▼ Often few alternative anesthetics for exotic species are available, due to the small numbers of these animals used in research. In this study, we evaluated the depth and duration of anesthesia in Xenopus laevis after their immersion in 3 doses of etomidate (15, 22.5, and 30 mg/L) and in 3 doses of benzocaine (0.1%, 0.5%, and 1%) compared with the ‘gold standard,’ tricaine methanesulfonate (MS222; 2 g/L). We then chose an optimal dose for each alternative anesthetic according to induction time, duration of surgical plane, and time to complete recovery. The optimal etomidate and benzocaine doses (22.5 mg/L and 0.1%, respectively) as well as the MS222 dose were then used to achieve a surgical plane of anesthesia, with the addition of flunixin meglumine (25 or 50 mg/kg) administered in the dorsal lymph sac at the completion of mock oocyte harvest. Efficacy of the analgesic was assessed at 1, 3, 6, and 24 h postoperatively by using acetic acid testing (AAT). Histology of the liver, kidney, and tissues surrounding the dorsal lymph sac was performed at day 3, 14, and 28 in each group of animals. Mild to moderate myocyte degeneration and necrosis were present in tissues surrounding the dorsal lymph sac at both flunixin meglumine doses after etomidate and benzocaine anesthesia. In addition, the 50-mg/kg dose of flunixin meglumine resulted in the death of 5 of the 12 frogs within 24 h, despite an otherwise uneventful anesthetic recovery. In conclusion, benzocaine and etomidate offer alternative anesthetic regimens, according to typical requirements for an anesthetic event. Flunixin meglumine at the 25-mg/ kg dose provided analgesic relief at the latest time point during etomidate dosage and at all time points during benzocaine dosage, but further characterization is warranted regarding long-term or repeated analgesic administration. O Choque Hemorrágico (CH) pode complicar-se por “insuficiência adrenal relativa” (IAR). Neste estudo avaliou-se a contribuição do etomidato e apoptose da adrenal na IAR pós-CH. Ratos machos Wistar foram divididos em 4 grupos: G0 (anestesia geral com isoflurano e ventilação mecânica, 16 ratos), G1 (como G0 mas com buprenorfina, 16 ratos), G2 (como G1 mas com CH, 16 ratos) e G3 (como G2 mas com etomidato antes do CH, 16 ratos). CH induzido por recolha de sangue (30% volume sanguíneo). Resuscitação aos 90 minutos pós-CH, consistindo no sangue recolhido associado a soro fisiológico. Aos 0, 90, 150 and 240 minutos após início do CH, mediram-se gasimetria e níveis plasmáticos de hormona adrenocorticotrópica, corticosterona, TNF-α, IL6, IL10 e por RT-PCR, TNF-α, IL6 e IL10. Apoptose avaliada por TUNEL e caspase-3 (immunofluorescência); necrose por escala específica. G1 apresentou taxa de apoptose mais elevada que G0. G2 e G3 tiveram níveis mais elevados de apoptose e necrose que outros grupos. Comparado com G2, G3 apresentou níveis de corticosterona compativeis com IAR e níveis mais baixos de PO2, PO2/FiO2, BE, bicarbonato, apoptose e necrose, e mais altos das citoquinas e TNF- α.IAR foi associada a etomidato mas não a apoptose neste modelo.

ABSTRACT - Critical Illness-Related Corticosteroid Insufficiency in Hemorrhagic Shock- an experimental study about its pathophysiologic mechanisms in a rat model (Rattus norvegicus) - Hemorrhagic Shock (HS) can be complicated by critical illness-related corticosteroid insufficiency (CIRCI). This study evaluated if etomidate and adrenal gland apoptosis contributed to HS-associated CIRCI. Male Wistar rats were divided in 4 groups: G0 (general anesthesia with isoflurane and mechanical ventilation, 16 rats), G1 (similar to G0, but with buprenorphine, 16 rats), G2 (similar to G1 but with HS, 16 rats) and G3 (similar to G2 but with etomidate before HS, 16 rats). HS was induced by collecting 30% of the blood volume and resuscitation (90 minutes later) consisted in the collected blood and normal saline. Blood gas analysis, adrenocorticotropic hormone, corticosterone, TNF-α, IL6, IL10 and TNF-α, IL6 and IL10 mRNA quantified by RT-PCR were determined at 0, 90, 150 and 240 minutes post-HS induction. Apoptosis and necrosis were determined by TUNEL and active caspase-3 immunofluorescence and a standardized necrosis score, respectively. G1 had higher adrenal apoptosis than G0. G2 and G3 had significantly higher levels of apoptosis and necrosis. G3 had significantly lower levels of corticosterone (compatible with CIRCI), PO2, PO2/FiO2, BE, bicarbonate, apoptosis and necrosis score and higher levels of cytokines and TNF-α than G2. Etomidate but not apoptosis was associated to CIRCI in this HS model.

