▼ Modernization of traditional Chinese medicines has been actively pursued over the past decades. Advanced biological, chemical, pharmacological and engineering research of traditional Chinese medicines has been pursued. Despite the advances, the lack of quality assurance of Chinese herbal medicinal (CHM) products with consistent quality remains a critical issue to be solved. Nowadays, a typical CHM product is manufactured by following a fixed recipe of extraction of a fixed herb formula. However, there is huge variation in the composition of herbs from different sources and inconsistent quality CHM products are common. The objective of this project is to develop a quality assurance (QA) methodology of CHM products, which provides a systematic and cost effective way of production of consistent quality CHM products irrespective of herb sources. To achieve this objective, a QA model was developed, which describes the physicochemical phenomena such as mass transfer and multiple-herb-extraction effect during herbal extraction. The solubility and the model parameters including partition coefficient, maximum available amount, mass transfer coefficient, etc. of chemical markers are determined using experimental methods developed as an integral part of the model. After specifying the product quality specifications in terms of proportions of chemical marker concentrations, the model along with the experimental data provides the extraction conditions, including the type of extraction solvent, volume, temperature and time, and the herb amount of each quality class needed to produce a CHM product that satisfies the product specifications. This QA methodology was demonstrated with three single-herb CHM products (Danshen, Gegen and Ginkgo leaf extracts) and a multiple-herb CHM product (Danshen-Gegen extract). Experiments showed that the determined QA solutions for the four CHM products were able to produce products with proportions of chemical marker concentrations fall within 10% of the specified chemical marker compositions. An H9c2 cell assay was used to test the efficacy of three Danshen-Gegen extracts of the same quality but were prepared by different herb combinations. The results showed that the three Danshen-Gegen extracts provided consistent biological efficacy against menadione-induced toxicity in H9c2 cells. In addition, the QA methodology is also capable of solving optimization problems, such as cost minimization and minimization of toxic chemical contents in a CHM product. Cost minimization of all the four examples was conducted and it was shown that the cost savings can be significant. The most expensive solution was 13 times the least expensive one in the case of Gegen extract. Another example was the production of Ginkgo leaf extract. A consistent quality extract with an acceptable amount of toxic alkylphenols was produced by the most cost effective option of production.

▼ Sheng Mai San (SMS) is a compound formulation in traditional Chinese medicine, consisting of Radix Ginsenq, Fructus Schisandrae and Radix Oohiooosonis. In the present study, the in vivo antioxidant properties of SMS were investigated using a rat model of carbon tetrachloride (CCl4)-induced hepatotoxicity. The antioxidant profile of SMS was also examined in several in vitro systems. Twenty-four hours after dosing rats with CCl4 at 1 ml/kg, an impairment in hepatic antioxidant status, as evidenced by increases in sensitivity of liver homogenates to glutathione (GSH) oxidation (induced by tert-butylhydroperoxide) and thiobarbituric acid reactive substances (TBARS) formation (induced by ferric chloride) and decrease in hepatic GSH content, was associated with elevations in hepatic malondialdehyde level and plasma alanine aminotransferase activities, indicating oxidative hepatocellular damage. Hepatic glutathione reductase and Cu,Zn-superoxide dismutase activities were also decreased. SMS-crude extract pretreatment caused significant protection against CCl4-induced hepatocellular damage, with a parallel enhancement in hepatic antioxidant status. SMS-aqueous fraction pretreatment significantly enhanced the hepatic GSH status and protected against hepatocellular damage in CCl4-treated rats. Although SMS-butanol fraction pretreatment inhibited ferric chloride-induced TBARS formation, it did not produce any effect on hepatic GSH status nor protect against hepatocellular damage induced by CCl4. SMS chloroform fraction, while increasing the sensitivity of liver homogenates to ferric chloride-induced TBARS formation, caused significant increase in hepatic glucose-6 phosphate dehydrogenase (G-6-PDH) activities and protection against CCl4-induced hepatotoxicity. A positive correlation between hepatic GSH status and hepatoprotection against CCl4 toxicity was apparent. Using in vitro systems measuring ferric chloride-induced peroxidation of erythrocyte membrane lipids and oxidation of ascorbic acid, SMS fractions were found to possess iron chelating activities. In addition, SMS fractions could scavenge hydroxyl and superoxide radicals as well as ferriheme oxidants. SMS fractions also inhibited GSH oxidation and enhanced GSH regeneration in liver homogenates. Although SMS butanol fraction had the highest in vitro antioxidant activities (except for the GSH regeneration), no apparent hepatoprotection was detectable in our in vivo rat model of CCl4 hepatotoxicity. The ensemble of results supports our hypothesis that the hepatoprotection afforded by SMS pretreatment is caused by its action in enhancing hepatic GSH status, possibly through G-6-PDH-mediated processes.

