▼ INTRODUÇÃO: As doenças inflamatórias intestinais têm chamado a atenção da comunidade científica pela multiplicidade de manifestações no trato digestivo, manifestações extra-intestinais e pela sua incidência em ascensão. Existe uma grande prevalência de manifestações extra-intestinais em portadores de doença de
Crohn e de retocolite ulcerativa, variando na literatura de 24 a 65%. Estas alterações podem surgir antes dos sintomas intestinais, concomitante ou ulteriormente, podendo ou não ter relação com a atividade da doença intestinal. O conhecimento destas manifestações extra-intestinais assim como seu quadro clínico, evolução e tratamento é importante, devido ao aumento da morbidade e mortalidade desencadeada por elas. OBJETIVO: Objetivou-se neste trabalho determinar a prevalência de manifestações extra-intestinais em retocolite ulcerativa e doença de
Crohn, correlacionando com diagnóstico do tipo de doença inflamatória intestinal, extensão, tempo de evolução e aparecimento dos sintomas, sexo e atividade da doença. MÉTODOS: Os pacientes que participaram do estudo estão cadastrados no Ambulatório de Doenças Inflamatórias do Serviço de Cirurgia do Cólon, Reto e Ânus do Departamento de Gastroenterologia do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo de setembro de 1984 até setembro de 2004. Os dados para a pesquisa foram colhidos retrospectivamente de mil protocolos que foram preenchidos em caráter prospectivo na primeira consulta do doente e atualizados em consultas subsequentes. Foram estudadas as manifestações articulares, dermatológicas, oftalmológicas, urológicas, hepáticas, pulmonares e vasculares. RESULTADOS: Entre os 1.000 pacientes estudados encontrou-se 468 com doença de
Crohn (46,8%) e 532 com retocolite ulcerativa (53,2%). Foram encontrados 627 pacientes com pelo menos uma forma de manifestação extra-intestinal(315-59,2% com RCUI e 312-66,7% com doença de
Crohn). A média de tempo de duração da doença inflamatória intestinal dos pacientes que tinham manifestações extra-intestinais foi de 10,23 anos, comparado a 7,89 anos daqueles que não possuíam, sendo estatisticamente significativo. Também foi evidenciado que as manifestações extraintestinais surgiram com maior frequencia ulteriormente aos sintomas intestinais da doença. CONCLUSÕES: Na retocolite ulcerativa a maior extensão da doença no cólon foi proporcional a incidência de manifestações extra-intestinais, enquanto na doença de
Crohn, quando havia algum acometimento colônico era maior a incidência destas manifestações. Apenas as manifestações urológicas tiveram uma predisposição maior pela doença de
Crohn, e nestes, no sexo masculino. As manifestações articulares e dermatológicas foram mais prevalentes no sexo feminino tanto em retocolite ulcerativa quanto na doença de

Introduzione. Numerosi fattori, tra cui eventi immunitari, agenti ambientali tossici, predisposizione
genetica e microbiota anormale svolgono un ruolo chiave nella patogenesi della malattia di
Crohn,
ma il principale meccanismo alla base è ancora oggetto di studio. L'omeostasi della mucosa
intestinale si basa su un delicato equilibrio tra auto-rinnovamento e differenziazione, che è garantita
dal ricambio delle cellule staminali intestinali all'interno delle cripte. Recenti studi suggeriscono che
una differenziazione alterata delle cellule staminali intestinali (ISC) può facilitare l'insorgenza della
malattia di
Crohn attraverso la formazione di una barriera antimicrobica difettosa, che consente ai
batteri intestinali di provocare l'infiammazione. Lo scopo del nostro studio è quello di esplorare se le
ISC sono danneggiate e le loro proprietà rigenerative sono alterate nella malattia di
Crohn,
rappresentando così un nuovo attore nell'insorgenza della malattia e un potenziale bersaglio
terapeutico per ripristinare le capacità di auto-rinnovamento della mucosa intestinale.
Metodi. Su campioni intestinali (sia area marginale che infiammata rispetto alla sede della malattia)
ottenuti da pazienti con e senza malattia di
Crohn è stata condotta un’analisi del trascrittoma per
valutare l'espressione dei marcatori per ISC. Le proprietà rigenerative delle ISC sono state studiate
utilizzando il test del mini-guts, in cui le cripte purificate sono state coltivate come organoidi
tridimensionali e la loro morfologia / sviluppo sono stati valutati. Infine, mini-guts sono stati
coltivati a partire dalle cripte di soggetti sani con siero di pazienti affetti da malattia di
Crohn per
dimostrare se i fattori circolanti / citochine potessero esercitare un effetto significativo
nell'abrogazione dello sviluppo di organoidi e influenzare l'espressione dei marcatori per ISC.
Risultati. I marcatori per ISC, in particolare LGR5 e EPHB2, sono scarsamente espressi in cripte
isolate da campioni intestinali di pazienti con malattia di
Crohn. È interessante notare che i
marcatori per ISC sono più preservati nella zona sana, mentre la loro espressione è quasi assente
nell'area infiammata. Ancora più importante, la capacità delle ISC di auto-rinnovamento testata nel
saggio mini-guts, è stata trovata significativamente ridotta nei pazienti con malattia di
Crohn, con
solo il 40% dei mini-guts che si sviluppano da cripte ottenute dall'area marginale e meno del 5% da
quelle isolate dalla parte infiammata del campione intestinale. Quando i mini-guts ottenuti dalle
cripte dei controlli sono stati coltivati in presenza di siero di pazienti affetti da malattia di
Crohn non
riuscivano a generare organoidi normali e mostravano una ridotta espressione dei marcatori per ISC
EPHB2 e LGR5.
Conclusione. Abbiamo dimostrato che il pool delle ISC è ridotto nella malattia di
Crohn e che
questo si associa a una perdita di proprietà rigenerative delle ISC all'interno delle cripte. La crescita

▼ Activation of the aryl hydrocarbon (AhR), a ligand-activated transcription factor present in many immune cells, can trigger immunosuppressive responses through the generation of regulatory cells. Several AhR ligands exist in the diet including environmental contaminants, such as its prototypical ligand 2,3,7,8-tetrachlorodibenzo-pdioxin (TCDD), and natural sources, such as indole-3-carbinol (I3C) and indirubin. Crohn's disease, a chronic inflammatory state of the gastrointestinal tract, affects millions worldwide; however, the exact cause is unknown and treatments do not cure the disease. The role of the AhR in mucosal immune responses is not well understood to date so we aimed to understand the consequences of AhR activation in gut inflammatory responses. The overall hypothesis of this project is that AhR agonists found in the diet have the potential to dampen inflammation associated with Crohn's disease. The first goal of this project was to examine the role of AhR activation in the development and progression of colitis using TCDD and the natural ligand I3C. TCDD suppressed TNBS-induced colitis, as demonstrated by the decreased disease severity and dampened inflammation in the gut. TCDD-treated mice exhibited decreased inflammatory mediator production and increased frequency of regulatory cells, both Foxp3+ Tregs and dendritic cells (DCs) in gut immune tissues. In comparison, I3C suppressed disease severity in females but not males. Sex-specific effects on colonic cytokine production and gene expression were observed. The second goal of this project was to define the cellular mechanisms by which AhR ligands elicit their effects on important immune cell populations present in the gut. Therefore, we investigated the immunomodulatory effects of the dietary AhR ligands I3C and indirubin in bone marrow derived DCs (BMDCs). I3C-and indirubin-treated BMDCs upregulated the expression of immunoregulatory genes, such as ALDH1A, IDO, and TGFâ, and also drove the generation of Foxp3+ Tregs. Following LPS stimulation, I3C-and IO-treated BMDCs suppressed the LPS-induced production of TNF-á, IL-1â, IL-6 and IL-12. Anti-inflammatory effects were also observed in intestinal epithelial cells (IECs) treated with AhR ligands. Thus, I3C and indirubin possess immunosuppressive and anti-inflammatory effects in BMDCs and IECs. Taken together, our data demonstrate that the AhR is a therapeutic target that warrants further investigation, as natural AhR ligands may effectively prevent the onset of chronic inflammatory diseases or more effectively induce and maintain remission when combined with conventional medicine.

