▼ <h1>ABSTRACT</h1> Aim: The aim of this retrospective study was to identify which indications prompted an MRI examination using the protocol ”Brain screening/Stroke” and whether the screening revealed recent infarcts and their localisation. Method: The data, which was subtracted from RIS (Röntgeninformationssystem), covered all consultation responses from stroke-rounds performed during 2015 at Akademiska Hospital in Uppsala (UAS). A total of 235 patients underwent an MRI-examination using the protocol ”Brain screening/Stroke”. Of this total population, 9 patients with a suspected or confirmed tumour were excluded. The information was collected at UAS during the period 11-22 April, 2016. Results: No indications were reported from the material collected. Hence, the method was insufficient to give an answer to the question at hand. In the final selection of 226 patients, no stroke could be detected in 42 % of the cases, 23 % had recent infarcts, 21 % had both recent and old infarcts and 14 % had only old infarcts. By gender the results were as follows: Of the total population, 85 patients were female. In 48% of these cases no stroke was detected, 21 % had recent infarcts, 20 % had old infarcts and 11 % had both recent and old infarcts. The remaining 142 patients were male. In 38% of these no stroke was detected, in 27% of the cases both an old an a recent stroke were detected , in 24% of the cases a recent stroke was detected and 11% had only old infarcts. Looking again at the total population, the consultation responses mentioned only recent infarcts and where these were located. The majority of the recent infarcts detected, 23 % were located in the Lobes of the brain, followed by 16% in the Cortex of the brain, 16% in the Brainstem, 13% in the Cerebellum, 10 % were locaded in ”Other parts of the brain”, 8 % were in the Talamus, 7 % were in White matter and the final 6 % in the Basala ganglia. The total number of recent infarcts detected was 189. Conclusion: The main question for this study was to see if it was possible to discover acute infarcts, using diffusion weighted MRI, and the localisation of those discovered. The method chosen worked well, but to get a better overview, more studies should be done, examining infarcts in greater detail. This knowledge may, in turn, be used as a basis for stroke diagnosis. It might also be of interest to complete this study, with a more in-depth analysis of the connection between different kinds of infarcts and the patient’s gender and age.

▼ Magnetic resonance imaging is utilized as an important tool in radiation oncology for delineation of healthy and cancerous tissues, and evaluating the functionality of those tissues, structures, and organs. Currently, the clinical imaging protocol at Virginia Commonwealth University includes anatomical imaging for tissue and structure delineation, and to observe treatment induced changes. Diffusion weighted imaging (DWI) is also acquired for calculation of apparent diffusion coefficient (ADC) values to provide quantitative information on tissue diffusivity and microstructure. However, anatomical images and ADC values may not display the true extent of changes in tissue. This work seeks to further utilize the capabilities of MRI and expand its role in treatment response monitoring for liver cancer patients treated with stereotactic body radiotherapy (SBRT). To do so, an imaging protocol and image analysis methodology to evaluate treatment changes on pre- and post-treatment image sets was developed. An extension of DWI, termed intravoxel incoherent motion (IVIM) imaging, was utilized to quantitatively assess levels of perfusion and diffusion within the liver and tumor. Acquisition of high-quality diffusion weighted images of the liver necessitated the development of an MR safe respiratory motion management device, which was designed, constructed and evaluated in this work. An imaging protocol was developed providing anatomical and functional images of the liver, acquired under breath hold, utilizing the respiratory motion management device. An IVIM parameter calculation and texture analysis workflow was developed using MATLAB, and applied to acquired data sets from multiple studies, including past clinical cases, investigator, healthy volunteer, and liver cancer patient . Differences in IVIM and texture analysis parameters were investigated for healthy and diseased tissue, and for select dose regions from pre- and post-treatment imaging sessions. Significant differences, at a voxel level, were found between healthy and diseased tissue, and pre- and post-treatment volumes, for multiple parameters, including apparent diffusion coefficient, pure diffusion, and perfusion, as well as for various texture features. Overall, this