guidelines are recommendations targeted at medical professionals which aim to optimise patient care. When successfully implemented, clinical
guidelines have been shown to improve processes of care and clinical outcomes. However, clinical
guidelines are often not successfully implemented and there exists a significant variation in rates of compliance between different doctors. Factors which affect compliance fall in to four categories: patient, doctor, environment and guideline.
In 2015 the Canterbury District Health Board changed to a new platform of online clinical guidance called Hospital HealthPathways. This platform differed significantly from its predecessor The Blue Book in both the development process and design of the clinical guidance. This study compared the use of and compliance with guideline recommendations between these two different platforms of clinical guidance. By doing so, this study was able to examine how guideline use and compliance is affected by the development process and design of a clinical guideline. Sub-group analysis examined how compliance varied between different clinicians, different patients, and different environments and also the interaction between these factors and guideline design.
A sub-analysis examined barriers to guideline compliance reported by clinicians to understand the perceived obstacles clinicians had to following guideline recommendations.
Finally, this study developed the concept of “appropriate non-compliance” by quantifying and describing cases where it was appropriate for clinicians not to follow local clinical guideline recommendations whilst managing patients.
Methods
This study adopted a mixed-methods methodology. Three conditions were selected for study: community acquired pneumonia (CAP), acute pancreatitis and intracranial haemorrhage (ICH) on warfarin. For each condition two distinct equivalent time periods were selected for comparison: the first when patients were managed by guidance in The Blue Book and the second when patients were managed by guidance in Hospital HealthPathways.
The use of the clinical guideline website was assessed by analysing unique page views of the guidance pages using Google Analytics. Compliance with guideline recommendations was assessed by a retrospective audit of the clinical notes. A survey was developed and sent to all hospital doctors to understand barriers to compliance. The concept of appropriate non-compliance was explored by documenting, quantifying and categorising reasons for non-compliance with clinical guideline recommendations documented in the clinical notes.
Results
This study found that a change in the guideline development process and design not only improved use and compliance but also reduced the variation in practice between doctors. Presence of co-morbidity and severity of illness, were significant factors contributing towards non-compliance.

▼ This paper addresses the nature of Social Work
supervision in the Directorate of Developmental Welfare Services.
It contains five chapters namely Research Orientation, Literature
Review, Social Structure in Namibia, Empirical Study and the last
chapter is the Conclusions and Recommendations. The Research
Orientation deals with motivating factors explaining the need for
this research as well as pointing out the goal and objectives of
the research. The goal of the research is to explore and describe
Social Work supervision in the Directorate of Developmental Welfare
Services in Namibia. The Literature Review gives comprehensive
explanations of the importance of Social Work supervision in
delivering social services. It provides opinions of different
authors and their views on the nature of supervision, the
supervisory needs of supervisees, and the functions and roles of
supervisors. The Research Methodology chapter looks at how the
research was done, how the researcher gathered information and the
tools that were used. The research data was examined using the
quantitative data analysis approach. Data analysis and
interpretation breaks down the data obtained from the
questionnaires, which are the instruments that the researcher used
to obtain the information. The result consists of graphs and tables
that clearly present the data. This chapter also covers the social
structure of Namibia, a brief description of the regions in Namibia
in order to help the reader understand the distances between
offices. The Conclusions are a summary of the findings obtained
from the respondents on the nature of supervision in the
directorate. Possible solutions to improve supervision in the
directorate are addressed in the recommendations. One of the
recommendations is that the directorate should develop standardised
supervisory
guidelines.

▼ The Alberta Nutrition Guidelines for Children and
Youth were released in June 2008 aiming to provide schools with a
tool to help students make healthy food choices. A multiple case
study design (n=3) was used to identify what factors influenced the
adoption and implementation of the guidelines and what healthy
eating strategies case schools implemented due to the guidelines.
Barriers and facilitators to implementation were also investigated.
Semi-structured interviews and direct observations were used to
collect data that were later analyzed using content analysis.
Various healthy eating strategies were implemented within Cases A,
B and C after the uptake of the guidelines. Support from the school
superintendent and the work of a health champion were commonly
described as facilitators to the adoption and implementation of the
guidelines. In contrast, parents represented a commonly described
barrier. This reinforces the importance of comprehensive school
based programs that involve parents and the school
community.

