▼ The Working Model of the Child Interview (WMCI) is a structured interview that elicits rich information about caregivers' internal representations of their children and the quality of the relationship within the parent-child dyad. This instrument is particularly useful in the assessment of foster and adoptive dyads in order to determine goodness of fit and prevent placement disruption by identifying problematic dynamics that threaten the relationship. Because the training and administration costs of the WMCI are immense, it is impractical for use in foster and adoption agencies. This preliminary investigation sought to validate a screening battery of brief, cost-efficient measures (This Is My Baby Interview (TIMB), Parenting Locus of Control-Short Form (PLOCSF), Revised Inventory of Parent Attachment (RIPA), Relationship Questionnaire (RQ), and the Behavior Assessment System for Children, Second Edition, Parent Rating Scales (BASC-2))against the WMCI using 44 caregiver-child dyads. Participants were recruited from public and private foster care and adoption agencies. Child participants ranged from age 2 ½ to 9 years old, and had lived with their caregivers for at least four months. Nonparametric tests (Kruskal-Wallis and chi-square), oneway ANOVA, and post hoc analyses were used to assess the differences in scores between caregiver classifications on the WMCI and fourteen variables measured by the screening interview and questionnaires. The Commitment subscale of the TIMB distinguished Disengaged-Suppressed caregivers from other groups (F=2.96, p<0.5), and the variable of Locus of Control from the PLOCSF was found to distinguish Distorted caregivers from the Balanced-Strained group (KWt=8.22, p<.05). The Internalizing Symptoms Index of the BASC-2 did not emerge as statistically significant but may indicate a trend in that Disengaged-Suppressed caregivers may rate their children as having fewer symptoms of anxiety and depression than caregivers from other WMCI classifications. The WMCI is a rich assessment instrument that may be difficult to break down and capture via screening measures; the results of this study indicated that the TIMB andPLOCSF possibly may be useful in capturing some of the information elicited by the WMCI with highest risk caregiver classifications (Disengaged-Suppressed and Distorted). Future investigation of caregiver contributions to the dyad's functioning is warranted with different screening measures, and should be conducted with a larger sample size for maximal results. Familjehemsplaceringar är en viktig insats i den sociala barnavården. Efterfrågan på familjehem är stor medan svårigheterna att rekrytera nya familjehem ökar. Synen på familjehemsvården har förändrats från att ha varit ett permanent ersättningshem till att bli ett kompletterande hem för barnet. Detta innebär att det placerade barnet skall uppmuntras till att ha kontakt och umgänge med de biologiska föräldrarna och målet är att barnet skall flytta hem. Detta innebär att fosterföräldrarna skall samarbeta med barnets biologiska föräldrar. Syftet med denna studie är att förstå hur fosterföräldrarna upplevde sin roll, och hur de samarbetade med de biologiska föräldrarna och vilken roll socialtjänsten hade i detta samarbete. För att få svar på detta så gjorde vi kvalitativa intervjuer med åtta familjehem. Som teoretisk utgångspunkt har vi valt Goffmans (2006) dramaturgiska perspektiv som belyser samspelet mellan olika aktörer. Fosterföräldrarna upplevde att de hade höga krav på sig själva, socialtjänsten och allmänheten. Resultatet visade att respondenterna hade olika erfarenheter, både positiva och negativa, av samarbetet med de biologiska föräldrarna och socialtjänsten. Fosterföräldrarna påtalade att det många gånger var problematiskt att samarbeta med de biologiska föräldrarna och att socialtjänsten hade stor betydelse för att samarbetet skulle bli bra. Fosterfamiljerna lyfte fram att de upplevde det svårt att få en kontinuerlig kontakt med sina socialsekreterare.

