▼ Tendons are frequently used for reconstructive surgery. This includes palmaris longus, plantaris, the long extensors of the toes and fingers as well as the flexors of the fingers. The surgeon must base his or her selection of the donor tendon for grafting on what is needed, for example tendon size, length, and width. The most desirable tendons in reconstructive surgery are the palmaris longus and plantaris tendons. These two muscles are also considered to be the easiest tendons to harvest, and therefore they remain the ideal choices for plantaris,or tendon grafts. Apart from what is mentioned in the literature, questions remain such as: which characteristics do the palmaris longus and plantaris muscles possess that make them suitable for use as flaps or grafts in reconstructive surgery and; how can knowledge of the characteristics of these muscles improve reconstructive surgery in South Africa? The palmaris longus and plantaris muscles are indeed subject to variation, whether in the general anatomy, form, attachment, actions and/or prevalence. A statistical significant difference was found between the male and female sample when considering the length of the palmaris longus muscle. When comparing the palmaris longus muscle to the plantaris muscles, it was found that there is a statistical significant difference between them as well. Therefore, although these muscles may look alike, when it comes to the surgical aspect it is suggested that the palmaris longus is used when a wider tendon is preferred and the plantaris muscle when a longer tendon is needed. The prevalence of the palmaris longus and plantaris muscles compared well with studies done on other population groups. No correlation was found between the sex of the cadaver and the prevalence of the palmaris longus and plantaris muscles. In addition it was established that there is no relationship between the prevalence of the palmaris longus and plantaris muscles in the same individual. A possible phylogenetic degenerative trend for the palmaris longus muscle was also examined. After studying various age groups, it was determined that such a trend could not be established for this sample. In conclusion, based on the morphology and prevalence of the palmaris longus and plantaris muscles in a South African population, they are ideal for the use of flaps and/or tendon graft in reconstructive surgery. But it is of utmost importance that the reconstructive surgeon, working on South African patients, remember that both these muscles are subject to variation and not only will it be beneficial to employ proper detection methods to determine the viability of either muscle before considering its suitability in reconstructive surgery but also a sound knowledge of the anatomy of these muscles must be known. ▼ Two separate studies were conducted over a span of two years, combining the two-step weaning method with a short-term creep feed period. In a 2 x 2 factorial, 203 (year 1, n = 103; year 2, n = 100) purebred angus calves were ranked by gender, body weight (BW), age, dam parity, and sire group and then randomly assigned to one of four groups: 1) two-step weaned by inserting a nose flap 1 wk prior to weaning (NF), 2) traditionally weaned, separated from their dam on d 0 (TRAD), 3) traditionally weaned and creep fed, separated from their dam on d 0 and offered free choice creep feed 3 wk prior to weaning (TRADC), and 4) two-step weaned by inserting a nose flap 1 wk prior to weaning, offered free choice creep feed for 3 wk prior to weaning (NFC). The objective of this study was to analyze the effects of the two-step weaning method coupled with a short-term creep feed period on weaning stress and post-weaning calf performance, feed intake, and antibody titer response, as well as steer carcass quality and heifer fertility. The preweaning period (d -28 to 0) was the same over both years. All pairs were brought in on d -28 and weekly BW were measured for both calves and cows. Starting on d -21 creep feeders were placed in TRADC and NFC pens, all calves received vaccinations against respiratory pathogens, and all calves were bled on a weekly basis (d -21, -14, -7, 0, 7, 14, 21, and 28) for the analysis of serum antibody titers. On d -7 nose flaps were inserted into NFC and NF calves. On d 0 all calves were administered a booster vaccine, nose flaps were removed, and all calves were weaned. The post-weaning period for the two years differed. For year 1, on d 0 all calves were moved to new housing at the same location after separation from their dams. Also, on d 0 and 1 circulating cortisol concentrations were analyzed. At this point all steer calves were transported to a different farm to be fed out while heifer calves remained at the same farm and were housed with other heifers the farm was keeping in their replacement program. Steers were scanned for carcass characteristics on d 28 and were harvested on d 263. Heifers were developed and were given reproductive tract scores before they were deemed fit to be artificially inseminated (AI) at approximately 14 months of age. For year 2, on d 0, all bull calves were transported for approximately 2.5 hours to a feedlot where they would remain until the conclusion