▼ Burkholderia pseudomallei is the causative agent of the disease melioidosis and is classified as a category B Select Agent. There are currently many challenges associated with both the study of this pathogen and its treatment in the clinical setting. Prior to these studies, there was no attenuated B. pseudomallei strain available that was exempt of Select Agent regulations and approved for study outside of biosafety level 3 (BSL-3) containment, and consequently basic research on this pathogen was largely hindered. The first purpose of these studies was to extensively characterize the attenuation of two B. pseudomallei mutant strains using melioidosis animal models. The two mutants constructed were Bp82 and Bp190, Δ purM derivatives deficient in adenine and thiamine biosynthesis. These mutants were found to be fully attenuated in immune competent and immune deficient mouse and hamster melioidosis models. Bp82 is currently exempt of all Select Agent regulations and can be safely handled in the BSL-2 setting, greatly accelerating research on this priority pathogen. Since basic research on B. pseudomallei was not common in the Western world until its Select Agent classification, much is still unknown regarding the bacterial factors contributing to its virulence. A second purpose of this research was to determine whether resistance-nodulation-cell division (RND) efflux systems and iron acquisition siderophores impact the virulence of B. pseudomallei in a pneumonic murine melioidosis model. This was examined using a clinical isolate naturally devoid of a characterized efflux system and the gene cluster for malleobactin siderophore synthesis, and by the construction of isogenetic mutants. The two characterized B. pseudomallei efflux pumps, AmrAB-OprA and BpeAB-OprB, were both found to be completely dispensable during in vivo murine infection. The removal of one or both of these systems did not reduce lethality of the mutant strains. Unlike that observed with similar bacterial pathogens, the lethality of B. pseudomallei was also not reduced upon the removal of either the malleobactin or pyochelin siderophores. This finding indicates B. pseudomallei is likely capable of utilizing alternative systems for iron acquisition within the host. In addition to the challenges associated with the study of this pathogen, there are also many clinical challenges associated with melioidosis, providing a basis for the final two purposes of this research. One particular challenge is the high frequency of patient relapse, even after appropriate prolonged antibiotic therapy. A third purpose of this research was to determine whether traditional antibiotic therapy could be augmented by the co-administration of immunotherapy. Cationic liposome-DNA complexes (CLDC), which are potent activators of the innate immune system, were found to synergistically reduce intracellular B. pseudomallei concentrations in macrophages in vitro when combined with the antibiotic ceftazidime. ▼ Burkholderia pseudomallei is a Gram negative, facultative intracellular pathogen which infects both phagocytes and non-phagocytes and causes severe acute infections in humans and animals. Due to its inherent resistance to many classes of antibiotics, new therapies are needed which can supplement or substitute for conventional treatments in order to combat this emerging infectious disease. We have previously shown that interferon (IFN)-γ can interact with the conventionally administered antibiotic, ceftazidime, to synergistically control intracellular bacteria burden of Burkholderia infected macrophages. The goal of the studies presented here was to determine the mechanism by which IFN-γ and ceftazidime exert their synergistic effect. After investigating several potential mediators of immuno-antimicrobial synergy, we showed that IFN-γ stimulation of macrophages led to increased generation of reactive oxygen species (ROS), which led us to hypothesize that IFN-γ induced ROS may interact with ceftazidime to control intracellular bacterial burden. We next found that ROS scavenging antioxidants such as N-acetylcysteine (NAC) and reduced glutathione (GSH) were capable of reversing the IFN-γ and ceftazidime synergistic effect, while the ROS-inducing drug buthionine sulfoximine (BSO) could not only potentiate the synergy, but could completely substitute for IFN-γ to synergize with ceftazidime and control intracellular bacterial burden. These results were consistent with a ROS interaction with ceftazidime. We further showed that IFN-γ prevented vacuolar escape and actin polymerization, a finding which was recapitulated with BSO. Taken together, these results suggested that generation of IFN-γ induced ROS responses synergized with ceftazidime to enhance control of intracellular bacterial burden. IFN-γ induced ROS was also responsible for preventing vacuolar escape and therefore may have limited intracellular replication and spread of infection. In the second half of our study we identified and then investigated the separate and compartmentalized contributions of IFN-γ and ceftazidime to the overall synergistic effect. We determined that ceftazidime alone controlled extracellular killing in our macrophage infection model while IFN-γ alone controlled the killing of Burkholderia in the intracellular compartment. We confirmed a role for IFN-γ induced ROS responses to kill intracellular bacteria and control intracellular replication, though we also conclude that other IFN-γ-dependent and ROS-independent pathways are at play. Overall we suggest a new model to describe the dynamics of the classically used macrophage infection model. We suggest that both intracellular and extracellular control of bacteria is required for the overall synergistic effect we see with combination