Spirituality has received increased attention in the psychiatric literature; however, it remains underexplored on a global level, especially among Chinese population. Knowledge about spirituality of people with schizophrenia is often hindered by their psychotic symptoms. This view not only results in a discrepancy in the understanding of spirituality between clients, professionals and family caregivers, but also limits clients’ expression of spiritual needs and impedes the provision of high-quality holistic care. Therefore, the understanding of spirituality in Chinese individuals with schizophrenia, and the roles spirituality play in the recovery process from the perspectives of clients, mental-health professionals and family caregivers warrants in-depth exploration. A qualitative design with semi-structured individual interviews was employed. The analysis was based on data collected from interviews with 19 clients diagnosed with schizophrenia, 19 mental-health professionals, and 11 family caregivers from public hospitals and mental-health community rehabilitation centres in Hong Kong. Data were collected and analyzed based on grounded theory principles. Ten components of spirituality emerged from the data: 1) Beliefs, 2) State of mind, 3) Sense of self, 4) Transformation/Growth, 5) Meaning in life, 6) Human nature, 7) Connection with others, 8) Connection with higher power, 9) Apparitional experiences, and 10) Religion. They were classified into personal domain (component 1-6) and communal domain (component 7-10), and formed a conceptual model which indicated the trajectory of spiritual development in people with schizophrenia. Three illness phases also emerged from clients’ illness experience: i) acute phase, ii) remission phase, and iii) stable phase. At different illness phases, components imposing the most prominent influences on clients’ recovery changed. The shift from one component to another indicated a change in the roles of spirituality in accordance with the degree of influences induced by the psychotic symptoms, as well as a change in clients’ spiritual pursuits throughout the process of recovery. The findings provided insights and perhaps a roadmap for clinical practices and research investigations in schizophrenia. First of all, a more inclusive definition of spirituality (i.e., beyond the boundary of religion) should be adopted so as to embrace the diversity of views expressed in discussions of spirituality in clinical setting and prevent biased results in research. Second, clinical practices and research investigations need to be phase-specific in order to meet clients’ spiritual needs at different illness phases. Finally, the trajectory of spiritual development in people with schizophrenia illustrated in the conceptual model indicates the value and feasibility of conducting spiritual interventions for this population, with prioritized goals.

Various studies from different universities and government departments indicated that Hong Kong is facing a serious urban decay problem. Redevelopment is costly and slow in progress. The average acquisition time used for redevelopment is over 6 years, and public doubted on social and environmental impact aroused from redevelopment projects. Relocated households lost their social support and networks after redevelopment and not all of them can opt for re-housing. In addition, the 7-years money compensation policy cannot catch up with the rapid increase of property market value and public quires about whether redevelopment can really improve people’s living condition and fulfill the original goal and objective of building redevelopment. By contrast, building rehabilitation was highly promoted by the government and professional bodies in solving the problem of urban decay. Without wasting time and cost on relocation of households, their living environment can still improve through building rehabilitation. However, recent studies found out that owners are unwilling to carry out building rehabilitation. In order to find out obstacles affecting building rehabilitation of old private residential building in Hong Kong and speed up the building rehabilitation to solve long term urban decay problem, personal in-depth interviews were conducted together with 77 questionnaires sent to owners of both single building blocks and large residential building estate to ascertain their views on building rehabilitation in old private residential building in Hong Kong. The findings showed that the positive effect of building rehabilitation is highly recognized by the public. However, it is difficult for owners to coordinate enough ownership to carry out building rehabilitation without the help of property management or Incorporated Owner and supports from government departments. The threat of government’s statutory orders and mandatory building inspection scheme had only little deterrent effect to owners who did not complied with the legal requirement and also the enforcement action from government departments is not strong enough too. Moreover, the findings showed that owners are motivated to carry out building rehabilitation if there is economic assistance to maintain their existing social network. Recommendations were proposed to provide more economic and technical supports from Urban Renewal Authority (URA) and Home Affair Department (HAD) for owners who carrying out building rehabilitation. Government should enhance the statutory enforcement along with enhancement propaganda of building rehabilitation.