▼ Objectives: Cervical cancer screening
guidelines have been revised, and now emphasize evidence-based medicine, resulting in recommendations for less frequent screening; however, these changes have been met with concern by both patients and providers. Understanding the patterns of acceptance versus reticence to accept these
guidelines by health systems, providers, and patients is critical to developing successful strategies for translating policy change into routine practice. Here we fill some of these knowledge gaps by incorporating both actual cervical cancer screening practice data and patient perspectives towards HPV testing and screening interval changes through the following specific aims: 1a) describe the uptake of co-testing and examine the correlates of receiving an HPV co-test, 1b) estimate the length of time until the next screening test following either a negative Pap smear alone or a dual negative co-test, and 2) investigate the correlates of reluctance to
adhere to revised
guidelines, which recommend the addition of HPV testing along with less frequent cervical cancer screening. Methods: Using cervical cancer screening records from the Pathology Data Systems (PDS) at Johns Hopkins Hospital, we estimate temporal trends in choice of screening strategy (cytology alone or with HPV DNA testing) and the interval between successive screening tests in routine clinical practice. We then incorporate the patient perspective by using data collected in the HPV in Perimenopause (HIP) natural history study of women age 35-60 years, we will compare women who indicate willingness versus reluctance to accept alternative screening strategies such as HPV testing and a longer interval between cervical cancer screening tests. Results: In clinical practice, we saw a significant increase in use of HPV co-testing over the last 10 years, reaching almost 80% of screening tests. We also saw a significant increase in time between screening tests following a
dual-negative co-test to almost 3 years, but essentially no change in time to next screening test following a normal Pap smear, remaining near 1.5 years. Among patients, we found a majority of study participants indicated a willingness to adopt a cervical cancer screening strategy of cytology alone or Pap-HPV co-testing every 3 years if recommended by their physician, but remain concerned about primary HPV testing and co-testing with 5-year screening intervals. Conclusion: HPV testing was incorporated into screening with an assumption of less frequent screening due to its greater sensitivity and negative predictive value. While intervals have increased following a co-test, more time will be needed to see whether they reach 5 years as recommended. We also found evidence of continued reticence to accepting newer HPV-based screening algorithms among routinely screened women over age 35, highlighting a need for more patient education regarding the use and meaning of HPV
testing.

▼ BACKGROUND: Antibiotics are essential medicines and their effectiveness is under threat due to antimicrobial resistance. Guidelines are one way to conserve antibiotic effectiveness given that they are intended to modify clinician prescribing. Guidelines that provide antibiotic recommendations should make explicit contextual considerations that influence antimicrobial resistance and their downstream effects on resistance emergence. METHODS: We conducted a systematic review of tuberculosis, gonorrhoea, and respiratory tract infection guidelines and recommendations to examine how and to what extent they are considering contextual factors that influence antimicrobial resistance. We also investigated whether there are guidelines and recommendations that can be adopted or adapted to local contexts. RESULTS: We found that within 74 included guidelines, two thirds of recommendations considered antimicrobial resistance. Of which only five guidelines considered all factors required to consider local aspects such as values, resource use, acceptability, feasibility, and equity. As such, these five guidelines can be either adopted or adapted to Canadian and other contexts. We also found that 39% of guidelines met credibility scores of 60% or greater in AGREE II domains: scope and purpose, rigor of development, and editorial independence. CLINCAL IMPLICATIONS: There are very few Infectious disease guidelines for highly prevalent diseases that do not consider all important contextual factors may influence antimicrobial resistance. Our findings can support societies and organizations, public health policy, and health care stakeholders to develop and implement guidelines that are applicable to local contexts efficiently and resourcefully. Our antimicrobial resistance recommendation framework, used in addition to GRADE Evidence to Decision frameworks, is a start to having this come to fruition.