▼ In the continuously developing world of modern medicine, traditional medicines have begun to lag behind and disappear from the medical community. However, in China, Traditional Chinese Medicine, (TCM,) continues to persist in the face of debates, societal and environmental changes as well as global condemnation. In spite of all this, the Chinese government has set up protective measures to defend TCM from these outside influences, but in doing so has harmed their credibility in the global theater. Critics of TCM say it should be destroyed because its lack of scientific explanations, however these critics have jumped to conclusions; it is currently impossible to say TCM is unscientific or not because of a lack of evidence. The laws and regulations surrounding TCM domestically have created an unscientific process of maintaining its continued appearance in the Chinese market, resulting in a lack of credible evidence. These lax regulations, while intended to protect the cultural relic that is TCM, have raised questions lately about the safety and efficacy of the medicine, adversely affecting the global opinion on the topic of traditional methods. The oral and experiential traditions of TCM can no longer be used as the answer to questions of TCM in a global society. In addition, TCM has become a means of soft power for the Chinese, albeit ineffective due to the lack of evidence for or against its continued existence. With the 1996 decision to Open up the FDA, China began to emphasize TCM as a viable means of soft power. However the failure of the resolution to get TCM on the American market and to give it a place of its own pushed TCM into a state of in-betweeness, where the west did not want it, and China did not want to take it back. In order to put TCM back into a respectable position while saving face for China, this thesis intends to analyze the problems TCM faces while providing a remedy to the 100 year long scar on the face of TCM. ▼ Chinese medicine is changing rapidly in response to scientific advances and technologies, and the biomedicalisation of its traditional practices has become a global trend. Australia seems likely to follow this trend for many reasons, not the least of which is a lack of access to traditional sources. The thesis argues that, to the extent that biomedicalisation by-passes Chinese medicine‘s traditional concepts and frameworks, it disrupts diagnostic reasoning and alters the clinician’s therapeutic options. The argument assumes that Chinese medicine’s traditional conceptions are the basis of its therapeutic reasoning and decision making, that traditional conceptions guide successful clinical practice, and that contemporary clinicians understand them. However, the odds against today’s English speaking clinicians understanding the Chinese medical tradition are high. Access to premodern primary sources is problematic for non-Chinese speakers, and Chinese medicine texts in English have removed the historical and cultural contexts and meanings of traditional concepts to ‘scientise’ their content. Conversely, historical and anthropological research into Chinese medicine investigates precisely this kind of contextual information, but without reference to its clinical relevance. Rather than preserve the distinction between academic and clinical research, the thesis adopts a multidisciplinary approach to some of the issues facing Chinese medicine in Australia and the West. The significance of the study is threefold. First, it uses a broad range of English-language research and scholarship to re-imagine the traditional Chinese medical body. Second, its restoration of traditional perspectives draws out the internal intelligibility of premodern concepts and methods, and their relevance for contemporary clinical decision-making. Finally, its multidisciplinary and synthetic approach connects the interests of academic scholarship and clinical practice, and reinstates the traditional connections between Chinese medicine‘s conceptual frameworks and its clinical methods. The thesis argues that it is possible for contemporary Westerners to maintain Chinese medicine’s intelligibility as a system of medical practice. The decision to do so requires the careful restoration of traditional terms and concepts, and of the medical gaze that privileges the living body’s functional systems and activities. The restoration and re-imagining of Chinese medicine‘s traditional perspectives and methods connects contemporary Chinese medicine students and professionals to the complexity of this distinctive medical system. Our ability to recognise and understand the traditional Chinese medical body has ramifications for guiding diagnosis and therapeutic decisions, and fundamentally changes the clinical encounter. ▼ Background: The use of traditional Chinese medicine (TCM) has attracted increasing attention for physical and mental health studies over recent years. To date, there have been few nationally representative studies examining TCM treatments as well as consultations with TCM practitioners, for women with arthritis. Methods: The thesis study outlined here consists of four related but separate sections of research. First, a critical review was conducted focusing on TCM and other complementary and alternative medicine (CAM) use amongst patients with arthritis via a search of the key medicine and health science databases for international peer-reviewed articles published in the previous eight years (2008-2015). The study also conducted secondary and primary analyses of data from large samples (including both patients and practitioners) obtained by drawing upon three distinct large-scale established studies in Australia – the Australian Longitudinal Study on Women’s Health (ALSWH), the Sax Institute’s 45 and Up Study and the Practitioner Research and Collaborate Initiative (PRACI) study. Statistical analyses involved chi-square tests, multiple logistic regression, two proportions Z tests, Student’s t-tests, etc. to examine associations between the use of TCM (i.e. Chinese herbal medicine and acupuncture) and arthritis-related symptoms. Chi-square tests or Fishers Exact tests were employed for categorical variables, and Student’s t-tests were employed for continuous variables, to examine TCM practitioners’ perceptions and the role of TCM practitioners in Australia regarding arthritis care. Results: Findings from the critical review show a high prevalence of TCM/CAM use amongst people with arthritis in a number of countries and many people with arthritis use TCM/CAM concurrently with their conventional medicine. The results from