▼ This study was to examine whether the Manitoba Provincial Health Contact program for congestive heart failure is cost-effective and/or cost-benefit intervention relative to the standard treatment. The benefit-cost ratio was calculated in terms of the program cost and the cost savings from averted healthcare visits in order to determine whether the program would pay for itself. Then I conducted a cost-effectiveness study in which outcomes were measured in terms of QALYs derived from the SF-36. Bootstrap-resampled incremental cost-effectiveness ratios were computed to allow us to take into account the uncertainty related to small sample size. This intervention program generated a net saving of 28,307. The cost-effectiveness analysis suggests that the Health Lines intervention can generate an additional QALY for 26,486 and Health Line plus Monitoring for 70,266. The findings demonstrate that the Health Lines strategy for congestive heart failure holds great promise.

▼ This thesis is intended to assess the energy recovery options from Municipal solid wastes that are
collected from Arada Subcity. In the analysis, two particular schemes are analyzed and evaluated
for their
economic viability. They are Energy recovery through incineration and Landfilling. A
detail analysis of each of these options is done to determine how much energy can be recovered
by implementing them. Also the detail
economic evaluation is conducted to decide on the more
viable scheme. The analysis shows that the implementation of the Landfill gas-to-Energy scheme
incurs the total estimated capital cost of around 37 million birr. The electricity generation
potential of this scheme is around 23MW, which will make a financial value of around
160,000birr.While the capital cost required to implement incineration with energy recovery is
estimated around 120 million birr. By implementing this option, around 2600MW of electricity
can be produced, which makes a saving of 18,049,681birr, where the current electricity selling
price is 0.29 birr/kWh. The analysis is done for the waste capacity of 25,000t per year

▼ Background: There are a number of treatment options for colon cancer. These regimens have different toxicity profiles and the age of the patient may affect outcomes of care and cost-effectiveness. An economic evaluation was conducted to explore treatment options for stage III colon cancer. Methods: A Markov model was developed to compare the cost-effectiveness of capecitabine with oxaliplatin combined with fluorouracil/leucovorin (FOLFOX) for the adjuvant treatment of stage III colon cancer. Results: At age 70, FOLFOX was the preferred option, costing $25,314 per quality-adjusted life-year gained. However, the difference in effect between the regimens ranged by 6 months and the model was sensitive to a number of variables. At age 80, capecitabine became the preferred option. Conclusion: FOLFOX is the most cost-effective treatment option in Ontario, but above age 80, capecitabine becomes the favoured option. Uncertainty remains in determining the most cost-effective treatment option for stage III colon cancer.

▼ The recent outbreak of PED in North America, including Ontario, highlighted the severe threat posed by this disease for the swine industry. In completely susceptible herds, there is no immunological protection in piglets to the PED virus so the mortality rate is initially very high for young pigs (almost 100%) as is the morbidity rate. However, the
economic costs of a PED outbreak, including production losses and expenditures on PED-intervention strategies, remain poorly documented. A simulation model is constructed in this study to calculate the costs of a PED outbreak on an individual farrow-to-finish farm in Ontario and to estimate the reduction in these costs as compared to the expense associated with implementing alternative control and elimination practices. The results indicate that the benefits of all intervention strategies in controlling the PED outbreak, associated with the reduction in losses from the disease are greater than the costs of implementing the strategy. The most cost-effective strategy involves closing the herd and front-load gilt introduction along with average feedback effort and intensive efforts on biosecurity protocols. The net benefits to that strategy are $257,000 for a 700 sow farrow-to-finishing farm. The costs of PED are estimated to be approximately 20 per market hog.