▼ In view of the increasing prevalence of female breast cancer in the global and local arenas, this disease has become a critical problem affecting women’s physical, psychological, spiritual and social conditions. With medical technology advancements, women can receive early diagnosis and effective treatment. However, maladjusted rehabilitation affects women’s long-term recovery. In order to improve women’s rehabilitation, more attention is being paid to breast cancer survivorship care after treatment. Therefore, how women experience and perceive breast cancer during the transition period is the important information for us to understand women’s breast cancer survivorship. The present study aims to explore how women perceive their breast cancer experiences during the transition period from a holistic perspective. This study adopted the qualitative inquiry approach. Movement-based focus group design was the present research design, in which dance/movement activities and group discussions were included. It is based on the belief that expressing through dance/movement enables an individual to reveal the experiences which have been stored in the body. Four focus groups were used, with a total of twenty recruited informants. Data collected from the focus group interviews included verbal (transcriptions) and non-verbal (writing/drawing and created body shapes) expressions. Interviews were transcribed verbatim and analyzed using the ATLAS.ti 7 qualitative data analysis software. The findings indicated that women's descriptions of their experiences during the transition period were associated with bodily, emotional, cognitive, spiritual, and self and social relational perspectives. Five domains (bodily, emotional, cognitive, spiritual and self and social relational experiences) were generated after data processing. Analysis of the contents in each domain reveals the body-mind-spirit connection. During this period, women consider their health and exercise to be important, they were relaxed and happy, they reflected more, they searched for purpose and meaning in life, and they appreciated developing relationships with others and self. Furthermore, the results showed that these women attained self-growth through their breast cancer experiences. Nevertheless, the self-growth areas and pace were different for each individual. The results also revealed that the non-verbal information (writing/drawings and created body shapes) enriched the women’s expression by illustrating their in-depth or hidden breast cancer experiences. The results provide practitioners with insight and direction into designing rehabilitation programmes for breast cancer survivors. Therefore, the contents of the programme include not only psychosocial aspects but also bodily, cognitive and spiritual perspectives. Furthermore, the present movement-based focus group demonstrated the use of non-verbal expressions, as well as dance/movement, to enhance communication. ▼ Introduction Rheumatic disease is an autoimmune disorder with an unpredictable course of exacerbation and remission. There is no known cure for the disease at the moment. The patients’ conditions may progressively deteriorate despite intensive therapies, and runs an erratic course with the possibility of disfiguration and alteration in body image. Pain, disabilities and psychological distress are common. Rheumatic patients may respond differently to the same level of pain and physical symptoms. The understanding of the needs of rheumatic patients and how they successfully manage the disease and optimize psychological adjustment can help develop effective psychosocial interventions. Aims The aims of the study are (1) to identify the needs of rheumatic patients and perceptions of their disease, (2) to develop a conceptual framework for psychological adjustment, and (3) to identify factors associated with resilience in rheumatic patients. Methods The present study consisted of two phases. The first phase was a focus group interview, aiming to understand the patients’ feelings and to design a questionnaire. The second phase was a prospective questionnaire survey that includes a baseline study and a six-month follow-up study. Patients were recruited from support groups in Hong Kong. The baseline questionnaire was self-administrated, and the follow-up questionnaire was administrated by telephone interview. The self-regulation model was chosen as the basis for the conceptual framework for psychological adjustment. The questionnaire included demographics, illness representation, coping efforts, appraisal of coping efforts, sense of coherence, quality of care, functional disability, and health-related quality of life. The outcome measures were functional and psychological health, change in adjustment, and positive and negative resilience. Results Having a good and caring doctor, more information on the disease, and public understanding of the disease were the needs of rheumatic patients. The patients perceived that the disease was chronic, cyclical, and had poor consequences. They perceived that the disease caused great pain, stress, depression and anxiety, and affected their daily activities, appearance, and relationship with family and friends. Poor adjustment was associated with chronic