Family home placement plays a contributing roll in social childcare. The demand is great for family homes and the need in finding new homes is ever increasing. The way in which family homecare is looked upon has changed and it is no longer regarded as a permanent replacement home but instead as a complementary home for the child. This means that the child who is placed in a family home must be encouraged to have contact and spend time with the biological parents in the aim that the child will return home. Therefore cooperation is necessary between foster parents and the child's biological parents. The aim of this study is to gain a better understanding of how the foster parents see their roll and how they interact with the biological parents, as well as the cooperation they receive from social services. In order to answer these questions we have implemented qualitative interviews with eight family homes. We chose to use Goffman's dramaturgical perspective as our theoretical basis in order to highlight the interplay between the different actors. Foster parents felt that they had high expectations of themselves, of the social workers and of the general public. The results of this study show that the respondents had different experiences, both positive and negative in regard to cooperation between the biological parents and the social services. Self-help groups in foster-parenting are based on collaboration and sharing of common experience. Group members help each other in solving not only child related but personal problems as well; they can attain help from the group leaders, and learn from each other. The research revealed that family- fostered children tend to be more independent, they are able to form certain skills. However, some difficulties during period of adaptation remain, including barely manageable aggression, differing life experiences that shape changes in foster families, and different paths of socialization. Social workers and foster parents understand self-help groups as a possibility to solve similar problems, helping oneself and each other. Social workers believe that self-help groups can offer significant improvement in relationships, problem solving, and exchange of good practices. Foster parents believe that in self-help groups they can enhance their competencies, which in turn can improve their relationships with a child, help understanding his/her needs, and enrich their knowledge, helping feel a part of the team in a foster- parenting situation.

Syftet med denna studie var att undersöka upplevelsen av stödet som ges till familjehem via en konsulentstödd verksamhet. Frågorna för studien var hur familjehemsföräldrarna och personalen upplever stödet. Tre familjehemskonsulenter och två familjehemsföräldrar intervjuades. Med en kvalitativ metod och en fenomenologisk inriktning kunde upplevelsen av konsulentstödet studeras. För att få en förståelse för intervjumaterialet användes två olika analys perspektiv i form av systemteorin och utvecklingsekologin. Resultatet visade i likhet med tidigare forskning att viktiga delar i stödet var tillgänglighet, information, relationer, utbildning, handledning och träffar med andra familjehem. Resultatet visade att båda familjehemsföräldrarna i studien var nöjda med konsulentstödet överlag. I studien framkom dock att verksamheten inte kunde erbjuda nya familjehem utbildning innan en första placering görs vilket familjehemsföräldrarna framhävde som väsentligt. Verksamheten hade även sämre möjligheter att erbjuda en bra kontinuerlig utbildning i form av föreläsningar. Studiens resultat tillsammans med tidigare forskning visade även att upplevelsen av ett bristande nätverk finns hos familjehemsföräldrar.

The purpose of this study was to investigate the experience of the support given to the family home through a consultant supported activities. The questions for the study was how the foster parents and staff feel the support. Three family advisers and two foster parents were interviewed. With a qualitative method and a phenomenological focus could be the experience of consultant support are studied. To get an understanding of the interviews, two different analytical perspectives in the form of systems theory and the development of ecology. The results showed, as in previous research that the key elements in supporting the accessibility, information, relationships, training, tutorial and meet with other foster homes. The results showed that both the foster parents in the study were satisfied with consultant support overall. The study showed, however, that the business could offer new foster training before an initial investment made which foster parents mattered significantly. Operations were also less able to offer a good continuous education in the form of lectures. The study's results together with previous research also showed that the perception of a lack of available network of foster parents.