▼ Sediments have been recognised as an important source and repository for many contaminants that enter the aquatic environment. In addition to influencing surface water quality, contaminated sediments also represent a potential hazard to benthic biota and the aquatic food chain. Accurately defining the extent of environmental risk posed by sediment-associated contaminants is becoming an important priority for regulators and industries worldwide.
Sediment quality guidelines (SQGs) are intended to delineate between sediments that cause adverse biological effects and those which do not. In the case of metals, however, an absence of toxicity is frequently observed in sediments with metal concentrations many times greater than the SQG trigger values. Factors that modify metal partitioning between dissolved and particulate phases in sediments (e.g. particle size, organic carbon, sulfide, pH) and thereby influence metal bioavailability and toxicity are believed to be responsible for the disparity. Further, the significance of exposure pathways (dietary particle ingestion and dissolved metal uptake) to a range of representative benthic organisms must also be understood to derive SQGs that offer protection for a full range of organism behaviours and benthic habitats.
Laboratory-based experimentation was undertaken to assess the sensitivity of benthic organisms to copper in sediments and the appropriateness of current SQGs for copper. Detailed information was obtained on the equilibration of copper-spiked sediments and the influence of sediment properties on the partitioning of copper between different sediment compartments (solid phases and dissolved copper in the pore water and overlying water) before and during tests. The research focused primarily on acute toxicity to sediment-dwelling estuarine and marine organisms, including the epibenthic amphipod, Melita plumulosa and the bivalve, Spisula trigonella, for numerous sediments with varying metal-binding properties.
The 10-day acute EC50s of the particle-ingesting amphipod varied from 200 to 20,000 mg/kg for sediments with copper-binding affinities ranging from low (sandy) to very high (mineralised). Similar patterns of acute toxicity across the varying sediment types were observed for the bivalve, despite having a dissolved metal uptake exposure pathway. Of the factors expected to influence copper binding, elevated acid-volatile sulfide, was demonstrated to temporarily ameliorate the bioavailability of copper, and sediment pH had a minor effect on dissolved copper toxicity. Far more influential were the sediment particle size and organic carbon content. Concentration-response relationships based on the total particulate copper and organic carbon (POC) concentrations of the less than 63 μm sediment fraction effectively normalised the influence of sediment properties on the measured copper effects threshold for all sediment-types tested. Em 1928, deu-se o início de uma revolução no universo farmacêutico com a descoberta da penicilina por Alexander Fleming. Este acontecimento abriu portas para a criação e desenvolvimento de muitos outros antibióticos, o que permitiu o tratamento de infeções até então associadas a elevadas taxas de morbilidade e mortalidade. Contudo, o que se julgava ser a solução para muitos problemas acabou por condicionar uma situação que, nos dias de hoje, exige a máxima atenção por parte de todos os profissionais de saúde
e entidades reguladoras: o surgimento de microrganismos resistentes à terapêutica antibiótica. Assim, tornou-se essencial uma adaptação das guidelines existentes, de forma a estarem ajustadas à realidade clínica que se encontra em constante mutação. Estas deverão apresentar princípios que visem a prevenção do surgimento de resistência, como sejam a prescrição de antibióticos de espectro estreito associada a uma duração de tratamento adequada. Esta monografia pretende abordar as infeções adquiridas na comunidade mais relevantes e os regimes terapêuticos associados; os microrganismos resistentes mais comuns e as diferentes taxas de resistência na Europa e no resto do mundo; as taxas de consumo das diferentes classes de antibióticos em ambulatório; o impacto das resistências antibacterianas em saúde pública e na economia e, por último, as estratégias de combate às resistências na comunidade.

▼ Effective patient care requires interprofessional collaboration and decisions based on clinical guidelines. The goal of this study was to determine how interprofessional collaboration influences the introduction of nursing Best Practice Guidelines. This study was a secondary qualitative analysis of data obtained from interviews and reports from two selected sites (long term care and community health care) that demonstrated interprofessional collaboration during the introduction of nursing Best Practice Guidelines. Findings emphasized the importance of communication, the role of an interprofessional team, and the understanding of the roles of all involved in the introduction of discipline specific clinical guidelines. In addition, unregulated staff members were involved in leadership roles and their work was important in providing effective interprofessional collaborative care during the introduction of guidelines. Therefore, it is essential to appropriately involve all members of the interprofessional team, regardless of discipline or educational level, during the introduction of clinical guidelines.

▼ The importance in finding components that may result in won market advantages for an organization has increased in the latest decade, as a result of the increasing competition across a range of industries. One factor that has been shown leading to efficient products and benefits in terms of time- and resource savings has been an implementation of user experience interaction guidelines, UXIG. These are guidelines with the purpose to improve the products within an organization out of a holistic perspective and increase the interaction between the product and its user, in a positive way. This study has been conducted at GE Healthcare Life Sciences, in Uppsala, focusing at chromatography. The purpose of this study was to investigate how to create and implement UXIG at this, large, international organization – with a heterogeneous product portfolio. In order to come up with recommendations in these questions a benchmarking study, interviews and a questionnaire was made. The purpose with the interviews and questionnaire was to find out how other organizations and their employers had been acting and thinking when reasoning about these questions whereas the benchmarking study consisted of a comparison of famous UXIGs. The result of this study shows that e.g. preparation, an overall clarity from the creators/management, a strong communication channel between the management, creators of the UXIG, its users and the users of the systems, how they are matching to the organization, their availability and structure, are important components in order to create/ develop UXIG in a successful way.

