Background: Despite the fact that cervical cancer is known to be a preventable cancer, it remains one of the major causes of cancer-related deaths in females. A number of studies have attributed differences in clinical outcomes of cervical cancer to several factors such as stage at presentation, treatment pattern, and socioeconomic status. However, the association of specific genetic alterations with differences in clinical outcomes remains largely unexplored. Objectives: The initial research purpose was to identify the most common oncogene and tumor suppressor gene in cervical cancer with mutations and copy number alterations (CNAs). The focused research purpose was to examine the association of the identified oncogene and tumor suppressor gene with clinical outcomes and racial differences. Methodology: This study made use of the Cancer Genome Atlas (TCGA) database. The TCGA cervical cancer data were submitted between 2011 and 2014. The two genomic profiles used were mutation data and CNA data. The Fisher’s exact and chi-square tests were used to test for associations between the categorical variables. Logistic regression analysis was used to quantify the strength of associations. Results: There were 309 cervical cancer cases. Phosphatidylinositol3-Kinase Catalytic Subunit Alpha (PIK3CA) and Phosphatase and Tensin Homolog (PTEN) genes were identified as the most common oncogene and tumor suppressor gene respectively. 63 patients had mutations in PIK3CA or PTEN or both, and 70 patients had CNAs. The ORs (Exp(B)) of death and tumor recurrence for patients with mutations were 3.300(1.625– 6.700) and 2.461(1.120–5.407) respectively. The ORs of death and tumor recurrence for patients with CNAs were 2.316(1.282–4.186) and 2.383(1.228–4.624) respectively. The ORs for CNA positive for the Black race compared to White race was 2.378(1.137–5.452). Conclusions: Genetic alterations in PIK3CA or PTEN or both are associated with a higher risk of cervical cancer mortality and tumor recurrence. These genes can be explored as therapeutic targets to improve cervical cancer treatment. High prevalence of CNAs in African American women could be due to the fact that a larger percentage presented at a later stage as stages III and IV are significant predictors of the presence of CNAs in these genes.

▼ Pregnancy presents unique challenges for the mother to support the developing fetus in utero. Presented in this thesis are the adaptations of the cervix and renal system in response to the transition from a non-pregnant to pregnant state. Among a multitude of mechanisms that contribute to these adaptations, my thesis focuses specifically on the involvement of the transport of water molecules through aquaporins (AQPs) in the cell membrane. This is because pregnancy is characterised by a dramatic increase in cervix wet weight and increase in plasma volume controlled by the kidney. Although AQPs are reported to play a role in female physiology during pregnancy, specific mechanisms that regulate AQPs remain to be established. The peptide hormone relaxin is one factor known to be involved in cervical remodelling and changes in renal function during pregnancy. Therefore, the overall objective of this study was to investigate novel mechanisms of relaxin on AQPs in the regulation of extracellular matrix (ECM) remodelling in the cervix and osmoregulatory processes in the kidney. To test the hypothesis that relaxin increases cervical water content and collagen fibre dispersal in the ECM by regulating AQPs, I first investigated the gestational profile of cervical aqp3, aqp5 and aqp8 in the cervices of wildtype (Rln+/+) and relaxin deficient (Rln-/-) mice at different stages of gestation. Cervical aqp3 expression was reduced in late pregnant Rln-/- mice, whereas aqp5 expression was higher than Rln+/+ mice. In the second part of Chapter 3, I examined the effects of recombinant human H2 relaxin (rhRLX) treatment on cervical AQP expression in late pregnant Rln-/- mice. In this study, 4 and 6 days of rhRLX treatment increased cervical aqp3 and decreased aqp5. The up-regulation of AQP3 might increase water movement from the luminal epithelium into the cervical stroma and increase cervical hydration. Inhibition of AQP5 during late pregnancy might prevent water loss and retain fluid within the stromal compartment. These observations are correlated with increased cervix wet weight in day 18.5 gestational Rln+/+ mice and in rhRLX treated Rln-/- mice. My second major objective was to investigate the potential regulatory role of relaxin on AQP expression in the kidney during pregnancy. I first established the gestational profile of AQPs in the rat and mouse kidney in Chapter 4. Expression of all four major renal aqps (aqp1-4) in the cortex and medulla regions was similar between NP and gestational animals. This was an unexpected finding as AQP2 mRNA and protein are reported to increase in the inner medulla of pregnant rats. I also investigated the expression of vasopressin receptor 