study showed the potential of IVIM and texture analysis to be used for discriminating between healthy and diseased tissues in the liver, and for indication of treatment response. Cilj: Neoadjuvantna hemioradioterapija (CRT) danas predstavlja standardni terapijski modalitet kod bolesnika sa lokalno uznapredovalim karcinomom rektuma jer dovodi do preoperativnog smanjenja tumorskog volumena i sniženja gradusa tumora, a u kombinaciji sa totalnom mezorektalnom ekscizijom, kao standardnom hirurškom opcijom za karcinom rektuma, i do sniženja stope lokalnog recidiva i produženja preživljavanja kod bolesnika sa karcinomom rektuma. Glavni ciljevi ove studije bili su da se odredi dijagnostička preciznost magnetne rezonance i difuzionog imidžinga u kvalitativnoj proceni stepena odgovora karcinoma rektuma na neoadjuvantnu terapiju, u kvalitativnoj proceni tumorske zahvaćenosti cirkumferentne resekcione margine i u kvantitativnoj proceni stepena odgovora karcinoma rektuma na preoperativnu hemioradioterapiju. Metodologija: Retrospektivnom kliničkom studijom obuhvaćeno je 62 bolesnika sa lokalno uznapredovalim karcinomom rektuma lečenih neoadjuvantnom hemioradioterapijom kojima je u inicijalnoj fazi i 6-8 nedelja nakon završetka neoadjuvantnog tretmana urađen MRI sa DWI radi procene stadijuma tumorske bolesti i procene tumorskog odgovora na primenjenu terapiju. Istraživanje je sprovedeno u Centru za radiologiju i magnetnu rezonancu Kliničkog centra Srbije u periodu od januara 2010. do juna 2013. Patološka analiza rađena je u Službi za patohistologiju Kliničkog centra Srbije na Odeljenju za Digestivnu patologiju. Istraživanje je odobreno od strane Etičkog komiteta Medicinskog fakulteta u Beogradu. Inicijalni i postterapijski MR pregled rađeni su po identičnom protokolu, pri čemu je procenjivan tumorski gradus pre i nakon CRT, tumorska infiltracija mezorektalne fascije nakon hemioradioterapije i tumorski odgovor na primenjenu terapiju metodom magnetne rezonance i difuzionog imidžinga na kvalitativan i kvantitativan način, kvalitativnom analizom MR i DWI slike i merenjem ADC vrednosti tumorskog tkiva pre i posle hemioradioterapije primenom tri različita ROI merna metoda. Histopatološka analiza rektalnih resekata vršena je procenom stepena tumorske regresije nakon neoadjuvantne terapije korišćenjem histopatološke skale za regresiju tumora (tumor regression grade – TRG) po Mandard-u i služila je kao referentni standard...

▼ Diffusion weighted imaging (DWI) is a branch within the field of magnetic resonance imaging (MRI) that relies on the diffusion of water molecules for its contrast. Its clinical applications include the early diagnosis of ischemic stroke and mapping of the nerve tracts of the brain. The recent development of filter exchange imaging (FEXI) and the introduction of the apparent exchange rate (AXR) present a new DWI based technique that uses the exchange of water between compartments as contrast. FEXI could offer new clinical possibilities in diagnosis, differentiation and treatment follow-up of conditions involving edema or altered membrane permeability, such as tumors, cerebral edema, multiple sclerosis and stroke. Necessary steps in determining the potential of AXR as a new biomarker include running comparative studies between controls and different patient groups, looking for conditions showing large AXR-changes. However, before designing such studies, the experimental protocol of FEXI should be optimized to minimize the experimental variance. Such optimization would improve the data quality, shorten the scan time and keep the required study group sizes smaller. Here, optimization was done using an active imaging approach and the Cramer-Rao lower bound (CRLB) of Fisher information theory. Three optimal protocols were obtained, each specialized at different tissue types, and the CRLB method was verified by bootstrapping. A test-retest study of 18 volunteers was conducted in order to investigate the reproducibility of the AXR as measured by one of the protocols, adapted for the scanner. Group sizes required were calculated based on both CRLB and the variability of the test-retest data, as well as choices in data analysis such as region of interest (ROI) size. The result of this study is new protocols offering a reduction in coefficient of variation (CV) of around 30%, as compared to previously presented protocols. Calculations of group sizes required showed that they can be used to decide whether any patient group, in a given brain region, has large alterations of AXR using as few as four individuals per group, on average, while still keeping the scan time below 15 minutes. The test-retest study showed a larger than expected variability however, and uncovered artifact like changes in AXR between