▼ Background Human papilloma virus (HPV) has been associated to conjunctival squamous cell carcinoma (CSCC) and has been documented in studies originating from different parts of the world. In Zambia, eye cancers are on the increase and between 2005 and 2009 they were ranked number three (3) among the top five (5) cancers of which conjunctival squamous cell carcinoma (CSCC) (78%) was the commonest. In this study, the aim was to establish the prevalence and association of HPV infection with CSCC at the University Teaching Hospital (UTH) and Lusaka Adventist Eye Hospital, Zambia. Materials and Methods This was a cross section study conducted at UTH and Lusaka Adventist Eye Hospital, Zambia for a period of three (3) months. A convenient sampling method was used. Conjunctival biopsies were collected from patients aged over 20 years with clinically suspected CSCC and met an inclusion criteria making the sample of 37 participants. Histopathology and polymerase chain reaction assay (PCR) analysis of 37 specimens of conjunctival lesions was undertaken. Tissues confirmed as CSCC (21) were grouped as cases and non-CSCC tissues (16) as controls. Findings were analysed using statistical software SPSS for windows version 18.0. Univariate analysis of variables which were CSCC, HPV, and HIV was conducted to determine the distribution patterns of HPV and HIV. Odds ratios (ORs) and 95% confidence intervals (CIs) were computed stratifying by age, sex, human papilloma virus and HIV status. Results This study revealed that, 59.5% of the conjunctival tissues were malignant of which 95.5% were CSCC and 4.5% were Kaposi’s sarcoma. Most of the participants 57% were HIV positive. 71.4% of patients with CSCC were HIV positive and HPV DNA was detected in 71.4% of CSCC tissues. HPV DNA was detected in 52.4% HIV+ patients and in 62.5% HIV- patients. Conclusion The study showed a high prevalence of HPV infection (71.4%) in CSCC. HPV and HIV infections were both associated with conjunctival SCC. Though HPV infection was found in patients with HIV, there were no significant differences in the results between HIV positive and HIV negative patients.

▼ Chlamydia is the most common cause of sexually transmitted bacterial infection worldwide and it leads to severe complications for women who become infected. Following initial infection, the bacterial pathogen ascends from the lower genital tract to the upper genital tract leading to pelvic inflammation and scarring of inflamed tissue. The scarring can lead to ductal obstruction, causing retention of fluid in the upper genital tract and ovaries, termed hydrosalpinx. This inflammation due to Chlamydia infection can lead to infertility. Despite the commonality of the disease and its severe consequences, the molecular pathogenesis and mechanism of pelvic inflammation following Chlamydia infection has yet to be elucidated. We investigated the molecular pathogenesis and its relation to host autophagy here. The ATG5 gene is required for autophagy. Our lab has previously shown via mouse model that deletion of the ATG5 gene in myeloid cells results in a significant increase in upper genital tract inflammation following Chlamydia infection. To further investigate the role of autophagy in Chlamydia pathogenesis, mice with deleted ATG5 gene in epithelial cells were intravaginally infected with Chlamydia. The infection was monitored through weekly vaginal swabbing and calculation of Chlamydia bacterial titers from each swab. After Chlamydia infection cleared, the animals were sacrificed, upper genital tract pathology was observed and cytokine analysis was conducted on the vaginal swabs that were taken throughout the infection. The p62 autophagy protein acts as a scaffold to bring bacterial pathogen to autophagy machinery. In the current study, p62-deficient mice were obtained and infected with Chlamydia. Vaginal swabbing throughout Chlamydia infection allowed us to estimate bacterial titers and assay the host cytokine response to the infection, and finally the upper genital tracts of the animals were removed for observation. We found no significant difference in the pathology, bacterial clearance, or cytokine analysis between epithelial Atg5-deficient animals and wild type animals. This suggests that the Chlamydia bacteria may be employing mechanisms to evade or inhibit host autophagy in epithelial cells to promote its own survival. Additionally, there was no significant difference in the pathology, bacterial clearance, or cytokine analysis between the p62-deficient