▼ The objective of this study is to determine how former foster parents perceive the fostering experience to have affected their own children. The literature deals primarily with foster parents, foster children and fostering in general. The natural children of foster parents are generally forgotten. This is an exploratory, descriptive and retrospective study. Interviews were conducted based on a semistructured questionnaire. The results were tabulated and analyzed on the basis of the structured questions. A content analysis was completed on the unstructured responses. The thirty-five subjects (19 families) were drawn from a population of 2,063 foster homes in Los Angeles County who relinquished their license during the years 1979-1981 and who met the limited criteria of this study. The focus of the study was limited to family systems, interfamilial relationships and crisis and loss theories. The questionnaire was divided into three sections: Individual and family characteristics; Foster family interaction and Crisis and Loss questions, with two questions at the end for the foster parents' subjective areas of concern. It is clear from the study that these foster families operated as a system. The decisions regarding fostering were bilateral or multilateral. The children they cared for were largely difficult to handle, emotionally disturbed or delinquent. It is apparent that the families had integrity and equilibrium already established within the family in order to cope with these children coming and going. The foster parents generally viewed the experience of fostering as a positive growth experience for their children. Their children learned compassion, acceptance, and closeness. The natural children of foster parents are an integral part of the foster family and need to be included from the beginning of the fostering experience to the end. Todėl dienos centrams šiandien daugiau skiriamas socialinių įgūdžių lavintojo, socialinės paramos bei užimtumo organizatoriaus vaidmuo. Trūksta informacijos, kaip vaikų dienos centras išnaudojamas kaip resursas galintis padėti tenkinti vaikų, lankančių dienos centrus, tėvų interesus. Tyrimo objektas – tėvų įtraukimo į socialines paslaugas dienos centre patirtis. Tyrimo tikslas - atskleisti tėvų įtraukimo į socialines paslaugas dienos centre patirtį. Uždaviniai: 1. Atskleisti kaip dienos centrą lankančių vaikų tėvai suvokia įtraukimą į paslaugų sistemą; 2. Atskleisti kaip dienos centrą lankančių vaikų tėvai suvokia socialinio darbo paslaugų pobūdį; 3. Child day centers in Lithuania have been existed only for 20 years, their legislative base is narrow and imperfect and does not provide a precise definition of child day centers activity. Therefore today these centers play a role of developer of social skills and organizer of social assisstance and employment. There is a lack of information on how child day centers are exploited as a resource which can help meet interests of childs attending day centers and their parents. The object of the research – the experience of the involvement of parents into the social services provided by the day center. The goal of the research – to disclose the experience of the involvement of parents into the social services provided by the day center. Tasks: 1. To disclose how partents of children attending day center undestand an involvement into the service system; 2. To disclose how partents of children attending day center undestand the nature of social work services; 3. To describe the guidelines of development of social services from the customers perspective; the research included five mothers whose children attend child day center. ▼ Background: The Centers for Disease Control indicated that in 2009, 90 million prescriptions were written for antibiotics in the United States, with half of those being "unnecessary or inappropriate". The highest rate of antibiotic use was evident in children younger than 15 years old. Physician’s perception of parents’ expectation to receive antibiotic prescription for their children is a significant predictor of overprescribing antibiotics. Objective: The objective was to manipulate two factors (parents’ ‘perceived benefits of using antibiotics’ and their ‘perceived barriers to visit doctors without any expectation of antibiotic prescription’) and then evaluate whether their level of expectation would change after the manipulation. Methods: A prospective experimental study was conducted using a structured data collection instrument. The purpose of the experiment was to manipulate two variables, perceived barriers and perceived benefits using four scenarios and keep other factors constant. Each subject viewed four situations and expectation associated with each situation was evaluated. Subjects who had at least one child (age ≤ 5 years) during the study and who could speak, read and write English were selected for the study. Data were collected at shopping malls and parks in Houston, TX. Descriptive analyses and repeated measures mixed method covariance adjusted analyses were performed using SAS® 9.3. The a-priori significance level was set as 0.05 for all tests conducted. Results: A total of 300 complete surveys were considered for analyses. The mean age for the sample was 30.36 (± 7.04) years; females represented 55.7% of the sample. The mean general expectation score (before reading any scenario) to receive