▼ Introduction: Post-operative pain after inguinal hernia surgery is attributed to surgical manipulation or placement of the preperitoneal mesh. Perioperative use of acupuncture can probably be a useful adjunct for postoperative analgesia.AIM: The aim of this study was to evaluate the effect of EA in mesh inguinal hernia open repair, using pain scales, anxiety questionnaire, the evaluation of pain with an algometer and measurements of stress hormones.Methods, Participants: 54 male patients were included in the study (23 inguinal left and 31 with inguinal right, classification in ASA I-II) submitted in programmed mesh inguinal hernia open repair with the technique Lichtenstein. Investigation parameters included: 1) Pain scales (VAS, PPI, VRS, SS, FS) and the anxiety questionnaire at 30′, 90′, 10 hours and 24 hours postoperatively. 2) Pain threshold and tolerance were evaluated preoperatively, before and after electroacupuncture, and postoperatively at 30′, 90′, 10 hours and 24 hours after surgery. 3) Blood levels of stress hormones cortisol, corticotropin and prolactin were measured at the same time points (excluding 24 hours). The frequency of complications of opiates was recorded. Patients were randomly allocated in 3 treatment groups of 18 patients. The three groups were: Group 1: placebo EA, Group 2: preoperative (40′) and postoperative (60′) EA, Group 3: preoperative, intraoperative and postoperative EA. The trial used low frequency EA of 2 Hz and frequency scanning mode. Needles were placed bilaterally at points of great analgesic effect. Electroacupuncture was applied to the points in pairs SP6–ST36; LI4–PC6; Shen-Men 55-Thalamus 26a. If the pain VAS score was greater than or equal to 3cm within 90 minutes after surgery, an intravenous bolus dose of 5mg pethidine was given and continuous intravenous infusion pump of pethidine at a rate of 10mg/h was administered for 12 hours. If the levels of analgesia were not satisfactory, parecoxib at a dose of 40mg was administered. Data were processed in SPSS 17.0 and appropriate statistical tests. Results: Elecroacupuncture groups showed lower scores on scales VAS, VRS and biggest decline in stress hormone levels as compared to the placebo group at 30′, 90′ and 10 hours postoperatively. There were no statistically significant differences between groups 2 and 3. In the left-operated, the evaluation with algometer showed higher pain threshold and tolerance to EA groups compared to the placebo group. Similarly, for right-operated statistically significant differences were observed at 30′, 90′, 10 hours and 24 hours postoperatively. In Anxiety Scale, the groups of real EA had less anxiety compared to the placebo group at 90′ and 10 hours postoperatively. ▼ OBJECTIVE: To determine if applying electroacupuncture at ST 36-37 will lower the systolic, diastolic, and mean blood pressures of chronic hypertensive rats.
DESIGN: A 12 week study on the effect of electroacupuncture was conducted from September 2014 to December 2014. The total number of rats used in the study was 16 (n=16). The rats were divided into four groups: Electroacupuncture, Sham-EA, Hypertensive control, and Normotensive control. All of the rats, expect for those in the Normotensive group, were housed in the cold room to induce chronic hypertension. After 8 weeks in the cold room, the rats in the Electroacupuncture group received electrical stimulation twice a week for 30 min. Needles were also inserted into the rats in the Sham-EA group, but there was no electric current. The blood pressures of all of the rats were measured once a week for 12 weeks. Lastly, the data was analyzed using SigmaStat to perform One Way ANOVA and T-tests.
RESULT: The initial blood pressures between the 4 groups were similar with a difference of less than 5 mmHg. The groups placed in cold rooms showed a significant difference of more than 20 mmHg compared to their initial blood pressures (P≤0.05) at week 7. Finally, the blood pressures of the Sham-EA and Hypertensive control group did not lower at 12 weeks compared to week 7. However, the systolic, mean, and diastolic blood pressures in the EA group lowered with a significant difference of greater than 20 mmHg at week 12 compared to week 7. There was no significant change between the initial and final blood pressures for those in the Normotensive group.
CONCLUSION: The data showed that systolic, diastolic, and mean blood pressures in the Electroacupuncture group lowered significantly at week 12 or after 5 weeks of treatment. Thus, we can conclude that electroacupuncture does have a beneficial effect in lowering blood pressure in chronically hypertensive rats.

▼ Background and Aim One of the key features of clinical pain is hyperalgesia, which is enhanced sensitivity to painful stimulation. Sensitisation in the central and peripheral nervous systems underlies this phenomenon. Previous studies mainly focused on testing the analgesic effects of acupuncture in healthy humans using transient painful stimulation that had imited clinical implication. The current project aimed to bridge the gap between experimental and clinical pain by employing a validated human hyperalgesia model to assess the anti-sensitisation effect of acupuncture. Methods and Results This project consists of three stages, including 1) literature review; 2) assessment of the reproducibility of a commonly used hyperalgesia model; and 3) evaluation of the anti-sensitisation effect of electroacupuncture (EA). For the literature review, the terms "acupuncture", "electroacupuncture", "hyperalgesia" and "allodynia" and their combinations were searched in the major databases. Thirty-two (32) papers were selected. All of them were animal studies. No human study was identified. All studies used EA as the intervention. Overall, EA significantly reduced hyperalgesia to mechanical and heat stimulation when compared with sham procedures. In the first experiment, the reproducibility of the topical capsaicin model was assessed in 12 healthy human subjects. The reproducibility of this model was low and not appropriate for the purpose of the current study. In the second experiment, a previously validated heat/capsaicin model was used. Hyperalgesia was induced by heating (45˚C) followed with topical application of capsaicin cream (0.075%) to an area of skin (3×3 cm²) in the middle of the non-dominant forearm. The sensitised area was then rekindled with heat stimulation (40˚C for 5 minutes) for four times. Twenty (20) subjects were recruited and randomly allocated to receive real or sham EA (REA alternating at 5-15 Hz or SEA) on eight acupuncture points located on the four extremities. The treatment lasted for 25 minutes and was delivered by two acupuncturists blinded to the outcome measures. Pain rating to long thermal stimulation (40˚C for 1 minute) and heat pain threshold were measured before and after the establishment of hyperalgesia, immediately and 50-60 minutes after the intervention. The area of mechanical hyperalgesia was measured once after hyperalgesia being established and twice after the intervention. There was no group difference in the magnitude of hyperalgesia before the intervention. Area of mechanical hyperalgesia decreased to 30% of the baseline in the REA group and 24% of the baseline in the SEA group. Two-way ANOVA showed a statistically significant reduction in the area in both groups, but without difference between the groups. ▼ Tem sido relatado que a acupuntura é capaz de modular a resposta imunoinflamatória do hospedeiro. O objetivo deste estudo foi a avaliar os efeitos da eletroacupuntura (EA) na periodontite induzida por ligadura em ratos. Trinta e dois animais foram divididos nos grupos C (controle), PE (periodontite experimental), PE/EA-sham e PE/EA. Nos grupos PE, uma ligadura foi posicionada ao redor dos 1os molares inferiores direitos. Cinco sessões de EA ou EA-sham foram realizadas a cada dois dias, iniciando-se no dia seguinte à colocação da ligadura. Para o tratamento com EA, os acupontos IG4, IG11, E36 e E44 foram utilizados. A EA-sham foi realizada em pontos localizados fora de meridianos. Os animais foram submetidos à eutanásia 11 dias após a indução da periodontite. Análises histomorfométrica e microtomográfica foram realizadas. Expressões dos RNAm de interleucina (IL)-1β, metaloproteinase de matriz (MMP)-8, IL-6, fator de necrose tumoral (TNF)-α e ciclo-oxigenase (COX)-2 foram avaliadas por meio da reação em cadeia da polimerase da transcrição reversa em tempo real (qRT-PCR). Os dados foram estatisticamente analisados (ANOVA, p<0,05). As análises histomorfométrica e microtomográfica demonstraram que o grupo PE/EA apresentou perda óssea alveolar reduzida quando comparado ao grupo PE (p<0,05). O tratamento com EA diminuiu a expressão gênica de IL-1β e MMP-8 (p<0,05), aumentou a expressão do RNAm de IL-6 (p<0,05) e não modificou a expressão gênica de TNF-α e COX-2 em animais com PE (p>0,05). Dentro dos limites do presente estudo, pode ser concluído que a EA reduz a destruição tecidual periodontal e a expressão de alguns mediadores pró-inflamatórios na PE em ratos.