ALSWH study identify women with arthritis are more likely to use TCM than women without arthritis, with 6.2-9.5%, and 4.0-5.7% of Australian women reported to be using acupuncture and Chinese herbal medicine, respectively, in the previous 12 months. Moreover, the analyses from the 45 and Up Study sub-study show that acupuncture use is positively associated with women experiencing a longer duration of time since initial diagnosis of osteoarthritis (OR=1.04), undertaking more exercise (OR=5.41), living in a rural area (OR=3.62), having consulted a psychologist (OR=12.21), and having consulted another CAM practitioner (OR=4.18). In addition, it is reported from the PRACI study that the majority of the TCM practitioners (82.2%) noticed that their patients with arthritis used other treatments alongside TCM and a large number of TCM practitioners who participated in the study believe TCM to be effective for treating arthritis. ▼ Chinese medicine (CM), a popular form of complementary and alternative medicine (CAM), has been increasingly used in the Western world over the last three decades. Quality, safety and efficacy of CAM therapies including CM have been major public concerns associated with the use of these interventions, for example, the variability of quality of herbal medicines, potential herb-drug interactions, organ injuries due to poor acupuncture practice, inadequate aseptic techniques, and the overall lack of high level clinical evidence of efficacy. Consequently, significant resources have been allocated to the generation of the new scientific evidence concerning quality, safety and efficacy of CAM therapies to address the public concerns and to support the clinical application of validated CAM therapies. Acupuncture and herbal medicines have been amongst the most popular CAM research areas. While the importance of such original research is well argued and supported, there has been a general negligence of the role of education in protecting public safety, promoting CM as a form of evidence-based healthcare and improving the quality of clinical CM services. This study aimed to determine the essential practitioner capabilities to ensure the safe, competent and contemporary CM clinical practice in complex healthcare systems such that of Australia. The specific objectives of this study are: a. to understand the utilisation of CM in Australia, China and a number of developed countries; b. to review issues related capabilitybased curriculum development and to compare the similarities and differences of CM education in China and Australia; c. to provide a comprehensive profile of the Victorian CM workforce based on reliable and authoritative source of data; d. to investigate registered practitionersâ€™ views on the desired capabilities for CM practice; and e. to identify knowledge and skill gaps of practitioners who were registered under the transitional arrangements (between 1 January 2002 and 31 December 2004) which will provide the basis for 15 determining the needs of continuing professional education. Based on outcomes from the two literature reviews (addressing objectives a and b, see Chapters 2 and 4, publications 1, 6, 7, and 9), this research involves a workforce study (addressing objective c, see Chapter 3, publications 3, 4, 5, and 10) and a capability survey (addressing objective d, see Chapter 5, publications 2, and 8). Ethics approvals were obtained for both studies. For the workforce study, a specific instrument with a coding system was developed and approved by the Chinese Medicine Registration Board of Victoria (CMRBVic) prior to the commencement of the data collection based on registration forms submitted by all practitioners registered with CMRBVic during the transitional (also known as grandparenting) period. ▼ Chinese Herbal Medicine (CHM) is a popular form of complementary and alternative medicine worldwide. Despite the numerous studies on the chromatographic fingerprinting, advanced extraction techniques and process development, the lack of a fundamental understanding of traditional manufacturing process has impeded the development of consistent and high quality CHM product. A traditional recipe based processing method for CHM consists of two main steps: maceration and extraction. A fundamental understanding of the traditional aspect for these two steps is of great importance for the quality assurance of CHM production. Maceration is generally utilized as a basic step before extraction in traditional recipe method for processing CHM. However little has been done to verify the effect. In order to understand the rationale behind it, different parts of herbs from herb plants, namely seed, stem, stem bark, root, root bark, flower and fruit, were used to study the maceration effect. It was found that maceration process could not affect the extraction yield of dashensu from Salviae Miltiorrhizae Radix (root) even though it caused swelling of the herbal tissue. Nevertheless, extraction yield of chrysophanol in Cassiae Semen (seed) was increased with an increase in maceration time. Decomposition of chemical marker was found in the extraction Lonicerae Flos (leaf) and Eucommiae Cortex (Stem bark) caused by hydrolysis of chlorogenic acid and pinoresinol diglucoside, respectively, during maceration. With an understanding of the plant physiology and anatomy, the relationship between extraction yield and herbal structural characteristics was revealed. On the other hand, herbs are usually sold in the herbal market in the form of slices instead of the raw herbs or powder form. However, difference of extraction performance in slice and powder form is often overlooked in the domestic and pharmaceutical industrial production. Therefore, by treating the herbal slice as slab form, the extraction behavior for the herbal slices was investigated and a one-dimension mass transfer model with internal diffusion controlled transport was proposed based on the physicochemical diffusion phenomena of herbal extraction. Herbal slice Astragali Radix with marker calycosin-7-O-beta-D-glucoside was selected as an example to demonstrate how to apply the model. Model parameters D’ was obtained by curve fitting and the calculated value was 4.0313×10^-10 m²/s. The overall objective of the thesis was to improve the traditional manufacturing process with an understanding of the herbal structural characteristics and intra-particle diffusion process for conventional extraction of phytochemicals from CHM. It is believed that it could improve the current CHM production process and present a modern engineering view on the traditional recipe method.