of the trial on d 28 while heifers remained on farm and were managed with the other heifers being kept in the farm's replacement program. Bulls had blood collected on d 0, 1, 2, and 3 for analysis of circulating cortisol concentrations. They were also monitored for disease and treated if signs of sickness were detected. During the preweaning period, there were no statistical differences for ADG between calves that were two-step weaned compared to those that were not for both year 1 and 2 (P = 0.75, 0.63, respectively). ▼ Background Maxillary defects subsequent to hemimaxillectomy have long been restored with a pedicled temporalis flap. Recently,the trend towards maxillary reconstruction using vascularized bone free flaps and soft tissue free flaps has been gaining popularity. However, the value in terms of functional rehabilitation of these surgical modalities remains unconclusive. Objective To ascertain and compare masticatory performance and quality-of-life in patients with surgically reconstructed maxillectomy Class 2 (subtype A) defects by vascularized bone free flaps, vascularized soft tissue free flaps and pedicled soft tissue flaps. Methods Eighteen patients divided into 3 groups (4 vascularized bone flap, 5 vascularized soft tissue flap, 9 pedicled soft tissue flaps) were evaluated for functional outcome and qualityof-life (QoL). All patients were objectively assessed using masticatory comminution test. Subjective evaluation was conducted using functional outcomes questionnaire and patient reported speech perception. Self image and body perception were assessed using Body Esteem Scale. Overall quality-of-life was assessed using University of Washington QoL questionnaires. Results Patients reconstructed with vascularized bone flaps and vascularized soft tissue flaps showed superior masticatory performance compared to patients reconstructed with pedicled soft tissue flaps. Speech, facial attractiveness and overall QoL were similar for all three groups. Conclusions Maxillectomy class 2 (subtype A) patients show comparable speech, facial attractiveness and quality-of-life levels after reconstruction with vascularized bone flaps, vascularized soft tissue flaps and pedicled soft tissue flaps. However, those reconstructed with pedicled soft tissue flaps achieved lowest masticatory performances amongst the three surgical reconstruction modalities. Thus, the choice of recontruction for maxillectomy class 2 (subtype A) defects should be guided by minimizing surgical time and long term morbidity given the apparent similarity in functional outcomes.

INTRODUÇÃO: A propriedade de induzir angiogênese torna o omento um promissor pedículo para pré-fabricar retalhos. OBJETIVO: Estabelecer a área abdominal a ser pré-fabricada por um pedículo omental e analisar o potencial de pré-fabricação (PPF) conforme o tempo de espera entre a introdução do pedículo e a elevação do retalho. MÉTODOS: Foram utilizados 44 coelhos divididos em quatro grupos (A, B, C e D). No grupo A, um fragmento de pele, tecido subcutâneo e músculo cutâneo abdominal foi totalmente separado e ressuturado. Nos demais grupos, um pedículo omental, contendo os vasos gastromentais ligados distalmente, com área equivalente a 9 cm² foi transposto e suturado ao músculo cutâneo abdominal. Um segundo procedimento de incisão e elevação de um retalho contendo pele, subcutâneo e músculo cutâneo abdominal pediculado apenas pelo omento transposto, foi realizado, variando apenas o período de espera entre os dois procedimentos de 7, 21 e 56 dias para os grupos B, C e D respectivamente. Após 15 dias do último procedimento, os retalhos foram visualizados e as áreas viáveis foram calculadas através do programa de computador Image Tool®. Cortes de áreas viáveis foram imunocoradas pelo anti-CD31 para cálculo da densidade microvascular (DMV). RESULTADOS: Os valores médio e máximo das áreas viáveis no grupo D foram respectivamente 45,29 cm² e 99,37 cm² (PPF mediano = 5,03 e PPF máximo = 11,04). Não houve diferença significativa entre as áreas viáveis do grupo D e C. As médias da DMV dos grupos B, C e D foram respectivamente 24,54 vasos/mm², 33,20 vasos/mm² e 27,03 vasos/mm² e maiores do que as médias da DMV das áreas controles de 14,63 vasos/mm², 17,33 vasos/mm² e 18,12 vasos/mm². No grupo A, houve necrose total em todos os retalhos. CONCLUSÃO: O PPF mediano do pedículo omental foi de 5,03 vezes sua área e o tempo de espera para o segundo procedimento foi de, no mínimo, 21dias