▼ Izhodišče – Namen naše dvojno slepe randomizirane raziskave je bila primerjava hemodinamskih učinkov z bispektralnim indeksom (BIS) vodene infuzije etomidata in propofola pri bolnikih, predvidenih za veliko operacijo v trebušni votlini. Metode – 46 bolnikov, predvidenih za veliko operacijo v trebušni votlini zaradi raka, smo naključno razporedili v dve skupini. Skupina E je za uvod v anestezijo dobila infuzijo etomidata, skupina P pa infuzijo propofola. Oba anestetika smo odmerjali do vrednosti BIS 60. Hitrost infuzije je bila 0,5 mg/kg/min za propofol in 0,05 mg/kg/min za etomidat. Meritve srednjega arterijskega tlaka (SAT), indeksa minutnega volumna srca (MVSI), indeksa sistemskega žilnega upora (SŽUI) in srčne frekvence (SF) smo začeli eno minuto pred odmerjanjem fentanila in jih nato ponavljali vsako minuto v naslednjih 20 minutah. Prav tako smo merili čas od začetka infuzije anestetika do: izgube palpebralnega refleksa, vrednosti BIS 60 in vstavljanja dihalne cevke. Zapisali smo porabljen odmerek anestetika. Vrednost p < 0,05 smo obravnavali kot statistično značilno. Rezultati – V analizo je bilo vključenih 46 bolnikov. Pred in med vstavljanjem dihalne cevke glede SAT, MVSI, SŽUI, SF, BIS in saturacije ni bilo statistično značilnih razlik med skupinama, le vrednost SAT je bila v skupini E med vstavljanjem dihalne cevke statistično značilno večja glede na skupino P (p = 0,019). Po vstavitvi dihalne cevke je bila vrednost SAT v skupini E statistično značilno večja od vrednosti v skupini P 1 (p = 0,002), 2 (p = 0,000), 3 (p = 0,002), 4 (p = 0,002), 5 (p = 0,018), 6 (p = 0,012) in 7 (p = 0,004) minut po vstavitvi. V skupini E je bila vrednost MVSI statistično značilno večja 2 (p = 0,039), 6 (p = 0,038) in 7 (p = 0,024) minut po vstavitvi dihalne cevke, medtem ko so bile razlike v vrednosti MVSI med skupinama 3 (p = 0,093), 4 (p = 0,055) in 5 (p = 0,068) minut po vstavitvi dihalne cevke na meji statistične značilnosti. V skupini E so bile večje kot v skupini P. Vrednosti SŽUI in SF se celoten čas po vstavitvi dihalne cevke med skupinama niso statistično značilno razlikovale. Med vstavljanjem dihalne cevke sta se vrednosti SAT in SF v skupini E statistično neznačilno razlikovali od izhodne vrednosti, medtem ko je bila vrednost SAT v skupini P statistično značilno nižja od izhodne vrednosti, vrednost SF pa se od izhodišča ni statistično značilno razlikovala. V obeh skupinah je bila vrednost MVSI v času od 3. minute pred vstavljanjem dihalne cevke do konca meritev statistično značilno manjša od izhodne vrednosti. Pri 23 bolnikih so se pojavili zapleti, ki so zahtevali rešilne odmerke zdravil: pri 10 bolnikih v skupini P in pri 13 v skupini E. Glede na pojav zapletov se skupini v splošnem nista razlikovali (p = 0,376). V skupini P je bilo več hipotenzije (p = 0,08), v skupini E pa statistično značilno več hipertenzije (p = 0,007) Zaključek – Z raziskavo smo ugotovili, da je uporaba propofola za uvod v anestezijo povezana z manj hipertenzije in tahikardije med vstavljanjem dihalne cevke v primerjavi z uporabo etomidata.… Advisors/Committee Members: Kamenik, Mirt.