▼ Jujuba Fructus (JF), also known as jujube or Chinese date, is the fruit of Ziziphus jujuba (Mill.), and which is considered as one of five valuable fruits in China. JF serves as daily food, as well as being prescribed as a tonic medicine for blood nourishment and sedative effect. Interesting, the usage of jujube is not only restricted as single herb, which very often included in multi-herbal decoctions. The make-up of a multi-herbal decoction is believed to improve the efficacy of single herb and enhance the pharmaceutical value via the herbal compatibility. Here, three jujube-containing herbal decoctions written from different dynasties, i.e. G̲uiz̲hi T̲ang (GZT; ~200 AD), N̲eibud̲angguijianzhong T̲ang (NDT; 652 AD) and Z̲ao̲ T̲ang (ZOT; 1078~1085 AD), were chosen for study and comparison. These jujube-containing herbal decoctions share common herbal compatibility, i.e. G̲lycyrrhizae R̲adix et R̲hizoma P̲raeparata cum M̲elle (GRRPM), Z̲ingiberis R̲hizoma R̲ecens (ZRR) and JF. ZOT comprised of GRRPM, ZRR and JF, while GZT and NDT differed by the addition of C̲innamomi C̲ortex (CC) / C̲innamomi R̲amulus (CR), P̲aeoniae A̲lba R̲adix (PAR) and Angelicae Sinensis Radix (ASR). Clinically, these herbal decoctions possessed similar pharmacological functions, implying that this herbal compatibility might be important in disease healing. Here, we aimed to disclose the underlying rationale for herbal compatibility within a herbal mixture by comparing the chemical constitutions and biological performances of three herbal decoctions. The chemical profiles of three jujube-containing herbal extracts were revealed. Cyclic AMP, cGMP, rutin, calycosin, formononetin, glycyrrhizic acid, liquiritin, 6-gingerol, cinnamic acid, paeoniflorin, ferulic acid and Z-ligustilide in jujube-containing decoctions were determined. Three jujube-containing herbal decoctions showed similar chemical profiles; however, the amount of the major chemicals showed great variations. Meanwhile, the solubility of some chemicals within these herbal decoctions were enhanced, implying that the herbal decoctions increased the extraction efficacy of certain chemicals. In erythropoietic function, jujube-containing herbal extracts demonstrated higher erythropoietic functions, as compared to JF extract alone. The application of three jujube-containing herbal decoctions in cultured Hep 3B cells induced the expression of erythropoietin via the activation of HRE-mediated signaling pathway. ZOT exhibited the strongest erythropoietic function, giving the highest induction by ~200% of increase for HRE-mediated transcriptional activity. Meanwhile, jujube-containing herbal decoctions promoted the gene expression of heme iron recycling enzymes in cultured RAW 264.7 macrophage cells, giving the highest induction of heme oxygenase 1 (HO-1) and ferroportin (FPN) at ~28 and ~200 folds of increase by ZOT, respectively. ▼ Background: This author's previous MSc research concluded that traditional Chinese medicine (TCM) practitioners had poor levels of inter rater reliability (IRR) when observing tongue characteristics. This study suggested that IRR could be improved by developing operational definitions for the different tongue characteristics and a standardised protocol for observing the tongue. To this end this thesis documents the development and evaluation of a reliable tongue inspection tool and protocol. Aim: To develop a reliable tongue inspection reference tool and protocol by examining different groups' tongue inspection interpretations. Method: In addition to a reanalysis of the previous MSc study data (Study 1), five studies were undertaken to obtain inter rater reliability (IRR) levels for different groups of subjects while developing and refining the inspection tool and protocol. For three within subjects experimental design studies (Study II, V and VI), IRR levels were measured before and after a formal training session of tongue inspection. For the other two studies (Study III and IV), a between groups comparison of IRR levels was undertaken both before (Study III) as well as after (Study IV) a session of training using the newly developed tongue inspection tool. All data collection sessions involved subjects viewing a variety of digital tongue images using a data projector or computer screen and the completion of a tongue inspection form. Study II measured the IRR levels of 28 Year 2 TCM students both before and after the standard TCM curriculum training of tongue inspection lecture. Study III compared the IRR levels of 22 Year 2 students who had not undertaken any formal training in tongue inspection to 23 Year 4 students who had previously had formal instruction in tongue inspection in their second year of TCM education. Study IV evaluated this same cohort of students some five weeks later following training with the improved inspection method using operational definitions, colour charts and a standardised protocol. Study V, a within group repeated measures experimental design, measured IRR levels for both 21 inexperienced Year 2 TCM students while Study VI, used the same design, and involved 33 Year 4 TCM students. Both student groups had their IRR levels evaluated at baseline as well as after instruction in the newly developed tongue inspection tool. Statistical analysis involved assessing IRR with an agreement level of ≥80% was used as a criterion for an acceptable level of reliability. In addition, t-tests and Pearson product correlation coefficients were also computed where applicable. Results: Study I showed that using the new scoring method, the practitioners were poor at choosing the 'correct' answers and had a low level of agreement. Their performance did not improve or change across the two sessions. The two philosophy concepts of ‘Won Taoist’ and ‘Concept of Time and Space’ in “The Book of Changes” were used to comparatively analysis the relationship between Traditional Chinese Medicine and the philosophy of “The Book of Changes”, thus to explain the influence of “The Book of Changes” on the theories development of Traditional Chinese Medicine. “The Book of Changes” has indicated that ‘Won Taoist’ is the Tao of Heaven, Won Shaped movement is the basic characteristics of Heaven. Under the influence of the concept of ‘Won Taoist’ in “The Book of Changes”, Traditional Chinese Medicine has established the theories of Twelve-Meridians, Viscera theories of five organs and the theories of doctrine of five evolutive phases and six climatic factors. ‘Right Time’ and ‘Appropriate Position’ are the two main rules in “The Book of Changes” for making judgments of good or bad for all things. Under the influence of the ‘Concept of Time and Space’, Traditional Chinese Medicine has established the theories for treatments that follow the law of ‘Change with Time’ and ‘Change with Region’. Tao of Changes and Tao of Medicine are interconnected, the philosophy of “The Book of Changes” and Traditional Chinese Medicine are also interconnected. The results of this study have shown the existence of historical origins and relationships between “The Book of Changes” and Traditional Chinese Medicine. This study has laid a good basis for the work to further explore the influence of the philosophy of “The Book of Changes” on the theories development of Traditional Chinese Medicine.

