Objective: The purpose of our study is to determine clinical and evolutive factors to ESRF of the children diagnosed of primary focal segmental glomerulosclerosis. Patients and methods: Retrospective study. Patients diagnosed of FSGS in Materno-Infantil Vall d'Hebron Hospital during the years 1979 to 2000. During this period 42 patients have been diagnosed of FSGS, 22 children and 20 girls of FSGS. The mean age at the time of diagnosis was 4.5 years. The main symptoms of diagnosis were: nephrotic syndrome (64%) and isolated proteinuria (17,5%). In the moment of the diagnosis 4 cases presented renal failure (9,5%). Thirty-five patients were initially treated with steroids reasing a remission in 20%. We found a survival rate of 64% after 5 years, 48% after 10 years and 38% after 15 years. The mean follow-up period was 131 months. Twelve transplants in 9 children of which they continue running 6 have been practiced upon finishing the study. Three patients have died during the evolution. Results (predictors factors to ESRF): It has not been any differences according to the sex or to the age. The presence of family antecedents of the illness, the consanguinity or the ethnos gipsy marks a worst presage. In relation to the symptom of diagnosis, the children that debut with isolated proteinuria present worse prognosis that those who debut with nephrotic syndrome. The presence of haematuria or the level of proteinuria does not represent a superior risk of evolution to ESRF. However, maintained hypercholesterolemia or the presence of arterial hypertension are associated with more risk. The presence of focal hyalinosis, glomerular sclerosis or absence of hypercellurarity are associated with worst prognostic. In relation to the treatment, the initial positive answer to the steroids represents a factor of good renal presage. In a logistic regression analysis, the factors associated with evolution to ESRF are: family antecedents, consanguinity, gipsy, arterial hypertension, trombocitosis and hypercholesterolemia., Conclusions: The factors of wrong prognostic to ESRF in our sample are the presence of family antecedents, consanguinity, gipsy ethnos, isolated proteinuria, the maintained hypercholesterolemia, thrombocytosis, arterial hypertension, presence of focal hyalinosis, glomerular sclerosis and the bad answer to the steroid treatment Advisors/Committee Members: [email protected] (authoremail), true (authoremailshow), Casaldàliga i Ferrer, Jaume (director), Callis (director).
