, with increased levels of maternal control associated with increased adolescent physical activity. Mother-child communication and religiosity were significantly positively associated with MVPA for African American females only. Maternal support and family cohesion were not found to be significant predictors of adolescent girls' physical activity in any of the models. Race significantly influenced the strength of the relationship between mother-child communication and adolescent MVPA, and between religiosity and adolescent MVPA, with findings revealing a stronger relationship for African American female adolescents than for White female adolescents. Strategies to engage female adolescents in physical activity, particularly African American female adolescents, should focus on increasing mother-child communication and girls' involvement with religious institutions.

▼ Cancer survivors are a growing population, many with ongoing medical conditions that can potentially affect employment. This dissertation investigated two groups of cancer survivors to assess employment and occupational status following cancer treatment. The first study examined a cohort of hematopoietic cell transplantation (HCT) survivors to evaluate how physical and mental health status 6 months after transplant predicted return to full-time work using multivariable regression analyses. Survivors with normal physical functioning returned to work 2 times faster than those with lower functioning over the 5 years of follow-up. Female survivors were 47% less likely to return to work than males. Between 80%-87% of survivors working over the first three years after HCT reported limitations in their work ability compared to pre-illness. The second and third papers examined employment outcomes in childhood cancer survivors. Paper 2 assessed whether survivors were at higher risk of health-related unemployment and being unemployed but seeking work. In multivariable regression analyses, survivors were six times more likely to report health-related unemployment when compared to both a sibling sample and to age, sex, and race matched cohort from the National Health and Nutrition Examination survey. Both female sex and a history of certain surgeries were associated with health-related unemployment. Survivors with high-dose cranial radiation were at high risk for both unemployment outcomes. Unemployed survivors reported poorer physical functioning than employed survivors and were less educated, had lower incomes and were more likely to have public health insurance coverage than unemployed siblings. Paper 3 assessed occupational outcomes. In multivariable regression analyses survivors were less likely to hold higher-wage and higher-skill professional or managerial jobs than siblings. Survivors with a history of higher dose cranial radiation and central nervous tumor resection were also less likely to hold professional jobs and, if employed, were working in lower wage jobs that were nonphysical in nature. Survivors tended to have lower incomes than siblings within the same occupational classifications. By preparing patients, physicians and employers with realistic expectations regarding work-related abilities following cancer and developing strategies to address limitations, work outcomes can be improved for cancer survivors.

▼ The ability to notify and assemble staff quickly in order to begin the decision-making process is critical when faced with a public health emergency. This study examined state infrastructure factors that affect response time during a public health incident, specifically, the effect of federal public health emergency preparedness (PHEP) funding and state governance structure on staff response time. A retrospective cross-sectional mixed methods study design utilized data from the Centers for Disease Control and Prevention's (CDC) Preparedness Emergency Response System for Oversight, Reporting and Management Services (PERFORMS) and the Association of State and Territorial Health Officials (ASTHO) for all 50 states between 2007 and 2010. After removing outliers, PHEP funding did not significantly reduce response time for staff notifications R = -.008, F(_1,577) = .039, p = .422 or staff assemblies R = -.039, F(_1,441) = .661, p = .208. However, the type of state governance structure significantly reduced response time for staff notifications t(₅₇₇) = -2.02, p = .022 and staff assemblies t(₄₄₁) = -2.96, p = .02. States with a centralized form of governance structure took longer to notify (M = 23.44 minutes vs. M = 27.54 minutes) and to assemble staff (M = 32.36 minutes vs. M = 41.74 minutes). The number of staff responding, the type of incident, whether the incident was announced or unannounced and whether the incident was during normal business hours or outside normal business hours did not explain the observed response time. For states that exceeded CDC-recommended response times, qualitative analysis identified barriers to a timely response. Three themes emerged: staff, the organization and channels of communication. Barriers related to the organization and channels of communication were identified and reduced or eliminated over time. Barriers related to staff were ongoing and hard to reduce or eliminate. Changes in the survey questionnaire in 2009 and the addition of two survey modules had a negative effect on mean response times and response rates. State governance structure, increased reporting burden and staff burnout may explain some of the lack of improvement in response time over the four year study period.