INTRODUCTION: The angiogenic induction property of the omentum makes it a promising pedicle to prefabricate flaps. OBJECTIVE: To establish the abdominal area to be prefabricated by the omental pedicle and to analyze the prefabricate potential according to the time delay between the pedicle introduction and the flap release. METHODS: 44 rabbits were divided into four groups (A, B, C and D). In group A, a piece of skin, subcutaneous tissue and abdominal cutaneous muscle has been fully released and sutured again in its place. In other groups, a 9 cm2 omental pedicle containing the gastromental vessels distally tied has been transposed and sutured to abdominal cutaneous muscle. A second procedure, an incision and release of the flap that contained skin, subcutaneous and cutaneous abdominal muscle pediculated only by the omentum, has been carried out. The only variation was the time delay between the two procedures: 7, 21and 56 days for groups B, C and D, respectively. ▼ Operating a helicopter in a turbulent wind environment is a difficult challenge for a pilot. Tasks that would be simple in a calm environment become a unique challenge when wind gusts begin changing the behavior of the rotorcraft. The requirements of constantly correcting the attitude of the aircraft can increase the pilot workload to unacceptable levels. Recently research has been focused on using automated control designs to compensate for wind disturbances, relieving some of the workload from the pilot. Control designs using swashplate actuation have run into trade-off issues between stability margin constraints and disturbance rejection considerations. The topic of this thesis is the modeling and design of an alternative control mechanism, on-blade control devices called trailing edge flaps, to improve handling qualities and disturbance rejection capabilities over those of the swashplate. A primary objective of this thesis is to establish an accurate model of the aerodynamic effects and dynamics of trailing edge flaps. A study of the dynamics of the rotor shows that a flap on the UH-60 would behave as a moment flap, working to twist the blade to change the distributed lift as opposed to creating only a local lift increment. The Genhel-PSU model is modified with a torsion degree of freedom to accurately model the effects of a flap on a UH-60 helicopter. To verify the accuracy of this implementation, the Genhel-PSU model is configured for the SH-2G Helicopter, since significant flight data and identified model is available for this aircraft. The identified model from previous research is compared with a linear model produced by the converted Genhel-PSU model. Significant differences are noticed in roll-flap and pitch-flap modes causing changes in the frequency responses. Model structure analysis reveals that the rotor time constant is significantly different from the value in the identified model. The identified value was substituted into the linear model, which corrected the discrepancies. Flap damping was added to the non-linear model, and the resulting linear model had a much closer frequency response match, as well the rotor time constant value matched the identified model much more closely. The rotor Lock number was varied in an attempt increase the predicted flap damping. A significant decrease in flapping inertia (~40%) created a model with a rotor time constant that matched the identified model, however the response for the modified model did not match the identified model. The cause of this discrepancy could not be identified. Actuation methods are an important consideration when using on-blade trailing edge flaps. Piezo-electric tube actuators are experimental designs that are both extremely fast, compact, and can be made to achieve the desired deflection. These actuators are still experimental and are just being used as a theoretical design. A model of a Piezo-electric actuator implementation for trailing edge flaps is developed. ▼ Cruise flaps are trailing-edge flaps which minimize the profile drag of a wing by moving the low-drag-region (or bucket) of a drag polar such that it spans the current coefficient of lift. Previous research has explored the use of a pressure-based technique for automating cruise flaps. Data obtained using this technique can be presented in a number of different formats, and different presentations of the same data tend to lead to the development of different types of automating controllers. The presentation used by previous researchers led to the development of a drag-minimizing controller that required a low-pass filter to prevent instability. This prevented the controller from being used for purposes which required a fast-acting flap. The presentation of pressure data used in this research led to the development of a multi-function controller that includes both slow-acting functionality (drag reduction) and fast-acting functionality (gust alleviation). The pressure-based technique developed by previous researchers using natural-laminar-flow (NLF) airfoils must be modified somewhat for the low Reynolds number SD7037 airfoil used in this research. Drag polars for low Reynolds number airfoils do not behave as predictably as those for NLF airfoils at much higher Reynolds numbers. A series of rigid-aircraft simulations were conducted to show the effectiveness of the multi-function controller, which was able to simultaneously reduce drag and alleviate the effects of vertical gusts. A flight controller was developed using low-cost microcontrollers and pressure transducers. The controller implemented a slow-acting drag-reduction function and a fast-acting function to handle pilot-commanded elevator inputs. Gust alleviation was not implemented due to limitations of the available hardware and flight test conditions. The controller was tested on an radio-controlled sailplane with a wingspan of 100 inches. Although direct measurement of the drag of an aircraft this size is difficult and was not