measurements. Reproducibility of AXR values ranged from modest to acceptable, depending on the brain region. Group size estimations based on the collected data showed that it is still possible to detect AXR difference larger than 50% in most brain regions using fewer than ten individuals. Limitations of this study include an imprecise knowledge of model priors and a possibly suboptimal modeling of the bias caused by weak signals. Future studies on FEXI methodology could improve the method further by addressing these matters and possibly also the unknown source of variability. ▼ The transition from childhood to adolescence is a particularly vulnerable period for the development of internalising symptoms and disorders. Hormonal changes as well as changes in brain structure and function may play a role in this increased vulnerability. Most of the research to date has focused on the hormonal and brain changes during gonadarche, whereas the literature is much more limited for adrenarche, an earlier pubertal phase that takes place prior to gonadarche. Therefore, this thesis aimed to examine how (changes in) adrenarcheal hormones relate to the development of brain structural and functional connectivity, and how that in turn affects internalising symptoms in late childhood to early adolescence. Data were used from two longitudinal community-based samples with two time points each (sample 1 M ages 9.5 and 12.2 years; sample 2 M ages 8.5 and 10 years). At each time point in each study, levels of dehydroepiandrosterone (DHEA), its sulfate (DHEAS) and testosterone were measured and averaged from morning saliva samples collected across several days. Participants also underwent Magnetic Resonance Imaging (MRI) scans, and completed self-report questionnaires, at both time points. Diffusion-weighted imaging scans were analysed to examine white matter structural connectivity. Functional Magnetic Resonance Imaging (fMRI) scans during an affective face processing paradigm were analysed to examine functional connectivity. Levels of internalising symptoms were based on self-report questionnaires: the Spence Children’s Anxiety Scale, the Children’s Depression Inventory, and the Positive And Negative Affect scale. Analyses were conducted to investigate associations between hormone levels (initial levels and changes in levels over time) and brain structural and functional connectivity (baseline and change over time). Analyses were also conducted to investigate whether hormone-related changes in structural/functional connectivity were associated with symptoms. The results showed that children with high DHEA levels at age 9 had higher mean diffusivity (cross-sectionally) in a wide range of white matter tracts, suggesting that relatively early exposure to DHEA might be negatively associated with white matter microstructure. Changes in testosterone from age 8.5 to 10 years were negatively associated with the development of white matter structure as quantified by fibre cross-section in posterior white matter tracts. Higher levels of testosterone at age 8.5 years, however, were related to stronger development of fibre cross-section from age 8.5 to age 10 years. These hormone-related changes in white matter structure were not significantly associated with levels of internalising symptoms. ▼ Breast cancer is the most common and the second most fatal cancer among women in the U.S. Current knowledge indicates that there is a relationship between high breast density (measured by mammography) and increased breast cancer risk. However, the biology behind this relationship is not well understood. This may be due to the limited information provided by mammography which only yields information on the relative amount of fibroglandular to adipose tissue in the breast. In our studies, breast density is assessed using quantitative MRI, in which MRI-based tissue-dependent parameters are derived voxel-wise by mathematically modeling the acquired MRI signals. Specifically, we use data from a radial gradient- and spin-echo imaging technique, previously developed in our group, to assess fat fraction and T₂ of the water component in relation to breast density. In addition, we use diffusion-weighted imaging to obtain another parameter, apparent diffusion coefficient (ADC) of the water component in the breast. Each parametric map provides a different type of information: fat fraction gives the amount of fat present in the voxel, the T₂ of water spin relaxation is sensitive to the water component in the tissue, and the ADC of water yields other type of information, such as tissue cellularity. The challenge in deriving these parameters from breast MRI data is the presence of abundant fat in the breast, which can cause artifacts in the images and can also affect the