▼ In dit onderzoek wordt bekeken wat de invloed is van spelfouten in sollicitatiebrieven en sponsorbrieven op de tekstwaardering, overtuigingskracht en het imago van de zender. Uit eerdere wetenschappelijke onderzoeken en scripties weten we dat lezers van sollicitatiebrieven letten op spelfouten. Lezers van andere soorten brieven letten niet of minder op spelfouten. Vanwege een verschil in materiaal en vragenlijsten zijn de resultaten van eerdere onderzoeken echter niet goed te vergelijken. Daarom werden in dit onderzoek twee zoveel mogelijk op elkaar lijkende brieven met zes spelfouten en zonder spelfouten opgesteld. In hoeverre is er een verschil in beoordeling tussen sollicitatie- en sponsorbrieven met spelfouten? Het type spelfout dat in dit onderzoek centraal staat is een ten onrechte weggelaten –t bij de stam van een werkwoord in de derde persoon enkelvoud (‘hij vind’). Er is een sollicitatiebrief en een sponsorbrief van vijf alinea’s lang geschreven met fictieve bedrijfsnamen en personages. Van elke brief werden vier versies gemaakt: een foutloze versie van schrijver Anna de Jong, een foutloze versie van schrijver Peter van der Berg, een versie met zes spelfouten van schrijver Anna de Jong en een versie met zes spelfouten van schrijver Peter van der Berg. Proefpersonen schreven na het lezen van één van de brieven ieder maximaal vijf gedachten op die ze hadden tijdens of direct na het lezen van de brief. Daarna beantwoordden zij vragen over de afhankelijke variabelen. Er deden 160 werknemers van organisaties in allerlei bedrijfstakken aan dit onderzoek mee. Van alle proefpersonen was 45 procent man en 55 procent vrouw. De gemiddelde leeftijd van alle proefpersonen was 43 jaar. Uit dit onderzoek blijkt dat spelfouten een negatieve invloed hebben op de tekstwaardering, overtuigingskracht en het imago van de zender. Deze invloed is bij splitsing van de proefpersonen op kennis van spelling het sterkst voor de groep met kennis van spelling. Proefpersonen waarderen een tekst met spelfouten minder positief dan een foutloze tekst. Zij vinden een brief met spelfouten ook minder overtuigend dan een foutloze brief. De meerderheid van de proefpersonen heeft bij brieven met en zonder spelfouten geen gedachten over de schrijver of deze in ieder geval niet opgeschreven. Wordt de schrijver wel genoemd, dan is dit vooral door lezers van een brief met spelfouten. Kenners van spelling vinden de schrijver van een brief met spelfouten minder deskundig, minder betrouwbaar en minder aantrekkelijk dan de schrijver van een foutloze brief. Het oordeel van de niet-kenners staat hier haaks op. Zij beoordelen het imago van de schrijver van een brief met spelfouten juist positiever dan het imago van de schrijver van een foutloze brief. Alle proefpersonen vinden de sollicitatiebrief begrijpelijker dan de sponsorbrief. Het negatieve effect van spelfouten geldt voor kenners vooral voor de sponsorbrief en juist minder voor de sollicitatiebrief. ▼ Obesity is increasingly considered as an inflammatory disorder. In adults, obesity induces inflammation of adipose tissue (AT). Through the release of inflammatory lipids and immune mediating proteins called adipokines, AT inflammation spreads to other tissues ranging from liver and muscle to the cardiovascular system and circulating immune cells, and drives low-grade systemic inflammation. The low-grade systemic inflammation provides a pivotal link between obesity and its metabolic and cardiovascular sequelae. In a recent Diabetologia paper, we showed that already in obese children, adipose tissue (AT) inflammation occurs (1). Interestingly, the severity of AT inflammation correlates with clinical parameters for insulin resistance. Furthermore, we showed that AT inflammation in childhood obesity drives low-grade systemic inflammation, and affects circulating immune cells such as monocytes, which are known to be involved in the development of atherosclerosis later in life. Taken together, our results suggest that modulating AT inflammation in childhood obesity provides a novel avenue for the prevention of long-term metabolic and cardiovascular disease. Next, we discovered a novel pathway to modulate AT inflammation. As recently published in JCI (2), we showed that AT-resident immune cells such as invariant natural killer T (iNKT) cells play a pivotal role in maintaining healthy adipose tissue. Through direct interplay with lipid-presenting adipocytes, iNKT cells suppress AT inflammation, and prevent systemic inflammation and insulin resistance in