2 (avpr2), urea (ut-a1) and salt transporters (Scnn1a, Scnn1b, Scnn1g and atp1a) on the basis that APQ2 is regulated by vasopressin, and that relaxin may potentially affect renal salt reabsorption. La supervivencia a los 5 años de esta enfermedad, es superior al 90 % en los estadios precoces, lo cual ha hecho que se dirija la atención a la calidad de vida que tienen estas pacientes. La edad de las pacientes y el buen pronóstico hace que, además de asegurar buenos resultados oncológicos, se requiera como objetivo asegurar una mínima invasión y se persiga minimizar los efectos secundarios y secuelas derivadas del tratamiento . La cirugía radical del cáncer de cérvix inicial tiene como objetivo el extirpar el cérvix y el cuerpo uterino junto con el tejido paracervical y parametrial para erradicar el tumor y las vías principales de diseminación tumoral. La inervación autonómica visceral de la pelvis está ubicada mayoritariamente en esta región por lo que si no se realiza ninguna maniobra de protección de la misma su lesión es prácticamente segura. Funciones tan importantes como la defecación, la micción o la función sexual son referidas como aspectos muy importantes en la calidad de vida de estas pacientes.El presente estudio se subdivide en dos partes: 1-. ESTUDIO RETROSPECTIVO. Estudio observacional de cohortes retrospectivo que incluye a 102 pacientes diagnosticadas de cáncer de cérvix estadios IA2, IB1 y IIA1 tratadas mediante tres técnicas mínimamente invasivas en los hospitales Arnau de Vilanova de Valencia y el Hospital General Universitario de Valencia entre los años 2000 y 2015. 8 pacientes se trataron mediante un abordaje mixto laparoscópico-vaginal (Histerectomía radical vaginal asistida por laparoscopia-LAVRH). 29 pacientes se trataron mediante histerectomía radical totalmente laparoscópica sin preservación nerviosa (TLRH) y 65 pacientes fueron tratadas mediante una histerectomía radical totalmente laparoscópica con preservación de nervios autonómicos de la pelvis (NLRH). En todas las pacientes se realizó una linfadenectomía pélvica laparoscópica incluyendo los territorios iliaco externo, interno y obturatriz. Todas las pacientes fueron intervenidas por dos cirujanos (Dr. Gilabert-Aguilar y Dr. Gilabert-Estellés). Ante sospecha macroscópica de afectación ganglionar se procedió a estudio intraoperatorio. Las pacientes con positividad ganglionar fueron derivadas a tratamiento quimioradioterápico y no se incluyeron en el presente estudio. En todas las pacientes se recogieron variables epidemiológicas, clínicas y quirúrgicas a partir de las historias clínicas. ▼ Background: Cervical cancer has been identified as the second most common female malignancy and leading cause of death among women in the world. Despite concerted efforts through the Cervical Cancer Prevention Program in Zambia (CCPPZ), there has been observed low uptake of the service, with 80% of women presenting very late when only palliative care can be given. This study aimed at determining the predictors of abnormal cervical lesions among women (15-49 years old) in Zambia. Methods: The study was a cross-sectional study. 14,301 women (15-49 years old) from 6 provinces of Zambia were recruited. Secondary data from the CCPPZ was used for the study. SPSS version 21 was used for data analysis. Descriptive statistics using the chi2 test were used to explore proportions of women by their various socio-economic, demographic, medical and obstetric characteristics. Univariate logistic regression analysis was used to explore associations between the outcome variable (cervical lesions) and various predisposing factors. Finally, multiple logistic regression analysis was used to control for confounding, and thereby determine the predictors of abnormal cervical lesions in the final model. Results: The prevalence of abnormal cervical lesions was 10.7% (n=14,301). The predictors of abnormal cervical lesions were; education [AOR 0.684; 95% CI 0.505, 0.925; p=0.014] where women with tertiary education had a reduced risk compared to those with no formal education, marital status (married vs. never married) [AOR 0.776; 95%CI 0.624, 0.964; p= 0.022]. Other predictors were cigarette smoking (yes vs. no) [AOR 1.657; 95% CI 1.182, 2.322; p=0.003], condom use [0.652; 95% CI 0.434, 0.979; p=0.039] where women who always used a condom had a reduced risk compared to women who never used one, and HIV (positive vs. negative) [AOR 1.969; 95% CI 1.681, 2.305; p<0.001]. Conclusion: One in every ten women who screened for cervical cancer had abnormal cervical lesions. Education, marital status, smoking, condom use with regular partner and HIV status were significantly associated with having abnormal cervical lesions. There is, therefore, need for concerted efforts to design cervical cancer awareness programs targeting the high risk populations, including lobbying for a policy to make cervical cancer screening mandatory and routine for early detection and prevention of the development of cervical cancer.