고혈압 환자는 술중 극심한 혈압 변동을 예방하기 위해 수술 당일 아침에도 평소 복용하던 항고혈압제를 복용하는 것을 권장하고 있으나(Prys-Roberts 등, 1971) 이러한 수술 전 혈압 조절에도 불구하고 많은 고혈압 환자들은 마취 유도 후 기관 삽관전까지는 저혈압을 보이고, 기관내 삽입 시에는 심한 혈압상승을 나타낸다. 마취 유도 후 발생하는 저혈압의 원인은, 고혈압 환자들은 체내 수액량 부족 상태인 경우가 많고 또한 대부분의 전신마취제와 항고혈압제는 혈관확장효과나 심근억제효과를 가지고 있기 때문이다(Morgan, 2006). 현재 전신 마취 유도제로 가장 널리 사용되고 있는 thiopental sodium은 작용 발현시간이 짧고 회복이 빠른 장점을 가지고 있으나 심혈관계에 대한 영향으로 심혈관계 기능이 저하되어 있는 환자에서 투여 시 현저한 혈압하강이 야기되는 단점이 있다 (Chamberlain 등, 1977; Berthoud 등, 1993). 근래 임상에 도입되어 전신마취 유도제로 사용되고 있는 etomidate는 정주 시 신속하고 강력한 효과를 보이고 심혈관계와 호흡기계에 대한 작용이 미미하여 심폐기능이 감소된 환자에서 유용하게 사용되는 약물이다 (Holdcroft 등, 1976; Criado 등, 1980; Gooding 등, 1979; Giese 등, 1985). 본 연구에서는 고혈압 환자에서 전신 마취 유도 시 통상적으로 사용되고 있는 thiopental sodium 에 비하여 etomidate를 사용하면 좀 더 혈역학적으로 안정적인 마취 유도가 가능한지를 알아보고자 하였다. 계획수술이 예정된 환자 중 고혈압이 있으면서 미국마취과학회 신체등급 분류 2 또는 3등급에 속하는 75세 이하의 성인 30명을 대상으로 하였고, 무작위로 thiopental sodium 과(Group 1, n=18) etomidate(Group 2, n=12)로 마취 유도를 하는 군으로 나누었다. 연구 대상 환자의 성별, 미국마취과학회 신체등급분류, 나이, 키, 몸무게는 두 군간 유의한 차이가 없었다. 환자가 수술방에 도착하고 3 전극의 심전도, 혈압, 맥박산소 포화도 감시를 시작하였으며 200-300 ml의 정질액을 투여하여 혈역학적 안정을 도모하였고 수축기혈압, 이완기혈압, 평균혈압, 심박수를 측정하여 기준값으로 하였다. 환자들에게 fentanyl 1.5 ㎍/kg를 정주한 후 무작위로 두 군으로 분류하여 thiopental 4 mg/kg(1군) 또는 etomidate 0.2 mg/kg(2군)을 정주하였고 이 시점부터 수축기혈압, 이완기 혈압, 평균동맥압, 심박수를 측정하였고, 1분 간격으로 상기 활력지수를 측정하였다. 환자의 의식이 소실되었을 때 sevoflurane 4 vol%로 용수조절 환기를 시행하고 rocuronium 0.7mg/kg을 정주하였으며, 5분간 용수조절 환기 시행 후 기관 내 삽관을 시행하였다. 정맥 마취제 투여하고 5분이 지난 이후에도 수축기혈압이 130mmHg 이상일 경우 nicardipine 0.5mg 을 투여하였고 (Tobias, 1995; Song 등, 1997), 지속적으로 용수조절환기를 시행하며 수축기혈압이 130mmHg 미만이 되거나 기준치 혈압의 20% 이하가 될 때 기관 내 삽관을 시행하였으며, 5분 경과하기 전에 수축기혈압이 90mmHg 미만이거나, 평균혈압이 60mmHg 미만으로 감소할 경우 3분이나 4분 경과시에도 기관 내 삽관을 시행하였다. 기관 삽관 직후와 1분 후부터 5분 후까지의 수축기혈압, 이완기혈압, 평균혈압과 심박수를 1분 간격으로 기록하였으며, 삽관 직후 수축기혈압이 190mmHg 이상인 경우 nicardipine 0.5mg을 투여하였다. 이후 sevoflurane 2.0 vol%와 산소 분당 2 L, 공기 분당 2 L로 마취 유지를 하고 관찰 기간 동안 아무런 자극을 주지 않았다. thiopental 군과 etomidate 군 모두 마취 유도제 투여시점 부터 기관내 삽관 전까지 심박수, 수축기 혈압, 이완기 혈압, 평균 혈압이 감소하였고, 기관내 삽관 시 심박수, 수축기 혈압, 이완기 혈압, 평균 혈압이 상승하였으나 두 군간 유의한 차이는 없었다. 고혈압 환자에서 마취 유도시 저혈압이 생길 가능성이 매우 높다. 이러한 이유로 저혈압이 있는 환자에서 마취 유도시 혈역학적 안정성을 위하여 사용하는 etomidate를 고혈압 환자의 마취 유도시 사용한 결과 thiopental sodium과 안정성이 차이가 나지 않았다. 이는 고혈압 환자의 전신마취 유도시 어떤 마취제가 더 우수하다고 명확히 입증되지 않았으며 동일한 안정성을 갖고 있다는 기존의 결과와 비슷한 결과를 보였다 (Miller, 2010). 이는 본 연구에서 심혈관 예측 지표 중 중요한 위험지표들을 가진 환자나, 심장위험도 평가에서 높은 위험도의 수술은 제외하였기 때문이라고 생각된다.