▼ Introdução: Aproximadamente 25 a 30% dos pacientes com DC iniciam a doença antes da idade dos 20 anos. O déficit de crescimento é a complicação mais específica da DII pediátrica, causada pela combinação de ingestão calórica inadequada, aumento das perdas calóricas e inflamação ativa persistente da mucosa intestinal. Pacientes com DII comumente sofrem de desnutrição protéico-energética no diagnóstico e com flutuações durante todo o curso da doença. A perda de peso é uma característica comum no paciente com DII recém diagnosticada, particularmente na DC, e acompanha quase todas as recaídas. Aproximadamente 60% das crianças com DC apresentam perda de peso recente no momento do diagnóstico. Objetivos: verificar o estado nutricional de pacientes portadores de DC na adolescência, através da avaliação da composição corporal, dosagem de macro e micronutrientes e consumo alimentar. Casuística e Métodos: vinte e dois pacientes com DC em atividade (leve ou moderada), vinte e nove com DC em remissão e 35 parentes de 1º grau desses pacientes de mesma faixa etária (grupo controle) foram selecionados para participar prospectivamente. Antropometria (peso, estatura e índice de massa corporal (IMC) expressos em z escore, bioimpedância e estágio de Tanner), níveis séricos e a ingestão de macro e micronutrientes foram avaliados. Resultados: os adolescentes com DC em atividade seguidos pelo DC em remissão tiveram menor valor de escore z da estatura/idade e IMC para a idade, da massa magra e do estágio pubertário (p<0,05) do que o grupo controle. Os pacientes com DC apresentaram alterações significativas da qualidade da ingestão alimentar, principalmente calorias, proteínas, fibras e micronutrientes, refletindo nas dosagens séricas principalmente das vitaminas A e E (p<0,05). Conclusão: Adolescentes com DC, mesmo quando na fase de atividade da doença leve e inativa, apresentam riscos nutricionais, reforçando a importância da avaliação nutricional
Introduction: About 25 to 30% of patients with Crohns disease begin before age 20 years. The growth deficit is the most specific complication of pediatric CD, caused by a combination of inadequate energy intake, increased loss of calories and persistent active inflammation of the mucosa intestinal. ▼ The Crohns disease is a chronic, inflammatory, multisystem bowel disorder with a relapsing course. Her etiology remains unknown but probably theres a scenario characterized by inibition of innate immunity and exacerbation of the adaptative immunity with exacerbated mucosal inflammatory activity. Then, the corticosteroids have been the mainstay of pharmacologic treatment for inducing clinical remission in the disease. However, the prohibitive side effects of prolonged corticosteroid therapy associated with high frequency of the corticosteroid- dependency in the disease taken to alternative treatments for long- term maintenance of clinical remission. The immunomodulators, especially, the purine analogs, azathioprine(AZA) or 6- mercaptopurine are the major option for this indication, with corticosteroid- sparing proprieties. The use of these agents is supported by randomized, controlled clinical studies. However, these trials have not evaluated the specific issue of the steroid dependence. Thus, our objectives in this study were to asses the efficacy and safety of AZA therapy in patients strictly with steroid-dependent Crohn`s disease and possibly, factors associated with sustained clinical remission. Therefore, 106 adults patients with steroid-dependent Crohn`s disease were prospectively included for treatment with AZA (2-3mg/Kg/day) during the period of follow up as long as 10 years. The proportion of patients remaining in sustained steroid-free remission at 12, 24, 36, 48 and 60 months was 0.61, 0.73, 0.72, 0.70 and 0.70, respectively. Thereafter, the annual rate of weaning from steroids decreased gradually, reaching a nadir of 0.41 at 108 months. Median time to complete steroid withdrawal was 6 months. Demographics, disease-related data and the AZA dose did not correlate with sustained remission. Only the reduced mean leukocyte count during the follow up was associated with remission free steroids (P= 0.01). Serious adverse effects related to AZA therapy were uncommon
A doença de Crohn é uma afecção intestinal inflamatória crônica, multissistêmica e de caráter recorrente. Sua etiopatogênese permanece ainda desconhecida, mas considera-se haver um cenário fisiopatológico caracterizado, principalmente, pela inibição da imunidade inata com exacerbação da imunidade adaptativa, com atividade inflamatória mucosa intensificada. Por isso, os corticosteróides constituem a base fundamental do tratamento farmacológico na indução da remissão clínica na doença, graças às suas potentes ações como supressores do processo inflamatório. Entretanto, os proibitivos efeitos colaterais da corticoterapia prolongada associados à alta freqüência da cortico-dependencia na doença estimulou a busca de alternativas terapêuticas para manutenção da remissão no longo prazo. ▼ A doença de Crohn (DC) é uma doença crónica de etiologia complexa, envolvendo fatores ambientais, genéticos e imunológicos. A DC apresenta um quadro clínico complexo que inclui alterações intestinais e extra-intestinais e, entre estas últimas, estão as manifestações orais que são classificados em lesões específicas e não específicas. Estas lesões são variadas incluindo úlceras, hiperplasia gengival, aftas, entre outras e afetam essencialmente a mucosa, gengiva, lábios, área vestibular e retromolar. As lesões orais podem não ser derivadas da própria patologia mas, também, do uso prolongado de fármacos por estes pacientes e pela sua deficiente absorção de nutrientes. Por estas razões, o paciente com DC necessita de uma atenção própria do médico dentista, especialmente durante os períodos de exacerbação da doença. A terapia farmacológica da DC inclui imunossupressores, corticosteróides, e aminossalicilatos, que são utilizados para o controlo dos sintomas intestinais, o que, regra geral, é também suficiente para o tratamento das manifestações orais.
Crohn's disease (CD) is a chronic disease of complex etiology, involving environmental, genetic and immunological factors. CD presents a complex clinical picture that includes intestinal and extra-intestinal manifestations, and these last ones include oral manifestations that are classified into specific and non-specific lesions. The oral lesions are several including ulcers, gingival hyperplasia, aphtha’s, between others affecting mainly the mucosa, gingiva, lips, vestibular and retromolar areas. Other oral lesions are those caused by the prolonged use of drugs by this patients and nutrients malabsorption. For these reasons the patient with CD needs more attention from the medical dentist, especially during periods of exacerbation of the disease. Pharmacological therapy of Crohn Disease includes immunosuppressant’s, corticosteroids and aminosalicylates, which are used to control intestinal symptoms and also are generally sufficient for the treatment of oral manifestations.