parameter estimation. We approached this problem by modifying the imaging sequence (as in the case of diffusion-weighted imaging) and by exploring new signal models that describe the MRI signal accounting for the presence of fat. In this work, we present the improvements made in the imaging sequence and in the parametric mapping algorithms using simulation and phantom experiments. ▼ Current breast MRI practice in New Zealand closely emulates the American and European guidelines. The most commonly used protocols used for imaging breasts using these guidelines are dynamic contrast enhanced (DCE) sequences that involve the intravenous injection of contrast and generation of contrast kinetic graphs to differentiate any suspicious lesions. Breast MRI has been widely researched and is described as having high sensitivity to breast lesions. Specificity of breast MRI (DCE) is more debatable with wider variations reported resulting in false positives. This research has set out to examine the introduction of DWI and spectroscopy to routine protocols in order to evaluate their effect on sensitivity and specificity. This research was undertaken on a 1.5 Tesla MRI scanner using only pathology confirmed lesions. This selection of lesions included a wide range of all of the commonly presenting malignant lesions (Lobular, Invasive ductal, DCIS and Mucinous). In order to fully evaluate the performance of DWI and spectroscopy an audit of 68 pathology confirmed breast lesions was undertaken. DCE MRI gave sensitivity and specificity of 90% and 62.5% respectively. When evaluating the same sample with DWI, specificity was increased to 88%. Perhaps the most significant finding of this research was 100% sensitivity to DCIS (n=11) when using DWI alone. Spectroscopy was undertaken using a smaller sample of mass-like lesions (n=21) achieving sensitivity of 100%. Positive predictive value for this sample was 90%. In drawing conclusions from this research DWI using a b-value pairing of 0 and 750 mm2/s increases the specificity of breast MRI to malignancy in a clinically manageable time of less than four minutes and as such would be a worthy addition to routine breast MRI protocols. Spectroscopy demonstrated both high sensitivity and PPV to breast lesions but with a specificity of 66% and imaging times of between five and eight minutes needs further evaluation before being undertaken routinely. ▼ The amygdala, a subcortical structure known for social and emotional processing, can be subdivided into multiple nuclei with unique functions and connectivity patterns. Tracer studies in adult macaques have shown that the lateral and basal amygdala subnuclei decrease in connectivity to visual cortical areas moving from anterior to posterior, and that infants have similar adult-like projections plus additional connections that are refined with development. Can we delineate the connectivity between the amygdala subnuclei and occipitotemporal cortex in humans, and will it show similar developmental differences as macaques? If so, what functional regions may be contributing to this pattern of connectivity? To address these questions, we anatomically defined the lateral and basal amygdala subnuclei in 20 adult subjects, 27 kids (aged 7-8), and 15 neonates. We then defined the occipitotemporal region in each individual’s native anatomy, and split this entire region into five equal sections from anterior to posterior. We also defined visual functional parcellations in the occipitotemporal cortex (e.g., FFA, PPA) and anatomically defined primary visual cortex (i.e., V1). Using Diffusion Weighted Imaging data, we ran probabilistic tractography with FSL between the amygdala subnuclei as seeds and the occipitotemporal cortical parcellations as targets. Results showed that like macaques, the mean connectivity across subjects to the occipitotemporal cortex significantly decreased on a gradient from anterior to posterior, and that connectivity in kids and neonates was adult-like but became more refined across development. Further, refinement of connectivity to mid and posterior occipitotemporal cortex was largely driven by anterior PPA, LO, and V1, with connectivity to higher order visual areas increasing with age. The functional maturation of these regions may contribute to the continued refinement of these connections, in line with Interactive Specialization hypotheses of brain development. Trabalhos anteriores sobre a síntese de imagens de difusão por ressonância magnética se limitaram a estudos sobre estruturas microscópicas, menores que as dimensões típicas de um voxel (e.g., [BF08] [BF13] [LFS + 10] e [BA94]). Isto decorre em parte devido às metodologias utilizadas, que têm como ponto em comum serem simulações de tipo Monte Carlo, nas quais os