▼ While many clinical practice guidelines that use the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach have ample published evidence to support their recommendations, certain situations (e.g. rare, rapidly emerging, or understudied diseases) may be limiting. As a result, it is common for guideline developers to seek out indirect evidence from other areas to fill in these evidence gaps. The GRADE evidence-to-decision (EtD) framework, which offers a structured and transparent development process for guidelines, includes additional research evidence domains (e.g. feasibility, acceptability, equity) for panels to consider in their decision-making process. This confluence of both considerations of indirect information and increasing literature domain sources to draw from when making decisions weighs on the simplicity of literature presentation. Herein, firstly, we described an example of specific decision-rules for including indirect evidence and the implications of the rules for presenting results to decision-makers. Secondly, we provided a comprehensive overview of how guideline developers currently report economic information across GRADE evidence-to-decision frameworks. Lastly, we ranked the most important study characteristics suggested in the literature by economists to consider as decision-rules when assessing indirectness (transferability) of economic evaluations chosen as research evidence in a GRADE guideline. We conclude that developers, with the help of their panels, should work to establish and report clear decision-rules and the rationale for indirect evidence that they select for their clinical practice guidelines. This has the potential to simplify the presentation of indirect evidence for panels and developers, as well as, to reduce decision-making confusion, time demands and guideline funder costs.

While many clinical practice guidelines have ample published evidence to support their recommendations, certain situations (e.g. rare, rapidly emerging, or understudied diseases) may be limiting the available literature. As a result, it is common for guideline developers to seek out indirect evidence from other, but related areas, to fill in these evidence gaps. Selection of available indirect evidence may be better than basing decisions on no evidence, in particular in situations of clinical equipoise. However, including all potentially relevant indirect evidence may represent an overuse of evidence. Indirect evidence refers to information sources with related populations, interventions, outcomes or comparisons, which could reasonably be extrapolated; but, are not entirely specific to the research topic at hand. This confluence of both indirect information considerations and desire by many for increasing literature sources to draw from weighs on the simplicity of an overall summary of literature presented during a guideline recommendation decision-making process. ▼ Developing mathematical formulations for design problems requires determining an objective function to compare design alternatives, a set of design features and options to be included for consideration, and a predictive model that reflect any unavoidable cause and effect relationships that are relevant. There are no formal principles guiding the formulation process, and heuristics prevail. There are some instances when the level of effort required to formulate the problem and solve the problem are excessive, not worth the improvement that might be realized in the overall design objective. In other words, a hypothetically “perfect” design problem formulation that takes all possible factors into account might be so difficult to fully compose and solve that it is not worth the effort. This thesis presents a set of
guidelines for formulating design problems that seeks a middle ground. The method presented defines three different tasks in the formulation process: comparison metrics, predictive model and design representation. Each task offers opportunities for the practitioner to balance the expected quality of the solution with the level of effort and time required to reach that solution. This thesis demonstrates how using the
guidelines can help create alternative formulations for the same design problem, and then how the resulting solutions can be evaluated and compared. Using a vibration absorber design example, the
guidelines are enumerated, explained, and used to compose six alternative optimization formulations of the problem. These alternatives formulations vary in objective functions, decision variables, and some other design formulation practices. The overall goal is to maximize surface finish quality of a machined component processed on a platform to which the vibration absorber is attached. Vibrations of the platform can have a detrimental effect on surface quality. The goal of the vibration absorber system is to minimize these detrimental effects. The six alternative optimization formulations are subsequently solved, and their scores reflecting their complexity, computational time and solution quality are quantified and compared. The results illustrate the unavoidable tradeoffs among these three attributes. The best formulation depends on the set of tradeoffs that are best in that situation, given the decision maker’s risk attitude and preference.

guidelines in status epilepticus. Status epilepticus (SE) is a common neurological emergency associated with significant morbidity and mortality. There is a gap between scientific evidence and the management of SE. To decrease this gap, clinical
guidelines, based on available evidence are employed.Due to a gap between published clinical
guidelines on status epilepticus SE and clinician management of SE, a systematic review of the literature was performed to investigate treatment adherence to SE. This review provides preliminary evidence that non-adherence to SE
guidelines negatively impacts on patient outcomes. Appropriate and timely treatment is imperative for rapid seizure termination and improving outcomes.Treatment of children within NSW with status epilepticus transported by NETS to a tertiary paediatric hospital, over 1 year, was evaluated. The variation in management from the NSW clinical practice guideline (CPG) was assessed. This study identified there was non-adherence to the
guidelines and delay to treatment of SE. It also identified excessive use of benzodiazepines, and established higher doses were associated with significantly higher odds ratio for intubation.A multicentre study evaluated the clinical management of a series of paediatric patients presenting with SE in a paediatric tertiary referral hospital and a mixed adult and paediatric emergency department and also established there was non-adherence to the
guidelines and delay to treatment. There was excessive use of benzodiazepines and delay in administration of appropriate second-line treatment of SE.In conclusion, studies undertaken in this thesis have established that there is excessive administration of benzodiazepines and a delay in escalation of treatment to an appropriate second-line therapy in the treatment of SE in children. historic elements present within the urban section of Dadar West. The thesis also evaluates, the city’s preservation guidelines with respect to historic elements within the Dadar West study area that are missing from the city’s preservation program. This
evaluation identifies the strengths and weaknesses of the city’s preservation model. The thesis concludes with recommendations for strengthening the city of Bombay’s preservation program.