Vastaavasti Crohnin taudin tyyppioireita ovat vatsakipu ja ripuli. Nämä etiologialtaan tuntemattomat suoliston krooniset tulehdussairaudet ovat lisääntyneet viimeisten vuosikymmenten aikana etenkin länsimaissa. Geneettisen alttiuden lisäksi sairauden puhkeamiseen vaikuttavat esimerkiksi ympäristötekijät. Etenkin lapsilla kroonisiin tulehduksellisiin suolistosairauksiin sairastuminen voi vaikuttaa laajalti kehitykseen aina pituuskasvusta elämänlaatuun.
Tämän väitöskirjatutkimuksen tavoitteena oli selvittää lasten IBD:n ilmaantuvuus Suomessa vuosien 1987 ja 2003 välillä. Tavoitteena oli lisäksi tutkia ympäristötekijöiden vaikutusta ilmaantuvuuden alueellisiin eroihin. Lasten kroonisten suolistosairauksien taudinkuvaa, käytettyjä hoitomuotoja ja sairauden vaikutusta elämänlaatuun selvitettiin sairauskertomusten ja kyselytutkimuksen perusteella.
Tulehduksellisten suolistosairauksien ilmaantuvuus kolminkertaistui suomalaisen lapsiväestön keskuudessa tutkimusjakson aikana. Alueilla, joissa on matala lapsiväestöntiheys, havaittiin suuremmat ilmaantuvuusluvut verrattuna alueisiin, joissa lapsiväestöntiheys on korkea. Maanviljelijöiden määrällä, merenrannan läheisyydellä, paperitehtaiden sijainnilla tai vesijohtoveden kemikaalipitoisuuksilla ei todettu olevan merkittävää vaikusta ilmaantuvuuden alueellisiin eroihin. Lapsilla taudinkuva on usein aggressiivinen; jopa joka viides lapsipotilas joutui leikkaukseen. Elämänlaadun havaittiin olevan alentunut varhaisaikuisuudessa verrattuna terveisiin ikätovereihin. Tulevaisuudessa onkin tärkeää selvittää lasten tulehduksellisten suolistosairauksien ilmaantuvuuden nopean kasvun taustalla olevia tekijöitä.; Background:
The incidence of inflammatory bowel disease (IBD) has been increasing among children especially in Western countries over the past decades. Crohn’s disease (CD) predominates over ulcerative colitis (UC) in most countries. The factors behind this increase are mainly unknown. Environmental factors influence, however, unquestionably the pathogenesis of IBD. The disease course in pediatric IBD is suggested to be more severe compared to adults which may have particularly impact on child development. The possible long-term sequelae of pediatric onset disease include growth impairment, risk of surgery and reduced quality of life.
Objectives:
This epidemiological study aimed to evaluate the incidence of IBD among Finnish children between the years 1987-2003. The role of environmental determinants (pediatric population density, agricultural industry, paper mills or sea shore close to the place of residence, north-south gradient and chemical concentrations of tap water) in the spatial variations of pediatric IBD was explored. Crohn is an inflammatory bowel disease (IBD). The disease is a result
of genetic disposition, immunologic disorders and environmental influences as
much as differences of the intestinal mucosa. On the immunologic site, there
is an imbalance between immunologic response and tolerance against the
microbiom. Regulatory t cells play an essential role sustaining the
immunologic balance. For IBD, current data show that Treg proportion in
peripheral blood is decreased in active disease. So far, no functional defects
of Treg from IBD patients have been described since they maintain their normal
function of cell-contact-dependent, cytokine-independent suppressive capacity,
even against pathogenic T-effector cells derived from the inflamed mucosa. In
contrast to peripheral Treg, their frequency is comparatively low in inflamed
IBD mucosa compared to diverticulitis. Currently, it is not clear whether this
phenomenon is caused by a disturbed peripheral induction or migration of Treg.
Here, we demonstrate that Treg from CD patients show a functional defect
caused by an impaired CCR4-dependent migration capacity and mediated by a
decreased expression of CCR4. This may explain decreased Treg frequencies in
inflamed IBD mucosa. It is probable that CCR4 is not the only molecule that is
involved in the inappropriate migration of Treg in CD. It has been described
in CD that fewer CD4+ memory cells express CD62L. This receptor decelerates
lymphocytes by engaging ligands expressed on high endothelial venules. CD62L
is therefore required for recirculation and compartmentalization of CD4+ cells
between blood and lymph nodes. Although we found an accumulation of Treg in
mesenteric lymph nodes, there is also a lower percentage of CD62L+ peripheral
Treg, some of them also express CCR4. This may point to a more extensively
disturbed migration of Treg in CD. Based on these results, it becomes
conceivable to design therapeutic strategies which manipulate the intestinal
recruitment of Treg and thus suppress the pathologically increased immune
reaction in IBD.

▼ Bakgrund: En kronisk oförutsägbar sjukdom som Crohns sjukdom framkallar fysisk, psykologisk och social stress. Sjukdomen påverkar vardagen, försämrar livskvalitet och skapar oro. För att bedriva personcentrerad vård krävs insikt i hur denna oro upplevs. Syfte: Syftet med studien var att belysa sjukdomsrelaterad oro hos personer med Crohns sjukdom. Metod: Åtta kvinnor och fyra män mellan 30 och 64 år som skattat sin sjukdomsrelaterade oro som hög eller mycket hög identifierades. Intervjuer genomfördes och analyserades med kvalitativ innehållsanalys. Resultat: Analysen genererade tre kategorier: (1) Oro för sjukdomen i sig, (2) känslor runt oron, (3) samt hantering av oron. Sjukdomens oförutsägbarhet samt nedsatt funktion till följd av trötthet och bristande kontroll över tarmfunktionen var de mest framträdande orsakerna till oro. Oron skapade känslor av stress, skuld och besvikelse. Deltagarna uttryckte önskemål att få prata om och synliggöra oron men hade också funnit egna sätt att hantera denna. Slutsats: Det finns ett kvarstående behov av att ventilera och få förståelse för sjukdomsrelaterad oro, även efter många års sjukdom. Personer med Crohns sjukdom måste få prata om sin oro, vara en aktiv partner i vården och tillsammans med vårdgivaren finna sätt att hantera oron så att denna kan lindras.
Background: A chronic, unpredictable disease as Crohn's disease provides physical, psychological and social stress. The disease affects everyday life, impairs quality of life and create worries. To conduct person-centered care requires insight into how this worry is experienced. Objective: To explore disease-related worries in persons with Crohn´s disease. Method: Eight women and four men between 30 and 64 who estimated their disease-related worries high or very high were identified. Interviews were conducted and analyzed with content analysis. Results: The analysis generated three categories: (1) Worries about the disease itself, (2) feelings around the worries, (3) management of the worries. The unpredictable course of the disease and the impaired function due to fatigue and lack of control of bowel function were the most prominent causes of worries. The worries created feelings of stress, guilt and disappointment. The participants expressed the wish to talk about and make the worries visible but had also found their own ways to handle it. Conclusion: There is a persistent need to vent and get an understanding of disease-related worries, even after many years of disease. Persons with Crohn's disease need to talk about their worries, be an active partner in healthcare and together with the health-care providers find ways to handle the worries so it can be relieved.