animals and wild type animals. Espécies de lactobacilos são os principais componentes da microbiota vaginal e a manutenção do predomínio lactobacilar é importante para proteção desse ambiente contra possíveis patógenos. A vaginose bacteriana é uma condição em que se observa a perda de lactobacilos e substituição desses microrganismos por espécies bacterianas, anaeróbias em sua maioria. Tal condição pode acarretar inúmeras complicações ginecológicas e obstétricas, como o aumento do risco de aquisição de infecções sexualmente transmissíveis, parto prematuro e baixo peso ao nascimento. O principal método utilizado para o diagnóstico da vaginose bacteriana é o proposto por Nugent et al. (1991) e se baseia na classificação da microbiota vaginal em flora normal, intermediária e vaginose bacteriana. Enquanto que o perfil imunológico e microbiológico da vaginose bacteriana tenha sido amplamente estudado, pouco se sabe sobre tais características na flora intermediária. Portanto, o objetivo desse estudo foi caracterizar a flora intermediária quanto aos níveis cérvico-vaginais de Interleucina (IL)1-beta, IL-6, IL-8, IL-10, fator de necrose tumoral (TNF)-alfa, antagonista de receptor de IL-1(IL-1ra), sialidases bacterianas e quanto às cargas de Gardnerella vaginalise de bactérias totais, além de verificar se o perfil geral observado na flora intermediária se assemelha ao de mulheres com flora normal ou com vaginose bacteriana. Foi realizado um estudo transversal que incluiu 526 mulheres não grávidas em idade reprodutiva. Deste total, foram constituídos os grupos de estudo de acordo com o padrão de flora vagina, segundo Nugent et al. Todos os 145 casos de vaginose bacteriana foram incluídos nas análises, bem como os 63 casos de flora intermediária e 145 das 318 mulheres que apresentaram flora normal. O Papiloma Vírus Humano (HPV), é considerado um problema mundial de saúde pública, sendo a doença sexualmente transmissível mais prevalente. Guarda uma relação direta com o risco e a incidência do câncer do canal anal. Seu diagnóstico, tratamento e seguimento são de extrema importância. Neste sentido o escovado do canal anal tem um papel fundamental no rastreamento e seguimento das lesões HPV induzidas e consequente evolução para o câncer anal. Determinar a ocorrência de lesão HPV induzida em mulheres que participam dos programas de prevenção do câncer de colo uterino nas Unidades Básicas de Saúde (UBS) no município de Botucatu. Trata-se de um estudo transversal observacional que teve 228 mulheres submetidas ao escovado do canal anal a fim de estabelecer a ocorrência de lesão HPV induzida e suas correlações com dados sociais e comportamentais. Os 11 casos que apresentaram alteração de ASCUS e LSIL no escovado do canal anal traziam relação com estado civil, baixa escolaridade, não prática do sexo seguro, e a prática do sexo anal

Human Papillomavirus (HPV) has been a world concern in Public Health, and it is the most prevalent sexually transmitted disease. It has a direct association with the risk and incidence of cancer in the anal canal. Its diagnosis, treatment and follow-up are extremely important. Using this approach, the smear of the anal canal has a crucial role in the screening and follow up of HPV-induced lesions and in the resulting development of anal cancer. To determine the occurrence of HPVinduced lesions in women who attended programs of uterine cervix cancer prevention in Basic Health Units (BHU) in Botucatu city. It is a cross sectional observational study, in which 228 women underwent brushing of the anal canal in order to establish the occurrence of HPV-induced lesion and its correlation with social and behavioral data. The 11 cases which had ASCUS and LSIL changes in the smear of the anal canal were associated with marital status, low education level, practice of unsafe intercourse and anal intercourse