and cyclical timeline, and poor perception of personal and treatment control. The analysis of resilience shows that positive perception of treatment control and disease consequence, correct understanding of disease causes, and high sense of own value and importance to the society, were protective. While those who lacked family support and blamed themselves or their families to be the cause of disease, were vulnerable. The number of patients with colorectal cancer had increased dramatically in recent years (Hong Kong cancer registry, 2012), and surgical resection is the first line treatment of colorectal. To enhance patients’recovery process, there is a need to develop a comprehensive and user-friendly, with most important, an evidence-based guideline for promoting patients’ recovery process. Traditional post-operative management is associated with different postoperative complications, delayed recovery, and lengthened hospital stay. Recent research documented that using a specific rehabilitation programme focused on education; early mobilization and early diet regime could enhance patients’ recovery. Therefore, this transitional research aims to evaluate the current evidence on the effect of adopting a specific rehabilitation programme, to formulate an evidence-based guideline, assess its implementation potential, and to develop an implementation and evaluation plan. Ten related literature were retrieved from four electronic bibliographical databases. Critical appraisal had been done to ensure the quality and validity of the selected evidences. A clinical guideline is developed based upon the information from the identified high level of literature. The implementation potential is assessed based on the similarity and the readiness of the target setting to the proposed environment. It was found that the transferability of the protocol was high and it was feasible to be implemented into the target site. Little expenditure and input was expected, as the protocol was a systematic reformation of practice, rather that developing a set of totally new practice to current clinical setting. An implementation plan was then planned, which included the communication plan with all the stakeholders. After reaching a consensus among the stakeholders, a two-month pilot study will be carried out for examining the readiness before the full-scale implementation of the program. The evaluation plan of the effectiveness of the proposed program is developed. Result will be used to provide recommendation for further adjustment on the protocol to yield a better outcome. The implementation of this nurse-led rehabilitation program is suggested to be worthy of adoption in the clinical setting for bringing benefits to patients, the hospital and staffs.

▼ The rise of Chinese economy has favoured the growth of several financial centres in China. This phenomenon has drawn much attention to several scholars who focus on the relationship between Hong Kong and Shanghai. With a few exceptions, the relationship among Beijing, Hong Kong, Shanghai and Shenzhen have not been satisfactorily revealed, while financial competition and cooperation among them are also far from clear. This thesis examines the financial systems of Hong Kong, Beijing, Shanghai and Shenzhen to show their strengths and weaknesses. Financial markets, such as the stock market, bond market, funds market, insurance market, futures and derivatives market as well as the foreign exchange market are all included in this study. In addition, other important players (banking industry and multinational corporations) in the financial system are covered. This study shows that Hong Kong’s strengths lie in its stock, fund, insurance, financial futures and options, foreign exchange and its related derivatives markets, banking industry as well as the attractions of multinational corporations; whereas it is weak in parts of the stock market, the GEM, bond, PE/VC funds and commodity futures markets. Beijing is an important player in the banking industry, stock, bond, private equity fund, VC fund and insurance markets. However, it is not so developed in regard to foreign currency bond, fund (excluding PE and VC funds), financial futures and options, foreign exchange and related derivatives markets, foreign banking and the attractions of multinational corporations. Shanghai is considered to be at a similar level of ranking as Beijing. It is the capital centre of China and has the no.1 fund market in the Mainland although weaker than that of Hong Kong. Shanghai is also strong in foreign banking industry and the attraction of multinational corporations. Whereas Shanghai is weak in its banking industry (excluding foreign banking), bond market, PE/VC funds, financial futures and options as well as foreign exchange markets. Shenzhen is the weakest financial centre, with its advantages in the SME board, ChiNext and VC funds. Another finding of this study reveals that, except for the previous studies on financial centre competition and cooperation in China, there are still some potential areas such as the financial cooperation between the HKEx and SSE, the GEM and ChiNext as well as the financial competition of the private equity fund industry between Hong Kong and Beijing for the financial