▼ Introduction: There is a high prevalence of drug-related problems (DRPs) in community-dwelling residents which results in increased morbidity, mortality and healthcare expenditure. The Australian government has reimbursed pharmacists to identify and prevent or resolve DRPs through Home Medicines Reviews (HMRs) in these patients since 2001. Funding for the HMR program was based on studies undertaken prior to its implementation that indicated HMRs would be cost-effective due to reductions in drug costs and health service utilisation. Since the introduction of the program, there has been limited Australian research into the economic outcomes of HMRs. Furthermore, the results of international studies have questioned the clinical and cost-effectiveness of pharmacist-led medication reviews. The overall aim of the research described in this thesis was to investigate the clinical and cost-effectiveness of HMRs. To achieve this aim, the following objectives were formulated and addressed:
* to investigate novel methods for assessing the clinical and cost-effectiveness of HMRs;
* to investigate the characteristics of the DRPs identified in HMRs, including the drugs and conditions involved, and the recommendations made to resolve or prevent DRPs;
* to estimate the clinical effectiveness and cost-effectiveness of HMRs; and
* to investigate potential avenues to optimise the clinical effectiveness and cost-effectiveness of HMRs.
Method: A methodology that used expert opinion to predict the outcomes of HMRs was developed, based on previous research of interventions undertaken in community pharmacies. The development of this methodology necessitated two studies to be undertaken whereby the healthcare system costs and quality of life effects of numerous common clinical conditions resulting from medication use were estimated using a combination of expert opinion and literature values.
To evaluate HMRs, an observational cohort study was conducted across all states in Australia. Pharmacists accredited to perform HMRs submitted a random sample of HMRs that they had undertaken in 2008. Information from the HMR referral, report and outcomes of the HMR were classified according to standardised systems and entered into an electronic database for analysis. A panel of experts reviewed a sample of the HMRs and estimated the clinical outcomes of the recommendations made in the HMRs. A cost-consequence and cost-utility analysis was performed to evaluate the cost-effectiveness of HMRs from an Australian government perspective.
A sub-study was undertaken that investigated the relationship between cost-effective HMRs and factors that prior research suggested may influence the cost-effectiveness of HMRs.
Results: A “consequences table” was developed whereby estimates of healthcare system costs and quality of life effects of 51 common clinical consequences were evaluated at three levels of severity. ▼ Introduction HIV testing is an important step in controlling the spread of HIV worldwide. Data from the UNAIDS suggests that about 50% of people living with HIV are unaware of their diagnosis. Canadian estimates suggest that about 25% of HIV infected persons are unaware of their status. Ontario specific estimates suggests that there may be close to 40,000 persons living with HIV in Ontario; however, 27,000 persons are known and documented to be living with HIV in Ontario. The importance of testing and early diagnosis is well documented with evidence demonstrating that early treatment improves outcomes both for infected individuals and the communities they live in. This premise forms the foundation for the highly effective “treatment as prevention” approach. There are currently many HIV testing approaches including serum and saliva-based technologies. Testing can also be categorized based on duration to receipt of test result. With conventional HIV testing, results become available after greater than 24 hours. Rapid testing approaches such as rapid - facility based testing; rapid - location based testing; and rapid - mobile testing ensure results are available within 24 hours and in many cases, within a few minutes. Effectiveness studies of individual approaches have been conducted, however, studies on the cost-effectiveness of the various approaches in Canada are presently lacking. Most of the available economic studies have been conducted in high prevalence low-income countries, and as such, are not entirely applicable to the Canadian experience. Research objectives The overall aim of the thesis is to investigate the program and performance effectiveness, as well as the economic evidence for the use of rapid HIV testing options compared with existing conventional testing options. To achieve this there were three specific objectives, namely: i. To perform a systematic review of the available cost effectiveness evidence of rapid HIV testing approaches. ii. To perform a second systematic review and determine relative effect estimates for the use of available rapid HIV testing approaches versus conventional approaches iii. To determine the cost effectiveness estimates of the use of these testing approaches as they apply to a Canadian context. Results Rapid HIV Testing for improving uptake of HIV/AIDS services in people with HIV Infection - A Systematic Review and Network Meta-analysis HIV testing has evolved to include rapid testing done in hospitals, non-clinical environments such as bathhouses, places of worship, learning environments and in other cases mobile HIV testing options have also been offered using mobile vans or other motorized vehicles. ▼ Introduction The concept of this thesis was driven by stagnation within the Irish healthcare system. Multiple reports from pharmacy organisations had outlined possible future directions for the profession but progress was minimal, especially in comparison with other countries. The author’s directive was to evaluate the
economic impact of a series of clinical pharmacy services (CPS) in hospital and community settings. Methods A systematic review of
economic evaluations of clinical pharmacy services in hospital patients was undertaken to gain insight into recent research in the field. Eligible studies were evaluated using the Consolidated Health
Economic Evaluation Reporting Standards (CHEERS), to establish the quality, consistency and transparency of relevant research. A retrospective analysis of an internal hospital pharmacy interventions database was conducted. A method first described by Nesbit et al. was implemented to estimate the level of cost avoidance achieved. A cost-effectiveness analysis based on data from a randomised controlled trial of a pharmacist-supervised patient self-testing (PST) of warfarin therapy is presented. Outcome measure was the incremental cost associated with six months of intervention management. A similar cost-effectiveness analysis based on previously published RCT data was used to evaluate a novel structured pharmacist review of medication in older hospitalised patients. Cost-effectiveness analysis was presented in the form of an incremental cost-effectiveness ratio (ICER). An ICER is an additional cost per unit effect, in the case of this study, the cost of preventing an additional non-trivial ADR in hospital. A method described by Preaud et al. was adapted to estimate the clinical and
economic benefit gained from vaccination of patients by a community pharmacist in Ireland in 2013/14. Sample demographic data was obtained from a national chain of community pharmacies and applied to overall national vaccination data. Results Systematic review identified twenty studies which were eligible for inclusion. Overall, pharmacist interventions had a positive impact on hospital budgets. Only three studies (15%) were deemed to be “good-quality” studies. No ‘novel’ clinical pharmacist intervention was identified during the course of this review. Analysis of internal hospital database identified 4,257 interventions documented on 2,147 individual patients over a 12 month period. Substantial cost avoidance of €710,000 was generated over a 1 year period from the perspective of the health care provider. Mean cost avoidance of €166 per intervention was generated. ▼ This dissertation explores how, why, and under what circumstances cost benefit analysis (CBA) and cost effectiveness analysis (CEA) are being used in assessing World Bank education Project Appraisal Documents (PADs) from 2010 to 2014. Choices made when allocating limited education funding to unlimited education needs are challenging as they involve a multitude of factors and CBA/CEA tools are intended to help guide these choices. However, the literature shows that the field of education leverages CBA/CEA tools with low frequency and poor quality compared to other social services. The World Bank offers an opportunity to explore and gain insight into the use of CBA/CEA tools in education projects, as they are the largest funder of education projects and a pre-eminent source of applied development economics.
I have adopted a two-phased mixed methodology approach giving priority to the qualitative dimension. I use a lens rooted in the context of an institutional system—including organizational, personal, project, and worldwide dimensions—to analyze data collected via semi-structured interviews revolving around six specific World Bank Education projects.
I found that CBA dominates CEA use, and CBA use swayed upward from 2011—with CBA becoming the norm by 2014. CBA/CEA tools are used for their value well beyond the initial choice decision, with an emphasis on facilitating rich education discussions and refining project details. Multiple inter-related factors direct how CBA/CEA is used—with data, timeline, and skill set leading the way. However, the systemic organizational norm that roots CBA use in human capital theory has resulted in an overly mechanistic CBA process that is often out of context. This study shows the importance of thoughtful deliberation and concise governance over how CBA/CEA is defined, the triggers that indicate when to use CBA/CEA, and the protocols for authoring a CBA/CEA for an education project.