attempted, analysis of flight data indicated that the controller was successful in dynamically deflecting the flap to desired angles without adversely affecting flight stability and controllability. Infectious complication remains a major cause of flap failure. Serum complement reactive protein (CRP) level is a customary measure for early recognition of infections. The aim of this study was to describe the time course of CRP in patients with free flap for trauma and chronic wound problems of the lower extremity. We further evaluated the effect of patient comorbidities influencing serum CRP level. We retrospectively analyzed the data of all 21 patients who received free flap from June 2009 to March 2012 at the Department of Plastic and Reconstructive Surgery, Ajou University School of Medicine. CRP levels were measured routinely until postoperative 2 weeks. Patient CRP values were expressed as mg/dl and were averaged per day in each group. Fourteen (66.7%) of the patients reached peak CRP within 3 days. Of the seven patients peaking after day 3, two patients (28.57%) had infective complications(postoperative wound infection and remained wound abscess) and showed a high level and a second peak of CRP about 2 days before symptoms were evident. The other two patients (28.57%) had side effects of antibiotics (drug fever and pseudomembranous colitis) and also displayed a high level of CRP, which subsequently lowered slowly. In these four complication cases, a change in antibiotics guided by a consultant microbiologist was necessary. In all disease-free patients, mean CRP rose from postoperative day 1, peaked at day 2, and decreased significantly from day 4. On the contrary, in the diabetes and hypertension group, the mean CRP rose from postoperative day 2, peaked at day 3, and decreased significantly from day 5. Also, an unexpected and irregular pattern was shown in the diabetes and hypertension group. There was statistical relevance of diabetes and hypertension on CRP trends. However, the non-osteomyelitis group showed more irregular and higher CRP trends than did the osteomyelitis group. There was no statistical significance, perhaps because patients who had complications were all included in the non-osteomyelitis group. Also, all patients of osteomyelitis were early phase and were well-controlled by antibiotics. Our results confirm that CRP is a very efficient parameter for diagnosis and prognosis of infectious complications after free flap. We also noted the positive correlation between the diabetes, hypertension, and the CRP level in free flap. However, study limitations include the number of patients who met the inclusion criteria. ▼ This research focuses on the development of an active rotor system capable of primary control and vibration reduction for rotorcraft. The objective is to investigate the feasibility of a novel Trailing Edge Flap (TEF) actuation system driven by Pneumatic Artificial Muscles (PAMs). A significant design effort led to a series of experimental apparatuses which tested various aspects of the performance of the actuators themselves and of TEF systems driven by them. Analytical models were developed in parallel to predict the quasistatic and dynamic behavior of these systems. Initial testing of a prototype blade section with an integrated PAM driven TEF proved the viability of the concept through successful benchtop testing under simulated M = 0.3 loading and open jet wind tunnel tests under airspeeds up to M = 0.13. This prototype showed the ability of PAM actuators to generate significant flap deflections over the bandwidth of interest for primary control and vibration reduction on a rotorcraft. It also identified the importance of high pneumatic system mass flow rate for maintaining performance at higher operating frequencies. Research into the development and improvement of PAM actuators centered around a new manufacturing technique which was invented to directly address the weaknesses of previous designs. Detailed finite element model (FEM) analysis of the design allowed for the mitigation of stress concentrations, leading to increased strength. Tensile testing of the swaged actuators showed a factor of safety over 5, and burst pressure testing showed a factor of safety of 3. Over 120,000,000 load cycles were applied to the actuators without failure. Characterization testing before, during, and after the fatigue tests showed no reduction in PAM performance. Wind tunnel testing of a full scale Bell 407 blade retrofitted with a PAM TEF system showed excellent control authority. At the maximum wind tunnel test speed of M = 0.3 and a derated PAM operating pressure of 28 psi, 18.8° half-peak-to-peak flap deflections were achieved at 1/rev (7 Hz), and 17.1° of half-peak-to-peak flap deflection was still available at 5/rev (35 Hz). A quasistatic system model was developed which combined PAM forces, kinematics and flap aerodynamics to predict flap deflection amplitudes. This model agreed well with experimental data. Whirl testing of a sub-span whirl rig under full scale loading conditions showed the ability of PAM TEF systems to operate under full scale levels of centrifugal (CF), aerodynamic, and inertia loading. A commercial