mice and humans. Knockout or antibody-mediated depletion of iNKT cells in mice has a devastating impact on AT inflammation and insulin resistance, while ligand-mediated stimulation of iNKT cells prevents metabolic derailment in obesity. Summarizing, our results show that modulation of iNKT cell function may prevent metabolic and cardiovascular complications in obese patients, including children. In conclusion, considering childhood obesity as an inflammatory disorder offers a novel perspective on the prevention of long-term metabolic and cardiovascular complications. ▼ Dit onderzoek trachtte na te gaan, wat de invloed is van het foutief niet toevoegen van een -t aan de stam van een werkwoord, in de derde persoon enkelvoud. De verwachting was dat het aantal fouten hierbij een belangrijke rol zou spelen. Volgens Jansen (2009) wordt een enkele spelfout de schrijver vergeven, en treedt er pas een negatief effect op wanneer dezelfde spelfouten systematisch worden gemaakt, zodat de lezer gaat beseffen dat de schrijver de spellingsregels onvoldoende beheerst. Op basis van deze verwachting, is allereerst door middel van een kwalitatief onderzoek vastgesteld bij hoeveel spelfouten lezers beseffen dat de schrijver systematisch dezelfde fout maakt. Vijftien proefpersonen lazen de tekst van een wijkfolder met daarin vijftien spelfouten hardop voor, en spraken hierbij hun gedachten uit. Tien personen wisten de fouten in de tekst niet te ontdekken, en vijf personen detecteerden de fouten wel. Op basis van de uitspraken van deze personen is vastgesteld dat lezers bij vier fouten nog niet beseffen dat steeds dezelfde regel wordt geschonden, en dat ze bij acht fouten wel door hebben dat de schrijver een ontoereikende regelkennis heeft. Op basis van de gegevens uit het eerste onderzoek, is het tweede onderzoek opgezet. Dit kwantitatieve onderzoek trachtte na te gaan, wat de invloed is van foutfrequentie op tekstwaardering, overtuigingskracht, imago en leesgemak. Van dezelfde wijkfolder werden drie versies gemaakt, een conditie met nul fout, een conditie met vier fouten en één met acht spelfouten. In totaal lazen 173 mensen één van deze versies, en beantwoordden ze aansluitend verschillende vragen over de tekst. De resultaten laten zien dat proefpersonen die de tekst met acht fouten lazen, de schrijver slordiger vinden dan de mensen die de tekst met nul of vier fouten lazen. Ook blijkt dat de lezers van de tekst met acht fouten meer gedachten hadden over de schrijver. Dit resultaat komt overeen met de bevindingen van Areni & Sparks (2005), die concludeerden dat lezers meer gedachten hebben over de schrijver bij het lezen van een tekst met powerless language, dan bij een tekst met powerful language. D/t-fouten zouden ook gezien kunnen worden als een kenmerk van powerless langugage; het zijn oppervlakkige tekstkenmerken die afleiden van de verwerking van de boodschap, en geen inhoudelijke bijdrage leveren. Hiernaast lijken d/t-fouten te fungeren als perifere cue: het aantal gedachten over de schrijver is groter bij meer d/t-fouten, wat betekent dat er minder vaak een kritische afweging van de argumenten plaatsvindt, omdat er meer aandacht is voor andere factoren. Verwerking van de boodschap vindt dan plaats via de perifere route (Petty & Cacioppo, 1986; Petty & Wegener, 1999). Er werden verder geen verschillen gevonden tussen de drie condities; het blijkt dat d/t-fouten vrijwel geen invloed hebben op tekstwaardering, imago, overtuigingskracht en leesgemak. ▼ FOXP3+ regulatory T cells (Treg) control T cell activation and effector functions and are key players in immune homeostasis and self-tolerance. Given their central role in preventing autoimmune responses, Treg are considered important targets for the treatment of autoimmune inflammation and several strategies are being explored to enhance Treg numbers or function for the treatment of autoimmune disease. In addition, numerous research groups have studied the presence, phenotype and function of Treg in patients with autoimmune disease. Whether deficiencies in Treg underlie human autoimmune pathology, however, is still a subject of debate. In this thesis we investigated Treg numbers and function in patients with juvenile idiopathic arthritis (JIA), one of the most common autoimmune diseases in children, characterized by chronic inflammation of the joints. In these patients Treg from the site of inflammation can be studied, because during treatment synovial fluid is taken from inflamed joints from which immune cells can be isolated. With