▼ This dissertation was continued the research work of “Tao of Changes and Tao of Medicine: The Influence of the Philosophy of ‘Won Taoist’ and ‘Concept of Time and Space’ in “Zhou Yi” on the Theories Development of Traditional Chinese Medicine”, it further systematically clarified the influence of the philosophy in the “The Book of Changes” on Traditional Chinese Medicine theories in four aspects on the level of "Tao": “The Thought of Unity of Nature and Human Being”, “The Thought of Holistic”, “The Thought of Change” and “The Thought of Harmonious”. The Thought of Unity of Nature and Human Being is the core of the philosophy of “The Book of Changes”, it believed that the law of Heaven can be used as the basic criterion for human affairs. Under the influence of The Thought of Unity of Nature and Human Being in "The Book of Changes", Traditional Chinese Medicine understand that the physiological function and pathological changes of the human body has a very close relationship with the nature, so that all treatments for patients required to grasp the factor of time and region. The Thought of Holistic in “The Book of Changes” emphasizes that there is a close relationship between all things in the world and each other will influence each other. When observing things, we should grasp the overall situation from a macro perspective. Under the influence of this mode of thinking, Traditional Chinese Medicine understand that human body, human and nature, human and society all have a whole relationship, therefore, it also emphasizes to investigate the phenomenon of life with holistic thinking mode, and pay attention to observe, analyze and deal with the disease from the macroscopic and holistic angle. ▼ Background: Atopic eczema (AE) or infantile eczema is an inflammatory skin disease, which affects 10-20% of children in industrialised countries. Australia was the 12th highest rank of AE incidence in 55 participating countries. AE is characterised by poorly demarcated redness of the skin and associated surface changes such as scaling, swelling, accentuation of the hair follicles and skin thickening as a result of chronic scratching. There are three common quoted diagnostic criteria for determination of AE for the purpose of research and clinical studies. In addition, reliable scoring instrument such as Scoring Atopic Dermatitis (SCORAD) is fundamental for clinicians to verify the severity, the course and outcomes of treatment for AE. Certain quality of life (QoL) questionnaires have also been developed and validated for assessment of the personal impact and outcomes of the treatment for AE. The conventional (Western) medicine treatment for AE is not satisfactory. Complementary and alternative medicine including Chinese herbal medicine (CHM) has been increasingly used for AE. There is some promising published evidence on oral administration of CHM for AE. However, the benefit of topical application of CHM for AE is not clear. Aims: a) To review fundamental knowledge of AE in perspectives of both conventional medicine and Chinese medicine (CM); b) To evaluate the effectiveness and safety of topical application of CHM for AE by systematically reviewing currently available randomised controlled trials (RCTs); and c) To develop a protocol for pilot randomised double-blind placebo-controlled clinical trial for evaluation of the efficacy and safety of topical application of CHM for AE. Methods: We searched any RCTs with topical application of CHM in electronic databases and journals. Trial-design quality was evaluated and intervention outcome data were extracted and analysed. Meta-analysis was conducted. Development of a protocol for pilot RCT of topical application of CHM for AE followed the Australian Governmentâ€™s guidelines in compliance with high ethical standard. Results: Three studies involving a total of 452 participants were selected for analysis after screening 164 potential studies. All three included studies reported significant differences between the treatment group and controlled group and claimed that effects of treatment interventions were superior to control. However, due to the low quality of study design which resulted in low level of evidence strength, these claims require more vigorous scientific proof employing well designed RCTs. As a result, a protocol for pilot randomised double-blind placebo-controlled clinical trial for evaluation of the efficacy and safety of topical application of CHM for AE was developed. ▼ Many complementary and alternative medicine (CAM) interventions, such as acupuncture and herbal medicines, are rooted in traditional medicine (TM) systems indigenous to particular cultures and lands. The World Health Organization has recommended that nations take steps to regulate both TM and CAM practitioners worldwide, with the aim of enhancing their ‘safety, quality and effectiveness’. Several United Nations bodies have moreover recommended that nations take steps to protect TM knowledge, and prevent further misappropriation of TM practices. However, to date, very few studies have investigated or discussed how regulators may address the unique complexities of regulating TM practitioners and TM-rooted practices. This PhD dissertation – a qualitative case study of traditional Chinese medicine and acupuncture regulation in the province of Ontario, Canada: a) explores the range of factors that distinguish the regulation of traditional medicine practices and professionals from biomedical professionals; and b) considers how the ‘public interest’ may be conceptualized to appropriately address these distinguishing factors in regulatory context. Data for the study was generated from several sources, including mixed methods survey, documentary review, and 32 key informant interviews. Analytic approaches included thematic and critical discourse analyses using the principle of ‘regulatory equity’ as a guiding concept. Postcolonial, multiculturalism, and boundary work theories, as well as legal principles of disparate impact discrimination and reasonable accommodation, were applied across the study. This body of work points to ways in which regulatory structures in Western liberal democracies may systemically privilege biomedical and non-immigrant practitioners by examining state risk discourses and the construction of regulatory boundaries for acupuncture; the issue of English-language fluency for immigrant