competition and cooperation of Beijing, Shanghai, Shenzhen and Hong Kong. The research findings of this thesis have both theoretical and political implications. Theoretically, the study reflects the significance of an information hinterland as well as law and finance theory in the Chinese financial market and also proves the importance of Beijing as a financial centre in China. Empirically, this thesis offers some implications for policy marking to enhance the better development of these financial centres. Successive episodes of financial crises raise the question of whether international financial law is insufficiently robust to support global financial stability. International financial law can be characterized today as a set of “soft law” rules and standards adopted voluntarily and differentially by individual states and lacking any real international enforcement mechanism. The pre-2009 system of international financial regulation failed to check financial excesses in globalized financial markets. Liberalization and globalization led to under-regulation accompanied by lack of accountability mechanisms and brought the failure of international financial institutions and systemic instability. The crisis that commenced in 2008 -the most severe blow to the international financial system since 1929 -calls for a fundamental re-design of regulatory standards, financial risk prevention and management mechanism and the international financial architecture. This study argues that a hard-law based international financial law (based on the enforcement of regulations) is essential for maintaining global financial stability as the existing soft-law based financial regulatory architecture failed not only to prevent and manage the crisis in 2008 but also in checking those financial excesses committed by the de-regulatory and liberal regimes which threatened global financial stability, the discipline of international financial law and its regulatory institutions. This study thus argues that a law-based enforcement mechanism for international financial regulation is essential for global financial stability. This research questions the fundamental basis of the existing international financial architecture (soft law) and explores the need for a compliance-based model (based on legitimacy of regulations and accountability of the regulatory bodies) in the international financial system. In so doing, this thesis proposes fundamental changes and alternative perspectives to the existing framework of international financial law which is in need of a fundamental re-design as has been proved by recurring episodes of financial disruptions.

▼ Disengagement from mental health services by patients with schizophrenia spectrum disorders is an important issue. Most research in this area has been focused on studying the patients’ demographic and clinical characteristics that may be related to disengagement. However, research on patients’ perspective in regard to their decision of disengagement has been limited. An assessment tool developed based on an in – ‐‑depth understanding of patients’ subjective experiences may capture other crucial factors. This may widen our scope of understanding of this phenomenon. Therefore, a mixed research method was employed in this thesis and three independent studies with distinctive research aims were carried out. The first study was a qualitative study which aimed at exploring reasons and factors that patients perceived influential to their decision of disengagement. Six ever – disengaged male patients aged between 18 and 28 with schizophrenia spectrum disorder were interviewed as a purposive sample after a thorough subject identification procedure. A thematic analysis of the data yielded seven themes which grouped into three domains: service (patient – ‐‑clinician communication, service orientation, clinic operation); patient (psychological response, perception of illness); and medication (side effects and uncertain efficacy). A 16 – ‐‑item self – ‐‑administered questionnaire, the Patient’s Perception of Psychiatric Service (PPPS) questionnaire, was developed. The construction of PPPS was based entirely on the themes identified from the first qualitative study. This questionnaire measured the subjective perception of the patient about the service and patient – clinician communication. Validation of the PPPS questionnaire was conducted in the second study by recruiting 50 patients with a diagnosis of a schizophrenia spectrum disorder from a psychiatric outpatient clinic. Results demonstrated that the PPPS questionnaire has good internal consistency, test-retest reliability, and convergent validity. The Singh O'ʹBrien Level of Engagement Scale (SOLES), Client Satisfaction Questionnaire (CSQ), and an internalized stigma scale were also translated and validated in this study for use in the third study. The third study explored the relationship between disengagement and the patient’s perception of service, using PPPS, level of engagement (SOLE), satisfaction with service (CSQ), and other factors including clinical characteristics and service utilization. One hundred and fifty patients with schizophrenia spectrum disorder were recruited from two specialized outpatient clinics. In view of local clinical