elementos mínimos da simulação são as partículas de água. Portanto o custo computacional destas simulações é proporcional ao número de partículas a simular, e isto limita os volumes que podem ser simulados a tamanhos microscópicos. Propomos uma metodologia alternativa, que utiliza a imagem T 2 de uma amostra para sintetizar imagens de difusão por ressonância magnética. Os elementos mínimos desta nova metodologia são os pontos da imagem T 2 , e portanto seu custo computacional é proporcional à resolução da imagem T 2 utilizada, o que permite a síntese a partir de amostras de qualquer tamanho físico. Estas sínteses são realizadas por meio da integração numérica da equação do artigo seminal de Stejskal e Tanner [ST65] que relaciona a atenuação do sinal de ressonância magnética devida à difusão com os parâmetros da sequência de pulsos PGSE. Usamos os parâmetros típicos dessa sequência (b, gamma, tau\', g 0, g, delta e Delta), que podem ser configurados explicitamente em máquinas de ressonância magnética, para calcular valores do coeficiente de difusão aparente D em direções arbitrárias. Desenvolvemos software, disponibilizado [Bor] por licença GPL [Fou07], para realizar estas simulações, e para especificar uma máscara de direções, útil para modelar a difusão de uma amostra. Estas ferramentas permitem o estudo sistemático das variações dos parâmetros na síntese de imagens de difusão por ressonância magnética. Apresentamos um estudo de um fantoma de capilares imersos em água, exemplificando como utilizar as ferramentas para investigar a influência destes parâmetros na difusão da água da amostra.

Previous work on the synthesis of diffusion-weighted magnetic resonance imaging are limited to microscopic structures, smaller than the typical dimensions of a single voxel (e.g., [BF08] [BF13] [LFS + 10] and [BA94]). This is consequence, in part, of the methodologies used, that have in common the adoption of Monte Carlo simulation strategies, in which the minimal elements of simulation are the water particles. Therefore the computational cost of these simulations is proportional to the number of particles to simulate, and this limits the volume to be simulated to microscopic sizes. We propose a novel methodology, that uses the T 2 image from a sample to synthesize diffusion-weighted magnetic resonance images. The mininal elements of this novel methodology are the points of the T 2 image, and therefore its computational cost is proportional to the resolution of the T 2 image to be used, which allows the synthesis from samples of any physical size. These syntheses are made through numerical integration of the equation from the seminal article by Stejskal and Tanner [ST65]…

Cognitive control processes support purposeful, goal-directed behaviour in the presence of conflicting demands from our environment. Given advance information, this type of control can be engaged in anticipation of a change in behaviour. The cued-trials task-switching paradigm can temporally dissociate proactive and reactive cognitive control processes involved in switching between sets of abstract task rules. Typically, there is a performance cost for switch relative to repeat trials, which is attributed partly to proactive control processes required to prepare for a switch in task and partly to reactive control processes required to deal with between-task interference. Despite two decades of research into preparatory processes in task-switching, the cognitive processes and neural substrates that support proactive control remain underspecified. This thesis uses a model-based neuroscience approach to define the temporal and spatial characteristics of cognitive processes that contribute to proactive control in task-switching. Using converging evidence from ERPs, a novel multivariate pattern misclassification analysis of EEG data and cognitive modeling, we showed that a switch-specific preparation process is temporally and spatially distinct from more general task preparation for both switch and repeat trials. Consistent with a conflict control mechanism, we show that this switch-specific preparation process is linked to a right inferior frontal source and is related to upward adjustment of response caution in anticipation of more difficult switch trials. We also used fMRI- and DWI-based analyses to examine the neural basis of these cue-related adjustments in response caution, showing that distinct cortico-basal ganglia networks are associated with the ability to flexibly adjust response caution in anticipation of easy or difficult decisions, as well as intrinsic tendencies to set overall response caution high or low. We discuss implications of these findings for our understanding of the organization and timecourse of cognitive control mechanisms.