▼ Mental disorders are associated with considerable disease burden and financial costs. The cost-effectiveness of preventive interventions for mental disorders in the Australian context is currently not known. This thesis has applied a priority-setting approach, called Assessing Cost-Effectiveness (ACE), to preventive interventions for mental disorders. The ACE approach has been developed to answer questions regarding the economic credentials of competing interventions, by using economic modelling techniques, but also having regard to broader issues of relevance to healthcare decision-makers. The economic modelling techniques assessed the population costs and outcomes, expressed as Disability-Adjusted Life Years (DALYs), of selected interventions. Extensive uncertainty and sensitivity analyses accompanied all evaluations. A Project Steering Committee considered the findings in the light of “second stage filters” (e.g. equity concerns, quality of the evidence base, feasibility of implementation and acceptability to stakeholders).
Six preventive interventions for mental disorders were evaluated using this approach. Only interventions which had diagnostically assessed outcomes at follow-up were eligible for inclusion in the study. Using a value-for-money criterion of AUD50,000/DALY averted, all preventive interventions for mental disorders met the criteria of at least good value-for-money and a few were deemed excellent or very good value-for-money. Screening children and adolescents for early signs of depression and then subsequently providing a psychological intervention to prevent a full depressive disorder had a median incremental cost-effectiveness ratio (ICER) of AUD5,400 (95% uncertainty interval: 1,400-32,000). The effectiveness credentials of this intervention were deemed “sufficient” and the second stage filter issues of concern were not insurmountable. As a consequence, it is recommended that this intervention be seriously considered for national roll-out. A parenting intervention to prevent childhood anxiety had a favourable ICER of 7,000/DALY averted (5,400-24,000). However this analysis was based on a single randomised trial and further piloting to ensure intervention effectiveness is recommended. Screening adults for signs of depression and treatment with a brief bibliotherapeutic intervention or group-based psychological therapy to prevent a full depressive disorder were also very good value-for-money (median ICERs of AUD8,600 and 20,000 respectively). Again, however, further piloting to ensure effectiveness is recommended. Screening and treating women at risk of postnatal depression was cost-effective, but the effectiveness credentials of this intervention require strengthening.

▼ Background: Increased costs and demand for accessibility to mental health services accompanied by a decline in resources has forced mental health service providers and government to innovate and develop a variety of new programs and service delivery strategies. As a result, a substantial number of organizations in Ontario have adopted the walk-in/single session therapy model. Further, many more family services agencies are planning to open a walk-in counselling center. Although, there have been some studies on clinical effectiveness, only one previous study has examined the cost-effectiveness exclusively focusing on single-session therapy/walk-in counselling service and that study suffered from a small sample size and the lack of a control group. Therefore, a rigorous research that examines the economic effectiveness is of paramount need. With a large sample size and a control group, this study aims to close the existing gap in the economic evaluation of single-session therapy/walk-in counselling.
Objective: To determine the cost-effectiveness analysis of a single-session walk-in counselling model of service delivery compared to the traditional counselling model.
Methods: Cost effective analysis was undertaken with effectiveness measured by the General Health Questionnaire (GHQ-12) score. Cost was measured using data on direct medical costs: physician cost, hospital cost, emergency visit cost, counselling cost, and other social service cost, and indirect costs: the cost of lost work days and the cost of lost usual activities. To make the results comparable to other interventions, the GHQ-12 score was converted to QALYs using Serrano-Aguilar et al.’s (2009) algorithm. Incremental Cost-effectiveness Ratio (ICER) was calculated comparing walk-in counselling to being on the waitlist for traditional counselling. A probabilistic sensitivity analysis (PSA) was performed to account for uncertainties of parameters. In-depth analysis was done using Mixed effect modeling (also called multilevel models) to analyze the data from both individual level and group/context level and also to study growth or change trajectories (of the outcome measure) over a period of time, in order to measure the effectiveness.
Perspective: The societal perspective was used for our analysis with a time period of 10 weeks.
Data sources: The data from a CIHR-funded project was used. The data were collected from two family service agencies in Ontario, Canada; Kitchener-Waterloo Counselling Services (KWCS) and Family Service Thames Valley (FSTV) at three different time points: baseline, 4 weeks and 10 weeks over a period of 6 months.
Results: During the ten weeks of the study, the total mean incremental costs were (1,499.55-1,865.10) =-365.55, indicating that walk-in counselling was less costly than the traditional counselling. ▼ As the understanding of the biology of cancer intensifies, biomarkers with the potential to guide precision cancer medicine (PCM) are discovered with increasing regularity. Yet the cost-effectiveness of the biomarker-guided PCM strategy is rarely evaluated. Utilising colorectal cancer (CRC) as a bellwether for PCM implementation, I conducted a systematic review of health economic analyses of molecular biomarkers (MBM) to stratify CRC patients for PCM in high-income countries. This analysis found that: 1. Oncotype DX® is cost-effective in sparing treatment toxicity. 2. Dihydropyrimidine dehydrogenase gene (DPYD) status testing may be cost-effective to avoid 5-Fluorouracil (5-FU) toxicity. 3. Uridine 5'-diphospho-glucuronosyltransferase family 1 member A1 gene (UGT1A1) polymorphism status is not cost-effective in guiding irinotecan dosing. 4. The cost effectiveness of testing of KRAS/NRAS mutational status for guiding cetuximab or panitumumab therapy is inconclusive. Overall, the systematic review revealed that there was a paucity of cost-effectiveness studies in CRC to support biomarker-enriched PCM approaches. To discern the real-world impact that PCMs and other therapeutic modalities are having on CRC survival, the economic burden of CRC in Europe for the year 2015 was determined by an enhanced cost-of-illness study, covering the EU27, together with the United Kingdom, Iceland, Norway, Switzerland, Serbia and Turkey (EUR33). CRC costs in 2015 were €21.9bn, broken down as follows: healthcare costs: €7bn (32% of overall costs); mortality costs: €5.7bn (26%); morbidity costs: €7bn (32%); informal care costs: €2.2bn (10%). Costs for managing a CRC patient varied significantly across each country, as did hospital care costs. Amongst overall healthcare costs, pharmaceutical costs represented the second largest expenditure after hospital care costs, and in several countries pharmaceutical costs were higher than hospital costs. The proportion of total healthcare expenditure on CRC was notably highest in Central and Eastern European countries in terms of percentage spent: conversely, these countries experienced the lowest CRC survival rates. Finally, as biomarker guided continuous cetuximab (CC) and intermittent cetuximab (IC) demonstrated equivalent clinical efficacy in treating advanced CRC in the COIN-B trials, I undertook a retrospective cost-minimization analysis to determine which therapy was the least costly. By employing both partition survival modelling (PSM) and Markov chain Monte Carlo (MCMC) simulation, IC therapy did prove to be the dominant therapy when compared to CC therapy, with a cost reduction of £33,679 (by PSM) or £29,708 (by MCMC) per patient. In addition to the cost savings realised by IC therapy, the intermittent strategy allows for less invasive tumour monitoring during treatment and the opportunity for tumours to potentially re-sensitise to cetuximab treatment. ▼ The research in this dissertation addresses the question of costs and benefits of two policy options for cannabis in the context of New South Wales, Australia. The first policy is the current one, where cannabis is illegal, although a cannabis cautioning program is available for the use or possession of a small amount of cannabis. In the second policy option, cannabis is a legal but a highly regulated good. As no legalised-regulated policy currently exists, as part of this research the policy framework was developed with the objective of minimising the harms associated with the use of cannabis and with the policy itself. The societal value and preferences for the policies were evaluated in two ways. First, through the use of a traditional cost benefit analysis (CBA) and secondly, with a discrete choice experiment (DCE). The CBA estimated important costs and benefits in monetary terms for each policy with the results presented as a net social benefit. The DCE assessed the preferences for different policies for cannabis among a representative sample of the population. Potential trade-offs between key outcomes including the prevalence of consumption, expenditures by the criminal justice system and health care systems related to cannabis were explored.The results from the CBA indicate there is no clear difference in the net social benefit between the two policy options, although there is a potential revenue gain for government in the legalised-regulated option. The results from the DCE reveal that compared to the current policy there is a moderate preference for legalisation of cannabis among a community sample and strong preference against the complete criminalisation of cannabis. The results also demonstrate the trade-offs between different harms and benefits as well as the interactions between personal characteristics and the policy preferences. The findings from this analysis of the costs and benefits of two cannabis policy options will start to redress some of the evidence gaps that arise when making public policy in this area.