observation, patients who had more than one disengagement episode and each lasted more than two weeks were classified into the severe – ‐‑disengagement group. In a forward stepwise regression model, results suggested that PPPS and length of service predict severe disengagement. ▼ Disability and dependence are the two major sufferings experienced by stroke survivors and their caregivers. In addition to the use of conventional management, Traditional Chinese medicine (TCM) is being more accepted as one of the treatment modalities for post-stroke rehabilitation worldwide. This thesis is comprised of a study with an aim to identify the prevalence of patients whom receive TCM treatments additionally during the post-stroke rehabilitation in Hong Kong, and also to measure the pragmatic effectiveness per se. This thesis began with a comprehensive literature review describing the epidemiology, classification, pathophysiology, clinical manifestations and conventional managment of stroke. Pathogenesis and treatment for stroke were outlined in TCM perspective, and its current research progress was also provided as the background for the study reported in this thesis. The first part of this study was a cross-sectional survey which estimated the prevalence of patients with TCM treatments added in their post-stroke rehabilitation. The second part of the study was a retrospective cohort analysis which evaluated the pragmatic effectiveness of additional TCM treatments for post-stroke rehabilitation. In parallel, a sub-group analysis was conducted to identify the add-on effectiveness of an individual and/or combined TCM treatments, the treatment modalities included acupuncture, Chinese herbal medicine (CHM) and also Tui-na. 209 patients were included in this study. 130 (62.2%) of them were identified attempting at least one type of TCM. Additional use of TCM treatment was found to be more common in the out-patient setting than the in-patient one. When comparing with patients received conventional management alone, those receiving additional TCM treatment showed better improvement in MBI-C and prognosis in SSQOL-12-C (p<0.05). Additional TCM treatment was also found to be a significant protective factor against unfavorable dependent status and anxiety (p<0.05). Sub-group analysis showed that only the additional combined use of acupuncture and CHM; and combination of acupuncture and Tui-na treatments had better improvement in MBI-C. Combination of acupuncture and CHM treatments might predict good prognosis in SSQOL-12-C, and acupuncture might protect patients against poor prognosis in HADS-C (p<0.05). While combination of acupuncture and CHM was a protective factor against unfavorable dependent status, acupuncture might help patients to fight against anxiety in addition to unfavorable dependent status (p<0.05). The results indicated that the use of additional TCM treatment might be effective in improving the disability and reducing dependency of stroke patients. This study yielded generalizable results by controlling most of the potential cofounders that are unavoidable in the real world practice, and the result also provided insights and future directions for explanatory research in this area. Background: Individuals with autism spectrum disorders (ASD) are characteristically deficient in social awareness and communication skills, while displaying repetitive behaviours. Because of this, integration of individuals with ASD into society is often difficult as people do not know how to respond to their atypical behaviour. Particularly in Asian cultures, this vulnerable group is often subjected to stigmatization. However, recent literature has highlighted some benefits of assistive animal intervention (AAI) for individuals with ASD, which may improve their ability for social interaction. This review examines the effects of AAI in terms of service dogs for individuals with ASD. Methods: Journal articles were identified through a systematic review on psycINFO, Pubmed, Scholars Portal and Web of Knowledge. Additional searches were conducted for inclusion of grey literature and doctoral theses in the review. Results: Of the 170 studies initially found, 7 were selected; 3 cohort studies, 3 case-control studies, and 1 cross-sectional study. Reported outcomes included increased social reciprocity, decreased behavioural problems and decreased stress in individuals with ASD. Despite these positive outcomes, they should be taken with caution, as the designs of the seven studies included in this review show methodological weaknesses, such as small sample sizes and possible information bias from data collection. Conclusions: Additional research focused on varying age groups with ASD, longitudinal studies and standardized outcome measures are required for further research. What this review suggests though is that there is a need to address social services gaps for various disability groups, not just for those with ASD. Some legislative measures are in place in Hong Kong to support the accessibility of assistance dogs in public; however, since there are so few in Hong Kong, their exposure is not enough to gain widespread acceptance. Expansion of assistance dog services to serve other disability groups may help to enhance their quality of life.