pneumatic rotary union was used to provide air in the rotating frame. Extrapolation of the results to 100% of CF acceleration predicts 15.4° of half-peak-to-peak flap deflection at 1/rev (7 Hz), and 7.7° of half-peak-to-peak flap deflection at 5/rev (35 Hz). A dynamic model was developed which successfully predicted the time domain behavior of the PAM actuators and PAM TEF system. Os retalhos cutâneos são amplamente utilizados na cirurgia plástica, principalmente na reparadora. Após o procedimento operatório, uma das principais complicações é a isquemia, podendo ocasionar a necrose do retalho. Vários recursos têm sido estudados com o intuito de aumentar a viabilidade desses retalhos. Dentre esses recursos, o laser de baixa intensidade é uma alternativa de tratamento, uma vez que pode promover um aumento da microcirculação e da neoformação vascular. Entretanto, existem discrepâncias na literatura em relação aos parâmetros empregados no uso do laser de baixa intensidade, principalmente das fluências utilizadas nos tratamentos. Este estudo teve como objetivo verificar o efeito de diferentes fluências do laser de 670nm, na viabilidade do retalho cutâneo randômico em ratos. Foram utilizados 100 ratos, da linhagem Wistar, distribuídos em 5 grupos de 20 animais cada. O retalho cutâneo randômico de base cranial foi realizado com dimensões de 10 X 4 cm e uma barreira plástica foi interposta entre o mesmo e o leito doador. O grupo 1 (controle) foi submetido à simulação de tratamento com o aparelho desligado. O grupo 2 foi submetido à radiação laser com fluência de 3 J/\'CM POT.2\'. Os grupos 3, 4 e 5 foram irradiados com fluências de 6 J/\'CM POT.2\', 12 J/\'CM POT.2é 24 J/\'CM POT.2\', respectivamente. Todos os grupos experimentais receberam a radiação laser imediatamente após o procedimento operatório e nos 4 dias subseqüentes, utilizando-se a técnica pontual em contato em 24 pontos distribuídos sobre e ao redor do retalho. No sétimo dia pós-operatório, as porcentagens da área de necrose dos retalhos foram avaliadas pelo método do gabarito de papel e também foi coletada uma amostra do retalho de 10 animais de cada grupo, escolhidos aleatoriamente, para a realização da contagem dos vasos sangüíneos. O grupo 1 apresentou média de área de necrose de 49,92%; o grupo 2 - 41,84%; o grupo 3 - 36,51%; o grupo 4 - 29,45% e o grupo 5 - 20,37%. Na contagem dos vasos, o grupo 1 obteve média de 65,2; o grupo 2 - 92,6; o grupo 3 - 105,5; o grupo 4 - 128,7 e o grupo 5 - 171,0. Na análise estatística, realizou-se a ANOVA, seguida do teste de comparações múltiplas de Tukey. Os resultados mostraram que todos os grupos experimentais apresentaram valores estatisticamente significativos comparados ao grupo controle, sendo que o grupo 5 apresentou a menor área de necrose e o maior número de vasos quando comparado aos demais grupos deste estudo (p < 0,01). O teste de correlação linear de Pearson indicou alta correlação negativa entre a porcentagem de necrose e o número de vasos dos retalhos (-0,972 / p = 0,0001). O laser de baixa intensidade (670nm) aplicado com fluência de 24 J/\'CM POT.2f́oi mais eficaz no aumento da viabilidade do retalho cutâneo randômico em ratos, comparado às outras fluências utilizadas neste estudo.

Introduction: Our study aims to highlight anatomical similarities, differences and variations in the microvascular anatomy between SGAP and IGAP flaps.Method: Thirty gluteal flaps (15 SGAP and 15 IGAP) were dissected on 22 adult fresh cadavers. We recorded the number and location of perforators, the characteristics of the main perforator (course, intramuscular/total length and diameter at the level of division at the greater sciatic foramen) and the anatomical variations of the submuscular venous plexus.Results: The mean number of perforators was similar in both flaps (seven). We found a statistically significant difference in the total length of the vascular pedicle between SGAP (mean:9.80cm) and IGAP (mean:13.36cm) flaps, which correlated strongly and was due to the difference in the intramuscular length of the perforator (mean difference of intramuscular length: 3.40cm). There was no statistically significant difference in the diameter of superior and inferior gluteal vessels at the point of pedicle division. Five superior gluteal flaps were found, on their lower half, to have perforators originating from the inferior gluteal artery. These were dominant perforators in two cases. In both flaps, there was significant variability in the thickness of sacral fascia and the amount of fibrofatty tissue deep to it. The close proximity of the inferior gluteal vessels to the sciatic nerve and the posterior cutaneous nerve of the thigh is demonstrated. The complex submuscular venous plexus with confluence of veins, which is typically found on SGAP flaps deep to the sacral fascia, is never encountered on IGAP flaps. We also report a rare anatomical communication between superior and inferior gluteal vessels deep to piriformis muscle and a case of atheromatous disease of the inferior gluteal artery.Conclusion: Our findings are highly relevant to clinical practice and contribute to the understanding of the complex and variable vascular anatomy of these challenging flaps.