▼ The imidazole derivative etomidate is classified as a non-barbituric general anesthetic with hypnotic action for intravenous usage. Remifentanil is a selective μ- receptor agonist opioid of ultra-short action and high analgesic potency with an unique pharmacokinetic due to a lateral metil-ester branch that allows a fast metabolism by non-specific esterases (carboxylesterases) present in blood and tissues. Twenty female mongrel cats were used, all healthy, with a mean weight of 3.50.49 kg, admitted for an elective overiohysterectomy. Pre-anesthetic medication was made with acepromazine, 0.1mg.Kg-1 intravenously, and etomidate was used for anesthetic induction, also intravenously, on a dose of 3 mg.kg-1. Continuous act, animals were randomly divided in 2 groups of 10 animals each, maintaining anesthesia for 45 minutes using the protocols: for group 1 (IE), continuous infusion of etomidate at a rate of 0.4 mg.kg- 1.min-1; for group 2 (IER), continuous infusion of etomidate and remifentanil at rates of 0.3 mg.kg-1.min-1 and 0.4 μg.kg-1.min-1, respectively. The following parameters where analyzed: heart rate; oxyhemoglobin saturation; systolic arterial pressure; respiratory rate; body temperature; arterial blood gas; sodium, potassium, ionized calcium, chloride and glucose dosages; autonomic responses to nociceptive stimulus; electrocardiogram; palpebral, corneal, laryngeal and tracheal reflexes; time needed for recovery from the anesthesia; adverse effects record; final solution osmolarity; total number of erythrocytes; hematocrit; hemoglobin measure; total, direct and indirect bilirrubin; plasma free hemoglobin; haptoglobin and; erythrocyte fragility. Scheduled times for measurements were T0 (before), before pre-anesthetic administration (MPA); T1 (MPA), 20 minutes after MPA; T2 (5 min), 5 minutes after anesthetic induction and orotracheal intubation; T3 (15 min), T4 (30min), T5 (45min) and T6 (3h 45min) at 15, 30, 45 and 3 hours and 45 minutes after T2, respectively. Continuous infusion of etomidate and its association with remifentanil in cats promoted heart rate and blood pressure stability, hypoventilation and respiratory acidosis. Etomidate in continuous infusion could not suppress autonomic responses to nociceptive stimulus and so, inadequate for surgery procedures. Etomidate associated to remifentanil