▼ This thesis explores the Rapid Recommendations process, a new responsive way of creating clinical practice guidelines.
Healthcare workers rely on clinical practice guidelines to inform their practice. However, most guidelines are not trustworthy when judged by accepted standards and they typically take several years to produce. Guideline trustworthiness is undermined by panel members who often have conflicts of interest, by including representation from only a subset of stakeholders, by failing to examine the entirety of the evidence systematically, and by rapid obsolescence. Further, they are often difficult for users to understand in limited time.
Rather than updating guidance on a fixed schedule, the Rapid Recommendations approach involves continuous monitoring of the literature and produces guidelines in response to new potentially practice-changing evidence. A collaborative network of clinicians, methodologists, and patients respond by rapidly producing trustworthy evidence syntheses and guidance. We have identified efficiencies at every step of the guideline development process.
The guideline panel does not include anyone with a financial conflict of interest and there are strict limits professional and intellectual conflicts. Systematic reviews are produced on the relative effects of each option, on prognosis, and on patient values and preferences with the explicit intent to inform the question at hand. The panel also considers practical issues. Rapid Recommendations are published in a concise multilayered user-friendly format headed by an interactive infographic that contains all of the necessary information for users need to make informed decisions at the point of care. The guideline is published simultaneously in print and electronically, including decision aids that can be used at the point of care and integrated into electronic medical records.
In this thesis, you will find a selection of exemplary publications relevant to the Rapid Recommendations process. We show that a responsive approach to rapid and trustworthy guideline creation is possible. It represents a way forward from the current limitations that plague most current clinical practice guidelines.

Healthcare workers often decide what to do in practice based on the advice of experts through clinical practice guidelines. However, most clinical practice guidelines are not completely trustworthy. Guideline authors often have conflicts of interest, do not include patients or patient views, and are created so slowly that they rapidly fall into obsolescence. This thesis explores a new way of developing clinical practice guidelines that we call Rapid Recommendations. Instead of creating them on a fixed schedule (i.e., every few years), they are created in response to new studies that might change practice. clinical
guidelines. The aim
of this study was to use the AGREE II
instrument to evaluate the variability of the
quality of selected
guidelines, to determine a baseline for the quality of current
guidelines and determine whether
guidelines demonstrated good
standard practice
during their development.
The AGREE II
instrument was used to assess a selection of
guidelines published
between January 2012 and June 2013. Eleven
guidelines were selected
for review.
Overall,
guidelines scored highest in domain 1 (Scope
and purpose) and 4 (Clarity of
presentation); and lowest in domain
3 (Rigour of development) and 6 (Editorial
independence) with the
overall assessment score of three out of seven. The study
demonstrated that the quality of
guidelines was variable and that
there are
deficiencies in the guideline development process. The
results from this study
provide a baseline to measure the quality
of future
guidelines.

has performed a study on tax incentives for renewable energy. The
CRS has found that tax incentives are effective, powerful and
highly flexible instruments to encourage the development of
renewable energy. They have further identified various types of tax
incentives and various
guidelines for effective tax incentives. The
research question was whether the tax incentives and
guidelines as
identified by the CRS are internationally acceptable and whether
South-Africa can consider these tax incentives and
guidelines to
design and implement a renewable energy tax incentive policy of an
international standard in South-Africa. Chapter 1 contains the
background, research question, objectives and methodology of this
study. In Chapter 2 the advantages and disadvantages of each type
of tax incentive, as identified by the CRS, were analysed through a
literature study of international views on these tax incentives. In
Chapter 3 the
guidelines for effective renewable energy tax
incentives, as identified by the CRS, were analysed through a
literature study of international views on these tax incentives.
Chapter 4 contains the conclusion that the tax incentives and
guidelines as identified by the CRS are internationally acceptable
and that South-Africa can consider these tax incentives and
guidelines to design and implement a renewable energy tax incentive
policy of an international standard in South-Africa.