practitioners; and the negotiation of safety, quality and epistemology in acupuncture standard-setting across professions. In this light, a series of strategies is proposed to assist regulators in negotiating equitable approaches to CAM and TM regulation. Emphasizing a broad conception of the ‘public interest,’ these strategies prioritize protection of traditional knowledge frameworks, while seeking to accommodate biomedical practitioners’ safe and skilled adoption of TM-rooted health care approaches. ▼ Over the past 30 years, patients with chronic renal failure in Taiwan as the world generally shows an alarming increase in multiples. These patients with chronic renal failure once entered the long-term dialysis treatment to sustain life, consume a significant number of national health-care resources unproportionally, and sustained other complications happen again, so that their quality of life generally low. Development of follow-up of chronic kidney disease has aroused the concern of scholars. The study analyze the related academic field of chronic kidney disease and drugs. Chronic renal failure comes from chronic renal insufficiency, and patients with chronic renal failure already have many changes in physiological and metabolic abnormalities. These phenomena not only because more kidney function decline, may also increase mortality in the coming period. Recent academic research the association of taking traditional Chinese medicine or Western medicine with the occurrence of chronic kidney disease. According to scholars noted that patients taking drugs got chronic kidney disease risk will be an increasing trend, but related academic research is not yet sufficient. Early detection and early treatment of renal insufficiency causes can slow the rate of decline in renal function, and reduce the demand for dialysis; however, patients with chronic of renal insufficiency seems to be the forgotten group, and quality of medical care for pre-dialysis renal failure unheeded whole medical community. This study aims at evaluating the association of taking traditional Chinese medicine or western medicine continuously with the occurrence of chronic kidney disease in Taiwan by analyzing data retrieved from National Health Insurance Research Database (NHIRD) total of 1,000,000 people between 2005 and 2010. Search for the first time in 2010 was diagnosed with CKD 810 people, and investigate whether taking traditional Chinese medicine or Western medicine continuously increases the risk of getting CKD. The study shows that taking traditional Chinese medicine continuously rate increased along with the number of days increased. Continuously taking traditional Chinese medicine longer, the higher the rate of patients have been diagnosed with chronic kidney disease. For taking western medicine continuously is not significant rate increased along with the number of days increase. In addition, the study further to search stopping taking drugs to the onset days and the top five drugs and doses of traditional Chinese medicine or western medicine with CKD patients. Then, statistic gender, age, high blood pressure and other factors. ▼ Evidence-based medicine (EBM) is a contemporary practice paradigm with a focus on improving outcomes of diagnosis and treatment through effective integration of the best available evidence, practitioners’ expertise and individual patients’ preference. In this project, Chinese herbal medicine (CHM) for insomnia is used as an example to illustrate the available evidence and identify CM practitioners’ attitudes, knowledge, skills, behaviours and barriers towards evidence-based practice (EBP). Understanding and addressing these factors will contribute to the enhancement of the practice of Chinese medicine (CM) as well as to set the priorities for CM education, research and policy development. CHM is increasingly used for insomnia in China and worldwide, and has been extensively researched and used in clinical practice. Therefore, it is a suitable exemplar providing insight into EBM in the context of CM practice. Insomnia is the most common sleep-wake disorder with notable health, social and economic burden. Cognitive and behavioural therapy for insomnia (CBT-i) and pharmacotherapy are commonly used to manage symptoms. However, many sufferers seek complementary and alternative medicine (CAM) to improve their sleep and to manage daytime dysfunction. As a form of CAM, Chinese medicine has a long history of use for insomnia and continues to be frequently utilised. Emerging preclinical and clinical evidence suggests that CHM may be beneficial for improving sleep quality. Despite the frequent use of CHM for insomnia it remains largely unclear if CM doctors use EBM to inform their clinical practice. Therefore, this project uses a step-wise approach to understand, firstly, the best available evidence of CHM for insomnia and secondly, to what extent is the evidence being translated into clinical practice. The present study consists of four components: 1) a systematic review (SR) of randomised controlled trials (RCTs); 2) a systematic analysis of the historical literature; 3) a quantitative survey of CM practitioners; and 4) a qualitative interview of CM practitioners. Part One: Current Clinical Evidence of CHM for Insomnia: A Systematic Review and Meta-Analysis This SR with meta-analysis answers the following research questions: Does CHM produce beneficial effects in people with insomnia in terms of sleep quality?’ Does CHM cause any adverse events in people with insomnia and, if so, what is the nature of the adverse events?’ Seventy-nine RCTs involving 7, 886 participants were included in the SR and 76 in the meta-analyses. Findings from this review revealed that CHM improves subjective sleep quality and quantity in people with insomnia. There was no significant difference between CM and the placebo with respect to the frequency and severity of adverse events. Firm conclusions could not be drawn on the comparative effectiveness between CHM and benzodiazepine drugs or psychotherapy due to heterogeneity and risks of bias in the included RCTs. ▼ Herpes zoster (HZ) is a neurocutaneous disease caused by reactivation of the varicella zoster virus. The characteristic clinical manifestations of HZ include grouped blistering vesicles erupting on a red base of skin, and burning or stabbing pain with a unilateral dermatome distribution. The pain of HZ is associated with significant reduction on patients’ health related quality of life (HRQoL). Conventional treatments include antiviral therapies and pain management. These therapeutic approaches have not been able to provide effective relief of these symptoms for all patients. Hence other therapies such as Chinese medicine (CM) have been used to treat symptoms of HZ, either alone or as add-on therapy to conventional approaches. Over the long history of Chinese medicine development, valuable information on clinical manifestations, diagnosis, and treatment for HZ have been described and explained in-depth in the classical literature, which continues to be included in the contemporary guidelines and textbooks for CM practice. Currently the classical literature evidence for HZ has not been systematically evaluated. Chinese herbal medicine (CHM) and acupuncture are the two main CM therapies for HZ included in contemporary CM textbooks and guidelines. The clinical evidence based on several systematic reviews (SRs) of CM indicated that they are beneficial for pain control and cutaneous outcomes. However, the quality of current evidence is limited by the databases searched, study inclusion criteria, and statistical analyses. To address this gap, SRs based on a more comprehensive search of databases and strict inclusion criteria are needed. Objectives Guided by the “Whole Evidence” approach, this thesis aims to: 1. Examine the evidence of Chinese medicine classical literature for HZ. 2. Evaluate the current clinical trial evidence of CHM and acupuncture for HZ. 3. Review the current experimental evidence of CHM treatment for HZ. 4. Design a rigorous trial protocol on a CHM formula for HZ. Methods Classical literature evidence research The comprehensive classical literature search was conducted using the Zhong Hua Yi Dian 中华医典 (ZHYD). Citations identified by search terms were extracted for further analysis. Descriptive statistical analyses were performed to identify the frequency of each search term, signs and symptoms and treatment. Cluster analyses were performed to discover the potential relationships of the HZ symptoms in the data set which were judged to be most likely HZ citations. Modern literature evidence research Nine English and Chinese databases were searched for evidence from randomised controlled trials (RCTs). Overviews of CHM and acupuncture therapies were conducted to summarise and discuss the characteristics of included studies. ▼ Atopic eczema (AE) is one of the most common skin diseases that patients frequently present with to dermatological practices in South Africa (SA). It has shown to impact negatively on the quality of life of many patients suffering from it. Epidemiological studies have shown high rates of AE prevalence, ranging from 2-7% in adults and 7-20% in children. Over the last decade, the lifetime prevalence of physician-diagnosed AE has almost doubled in SA. This rise continues despite accessible effective treatments. Due to AE’s chronic and relapsing nature and the unattainability of complete clinical cure, patients are progressively exploring complementary and alternative medicines (CAM) in search of a solution. Although the global popularity of CAM for AE is on the rise, a review of the literature demonstrated contradictory evidence with regards to their efficacy with shortcomings in many of the published data thus making it difficult for clinicians to assess their role, if any, in the management of AE. Objective One To objectively evaluate the information on the efficacy and safety of CAM in light of the most recent findings, the study entitled “Complementary Therapy in Atopic Eczema: The Latest Systematic Reviews” in Chapter Two of this thesis collectively evaluated all published systematic reviews (SRs) to date on the most popular CAM modalities for AE. These SRs included those of Chinese herbal medicines(CHM), homeopathy, oral herbal remedies (including evening primrose oil and borage oil), probiotics and certain dietary supplements. The study concluded that none of the alternative therapies evaluated demonstrated obvious and indisputable evidence of efficacy due to many limitations in study design, poor methodologies, patient numbers etc. Further studies may be warranted with some therapies (CHM, different probiotic strains and fish oil), whereas homoeopathy failed to show any treatment effect and further studies with evening primrose oil and borage oil may be difficult to justify. This overview was able to provide objective information to enable dermatologists and general practitioners to advise and manage their patients holistically in the light of the most recent findings. Objective Two Topical corticosteroids remain the mainstay of treatment for AE. However, many patients are concerned about their long-term safety and thus seek evidence-based safer alternatives. Many published papers have made reference to the wide use of topical herbal creams for AE and many of these been tested, but few in controlled clinical trials. No SRs of these trials could be found, although SRs of topical herbal extracts have been published for other chronic skin conditions. The study entitled “Topical Herbal Medicines for Atopic Eczema: A Systematic Review of Randomised Controlled Trials” in Chapter Three of this thesis was the first SR to be conducted for topical herbal preparations for AE. ▼ T̠raditional U̠yghur m̠edicine (TUM; 維吾爾醫藥) has long been used to enhance physical condition and to treat diseases in northwest province of China. However, very few researches are focusing on the quality control and action mechanism of this herbal medicine. Here, Song Bu Li decoction (SBL; 松補力糖漿), the simplest TUM preparation, was analyzed. SBL contains only one herb, Nardostachyos Radix et Rhizoma (NRR, the root and rhizome of Nardostachys jatamansi DC). According to TUM theory, SBL can treat functional reduction of brain and heart caused by moist-cold and phlegm. The quality control parameters, as well as the biological efficacies of SBL have not been established. Therefore, we aim at setting up the criteria for preparation of SBL and reveal its biological mechanisms. The quality control parameters of NRR were constructed by measuring the amounts of nardosinone and total volatile oil, as well as the establishment of HPLC fingerprint chromatogram. Both water extract and volatile oil of NRR were chemically standardized. The