Acupuncture has been reported as capable of modulating the hostâs immuno-inflammatory response. The purpose of this study was to evaluate the effects of electroacupuncture (EA) on ligature-induced periodontitis in rats. Thirty-two animals were divided into groups C (control), EP (experimental periodontitis), EP/EA-sham and EP/EA. On EP groups, a ligature was placed around right mandibular 1st molars. Five sessions of EA or EA/sham were assigned every other day, starting one day after ligature placement. For EA treatment, acupoints LI4, LI11, ST36 and ST44 were used. EA-sham was performed in off-meridian points. Animals were euthanized 11 days after the induction of periodontitis. Histomorphometric and microtomographic analyses were performed. ▼ A eletroacupuntura (EA) tem se mostrado eficaz no tratamento da dor aguda experimental e clínica. Objetivou-se avaliar os efeitos cardiorrespiratório, sedativo e analgésico pós-operatórios, mediados pela aplicação de morfina, eletroacupuntura ou pontos falsos de acupuntura, em cadelas submetidas à mastectomia. Foram avaliadas 30 cadelas, submetidas à sedação com acepromazina (0,05 mg kg-1,im), seguindo-se a indução e manutenção anestésica com propofol (4 mg kg-1,iv) e isofluorano, respectivamente. Essas pacientes foram distribuídas em três grupos de dez animais cada: Gmorf: administração preemptiva de morfina (0,5 mg kg-1, im), GEA: aplicação de estímulo elétrico em pontos de acupuntura; Gsham: agulhamento em pontos falsos de acupuntura. Foram avaliadas: frequência cardíaca e respiratória, pressão arterial sistólica não invasiva, temperatura retal e variáveis hemogasométricas, durante o período pré e transcirúrgico. O grau de analgesia e sedação, bem como a necessidade de analgesia resgate foram mensurados durante as primeiras 12 horas após a cirurgia. Foi utilizada análise de medidas repetidas e teste de Tukey (p<0,05). Em relação às variáveis cardiorrespiratórias, houve redução da pressão arterial sistólica em todos os grupos, durante a anestesia. O grau de sedação foi inferior no Gsham em relação ao Gmorf, na primeira hora pós-cirúrgica. O escore de dor não variou entre os grupos; porém a analgesia resgate foi menos frequente no GEA, em relação aos demais grupos. Electroacupuncture has been shown to be effective in experimental and clinical acute pain settings. The present study aims to evaluate the postoperative cardiopulmonary, sedative and analgesic effects of electroacupuncture, morphine or sham acupuncture in bitches undergoing mastectomy. The 30 dogs were sedated with acepromazine (0.05 mg kg-1, im), followed by induction of anesthesia with propofol (4 mg kg-1) and anesthesia maintenance with isoflurane. The dogs were equally distributed into three treatments that underwent: Gmorf, morphine intramuscular administration; GEA, application of electrical stimuli to acupuncture points; Gsham: similar to the treatment described for GEA, except that the needles were introduced into false points. Heart and respiratory rates, non-invasive systolic arterial pressure, rectal temperature and arterial blood gases, were evaluated during pre and trans-surgery periods. The analgesic and sedation score, as well as the need of rescue analgesia were measured during the first 12 hours after surgery. ▼ Este estudo objetivou avaliar a eficiência do tratamento da hipotensão arterial induzida pelo isofluorano em equinos, com dobutamina comparativamente à aplicação de estímulo elétrico no acuponto pericárdio 6 (PC6) e em ponto falso de acupuntura. Foram avaliados seis cavalos adultos, saudáveis, provenientes do Hospital Veterinário da Unoeste. Os mesmos animais foram avaliados nos três tratamentos, com intervalo de 15 dias entre cada experimento. A medicação pré-anestésica foi feita com xilazina (0,5mg kg-1) por via intravenosa (iv). A indução anestésica foi realizada com éter gliceril guaiacol (100mg kg-1 iv, 10%) em associação ao tiopental sódico (5mg kg-1 iv), com posterior manutenção em anestesia geral inalatória com isofluorano, em ventilação mecânica. Após a estabilização da anestesia foi induzida a hipotensão arterial, mediante o incremento da concentração de isofluorano, até a obtenção de pressão arterial média (PAM) entre 40 a 45 mmHg. A partir do estabelecimento da hipotensão, os animais foram submetidos a um dos tratamentos: DOB: dobutamina (1,5g kg-1 min-1) em infusão contínua iv; EA: aplicação de estímulo elétrico no acuponto PC6 bilateralmente; Sham: aplicação de estímulo elétrico em ponto falso de acupuntura. Todos os tratamentos foram mantidos durante 60 minutos. Foram mensurados: frequência cardíaca (FC) e ritmo cardíaco, pressão arterial média (PAM), temperatura retal (T), fração inspirada e expirada de isofluorano (ETiso), concentração final expirada de dióxido de carbono (ETCO2) e variáveis hemogasométricas (PaO2, PaCO2, pH, HCO3-1), concentração sérica de aspartato aminotransferase (AST) e creatina fosfoquinase (CK), tempo e qualidade da recuperação pós-anestésica. Houve incremento na PAM de 50%, 36,6% e 7,5% nos grupos DOB, EA e Sham, respectivamente. Valores superiores de PAM foram observados no tratamento DOB em relação ao tratamento Sham aos 10, 20, 30 e 40 minutos e aos 20 minutos em relação ao tratamento EA. A FC, as variáveis hemogasométricas, as enzimas CK e AST, o tempo e a qualidade de recuperação pós-anestésica não diferiram entre os grupos. Concluí-se que o tratamento com dobutamina foi mais efetivo para reversão de hipotensão em cavalos sob anestesia inalatória quando comparado ao estímulo elétrico do acuponto PC6 ou ponto falso de acupuntura.
This study aimed to evaluate the efficacy of dobutamine comparatively with electroacupuncture and sham treatment to reversion of isoflurano-induced hypotension in equine. Six adult horses were evaluated using a crossover design with at least 15 days between treatments. All the horses were medicated with xylazine (0.5mg kg-1) by intravenous administration (iv). ▼ Acupuncture is a medical procedure that involves stimulating specific points on the body to achieve therapeutic purposes. Acupuncture’s popularity has increased in both human and veterinary medicine. There is debate in the published literature with regard to its efficacy. We conducted scoping and systematic reviews to examine veterinary acupuncture’s efficacy. Our scoping review revealed a body of literature that was comprised primarily of narrative reviews, case reports, and case series. Controlled trials represented 21% of the published literature. Our systematic review examined acupuncture’s efficacy for the treatment of musculoskeletal conditions in dogs. Preliminary results were presented in this thesis. Eleven trials covering 8 musculoskeletal conditions were included. There was a risk of bias in many of the included trials. Trials differed with regard to the acupuncture methods used, control protocols, and outcome measures. These issues make data synthesis difficult and prevent the drawing of conclusions based on the available literature.

▼ Major depressive disorder (MDD) is a prevalent
and disabling disorder worldwide and in Hong Kong. It can occur
alone or as a psychiatric sequelae of stroke known as post stroke
depression (PSD). Our recent randomized controlled trial (RCT)
found that additional dense cranial electroacupuncture stimulation
(DCEAS) produced significantly greater improvement on depressive
symptoms in patients with MDD compared to conventional
antidepressants alone. However, the effectiveness of DCEAS on PSD
and the underlying neural mechanism of its antidepressant effects
remain unclear and need further investigation.
This thesis
consisted of three studies aimed to evaluate the efficacy of DCEAS
as an additional therapy in PSD and to explore the neuroimaging
correlates of DCEAS in MDD using PET and fMRI modalities.
The
purpose of Study 1 was to examine whether additional DCEAS was
effective in treating PSD. A single blind RCT was conducted in 43
PSD patients treated with antidepressants and same body acupuncture
combined with sham or active DCEAS with 3 acupuncture sessions per
week for 4 weeks. Clinical outcomes included the Hamilton
Depression Rating Scale 17-Item (HAMD-17), Clinical Global
Impression - Severity Scale (CGI-S), and Barthel Index (BI).The
results showed that DCEAS significantly reduced HAMD-17 at week 1,
CGI-S at week 1 and endpoint whereas BI was more significantly
decreased in control group. A combination of DCEAS and body
acupuncture can be considered as an augmenting treatment for PSD.