▼ Cancer is one of the leading causes of mortality worldwide. About 44% of all cancer morbidity and 53% of all cancer mortality occur in countries with a low to medium Human Development Index (HDI). Thus, cancer is rapidly emerging as a serious threat to public health in Africa and most especially, sub-Saharan Africa. The International Agency for Research on Cancer (IARC) projects that there will be 1.28 million new cancer cases and 970 000 cancer deaths in Africa by the year 2030 owing to the increase in economic development associated lifestyles. The dominant types of cancer in Africa are those related to infectious diseases such as Kaposi’s sarcoma and cervical, hepatic and urinary bladder carcinomas. The main challenge to cancer treatment in Africa is the unavailability of efficacious anticancer drugs. This is because most developing countries can only afford to procure the most basic anticancer drugs, which are also frequently unavailable due to intermittent supplies. This results in patients progressing to more advanced cancer states. One way of combating this African problem is to focus on research that aims at discovering efficacious and cost effective cancer therapies from available natural resources within the African continent. This study investigated the potential anti-proliferative activity (against HeLa cervical cancer cells) of four plants (Adansonia digitata, Ceiba pentandra, Maytenus senegalensis and Drimia altissima) commonly used in the African traditional treatment of malignancies. After in vitro bio-assay screening using the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide (MTT) assay, M. senegalensis root extract (MS-R) and D. altissima bulb extract (DA-B) showed anti-proliferative activity against HeLa cervical cancer cells with IC50 values of 25 μg/mL and 1.1 μg/mL respectively. By possessing the strongest anti-proliferative activity among the tested extracts, D. altissima was selected for further studies. Liquid-liquid partitioning of the Drimia altissima bulb extract with n-hexane, ethyl acetate, and n-butanol, yielded partitions 79a – d, with the n-butanol fraction, 79d, exhibiting the strongest cytotoxic activity (IC50 = 0.497 μg/mL). Through High Content Analysis (HCA) screening, fraction 79d was found to induce marked early mitotic cell cycle arrest. Fractionation of 79d using Diaion® HP-20 open column chromatography and a stepwise gradient of reducing polarity (water-methanol-ethanol-ethyl acetate) yielded cytotoxic fractions 82b, 82c, 82d and 82e, all with significant anti-proliferative activities at the tested concentrations of 0.1, 1.0 and 10 μg/mL. Bio-assay guided fractionation of 82c (the most effective fraction at the lowest tested concentration of 0.1 μg/mL) using Sephadex® LH-20 open column chromatography and 50% MeOH led to the isolation of compound 3.17. ▼ Preterm birth, a delivery that occurs prior to 37 weeks of a 40 week gestation in women, is a leading cause of infant morbidity and mortality. Additionally, children born preterm who make it through their first year of life are at increased risk of medical complications throughout their lives. Costs associated with prematurity exceed $26 million each year in the United States alone, where nearly 1 in 10 babies is born preterm. The fact that preterm birth rates have only slight declined overall in the past 20 years can be attributed to the multiple, and most yet-to-be-identified, etiologies of the syndrome. More than 65% of all preterm births have no clinically identifiable cause. Of the premature deliveries with an identifiable cause, infection contributes to 40%. Regardless of the cause or timing of delivery, changes in the cervix precede the onset of labor. A better understanding of the pathological processes involved in premature cervical remodeling will allow for development of detection technologies and therapeutic approaches to preventing preterm birth. The goal of this study was to identify cervical pathways, distinctly regulated in response to inflammation, that lead to premature changes in extracellular matrix components, decreasing tissue biomechanical integrity and leading to preterm birth. Cervical elastic fiber ultrastructure becomes acutely disrupted in response to inflammation but not at term. RNA-seq studies identified enrichment of inflammasome activation and protease pathways in the cervix, both also exclusive to inflammation. Inflammasome-induced protease upregulation and subsequently increased activity targeting elastic fibers are potential mechanisms of premature cervical remodeling in response to inflammation, leading to preterm birth. These findings add to the understanding of how the tissue responds to inflammation and how this response can induce extracellular matrix changes that impact the biomechanical integrity of the cervix. Future investigations will focus on potential therapeutic approaches that target mechanisms upstream of protease activation to prevent disrupted extracellular matrix architecture. The effects of these studies have the potential to extend beyond