of IFN-γ and ceftazidime. Together our studies have implications for the use of IFN-γ, or other ROS-inducing drugs, as non-specific antibiotic potentiating agents for enhanced clearance of bacterial pathogens. ▼ The increasing prominence of pulmonary drug delivery for local and systemic treatments coupled with the complex lung structure and the numerous factors affecting the pulmonary delivery of drugs has necessitated extensive research on this drug delivery route. The research mostly includes studies on characterization of aerosolized formulations in terms of their aerodynamic properties and particle size, and also in vitro, ex vivo and in vivo evaluation of the efficacy and the pharmacokinetic and pharmacodynamic properties of the formulations. However, a need is felt for improved models of aerosol testing, which closely simulate the aerosolization of the formulation in the lungs and in which two or more parameters for aerosol characterization can be simultaneously tested. This Master’s thesis describes the development and validation of an improved in vitro model of the diseased lung which can be used for accurate evaluation of inhaled formulations.|An enhanced in vitro model of the lung was created by carrying out modifications in the collection cups of the Next Generation Impactor (NGI) to allow incorporation of bacterial cultures for evaluation of formulation efficacy. The regular collection cup in the stage 4 of the NGI was replaced with a deep collection cup for incorporating bacterial cultures without affecting the critical impaction distance. Various modifications using agar plates, polymer as a collection surface and liquid collection surface were carried out. Theophylline solution was nebulized in both the regular and modified NGI and the particle size distribution and aerodynamic parameters such as Mass Median Aerodynamic Diameter (MMAD), Geometric Standard Deviation (GSD) and Fine Particle Fraction (FPF) in both the models were compared. The modification using liquid collection surface was chosen for further studies as the parameters obtained in the modified NGI modified did not display any statistically significant difference from those obtained in the regular NGI.|Ceftazidime was chosen as the antibiotic of choice because of its broad spectrum of activity and potential use in the treatment of pneumonia. Escherechia coli K12 MG 1655 was chosen for the bacterial studies because of its susceptibility to antibiotics, allowing for unbiased evaluation of the modified NGI performance. The sterility testing in the modified NGI in the laboratory environment indicated prevention of environmental contamination in the NGI. The airflow in the NGI and the excepients of ceftazidime formulation did not cause any reduction of bacterial count in the modified NGI. Ceftazidime solution nebulized in the modified NGI was able to completely inhibit bacterial growth.|The selected modification in the NGI allowed for testing of aerosolized antibiotic formulation efficacy. However, further studies need to be performed to widen the scope of the application of the modified NGI as an enhanced model for in vitro aerosol testing. Bacterial keratitis is an infectious disease of the cornea that is characterised by inflammation. The common pathogens associated with this disease include Staphylococcus aureus, coagulase-negative Staphylococcus, Pseudomonas aeruginosa, Streptococcus pneumonia and Serratia species, causing 90% of the bacterial keratitis cases. Currently, treatment of bacterial infections and inflammation in the eye has the problem of anatomic barriers and the delicate nature of the eye. Local drug applied to the eye represents a non-invasive, safe and less painful solution than surgery, laser treatments or eye injections. Ceftazidime is a third generation cephalosporin antibiotic which is effective mainly against pathogens that are usually responsible for ophthalmological infections. Ceftazidime offers a good coverage against Pseudomonas aeruginosa as well as resistance to several types of beta lactamases. The aim of the present work was to prepare a mucoadhesive nanoparticle eye drop formulation containing ceftazidime to treat eye infections. The first step for the development of the eye drop formulation was to prepare a vehicle for nanoparticles. Two different polymers were selected: carboxymethyl cellulose and hydroxypropylmethyl cellulose, both in low and high viscosity. The selection was based in studies of viscosity, zeta potential and interaction with the nanoparticles. After the tests, the work continued with hydroxypropylmethyl cellulose. The development of mucoadhesive chitosan-nanoparticles are proposed as effective delivery systems for ceftazidime through ocular epithelium, taking advantage of the favourable biological properties of hyaluronic acid and chitosan to prolong precorneal residence time of the antibiotic, enhancing drug accumulation and permeation. Nanoparticles were prepared by ionotropic gelation between sodium tripolyphosphate and chitosan with the objective to encapsulate ceftazidime. The formulations were characterized in terms of pH, osmolality, viscosity, zeta potential, particle size distribution and encapsulation efficiency. In addition, the in vitro release and permeation studies were performed and the results suggest a prolonged drug release from the nanoparticles. The results of nanoparticles interaction with mucin show their mucoadhesivity and ability to interact with the ocular surface increasing the drug residence time in the eye. The prepared nanoparticles were subjected to stability and microbiological studies with satisfactory results. In conclusion, Chitosan/TPP-Hyaluronic Acid nanoparticles proved to be a promising platform for ceftazidime delivery in the eye.