The literature suggests that much of this process is subconscious. Perspective-taking, a suggestion that the subject put him or herself in the shoes of another, has been shown to reduce subconscious stereotyping and improve empathy. Improved patient-provider interactions may aid in reducing health disparities. Standardized patients offer a well accepted strategy for facilitating and assessing the patient-provider interaction. Purpose. To assess the effect of perspective-taking on physician assistant students in clinical interviews with African-American standardized patients (SPs). Methods. A randomized, controlled, double-blind study was designed to examine the effect of perspective-taking, on interactions between physician assistant (PA) students and African-American standardized patients (SPs). The experimental (E) group was instructed to take the perspective of their patient (i.e., imagine how their patient was feeling) prior to their interaction. The control (C) group received standard instructions. Both groups interacted with three separate standardized patients portraying three different clinical cases. SPs scored students using a validated instrument rating them from a low (1) to high (5) on their interpersonal skills. Results. One-hundred and five PA students, balanced for race and gender prior to randomization, each participated in three SP cases for 311 interactions. The E group (n=52) achieved higher scores than the C group (n=53) on interpersonal skills overall with a mean of 3.38 (SD 0.35) vs. 3.26 (SD 0.42) (p = .005) and on rapport in particular with a mean of 3.45 (SD 0.43) vs. 3.25 (SD 0.51; p<.000). The effect size was small, at 0.31. Conclusions. A brief perspective-taking instruction made a small but statistically significant difference in SP ratings of interactions with physician assistant students. Improved interpersonal skills obtained through this simple intervention could help to improve equity and quality of healthcare.

▼ While there are reports of developing sexual relationships on the Internet (I) among MSM, there are few reports that have examined the process of developing sexual relationships on the I and comparing to that in real life (IRL). This study examines the process to provide insight into how MSM make decisions about courtship, engages in negotiations for sex, and choose sexual partners and examines the comparative sexual risks taken between I vs. IRL negotiation. This self-selected convenience sample at a national level (n=1001) of MSM recruited through the I, systematically explored the different steps, the process of courtship in a flow chart of I and IRL dating to portray the process of filtering, courtship and/or negotiation for sex. Risk behaviors in both environments are presented along with interactions that create predictable sequences or "scripts". These sequences constitute 'filtering' and 'sexual positioning'. Differences between I & IRL suggest discussion of HIV/STD status to have consistent differences for all variables except 'unprotected sex' meaning no condom use. There was more communication on the I in regards to self revealing information or variables relating to reducing risks which enable 'filtering' (including serosorting). Data indicate more steps in the I process, providing more complex, multiple steps to filter and position with regard not only to HIV/STD risk but also to negotiate position for complementary sexual interest. The study established a pattern of MSM's courtships or negotiation for sex and a pattern of acquisition, and more I negotiation. Data suggest negotiation opportunities which could lend to intervention to advise people how to negotiate safely. Previous studies have reviewed MSM and drug use. This is a study to review the process of drug use associated with sexual behavior regarding the Internet (I) and in real life (IRL) using a self-selected, convenience sample of MSM (n=1001) recruited nation-wide through the Internet. Data on MSM and drugs illustrate the Internet being used as a tool to filter for drug use among MSM. MSM's drug use in both environments highlights the use of sexual performance drugs with an IRL pursuit of intimacy or negotiation for sex. IRL encounters were more likely to involve drug use (both recreational and sexual performance-enhancing) than Internet encounters. This may be due to more IRL meetings occurring at bars, clubs or parties where drug use is a norm. Compared with IRL, the Internet may provide a venue for persons who do not want to use drugs to select partners with similar attitudes. This suggests that filtering may be occurring as part of the internet negotiation. Data indicated that IRL persons get drunk/high before having sex in past 60 days significantly more often than Internet participants. Age did not alter the pattern of results. ▼ Introduction. This dissertation represents the first quantitative investigation of the effect of conflicting information on medication adherence in a sample of chronically ill patients. Using the Information-Motivation-Behavioral Skills model as a theoretical guide, the primary aim was to explore the relationships between conflicting information, adherence support, outcome expectations for medications, adherence self-efficacy, and medication adherence. A secondary aim was to describe patients’ most frequently used information sources, determine which sources patients perceived as credible, and investigate whether there were gender differences in the use and perceived credibility of sources. Methods. Vasculitis patients (n=232) completed two online questionnaires as part of the Accessing Social Support in Symptom Treatment Study. A bootstrapping approach was used to determine whether self-efficacy and outcome expectations mediated the effects of conflicting information and adherence support on medication adherence. For the second aim, MANCOVA tested the significance of a source*gender interaction term and follow-up contrasts determined which sources male and female patients used differently. T-tests compared patients’ perceived credibility ratings. Results. A majority of patients (51.3%) received conflicting medication information. Conflicting information had a direct negative effect on medication adherence, which was not mediated by self-efficacy or outcome expectations. Alternatively, self-efficacy mediated the positive effect of adherence support on medication adherence. Patients used physicians and the Internet most often to obtain medication information and also rated them as the two most credible information sources. Male patients consulted their spouse/partner more often and rated them as more credible than female patients. Female patients were more likely to use medication package inserts and the Internet and less likely to consult nurses than male patients. Conclusion. Vasculitis patients seek medication information from multiple sources, which can result in the discovery of conflicting information. Patients who receive conflicting medication information are less adherent to their medication regimens. Policy initiatives to standardize medication practices may reduce the amount of conflicting information available to patients. Future research should attempt to uncover the determinants and outcomes, both immediate and distal, of receiving conflicting information and explore whether conflicting information is an issue for other disease populations.