antibiotic prescription for children was 53.6 (± 25.7). The repeated measure mixed methods analyses indicated that there was 12 point reduction (p < 0.0001) in expectation score after removing perceived barriers from the situational scenarios. Almost 16 point decrease (p < 0.0001) in expectation score was observed after removing perceived benefits from the scenario. There was 18 point decrease (p < 0.0001) in expectation score after removing perceived barriers and perceived benefits from the situational scenario. The study result also indicated that general expectation toward an antibiotic prescription, training in the healthcare field and parents’ preference for communication had significant effect on parents’ expectation. Conclusions: Perceived barriers, perceived benefits alone and in combination have effect on parents’ expectation to receive antibiotic prescription for children. ▼ The purpose of this qualitative study was to determine the factors which helped these foster care alumni to persevere and to succeed. The intent was to provide a framework for equipping youth in foster care more effectively by building on their strengths and the resources available in foster care. The study’s method incorporated the interviewing of ten ethnically diverse individuals who had experienced the foster care system. Based on their insights and the themes which emerged, the WARRIORS Model was created. This acronym represents the key themes derived from the interviews: Wounded, Advocacy, Reality of Belonging, Resources, Inspired to Succeed, Obtain Life Skills, Resilience and Spirituality. A Wounded past was the reality of every participant yet many found purpose and meaning in their Advocacy roles. The Reality of Belonging was vital in each of their lives in addition to accessing Resources for post-high school educational and housing options, both implicating the significance of foster parent involvement. All were Inspired to Succeed both personally and educationally and were receptive to Obtain the Life Skills necessary for their future independence and success. They demonstrated such hopeful countenance and profound Resilience, in addition to accessing and benefitting from their chosen avenues of Spirituality. Finally, while navigating through both the negative and positive aspects of their lives, these youth have revealed the strength of the human spirit which has given them every right to be identified as WARRIORS. The WARRIORS Model can be utilized by clinicians and professionals involved in the systems addressing the needs of youth in foster care and training foster parents. Greater opportunities are needed for youth in foster care to advocate for themselves and others with the hope of gaining a sense of worth as their opinions are valued by the adults claiming to represent them. Finally, further research is needed concerning the spiritual development of youth in foster care and ways in which we can assist in this promising aspect of cultivating resilience as evidenced in this study and other established research. ▼ Objectives: This study describes dental utilization, expenditures and use of dental services among children in Washington State foster care. Methods: Washington State enrollment and claims data were used to identify children ages 0 months through 17 years who were enrolled in the foster care program for 11 months or more during 2008 (N=10,589). The overall dental utilization rate and for groups of services (diagnostic, preventive, routine restorative, complex/surgical, orthodontic) were calculated as the proportion of children who had any dental claim or for that service grouping. Individual total expenditures and by service grouping were calculated among children with at least one claim. Using Patrick et al.'s model of oral health disparities as a framework, measures were analyzed under three domains: individual (sex, race/ethnicity, age group, disability status, child history of substance abuse, child history of behavior problems); interpersonal (placement type); and community (placement urbanicity). Bivariate analysis was used to assess differences in utilization or expenditures across each measure. Results: Among the study population, 44% of children had at least one dental claim. By service grouping, the proportion of children with at least one claim was: diagnostic 41%; preventive 41%; routine restorative 12%; complex/surgical 14%; and orthodontic <1%. Utilization differed significantly by all measures except sex and a child's history of substance abuse. The mean individual dental expenditure was $287 (2008 dollars) and differed significantly by age group, race/ethnicity and behavior problems. Conclusions: The majority of children in this study did not receive an EPSDT dental service, to which they were entitled. Children in this study were more likely to receive a complex or surgical than a routine restorative service, and less than 1% of children had an orthodontic claim suggesting restorative and orthodontic services are underutilized, and delivery of care may be episodic. Children's individual measures were directly associated with their receipt of care, and several suggest a child's connectedness to the health system influences their access to dental services. Overall, these findings strongly suggest that unmet dental need exists among children in foster care, and that disparities in receipt of dental