▼ The African pharmaceutical industry lacks the capacity to adequately supply the continent with essential medicines to combat the heavy disease burden that is grappling the continent. As a result, Africa relies heavily on imported medicines and vaccines to meet the growing needs of the population. Local pharmaceutical production promises to provide a sustainable solution to public health, industrial development and socio-
economic issues on the continent. However, local pharmaceutical production does not make
economic sense if the industry is unable to competitively produce quality medicine at prices that are comparable to or better than those of imported medicine. The aim of this study was to evaluate whether locally filling a multivalent vaccine used in paediatric immunization is economically viable when compared to the same vaccine currently imported semi-finished for labelling, packaging and distribution. Can a South African vaccine manufacturer produce the same vaccine cheaper than it currently imports? The objectives were to determine the production costs for both semi-finished product and local vaccine filling operations and to assess which option makes
economic sense to pursue. The sub-objectives were to establish the extent and impact of the market size and demand for this vaccine on the decision to pursue local production versus importation. A case study approach was adopted as a research method to gain an in-depth understanding of the
economic and production factors within the context of vaccine manufacturing with The Biologicals and Vaccines Institute of Southern Africa (Biovac) as a unit of analysis. Multiple sources of data were used to collect data for analysis. The study found that local production of this vaccine is economically viable and more favourable over imported product at current annual demand of 4.5 million doses. Local vaccine filling operations break-even point was found to be at 1.3 million doses when compared to 2.6 million doses for imported semi-finished product Whilst economies of scale cannot be disregarded for long-term profitability, this study found that
economic viability can be achieved with an annual demand of 4.5million doses. The aim of this study was accomplished. This study contributes to the body of knowledge on local pharmaceutical production and serves as a baseline for further research in this area.

▼ This paper examines an overview of the
economic and environmental aspects of Underground Coal Gasification (UCG) as a viable option to the above ground Surface Coal Gasification (SCG). In addition, some highlights, hurdles and opportunities from early investment to successful commercial application of some worldwide UCG projects will be discussed. Global energy demands have prompted continual crude oil consumption at an astronomical pace. As such, the most advanced economies are looking for local and bountiful resources to challenge crude oil's dependence for which coal provides the best alternative so far. In the U.S, the Department of Energy (DOE), the National Energy Transportation Laboratory (NETL) along with the Lawrence Livermore National Laboratory (LLNL) continue to support pilot programs that develop improved methods for clean coal technologies to produce coal derived fuels competitive with crude oil fuels at about $30 per barrel. Lignite, the softest of the four types of coal, is the best candidate for underground coal gasification due to its abundance, high volatility and water to carbon content in its rock formation. The biggest challenge of modern humans is to find a balance of energy consumption, availability of resources, production costs and environmental conservation. Additionally, UCG has environmental benefits that include mitigating CO₂ emissions through Carbon Capture and Storage (CCS) and reduced overall surface pollutants, making it the preferred choice over SCG.