first pregnancies impacted by infection; risk of preterm birth in subsequent pregnancies, which increases exponentially as the number of preterm deliveries a woman experiences increases, may also be lessened. ▼ The cervix serves as the passage for the fetus during birth. The mechanical function of the cervix is crucial for a healthy term pregnancy: 1) prior to term it must remain closed and resist the increasing mechanical load from the growing pregnancy and 2) at time of parturition it must soften, deform and dilate to allow for delivery of the fetus. After delivery, the cervix must repair and close. The timing and characteristics of this remodeling behavior is currently an active research focus because it is hypothesized that premature remodeling in pregnancy can lead to preterm birth, a leading cause of neonatal death or significant neonatal morbidity. The research goal was to measure and characterize anisotropic material properties because they contribute to keeping cervix shut. In this thesis, the collagen fiber network orientation and dispersion of non-pregnant and pregnant human cervical tissue samples were analyzed using optical coherence tomography, and the samples were tested using mechanical indentation and digital image correlation techniques. Human cervices were acquired from non-pregnant and pregnant consented patients that went through hysterectomy. Axial cervical slices were imaged using optical coherence tomography and fiber orientation and dispersion data was analyzed using a new pixel-wise fiber orientation algorithm and was compared among four anatomical quadrants and among patients with different obstetric backgrounds. Two radial zones with different fiber orientations were found. The posterior and anterior quadrants of the outer zone were found to have distinct fiber dispersion features and their fiber dispersion shifted most dramatically from non-pregnant to pregnant. In an effort to characterize the compressive mechanical behavior of human cervical tissue, we present a novel indentation test with digital correlation imaging to visualize the real-time deformation of cervical slice during indentation and measure the compressive mechanical properties through coupled finite element analysis with collagen fiber orientation and dispersion information informed by OCT of non-pregnant and term pregnant cervical tissue. Heterogeneity within the same cervix and difference between non-pregnant and pregnant cervices were found. The upper cervix was found to have a stronger ground substance. The anterior and posterior quadrants were less compressible than the left and right quadrants for non-pregnant specimens. The upper cervix of non-pregnant patients had a stronger ground substance than that of pregnant patients. A workflow of optical, mechanical, and chemical experiments on the same piece of specimen with most fibers intact was also proposed in this thesis and these experiments would validate and inform each other.

▼ Cervical ripening is fundamental to the process of parturition. Recent animal studies have suggested that the inflammatory mediator nitric oxide is involved in this process. The aim of the studies reported in this thesis was to investigate the potential role of the L-arginine-nitric oxide system in ripening the cervix in humans. The nitric oxide donor isosorbide mononitrate (IMN) administered per vaginam has previously been shown to ripen the human cervix. The studies reported in chapter 2 were performed in order to compare the effectiveness and side effect profile of the nitric oxide donor (IMN) with the prostaglandin analogue gemeprost. 66 primigravid women in the first trimester of pregnancy were therefore recruited to this randomised controlled trial. A significantly greater percentage of women remained asymptomatic following IMN (64%) than following gemeprost (14%). Pre-treatment with gemeprost resulted in abdominal pain in 73% of women and vaginal bleeding in 32% compared with 3% and 0% respectively following IMN. These data suggested that IMN could be used as an alternative to gemeprost for this indication. A further randomised controlled trial is reported in chapter 3. This explored the hypothesis that combined therapy with IMN (40mg) and the prostaglandin analogue misoprostol (400?g) for preoperative cervical ripening in the first trimester would result in improved clinical effectiveness and fewer side effects compared to each agent used alone. There was no difference in the number of women remaining asymptomatic following either IMN or misoprostol or combination therapy (14/22 [64%] vs 11/21 [52%] vs 11/22 [50%], Fisher's exact test). Pre-treatment with misoprostol used alone resulted in lower cervical resistance than IMN (18.5 N (Newtons) vs 39 N, p=0.04, Mann-Whitney U). The cervical resistance following combination therapy with IMN and misoprostol was not significantly lower than following misoprostol alone (24.5 N vs 18.5 N). These data suggest that the nitric oxide donor IMN alone or in combination with misoprostol has no advantages over misoprostol