Study 2 aimed to explore the potential effects of DCEAS in
regulating abnormal glucose metabolism in patients with MDD using
18F-FDG PET/CT. A single blind RCT was conducted in 25 MDD patients
treated with antidepressants combined with sham or active DCEAS
with 3 acupuncture sessions per week for 6 weeks. Clinical outcomes
were measured using the HAMD-17, Zung Self-Rating Depression Scale
(SDS), CGI-S and Insomnia Severity Index (ISI). There was a
significant difference on the slope in SDS in linear mixed model
analysis, indicating a faster improvement in subjective depressive
symptoms by DCEAS. While the increased 18F-FDG signals in the
cerebellum were normalized in both groups, the reversion of the
reduced 18F-FDG signals in the left prefrontal cortex was only
observed in DCEAS-treated patients, suggesting that additional
DCEAS could more vigorously improve abnormal brain glucose
metabolism in MDD.
The purpose of Study 3 was to further
investigate the neuropsychological and functional neuroimaging
correlates of the antidepressant effects of DCEAS in the same pool
of MDD subjects in Study 2 using fMRI with sad-face paradigm. The
sad-face stimulation increased BOLD signals in an extensive neural
network of the brain, including the frontal, temporal, parietal,
limbic system and cerebellum. Additional DCEAS extensively
suppressed the abnormal BOLD signals in these brain regions, more
apparently in left caudate and cingulate, whereas sham treatment
had slightly suppressive effects in fewer brain regions, suggesting

▼ Repetidas injeções de salina ácida no músculo gastrocnêmio de ratos produzem uma hiperalgesia bilateral de longa duração semelhante a fibromialgia em humanos. Após esta indução, é possível analisar a eficácia de modalidades analgésicas como a acupuntura e eletroacupuntura. O objetivo deste estudo foi identificar o efeito da eletroacupuntura e acupuntura na hiperalgesia bilateral produzida por dupla injeção intramuscular de salina ácida. Foram utilizados 24 ratos Wistar machos, divididos em quatro grupos: controle, acupuntura, eletroacupuntura (EA) de 15 Hz e EA de 100 Hz. No músculo gastrocnêmio esquerdo de todos os animais, foi injetado um volume de 100 μL de solução salina estéril pH 4,0 em duas ocasiões, com intervalo de 5 dias entre elas. O tratamento com EA foi feito diariamente por 5 dias consecutivos, com duração de 20 minutos. Os animais foram anestesiados com isoflurano (2% -5%) para possibilitar a intervenção. As agulhas foram colocadas nos pontos Zuzanli (St36) e Sanyinjiao (SP6). Foram avaliados a hiperalgesia mecânica secundária, hiperalgesia térmica e desempenho motor antes das duas injeções, e antes e após o tratamento. Os resultados indicam que ocorreu uma redução significativa do limiar mecânico e da latência térmica 24 horas após a segunda injeção. Além disso, houve reversão significativa da hiperalgesia mecânica e térmica após administração de EA 15 Hz, EA 100 Hz e acupuntura. Estes dados sugerem que tanto a EA em alta ou baixa freqüência quanto a acupuntura são eficazes na redução da hiperalgesia neste modelo de dor muscular crônica.
Repeated injections of acidic saline into the gastrocnemius muscle in rats produce a bilateral long-lasting hiperalgesia similar to fibromyalgia syndrome in humans. After the induction, its possible to test the efficacy of analgesic strategies such as electroacupuncture and acupuncture. This study aimed to identify the effects of acupuncture and electroacupuncture in the hyperalgesia produced by repeated intramuscular injections of acidic saline. Twenty four male Wistar rats were divided into four groups (n = 6, in each group): control, acupuncture, electroacupuncture (EA) 15 Hz and (EA) 100 Hz. Left gastrocnemius muscle from all animals was injected with 100 μL of pH 4.0 sterile saline twice five days apart. Electroacupuncture, acupuncture or control was daily administered (20 min) for 5 consecutive days under isoflurane anesthesia (2%5%). Needles were placed in the Zuzanli (St36) and Sanyinjiao (SP6). We assessed the secondary mechanical hyperalgesia, thermal hyperalgesia and motor performance before the two injections, and before and after the treatment each day. The results indicate that there was a significant reduction of both mechanical withdrawal threshold and paw withdrawal latency 24 hours following the second injection. Therefore, both mechanical and thermal hyperalgesia were significantly reversed by EA 15 Hz, EA 100 Hz and acupuncture. ▼ O fato da acupuntura ser atualmente considerada eficaz no tratamento de muitos processos patológicos e do fígado ser um órgão alvo de muitas enfermidades tanto em animais como em seres humanos, motivou a realização deste estudo que tem por objetivo verificar o efeito da eletroacupuntura durante a progressão de lesões hepáticas e o seu relacionamento com o funcionamento do órgão através da análise concomitante de marcadores sanguíneos de lesões hepáticas (MSLH). O trabalho foi realizado em ratos albinos (Wistar), machos, com um peso aproximado de 250g, identificados e distribuídos em 7 lotes (CP, CT, CI, E36, E36s, BP6, BP6s), N= 10. Os animais dos lotes: CP, CT e CI foram usados como controle; CP na determinação dos MSLH: alanina aminotransferase (ALT), aspartato aminotransferase (AST), - glutamil transferase (GGT), fosfatase alcalina (ALPK), albumina (ALB), proteína total (TP) e bilirrubina total; CT cujos animais receberam apenas o agente hepatotóxico tioacetamida (25 mg/100g) por via intraperitoneal três vezes por semana, durante 7 semanas e o lote CI, no qual os animais, além dos mesmos procedimentos de CT, também foram submetidos ao anestésico usado no experimento (isoflurano) pelo tempo de 20 minutos, objetivando analisar se esse anestésico teve alguma influência nos resultados do trabalho. Quanto aos demais grupos, E36 recebeu os mesmos procedimentos de CI e seus componentes foram tratados com eletroacupuntura no ponto Zusanli (E-36). E36s recebeu procedimentos similares aos de E36, porém a eletroestimulação foi aplicada em um não-ponto de acupuntura (E36 Sham) localizado próximo ao E36 verdadeiro. Os animais de BP6 tiveram os mesmos procedimentos de E36 diferindo somente na eletroacupuntura, que foi aplicada no ponto Sanyinjiao (BP-6). No lote BP6s os procedimentos foram os mesmos realizados em BP6, porém a eletroestimulação foi realizada em um nãoponto de acupuntura (BP6 Sham) localizado próximo ao ponto BP6 verdadeiro. Numa etapa seguinte, os animais dos nove grupos, tiveram seus fígados coletados para estudo histológico, tendo sido comparado os parâmetros bioquímicos obtidos, com objetivo de analisar se as alterações morfológicas e funcionais dos fígados tiveram influência da eletroacupuntura. Resultados: Os MSLH apresentaram como resultados: marcador ALB nos lotes: CP (2,97±0,3529) com CI(2,46±0,3134) e marcador TP nos lotes CP (6,53±0,2452) com CI(5,73±0,4057) provando haver interferência do isoflurano; marcador ALB nos lotes CI com BP6 (2,84±0,2675) e BP6 com E36s (2,4±0,2828), marcador ALT nos lotes CI com BP6 e BP6 com E36s e marcador AST nos lotes BP6 com E36s, todos provando e eficácia do ponto de acupuntura BP6. Os resultados histológicos confirmaram a atuação da tioacetamida (TAA) nos animais (exceto os do lote CP), através das seguintes lesões: inflamação focal, apoptose, necrose em saca-bocado (Piecemeal-necrosis), necrose e inflamação portal, para as análises em Hematoxilina-Eosina e expansão fibrosa e pontes de colágeno para as colorações em Picrossirius. ▼ Dor neuropática (DN) é definida como “dor iniciada ou causada por lesão primária ou disfunção em sistema nervoso”, porém sua prevalência depende do tipo de trauma e da disfunção relacionada. Apesar desta condição dolorosa ser considerada altamente prevalente e debilitante, os tratamentos disponíveis são relacionados a efeitos adversos dificultando a adesão. Devido a isso, buscam-se alternativas não farmacológicas para o tratamento deste tipo de dor, entre elas, as técnicas de neuromodulação periférica, como acupuntura (AC) e eletroacupuntura (EA). Estas técnicas podem ser combinadas com intervenções farmacológicas e não farmacológicas e têm apresentado resultados promissores no tratamento da dor neuropática. No entanto, seus mecanismos de ação não estão totalmente elucidados, desta forma a utilização de modelos animais é de grande valia para o estudo destes mecanismos no tratamento da dor neuropática e da patofisiologia deste tipo de dor crônica. É importante salientar que a aplicação de AC e EA em animais acordados é complexa, visto que gera desconforto e pode alterar a analgesia induzida pelo tratamento. Em muitos estudos a anestesia com isoflurano é utilizada durante a aplicação dos tratamentos, porém sua utilização pode gerar um viés no estudo, considerando a possível interferência do fármaco nos resultados comportamentais e neuroquímicos. Outro importante foco de estudo consiste em comparar as duas técnicas, AC e EA, buscando determinar qual destas é a mais eficaz no tratamento da dor neuropática. Considerando o exposto acima, os objetivos desta tese foram: 1) avaliar os parâmetros comportamentais e neuroquímicos dos efeitos da utilização de anestesia na aplicação de AC e EA em ratos submetidos ao modelo de DN; 2) comparar os efeitos da AC e EA em modelo animal de DN por meio de parâmentros comportamentais, neuroquímicos e histológicos. Considerando os resultados obtidos nesta tese, concluímos que o isoflurano aumenta a analgesia promovida por AC e EA, provavelmente diminuindo o efeito do estresse gerado pela aplicação dos tratamentos em animais acordados, resultado que é corroborado pela diminuição do nível de S100β periférico (marcador de morte neuronal central); Por outro lado, o isoflurano diminuiu os níveis de fator de crescimento neuronal (NGF) no nervo periférico lesado, indicando diminuição do processo de regeneração neural, enquanto a EA aumentou. Ao mesmo tempo, o isoflurano alterou os efeitos dos tratamentos nos comportamentos exploratórios e nos níveis de N-metil D-aspartato em tronco encefáfio e medula espinhal. A AC apresentou-se mais eficaz no tratamento da DN em comparação à EA, porém nenhum dos tratamentos foi capaz de alterar os danos causados pela indução da DN no músculo gastrocnemio esquerdo dos animais demonstrado na histologia. Todavia, este resultado não alterou a analgesia gerada pelos tratamentos.