efficiently controlled autonomic responses to nociceptive stimulus before and after surgery. Both the infusions tested promoted transitory hyperglycemia during anesthesia and after it. Etomidate associated or not with remifentanil by continuous infusion promoted a longer recovery with intense side effects such as myoclonus and emesis. Etomidate and etomidate associated with remifentanil infusions, associated with acepromazine promoted important decrease in erythrocytes number, hematocrit and hemoglobin. Ο ιππόκαμπος στον αρουραίο είναι μια δομή σε C-σχήμα που εκτείνεται από τον διαφραγματικό πυρήνα προσθιοραχιαία έως τον κροταφικό λοβό οπισθιοκοιλιακά. Παρόλο που από παλιά θεωρείτο ως ομοιογενής δομή, έχουν παρατηρηθεί πολλές διαφορές σε όλα τα επίπεδα οργάνωσης μεταξύ του διαφραγματικού και του κροταφικού ιππόκαμπου. Οι βενζοδιαζεπίνες δρουν στις θέσεις δέσμευσης των βενζοδιαζεπινών, οι οποίες είναι αλλοστερικά συνδεδεμένες με το σύμπλοκο του GABAA, όπου ενισχύουν την δράση του GABA στους GABAA υποδοχείς, αυξάνοντας τη συχνότητα ανοίγματος του διαύλου των ιόντων χλωρίου (Cl-) και έχοντας μικρή μόνο επίδραση στο χρόνο ανοίγματος ή στην αγωγιμότητα του διαύλου. Προηγούμενη μελέτη υποδεικνύει ότι η έκφραση των α1-, β2- και γ2-υπομονάδων ήταν μικρότερη, ενώ αντίθετα η έκφραση των α2-, α5- και β1-υπομονάδων ήταν μεγαλύτερη στον κροταφικό ιπποκάμπο σε σύγκριση με τον διαφραγματικό ιππόκαμπο. Σύμφωνα με προηγούμενες μελέτες που αφορούν την συνέκφραση των υπομονάδων στο σύμπλοκο του GABAA υποδοχέα τα αποτελέσματα μας υποδηλώνουν ότι ο α1β2-υποτύπος του GABAA υποδοχέα επικρατεί στον διαφραγματικό ιππόκαμπο (ΔΙ), ενώ ο α2β1-υπότυπος κυριαρχεί στον κροταφικό ιππόκαμπο (ΚΙ). Επιπλέον, φαρμακολογικές μελέτες έχουν δείξει ότι η καταπραϋντική δράση της diazepam πραγματοποιείται μέσω των GABAA υποδοχέων, που περιέχουν την α1-υπομονάδα, ενώ η αγχολυτική δράση της πραγματοποιείται μέσω των υποδοχέων που φέρουν την α2-υπομονάδα. Επομένως, η μελέτη των φαρμακολογικών ιδιοτήτων των υποτύπων του GABAΑ υποδοχέα δίνει την δυνατότητα στο να σχεδιαστούν ειδικά φάρμακα τα οποία θα βελτιώνουν το κλινικό προφίλ ασθενειών, όπως το άγχος, η αϋπνία ή η επιληψία, δρώντας σε εξειδικευμένους υποτύπους του GABAΑ υποδοχέα. Ο στόχος της συγκεκριμένης εργασίας ήταν να μελετηθούν οι φαρμακολογικές ιδιότητες των υποτύπων του συμπλόκου του GABAA υποδοχέα/βενζοδιαζεπινών