▼ Background and aim The workplace has been identified as an ideal setting in which to promote healthy dietary behaviours. However, uncertainty surrounds both the effectiveness and costeffectiveness of workplace dietary interventions and a dearth of evidence exists with regards to evaluating the implementation process of such interventions. The core aim of this thesis was to conduct both a process
evaluation and an
economic evaluation of the Food Choice at Work (FCW) complex workplace dietary intervention. Methods The FCW study was a pragmatic trial which measured the effectiveness of a complex workplace dietary intervention in ideal settings. Firstly, a cross-sectional analysis which employed a zero-inflated negative binomial (zinb) regression model to examine associations between objective health status outcomes, lifestyle characteristics and absenteeism was conducted. Secondly, a detailed process
evaluation addressed the implementation of the complex workplace intervention. Interviews were conducted at baseline (27 interviews) and at 7-9 months follow-up (27 interviews) with a purposive sample of workplace stakeholders (managers, caterers and employees). Data were analysed using a thematic framework. Thirdly, micro-costing methods were employed to estimate the cost of implementing and delivering the complex workplace dietary intervention from an employer’s perspective. Finally, an
economic evaluation of the complex workplace intervention was conducted. This consisted of 1) a baseline costutility analysis (CUA) which measured the cost-effectiveness of the interventions in terms of quality-adjusted life-years (QALYs), 2) sensitivity analyses to test the robustness of the QALYs which involved performing cost-effectiveness analyses (CEA) using clinical measures to measure health outcomes and 3) a cost-benefit analysis (CBA) where the monetary value of absenteeism was employed so as to report the net benefit of the interventions relative to the control. Results Central obesity was positively associated with absenteeism and increased the expected rate of absence by 72% (mean number of absences was 2.5 days). Consuming a highquality diet and engaging in moderate levels of physical activity were negatively associated with absenteeism and reduced the expected frequency by 50% and 36% respectively. The process
evaluation revealed that managers’ desire to improve company image, stakeholder buy-in, organisational support and stakeholder cohesiveness facilitated the implementation of the interventions. Anticipation of employee resistance towards menu changes, workplace restructuring and target-driven workplace cultures were found to impede intervention implementation. With regards to the cost-analysis, 3 main cost categories were identified 1) set-up costs 2) maintenance costs and 3) physical assessment costs. The combined intervention xvi reported the highest total costs (€47,305), followed by nutrition education (€44,726), environmental modification (€24,474) and the control (€21,412). In the

▼ The South African policy framework on Local Economic Development encourages the establishment of community-based projects in an attempt to address the challenges posed by poverty in local communities. It appears that community-based projects have a role to play in poverty alleviation. Municipalities are supposed to encourage and support the establishment of projects aimed at poverty alleviation in their jurisdictions. The Mbombela Local Municipality has played its role in this regard by supporting the establishment of the Daantjie Bakery Project in 2000. In an attempt to evaluate the impact of community-based projects on poverty alleviation, this study investigated the role played by the Daantjie Bakery Project in local economic development of the Daantjie local community in the Mbombela Local Municipality. Participants included community members of Daantjie, employees of the bakery, and the project manager. The study found that, indeed community-based projects play a role in local economic development since they contribute towards employment creation and poverty alleviation.

▼ Cette thèse s’inscrit dans le débat scientifique actuel autour de l’évaluation économique des services écosystémiques fournis par les aires marines protégées (AMP) dans les pays en voie de développement. Appliquant la méthode de l’évaluation contingente, elle s’est intéressée à l’analyse de la perception de la population mauritanienne des services écosystémiques fournis par l’aire marine protégée du parc national du banc d’arguin (PNBA) en Mauritanie et la valeur qu’elle consente à payer pour maintenir l’offre de ces services dans son état actuel. A travers l’analyse des résultats de trois enquêtes réalisées au PNBA (niveau local), à Nouakchott (zone adjacente) et à Tidjikja (arrière-pays), la thèse discute l’impact de la distance, dans le sens de l’éloignement et du temps nécessaire pour accéder à l’AMP, tant sur la perception des services écosystémiques fournis par le PNBA que sur le consentement à payer pour sa conservation. Par ailleurs, elle aborde l’applicabilité de la méthode de l’évaluation contingente dans les pays en voie de développement et propose quelques améliorations du protocole de l’enquête pour améliorer la fiabilité des résultats.
This thesis is part of the current scientific debate around the economic evaluation of ecosystem services provided by marine protected areas in developing countries. Using the contingent valuation method, it focused on the analysis of the Mauritanian population's perception of ecosystem services provided by the marine protected area of Banc d'Arguin National Park in Mauritania and the value that this population agrees to pay to maintain the supply of these services in its current state. Through the analysis of the results of three surveys conducted at the PNBA (local level), Nouakchott (adjacent zone) and Tidjikja (inland), the thesis discusses the impact of distance, in the sense of the remoteness and the time required to access the MPA, both on the perception of the ecosystem services provided by the PNBA and on the Willingness To Pay for its conservation.In addition, it discusses the applicability of the contingent valuation method in developing countries and proposes some improvements to the survey protocol to enhance the reliability of the results.