services exist among subpopulations of children in foster care. ▼ There have been various changes to the manner in which early intervention services for children with disabilities have been provided in recent decades. One of the most significant paradigm shifts that has accoured pertains to a change in the level of family involvement in early intervention, so that families are now required to be equal partners with professionals in the service provision process. It is now policy in Victoria that early intervention services follow a family-centred model of practice. Services adopting this model aim to empower parents, so that they may have impact on their lives, and the lives of their family members, both during and beyond the years of direct service participation. Much of what is known about empowerment to date is based on theory, author opinion, and research that is largely survey-based. There has been little interview-based research, particulary involving parents of children with disabilities, as well as little Australian research conducted regarding empowerment. To the researcher's knowledge, there has been no interview-based research that specifically asked parents of children with disabilities about their perspectives on empowerment and disempowerment. Parents of children with disabilities are not invited to contribute their opinions in services and research. Empowerment is an individual concept and this research provided parents with an opportunity to express their views on this topic. Parent's perspectives on empowerment are vital for service providers who aim to follow the intervention model required by policy. This research, which was guided by the principles of ecological theory and critical theory, involved to individual semi-structured interviews with 37 Victorian families of children with disabilities. Twenty-one of these families had children currently participating in early intervention services, and 16 families had children of mid-primary school age, who had previously participated in early intervention experiences; the factors that they believe influence empowerment and disempowerment; and helpful and unhelpful experiences with early intervention staff and other people in their lives. Data were analysed primarily inductively, in the context of grounded theory. Responses from the two groups of parents were then compared, as were different emergent themes according to helpfulness and empowerment. The nature of enduring empowerment, one of the key objectives of early childhood intervention, was also considered. From the analysis of data, several themes emerged as influential in the empowerment process for both groups of parents including: information, education and knowledge; meeting and talking with other families of children with disabilities; decision-making and choice; having confidence; participation, involvement and input; meeting or addressing families' practical needs; and having a child with a disability. This dissertation investigates the ability of two service treatment systems—one focusing on a lifestyles and the other on a life-chances perspective—have in addressing barriers to self-sufficiency faced by homeless clients enrolled in the Newark (New Jersey) Department of Health and Human Services (NDHHS) Substance Abuse and Mental Health Services Administration (SAMHSA) homeless program. The study examines how program clients fare in regards to overcoming mental health and substance abuse (MH/SA) disorders, obtaining housing and employment, and halting criminal activity after receiving program treatment services. This research provides an overview of the prominent challenges faced by homeless individuals, and adds to the growing body of knowledge on effective service interventions to assist homeless individuals achieve independent living. This investigation was accomplished through a quantitative and qualitative analysis. The quantitative analysis examined data from the Government Performance Results Act (GPRA) on 181 clients enrolled in the NDHHS SAMHSA program between 2007 and 2011. The qualitative analysis included a direct observation of program activities and structured staff and client interviews. The results from this study found a comparative benefit of the lifestyles service interventions over the life-chances service interventions in addressing clients’ outcomes related to: 1. Anxiety disorders – reducing symptoms by more than 11 days per month 2. Housing – causing a 6 times higher odds of housing for clients receiving brief treatment service intervention. 3. Employment – for each increase in lifestyles services received, clients had a 2.362 times higher odds of being employed compared to being unemployed and not looking for work. Clients receiving the lifestyles intensive outpatient service had a 53.324 times higher odds of being employed versus to being unemployed and not looking for work. 4. Illegal drug use – decreasing use by 41 percent as services got more intense. 5. Criminal activity – decreasing activity by 47 percent as intensity of services increased. The qualitative results from this study reveal that the NDHHS SAMHSA program staff viewed clients’ inability to manage existing psychiatric and addictive disorders as the primary cause of homelessness. Accordingly, the staff regarded the treatment of MH/SA disorders as essential to helping clients achieve independent living.