volatile oil was analyzed by GC-MS: 14 compounds were identified, which accounted for ~80% of the total volatile oil. NRR water extract was standardized by HPLC fingerprint and the minimal amounts of ferulic acid and linarin. Here, we recommended a standardized NRR extract should contain at least 24.8 μg ferulic acid and 114.9 μg linarin in 1 g of dried NRR extract. A well-standardized SBL was prepared with a mixture of NRR extract and volatile oil in a ratio of 6 (NRR water extract; w): 1 (NRR volatile oil; v). These well-established quality control parameters guaranteed the quality of SBL to be consistent. For a comprehensive assessment of neuro-beneficial effects of SBL, different biological assays were employed, including neuroprotection and neuronal differentiation in PC12 neuronal cells. The pre-treatment of SBL protected cell against tBHP-induced cell death in a dose-dependent manner. In parallel, SBL suppressed intracellular r̠eactive o̠xygen s̠pecies (ROS) formation. Besides, a pARE-Luc reporter gene (an anti-oxidant responsive element) was transfected into PC12 cells. The treatment of SBL increased the transcriptional activity of pARE-Luc in dose-dependent manner. In parallel, the treatment of SBL also elevated the key anti-oxidant stress proteins, which included glutathione S-transferase (GST), NAD(P)H quinone oxidoreductase (NQO1), glutamate-cysteine ligase catalytic (GCLC) and modulatory (GCLM) subunits. In cultured PC12cells, the expression of neurofilament, a protein marker for neuronal differentiation, was markedly induced by the applied herbal extract. Moreover, the nerve growth factor (NGF)-induced neurite outgrowth in cultured PC12 cells was significantly potentiated by the co-treatment of SBL. ▼ Doctor's use of medical terms is generally considered to be one of the reasons for patients' poor comprehension of medical information. However, little research has been done on the effects of Chinese medical terms, which are constructed rather differently compared to English medical terms. This study manipulated the use of Chinese medical terms in a simulated clinical consultation, to investigate its effects on patients’ comprehension of medical information and their confidence for seeking further information. One hundred and eighty one eligible Hong Kong laypersons were randomly assigned to watch one of three video clips, which differed in the use of words explaining the underlying pathological changes and the treatment of prolapsed intervertebral disc: one with lay terms only (group 1), another with medical terms alone (group 2), and another one with medical terms supplemented with lay terms (group 3). After that, a computer-based questionnaire was used to assess the participants' comprehension of the information and confidence for seeking further information, followed by a semi-structured face-to-face interview on their perceptions of the utility of Chinese medical terms. Results suggested that although Chinese medical terms were perceived to be useful in assisting patients to look for further information on their diseases, the use of Chinese medical terms, either used with or without the supplements of lay terms, did not add to patients' comprehension of medical information and confidence for seeking further information. Regression models showed a significant trend of decreasing comprehension of the cause of the disease (β = -0.241, P = 0.002) and knowledge test scores (β = -0.214, P = 0.006) with increasing age of the participant. However, older persons had better comprehension of the word used (β = 0.238, P = 0.001). Participants' educational level was the most important determinant of knowledge test scores (β = 0.364, P < 0.001). In addition, persons with higher education (β = 0.207, P = 0.024), professional occupation (β = 0.173, P = 0.015) had better comprehension of the cause of disease. The findings of the study have implications for doctor-patient communication as well as medical education. To improve the effectiveness of doctor-patient communication, it may be more rewarding to enhance doctors' overall communication skills, rather than simply advocating their use or not use of medical language.

▼ Traditional Chinese Medicine (TCM) diagnosis is based on four standard approaches, i.e., observation, smelling/listening, inquiry, and palpation. Observation tops the four ways of TCM diagnosis and tongue corresponds to the major subject of focus during observation. Traditional teaching methodology, namely, classroom lecture and textbook with a limited number of illustrations, were usually the major means of acquiring knowledge relevant to tongue diagnosis. The exemplary illustrations were usually captured under poorly controlled lighting environment and printed with low color quality. The teaching material cannot be updated dynamically and the static assessment questions cannot be adjusted to meet the needs of each individual learner. The limitations in terms of learning location, time and type, and teaching and assessment methods leave much to be desired. With the help of computer technology, a diversified digital learning and assessment platform is constructed to lift these aforementioned constraints. In light of the pitfalls of traditional TCM education and static assessment, this paper focuses on the dynamic assessment system of tongue diagnosis based on Mashup technique (DASTD). The continuum of assessment is dynamically adapted according to specific userâs answering pattern of previous questions. Supplemented by the teaching materials constantly updated through the Mashup technique, the user is provided with pertinent information tailored to his needs. Dynamic assessment, coupled with Mashup-powered digital education, form an efficient framework for the learning of TCM tongue diagnosis. 34 college students, ranging from 1st to 3rd year, in TCM department are tested by the DASTD developed. The efficacy of the DASTD is evaluated based on the pre-test and post-test performance, with interleaving learning sessions targeting on the weakness of the student under test. The result of t-test demonstrates that significant difference is reached between gain scores of pre- and post-tests, and positive correlation between gain scores and length of time spent on learning, while no significant differences between the gender and post-test gain score, and the years of students in school and the progress in gain score.