▼ Cette thèse examine l’effet des catégories — les groupements effectués à partir de similarités sociales et cognitives — sur l’évaluation des organisations sur les marchés. La recherche montre que les audiences sur les marchés ont tendance à pénaliser les organisations associées à plusieurs catégories. Interrogeant les hypothèses des précédentes études, mon travail met en perspective cette exigence de pureté catégorielle en étudiant le rôle de (i) l’imbrication catégorielle (« category nesting »), c’est-à-dire la disposition hiérarchique des catégoriques, (ii) l’hétérogénéité, au sein des audiences, de l’expertise et des modes de catégorisation (la manière dont les individus regroupent les entités), et (iii) les différences de valeur entre les catégories. A partir de tests expérimentaux (Chapitre 1), de données longitudinales sur les transactions de capital-risque à travers le monde de 1994 à 2017 (Chapitre 2) ainsi que de développements théoriques (Chapitre 3), cette thèse explicite certaines conditions sous lesquelles les organisations affiliées à des catégories atypiques sont perçues plus favorablement que les organisations pures sur le plan catégorique. Ce travail contribue à la théorie des organisations et la sociologie économique, en s’adressant à la recherche sur les catégories de marché, les évaluations des experts et la spécificité optimale (« optimal distinctiveness »).
This dissertation examines how categories — groupings that are built upon social and cognitive similarities — affect the evaluation of organizations in markets. Research shows that market audiences are likely to penalize organizations that combine multiple categories. Questioning the past studies' underlying assumptions, my work contextualizes this demand of categorical purity by studying the role of (i) category nesting, that is the hierarchical disposition of categories, (ii) audience members’ heterogeneity in expertise and modes of categorization (the way individuals group entities together), and (iii) the differences of categorical valence. Using experimental tests (Chapter 1), longitudinal data on venture capital deals worldwide from 1994 to 2017 (Chapter 2) as well as theoretical developments (Chapter 3), this dissertation provides evidence of conditions under which categorically atypical organizations are better appraised than categorically pure organizations in markets. This work offers contribution to organization theory and economic sociology by speaking to research on market categories, experts’ evaluations and optimal distinctiveness.

▼ Amongst other socio-economic challenges that South Africa has, unemployment still remains high; in July 2010 unemployment was at 25.3 percentage and in July 2011 it was at 25.7 percentage ; in a space of a year it grew by 0.4 percentage (Trading Economics, 2012). Even though South Africa has made some significant progress with its democracy; there is still significant socioeconomic problems. Poverty, lack of economic empowerment among the disadvantaged groups are amongst and lack of proper education are amongst some of these pressing challenges in South Africa. More than a quarter of South Africa's population currently receives social grants (Index mundi, 2011). The Eastern Cape Province is still faced with significant socio-economic challenges such as high unemployment rate. 74 percentage unemployed are the economically active group as they are under the age of 35 (ECDC, 2011:22). “More than a quarter (26.4 percentage) of households in the Eastern Cape relies on government grants as their main source of income. Nationally, the youth unemployment rate is estimated to be close to 35 percentage, with more than three million young people out of work. Youth unemployment in the Eastern Cape averages 41.4 percentage. This is more than twice the adult unemployment rate (18.4 percent). A total of 695,175 young people have been without work every year in the Eastern” (ECDC, 2010-2011: 23). Besides, this province has a third largest number of people who are living with HIV/AIDS in the country. These statistics have a major significance on the province’s economic prospects and progress (ECDC, 2010-2011:22). The RSA Constitution 1996, the White Paper on Local Government 1998 has mandated local governments to facilitate LED. South Africa adopted LED as a way of fighting poverty and stimulating economic growth and development for local residents. LED is promoting skills development, SMMEs and tourism to encourage economic development. However the success of LED has received mixed views in that; some successes of LED were recorded in urban or metropolitan municipalities. Few successes have been recorded of LED projects in rural municipalities. Some of the constraints of LED consist of capacity problems within the municipalities, understanding the role of LED in the municipality and lack of funding. In Makana Municipality, about 23 percentage of households live below poverty line of (R800.00 – R9600 a year). Most of the population earns between R801.00-R1600.00 per month with 24.7 percentage. A mere 0.6 percentage earn R204801.00 or more compared to 12.7 percentage with no income. Inadequate infrastructure poses as a threat to attracting and retaining investment in this municipality. ▼ Multi-horizon forecasts from large scale models play important roles in ongoing policy debates across a wide range of disciplines. Although there are many ways to evaluate imperfect forecasts, we show the importance of taking a more general approach and considering the joint distributions over variables and horizons. Once a multi step system context is envisaged, potential singularities need to be addressed, as do tests of significant differences between the forecasts and assessments of the value of improved forecasts. These important issues are the focus of the thesis and are illustrated using applications in the fields of macroeconomics and climate.

▼ Many billions of dollars each year are spent in pursuit of economic and social development goals. The field of program evaluation aims to measure the efficacy of these programs and allocate funds to achieve optimal results. However, current research on program design and evaluation tends to focus on determining causality through complex statistical methods, neglecting intermediate measures of data, such as network metrics. Similarly, research in computational social science has focused on generating hypotheses and validating theory rather than economic development applications. This thesis develops a novel technique for using computational social science to design and evaluate social and economic programs. A framework for program design and evaluation using network metrics is presented, along with two case studies that illustrate the use of this technique. In the first, we consider Start-Up Chile, an economic development program whose goal is to foster networks between Chileans and international entrepreneurs, using network metrics to evaluate its ability to facilitate connection between Chilean and non- Chilean entrepreneurs. Second, an agent-based model for designing entrepreneurial incubators is developed, with novel conclusions for more efficient design of economic development programs.