▼ Background Pediatric IBD can limit the dayly life and is associated with psychosocial strain and future illness. However, physical activity and exercise are at great importance through childhood. Health, as a health science concept was used as a theoretical framework to establish an understanding where a balanced rhythm of life between movement and rest is a part of experiencing wellbeing and health. Aim The aim of this study was to illuminate research about how IBD affects childrens and adolescents physical activity. Approach A literature study based on eight scientific articles. Result The results showed that IBD can complicate physical activity and exercise, especially in active disease but also in periods of remission. The results illustrated three areas related to physical activity in children and adolescents with IBD; physiological function, limitations in physical activity and movement in IBD. Two factors that significantly contributed a decreased physical activity were fatigue, and a psychosocial strain in teenagers. Conclusion The result indicate that physical activity and exercise is complicated with physical, physiological and social dimensions. To consider health from a holistic approach in the care of a child or a adolescent with IBD pay attention to the importance of physical activity in life and can forms the basis of the nurse´s care in order to strengthen wellbeing and a healthy lifestyle.

Aims: This study aimed to analyse the oral mucosal microbiota (bacteria and fungi) in paediatric IBD patients. The mucosa was selected rather than the saliva as this is the region where tissue and immune cells directly interact with the microbiome
Methods: The oral microbiome and mycobiome were examined in a cohort of children diagnosed with
Crohn s disease (n=38, CD), and ulcerative colitis (n=21, UC). Children were treatment na?ve at the time of sampling. A cohort of 36 children who were attending DDUH were grouped as a healthy control (HC) group. DNA was extracted from tongue and buccal swabs and the V1-V2 region of the 16S gene and ITS2 barcode of fungal DNA were amplified and sequenced using the Illumina MiSeq. Sequences were analysed with the Mothur and DADA2 pipelines. Additionally, IBD children following therapy were also analysed.
Results: The tongue and buccal microbiota of CD children exhibted reduced biodiversity and lower species richness compared to healthy children. Our finding shows a reduction in the levels of several major oral bacterial genera, including Veillonella, and H. parinfluenza, and increased levels of oral Enterobacteriaceae and Actinobacteria in patients with severe IBD. Previous studies have implicated these taxa in the pathogenesis of IBD. In our follow up studies, remission from IBD symptoms was associated with loss of Enterobacteriaceae from the oral microbiome. We observed an increase in C. albicans and
S. cerevisiae in the oral mycobiota in IBD. Interestingly, C. krusei and C. dubliniensis were also observed in the oral cavity in IBD. Overall, the mycobiome changes observed were more related to IBD disease activity compared to microbiome changes. Shifting of the oral microbial community was noted in follow up samples from patients after therapy, returning some taxa (Veillonella, Enterobacteriaceae) to levels close to the normal oral community.
Conclusion: Children with IBD have an altered microbiota that may contribute to their oral health problems and may be linked to the pathogenesis of IBD. These data could potentially important to understand the aetiology of IBD and could be used in practical applications in diagnosis and monitoring disease progression.

s ability to survive within macrophages enables it to disseminate throughout the rest of the host, possibly infecting other circulating blood leukocytes. In this study, the survival and fate of map strain atcc 43015 (human isolate) following phagocytosis was determined using in vitro murine macrophage cell line j774a.1 and polymorphonuclear cells (pmnc'
s) from five

s from three healthy individuals and two ulcerative colitis patients, as well as escherichia coli (atcc 11303) and mycobacterium tuberculosis strain h37ra (atcc 25177), were included as controls (moi 10:1). Maturation of the phagosome was determined by evaluating the presence of stage specific markers on the surface of the phagosomal membrane. The endosomal protein, transferrin receptor, and the lysosomal marker, lamp-1, were then immunostained with cy-5 conjugated secondary antibodies, and colocalization of bacteria with each marker was evaluated separately using confocal scanning laser microscopy (cslm). In both tissue models, colocalization of viable map and m. Tuberculosis with the early endosomal marker, transferrin receptor occurred with an estimated five fold higher frequency than did association with the late lysosomal marker, lamp-1, as compared to live e. Coli, and all dead bacterial species. Using differential live/dead staining and fluorescent microscopy, survival of m. Tuberculosis and map was estimated at 85% and 79%, respectively compared to only 14% for e. Coli. The outcome was similar for both tissue culture and pmncs from all patients tested, suggesting that map and m. Tuberculosis can survive readily in both cell types, and regardless of the disease state of the host or the killing power of the cell. Map'
s survival appears to mimic m. Tuberculosis', suggesting the ability to resist phagolysosome fusion, by maintaining association with the early endosomes. Overall, the data confirms map virulence in host human blood leukocytes.

In patients with inflammatory bowel disease (IBD), restless legs syndrome (RLS) may occur as extraintestinal complication. A modified cerebral iron metabolism or folate/vitamin B12 deficiencies (FD/VB12D) have been described to cause RLS. Here, we determined the prevalence and severity of RLS in IBD patients and evaluated the effect of iron, and/or folic acid/vitamin B12 supplementation.
Methods:
Patients were screened for iron deficiency (ID), FD, VB12D and RLS by a gastroenterologist. If RLS was suspected, a neurologist was consulted for definitive diagnosis and severity assessment. Patients with RLS and ID, FD or VB12D received supplementation and were followed-up at week 4 and 11, respectively.
Results:
A total of 353 IBD patients were included in the study (Crohn´
s disease (CD) 224, ulcerative colitis (UC) 125, IBD unclassified (IBD-U) 4). Prevalence for RLS was 9.4% (n=21) in CD and 8% (n=10) in UC. Prevalence for the subgroup of clinically relevant RLS (symptoms ≥ twice/week with at least moderate distress) was 7.1% (n=16) for CD and 4.8% (n=6) for UC. 38.7% of RLS patients presented with ID, FD and/or VB12D. Most frequently ID was seen (25.8%; n=8). Intravenous iron supplementation resulted in RLS improvement (p=0.029) at week 4 in 7/8 patients.
Conclusion:
Although the prevalence of RLS in IBD did not differ to the general population, clinically relevant RLS was more frequent in IBD patients. Diagnosis of RLS represents a clinical challenge, hence a neurologist should be consulted. Improvement of secondary RLS after iron supplementation points to a pathophysiological link between RLS, ID and IBD.