▼ It is estimated by the World Health Organisation (WHO, 2004) that globally, over 50% medicines are prescribed inappropriately. In Namibia, suboptimal prescribing practices have been reported in previous medicine use surveys. Prescribing patterns in the out-patient settings have however not been evaluated in the Khomas Region. The aim of this study was to evaluate the medicine prescribing patterns in out-patient departments of public health facilities in the Khomas Region, Namibia.
The study adopted the WHO descriptive cross-sectional observational design for a medicine use survey using both quantitative and qualitative methods of data collection. The study was conducted at three levels of health care: Hospital, Health Centre and Clinic. Data on prescribing indicators were collected from patient prescriptions and data on prescribers’ use of treatment guidelines was also collected. The main outcome variables were the use of STGs in prescribing of medicines at OPD units and five prescribing indicators recommended by WHO.
A total of 1,243 prescription records were reviewed and 37 prescribers participated in the study. Four out of the five prescribing indicators were below acceptable prescribing thresholds clearly showing that rational prescribing in Khomas Regions is suboptimal. The very high level of antibiotic prescribing (69%) is very worrying and will directly lead to increase in Anti-microbial resistance. The average of over three medicines per prescription shows that there is a degree of polypharmacy and the low level of generic prescribing only (64%) is also worrying. Adherence to STGs was found to be 73%, which is better than found in some other studies but still below the acceptable level. The main factors driving the use of STGs were access to STGs and/or training; access to STG recommended medicines at the health facility and the simplicity in indexing of the STG. The study recommends implementation of a prescribing performance management systemto standardize, continuously monitor and improve on the prescribing of medicines at OPD units in Khomas Region.

performed a study on tax incentives for renewable energy. The CRS
has found that tax incentives are effective, powerful and highly
flexible instruments to encourage the development of renewable
energy. They have further identified various types of tax
incentives and various
guidelines for effective tax incentives. The
research question was whether the tax incentives and
guidelines as
identified by the CRS are internationally acceptable and whether
South-Africa can consider these tax incentives and
guidelines to
design and implement a renewable energy tax incentive policy of an
international standard in South-Africa. Chapter 1 contains the
background, research question, objectives and methodology of this
study. In Chapter 2 the advantages and disadvantages of each type
of tax incentive, as identified by the CRS, were analysed through a
literature study of international views on these tax incentives. In
Chapter 3 the
guidelines for effective renewable energy tax
incentives, as identified by the CRS, were analysed through a
literature study of international views on these tax incentives.
Chapter 4 contains the conclusion that the tax incentives and
guidelines as identified by the CRS are internationally acceptable
and that South-Africa can consider these tax incentives and
guidelines to design and implement a renewable energy tax incentive
policy of an international standard in South-Africa.

Clinicians in general value the use of the best evidence in decision-making and consider
that can improve patient care. However, a successful evidence based practice is hard to
achieve in real life. In recent years, with the consolidation of the Grading of
Recommendations Assessment, Development and Evaluation (GRADE) approach, the
development of improved standards to judge the trustworthiness of guideline
recommendations and the adoption of more strict policies to limit the influence of
conflict of interests, trustworthy guidelines have become an attractive alternative for an
evidence-based clinical practice.
In this thesis we offer an explicit and easy-to-use guidance to clinicians regarding how to
use guideline recommendations in the context of a real life practice. We also provide an
in-depth explanation of the judgments involved in determining the direction and strength
of recommendations. Finally, we expand the knowledge about how to manage conflict of
interests in guideline developers. Through two studies evaluating the conflict of interest
policy implemented at the American College of Chest Physicians 9th edition of the
Antithrombotic Guidelines, we show what aspects of the policy were successful and what
aspects need to be reformulated.