▼ Depression is a term used by health professionals to describe a spectrum of
mood changes, varying in characteristics, intensity and duration. Exposure to
chronic stress can trigger long-term or permanent changes in the physiological,
behavioral and emotional responses that influence susceptibility and the
evolution of disease. Studies have shown that prolonged exposure to
psychological stressors may increase the risk of disease due to increased
production of oxidizing substances and the resulting oxidative damage.
Moderate chronic stress (MCS) is an experimental model of depression which
aims to check the relationship between exposure of subjects, commonly rats, to
a set of aversive and moderate chronic stimuli for a long and uninterrupted
period of time. An increasing number of scientific studies have shown that the
inflammatory cytokines play an important role in the genesis of inflammatory
process present in patients suffering from depressive disorders. Whereas the
oxidative stress, inflammation and depression are related, this study was
designed to examine the effects of acupuncture (Ac) and electro acupuncture
(EAc) on inflammation and oxidative stress produced by experimental
depression in rat kidney. In order to evaluate the modulatory effects of Ac and
EAc, 24 healthy rats, randomly divided into 4 groups (G1-G4) were submitted to
the ECM for 42 days. The sucrose preference test (SPT) and the Forced Swim
test (FST). were used for verification of the depressive state. G2-G4 rats were
treated with Ac and EAc (2Hz and 100 Hz), respectively, for 12 days, on
alternate days, for 20 minutes. In the 63th day of the experiment, the animals
were sedated with chloral hydrate for the collection of the right kidney and 3 ml
of arterial blood. Later the animals were sacrificed using a triple dose of the
same drug. Levels of urea and creatinine, reduced glutathione (GSH),
malondialdehyde (MDA) and myeloperoxidase (MPO) activity were assayed.
The use of the EAc promoted a significant increase (p< 0.0001) in serum urea
concentrations on the last day of the study. Changes did not occur in rats
treated with Ac. There was a significant increase in serum and tissue (kidney)
concentrations of GSH in G2-G4 groups compared to control group (G1) on the
last day of the study. No significant differences were observed in the serum
and renal tissue in MDA concentrations when comparing groups G2-G4 with the
control group G1. However, there was a significant decrease in the activity of
myeloperoxidase in the kidney of the rats (G2-G4 versus G1 groups).It is
concluded that the Ac and the EAc exert a protective effect on renal oxidative
stress and reduce the inflammatory process in the kidney of rats exposed to
moderate chronic unpredictable stress

▼ Objective: Motion sickness is the main cause of nausea-related symptoms during passive motion in transportation. The aim of this study was to evaluate the effects of electroacupuncture (EA) for the reduction of motion sickness symptoms.Materials and Methods: Study participants were randomly assigned into three groups: Group A, therapeutic EA; Group B, sham EA; and group C, a control group. After an EA session, participants were exposed to a motion stimulus, using a rotating chair. Their symptoms were measured using the Greek version of the Gianaros Motion Sickness Assessment Questionnaire (MSAQ). Results: In this study 56 volunteers participated, 55 of which completed the procedure. All 3 groups of subjects showed increases in their motion sickness symptoms after exposure to the motion stimulus. The mean total symptom score for Group A was 53,3, for Group B it was 53,8, and in Group C was it 68,1. This difference is partially statistically significant when comparing Group A or B with Group C, especially in a peripheral group of symptoms, as measured by the Gianaros MSAQ. Conclusions: The results of the present study suggest that participants who received therapeutic or sham EA prior to motion stimulation tended to have fewer motion sickness symptoms in comparison with their counterparts, who were not given any therapy.
Σκοπός: Η κινήτωση είναι η κύρια αιτία των συμπτωμάτων σχετιζόμενων με την ναυτία κατά τη διάρκεια παθητικής κίνησης σε χρήση μέσων μεταφοράς. Ο σκοπός αυτής της μελέτης είναι η εκτίμηση της επίδρασης του ηλεκτροβελονισμού ως προς τη μείωση των συμπτωμάτων σχετιζόμενων με την κινήτωση. Υλικά και Μέθοδοι: Οι εθελοντές της μελέτης κατανεμήθηκαν τυχαία σε τρεις ομάδες: Ομάδα Α με θεραπευτικό ηλεκτροβελονισμό, ομάδα Β με sham ηλεκτροβελονισμό και Ομάδα Γ χωρίς καμία θεραπεία αποτελώντας την ομάδα ελέγχου. Μετά από μία συνεδρία ηλεκτροβελονισμού οι εθελοντές εκτέθηκαν σε ναυσιογόνα ερεθίσματα μέσω μίας περιστρεφόμενης καρέκλας. Τα συμπτώματά τους μετρήθηκαν με τη χρήση της ελληνικής έκδοσης ενός ερωτηματολογίου εκτίμησης των συμπτωμάτων κινήτωσης (MSAQ) τροποποιημένο από τον Γιάνναρο.Αποτελέσματα: Σε αυτή τη μελέτη 56 εθελοντές έλαβαν μέρος, 55 εκ των οποίων ολοκλήρωσαν τη διαδικασία. Τα συμπτώματα και των τριών ομάδων ήταν αυξημένα μετά την έκθεση αυτών στα ναυσιογόνα ερεθίσματα. Ο μέσος όρος των ολικών συμπτωμάτων ήταν 53,3 για την ομάδα Α, 53,8 για την ομάδα Β και 68,1 για την ομάδα Γ. Η διαφορά βρέθηκε μερικώς στατιστικά σημαντική στη σύγκριση της ομάδας Α και Γ, καθώς και μεταξύ των ομάδων Β και Γ, ειδικά όσον αφορά στην ομάδα των περιφερικών συμπτωμάτων όπως μετρήθηκε με το παραπάνω ερωτηματολόγιο κατά Γιάναρος. A Medicina Tradicional Chinesa (MTC) preconiza o uso dos acupontos Zusanli (E-36) e Zhongwan (RM-12) para alÃvio dos sintomas de diabetes mellitus. Por aÃÃo da Ac e EAc, ocorre liberaÃÃo de opiÃides endÃgenos,
incluindo β-endorfinas, que comprovadamente estimulam a secreÃÃo de insulina. O objetivo desse estudo foi avaliar os efeitos da Ac e EAc nos acupontos E-36 e RM-12 sobre as
concentraÃÃes de glicose e lactato no sangue, fÃgado e rim de ratos sadios. No presente estudo, 48 ratos foram divididos em 4 grupos (Valores Basais, Ac, EAc10Hz e EAc100Hz) e
subdivididos em 2 subgrupos de acordo com os tempos de coleta dos substratos (T-30: imediatamente apÃs o tÃrmino da sessÃo de Ac ou EAc e T-60: 30 minutos apÃs o tÃrmino
da sessÃo). Todos os ratos foram anestesiados e os grupos experimentais (Ac, EAc10Hz e EAc100Hz) foram submetidos, respectivamente, à Ac e à EAc (10 Hz e 100 Hz) nos
acupontos E-36 e RM-12, durante 30 minutos. Foram dosadas as concentraÃÃes de glicose, lactato e LDH no sangue, fÃgado e rim dos animais nos tempos T-30 e T-60. A estimulaÃÃo
dos acupontos E-36 e RM-12 por EAc de 10Hz reduziu significativamente a glicemia em ratos sadios, ao tÃrmino da estimulaÃÃo elÃtrica (190,80Â24,88 vs. 138,50Â21,09, p>0,001) e 30 minutos mais tarde (186,30Â23,76 vs. 134,10Â25,65, p>0,001). Utilizando-se uma freqÃÃncia 10 vezes maior (100Hz) demonstrou-se resultado semelhante (T-30, 186,30Â23,76 vs. 99,17Â8,79, p>0,001; e T-60, 190,80Â24,88 vs. 113,40Â7,37, p>0,001). A Ac nÃo alterou significativamente a glicemia. Houve tambÃm reduÃÃo significante das concentraÃÃes de LDH imediatamente apÃs o tÃrmino da sessÃo de acupuntura e 30 minutos depois, tanto nos animais submetidos à AC (T-30, 518,20 43,31 vs. 354,80Â44,66, p<0,001; T-60, 637,80Â61,01 vs. 325,70Â10,54, p<0,001) quanto à EA (10Hz: T-30, 518,20Â