Εισαγωγή: Η χρήση ελεύθερων κρημνών SGAP και IGAP θεωρείται μία από τις δυσκολότερες τεχνικές στη μικροχειρουργική.Σκοπός: Σκοπός της παρούσας μελέτης είναι να περιγράψει και να συγκρίνει την μικροαγγειακή ανατομία των δύο αυτών περιτονιοδερματικών κρημνών.Υλικό και Μέθοδος: Μελετήθηκαν 30 κρημνοί (15 SGAP και 15 IGAP) σε 22 φρέσκα πτώματα. Η ανάσπαση έγινε αρχικά υπό μορφή μυοδερματικού κρημνού, διατέμνοντας τα άνω ή κάτω γλουτιαία αγγεία στο επίπεδο του μείζονος ισχιακού τρήματος. Introduction: Perforator flaps whether in a free or pedicled form are essential in leg reconstruction, requiring meticulous dissection based on a detailed understanding of vascular topographic anatomy. Numerous investigators have addressed this issue. The aim of the current study was to investigate the vascular anatomy of Posterior Tibial Artery (PTA) evaluating a wide range of parameters, including perforators’ subfascial directionality, in order to precisely determine constant reliable perforator sites, in relation to surface landmarks on the medial aspect of the lower leg. Material and Methods: Anatomical dissections in 30 lower legs from 25 fresh cadavers was performed. The lower leg was divided into three equal vascular zones. Measurements were taken in reference to anatomical landmarks. Perforator clusters to 5cm intervals from medial malleolus were recorded and analyzed. Vessels with external diameter less than 0.5mm were excluded. Data regarding the number, distribution, type, external diameter, length from PTA , distance and subfascial directionality were collected and treated. Results: A total of 155 perforators were identified (average number: 5 per leg; average diameter: 1.0mm). Septocutaneous (127/155) perforators predominated, followed by musculocutaneous (19/155) and septomusculocutaneous (9/155). The majority was concentrated in the middle (73/155) and distal (64/155) tertile. There were no septomusculocutaneous perforators at the distal third of the leg, while septocutaneous perforators were encountered into all vascular tertiles. An average of 2 comitant veins accompanied each perforator. Length and diameter related to the perforators‘ location. There was a significant association between perforator length and type. Cluster analysis revealed that reliable perforators were identified within the 21-25cm, 26-30 cm and 16-20 cm intervals. Two constants perforators were identified, one in the middle of the tibia (± 2cm) and the second at a distance of 5-7cm from the medial malleolus. Conclusion: Clinically optimal perforators for the first time were precisely located in relation to subfascial directionality, vascular diameter and length from the source artery. Moreover, two constant perforators of the PTA to the medial surface of the tibia were identified. Continuous improving details of vascular anatomy will further evolve perforator flaps’ applications.

Εισαγωγή: Οι κρημνοί των διατιτραινουσών, μισχωτοί ή ελεύθεροι, κατέχουν ουσιαστική θέση στην αποκατάσταση της κνήμης. Για την παρασκευή τους, απαιτείται λεπτομερής γνώση της αγγειακής ανατομίας της περιοχής. Αρκετοί ερευνητές έχουν ασχοληθεί με την επέκταση της γνώσης αυτής. ▼ O insucesso de retalhos cutâneos devido à necrose total ou parcial não é um fato isolado. Várias pesquisas que utilizaram recursos farmacológicos e não farmacológicos foram realizadas para aumentar a viabilidade destes. O óleo de copaíba tem sido utilizado em vários modelos experimentais como cicatrizante de feridas. No entanto, não foram encontrados relatos sobre a utilização deste óleo na viabilidade de retalhos cutâneos. O objetivo deste trabalho foi avaliar os aspectos morfológicos e morfométricos de retalhos cutâneos em ratos tratados com óleo de copaíba (Copaifera langsdorffii). Trinta ratos Wistar foram distribuídos em três grupos: controle absoluto (GCA), controle (GC) e tratado (GT). Os ratos de todos os grupos foram submetidos à cirurgia de retalho cutâneo dorsal e avaliados por oito dias de pós-operatório. Os animais do GCA não receberam tratamento pós-operatório, enquanto os animais do grupo GC receberam tratamento tópico com pomada contendo apenas o veículo (vaselina e glicerina) e os animais do grupo GT foram tratados com óleo de copaíba em pomada a 10%, utilizando vaselina e glicerina como veículo. No oitavo dia de pós-operatório os animais foram ortonasiados e foram coletados fragmentos das porções cranial, média e caudal do retalho cutâneo para análise histopatológica. As análises de variância mostraram-se significativas (p<0,05) para área de necrose (GCA >GC e GT), número de vasos sanguíneos nas partes média e caudal (GT >GCA e GC), número de leucócitos nas partes cranial (GT >GC e GCA), média (GT e GC >GCA) e parte caudal (GT >GC >GCA), número de fibroblastos nas partes média (GT e GC >GCA) e caudal (GT >GC e GCA). Não houve diferença significante entre os grupos na contagem de vasos sanguíneos e fibroblastos na parte cranial e para fibras colágenas em todas as partes (p>0,05). A paralisia facial é uma patologia de etiologia ampla, com prejuízos múltiplos e severos aos indivíduos afetados. Dentre os tratamentos cirúrgicos atualmente propostos a reanimação facial com retalho de músculo temporal se posiciona intermediariamente no espectro de complexidade. Neste estudo objetivou-se a identificação dos resultados com seu uso em nossa disciplina, pela revisão dos prontuários dos pacientes acometidos de tal patologia nos últimos 17 anos. Dos 152 prontuários avaliados, 93 se enquadravam nos critérios de inclusão. Destes, 36 casos com paralisia facial periférica completa foram passíveis de estudo, correspondendo a 39% dos casos de transposição. Os resultados foram uma predominância de pacientes do sexo feminino (26) em relação ao sexo masculino (10). A reanimação conjunta de pálpebra e boca com a transposição do músculo temporal correspondeu a 16 casos; pálpebra a 12 casos; e boca a 8 casos. Nos casos com transplante muscular microcirúrgico, o músculo grácil foi utilizado. A média de procedimentos ancilares das pacientes do sexo feminino foi superior a do sexo masculino. Alterações da técnica da transposição do músculo temporal foram observadas em casos a partir de 2001, com a neurotização muscular por enxerto transfacial de nervo sural em 9 casos. A técnica utilizada em nosso serviço se baseia no retalho de Gillies, mas com utilização da própria fáscia do temporal para seu alongamento, tanto para o tratamento das pálpebras como da boca. A neurotização do músculo temporal como descrita é inédita na literatura, e passível de futura comparação de resultados com as técnicas mais atuais.