▼ This research was designed to establish better
guidelines for photocontrol systems in common daylighting applications. The
guidelines are based on a statistical analysis of the correlation between two control system calibration parameters: photosensor signal and the preferred dimming level. The ideal dimming level for a controlled lighting zone was the light output required to achieve the desired target illuminance at all points within the space. A 36’ by 25’ classroom was studied. Façade orientation, different daylight delivery systems, interior shading devices, furniture and photosensor characteristics were included in this research as study variables. Energy consumption and system calibration were carefully explored with a focus on the accuracy of photosensor system’s performance.
For a specific situation, scatter plots of the optimal dimming levels versus the photosensor signal for a specific situation numerically and graphically describe the sensor’s response to the daylight conditions. A tightly clustered optimal dimming level versus signal band indicates high correlation and better tracking of the daylight level with the space. A calibration condition that produces limited over-dimming across the work plane analysis points can be determined given the control algorithm. Points located on the calibration line at low dimming levels are suggested commissioning conditions. The background information on these points, such as the shading condition, the time of the day, sky condition and so on, can be obtained to support the commissioning process.
In this work, three metrics are applied to evaluate system performance: the correlation coefficient between the sensor signal and the work plane illuminance; the Root Mean Square Error (RMSE) between the ideal dimming levels and those predicted by the calibration line, and the energy savings provided by the calibration compared to the optimum.
According to the simulation results, the S/E ratios between different interior shade setting heights serve as the key to photosensor performance. These ratios largely depend on the setting of the interior shading devices. A photosensor location with a relatively constant S/E ratio across different shade settings is desirable. Light shelves, in many cases, help to improve photosensor performance. In spaces with light shelves, a sensor with a wider spatial sensitivity distribution usually provides acceptable performance, while a sensor with a narrower spatial sensitivity distribution is better for spaces without light shelves. A sensor with its maximum sensitivity angled toward the interior back wall reduces daylight S/E ratio fluctuation. In most applications, the photosensor response to daylight has a higher S/E ratio than for electric light, and thus the daylight signal dominates. However, in cases when luminaires with indirect lighting are in use, the electric light signal can dominate the system and lead to system malfunctioning. ▼ People are seeking health information from a wide variety of sources. The comprehensive information in clinical practice guidelines (CPGs) represents an excellent source of evidence based information which should be communicated to this audience. Currently, there is little research about how to write a version of a CPG that would be easily accessible to people and more information is needed to identify barriers and supports, and potential solutions to disseminate CPGs to this audience (i.e. patients and the public).
This thesis represents a body of research consisting of four scientific papers with an overarching objective to understand and explore how CPGs and recommendations primarily developed and written for health care professionals can be disseminated to patients and the public. A CPG was developed using the rigorous methods of the GRADE approach; a randomised controlled trial was conducted to evaluate a format to disseminate synthesised evidence to patients and the public; a systematic review of the literature with a thematic and narrative synthesis of patient and public attitudes towards and awareness of CPGs was performed; and a qualitative description and content analysis of a sample of patients versions of CPGs was conducted. The studies found that people are interested in patient versions of CPGs for a variety of purposes, such as for decision making, as a tool to prepare for consultations with health care providers, and as advice for self-care management. However, barriers to their use may include lack of personalisation of information, negative attitudes towards guidelines as ways to restrict and control access to care, and lack of understanding of the recommendations and the evidence. A format to disseminate the evidence from a guideline is proposed, but future research should focus on strategies to personalise the information, to overcome the negative attitudes towards guidelines, and to communicate the recommendations and the evidence informing the recommendations.

guidelines for improving
cooperation between public libraries and school libraries in the
Eastern Cape Province of South Africa. A literature review
contextualizes recent developments in the library and information
services (LIS) sector, as well as cooperation initiatives abroad
and in some of the country’s provinces. Data was collected through
questionnaires distributed to public and school librarians,
interviews with senior managers in the public library and school
library sub-sectors of the Eastern Cape Province, and from official
records and policy documents. Analysis and interpretation of this
data provided a platform for developing
guidelines for cooperation,
and indicating what conditions as well as what roles and
responsibilities at national, provincial, and local levels are
necessary for their successful implementation. The
guidelines,
while specifically informed by data relevant to the Eastern Cape
Province, follow the broader pattern set out in the recent
Strategic
Guidelines for Collaboration between Community Libraries
and Schools 2013-2015 document crafted by the National Department
of Basic Education (DBE) and the National Department of Arts and
Culture (DAC).

▼ As a result of the Good Start, Grow Smart Initiative (GSGS) in 2002, states began developing Early Learning Guidelines (ELGs), which are utilized by state funded programs for children ages 3 to 5. Although GSGS recommended that states include literacy, language, and pre-reading skills in ELGs, many states developed more comprehensive documents that included broader ranges of skills and activities consistent with each state's conceptualization of what constitutes quality learning environments for this age group. Consequently, music content, if included in ELGs, may vary from state to state.
Three national standards documents may have informed music standards included in ELGs, such as The National Music Standards for Prekindergarten, Head Start Child Development and Early Learning Framework, and the National Association for the Education of Young Children (NAEYC) All-Criteria Document. I conducted a content analysis of state ELGs: 1) to determine what musical behaviors and activities states have codified as official knowledge within quality learning environments for young children, and 2) to ascertain the extent to which the musical content reflects music standards included in national prekindergarten standards documents. Findings indicated that the broadly-framed Head Start and NAEYC music indicators were well-represented in state ELGs. The majority of ELGs have addressed two of the Content Standards of The National Music Standards for Prekindergarten, "Singing and Playing Instruments" (88%) and "Responding to Music" (74%). The Content Standards "Creating Music" and "Understanding Music" were addressed in far fewer ELGs (27% and 33%, respectively). The more prescriptive Achievement Standards were less frequently included in ELGs. Additionally, 14.33% of all music-related indictors included in state ELGs did not reflect the music content of any of the national standards and guidelines; rather, the indicators represented additional music content (for example, audience skills), music activities that promote social and emotional development, and facilitating learning in other curriculum areas. When ELGs were examined through a conceptual framework of policy and Developmentally Appropriate Practice, a co-mingling of standards and guidelines constructs was found, suggesting that state ELG committees attempted to balance the opposing segments, resulting in re-contextualization of the content selected for inclusion in the ELGs.