▼ Background: Arias-Stella reaction is a spectrum of benign cellular changes which is associated with exogenous progestogens or pregnancy. Histologically, this reaction is characterized by striking nuclear atypia with or without clear cytoplasm and can potentially mimic clear cell carcinoma. The diagnostic difficulty may be encountered in small biopsy samples containing atypical clear cells and in which the drug history or pregnancy status is unknown. In order to exclude a diagnosis of clear cell carcinoma, immunohistochemistry is often used. Recently, two immunohistochemical markers, napsin A and hepatocyte nuclear factor-1-beta (HNF-1β) have been proposed to be useful in the diagnosis of endometrial clear cell carcinomas as other tumors containing clear cells, such as endometrioid carcinomas, are often negative. However, the expression of these markers in Arias-Stella reaction has not been fully evaluated. Aim: To study the frequency and extent of napsin A and HNF-1β expression in cases of Arias-Stella reaction in small biopsy samples. Design: Immunohistochemistry was performed on 41 endometrial and cervical biopsies. The extent of staining for each marker was assessed and classified as negative, <5%, 6-33%, 34-66% and >66%. Results: Thirty-nine cases were gestational and two were on oral progestogen. Six cases were endocervical polyps and one was an endometrial polyp, and the rest were endometrial biopsies. In each case, the Arias-Stella reaction was either of the secretory/hypersecretory type or monstrous cell type with various degrees of atypia. Stromal decidual change was found in 31 cases (75.6%). All 41 cases (100%) were positive for napsin A and HNF-1β. For napsin A, positive staining involving <33%, 34-66% and >66% of glands were found in 28 (68.3%), 13 (31.7%) and 1 (2.4%) cases, respectively. For HNF-1β, positive staining involving 34-66% and >66% of glands were found in 6 (14.6%) and 35 (85.4%) of cases, respectively. In 6 cases in which the Arias-Stella reaction was focal (<50%) and unaccompanied by any stromal decidual change, both markers were also positive. Conclusion: In our series, both naspin A and HNF-1β were positive in gestational and progestogen-induced Arias-Stella reaction. These markers may stain atypical cells with or without clear cytoplasm. Despite their usefulness in the histologic subtyping of an established carcinoma, both naspin A and HNF-1β are not useful for distinguishing carcinomas from Arias-Stella reaction. The interpretation of their staining in limited samples containing atypical glands should be made with caution.

▼ The goal of this research was to determine the in vitro and in vivo effects of the Selective Estrogen Receptor Modulators (SERMs), methyl-piperidino-pyrazole (MPP), raloxifene, ICI 182,780 and 17[beta]-estradiol on endometrial carcinoma cells in culture and on murine uterine tissue. These SERMs have been developed to target and understand the role of estrogen receptor action in estrogen-responsive organs. Based on their antagonistic actions, SERMs have both real and potential value in treating estrogen-responsive cancers, including endometrial cancer. The studies described herein verify that the SERMs MPP and raloxifene demonstrate partial agonistic effects in ovariectomized wild-type and ER-[beta] knockout (ER[beta]KO) mice but also induce apoptosis and proliferation in vitro in cultured endometrial cell lines, Ishikawa and RL-95. Thus, MPP and raloxifene exert apparently contrasting in vitro and in vivo effects due to their mixed agonist/antagonist action on murine uterine estrogen receptor in vivo. In addition to these data, I report gene expression changes in the uterus of mice treated individually or with the combination of 17[beta]-estradiol and the SERMs, MPP, raloxifene, ICI 182,780 are reported herein. A greater number of genes showed up- or down regulation when the mice were co-treated with estrogen and one of the SERMs than when dosed with one of the compounds alone. A combination of a SERM and 17[beta]-estradiol resulted in a combinatorial or synergistic effect. These data may be explained by the fact that dual administration of a SERM with [beta]-estradiol allows the compounds to bind combinatorially to heterodimers as well as homdimers of the two receptors ER[alpha] and ER[beta]. These studies provide a framework of how 17[beta]-estradiol and various SERMs act in the uterus and might result in future therapies for gynecological maladies, including endometrial cancer. Branchiuran crustaceans are fairly common parasites of freshwater fishes in Africa and are presently represented by not more than 200 described species belonging to four genera, namely: Argulus Miiller,1785; Dolops Audouin,1837; Chonopeltis Thiele,1900; and Dipteropeltis Calman,1912. Chonopeltis is, however, the only one of these genera which is endemic to Africa and species of this genus have been reported from a number of different freshwater fish hosts and localities throughout the Ethiopic region. Although 13 species of Chonopeltis have hitherto been described, lack of morphological detail and morphometric data in taxonomic and subsequent descriptions, has led to confusion and much