Title: ” Effect of diurnal blood pressure profile on cardiovascular risk factors inhypertensives”Part1. Cardiovascular (CV) risk factors (RFs) and target organs damage (TOD) arecommon in hypertensives patients.The purpose of this study was to determine cardiovascular(CV) risk factors (RFs) andtarget organ damage clustering in 21.280 Greek hypertensives according to dippingstatus - as 8.861 of them underwent in 24h (ABPM)- stratified by gender and age.Glycemic and lipid profile were determined, left ventricular mass index, estimatedgromerular filtration rate (eGFR), 10-years CV risk according to Framinghm riskscore (FRS) and Heart-Score (HS) were calculated. Only 10.2% of patients had noconcomitant RFs, 53.1% had one (48.8% dyslipidemia,3.4% smoking, 0.9% diabetes),32.9% had two (26%dyslipidemia and smoking, 6.6% dyslipidemia and diabetes,0.3% smoking and diabetes) and 3.7% had all four traditional RFs. Obesity waspresent in 30%,metabolic syndrome in 38%,eGFR in 24% and left ventricularhypertrophy in 49%. Mean FRS risk was35% for males, 24.1% for females whereas inhigh risk(420%) were 68.7 and 50.7%, respectively (P=0.0001).Mean HS riskwas8.4% for males, 6.2% for females whereas in high risk (45%) were 48.6 and36.2%,respectively(P0.0001). Age was correlated to pulse pressure, eGFR, leftventricular mass index and CV risk (P0.0001). Ageing increased the risk differencebetween genders for total (P=0.001) but not for fatal events (P=nonsignificant).In conclusion, as RFs cluster in hypertensives, CV risk calculation should guidetreatment decisions.Part2: β-thalassemia trait (β-TT) is a common genetic disorder in Mediterraneancountries, including Greece. Previous studies have shown the protective effect ofβ-TT against myocardial infarction. However, the ambulatory blood pressure (BP)profile of such patients has not yet been investigated. Thus, the purpose of the presentstudy was to investigate the ambulatory BP monitoring(ABPM) profile ofhypertensives with β-TT, in comparison with all-cause anemic and non-anemicessential hypertensive patients.The study ultimately comprised of 8861 essential hypertensive, nondiabetic patientswho were divided into three groups: group I(n=191, with β-TT), group II (n=655,anemic) and group III (n=8015, nonanemic). All patients underwent fullclinical,laboratory and echocardiographic evaluations, whereas all were subjected toABPM. Anemia was defined as Hb <12 g per100 ml for women and o13 g per 100 mlfor men, whereas patients with b-TT were self-referred. The distribution of dippingpatterns among the three groups was 61.3 vs. 41.2 vs. 45.8% (P<0.001), whereas fornondippers it was 20.4 vs. 31.5 vs.27.7% (P<0.001), for extreme-dippers it was 15.7vs. 15.0 vs. 17.5% (P<0.001) and for reverse dippers it was 2.6 vs. 12.4vs. 9.0%(P<0.001). Furthermore, mean daytime systolic BP (SBP) among the three groupswas 140.13±7.79 vs.142.02±11.61 vs. 141.99±9.87mmHg (P=0.03), and meannighttime SBP was 125.87±10.4 vs. 131.13±15.7 vs.129.62±13.31mmHg (P<0.001).