this approach we established that Treg are not deficient in JIA patients and are even enriched at the site of autoimmune inflammation. Furthermore, Treg from inflamed joints display efficient suppressive capacity. Still, effector T cells (Teff) from the site of inflammation are insufficiently controlled, because these cells are resistant to suppression. This unresponsiveness to regulation has now been shown for Teff from patients with other autoimmune diseases as well, including type 1 diabetes and inflammatory bowel disease, indicating that it is a general mechanism underlying autoimmune pathology. As a consequence, Teff resistance to suppression might be an attractive target to treat autoimmune inflammation. However, to effectively target unresponsiveness to suppression the underlying mechanism needs to be clarified. Our experiments using JIA patient material demonstrate that Teff resistance to suppression is, at least partially, caused by PKB/c-akt hyperactivation. Teff from inflamed joints display enhanced PKB/c-akt activation and treatment with a specific PKB/c-akt inhibitor restores responsiveness of the cells to suppression. Pro-inflammatory cytokines TNFa and IL-6, which are highly present in inflamed joints, are involved in inducing this PKB/c-akt activation and subsequent resistance to suppression. As a result, inhibition of TNFα, and to lesser extend IL-6, reduces PKB/c-akt activation in inflammatory Teff and enhances their responsiveness to suppression. In addition, we show that pro-inflammatory monocytes present at the site of autoimmune inflammation contribute to impaired T cell regulation as well. The data described in this thesis indicate that targeting Treg for the treatment of autoimmune disease might be less efficient than initially anticipated. Instead, restoring responsiveness of Teff to suppression, either directly, by targeting PKB/c-akt, or indirectly, by interfering with the pro-inflammatory environment, might hold greater promise. ▼ AD skin is characterized by the infiltration of CD4+ T cells in the dermis and epidermis. Chemokines (chemoattracting cytokines), including TARC and CTACK play a pivotal role in the recruitment of T cells to the skin of AD patients. In chapter two we describe that the serum levels of chemokines TARC and CTACK are increased in AD patients compared to patients with allergic asthma and healthy controls. In addition we show that the serum levels correlate to disease severity. We put serum TARC levels forward as an objective parameter for disease severity. When other treatments fail, patients with severe refractory AD are regularly being treated with cyclosporin A (CsA). In chapter three we show that CsA treatment is highly effective and relatively safe, with regard to the most common side effects, including increased blood pressure and renal toxicity. However, patients regularly experience a relapse after discontinuation of therapy. In addition, in chapter four we describe a small subset of AD patients that experience an exacerbation during CsA treatment with signs and symptoms more severe than at onset of treatment. Increasing total serum IgE levels were found to parallel these exacerbations of eczema. Increasing IgE production had previously been described using in vitro experiments. We hypothesize that in this subset of AD patients CsA induces a shift towards Th2, resulting in increased IgE synthesis. In order to study the in vivo effects of CsA treatment, in chapter five we performed a detailed study of the effects of CsA treatment on peripheral blood T cell subsets with a focus on regulatory T cells (Tregs). We show that CsA treatment significantly reduces the percentage of Tregs. In addition, it was found that CsA treatment decreases the expressions of genes FOXP3 and GADD45A, involved in T cell regulation. Using a microarray approach we have found several genes that are suggested to be involved in the pathogenesis of AD. Chapter six describes two groups of genes found differentially expressed in CD4+ T cells from AD patients compared to allergic asthma patients and healthy control subjects. The first group included genes involved in migration of T cells to the skin, a second group of genes was related to T cell survival. In chapter seven we examined the expression of these genes in the preferentially skin homing, CLA-positive T cell population. These skin homing T cells were found to show decreased expression of apoptosis related genes, suggesting their increased survival potential in patients with AD.