alone for preoperative cervical ripening in the first trimester. The studies reported in chapter 4 were performed to investigate whether an increase in nitric oxide production occurs in the human cervix in conjunction with cervical ripening. Using Western blotting and immunohistochemistry, the expression and localisation of the enzymes responsible for the production of nitric oxide, inducible nitric oxide synthase (iNOS), endothelial nitric oxide synthase (eNOS) and neuronal nitric oxide synthase (bNOS) were determined in the human uterine cervix during pregnancy and parturition. The expression of each of the NOS isoforms was greater in cervical tissue from pregnant women compared to non-pregnant. iNOS and bNOS protein expression increased in the first trimester of pregnancy compared to non-pregnant samples (p < 0.005 for iNOS and bNOS respectively). iNOS was further upregulated in the third trimester of pregnancy prior to the onset of labour (p < 0.01).…

▼ textabstract(English): There is a strong biological rationale for the use of hyperthermia as an oncological treatment modality. Fifteen randomized trials have shown significant improvement in clinical outcome when hyperthermia was added to radiotherapy, chemotherapy or both. At temperatures ≥ 40 0C, heat can cause direct celldeath, especially affecting cells that are relatively resistant to chemotherapy and radiotherapy. In this thesis, the status of hyperthermia in the treatment of locally advanced cervix cancer in the Netherlands is presented and factors predicting outcome are identified. Further, one possible way of improving treatment quality, i.e. by using hyperthermia treatment planning, is explored. The addition of hyperthermia to in this clinical setting results in a significant improvement in local control and a doubling of overall survival, without adding to long-term treatment-related toxicity. Patient-related predictive factors in this clinical setting are tumor stage, tumor size and patient performance status, while the radiation dose as well as thermal dose are predictive treatment-related factors. We found a significant relationship between thermal dose and clinical outcome for 420 patients, indicating that improvement in clinical outcome can be achieved with increased thermal dose. With the help of a hyperthermia treatment planning system, a computer program that optimizes temperature distributions for specific patients, possibly treatment quality can be improved and the thermal dose can be increased. In this thesis, such a computer program is described and adjusted for routine clinical use. Lastly, we conducted a randomized trial to assess its current contribution to treatment quality and thermal dose. Vijftien gerandomiseerde onderzoeken hebben aangetoond dat de klinische uitkomst significant verbeterd als hyperthermie wordt toegevoegd aan radiotherapie, chemotherapie of beide. Bij temperaturen ≥ 40 0C, kan warmte direct celdood veroorzaken, met name bij cellen die relatief ongevoelig zijn voor de effecten van radiotherapie en chemotherapie. In dit proefschrift wordt de huidige status van hyperthermie in de behandeling van locaal uitgebreide baarmoederhalskanker onderzocht en worden prognostische factoren geïdentificeerd. ABSTRACTBackground: Recognizing resistance or susceptibility to the current standard cisplatin and paclitaxel treatment could improve therapeutic outcomes of metastatic or recurrent cervical cancer.Methods: Forty-five tissue samples from patients participating in a phase II trial of cisplatin and ifosfamide, with or without paclitaxel were collected for retrograde analysis. Immunohistochemistry and genotyping was performed totest ERCC1, III β-tubulin, COX-2, CD4, CD8 and ERCC1 (C8092A and N118 N) and MDR1 (C3435T and G2677 T) gene polymorphisms, as possible predictive and prognostic markers. Results were statistically analyzed and correlatedwith patient characteristics and outcomes.Results: Patients with higher levels of ERCC1 expression had shorter PFS and OS than patients with low ERCC1 expression (mPFS:5.1 vs 10.2 months, p = 0.027; mOS:10.5 vs. 21.4 months, p = 0.006). Patients with TT in the siteof ERCC1 N118 N and GT in the site of MDR1 G2677 T polymorphisms had significantly longer PFS (p = 0.006 and p = 0.027 respectively). ERCC1 expression and the ERCC1 N118 N polymorphism remained independent predictors of PFS. Interestingly, high III beta tubulin expression was associated with chemotherapy resistance and fewer responses [5/20 (25%)] compared to lower III β-tubulin expression [15/23 (65.2%)] (p = 0.008). Finally, ΙΙΙ β-tubulinlevels and chemotherapy regimen were independent predictors of response to treatment.Conclusions: ERCC1 expression proved to be a significant prognostic factor for survival in our metastatic or recurrent cervical cancer population treated with cisplatin based chemotherapy. ERCC1 N118 N and MDR1 G2677 T polymorphism also proved of prognostic significance for disease progression, while overexpression of III β-tubulin was positively correlated with chemotherapy resistance.