▼ The Twickenham Platinum Mine (TPM) Project is
located in the north eastern limb of
the Bushveld Complex, north
west of Steelpoort in the Limpopo Province. The
property hosts
platinum group metals (PGM) mineralisation in the Merensky Reef
(MR) and Upper Group 2 Chromitite (UG2). The two reefs are
separated by 400 m of
mafic and ultramafic rocks of the Rustenburg
Layered Suite.
The question that must be answered with this study
relates to the
economic viability
of the MR compared to that of
the UG2 at the TPM Project, as it stands in 2011.
The assumption
is that no mining has commenced on this project and that there is
an
equal opportunity to commence mining on one of the reefs.
The
study describes the ore body characteristic for each reef,
focussing on the
lithologies, structure, and resources available.
The discounted cash flow (DCF)
method was used to determine the
economic value of each reef. The net present
value (NPV) and
internal rate of return (IRR) were calculated and used to compare
the ore bodies. The input parameters to the DCF are the main
limiting factors to this
method, as the results are heavily
dependent on the assumptions made. The input
parameters used were
based on actual published values and generally accepted and
motivated assumptions.
A sensitivity and risk analysis was
completed to identify value ranges and potential
risks to the
projects. The outcome of the analysis has been compared to other
projects as a benchmark to ensure the project assumptions were
realistic. The world
markets supply and demand for PGM is
intricately related to exchange rates, metal
prices, inflation,
and investment risk. These have an influence on the strategic
planning for a company as well as investment decisions through
various project
evaluation methods.
South Africa has a long
history of mining and metals extraction. Extensive mining
legislation has been developed to ensure the country’s mineral
wealth is protected
and the health and safety of employees are
high priority. Specific challenges related

to mining on
the Eastern Limb are discussed in order to justify the high risk
assigned
to the project for this
evaluation.
The DCF was
calculated and the outcome indicated that neither the MR nor the
UG2
is economically viable using these parameters in the 2011
economy.
The MR
evaluation produced a negative NPV (R
-1,664,541,443.47) and an IRR of 9
%, which is well below the
required discount rate of 12 %. The initial project capital
will
be repaid after 19 years of the 33 year life of mine. The
sensitivity analysis
showed that by reducing the initial capital
by 30 %, the project produces a positive
NPV. The other factor
that produced a positive NPV was by reducing the operating
cost by
50 %. This project will have to be re-evaluated after all
parameters have been
tested and some re-engineering has been done
to optimise the extraction of the MR
ore body.
The UG2
evaluation
produced a negative NPV (R -109,614,208.27) and an IRR of 12
%,
equal to the required discount rate. The…

▼ Abstract Background Presenteeism is an
economic
concept that is difficult to identify, measure, and value.
Rheumatoid arthritis (RA), ankylosing spondylitis (AS) and
psoriatic arthritis (PsA) are three chronic auto-immune conditions
that increase levels of presenteeism. Workplace interventions
(WPIs) help individuals to manage their health condition at work.
Existing methods used to quantify the impact of presenteeism are
unable to adequately inform the employer of the productive benefits
of WPIs. The overall aim of this thesis was to appraise current
methods used to quantify presenteeism and to develop methods to
value the impact of presenteeism suitable for use in
economic
evaluations (EE) of WPIs. Methods Two systematic reviews were
conducted: 1) to assess the extent to which self-report measure of
presenteeism were underpinned by
economic theory; and 2) to explore
if, and how, productivity was quantified and included in EE of WPIs
for musculoskeletal conditions (MSDs). Thematic analysis methods
were used to analyse qualitative data collected from working
individuals with RA, AS or PsA (n=22) that explored the extent to
which measures of health status (EQ5D; SF6D) and capability
(ICECAP-A) capture the impact on ability to work caused by RA, AS
or PsA. Econometric methods were used to specify prediction models
that included measures of health status, capability and
presenteeism, using a sample of 542 working people with RA and AS.
Results The first systematic review identified 24 self-report
measures of presenteeism; all, except one measure were not
underpinned by
economic theory. The second systematic review
identified 20 EE of WPIs for MSDs. Absenteeism was included in all
studies (n=20); however, presenteeism was included in only four.
The qualitative data confirmed measures of health status and
capability had the ability to capture those aspects of RA, AS and
PsA that impact an individual’s ability to work. The best
performing prediction model used an OLS specification including
SF6D, age and gender to predict presenteeism measured by the WPAI.
Conclusion The results suggest that HRQoL measures, specifically
the SF6D, can be used to capture and predict levels of presenteeism
caused by RA, AS and PsA.

▼ Background: Currently, there are no population-level smoking cessation interventions widely promoted in China. Economic concerns are one of the major barriers to a greater promotion of smoking cessation interventions in China.
Objectives: The objective of this study was to use evidence on the effectiveness of physician counselling and nicotine replacement therapy (NRT) patches use from Western countries, with the most recent smoking data from China to predict the potential effectiveness and cost-effectiveness of physician counselling and NRT patch in the healthcare system in urban China.
Methods: In Study 1, statistical analysis was conducted to estimate smoking and cessation rates in urban China. In Study 2, a Comparative Relative Assessment model was used to estimate the effectiveness and cost-effectiveness of physician counselling and NRT patch use for smoking cessation. Study 2 determined the estimates and costs of additional quitters and avoided lung cancer deaths from the implementation of physician counselling and NRT patch use in the healthcare system in urban China.
Results: In Study 1, smokers intending to quit were significantly more likely to have quit at follow-up than those not intending to quit. A total of 35.4% of smokers in urban China reported visiting a doctor in the past 12 months. Smokers who visited a doctor were significantly more likely to intend to quit and to have quit smoking at follow- up compared to those who did not visit a doctor. In Study 2, brief counselling to all smokers visiting the healthcare system in China was the most effective and cost-effective smoking intervention by generating a total of 2.35 million quitters at 2.32-7.73 per quitter. Smoking cessation counselling were found to be cost saving when compared with the total cost of lung cancer to Chinese society. The wide promotion of the NRT patch would be costly, requiring significant financial investments.
Conclusions: Implementing smoking cessation counselling in the healthcare system in urban China will result in cost savings from lung cancer. The high retail price and low acceptance of NRT patches in China may be required to achieve a population-level impact from pharmaceutical interventions.