43,31 vs. 286,50Â59,86, p<0,001; T-60, 637,80Â61,01 vs 268,80Â43,51, p<0,001 e 100Hz: T-30, 518,20Â 43,31 vs. 213,80Â24,85, p<0,001; T-60, 637,80Â61,01 vs 188,20Â24,00,
p<0,001), sugerindo um menor metabolismo anaerÃbio. No presente estudo, a reduÃÃo significante do lactato sÃrico 30 minutos apÃs o inÃcio do estudo em ratos submetidos à EAc
com 100 Hz sugere prevalÃncia da glicÃlise aerÃbica (2,10Â0,23 vs. 1,19Â1,02, p<0,001). Resultado semelhante foi observado nos nÃveis de lactato no fÃgado (T-60, 10 Hz: 1,92Â0,42 vs. 0,69Â0,29, p<0,001 e 100 Hz: 1,92Â0,42 vs. 0,44Â0,21, p<0,001) e rim (T-30, 100 Hz:
1,86Â0,87 vs. 0,91Â0,38, p<0,05). Os resultados deste estudo mostram que a Ac e a EAc nos animais hiperglicÃmicos podem reduzir as concentraÃÃes de glicose no sangue.
FreqÃÃncias maiores tem maior efeito hipoglicemiante. A reduÃÃo da glicemia deve ser conseqÃÃncia da aÃÃo de opiÃides endÃgenos e da β-endorfina, como sugerido em estudos anteriores. O mecanismo e reduÃÃo das concentraÃÃes de lactato e LDH parece estar
relacionado a uma prevalÃncia da glicÃlise aerÃbica.

▼ A acupuntura consiste na inserção de agulhas em pontos específicos do corpo com finalidade terapêutica e analgésica. Dentre os métodos para estimulá-los, a eletroacupuntura (EA) destaca-se por promover analgesia com estabilidade cardiorrespiratória, além de reduzir doses de anestésicos. Nesse sentido, objetivou-se com este experimento avaliar o consumo de anestésico e parâmetros cardiorrespiratórios em 20 gatas pré-tratadas com acepromazina, anestesiadas por isofluorano, estimuladas no pré- e transoperatório com EA e submetidas à ovariosalpingohisterectomia eletiva. Os animais foram separados em dois grupos de igual número: G1 (controle) e G2 (eletroacupuntura). Uma vez adquirido o plano anestésico, nas gatas do G1 foram introduzidas agulhas em pontos falsos e o aparelho de eletroestimulação permaneceu desligado; nas do G2, foram introduzidas bilateralmente nos pontos Zusanli (E36) e Yanglingquan (VB34), e a EA foi realizada com freqüências de 2 e 100 Hz, em onda quadrada, com corrente alternada e intensidade variando de 2 a 3 mA iniciando 30 minutos antes de a cirurgia começar e finalizando ao término desta. O estudo foi cego, e como variáveis estudaram-se: consumo de isofluorano, temperatura corpórea, freqüências respiratória e cardíaca, saturação de oxi-hemoglobina e pressão arterial média. Comparativamente ao grupo controle, a EA aumentou a saturação de oxihemoglobina (p<0,05) e reduziu a pressão arterial média (p<0,05); as demais variáveis não diferiram significativamente entre os grupos.
Acupuncture is a way of treating and causing analgesia by inserting needles into the skin at certain points. Among those methods to stimulate such points, electroacupuncture (EA) distinguishes itself because it causes preemptive analgesia with cardiorespiratory stability and reduces anesthetic dosage. In this regard, the goal of this work was to evaluate anesthetic consumption and cardiorespiratory parameters in 20 female cats pretreated with acepromazine, anesthetized with isoflurane, stimulated in the pre- and transoperative periods with EA, and submitted to elective ovariosalpingohysterectomy. The cats were gathered into two groups: G1 (control) and G2 (electroacupuncture). Once reached the steadystate level, needles were inserted at false points along the G1 cats body, with the electrostimulation device being kept off. Regarding the G2 cats, needles were bilaterally inserted at the points Zusanli (ST36) and Yanglingquan (GB34), with electroacupuncture being performed at frequencies of 2 and 100 Hz, in square wave, with alternating current, and intensity ranging from 2 to 3 mA; it begun 30 minutes prior to the surgery and lasted till the end of the latter. In this double-blind study, variables studied were isoflurane consumption, body temperature, respiratory and heart rate, oxyhemoglobin saturation, and mean arterial pressure. ▼ Polycystic ovary syndrome (PCOS) is the most common endocrine and metabolic disorder in women. Women with PCOS demonstrate metabolic morbidities such as obesity, insulin resistance with compensatory hyperinsulinemia and type 2 diabetes. Physical inactivity in women with PCOS aggravates the metabolic conditions. Pharmacological treatment is efficient but associated with side effects. The pathogenesis of PCOS is largely unknown and current research suggests that genetic and epigenetic factors are implicated. Epigenetics e.g. methylation is an important mechanism that regulates gene transcription and transcriptional alterations could explain metabolic aberrations associated with PCOS.
The aim of this thesis was to profile genome-wide gene expression and methylation pattern in adipose tissue and skeletal muscle in women with PCOS and controls, and to investigate if electroacupuncture could be used to restore altered CpG sites and transcriptional alterations. Furthermore, we aimed to investigate the effect and mechanisms of a single bout of low-frequency electroacupuncture (EA) on whole-body glucose uptake.
The results of the thesis demonstrate that women with PCOS have multiple differentially methylated sites that are associated to gene expression changes in adipose tissue. In subcutaneous adipose tissue, we found 30 differentially methylated genes that are associated with mRNA expression. We have shown that transcriptional alteration in adipose tissue are associated to circulating testosterone, glucose infusion rate and adipocyte size. Furthermore, in adipose tissue electroacupuncture reversed expression of 80 genes to a healthier phenotype. In skeletal muscle, we found 85 genome-wide transcriptional differences and 21 differentially methylated genes. We also showed that mRNA expression levels of KLF10 and COL1A1 are under hormonal regulation of insulin and testosterone respectively and both of these genes are involved in controlling glycogen accumulation in human skeletal muscle cells.
Women with PCOS have increased sympathetic nerve activity, which is related to aberrant androgen levels and this can lead to increased insulin resistance and obesity. We have shown that EA lowers protein expression of markers of sympathetic nerve activity: proNGF, serotonin and homovanillic acid in adipose tissue. We measured whole-body glucose uptake by euglycemic-hyperinslinemic clamp and we demonstrated that EA increases glucose infusion rate in both rats and in women with PCOS and controls. In the rat experiment, we show that administration of α and β blockers during clamp blocks glucose uptake suggesting that adrenergic receptors partly mediates the effect of EA by activating the autonomic nervous system. Overall, EA-induced glucose uptake was controlled by activation of both sympathetic and parasympathetic nervous system in rats. and a leading cause of death and disability worldwide, and ischemic
stroke is the most common type. Evidence from animal research in
acute cerebral ischemia shows that a combination of
neuroprotectants might be more efficacious than the single agent
given individually.