Facial palsy is pathology with a great number of causes, with important deficits to the patients. Among the surgical treatments used, the facial reanimation with temporalis muscle flap is a medium complexity procedure. This study intended to identify the results gained with this procedure in our division, analyzing the data of the patients who underwent the temporalis muscle transposition in the last 17 years. One hundred fifty two cases were reviewed, with 93 fulfilling the inclusion criteria. From these, 36 cases of complete facial paralysis reviewed, corresponding to 39% of the whole. Results showed a higher incidence of women in the study, with 26 against 10 men. Motion reanimation of the eyelids and mouth together with the temporalis transposition corresponded to 16 cases, 12 cases were to reanimation of the eyelids only, and eight cases to the mouth. When free muscle transplant was used for facial reanimation, all the cases were done with the gracilis muscle flap. Women had a higher rate of surgeries compared to men. Modifications of the procedure were observed in recent cases, after 2001. Neurotization of the transposed temporalis muscle flap was achieved with a cross-face sural nerve graft in 9 cases. Clinical interest on the flaps of the superficial back muscles has always been a challenging matter. The flaps of these muscles have been used for many years in the plastic and reconstructive surgery and with the developments in the field of microsurgery they tend to become some common flaps in the everyday practice. Furthermore their use has been proved not only as pedicles for the flaps, but as well as recipient vessels for free flaps in the area. Despite that, data on the morphometry and the variations of the arteries supplying these muscles are lacking. To investigate the subject authors performed a anatomical study on 40 adult fresh and embalmed Caucasian cadavers and examined the diameters and the variations of the blood vessels that supply this group of muscles and are used as pedicles. Our results on arteries dimensions and variations did not differ significantly from those of the literature regarding the latissimus dorsi. However although the variations of the blood vessels of the trapezius have been extensively discussed in the literature are similar to our results, not many studies have been performed regarding the size of the vessels. Therefore this study provides valuable figures in this field with an extensive amount of cadaveric dissections. Finally we performed an extensive literature review so to place our results in an anatomic, embryologic and especially clinical context. We believe that our results add knowledge that is valuable not only for the blood vessels of the area but also for the use of the muscles and the vessels in reconstructive surgery.

Η κλινική σημασία των κρημνών των ωμοραχιαίων μυών έχει αποδειχτεί ιδιαιτέρα σημαντική με την πάροδο των ετών. Οι κρημνοί αυτοί είναι από τους παλαιότερους και ευρύτερα χρησιμοποιουμένους κρημνούς στην πλαστική επανορθωτική χειρουργική και με την πρόοδο της μικροχειρουργικής, η χρήση τους έχει καταστεί συχνά πρακτική στην καθ’ ημέρα χειρουργική πράξη. Η χρησιμότητα των αγγείων των ωμοραχιαίων μυών (εκτός της προφανούς σημασίας τους ως αγγειακοί μίσχοι για τους προαναφερθέντες κρημνούς) έγκειται στην χρήση τους σε αναστομώσεις με αγγεία ελεύθερων κρημνών στην περιοχή, όπως στην αποκατάσταση μετά από μαστεκτομή. Ωστόσο, τα βιβλιογραφικά δεδομένα στην μορφομετρία και στις παραλλαγές αυτών των αγγείων είναι περιορισμένα. Οι συγγραφείς πραγματοποίησαν ανατομική μελέτη σε 40 (80 ημιμόρια σωμάτων) πτωματικά παρασκευάσματα (ταριχευμένα και μη) παρασκευάζοντας και μετρώντας τις διαμέτρους και τις παραλλαγές των αγγείων που αιματώνουν αυτήν την μυϊκή ομάδα (ωμοραχιαίοι μύες). Τα αποτελέσματα καταγράφηκαν και επεξεργάστηκαν και αναλύθηκαν με την βοήθεια της στατιστικής. Τα αποτελέσματα των διαμέτρων και των παραλλαγών των αγγείων της αιμάτωσης του πλατέως ραχιαίου μυός συμφωνούν με αυτά της διεθνούς ιατρικής βιβλιογραφίας. ▼ Objetivo:O presente estudo teve como objetivo investigar os efeitos da oxigenoterapia hiperbárica (OHB) e do dimetilsulfóxido (DMSO) na necrose tecidual, genotoxidade e na apoptose celular em retalhos cutâneos randômicos em ratos. Método: cinquenta ratos Wistar machos foram divididos aleatoriamente em cinco grupos: grupo