▼ Introdução: A doença inflamatória intestinal (DII) abrange uma gama de entidades
clínicas cuja característica principal passa pela inflamação crónica do tubo digestivo em
diferentes localizações, sendo a colite ulcerosa (CU) e a doença de Crohn (DC) as
formas mais representativas. Dada a sua associação a múltiplas manifestações
extraintestinais, a DII pode atingir diferentes órgãos e sistemas do organismo, incluindo
a cavidade oral.
Objetivo: O objetivo do presente estudo foi o de verificar a existência de associação
entre as alterações da cavidade oral e a DII.
Materiais e Métodos: A amostra, de conveniência, foi selecionada no Hospital Garcia
de Orta, em Almada, Portugal, sendo constituída por 171 indivíduos adultos de ambos
os géneros, 113 doentes (65 com doença de Crohn e 48 com colite ulcerosa) e 58
controlos saudáveis. Foram determinados os valores do índice de dentes cariados
perdidos e obturados (CPOD), do índice periodontal comunitário (IPC), do índice
higiene oral simplificado (IHOS) e pesquisadas as mucosas orais para presença de
lesões. Foram ainda obtidos dados referentes à sintomatologia oral, aos hábitos de
higiene oral, hábitos tabágicos, terapêutica farmacológica, duração e atividade da
doença inflamatória intestinal.
Resultados: A sintomatologia oral, a doença periodontal e a cárie dentária, foram
significativamente mais frequentes no grupo de doentes com DII. A sintomatologia oral
e as lesões da mucosa oral foram mais acentuadas durante a fase ativa da DII. Não
houve diferenças significativas entre as duas apresentações da doença (DC e CU) em
nenhum dos parâmetros avaliados. A terapêutica com corticosteroides mostrou uma
tendência para agravar a sintomatologia e as lesões da mucosa oral. Os hábitos
tabágicos não mostraram modificar os parâmetros pesquisados. O duração clínica da DII
mostrou agravar os índices CPOD, IPC e a presença de lesões da mucosa oral, no
entanto nenhuma destas tendências se mostrou estatisticamente significativa.
Conclusão: Foi possível verificar na população estudada que a cavidade oral é afetada
pela DII. Estes doentes apresentaram maior frequência de sintomatologia oral e maior
severidade de doença periodontal e de cárie do que o grupo de controlo. A atividade da
DII mostrou acentuar a sintomatologia e as lesões da mucosa oral.
Introduction: Inflammatory bowel disease (IBD) encompasses a range of clinical
entities. The most prominent feature is the chronic inflammation in different locations
of the digestive tract. Ulcerative colitis (UC) and Crohn's disease (CD) are the most
representative forms of IBD. Given its association with multiple extraintestinal
manifestations, IBD is likely to target different organ systems, including the oral cavity.

Materialet till litteraturstudien samlades in genom sökning i databaserna Cinahl, PubMed och ScienceDirect. De sökord som användes vid litteratursökningen i samtliga databaser var ”caries”, ”Crohn’s disease”, ”periodontal disease” och ”periodontitis”. Vetenskapliga artiklar som fick ingå i litteraturstudien begränsades till ”English”, ”humans” och ”title/abstract” alternativt ”abstract/title/keywords”. Litteraturstudiens resultat baserades på en sammanställning av resultaten från tolv vetenskapliga artiklar. Tio av de vetenskapliga artiklarna berörde förekomst av karies hos personer med Morbus Crohn och i sju av dem redovisades att en signifikant förhöjd förekomst av karies fanns hos dessa personer. Tre av de tio vetenskapliga artiklarna redogjorde för att det fanns en förhöjd förekomst av karies hos personer med Morbus Crohn men det framgick inte om den var signifikant eller ej. Fem av de tolv vetenskapliga artiklarna berörde förekomst av parodontit hos personer med Morbus Crohn och i fyra av dem redovisades att en signifikant förhöjd förekomst av parodontit fanns hos dessa personer. Den kvarvarande vetenskapliga artikeln som berörde förekomst av parodontit hos personer med Morbus Crohn redogjorde för att det inte fanns en liten men inte signifikant förhöjd förekomst av parodontit hos dessa personer. The aim of the study was to investigate the occurrence of dental caries and periodontitis in people with Crohn’s disease. The method that was used in this study was a general literature review. The material for the literature review was collected by searching in the databases Cinahl, PubMed and ScienceDirect. The keywords that were used in the searches in all of the databases were ”caries”, ”Crohn’s disease”, ”periodontal disease” and ”periodontitis”. The scientific articles that were included in the literature review were limited to ”English”, ”humans” and ”title/abstract” alternatively ”abstract/title/keywords”. The result of the literature review was based on a compilation of the results from twelve scientific articles. Ten of the twelve scientific articles concerned the occurrence of dental caries in people with Crohn’s disease and seven of them reported a significantly elevated prevalence of dental caries in these individuals. s disease is a multifactorial chronic inflammatory disease of the gut,
which is characterized by a Th1-type- and a Th17-type-immunopathology. A
dysfunction of the gut barrier is associated with a not fully understood
cascade of inflammatory mediators and cytokines, among others TNF-alpha, IFN-
gamma, Il-1, IL-6 und IL-17. An important role in the final tissue destruction
is attributed to matrixmetalloproteinases as terminal mediators. At present
the established therapy is mainly confined to alleviation of symptoms
respectively concerning the TNF-alpha antibody therapy to immunmodulation,
which however often leads to not inconsiderable side effects. Other therapies
with less side effects are desirable. The oral infection of C57BL/6-mice with
Toxoplasma gondii results in a Th1-type-immunopathology as well, characterized
by the above-named inflammatory mediators and is suitable for the experimental
study of terminal mediators within the inflammation cascade initiated by
Toxoplasma gondii. Because of the extensive immunological similarities the
oral infection of mice with Toxoplasma gondii serves as a model for Crohn´
s
disease and offers beside pathophysiological findings the possibility to
investigate new therapy options. In this dissertation the increased expression
of MMP-2 and MMP-9 in the terminal ileum of C57BL/6-mice following oral
infection with Toxoplasma gondii could be shown using immunohistochemical
staining. The expression of both gelatinases increases considerably on day 3
post infectionem and significantly until day 8. This concerns MMP-2 and MMP-9
to a similar extent. The expression correlates with intestinal pathology and
loss of weight. Therapy with both specific and unspecific MMP-Inhibitors
reduces intestinal pathology and loss of weight of Toxoplasma gondii-infected
C57BL/6-mice considerably. Therefore the present paper could show the
importance of MMP-2 and MMP-9 concerning the development of intestinal
pathology in the context of a Th1-type-immunopathology in a mouse model
following infection with Toxoplasma gondii. Direct MMP-Inhibition could serve
as an therapy option for Crohn´
s disease. However it has to be considerd that
MMP-2 and MMP-9 are not the only mediators which contribute to intestinal
pathology and that this would also not be a curative therapy. Other factors
such as the pathophysiological impact of gut flora and other exogenic as well
as endogenic components, which are etiologically jointly responsible, should
be included in the discussion about therapy alternatives.