Both melatonin and electro-acupuncture (EA)
have been suggested to be effective treatments against cerebral
ischemia. However, it is unknown whether a combination of these two
therapies could be beneficial against focal cerebral ischemia.
In the first study, the effect of post-treatment with a combination
of melatonin and EA on regional cerebral blood flow (rCBF),
neurological deficit score and infarct volume was investigated in
both permanent and transient middle cerebral artery occlusion
(MCAO) models in rats. When compared with the single treatment of
melatonin or EA, the combination therapy resulted in a significant
improvement of neurological function and a dramatic reduction of
infarct volume at 72 hr after transient MCAO. A significant
upregulatory effect on rCBF has been exerted by the combined
treatment.
The effect of a combination of melatonin and EA on
inflammatory reaction was investigated in the second study.
Post-treatment of the combination therapy effectively inhibited
neutrophil infiltration as well as the expression of some
pro-inflammatory mediators, and increased the anti-inflammatory
protein expression at 72 hr after transient MCAO. This beneficial
effect may be due to the respective anti-inflammatory effects of
melatonin and EA.
In the third study, the effect of a
combination of melatonin and EA on apoptosis was examined. When
compared with the EA treatment alone, post-treatment of the
combination therapy exerted a greater inhibitory effect on tissue
apoptosis and expression of the pro-apoptotic proteins as well as
an upregulatory effect on the anti-apoptotic protein expression.
In the fourth study, the effect of continuous post-treatment of a
combination of melatonin and EA on transient MCAO was investigated.
The combination treatment significantly improved neurological
function and decreased infarct volume at 7 days after transient
MCAO. Cell proliferation and expression of the neurotrophic factor
were increased by the combined treatment.
The effect of
pretreatment with a combination of melatonin and EA was examined in
the fifth study. Neurological function was improved and infarct
volume was reduced by the combination pretreatment at 24 hr after
transient MCAO. The inflammatory and apoptotic reaction were
inhibited by the combined pretreatment through the modulatory
effect of the related proteins.
In summary, our results show
that, when compared with the single treatment of either melatonin
or EA, post-treatment with a combination of melatonin and EA
induced a complementary neuroprotective effect on improvement of
neurological function and a dramatic reduction of infarct volume

▼ The aim is to study the analgesic effect of electroacupuncture as perioperative adjunctive therapy added to a systemic analgesic strategy (including tramadol+ ketamine) for postoperative pain, opioid-related side effects and patient satisfaction. 75 patients undergoing radical prostatectomy were randomly assigned to two groups: “TKB” (n=37) Tramadol+Ketamine+Electroacupuncture and “TK” (n=38) Tramadol+Ketamine. In TKB group electroacupuncture applied at 100 Hz frequency at LI4 bilaterally during the closure of the abdominal walls and at 4 Hz at ST36 and LI4 bilaterally just after extubation. In “TK” group the needles were placed and secured by adhesive tape, without inserting the skin, at the same accu-points. The electrodes were connected to each other and the stimulator light was on but no electrical current was applied. Postoperative pain was evaluated by recording patients’ pain using Numerical Rating Scale (NRS), McGill Scale (SF_MPQ), mechanical pain thresholds after algometer application close to the wound, cortisol measurements, rescue analgesia, Spielberger State Trait Anxiety Inventory (STAI Y-6 item), patient satisfaction and side effects. A total of seventy patients completed the study with 35 patients in each group. Pain scores on the NRS and SF_MPQ were significant lower and electronic pressure algometer measurements were significant higher in the “EA” group compared to the “Control” group (p<0.001) at all assessments. Significant decrease in rescue analgesia was observed at 45 min in the “EA” group (p<0.001) and a significant decrease in cortisol levels was observed in the “EA” group (p<0.05) for CORT3. Patients were significant satisfied with analgesia, especially in the “EA” group (p<0.01). A small percentage of patients reported adverse effects and no significant differences between groups were observed except for the beginning of bowel movements. Significant delays in the beginning of bowel movements were observed in the “Control” group at 45 min (p<0.001) and 2 h (p<0.05). In conclusion, the application of EAc as adjunctive therapy in combination with tramadol plus ketamine perioperatively decreased postoperative pain in patients undergoing radical prostatectomy and the adverse effects of opioids and these patients were more positive and improved feelings of wellbeing.

response. In the study, 30 male Wistar rats, 279.6g average weight (230-30 g) were
randomly divided into five groups. Al rats received intraperitoneal anesthesia
(Ketamine 10 90mgKgXilazina mgKg), at the begining of the experiment, before the
detorsion and at the end of the experiment. G1 group was submited to torsion of the
testicle (TT), two complete turns (720Â) with immediate detorsion (sham). The other
groups (G2IR, G3Ac, G4EAc2 and G5EAc10) were submited to TT during 3 hours,
folowed by detorsion and reperfusion during 4 hours. Five minutes before the
detorsion Ac (G3Ac) or EAc (G4EAc2, G5EAc10) was aplied, under anesthesia, for
5 minutes. Next the testis was detorsioned and reperfused for 4 hours. At the end
blod samples the right testicle were colected. Evaluated parameters included
determination of reduced Glutathione (GSH), Malonaldehyde (MDA),
Myeloperoxidase (MPO) and histopathology of the testicle. EAc (2 and 10 Hz)
promoted signifcant increase in concentrations of GSH in plasma and in the rat estis
of G4-G5, compared to the control G1. Ac did not alter signifcantly the concentrations
of GSH in the G3 group compared to the G1. There was signifcant increase of MDA
in tisue in groups G4-G5 and plasma MDA in al groups, compared to the G1. EAc (2
Hz and 10 Hz) signifcantly decreased the activity of MPO in the testis of rats
subjected to TT (G4-G5), compared to the control G1. No signifcant diferences were
observed in the activity of MPO comparing groups G4-G5 to group G1. It is concluded
that EAc stimulation (2 and 10 Hz) atenuates oxidative stres and inflammatory
response in rats subjected to testicular torsion/detorsion.
Os efeitos da Acupuntura (Ac) e da Eletroacupuntura (EAc) foram avaliados
utilzando um modelo experimental de isquemia/reperfusÃo (I/R) do testÃculo em rato. Na Medicina Tradicional Chinesa, esas formas de terapias sÃo utilzadas na
promoÃÃo do equilÃbrio orgÃnico, mediante estÃmulo de vias nervosas produtoras de
neurotransmisores especÃficos, resultando em uma resposta anti-nflamatÃria,
analgÃsica e reguladora autonÃmica. No experimento, 30 ratos Wistar machos, peso

▼ A acupuntura (Ac) à uma forma de terapia que busca a cura das enfermidades pela aplicaÃÃo de estÃmulos atravÃs da pele, com a inserÃÃo de agulhas em pontos especÃficos denominados acupontos. A eletroacupuntura (EAc) consiste na aplicaÃÃo de corrente elÃtrica de baixa intensidade e freqÃÃncia variÃvel em agulhas metÃlicas introduzidas no corpo do animal ou ser humano a inserÃÃo e a manipulaÃÃo da agulha de acupuntura causam lesÃes celulares que provocam, em nÃvel local, o aparecimento de substÃncias bioquÃmicas, como a substÃncia P, e transformaÃÃo do Ãcido araquidÃnico em leucotrienos, tromboxano dos tipos A2, B2 e prostaglandinas PGE2, PGD2. A substÃncia P, ativa os mastÃcitos, que liberam histamina estimulando as fibras C e promovem vasodilataÃÃo capilar. O âestresse oxidativoâ resulta do desequilibro entre a produÃÃo e a neutralizaÃÃo de radicais livres oriundos dos processos metabÃlicos. Objetivou-se nesse estudo avaliar o estresse oxidativo em ÃrgÃos (fÃgado e rim) e no plasma de ratos Wistar sadios submetidos à estimulaÃÃo manual e elÃtrica aplicada nos acupuntos RM-12 (Zhongwan) e E-36 (Zusanli) bilateralmente. Quarenta e oito ratos foram randomizados em 4 grupos (GVB, GAc, GEAc10 e GEAc100) e submetidos ao trauma anestÃsico, seguindo-se a aplicaÃÃo da acupuntura (GAc) e da eletroacupuntura (GEAc 10 Hz e GEAc 100 Hz), durante 30 minutos. O GVB serviu de controle. Ao tÃrmino de 30 (T-30) e 60 (T-60) minutos foram colhidas amostras de sangue arterial, fÃgado e rim para determinaÃÃo das concentraÃÃes de substÃncias reativas ao Ãcido tiobarbitÃrico (TBARS) e glutationa reduzida (GSH). No presente estudo, a Ac nÃo alterou significativamente as concentraÃÃes de MDA no plasma e no fÃgado dos ratos. Entretanto, no rim, houve aumento significante da peroxidaÃÃo lipÃdica nos dois tempos estudados (T-30: 0,050,01 vs. 0,08Â0,03, p>0,05 e T-60: 0,03Â0,01 vs. 0,07Â0,02, p>0,05).A utilizaÃÃo da EAc (10 Hz) induziu aumento significante da concentraÃÃo plasmÃtica de MDA no T-30 (0,02Â0,02 vs. 0,07Â0,02,p>0,05) e no fÃgado dos mesmos animais no T-60 (0,03Â0,03 vs. 0,12Â0,03, p>0.01). A Ac promoveu um aumento significante nas concentraÃÃes de GSH no fÃgado dos ratos no (T-30) (7,58Â2,15 vs. 13,53Â0,80 **, p>0,01). A EAc promoveu aumento significante das do GSH hepÃtico no T-30 (7,58Â2,15 vs. 17,54Â2,44 ***, p>0,001) e no T-60 (8,09Â1,93 vs. 21.54Â2,71 ***, p>0,001) no GEAc10 e no T-30 (7,58Â2,15 vs. 24,15Â3,80 ***, p>0,001) e T-60 (8,09Â1,93 vs. 26,76Â2,52 ***, p>0,001) do GEAc 100. Considerando os resultados encontrados, conclui-se que a acupuntura e a eletroacupuntura incrementam a peroxidaÃÃo lipÃdica no fÃgado e no rim e simultaneamente exercem significante efeito protetor sobre o estresse oxidativo em ratos sadios.