▼ Objective: To estimate the cost-effectiveness of an influenza immunization strategy directed at healthy children 36 months to 15 years on the herd immunity of entire communities, versus not implementing this strategy. Design: An economic evaluation, cost-effectiveness analysis (CEA). Costs and effects were estimated jointly with a two-stage bootstrap with shrinkage correction. Uncertainties around input parameters were tested with one-way and multi-way sensitivity analysis. Data Sources: Effect and resource consumption data were from the Hutterite Influenza Prevention Study. Unit costs were collected from multiple sources including, government reports and schedules, local suppliers, peer-reviewed articles and systematic reviews, Internet searches and study data on file. Outcomes: Mean costs and effects, incremental cost-effectiveness ratio (ICER), net monetary benefit (NMB) statistic and cost effectiveness acceptability curves (CEAC). Results: The average cost per patient in the treatment arm was 69.08 and 32.66 in the control arm. The average number of influenza-free cases was of 0.96 in the treatment arm and 0.73 in the control arm. ICER was 164.19 per case of influenza averted, 95% confidence interval 28.38, 2,767.75. CEAC showed that at a willingness to pay of 177, the probability of the treatment strategy being cost effective compared to the control was 0.50. Results from sensitivity analyses were slightly different compared to base case results, supporting the robustness of base case estimates. Conclusion: This strategy is likely to be cost effective relative to the comparator as the ICER estimate is low and because the estimate is conservative given that the study population was very healthy and the influenza season was mild. A more virulent season and a less healthy population would have produced a lower ICER or seen the treatment arm dominate the control.

▼ Bitumen extraction from surface-mined oil sands results in the production of large volumes of Fluid Fine Tailings (FFT). Through Directive 085, the Province of Alberta has signaled that oil sands operators must improve and accelerate the methods by which they deal with FFT production, storage and treatment.
This thesis aims to develop an enhanced method to forecast FFT production based on specific ore characteristics. A mass relationship and mathematical model to modify the Forecasting Tailings Model (FTM) by using fines and clay boundaries, as the two main indicators in FFT accumulation, has been developed. The modified FTM has been applied on representative block model data from an operating oil sands mining venture. An attempt has been made to identify order-of-magnitude associated tailings treatment costs, and to improve financial performance by not processing materials that have ultimate ore processing and tailings storage and treatment costs in excess of the value of bitumen they produce.
The results on the real case study show that there is a 53% reduction in total tailings accumulations over the mine life by selectively processing only lower tailings generating materials through eliminating 15% of total mined ore materials with higher potential of fluid fines inventory.
This significant result will assess the impact of Directive 082 on mining project economic and environmental performance towards the sustainable development of mining projects.

▼ The Twickenham Platinum Mine (TPM) Project is located in
the north eastern limb of
the Bushveld Complex, north west of
Steelpoort in the Limpopo Province. The
property hosts platinum
group metals (PGM) mineralisation in the Merensky Reef
(MR) and
Upper Group 2 Chromitite (UG2). The two reefs are separated by 400
m of
mafic and ultramafic rocks of the Rustenburg Layered Suite.
The question that must be answered with this study relates to the
economic viability
of the MR compared to that of the UG2 at the
TPM Project, as it stands in 2011.
The assumption is that no
mining has commenced on this project and that there is an
equal
opportunity to commence mining on one of the reefs.
The study
describes the ore body characteristic for each reef, focussing on
the
lithologies, structure, and resources available. The
discounted cash flow (DCF)
method was used to determine the
economic value of each reef. The net present
value (NPV) and
internal rate of return (IRR) were calculated and used to compare
the ore bodies. The input parameters to the DCF are the main
limiting factors to this
method, as the results are heavily
dependent on the assumptions made. The input
parameters used were
based on actual published values and generally accepted and
motivated assumptions.
A sensitivity and risk analysis was
completed to identify value ranges and potential
risks to the
projects. The outcome of the analysis has been compared to other
projects as a benchmark to ensure the project assumptions were
realistic. The world
markets supply and demand for PGM is
intricately related to exchange rates, metal
prices, inflation,
and investment risk. These have an influence on the strategic
planning for a company as well as investment decisions through
various project
evaluation methods.
South Africa has a long
history of mining and metals extraction. Extensive mining
legislation has been developed to ensure the country’s mineral
wealth is protected
and the health and safety of employees are
high priority. Specific challenges related

to mining on
the Eastern Limb are discussed in order to justify the high risk
assigned
to the project for this
evaluation.
The DCF was
calculated and the outcome indicated that neither the MR nor the
UG2
is economically viable using these parameters in the 2011
economy.
The MR
evaluation produced a negative NPV (R
-1,664,541,443.47) and an IRR of 9
%, which is well below the
required discount rate of 12 %. The initial project capital
will
be repaid after 19 years of the 33 year life of mine. The
sensitivity analysis
showed that by reducing the initial capital
by 30 %, the project produces a positive
NPV. The other factor
that produced a positive NPV was by reducing the operating
cost by
50 %. This project will have to be re-evaluated after all
parameters have been
tested and some re-engineering has been done
to optimise the extraction of the MR
ore body.
The UG2
evaluation
produced a negative NPV (R -109,614,208.27) and an IRR of 12
%,