Obesity and Crohn´
s disease (CD) respresent diseases increasing worldwide. Both involve an inflammation of visceral adipose tissue (VAT). In hypertrophic, obese VAT, a low-grade inflammation has been associated with insulin resistance. Observations in obese patients imply an impairment of intestinal barrier function and an altered disease course in obese CD patients.
In CD patients a pathognomonic fat wrapping occurs around inflamed regions of the intestine, the so-called creeping fat (CF). However, the exact function of this tissue, consisting of a hyperplasia of small-sized adipocytes and infiltrating leukocytes, remains unsolved. Both a protective and a contributing role towards intestinal inflammation are discussed.
This study characterized T-cell subpopulations in VAT from obese patients as well as adipose tissue and adjacent mucosa in inflammatory bowel disease (IBD) patients. The study also focuses on expression of the gut-homing integrin α4β7.
Methods:
VAT (and in IBD patients mucosal) samples were obtained from 47 patients undergoing disease-related surgery. Fractions were fixed and stained for immunohistochemistry. Immune cells were isolated, stained and analyzed by flow cytometry. Cytometric bead array (CBA) was performed to quantify cytokine release in adipose tissue supernatants.
Results:
In obese VAT a high proportion of T-cells expressed integrin α4β7, significantly more than in subcutaneous adipose tissue. VAT was dominated by a pro-inflammatory T-cell milieu.
Macroscopic differences were found between colonic and ileal CF. Histology showed significantly smaller adipocytes and a high absolute T-cell infiltration only in ileal CF. Colonic CF resembled fat from UC patients. T-cell populations differed between IBD groups, ileal CF standing out by a higher amount of regulatory T-cells (Treg) and central memory T-cells. Furthermore, T-cells in ileal CF and mucosa consisted of distinct subgroups.
Correlation of adipose tissue T-helper cell and Treg share with disease activity was inverse in ileal and colonic CF.
When comparing obese VAT with ileal CF, VAT showed a higher amount of pro-inflammatory T-cells, also significantly more T-cells were α4β7-positive.
Conclusion:
In obese VAT α4β7+ T-cells are present in a pro-inflammatory milieu. These cells could be involved in intestinal barrier impairment and exacerbation of CD in obese individuals.
Ileal CF from CD patients presents a more balanced T-cell infiltration compared to VAT in obese individuals.
Interestingly, ileal and colonic CF differ regarding macroscopic and histological aspects, composition of T-cells and correlation with disease activity. This supports genetic data placing colonic CD between ileal CD and UC.

challenge to treat. Immunosuppressive, immunomodulatory and immunostimulatory
therapy models are the
subject of controversial debate. Against this
background, the effects of chronic immunosuppressive therapy as compared with
conventional anti-inflammatory therapy were of interest. The disease
progression of 41 IBD patients ulcerative colitis (UC) 30,
crohn`
s disease
(CD) 6, indeterminate colitis (CI) 5, who, on account of advanced primary
sclerosing cholangitis (PSC), had predominantly undergone a liver transplant,
was compared with a control group consisting of 42 (UC 29, CD 11, CI 2) IBD
patients, who did not undergo a transplant, assessed by means of retrospective
data collection, standardised surveys and updated telephone interviews. After
the transplant, oral types of immunosuppressive medication, such as
cyclosporine A, tacrolimus, and mycophenolate mofetil (MMF), were applied. On
the one hand, analyses were carried out by means of non-parametric test
procedures, according to short-term schemes of treatment at time intervals (6
month-intervals) irrespective of the patient; on the other hand, the Kaplan-
Meier method was applied at time intervals of more than 6 months (long-term
therapy). In the longitudinal analysis, after 14.5 years, 22 out of 41
patients in the transplant group remained permanently in remission (p =
0.0295) in comparison with 14 out of 42 of the control group patients. All
three types of immunosuppressive medication were associated with remission
inductions (cyclosporine: p =0.000, tacrolimus: p =0.000, MMF: p =0.001).
However, stable remission after 14.5 years was observed only during MMF
therapy (p =0.000). In the immunosuppressive group, over the course, 3
colectomies (UC patients) were necessary, compared with 7 within the control
group (3 UC, 4 CD patients). Furthermore, in the immunosuppressive group, over
the entire course, 5 cases of carcinoma arose, consisting of one case of
colorectal carcinoma in those with UC (2.4%) and 2 cases of cholangiocellular
carcinoma. In the control group, 4 cases of carcinoma occurred, among which
there was also one case of colorectal carcinoma in those with UC (2.4%) and
one case of cholangiocellular carcinoma. Extra intestinal symptoms occurred as
arthropathies during cyclosporine (p=0.000) and tacrolimus therapy (p=0.000).
Side effects observed in cyclosporine were attributed to dermatological
(p=0.005) classification, in tacrolimus to cardiovascular (p=0.000)
classification and during MMF medication, to dermatological (p=0.011) and
pulmonological (p=0.006) classification. 16 patients were completely free from
IBD symptoms immediately after the transplant; more than a year after the
transplant there were only 9 patients. Progression of the IBD symptoms
following liver transplant was only evident in CI patients (4 patients, while

▼ Background: Deficient immunoregulation through CD4+CD25+ Forkhead-box protein(FOX)P3+ regulatory T-cells (Treg) can cause Crohn´
s disease. Therefore, Treg were phenotypically characterized. Establishing transforming growth factor (TGF)β induced Treg (TiTreg), effector T-cells can acquire FOXP3 expression in the presence of cytokine in vitro. Which progenitor T-cell subpopulation either naive or effector T-cells will generate TiTreg with regulatory function?
Methods: Using flow cytometric analysis, the expression of CD127, CD49d and CD45RA were investigated. Naive and effector T-cells isolated from peripheral blood mononuclear cells via magnetic cell sorting were treated with anti-CD3/CD28 and in the presence of TGFβ and interleukin(IL)-2 in vitro. Suppressive capacity was assessed by mixed lymphocyte reactions.
Results: Patients with
Crohn'
s disease had significantly more CD4+CD25+CD127- T-cells than healthy donors, independent of treatment with infliximab or disease activity. Patients with
Crohn'
s disease had significantly more activated Treg and equal resting Treg in comparison to healthy donors. Naive and effector T-cells give rise to approx. 60% -80% CD4+CD25+FOXP3+ T-cells from both, healthy donors and patients with CD. Only FOXP3+ T-cells TiTreg converted from naive CD4+ T-cells suppressed mixed lymphocyte reactions and express CCR4, beta7.
Conclusions: The generation of TiTreg from naive T cells from patients was at least equally effective as from healthy donors. These cells express homing markers for potential migration to the inflammation.