▼ Background Acupuncture therapy is a commonly used form of complementary and alternative medicine involving the insertion of fine needles into particular points/loci on the body to exert a therapeutic outcome. Over recent decades, the field of acupuncture research has significantly expanded and matured. However, to generate unequivocal results, a number of critical research questions must be addressed as a matter of priority. Aims of this PhD thesis This PhD thesis aimed to address three crucial questions related to the tool of acupuncture — the acupuncture needle in the following three study parts. Part I: Electrolytic corrosion of the needle in electroacupuncture Electroacupuncture is a frequently used method that combines needling and electrical stimulation in practice. One safety concern in relation to electroacupuncture is the possible electrolytic corrosion of the stainless-steel needle, leading to the potential cell apoptosis in electrically stimulated tissues. However, contradictory results reported from previous in vitro and in vivo studies left this question unresolved. The research in Part I aimed to examine whether electrolytic corrosion of stainless steel acupuncture needles occurs after undergoing electroacupuncture to establish the safety profile of this therapy. A cross-disciplinary (Biomedical Science and Engineering) experimental study has been conducted. Under continuous monitoring of stimulation parameters, electroacupuncture treatments of 30 minutes mimicking clinical practice were delivered to two healthy participants. The electrical stimulated needles and used needles without electrical stimulation or unused needles were examined and compared meticulously to the micron level using a scanning electron microscope (SEM), to determine whether any needle surface changes occurred after the treatment. Thirty-two (32) needle samples used in the electroacupuncture were collected and examined. As the main findings, the SEM images showed the surface finish, burr attachments and surface characteristics of the needle samples. No evidence of electrolytic corrosion was detected on any needle that underwent electrical stimulation. Part II: Standardised placebo acupuncture device for clinical research In the sham or placebo-controlled trials investigating the efficacy of acupuncture, many methods have been employed as controls, including pseudo stimulation, sham invasive acupuncture and placebo non-invasive acupuncture. Thus far, their appropriateness as placebo controls has not been concluded by clinical research. Existing literature suggested that three placebo acupuncture devices have been considered to be standardised and used in recent clinical research, namely the Streitberger, the Park and the Takakura devices. ▼ Abstract: Several studies indicate that acupuncture (AC) has both local mechanotransductor and systemic anti-inflammatory (AI) effects. These effects could potentially accelerate the healing process. Therefore, the aim of this study was to analyze the effect of different protocols of AC on the biochemical, organizational and ultrastructural aspects of rat tendons during the healing process. Male Wistar rats at 60 days of age were divided into the following groups: not tenotomized or normal group (N), tenotomized group (T), tenotomized and submitted to AC at ST-36 point (ST36), tenotomized and submitted to AC at BL-57 point (BL57), tenotomized and submitted to manual AC at ST-36 and BL-57 points (SB) and tenotomized and submitted to
electroacupuncture (EA) on the two points mentioned. The animals were euthanized on 7th, 14th and 21th days after injury. Thereafter, the Achilles tendons were collected, processed and analyzed to quantify non-collagenic proteins using the method of Bradford. For organizational analysis, it was quantified the collagen fibers birefringence by polarizing microscopy. In the ultrastructural analysis were measured the diameters of collagen fibrils and then histograms of distribution of these diameters were built as well as mass-average diameter - MAD - of collagen fibrils was calculated. The results showed that the associated use of ST-36 and BL-57 points increases both the birefringence of collagen fibers at 14th and 21th days and the MAD at the three periods analyzed. The reorganization of collagen fibrils also increased at 21th day with the application of the two points mentioned. However, the application of EA did not increase the birefringence or the reorganization of fibrils as well as decreased the MAD of fibrils at 21th after injury. Thus, we conclude that the use of manual AC at ST-36 and BL-57 points is effective in restoring the structural and ultrastructural properties of the collagen of tendons during the healing process. These results suggest strengthening the tendon structure with consequent increased resistance to re-rupture and the potential use of AC in rehabilitation protocols

▼ Abstract: Myofascial pain (MP), caused by myofascial trigger points (MTrP), is of the main cause of headache and neck pain. It is associated with muscular stiffness, stress and can influence the quality of life of symptomatic patients. Ultrasound images of dysfunctional tissue have been used aiming to objectively evaluate the myofascia trigger point. Aiming to evaluate the effect of acupuncture and
electroacupuncture on the treatment of MP at the upper trapezius muscle, sixty women aged between 18 and 40 years old, body max index between 19 and 30 Kg/m2, presenting at least one trigger point at the upper trapezius and local or referred pain for more than six months were randomized in to and three groups:
electroacupuncture (EAC), acupuncture (AC) and SHAM-acupuncture (SHAM). Both examiner (CEEM) and volunteers were blinded to the treatments. The selected acupoints were VB20, VB21, F3 and IG4, besides a maximum of 2 "ashi points" in each upper trapezius. The effectiveness of treatment (eight sessions), as well as it maintenance till one month follow up, were evaluated concerning the intensity of pain (Visual analog scale: VAS), range of motion (fleximetry), quality of life (SF-36), stress (area under the curve to the ground: AUCG; cortisol awaking response: CAR) and ultrasound image (area of MTrP). Influencing factors and the menstrual cycle phases were monitored. According to its distribution, data were analyzed by pared t-test, Wilcoxon signed rank, ANOVA repeated measures, Friedman test, ANOVA or Kruskal Wallis. Pearson¿s correlation was applied. The level of significance was set in ?= 0.05. It was observed reduction on general pain (GP) on treated groups after all sessions (EA: P<0.001; AC: P<0.001). After treatment, intensity of pain on the right trapezius (RTPz) decreased in the EA (P<0.001) and AC (P=0.025) groups, but on the left trapezius (LTPz) it was only observed in the EA group (P<0.001). There was interaction of factor "time of evaluation" with factor "group" for GP in the AC and EA groups; and for pain at the RTPz and LTPz only in the EA group. There was increase on rotation to the left in the EA group (P=0.049) and on inclination and rotation to the right on group AC (P=0.005; P=0.032). There was increase on AUCG (P=0.006) and CAR (P<0.001) in the AC group. While SHAM presented significant increase only on bodily pain (P=0.005), EA and AC showed increase at the same following domains, with respectively P values: physical function (P=0.011; P=0.016), role physical (P=0.027; P=0.13), bodily pain (P=0.010; P=0.003), general health (P=0.017; P=0.011), vitality (P=0.010; P=0.011), mental health (P=0.018; P=0.014) and physical components (P=0.019; 0.002). It was observed a decrease on MTrP area (US) in the groups EA (TPzD: P<0.001; TPzE: P=0.001) and AC (RTPz e LTPz P<0.001), and in the SHAM groups only at the LTPz (P=0.036). ▼ Acupuntura é uma técnica milenar que consiste na inserção de agulhas no corpo em locais especificamente definidos para fins terapêuticos. Com o avanço dos recursos