▼ Introduction Orbital and intraocular masses are relatively uncommon and it is often difficult to differentiate malignant orbital masses from benign ones due to their similar clinical presentation and insufficient diagnostic tools. Hence, the purpose of this work was to evaluate the diagnostic performance of multiparametric magnetic resonance imaging (mpMRI) in distinguishing benign from malignant orbital masses. Material and methods This prospective IRB approved study with written informed consent included 65 patients. mpMRI at 3 Tesla including sAI (=standard anatomic imaging), DWI (=diffusion-weighted imaging) and DCE (=dynamic contrast enhanced MR imaging) was performed. Apparent diffusion coefficient (ADC) was calculated for each mass. To assess the capillary leakage parametric maps were generated for obtaining the perfusion parameters including Ktrans (= volume transfer constant between blood plasma and extravascular extracellular space (EES)), kep (= efflux rate constant from EES to blood plasma), ve (= EES volume per unit volume of tissue) and iAUC (= initial area under the gadolinium concentration curve during the first 60 s). Time-signal intensity curves were recorded to determine the curve pattern. Two radiologists estimated the likelihood of malignancy on a five-point Likert scale in two separate reading sessions (initially only sAI, afterwards sAI + either DWI or DCE). Data were analyzed using Mann-Whitney U test and Pearson Chi² test. Results 33 patients had malignant orbital masses and 32 patients had benign orbital masses (reference standard histopathology in 35 cases and clinical follow-up in 30 patients). The mean ADC of malignant masses differed significantly from the mean (SD) ADC of benign masses (825.4 [437.2] x 10-6 mm²/sec and 1257.8 [576.8] x 10-6 mm²/sec, respectively) (p = 0.001). Ktrans, kep and iAUC were significantly higher in malignant masses (p < 0.01). Benign masses demonstrated higher values while malignant lesions more often presented lower values of ve. 25 malignant masses showed a washout pattern and 21 benign masses showed a persistent pattern (p < 0.01). The reading of sAI resulted in an only moderate specificity and an unsatisfying sensitivity. Adding DWI and DCE images improved specificity and sensitivity considerably. Conclusion DWI and DCE MR imaging are helpful tools in differentiating malignant from benign orbital masses and should therefore be included in the routine diagnostic approach of orbital masses. ▼ Background: Angiogenesis is the process by which new blood vessels develop from existing vasculature and is a critical step in all tumours to facilitate growth beyond a few millimetres. As this process is largely inactive in physiological circumstances in adults, it represents an attractive therapeutic target in oncology. Drugs that target the angiogenic process are classified as anti-angiogenic agents. The first anti-angiogenic drug to be approved by the FDA was bevacizumab; a recombinant humanized monoclonal antibody against VEGF. Randomised studies in colorectal cancer (and other solid malignancies) have reported prolonged progression free survival and overall survival for bevacizumab. However, standard radiological criteria, Response Evaluation Criteria In Solid Tumours (RECIST), although widely employed to assess response to therapy in clinical trials, are generally insensitive to the predominantly cytostatic effects of anti-angiogenic and other targeted therapies. Alternative methods of predicting or assessing early response to such agents are needed, particularly given the cost and toxicity implications of such treatments. However, biomarkers to aid selection of patients for anti-angiogenic therapies, including bevacizumab, remain elusive. Purpose: To investigate Dynamic Contrast Enhanced Magnetic Resonance Imaging (DCE-MRI), Diffusion Weighted Imaging (DWI) and circulating angiocytokines, measured using an ELISA multiplex, as prognostic markers in patients with metastatic colorectal cancer treated with bevacizumab and chemotherapy. Results: Seventy patients were treated. DCE-MRI and DWI parameters showed good reproducibility with coefficient of variation between 3.7 to 23% for parameters. The median progression free survival, the primary end point of the trial, was 9.3 months. The overall response rate was 44%. The clinical variables which were significant for progression free survival on univariate analysis were: performance status (p=0.005), CEA (p=0.04) and serum LDH (p=0.005). Biomarkers which were significant for progression free survival on univariate analysis were serum VEGF-A (p=0.02), serum HGF (p=0.005), sVEGFR-2 (p=0.02). In each case, low values of the biomarker were associated with improved outcome. Multivariate analysis identified Ktrans (p=0.015), performance status (p=0.008) and serum HGF (p=0.003) as the most significant predictors of progression free survival. A prolonged progression free survival was associated with a good ECOG performance status, high Ktrans and low serum HGF.Conclusions: Whilst these results are encouraging, future work is required to establish whether HGF and Ktrans are prognostic markers for metastatic colorectal cancer and their precise role in the prediction of patients likely to benefit from treatment with bevacizumab.