▼ Despite the fact that E. coli supports limited posttranslational modification, this bacterium has been universally used as the expression system of choice. Expression of modified proteins in E. coli may lead to expression of recombinant proteins that lack essential immunomodulatory or catalytic components essentials for infectious processes. Previously in our laboratory, pMptb#28 plasmid containing a 4.8 kb insert from M. paratuberculosis has been identified which expressed 16 kDa recombinant protein in E. coli and 19 kDa recombinant protein in Mycobacterium smegmatis. The objective of this study is to identify the ORF sequence, investigate possible posttranslational modification and characterize the protein forms in the two hosts. Earlier in the study, the genome sequence for M. paratuberculosis was not available and therefore sequencing both the 5' and 3' ends of the 4.8 kb insert did not help in the identification of the ORF. However, unidirectional Exonuclease deletion resulted in identification of subclones containing possible ORF sequence. Later on, the publication of the M. paratuberculosis genome sequence along with BLAST analysis of sequences from the subclones resulted in the identification of 486 bp ORF with significant identity to that from M. tuberculosis and M. leprae. Cloning of the 486 ORF coding sequence in E. coli resulted in the expression of 16 kDa protein similar to the calculated predicted size of translated peptide. Cloning of the 486 bp ORF coding sequence in M. smegmatis resulted in the expression of 19 kDa protein similar to that from M. paratuberculosis. The 16/19 kDa forms of the same protein were verified using rabbit anti-M. paratuberculosis antibodies adsorbed in E. coli and M. smegmatis lysates. The size of the 19 kDa proteins was not reduced following treatment with deglycosylation enzymes in absence of any enzyme inhibitors. The 19 kDa product was confirmed not be a glycoprotein when failed to react with ConA stain. The 16/19 kDa forms of the protein were evaluated against T-lymphocytes from
Crohn'
s disease patients and normal controls. T- proliferation assay included controls such as PHA and PPD from M. paratuberculosis. There was not a significant difference between the two forms of the protein (16/19 kDa) against T-cell response from both populations. Overall, the study identified the ORF of the 19 kDa non-glycoprotein from M. paratuberculosis. Moreover, this is the first study which reports that the zoonotic M. paratuberculosis supports posttranslational modification similar to M. tuberculosis and M. leprae pathogens. Although the posttranslational modification component in this 19 kDa nonglycoprotein did not affect T- cell response, the finding is significant toward glycoproteins from M. paratuberculosis and their role in the pathogenesis of this bacterial infection in animals and humans.

s condições sociais e econômicas, como também, relacionada ao sistema de saúde que o atende. Ainda que existam inúmeros estudos sobre a adesão ao tratamento em portadores com doenças crônicas, são escassas as investigações sobre este tema nas doenças inflamatórias intestinais. O presente trabalho teve o objetivo de avaliar a adesão ao tratamento medicamentoso e os possíveis fatores que a influenciam, em pacientes portadores de doenças inflamatórias intestinais (DII): doença de
Crohn (DC) e retocolite ulcerativa (RCU), do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto (HCFMRP), da Universidade de São Paulo. Como controles, foram investigados pacientes portadores de pancreatite crônica e insuficiência pancreática (PC), com medicação fornecida pelo hospital, tal como os pacientes portadores de DII. Foram também investigados pacientes portadores de afecções digestivas variadas (ADV), grupo no qual a medicação prescrita não era fornecida pelo hospital. Por meio de estudo transversal e indireto, foi realizada entrevista estruturada para avaliar a adesão de 110 pacientes, que foram caracterizados como apresentando maior ou menor grau de adesão. Esta classificação foi baseada no cotejo entre os dados do prontuário e os informados pelo paciente em entrevista, considerando a afirmação do paciente que usava a medicação e que conhecia o nome da droga em uso. Utilizou-se, também, o teste de Morisky, que permite avaliar o padrão de comportamento do paciente em relação ao uso diário do medicamento. Este teste consiste de quatro perguntas padronizadas relacionadas ao esquecimento, descuido com o horário de tomada do medicamento, percepção de efeitos colaterais e ausência de sintomas. Na análise dos medicamentos utilizados pelo paciente, foram observadas as seguintes proporções de pacientes classificados como menos aderentes: 15,4% em pacientes portadores de DC, 13,3% na RCU, 8,4% na PC e 16,6% nos pacientes do grupo ADV. Porém, o teste de Morisky mostrou as seguintes proporções de menos aderentes: 50% de pacientes portadores de DC, 63,3% na RCU, 54,2% na PC e 63,4% na ADV. Não houve diferenças estatisticamente significativas, entre os grupos de pacientes, tanto na análise dos medicamentos utilizados pelo paciente como pelos resultados do teste de Morisky. Em análise univariada, nenhum dos fatores demográficos, sociais, clínicos ou referentes ao tratamento medicamentoso apresentou relação estatisticamente significativa, comum a todos os grupos, que indicasse influência sobre a adesão ao tratamento. profile as well as antioxidant vitamins in inactive IBD compared with active
IBD patients and healthy controls (HC). Methods: To obtain a detailed overview
of antioxidant vitamins and FA we analysed vitamin C, vitamin E, carotenoids
(lycopene, lutein/zeaxanthin, β-cryptoxanthin, a-/β-carotene), saturated (SFA,
C14:0, C15:0, C16:0, C18:0, C20:0, C22:0), mono-unsaturated (MUFA, C16:1n7,
C18:1n7, C18:1n9, C22:1n9) and poly-unsaturated FA (PUFA, C18:2n6, C18:3n6,
C20:3n6, C20:4n6, C22:4n6, C18:3n3, C20:5n3, C20:6n3, C22:5n3, C22:6n3) in
plasma. Inflammatory parameters (CRP; a-1-acid glycoprotein, AAG; faecal
calprotectin, FC) as well as clinical disease activity (CDAI, CAI) were
determined in 167 IBD patients and 45 sex, age and BMI matched HC. The IBD
study groups consisted of 138 inactive IBD (83
Crohn s disease, CD, 49
ulcerative colitis, UC) and 35 active IBD patients (17 CD, 18 UC). Results:
Overall nutritional status was normal in inactive (BMI 23.9±4.7 kg/m2), active
(22.7±4.2 kg/m2) IBD patients and HC (22.3±1.9 kg/m2). Inflammatory parameters
(CRP, AAG, FC) were significantly increased in inactive and active IBD in
comparison to HC. Plasma levels of carotenoids differed significantly between
inactive and active IBD as well as between inactive/active IBD and HC.
Significantly decreased plasma levels of vitamin C, increased levels of MUFA
and comparable levels of vitamin E, SFA and PUFA have been found between
inactive/active IBD and HC. FA inversely correlated with FC, vitamin C and
total carotenoids were found to inversely correlate with FC, CRP and AAG in
IBD patients. Discussion: This study showed for the first time that
antioxidant status and FA profile in a large population of inactive and active
IBD patients are disturbed independently from disease activity as compared to
matched HC, despite normal overall nutritional status. The compromised status
of antioxidant vitamins might offer a therapeutic opportunity to improve
antioxidant and inflammatory status.