controversy as to, for instance, the phylogenetic origin, identity and function of various structures. Consequently, the validity of the taxonomic identity or status and affinities of the various species are questionable. Lack of important morphological information has also caused a considerable number of gaps and deficiencies in our present understanding of various aspects of ecological and pathogenetic significance. In the present study, a comprehensive investigation on various aspects of the morphology, anatomy and histology of adult Chonopeltis specimens was conducted, using not only light microscopy, but also scanning electron microscopy (SEM), histological microtomizing and staining techniques and graphic reconstruction. This investigation is based on several specimens of Chonopeltis victori Avenant-Oldewage,1991 collected during surveys of freshwater fish parasites in 1990 and 1991 from four different fish hosts, namely: Labeo rosae S teindachner,1894; L. congoro Peters, 1852; L. ruddi Boulenger,1907 ; and Barbus marequensis A. Smith,1841, sampled at various localities in the Olifants River in the Kruger National Park, as well as several C. australis Boxshal1,1976 specimens found on L. capensis (A. Smith,1841) and L. umbratus (A. Smith,1841), sampled in Boskop Dam near Potchefstroom. By making use of the mentioned methods of investigation a more complete and reliable morphological description of the distinctive external structures, as well as the internal structures of the cephalic shield alae and especially the reproductive system of these parasites could be given. With the aid of semi-thin (2am) serial sections and graphic reconstruction the various components of the reproductive systems could clearly be identified and the mutual relations and histomorphology of these components be described, discussed and compared with those of other members of the Branchiura. In an attempt to determine the true taxonomic position and relationships of the recognised species of Chonopeltis, the original descriptions, morphometric data, geographical distribution, hosts and affinities are compared, summarised and discussed. The results of the present study reveal a horseshoe-shaped furrow dorsally that separates the alae from the cephalon. ▼ Background and purpose: Brachytherapy is one of the essential treatments in gynaecological cancers, including intra-cavitary irradiation (ICIr) for cervical cancers and vaginal cylinder brachytherapy (VBT)for corpus uteri cancer, and the techniques are evolving. The advancement draws concerns at the impact on intra-and inter-fractional applicator displacement as the immobilization adopted was unchanged. Therefore, this planning study aims at comparing conventional and modern brachytherapy and investigating on the dosimeric and biological impact on applicator shift to define an acceptable displacement tolerance through virtual simulation. Materials and methods: Fifteenpatients for ICIr and seven for VBT were recruited. Modern plans, 3D for ICIr and multichannel (MC) for VBT, were computed as delivered to patients, while conventional plans, 2D for ICIr and single channel (SC) for VBT, were generated retrospectively. The applicators were then virtually shifted in all plans from original to inferiorly with addition of 2 mm up to 10mm and to clockwise and anti-clockwise roll direction with increment of 3° till15°.The normal conventional plans were evaluated against modern plans and all mimicked shifted plans were compared against originals. Results: For ICIr, 3D achieved statistically significantly lower bladder and sigmoid dose (p < 0.001) than 2D plans while maintaining adequate target dose. Impact of applicator displacement was found exist with 2 mm inferior and 9°roll shift in 2D ICIr; and with 2 mm inferior and 3°rotation displacement in 3D ICI. 3D ICIr was found to require a more rigid rotational tolerance than for 2D. For VBT, SC and MC VBT was statistically comparable in dosimetry (p > 0.05) for all structures but higher target dose in terms of absolute dose differences. Tolerance of applicator displacement was suggested to set at less than 6 mm inferior movement for SC; and less than 6mm inferiorly and 15°rotationallyfor MC VBT. Conclusions: Modern techniques in both ICIr and VBT brachytherapy were found better in dosimetry than conventional techniques. Conversely, the improvements have shown increase in the impact on applicator shifts and thus tightening of the displacement tolerance especially on rotational aspects was suggested.