▼ ENGLISH SUMMARY : Background: Cervical cancer is the second-most frequently diagnosed and the fourth commonest cause of cancer death among women worldwide. Almost 70% of the global burden occurs in areas of lower development. Incidence and mortality rates of cervical cancer among women in Ghana are of the highest in the world. According to the Ghana Health Service,16% of cancer mortality is attributed to cervical cancer. It has also been predicted by the World Health Organization that by the year 2025, 5 000 new cases of cervical cancer and 3 361 cervical cancer deaths will occur annually in Ghana. Purpose: This study aimed to determine factors affecting the utilisation of cervical cancer screening among women attending health services in the Kumasi metropolis of Ghana. Methods: A correlational cross-sectional quantitative study design was used. The target population was women of 18 years and above seeking reproductive health services in the Kumasi metropolis during the study period. A total of 369 participants were selected using a multistage sampling technique. Initially, two of the four hospitals were randomly selected. Subsequently, a systematic sampling technique was used in selecting participants to participate in the study. Data was collected using a researcher-designed questionnaire consisting of 38 closed-ended and open-ended questions. The questionnaire was printed in English and in the Twi language. A pilot study was conducted using 20% of the daily average attendance (200) in the four hospitals in the Kumasi metropolis. Ethical approval to conduct the study was obtained from the Health Research Ethical Committee at Stellenbosch University, as well as consent from the institutions under study. Informed consent was obtained from all the participants. Data was analysed using Statistical Package for Social Science (SPSS) Version 23.0 and findings are presented using descriptive and inferential statistics with 0.05 as the significance threshold. Results: Of the 369 participants, 58% were between the ages of 18 and 30 years, 46.1% were single, and 27.9% and 27.1% had primary and tertiary levels as their highest levels of education respectively. Of the participants, 67.5% were employed, 29.3% had only one child followed by 24.1% who had two children. Of the participants, 75.1% had adequate knowledge on cervical cancer with a mean knowledge score of 7.70±2.13 with range, 2–11 and about 88% had a good perception of cervical cancer and cervical cancer screening. The majority (n=300; 81.3%) had never been screened while 69 (18.7%) had been screened before. Of the 69 (18.7%), 17.3% had been screened only once and 1.4% had been screened twice. The majority of participants were not sure whether cervical screening was painful (46.6%) or expensive (32%), and 41.2% strongly agreed that their partners would not want them to have cervical cancer screening. Cervical cancer is the most frequently diagnosed cancer among women in Zambia. More recently, improved cure rates have been obtained with the concomitant use of radiotherapy and chemotherapy in locally advanced cancer of the cervix. However, the side effects associated with the treatment have a major impact on the quality of life (QoL) of these women. Prior to this study, QoL in Zambian women treated for cervical cancer with chemo-radiation had not been assessed thus creating a gap in the literature and hampering an attempt to improve QoL in this cohort of patients. The aim of this study was therefore to evaluate the impact of chemo-radiation treatment on QoL and to determine what socioeconomic and demographic factors are closely related with QoL decrements in Zambian women treated for cervical cancer at Cancer Diseases Hospital. This would then facilitate the introduction of intervention programmes aimed at improving QoL in these patients. The study was prospective and explored the phenomenon of QoL with the use of the European Organization for Research and Treatment of Cancer (EORTC) questionnaire and a demographics questionnaire answered by 45 women treated for cervical cancer with chemo-radiotherapy at Cancer Diseases Hospital. The use of the EORTC quality of life questionnaire added validity and reliability to the study as it is used extensively to measure health-related quality of life in cancer survivors worldwide. Analysis of the data indicates that patients with advanced cervical cancer treated with chemo-radiotherapy generally experienced a favourable QoL, and treatment was considered worthwhile by the majority of patients. However, women described problems with sexuality and marital relationships. Low education and living without a partner were depicted as risk factors for the development of the reported problems. To improve QoL in survivors, interventions focusing on more social support, education to improve patients’ understanding of their disease and treatment effects as well as physical rehabilitation through exercise interventions are recommended as mandatory.

Summary: Background: A global cervical cancer health disparity persists despite the demonstrated success of primary and secondary preventive strategies, such as cervical visual inspection (VI). Cervical cancer is the leading cause of cancer incidence and death for women in many low resource areas. The greatest risk is for those who are unable or unwilling to access screening. Barriers include healthcare personnel shortages, cost, transportation, and mistrust of healthcare providers and systems. Using community health workers (CHWs) may overcome these barriers, increase facilitators, and improve participation in screening for women in remote areas with limited access to clinical resources. Aim: To determine whether the accuracy of VI performed by CHWs was comparable to VI by physicians or nurses and to consider the affect components of provider training had on VI accuracy. Methods: A systematic review and quantitative meta-analysis of published literature reporting on VI accuracy, provider type, and training was conducted. Strict inclusion/exclusion criteria, study quality, and publication bias assessments improved rigor and bivariate linear mixed modeling (BLMM) was used to determine the affect of predictors on accuracy. Unconditional and conditional BLMMs, controlling for VI technique, provider type, community, clinical setting, HIV status, and gynecological symptoms were considered. Results: Provider type was a significant predictor of sensitivity (p=.048) in the unconditional VI model. VI performed by CHWs was 15% more sensitive than physicians (p=.014). Provider type was not a significant predictor of accuracy in any other models. Didactic and mentored hours predicted sensitivity in both BLMMs. Quality assurance and use of a training manual predicted specificity in unconditional BLMMs, but was not significant in conditional models. Number of training days, with ≤5 being optimal, predicted sensitivity in both BLMMs and specificity in the unconditional model. Conclusion: Study results suggest that community based cervical cancer screening with VI conducted by CHWs can be as, if not more, accurate than VI performed by licensed providers. Locally based screening programs could increase access to screening for women in remote areas. Collaborative partnerships in “pragmatic solidarity” between healthcare systems, CHWs, and the community could promote participation in screening resulting in decreased cervical cancer incidence and mortality.