▼ Background: The Department of Health and Human Services estimates that by 2020 the number of direct support professionals (DSPs) needed to meet the care of the projected increase of 1.4 million individuals with intellectual and developmental disabilities (ID/DD) will rise sharply to approximately 1.2 million full-time employees. A long acknowledged challenge of the ID/DD field is how to hire, retain, and train DSPs to carry out the person-centered plans (PCPs) in a way that improves clients’ quality of life and supports increases in their levels of independence. Literature has shown the importance of DSP practice behaviors as they relate to client outcomes, and the skills, knowledge, and values that are necessary to carry out person-centered plans. To date, there have been few studies focused exclusively on how DSPs understand, interpret, and put into daily practice these same skills, knowledge, and values. This study examines the ways in which individuals who are employed as DSPs in congregate or group home settings for individuals with ID/DD perceive and approach practice decisions within a framework of person-centered planning. Theory: The theoretical framework developed in this study combined a critical realist ontology with recognition theory, interbehavioral psychology, and street-level bureaucracy to envision the multiple generative mechanisms that may influence a DSP’s daily decision making. These daily decisions, and the attitudes, opinions, and viewpoints that reinforce them, require a fairly sophisticated understanding of the types of practice behaviors that encompass choice and autonomy. This study used the framework above in conjunction with the existing literature to explore the habitual practice behaviors of DSPs as viewed from a critical realist perspective to determine what types of “real level” variables (i.e., levels of recognition, paternalism, self-referential decision-making, agency policy and resources influence, and group home culture) are expressed by DSPs as significant “actual level” or activating factors in their day-to-day use of discretion at the “empirical level” of service provision. Methods: Primary data were collected in a mixed method fashion from in-person interviews and the application of Q methodology to elicit consensus viewpoints from 30 DSPs working in two ID/DD support organizations in New Jersey. This was accomplished through the development of 48 statements (i.e., Fisherian concourse) which represented the six areas introduced above which participants sorted according to their own subjective view on what areas were most influential to their daily practice. All participant sorts were analyzed in PQmethod, using the sequential application of three sets of statistical procedures: 1) correlation, 2) factor analysis and 3) the computation of factor scores. The study aimed to explore and describe the experiences, perceptions and needs of unmarried adolescent fathers and to assess the attitudes and attentiveness of selected social workers and organisations that provide services to adolescent parents. In view of the position in which teenage fathers find themselves and with regard to stereotyping and the disregard for their role as fathers, no concerted attempt is made by organisations to provide services to them. They are marginalized and despite changes in legislation and a Bill of Human Rights their needs are not attended to. A mixed research methodology was employed in conducting this exploratorydescriptive study. The study was advertised in the waiting rooms of relevant organisations, in consultation with pregnancy help centres, social work agencies and youth services. Those who responded were interviewed according to inclusion criteria for the study and those who did not qualify were referred to appropriate services. Confidentiality was assured and consent to participate in the study was discussed. A non-random sample of 32 participants was identified, ranging from 13 to 19 years were located of who 15 agreed to participate. Further attrition resulted in the sample being reduced to 12. Interview schedules were used to facilitate the discussion and to ensure that all participants were asked the same questions. The second part of the study explored the attitude and attentiveness of services to adolescent parents. All organisations providing such services were invited and seven eventually agreed to participate. Of these, four indicated that they were participating in their personal capacity. Interviews were guided by an interview schedule which aimed to explored attitude, attentiveness and capacity to provide services to unmarried teenage fathers. The findings of the study revealed that teenage fathers want to be involved with their children, but social stereotyping, unrealistic expectations of partners and family, and social discrimination militate against them. However, the support of family and the natal partner and her family, are factors that determine continued involvement. Services were perceived and experienced as negative and dismissive of their role as fathers.