The potential effect of hormonal contraception on HIV-1 acquisition and transmission represents an important public health issue. Several observational studies have suggested an association between the use of hormonal contraception, in particular injectable depot medroxyprogesterone acetate (DMPA), and an increased risk of HIV-1 acquisition and transmission. To delineate the biological mechanisms behind this phenomenon, we have recruited healthy, HIV-negative women using either DMPA, NuvaRing, or combined oral contraceptives for a vaginal biopsy and cervicovaginal lavage (CVL). Analysis of the functional capacity of circulating plasmacytoid dendritic cells (pDCs), distribution of vaginal immune cell populations, and systemic and genital levels of immune mediators was performed. The use of DMPA or NuvaRing was associated with reduced capacity of circulating pDCs to produce IFNα and TNFα in response to TLR-9 stimulation ex vivo. This was confirmed in vitro, with the progestin components of DMPA and the NuvaRing, medroxyprogesterone acetate (MPA) and etonogestrel, respectively, inhibiting the activation of T cells and pDCs isolated from healthy, premenopausal women. Systemic levels of IFNα and cervico-vaginal fluid levels of IFNα, CXCL10, MCP-1 and G-CSF were significantly reduced in DMPA users compared to women not using hormonal contraception. Additionally, DMPA use mediates effects on the structural protein expression of the vaginal epithelium. RNA-Seq analysis of gene expression in microdissected vaginal stratified squamous epithelium demonstrated that DMPA use is associated with a suppression of expression of genes critical for epithelial differentiation, formation of desmosomes, and wound healing capacity of the epithelial layer including desmoglein-1, corneodesmosin, keratins, and loricrin. DMPA or NuvaRing use is associated with suppression of human β defensin-2 and -3 production by the vaginal epithelium, as evidenced by the analysis of epithelial gene expression, immunohistochemistry and protein levels in cervico-vaginal fluid. Evidence indicating the immunosuppressive properties of DMPA strongly suggests that DMPA should be discontinued and replaced with other forms of effective and safe contraception. We demonstrate that norethisterone (NET) and levonorgestrel (LNG) did not exhibit detectable immunosuppressive activity on pDCs and T cells in vitro. These progestins should be considered as alternative contraceptives to DMPA; however, more in vivo testing is needed to confirm this data.

▼ Günekoloogiliste pahaloomuliste kasvajate koormus ühiskonnale on suur, sest nad moodustavad 15% kõigist naistel diagnoositavatest pahaloomulistest kasvajatest Euroopas. Töö eesmärk oli saada põhjalik ülevaade günekoloogiliste pahaloomuliste kasvajate pikaajalistest haigestumus-, suremus- ja elulemustrendidest Eestis. Uurimistöö tulemused aitavad kaasa vähiennetuse ja sõeluuringute ning tervishoiukorralduslike muutuste läbiviimisele. Andmed vähijuhtude kohta saadi Eesti Vähiregistrist ja surmajuhtude kohta Surma põhjuste registrist. Emakakeha-, emakakaela-, munasarja- ning häbeme- ja tupevähi trendide analüüsimiseks kasutati erinevaid statistilisi meetodeid. Uuringus leiti emakakeha- ja emakakaelavähi haigestumuse tõus, samal ajal kui munasarjavähi ning häbeme- ja tupevähi haigestumus on olnud stabiilne. Emakakehavähi haigestumuse tõusu saab seostada peamiselt ülekaalulisuse suurenemisega. Emakakaelavähi puhul ei täheldatud ennetustegevuse mõju haigestumusele, mis näitab riikliku sõeluuringu ebaefektiivsust. Kiiresti on vähenenud munasarjavähi suremus ja paranenud elulemus. Samuti on paranenud emakakehavähi elulemus ning see on tõenäoliselt seotud kirurgilise ravi sagenemisega nii vanemas eas kui hilisemate staadiumite puhul. Vähiravi parem kättesaadavus ja kvaliteet, tehnoloogia areng ning tõhusam kaasuvate haiguste käsitlus ja ravi on tõenäoliselt aidanud kaasa munasarja- ja emakakehavähi elulemuse paranemisele. Vanemate