▼ Naar schatting zijn er volgens de Stichting Lezen & Schrijven anderhalf miljoen mensen in Nederland laaggeletterd. Sinds een paar jaar is het inzicht in het belang van anticipatie op en herkenning van laaggeletterdheid steeds groter. In de gezondheidszorg hebben we ook te maken met laaggeletterde patiënten. Deze mensen zijn vaak onvoldoende gezondheidsvaardig. Gezondheidsvaardigheid houdt in dat iemand informatie over gezondheid kan begrijpen, gebruiken en kritisch kan analyseren. Afgelopen jaar is er een aantal instrumenten die gezondheidsvaardigheid beogen te meten vertaald naar het Nederlands. Deze zijn nog niet compleet gevalideerd. Voor het project "Laaggeletterde patiënten” in het UMC Utrecht ben ik nagegaan in hoeverre één van de vertaalde instrumenten gezondheidsvaardigheid meet bij patiënten op de Polikliniek voor Pre-Operatieve Screening. Het instrument dat ik hiervoor gebruikte was de Rapid Estimate of Adult Literacy in Medicine (REALM). De REALM blijkt geen inzicht te geven in hoezeer men informatie over gezondheid weet te verkrijgen, toe te passen en kritisch weet te analyseren. Gezondheidsvaardigheid is dus een overgewaardeerde term. De term functionele gezondheidsvaardigheid past beter. Dit houdt in dat iemand medische informatie begrijpt doordat hij voldoende basisvaardigheden heeft zoals lezen en schrijven. De REALM meet technische leesvaardigheid. Dit is niet direct het begrip van informatie over gezondheid. Bij het instrument worden lezers immers gevraagd om geschreven medische termen te verklanken naar gesproken woorden. Eerdere onderzoeken die inzichten beogen te geven in functionele gezondheidsvaardigheid door middel van directe begripsmetingen lieten wel correlaties zien met de REALM. In het huidige onderzoek is er in eerste instantie nagegaan of er een relatie was tussen de scores op de vertaalde REALM en de scores op een begripsmeting over een bestaande folder met informatie over sedatie onder vijftig patiënten op de Polikliniek voor Pre-Operatieve Screening (POS) in het UMC Utrecht. Bij een tweede groep van vijftig patiënten op de POS-poli zijn dezelfde metingen gedaan, maar hier is dezelfde folder zoveel mogelijk in begrijpelijk Nederlands geschreven. De reden hiervoor was te kijken of de moeilijkheid van de tekst invloed heeft op de onderzoeksresultaten. Uit het onderzoek bleek dat er een verband was tussen de scores op de Nederlandse versie van de REALM en de begripsmeting over zowel de bestaande als de vereenvoudigde folder. Hoe meer moeite men had met het uitspreken van de woorden van de REALM, hoe meer moeite men had met het beantwoorden van de vragen die gingen over beide folders en hoe langer men er over deed het correcte antwoord te vinden. Dit houdt in dat de vertaalde REALM daadwerkelijk functionele gezondheidsvaardigheid meet. Bij de vereenvoudigde folder waren er meer verbanden zichtbaar dan bij de bestaande folder. Functionele gezondheidsvaardigheid gemeten door de vertaalde REALM zegt dus eerder iets over de vaardigheden ten aanzien van het begrip van informatie dat zoveel… Advisors/Committee Members: Janssen, D.M.L..

▼ Heat shock proteins (HSP) are highly conserved proteins with various identities and intracellular functions. Most HSP are cell stress proteins, meaning that their intracellular transcription is up regulated upon noxious stimuli such as heat, hypoxia and inflammation. As intracellular chaperones they protect other intracellular proteins from denaturation under these circumstances. When cells are damaged, HSP are also found in the extracellular fluid, due to secretion and leakage from cells. Interestingly, HSP are important antigens for the immune system, capable of inducing both innate and adaptive immune responses. Although HSP were originally categorized as danger signals for the immune system, evidence on regulatory effects of class II presentation of HSP-peptides on T cells has accumulated. The experimental work presented in this thesis is aimed at studying T cell responses to peptides of HSP in the human immune system, with focus on autoimmune diseases. Three HSP families are discussed in this thesis: dnaJ (HSP40), HSP60 and HSP70. DnaJ. We present the first phase II clinical trial on mucosal tolerance induction with the HSP-peptide dnaJp1 in RA. Daily oral administration of peptide dnaJp1 resulted in a decrease in disease activity, without major side effects. We found a change in T cell responses to the peptide after 168 days of therapy with a significant decrease in TNFα production of CD4+ T cells and a non-significant increase in IL10. A higher gene expression of immune tolerance molecules in PBMC after incubation with dnaJp1 before initiation of the trial correlated with a better clinical response. HSP60. We found that CD4+ T cells from synovial fluid of Rheumatoid Arthritis (RA) patients respond to the self-HSP60-peptide p2. Interestingly, these responses were only apparent when CD4+CD25+ T cells were depleted from the culture. When CD4+CD25- synovial T cells were cultured with peptide p2, CD25 was up regulated on the cell surface and the resulting CD25high cells were functionally suppressive in vitro, suggesting an immune regulating effect. HSP70. We performed a literature search for HSP70-peptides eluted from HLA on human APC. We detected proliferation and cytokine production by healthy donor CD4+ T cells towards a selection of these naturally presented HSP70 peptides. This demonstrated that HSP70-specific T cells are present in the peripheral blood of healthy donors. We then turned to a tissue where HSP70 has a well-defined crucial function in the protection of its cells: the muscle. We showed that the presence of extracellular HSP70 is increased in the muscle of patients with the inflammatory myopathy Juvenile Dermatomyositis (JDM). We found that one of the selected HSP70 peptides (H443) induced TNFα, IFNγ and IL17 production by peripheral blood T cells from JDM patients.