Os maus-tratos a animais são um tema pouco abordado na área da Medicina Veterinária em Portugal. Esta dissertação de mestrado incide sobre a função do médico veterinário na sociedade atual e, especificamente, sobre como se deve proceder durante uma visita técnica cujo motivo seja suspeita de maus-tratos a animais. Atualmente, este profissional já não é visto apenas como um médico do animal mas também como um médico de família, capaz de identificar situações de abuso que podem estar integradas num ciclo de violência. Os fatores que podem levar à suspeita de maus-tratos são diversos, sendo fundamental a avaliação, não só do animal, mas também do seu detentor e do meio em que estes vivem. As situações de maus-tratos podem advir da realização de uma ação, ou, pelo contrário, estar na origem de diferentes categorizações dos maus-tratos a animais e, por sua vez, a variações no desfecho dos casos. Existem diretrizes que podem auxiliar na condução da análise de um caso, no entanto, os fatores Homem e animal variam de caso para caso obrigando o médico veterinário a adaptar-se a cada uma das situações. A continuação da discussão e a formação dos profissionais de saúde animal relativamente a este assunto pode garantir um aperfeiçoamento na identificação e resolução de situações de maus-tratos.
O desenvolvimento deste tema teve como base uma experiência de estágio no Centro de Recolha Oficial de Animais do Município da Amadora (CROAMA), e participações em Intervenções de Larga Escala, integrando, como auxiliar, a Equipa Médico-Veterinária responsável pelo trabalho em campo.
Como material de estudo, são referidos quatro casos; dois deles relativos ao período de estágio no CROAMA e, os outros dois, referentes à participação nas Intervenções de Larga Escala.
ABSTRACT - Animal abuse is a subject rarely addressed in the area of veterinary medicine in Portugal. This master's thesis deals with the role of the veterinarian in today's society and specifically on how to proceed in a situation of abuse. Currently, this professional is not only seen as a doctor of the animal but also as a family physician, capable of identifying situations of misstreat that may be integrated in a cycle of violence. There are several factors that can lead to the suspicion of abuse, being essential the evaluation, not only of the animal, but also of its holder and the environment in which they live. These situations may arise from the performance of an action, or, on the contrary, be the source of different categorizations of animal mistreatment and, in turn, variations in the outcome of the cases. There are guidelines that can assist in conducting the analysis of a case, however, the human and animal factors are case to case variables requiring the veterinarian to adapt to each of the situations. Continued discussion and training of animal health professionals on this subject can ensure a better identification and resolution of abuse situations.

▼ This research study explored and described a partogram as a clinical practice guideline for monitoring and managing women during labour at Vhembe District of Limpopo province.
A qualitative, explorative, descriptive and contextual design was used in order to achieve the study objectives. Purposive sampling was carried out to select three hospitals in order to obtain comprehensive data from the district. The research study was conducted in three phases.
Face-to-face semi-structured interviews were conducted with 17 midwives who were selected conveniently from the three hospitals. Document study was also conducted on 24 partograms that were conveniently selected from the three hospitals on the days that interviews were conducted, using a self-designed checklist.
Tesch’s eight steps of qualitative data analysis were used to analyse the data, and simple, descriptive statistics using frequencies were used to analyse the data obtained from the document study.
Four themes that emerged from the data are: benefits of implementing the partogram, challenges experienced, attitudes of staff and evaluation on how the guideline is being implemented in the wards, including giving feedback. These aspects were indicated as pivotal in the implementation of the guideline in monitoring a woman during labour. The findings on document study revealed gaps in recording, mostly on aspects that needed frequent observations and on aspects in which resources that needed to be used in monitoring were lacking.
The strategies to improve the implementation of a guideline were developed based on the research findings, sent to one of the hospitals for review and comments were included in the discussions. It is recommended that in-service training on the implementation of the partogram be conducted regularly for midwives. Hospital management and supervisors need to provide support, enough staff and equipment in order to enable midwives to use the partogram