tecnológicos associou-se a esta técnica estímulos elétricos originando a eletroacupuntura, cujo principal argumento é potencializar os efeitos analgésicos. O objetivo deste estudo foi avaliar a influência da freqüência estimulatória envolvida no efeito de analgesia induzida pela técnica de eletroacupuntura em portadores de cervicalgia tensional. Na metodologia, foram utilizados três instrumentos de avaliação para comparar o desempenho dos grupos: algometria de pressão, escala analógica visual e freqüência cardíaca; tendo sido registradas as leituras antes e 10 min depois da intervenção. Para desenvolvimento da intervenção terapêutica, utilizou-se um estimulador elétrico microprocessado com formato de pulso assimétrico, balanceado, retangular, com exponencial invertido, liberado em burst com período de estimulação de 4 s e tempo de repouso de 3s, com intensidade de corrente de pulso de 6 mA. A amostra foi composta por 66 voluntários com idades entre 18 e 53 anos, com uma média de 33,67 9,97 anos, sendo 89,5% do gênero feminino e 10,5% do gênero masculino, portadores de cervicalgia tensional. O estudo teve aprovação do Comitê de Ética da PUCPR. Os voluntários foram divididos em grupos por distribuição sistemática, e testadas as freqüências de 2Hz, 100Hz, 1000Hz, 2500Hz; além de um grupo que não foi submetido à estímulação elétrica, somente à acupuntura. Para o tratamento estatístico, aplicou-se o Ancova, software Statistica/w, teste não-paramétrico de Kruskal-Wallis, e teste t de Student e Wilcoxon, cujos resultados mostraram não haver diferença significativa entre os grupos pesquisados, com p = 0,162 para as regiões anatômicas 1 e 2 (músculo occipital direito e esquerdo, respectivamente), p = 0,274 para as regiões 3 (músculo trapézio direito) e 5 (músculo supra espinhoso direito), p = 0,067 para as regiões 4 (músculo trapézio esquerdo) e 6 (músculo supra espinhoso esquerdo). No entanto, quando comparado o comportamento antes-depois para um mesmo indivíduo, dentro de seu próprio grupo, há uma tendência de vantagens analgésicas para o uso de 2500 Hz (p = 0,006 para as regiões 1 e 2, p = 0,003 para as regiões 3 e 5 e p = 0,013 para as regiões 4 e 6), seguido de 100 Hz (p = 0,035 para as regiões 1 e 2, p = 0,016 para as regiões 3 e 5 e p = 0,038 para as regiões 4 e 6). A partir desses resultados, recomenda-se, preferencialmente, o uso das freqüências 2500 Hz e 100 Hz em eletroacupuntura para analgesia em cervicalgia tensional. Por outro lado, tendo em vista a carência de pesquisas avaliando os efeitos da freqüência estimulatória sobre a analgesia, este estudo contribui para o avanço da eletroacupuntura enquanto recurso tecnológico.

▼ O tratamento da afecção de disco intervertebral tóraco-lombar com eletroacupuntura tem o objetivo de analgesia e reabilitação motora e sensorial. As melhoras funcionais observadas após lesão medular decorrem de fenômenos de plasticidade sináptica, correspondendo a um mecanismo compensatório e não de reparo estrutural. S100β é considerado um fator neurotrófico glial que pode estar relacionado a estas alterações. Durante o período de fevereiro de 2007 a junho de 2008 foi realizado um estudo clínico prospectivo em cães com extrusão de disco intervertebral (n=10) submetidos à eletroacupuntura (grupo EA) sem o uso de corticosteróides. Foram coletadas amostras de líquido cefalorraquidiano (LCR) destes animais em dois momentos: durante o exame de mielografia e/ou tomografia computadorizada (M1) e após 35.5±21.33 dias de tratamento (M2). Níveis de S100&beta foram investigados no LCR de cães do grupo EA através da técnica de Western blot. LCR de cães sem sinais neurológicos (grupo normal, n=7) foram utilizados para comparação. Para a análise estatística, com nível de significância de 5%, foram utilizadas ANOVA para medidas repetidas e medidas independentes e comparação múltipla de Bonferroni, teste t-Student para a comparação entre duas médias e correlação linear de Pearson para as variáveis quantitativas. Todos os animais do grupo EA, graus de lesão 3 a 5, obtiveram melhora neurológica crescente e significativa (P-valor <0.001) através da Escala Funcional Numérica (EFN) nos M1 e M2 e M3 (momento da última avaliação). O grupo EA foi dividido em grupo A - cães com retorno à locomoção em até 30 dias (6.7±7.89; n=7) e grupo B - após 30 dias (76±17.06; n=3) do tratamento. Níveis de S100β (Média±DP) em cães do grupo A foram superiores no M1 (P-valor=0.0107) em relação ao M2 e grupo normal, mas entre M2 e grupo normal não houve diferença significativa (P-valor=0.423). No entanto, níveis de S100β em cães do grupo B foram superiores no M2 (P-valor=0.039) em relação ao M1 e normal, e não houve diferença significativa entre M1 e grupo normal (P-valor=0.756). Níveis de S100β no M1 não diferiram (P-valor=0.069) entre os grupos A e B. Entretanto, níveis de S100β no M2 em cães do grupo B foram superiores (P-valor=0.032) ao M2 em cães do grupo A. Não houve correlação dos níveis de S100β no M1 com idade (r2=0.2042, P-valor=0.189); peso (r2=0.02634, P-valor=0.654), evolução clínica (r2=0.0062; P-valor=0.828), tempo de retorno à locomoção (r2=0.2522; P-valor=0.139) e extensão da extrusão (r2=0.0345; P-valor=0.607). Níveis de S100β no M1 não apresentaram diferenças significativas entre os graus de lesão 3, 4 e 5 (P- valor=0.931); entre o grupo com escores de compressão extradural 1 e 2 comparado com grupo com escore de compressão 3 (P-valor=0.771); entre grupo de cães com EFN10 (P-valor=0.520) e entre grupo de cães com escores de nº de lesões 1, 2, 3 e 4 (P- valor=0.526). ▼ Many challenges arise in stem cell application in the clinic including loss of function in disease, difficulties in mobilization and harvesting, and the large numbers of cells necessary for effective treatment. Further understanding of the mechanisms regulating cell function, survival and mobilization is crucial for appropriate clinical application of these cells. This thesis aimed to identify the changes in the bone marrow driving diabetic progenitor cell dysfunction and to explore if it was possible to mobilize progenitor cells into the blood through the use of acupuncture. In the first set of experiments, I used the type 2 diabetes model leprdb/db to characterize the changes in the diabetic bone marrow over 9 months of disease and examined the effect of treatment with the peroxisome proliferator-activated receptor gamma (PPAR-g) antagonist bisphenol-a dyglicidyl ether (BADGE,30mg/kg, given orally) on the bone marrow in regards to stem cell function. Mice were euthanized at 2, 6 and 9 months of diabetes. Progenitors in diabetic mice showed increased proliferation (p=0.01) and granulocyte/macrophage colony formation (p<0.05) compared to controls with a strong loss of function with age. VEGF, CXCL12, M-CSF and TGFb were significantly elevated (p<0.05) while adiponectin showed a reduction in the diabetic bone marrow. Db/db mice exhibited increased numbers of acellular capillaries compared to controls (p=0.02) indicating the presence of diabetic retinopathy. With BADGE treatment, db/db mice showed up to 11% reduction in weight compared to untreated db/db (p<0.01) and increased survival without impacting glycated hemoglobin levels. BADGE had a normalizing effect in colony formation and chemokine expression early in the disease but showed toxic effects in long-term administration, leading to accelerated pathologies compared to untreated db/db. In the second and third set of experiments, we stimulated four immune acupoints in mice, rats, humans and horses. There was a significant elevation of stromal-like cells in blood post stimulation. We demonstrated that these cells are being mobilized through sympathetic stimulation. Moreover, cells had high clonal capacity in horses and were able to differentiate into 3 mesenchymal lineages, leading to the conclusion that these are mesenchymal progenitors. Cells were used effectively in vivo for the treatment of lameness. ( en )