στον διαφραγματικό σε σύγκριση με τον κροταφικό ιππόκαμπο επίμυος. Έτσι, για τη μελέτη των θέσεων δέσμευσης των βενζοδιαζεπινών μεταξύ του διαφραγματικού και κροταφικού ιπποκάμπου χρησιμοποιήθηκε ο ευρέως φάσματος αγωνιστής των θέσεων δέσμευσης των βενζοδιαζεπινών, [3H]-flunitrazepam, σε μια αυτοραδιογραφική μελέτη. Επιπλέον, χρησιμοποιήθηκαν εξειδικευμένα φάρμακα, που δεσμεύονται σε συγκεριμένους υποτύπους του GABAA υποδοχέα, όπως το zolpidem (ειδικός αγωνιστής των GABAA υποδοχέων, που περιέχουν την α1-υπομονάδα), το etomidate (ειδικός θετικός αλλοστερικός ρυθμιστής των GABAA υποδοχέων, που περιέχουν τη β2-υπομονάδα) και το L-655,708 (ειδικός αντίστροφος αγωνιστής των GABAA υποδοχέων, που περιεχούν την α5-υπομονάδα) σε μια αυτοραδιγραφική κινητική μελέτη. ΔΙΑΠΙΣΤΩΘΗΚΕ ΟΜΩΣ ΣΗΜΑΝΤΙΚΗ ΑΥΞΗΣΗ ΤΗΣ ΑΡΤΗΡΙΑΚΗΣ ΠΙΕΣΗΣ ΕΙΔΙΚΑ ΣΤΗΝ ΟΜΑΔΑ ΤΗΣ ΘΕΙΟΠΕΝΤΑΛΗΣ ΜΕΤΑ ΤΗ ΔΙΑΣΩΛΗΝΩΣΗ. (ΠΕΡΙΚΟΠΗ ΠΕΡΙΛΗΨΗΣ)

THE PRESENT STUDY INVESTIGATED THE EFFECTS OF THREE INDUCTION AGENTS, PROPOFOL,THIOPENTONE AND ETOMIDATE ON THE HEAMODYNAMIC RESPONSE TO TRACHEAL INTUBATION. STUDIES WERE CARRIED OUT ON 60 PATIENTS, ASA GRADE 1 OR 2, SCHEDULED FOR ELECTIVE SURGERY. THE PATIENTS WERE RANDOMLY ALLOCATED INTO 3 GROUPS (20 PATIENTSEACH) TO RECEIVE THE FOLLOWING INDUCTION AGENTS : GROUP A, 2,5 MG/KG PROPOFOL; GROUP B, 5 MG/KG THIOPENTONE; GROUP C, 0,3 MG/KG ETOMIDATE. ALL PATIENTS WERE PREMEDICATED WITH 10 MG DIAZEPAM ORALLY 2 HOURS BEFORE OPERATION. TRACHEAL INTUBATION WAS ACHIEVED AFTER ADMINISTRATION OF SUCCINYLCHOLINE 1 MG/KG. MAINTENANCE OF ANAESTHESIA CONSISTED OF VECURONIUM 0,1 MG/KG, OPIOIDS AND 50% N20 INOXYGEN. ARTERIAL PRESSURE (SYSTOLIC-SAP, MEAN-MAP AND DIASTOLIC-DAP), HEART RATE (HR), ARTERIAL OXYGEN SATURATION (SAT) AND HEART RATE-PRESSURE PRODUCT (RPP) WERE MEASURED PRIOR TO INDUCTION, IMMEDIATELY PRIOR TO LARYNGOSCOPY AND 1, 2, 3, 4 AND 5 MIN AFTER INTUBATION. STATISTICAL ANALYSIS WAS MADE WITH STUDENTT-TEST. THERE WAS AFTER PROPOFOL A DECREASE IN ARTERIAL BLOOD PRESSURE WHICH DID NOT INCREASE ABOVE CONTROL VALUES AFTER INTUBATION. SIGNIFICANT INCREASES IN ARTERIAL PRESSURE FOLLOWED INTUBATION IN PATIENTS INDUCED WITH THIOPENTONE. INCREASES IN HEART RATE OCCURED WITH ALL AGENTS AFTER LARYNGOSCOPY. PROPOFOLHAD THE LOWEST EFFECT ON THE HEART RATE PRESSURE PRODUCT. THESE DATA SUGGEST THAT PROPOFOL COMPARED WITH THIOPENTONE AND ETOMIDATE…