▼ Σκοπός της μελέτης ήταν να καθορισθεί εάν η μονοθεραπεία με σιπροφλοξασίνη σε υψηλές δόσεις (400 mg x 3φορές/ημέρα ενδοφλεβίως), είναι αποδεκτή ως αρχική εμπειρική αντιμικροβιακή θεραπεία σε εμπύρετα επεισόδια ουδετεροπενικών ασθενών με αιματολογικά νοσήματα σε σχέση με τον καθιερωμένο συνδυασμό κεφταζιντίμης+αμικασίνης, και επιπρόσθετα να διερευνηθεί εάν η χορήγηση σιπροφλοξασίνης από του στόματος (750 mg x 2 φορές/ημέρα) μπορεί αποτελεσματικά να υποκαταστήσει την ενδοφλέβια χορήγηση 72 ώρες μετά την έναρξη αυτής. Σε προοπτική τυχαιοποιημένη μελέτη μελετήθηκαν συνολικά 123 εμπύρετα ουδετεροπενικά επεισόδια, σε 61 από τα οποία χορηγήθηκε σιπροφλοξασίνη και σε 62 ο συνδυασμός κεφταζιντίμης+αμικασίνης. Επί του συνόλου σε 96 (78,1%) η υποκείμενη νόσος ήταν οξεία λευχαιμία, σε 24 (19,5%) μη-Hodgkin λέμφωμα και σε 3 (2,4%) απλαστική αναιμία. Επιτυχής κλινική ανταπόκριση στο τέλος της θεραπείας χωρίς τροποποίηση αυτής παρατηρήθηκε σε 30/61 (49,2%) επεισόδια στην ομάδα της σιπροφλοξασίνης και σε 31/62 (50%) στην ομάδα της κεφταζιντίμης+αμικασίνης (p=1,00). Σε 40/61 (65,6%) επεισόδια στην ομάδα της σιπροφλοξασίνης η ενδοφλέβια θεραπεία αντικαταστάθηκε με από του στόματος μετά από 4,3±069 ημέρες, με επιτυχή ανταπόκριση στο 75%(30/40) αυτών. Επί του συνόλου των επεισοδίων στις μικροβιολογικά αποδεδειγμένες λοιμώξεις κυριαρχούσαν οι Gram-θετικοί μικροοργανισμοί σε ποσοστό 56,8%, ενώ οι Gram-αρνητικοί αποτελούσαν το 43,2%. Η ανταπόκριση στην θεραπεία με σιπροφλοξασίνη στις μικροβιολογικά αποδεδειγμένες λοιμώξεις (βακτηριαιμίες και άλλες λοιμώξεις) ήταν 40,0%, στις κλινικά αποδεδειγμένες λοιμώξεις 50% και στα επεισόδια πυρετού αγνώστου αιτιολογίας 55,2%, έναντι 41,2%, 43,8% και 58,6% αντίστοιχα στην δεύτερη ομάδα. Ειδικότερα στις βακτηριαιμίες από gram-θετικά παθογόνα επιτυχής έκβαση παρατηρήθηκε στο 20% των επεισοδίων στην ομάδα της σιπροφλοξασίνης και στο 12,5% στην ομάδα του συνδυασμού. Στη διάρκεια της μελέτης παρατηρήθηκαν 6 επιλοιμώξεις μόνο στην ομάδα της σιπροφλοξασίνης και 4 επαναλοιμώξεις μόνο στην ομάδα του συνδυασμού κεφταζιντίμης/αμικασίνης, (p=0,006). Οι επιλοιμώξεις ήταν δύο μυκητιασικές πνευμονίες, μια λοίμωξη ουροποιητικού από P. aeruginosa, και τρεις βακτηριαιμίες από P. Aeruginosa, St. viridans και Enterococcus spp.. Οι επαναλοιμώξεις αφορούσαν δύο βακτηριαιμίες μία από P. aeruginosa και μια από St. Aureus, μία λοίμωξη ουροποιητικού από E. coli και μία γαστρεντερίτιδα από Ε. faecalis. Ο αριθμός των θανάτων στις πρώτες 72 ώρες ήταν 2 για την ομάδα της σιπροφλοξασίνης και 1 για την ομάδα κεφταζιντίμης+αμικασίνης. Επιπλέον 11 ασθενείς ακόμη (7 vs 4, p=0,363) κατέληξαν στη φάση της ουδετεροπενίας. Επί του συνόλου 12/14 θανάτους οφείλονταν σε λοίμωξη. Από τη στατιστική ανάλυση των δεδομένων προκύπτει ότι η παρατεταμένη και σοβαρή ουδετεροπενία αποτελεί σημαντικό παράγοντα για την ανταπόκριση στη θεραπεία. THE CONCENTRATION OF SIX CEPHALOSPORINS AFTER SINGLE OR REPEATED DOSAGE WAS STUDIED IN NORMAL