▼ The research described in this thesis concerns presence of asymptomatic vaginal Candida and vulvovaginal candidiasis. Vulvovaginal candidiasis (VVC) is an infection caused by abnormal growth of yeasts in the mucosa of the female genital tract. Acute vulvar pruritus and vaginal discharge are the usual presenting complaints, and vulvovaginal examination frequently reveals epithelial erythema and swelling of the labia and vulva together with adherent whitish discharge. However, due to lack of specificity of signs and symptoms, all patients with symptomatic vaginitis should be diagnosed on the basis of microscopic examination of their vaginal secretions. In the first chapters of this thesis, we examined how general practitioners (GPs) in The Netherlands diagnose and treat vaginal candidiasis, by sending questionnaires to 1160 Dutch GPs and by checking the microscopy performed by 324 Dutch GPs on stained vaginal smears of patients with symptomatic vaginitis. The questionnaires revealed that only 61 (16.1%) Dutch GPs always or often performs microscopy when diagnosing candidiasis, while 143 (37.6%) GPs never use a microscope to confirm their diagnosis. In addition, sensitivity and specificity of the microscopic diagnoses of the clinicians was only 52% and 89% respectively. This suggests that GPs often diagnose 'vulvovaginal candidiasis' in their practices, without (adequately) performing the laboratory examinations required to confirm their putative diagnosis, which could lead to wrong diagnoses and maltreatment. However, Candida (pseudo)hyphae and/or blastospores are also often discovered in cervical smears of asymptomatic women during their childbearing years. Since 1996 all women in the Netherlands between the ages of 30 and 60 receive an invitation once every five years to have a cervical smear taken. These smears are coded according to the Dutch national coding system for cervical cytology (KOPAC). KOPAC is an acronym, in which the O-category stands for ontsteking (inflammation) and the P-category reflects the status of the plaveiselcellen (squamous epithelium). The research described in this thesis involves cervical smears that were taken as part of the national screening program in the western region of The Netherlands and examined by the Leiden Cytology and Pathology Laboratory. The Leiden database, containing the results of the smears of almost 500,000 women, was used to study possible relationships between presence of asymptomatic Candida, immigrant status, (pre)neoplasia, and the incidence of acquiring other vaginal infections in time, by the cytological identification of Candida and other elements of the vaginal flora in smears. This research showed that presence of vaginal Candida is not associated with an increased risk for (pre)neoplastic cells in the same smear and in follow-up smears and does not predispose to an increased susceptibility for Trichomonas vaginalis, Gardnerella vaginalis and Actinomyces. ▼ Cervical cancer is the fourth most common cancer in women, with more than half a million new cases worldwide each year. In the majority of cervical cancers a persistent infection with high-risk (hr) Human papillomavirus (HPV) types has been proven to be the causative agent. Prevention of cervical cancer can be improved by studying the detection of cervical precancer by colposcopy and by understanding the natural history of different HPV genotypes. This thesis addresses different approaches to improve colposcopic accuracy. Furthermore, the biology of different hrHPV genotypes is studied in premalignant lesions. Despite the important role of colposcopy in detecting high-grade disease, sensitivity is relatively low. We studied whether colposcopic accuracy can be improved by hrHPV genotyping and by studying colposcopic characteristics systematically. In a population following European screening practice, HPV16 related lesions were detected at younger age and showed similar colposcopic impression as non-16 hrHPV high-grade lesions. There was no relation between any of the colposcopic characteristics and HPV16 status. To further investigate improvement of colposcopic diagnostic accuracy, we evaluated improved detection of high-grade lesions by taking multiple lesion-directed biopsies. We showed that a second lesion-directed biopsy is associated with a significant increase in disease detection. We suggest that performing an additional lesion-directed biopsy should become the standard practice of colposcopy. Atypical immature metaplasia (AIM) is an equivocal category used to describe changes in immature metaplasia (IM) suggestive of high-grade neoplasia which causes diagnostic and management problems. The relative infrequency of HPV18 in high-grade disease and its importance in cervical cacner might be explained by infection of glandular cells or IM cells . The role of hrHPV infected AIM in cervical neoplasia was studied. AIM was a particular feature of HPV18 infection in women with high-grade lesions. Women with high-grade lesions frequently present with multiple cervical