▼ International financial centre (IFC) development is a hot topic in today’s global arena at the political state level and within academic circles as they can have a significant impact on national, regional and local economies. A critical review of the literature on this topic reveals that not much scholarly attention has been directed towards how IFCs develop from within, more specifically how local entrepreneurial activity contributes to the advancement and evolution of an IFC. In addition, a number of different theories such as path dependence and the concept of social networks have been used as alternative frameworks to explain the phenomena of spatial agglomeration in international financial centres (IFCs); however, these theories haven’t either been properly constructed in a geographical context, empirically applied in a convincing manner or been further investigated using different methodological frameworks. Through the lens of the entrepreneurial hedge fund (EHF) firm and by incorporating a multiple methodological approach (quantitative, descriptive and spatial analysis); this research investigates four separate empirical lines of inquiry in regard to either the firm, its proprietor or the IFC that focuses on micro characteristics, spatial characteristics, the general business arena and development mechanisms. The major empirical findings are that the EHF firm can be classified as small and large based on a number of different factors; while the proprietor is a well educated individual who was previously employed as a high level manager of a large multinational corporation and has previous career ties to the investment banking and traditional fund management sectors. EHF firms agglomerate in IFCs with the most intense clustering occurring within close proximity to the nucleus of the main financial district and other agglomeration patterns are evident. Categorically, government and regulatory factors and people factors are considered as the most important competiveness factors of an IFC. When compared as a whole with previously conducted studies, the findings were found to be statistically indifferent; however, at the individual factor level there are distinct differences. The factors that trigger entrepreneurial behavior are endogenous in nature and the top barriers encountered were customer related followed by employee recruitment and regulatory issues. Finally, human agency and social networks are an integral part of the entrepreneurial process and can be categorized into five separate groups with professional and associate considered to be the most important. This study makes three theoretical contributions on developmental aspects of IFCs. First, a spatial agglomeration model is proposed based on areal differentiation that is derived from the established and changing patterns in the human landscape and its institutions. Patients often judge a successful dental encounter based on the professional ability of the dentist as well as his ability to personally interact with them. Quite often, dental practitioners encounter patients who are in distress as a result of orofacial pain or discomfort. A number of individuals seeking oral care may be suffering from pain, they may anticipate pain. Patients may have preconceived notions about dental care, based on past dental experience, stories by friends and family, or due to the presence of certain phobias. The dentist and staff who encounter such individuals must be able to communicate effectively, in order to make them comfortable and establish a positive relationship to help provide the best possible oral care. Common difficulties faced in attaining communicative influence with these patients, within the confines of a dental operatory, include - intimidating environment, anxiety provoking thoughts of pain & discomfort, use of ineffective communication techniques, etc. Studies in the past have described how the knowledge of silent language of people can help a dental team tremendously. Some studies have suggested that non-verbal messages conveyed by health care professionals can either encourage or discourage patient’s trust. The ability to study and use body language may therefore be a valuable tool in clinical dental practice. A review of the literature on the studies of non-verbal communication in dentistry was carried out as a part of this thesis. It pointed to the limited research work in this field, within the various disciplines of dentistry. There was no evidence-based support for the use of non-verbal communication for behavior management in paediatric dentistry. This study showed the presence of a gap in literature with regards to understanding and effective utilization of non-verbal communication in clinical practice. A mixed methods study was carried out to examine the body language of the dental team and the child patients (age 5 to 12 years), using audio-visual recording technique, during routine dental checkup and consultation visits. The study design paid emphasis on the qualitative analysis of dental interactions, occurring naturally, during a paediatric patient’s visit to the dental hospital. A new coding system was developed and applied to the digital data, to analyze various forms of body movements used in the dental setting. The analysis described how verbal as well as non-verbal communication strategies are used together in paediatric dentistry, to achieve cooperation of the young patients. This study also demonstrated how video-analysis and non-verbal coding system can be a useful tool for enhancing the understanding of non-verbal interactions between the dental team and their patients. 1