▼ Introduction: Although probiotic Escherichia coli strain Nissle 1917 (EcN) has
been proven to be efficacious to maintain remission of ulcerative colitis and
pouchitis, the underlying mechanisms of action still remain elusive. A major
role in the initiation and perpetuation of chronic IBD is attributed to the
luminal bacterial flora. Several studies have demonstrated that bacterial
products can activate T cells via Toll-like receptors even in the absence of
antigen-presenting cells or costimulatory molecules, suggesting a direct link
between the luminal bacterial flora and the immune system. Aims: To explore
the role of TLR`
s and costimulatory molecules in the response of T cells
towards EcN. Methods: Peripheral blood (PBMC) and lamina propria (LPMC)
mononuclear cells from wildtype, TLR-2 and TLR-4 knockout mice were stimulated
and cultered in the presence or absence of sterile filtered EcN supernatants
(EcN-SC). Flow cytometry analysis was used to assess DNA content (PI), T-cell
receptor and TLR expression. Cell adhesion to the ECM components fibronectin
and collagen was measured by using an adhesion assay based on luminometric
analysis of fluorescently labelled cells. TLR-negative HEK293 cells were
stably transfected with hTLR-2 and hTLR-4 and stimulated in the presence and
absence of EcN supernatants followed by measurement of NF-κB activity using a
chemiluminescence assay. Results: EcN-SN significantly increases TLR-2 and
TLR-4 expression in wildtype PBMC. EcN-SN significantly inhibits cell cycle
progression of activated wildtype PBMC, but not PBMC from TLR-2 or -4
deficient mice. Further EcN-SN increases NF-κB activity in TLR-2 and TLR-4
transfected HEK293 cells and inhibits adhesion of Wildtype and TLR-2 PBMC and
LPMC to collagen and fibronectin. In contrast, the inhibition of T-cell
adhesion to fibronectin is reduced in TLR-2 T-cells. Discussion: We firstly
demonstrated that cell free supernatants of EcN distinctively modulates T cell
function. EcN increases TLR-2 and -4 expression and activate NF-κB via these
receptors. Consequently, inhibition of T cell cycling by EcN is not observed
in TLR-2 or -4 knockout mice, demonstrating that these receptors are crucial
in mediating the effects of EcN to T cells. Whereas the effects of EcN on T
cell activation are TLR-2 and -4 dependent, the inhibition t cell adhesion to
the ECM to be only TLR-4 dependent, indicating that EcN use distinct signaling
pathways to exert its broad biological functions.

Ulcerative colitis and Crohnʼs disease are chronic inflammatory bowel diseases (IBD) of unknown etiology. In recent years, mucosal healing has emerged as the goal for therapy to achieve long-term remission and to change the natural course of IBD. Thus, it is essential to monitor thoroughly the disease activity. Fecal calprotectin is the best available biomarker of disease activity in IBD.
The overall aim of this thesis was to study the clinical usefulness of fecal calprotectin. Four different patient cohorts were investigated.
For patients with active ulcerative colitis, the fecal calprotectin levels varied considerably, even over a single day, and the variability was considered to be clinically important in up to one-third of the patients. However, the longer the time period between bowel movements, the higher were the concentrations of calprotectin. To reduce both the impact of the variability and the risk of false low calprotectin values, samples should be obtained from the first stool passed in the morning. In stool samples stored at room temperature, the concentrations of calprotectin were stable for 3 days, while the levels decreased significantly after 7 days. In a questionnaire, the patients declared that they did not find it burdensome to obtain stool samples, although suitable equipment was considered desirable.
The levels of fecal calprotectin did not distinguish between patients with endoscopic recurrence 1 year after ileocaecal resection for Crohn’s disease and those without. However, in patients with low calprotectin values, endoscopic remission was commonly noted, suggesting that a colonoscopy might be avoided in these cases.
In the group of patients with quiescent ulcerative colitis, dose escalation of 5-aminosalicylic acid (5-ASA) in those patients identified with increased levels of calprotectin significantly reduced the relapse rate. However, the overall relapse rate of the intervention group was not significantly lower than that of the control group.
At cut-off values for calprotectin of 169 µg/g and 262 µg/g, the clinical course in patients with newly diagnosed ulcerative colitis could be predicted with good specificity and moderate sensitivity, for 1 and 3 years, respectively.
Conclusions: These results facilitate standardization of the stool sampling procedure, which is necessary to improve the accuracy of this biomarker. Furthermore, fecal calprotectin might be used to select patients for ileocolonoscopy 1 year after ileocaecal resection for Crohnʼs disease. To treat patients with IBD in clinical remission, but with increased values of calprotectin suggesting subclinical disease activity, brings a new dimension to IBD care. Lipopolysaccharide (LPS) on epithelial transport and barrier function of
intact and experimentally injured human istal colon were studied with Ussing
technique. Methods: Intact human mucosa was experimentally injured by mucosal
exposition to Hydrochloric acid in concentrations from 20 to 45 mmol/l. The
effects of LPS, TGF-ß and diosmectite on intact and injured epithelia were
studied in the Ussing chamber model. Results: Mucosal addition of HCl in
concentrations of 20 and 25 mmol/l had no effect on epithelia. HCl (30 and 35
mmol/l) resulted in a decrease of transepithelial resistance which was within
180 min nearly complete reversible. 40 and 45 mmol/l resulted in an
irreversible damage of epithelia. Studying intact epithelia LPS 10µg/ml given
serosal and simultaneous mucosal and serosal resulted in a significant
increase of short-circuit current (Isc) within 90 min. Mucosal addition of LPS
had no effect on Isc. Examining experimentally injured epithelia by HCl in a
mucosal concentration of 35 mmol/l LPS (mucosal addition) resulted in a
sigificant increase of Isc. In this study we could not describe an effect of
TGF-ß and diosmectite on intact or experimentally injured epithelia.
Conclusions: Mucosal addition of HCl in a concentration of 35 mmol/l was taken
as a valid model for experimental epithelial injury. LPS stimulated Ion
secretion in our experimental studies with intact and injured epithelia.
Therefore LPS could contribute to diarrhea in inflammatory bowel diseases.

▼ Patients with IBD are at risk for intestinal disability (e.g. having a stoma, malnutrition). The general aim of the thesis was to investigate physiological, psychological and social consequences of having IBD and an ileostomy or short bowel syndrome (SBS).Patients and Methods: Six SBS patients were interviewed to investigate practical aspects on daily life. In 21 ileostomates, and 19 patients with SBS, bioelectric impedance spectroscopy (BIS) was compared with other methods for determination of water compartments and fat free mass (FFM). Differences were analysed using the Bland-Altman method. Concerns, health related quality of life (HRQOL), quality of life (QOL) and coping were assessed in 21 patients with an ileostomy and 28 patients with SBS. HRQOL were compared with a matched group from the general population. Results: The differences between methods (BIS, Dual-energy absorptiometry (DXA), Dilution and derived estimates) to assess total body water (TBW) and FFM were pronounced. The mean difference of extracellular water (ECW) between dilution and BIS was less in female ileostomates than male, (p<0.01). Hydration of FFM assessed by BIS gave a mean of 0.77 for ileostomates and 0.75 for SBS patients. Most SBS patients were maintained in a stable clinical condition within normal limits of body weight and BMI.The most serious limitation of every day life for SBS patients was the planning involved to get most out of life. Dominating concerns for the ileostomates were intimacy, access to quality medical care and energy level and for the SBS patients being a burden, further surgery and energy level. Vitality was significantly reduced in ileostomates. In SBS patients all dimensions in HRQOL were significantly reduced except physical function. More than 1/3 of SBS patients rated health worse or much worse than one year ago. Most definitions on " good quality of life " were found in the social dimension. Most " obstacles to good quality of life" were found in the psychological dimension for the ileostomates and in the physical dimension for the SBS patients. Perceived QOL was rated 70/100 for ileostomates and 64/100 for the SBS patients. Confrontation was the dominating coping style.Conclusion: The differences between methods to assess body water compartments and FFM were pronounced. Methods could not be used interchangeably. BIS should be further evaluated with repeated, longitudinal measurements. The high concern for being a burden needs further investigation and how it can be reduced. Concern for intimacy as the concern for loss of sexual drive and sexual performance needs to be further explored. Fatigue needs to be defined. It is also necessary to study how concern changes over time.