controle (CT), onde o retalho de pele retangular de (2x8cm) foi dissecado a partir da camada muscular dorsal, preservando o pedículo cranial, elevado e fixado ao leito. Grupo água destilada (AD) injetou-se AD na metade distal do retalho, assim como grupo DMSO onde foi injetado DMSO a 5%, o grupo OHB onde foi realizado apenas o tratamento com OHB e grupo DMSO+OHB (DMSO associado a OHB) onde foi acrescentado o tratamento com oxigênio a 100% a 2,5 atmosferas absolutas (ATA) por uma hora, duas horas após o experimento, diariamente, por dez dias consecutivos. No décimo dia, os animais foram fotografados e realizado a coleta de tecido da porção central do terço distal do retalho viável para avaliação microscópica. Resultados: Não houve diferença entre os grupos em relação ao percentual de necrose do retalho cutâneo (ANOVA, p=0,249). O percentual e o escore de dano do DNA foi menor nos grupos OHB e DMSO+OHB, quando comparado com os demais grupos estudados (Tukey,p<0,05). O percentual de células viáveis (91,00%) e o índice de viabilidade celular (91,00%) no grupo OHB foi maior do que aquele para os demais grupos (p<0,05). Por outro lado, o percentual de células em apoptose inicial (6,80%) e final (2,20%), bem como o índice apoptótico (9,00%), foi menor no grupo OHB, quando comparado aos demais grupos experimentais (p<0,05). Conclusão: O tratamento com OHB, DMSO ou sua combinação não foi capaz de diminuir a área de necrose no retalho cutâneo. Por outro lado, a terapia hiperbárica aumentou a viabilidade celular e diminuiu o dano no DNA e a apoptose das células do retalho cutâneo. ▼ Background: Perforator flaps are becoming increasingly popular methods of reconstruction. Its appeal lies in the predictability of tissue bulk and low donor site morbidity when compared to the more traditionally accepted musculocutaneous flaps. The deep circumflex iliac artery osteomusculocutaneous flap, first described in 1979, has been used in a variety of reconstructions. In particular, it is well documented for use in the hemi-mandible. Similarly, the superior and inferior gluteal musculocutaneous flaps have been used for various breast and perianal reconstructions. As perforator flaps, they are poorly understood and therefore, surgeons often avoid them. Both the deep circumflex iliac artery (DCIA) and superior and inferior gluteal artery perforator (SGAP and IGAP) flaps are located in the posterolateral region of the abdomen and pelvis. The skin overlying these flaps is hairless and both flaps are associated with well hidden donor scars. The perforators have versatile pedicle length, diameter and arc. A study was therefore undertaken to better understand the anatomy of the perforator associated with the DCIA perforator and the SGAP/IGAP with a view to improve preoperative assessment of appropriate patients and improving surgical outcome. Method: A combined cadaveric and clinical study of the vascular anatomy of the posterolateral trunk was undertaken. This included an ink and lead-oxide injection of cadavers to investigate the DCIA perforators. The lead-oxide specimen also underwent x-ray radiographic assessment of the perforators. A Computer Tomographic Angiography (CTA) assessment of 44 DCIA and 160 SGAP/IGAP flap perforators were also performed in patients who were undergoing preoperative CTA prior to breast reconstruction. The clinical study was a series of patients who underwent pre-operative imaging with a view to proceed to a DCIA or GAP perforator flap. Their pre-operative findings and post-operative outcomes were assessed. Appropriate institutional ethical approvals were obtained. Result: Both the cadaveric and CTA studies demonstrated that just over half of hemi-abdomens were suitable for DCIA perforator flaps. All patients studied had sizable SGAP perforators whilst 95% of patients had sizable IGAP perforators. All patients who underwent a planned DCIA, SGAP or IGAP perforator flap at one institute had pre-operative imaging and assessment of their perforators. Whilst just over half of these patients had suitable DCIA, all the patients had well sized SGAP or IGAP. Those with poor DCIA perforators underwent the traditional free fibula flap instead. Their outcomes were comparable with low morbidity and high flap survival rate. Conclusion: Both the DCIA perforator and SGAP/IGAP flaps are useful reconstructive options.