Purpose: To evaluate which technique among T2 weighted images, Dynamic Contrast Enhanced (DCE) or Diffusion Weighted (DW) MRI or combination of those is best suited for prostate cancer detection and local staging. To evaluate post therapy changes of prostate gland with the above techniques consistent with tumor regression Materials & Methods: Patients with biopsy proven adenocarcinoma of the prostate underwent MR imaging on a 1,5T scanner with a surface phased-array coil prior radical prostatectomy.Patients before, during and after radiotherapy were evaluated. Combined anatomical and functional imaging was performed with the use of T2 sequences, DCE and DW MRI. We compared the imaging results with whole mount histopathology.Results: For the multiparametric approach, significantly higher sensitivity values were obtained compared to each modality alone or any combination of the three modalities Among the three techniques, DCE had the best performance for tumor detection in both the peripheral and the transition zone. High NPV rates (>86%) were obtained for both tumor detection and local staging.Radiotherapy leads to gland shrinking and lower ADC values of cancerous lesions a finding consistent with cureConclusion: Multiparametric MRI (MpMRI) is a valuable tool for tumor detection , local staging and follow up after therapy (surgery ,radiotherapy)

ΣΚΟΠΟΣ : Σκοπός της παρούσας διδακτορικής διατριβής ήταν η μελέτη ασθενών με διαγνωσμένο καρκίνο του προστάτη με νεότερες τεχνικές απεικόνισης (διάχυση (DWI), αιματική διήθηση), η συσχέτιση των απεικονιστικών με τα παθολογοανατομικά ευρήματα καθώς και η λειτουργική μελέτη του αδένα κατά τη διάρκεια της θεραπείας με σκοπό την εκτίμηση της ανταπόκρισης σε αυτή. Επιπρόσθετα η σύγκριση των παραμέτρων της πολυπαραμετρικής μαγνητικής τομογραφίας δηλαδή των ακολουθιών Τ2 προσανατολισμού, των ακολουθιών διάχυσης, της δυναμικής εξέτασης ή του συνδυασμού τους για την καλύτερη διάγνωση της κακοήθειας στον προστάτη.Σαν ελάχιστες προυποθέσεις για την πραγματοποίηση πολυπαραμετρικής Μαγνητικής Τομογραφίας είναι ο συνδυασμός ακολουθιών Τ1 και Τ2 προσανατολισμού με ακολουθίες διάχυσης (DWΙ) ή δυναμική εξέταση μετά χορήγηση παραμαγνητικής (dynamic contrast-enhanced MRΙ). ΥΛΙΚΟ-ΜΕΣΑ : Κατά τη διάρκεια των τελευταίων ετών της παρούσας μελέτης εξετάστηκαν στο Β’ Εργαστήριο Ακτινολογίας ασθενείς με υψηλά επίπεδα ειδικού προστατικού αντιγόνου (PSA) ορού αίματος στους οποίους είχε τεθεί η διάγνωση του καρκίνου του προστάτη με διορθική βιοψία. ▼ The aim of this study was to optimize the image quality of magnetic resonance imaging (MRI) of the eye and orbit and to evaluate the clinical benefit of innovative pulse sequences. The application of various coils and coil combinations was evaluated on phantoms and 20 patients. Small surface coils were found to yield the highest signal-to-noise ratio (SNR) in the eye and orbit (Original papers 1 and 2). Adding the head coil for eye examinations should be avoided as coupling effects decrease the SNR and therefore may adversely affect image quality. The highest average SNR of 13.1 was achieved with the smallest surface coil (4 cm) at 3 Tesla, followed by 1.5 Tesla (9.6). Both quantitative and qualitative results indicate that the surface coil has a higher influence on image quality than magnetic field strength, showing that optimal image quality can also be achieved at lower field strength (1.5 Tesla) when a small surface coil is used (Original paper 2). High-resolution T1- and T2-weighted datasets of 62 patient eyes were used to develop a partial least square (PLS) regression model, which establishes a statistical relationship between biometric parameters of the eye and lens size. The lens size cannot be determined precisely with conventional methods preoperatively although it is crucial in cataract surgery (Original paper 3). The use of diffusion-weighted imaging (DWI) sequences in uveal melanoma for the first time enabled quantifiable differentiation from retinal detachment, which is frequently associated with this malignancy. Quantitative analysis of DWI datasets of 40 uveal melanomas revealed a mean apparent diffusion