naiste osakaal on uuringuperioodil peaaegu kahekordistunud. Nihe varasemas staadiumis diagnoosimise suunas ilmnes vaid emakakehavähi puhul. Et vältida naiste enneaegseid surmasid tulevikus, on vaja parandada emakakaelavähi sõeluuringu kvaliteeti ja tõsta osalemismäära ning saavutada optimaalne hõlmatus HPV vaktsineerimisega. Võitlus rasvumisega tuleb seada prioriteediks. Ühtlasi tuleb rõhutada regulaarsete günekoloogiliste läbivaatuste olulisust, esmajoones eakate naiste seas, et aidata kaasa kasvajate ennetamisele ja varajasele avastamisele.; The burden of gynecological cancers is considerable as they comprise 15% of all cancers among women in Europe. The aim of the study was to obtain a better understanding of the long term epidemiological trends of the incidence, mortality and survival of gynecological cancers in Estonia, to support policy decisions in cancer prevention, screening and cancer care. Data were obtained from the Estonian Cancer Registry and Estonian Causes of Death Registry. Long term incidence, mortality and survival trends of corpus uteri, cervical, ovarian and vulvovaginal cancer were analyzed using a variety of statistical methods. The incidence of corpus uteri and cervical cancer increased over the study period while no changes were observed for ovarian and vulvovaginal cancer. The growing prevalence of obesity is the most likely underlying cause of the corpus uteri incidence trend. No impact of health care related interventions on cervical cancer incidence was detected, showing the lack of effectiveness of nation-wide screening program. ▼ Cystic ovarian disease (COD) is an anovulatory condition in cattle that afflicts between 6-18% of all dairy cows in the US. Ovulation is dependent on the plasminogen activator (PA) family of proteases and protease inhibitor for proteolysis, culminating in follicular rupture. Failure of the follicle to ovulate suggests an aberration in proteolysis in cystic follicles. Polycystic ovarian disease in humans is phenotypically similar to COD is associated with elevated plasma PAI-1. PAI-1 regulates the proteolytic activity responsible for ovulation. Vitamin E supplementation reduces plasma PAI-1 and reduces the incidence of COD. Four experiments were conducted to elucidate the role of the plasminogen activator system in the etiology of COD and the ability of vitamin E supplementation to modulate the PA system in cattle. In experiment one, blood samples were collected from lactating dairy cows upon diagnosis of a follicular cyst at least 3.0 cm in diameter by a licensed veterinarian. Cows were classified based upon their history of follicular cysts. Tissue-type plasminogen activator (tPA) and plasminogen activator inhibitor-1 (PAI-1) concentrations and activities were measured using commercially available ELISA and chromogenic reagents. No differences were detected between cystic and non-cystic dairy cows. In experiment two, six non-lactating beef cows were supplemented with 2750 IU of α-tocopherol every four days for 24 days. Blood samples were collected every two days and assayed for tPA and PAI-1 concentrations and activities. Supplementation with α- tocopherol decreased the ratio of PAI-1 to tPA activities adjusted to the Day 0 measurement (P=0.097). In experiment three, mRNA was isolated from follicular cysts and preovulatory follicles and relative quantitative RT-PCR was performed for tPA, urokinase-type plasminogen activator (uPA), urokinase-type plasminogen activator receptor (uPAR), PAI-1, and β-actin. uPA expression was reduced (P<0.1) and uPAR expression was greater (P<0.1) in follicular cysts compared to preovulatory follicles. Lastly, in experiment four DNA was extracted from whole blood obtained from cystic and normal dairy cows and the promoter region of the PAI-1 gene was sequenced. More Jersey cows with COD possessed a four basepair deletion polymorphism than normal cows (P<0.1) and more Jerseys possessed the deletion polymorphism than Holsteins (P<0.01).