lesions and multiple concomitant HPV genotype infections. Studying causal genotype attribution to cervical disease is important to measure the effect of vaccination and to study HPV types to be included in HPV assays for cervical screening. We performed molecular mapping of cervical disease in a small pilot study in women with high-grade CIN to elucidate HPV genotype attribution in different regions on the cervix. HPV16 was identified as the causal type in all women with HPV16 in cytology. In this US population we were able to attribute an individual HPV type to each area of CIN. We demonstrated that HPV16 is even more etiologically dominant than previously thought, based on various genotype attribution models. Genotype attribution to high-grade CIN was also evaluated in a large population based study of 257 women with high-grade lesions. ▼ Infection \\'ith specific high risk human papilloma"iruses (HPV) has been shown to play a causal role in the development of ceJVical intraepithelial neoplasia (CIN) and cenical cancer in women. The development of a prophylactic vaccine to immull.ize women against HPV infection would play a \'ita! role in protecting women against HPV infection and ultimately ceMcal cancer. Despite cancer of the cer\'ix being the second most common cancer in South African women, a literature search reveals that few studies have been performed in South Africa on the types of HPV prevalent in women with CIN or cancer ofthe ceMx. HPVs that infect the anogenital tract have also been shown to infect the oral ca\'ity. However, the HPV prevalence rates vary greatly between studies and the significance of the presence ofHPV in the oral ca\'ity is still not understood. The primary objectives of this study were to establish the HPV prevalence rate infecting women with CIN lesions using a sensitive nested polymerase chain reaction (PCR) and to develop a novel restriction fragment length polymorphism (RFLP) method to type the high risk mucosal HPVs detected in these women. The secondary objective of this study was to establish the prevalence rate and HPV types infecting the oral mucosa of women with CIN lesions and to compare these HPV types with those detected in the ceMx. Cemcal punch biopsies were taken from 163 women with CIN lesions and buccal cells were collected from 33 of these participants. DNAwas extracted from the biopsies and buccal samples and PCR using CCRS primers performed to ensure sample adequacy. Nested PCR usmg consensus degenerate primers for HPV was performed on all samples sho\\'wg sufficient amplifiable DNA A novel restriction fragment length pol)morphism (RFLP) method was developed to identify the 10 high risk mucosal HPVs considered human carcinogens of group 1 by the International Agency for Research on Cancer (lARC) as well as HPV 11 which is commonly found in the oral cavity.

▼ Cervical cancer is one of the major health challenges amongst women in Zambia. While cervical cancer screening has been available in Zambia since 2006 with a relatively wide coverage in Lusaka District, it remains questionable whether women with disabilities have access to these cervical cancer screening services. The aim of this study was to determine factors which influence the accessibility of cervical cancer screening services among women with physical and sensory disabilities. Using a qualitative case study, data was collected through in-depth interviews and key informant interviews from 12 women with disabilities and 6 representatives from health centres and disability organisations respectively in Lusaka District. The data was collected focussed on factors which affect accessibility of cervical cancer screening services amongst women with physical and sensory disabilities. Supplementary document study and field observations were done. Data was analysed using Nvivo 10.0 Windows Evaluation Version to conduct thematic content analysis of key themes and sub-themes of service accessibility of cervical cancer screening. Results showed that cervical cancer screening services were not easily accessible for women with disabilities, with the type and severity of disability being major catalysts. Several factors were found to influence accessibility of cervical cancer screening services; among them were previous negative medical experiences, limited knowledge, communication challenges, social support, inadequate income, distance to nearest screening centre, inappropriate transport, little implementation of legislature and failure to incorporate women with disabilities into existing health policies. Efforts should be directed at improving service delivery of cervical cancer screening by integrating women with disabilities in health promotion activities. There is need to devise strategic ways in which existing legislature can be efficiently enforced so as to compel health facilities to be more accessible to women with disabilities. It is important that future health policy planning should consider the interests of vulnerable populations such as women with disabilities.