coefficient (ADC) of 891 (172) * 10-6 mm2 / s (Original paper 4). Furthermore, dynamic contrast- enhanced (DCE) MRI was used to quantify the signal behavior of uveal melanoma after contrast medium administration. This was the first study in a large population (n = 40) demonstrating that uveal melanomas show similar dynamic contrast behavior as other malignancies with a rapid initial increase in signal intensity followed by washout or a plateau phase. Moreover, we observed that patients with significantly higher signal intensity after contrast medium administration had a poorer clinical outcome (liver metastases or extraocular tumor spread) (Original paper 5). The benefit of multiparametric MRI (DWI and DCE) in differentiating benign and malignant orbital masses was demonstrated in a prospective study including 65 patients. Benign and malignant orbital lesions had statistically significant different ADC values (1258 (577) * 10-6 mm2 / s for benign lesions versus 825 (437) * 10-6 mm2 / s for malignant lesions; P = 0.001). Analysis of the DCE datasets also revealed significant differences, showing significantly higher Ktrans, kep, and iAUC values for malignant lesions (p <0.01). Interobserver variability among the radiologists classifying the lesions improved markedly when DWI and DCE were added (κ = 0.644). ▼ The importance of cerebral magnetic resonance imaging (MRI) in acute stroke has increased during the last years. The presented papers focus on improvement of the diagnostic workup of specific MRI sequences for additional information in acute stroke and on detecting risk factors for cerebral ischemic lesions in marathon runners. The diagnostic impact of specific vessel signs on MRI for prognostic and therapeutic questions is increasingly discussed. We evaluated the association of distal cortical vessel signs on stroke FLAIR MRI (FLAIR vascular hyperintensities; FVH) with infarct size and morphology and the clinical impairment of 84 patients with symptomatic internal cerebral artery (ICA) or middle cerebral artery (MCA) occlusion. FVH were detected in 75 (89.3%) patients with a median number of 4 [IQR 2-7]. Patients with FVH>4 had larger initial DWI lesions (p=0.008), perfusion deficits (p=0.001) and final infarction volumes (p=0.005) than patients showing less or equal than 4 FVH on admission. The initial perfusion-diffusion-mismatch (“tissue-at-risk”) was larger (p=0.005). We calculated a “rescue ratio” Q, representing the part of the “tissue-at-risk” that becomes necrotic after the initial imaging up to the final infarction volume, to set the size of initial mismatch in relation to the infarct volume. There was a positive correlation between the number of FVH and the “rescue ratio” Q (r=0.404; p=0.016), possibly showing a protective effect of FVH on the infarct evolution, may be implicating FVH as collateral supply. Because of our small number of patients and without DSA control as gold standard for detection of collateral vessels, we can’t finally prove this hypothesis with our data. Patients with FVH>4 and larger infarction volumes showed similarly higher NIHSS scores on admission and on discharge (p=0.021; p=0.093) implicating more severe strokes. In a second work we evaluated the validity of negative high-resolution DWI (hrDWI) in patients with TIA. The classical clinical definition of TIA has recently been challenged because of diffusion-restricted areas in MRI. A follow-up imaging 24 hours after admission showed 11% (7/63 patients) false-negative hrDWI. Indication to follow-up imaging for detection of those infarctions can be guided by persistence of neurologic deficits, measured by NIHSS, longer than 24 hours after onset (OR 17.5; 95% CI 2.83-108.12; p=0.002). Concerning the Berlin Beat of Running study we examined 110 experienced marathon runners to detect cardiac arrhythmias and associated silent brain lesions in context of the 38th BMW Berlin marathon 2011. The final results are expected during 2013. They might give information on individual risk factors of marathon runners for exercise associated cardio- and cerebrovascular events to protect exposed athletes.