C.S.F. OF 74 PATIENTS. THE ONLY CEPHALOSPORINE GIVING CONSISTENTLY MEASURABLE LEVELS IN C.S.F. IN ALL TEN PATIENTS TO WHOM IT WAS GIVEN WAS CEFTOZIDIME. ALSO THERE WERE DETECTABLE LEVELS OF CEFUROXIME AND CEPHALORIDINE. THESE WERE NO DETECTABLE LEVELS NEITHER OF CEPHOPERAZONE NOR OF CEFSULODINE AND CEFOTAXIME. ON THE BASIS OF THE ABOVE FINDINGS AND THE M.I.C. OF THE CEPHALOSPORINS WE RECOMMED CEFTAZIDIME AS THE CEPHALOSPORINE OF CHOISE IN GRAM NEGATIVE MENINGITIS INCLUDING THAT DUE TO PSEUDOMONAS, ESPECIALLY UNDER CONDITIONS WHERE CONCENTRATIONS OF THE ANTIBIOTIC IN NORMAL C.S.F. Laser Raman spectroscopy is an inelastic light scattering technique able to characterize molecules in aqueous environments. The purpose of this work is to develop a non-contact and non-invasive spectroscopic method to identify and eventually quantify the presence of medicines (e.g. antibiotics) and physiological substances (e.g. glucose) in the aqueous humor of the eye. Α new laser light delivery probe has been developed and adapted to a Raman spectroscopic system with the ability of favorable collection of the Raman light at 90o scattering geometry while scanning the anterior chamber of the eye. The technique is applied both, to porcine eyes in-vitro, max. 24 hours after death and extraction, for ceftazidime, amphotericin B and glucose and to a commercially available artificial anterior chamber (AAC) fitted with corneas of porcine eyes for ceftazidime, amphotericin B, amikacin sulphate and ciprofloxacin. Finally, a PLS chemometric algorithm has been developed to predict the concentration of antibiotics in AAC. This special illumination design gives the opportunity of reducing the direct exposure of the basic cordial ocular tissues, like lens and retina, to the laser beam, while at the same time an optimum collection of scattered light is accomplished. Concentrations close to the minimum inhibitory concentration (MIC) have been detected for antibiotics both in porcine eyes and AAC; the detection of glucose has been realized at concentrations close to the early pathological levels of patients with diabetes. Furthermore, the quantification of concentration of antibiotics in AAC is accomplished by a partial least-squares (PLS) chemometric regression algorithm and the RMS error of the validation procedure further emphasize the promising prospect of the application of the Raman spectroscopy to the Ophthalmology.

Η φασματοσκοπία Raman είναι τεχνική ανελαστικής σκέδασης φωτός, ικανή να ανιχνεύει και να χαρακτηρίζει μόρια σε ποικιλία υδατικών διαλυμάτων. Σκοπός της εργασίας είναι η ανάπτυξη μιας μη επεμβατικής, μη καταστροφικής φασματοσκοπικής μεθόδου για την ανίχνευση και τον ποσοτικό προσδιορισμό τόσο φαρμακευτικών ουσιών (π.χ αντιβιοτικών) όσο και φυσιολογικών ουσιών (π.χ γλυκόζη) στο υδατοειδές υγρό οφθαλμού. Στο πλαίσιο της παρούσας εργασίας αναπτύχθηκε μια νέα γεωμετρική οπτική διάταξη για την καθοδήγηση της δέσμης του laser στον οφθαλμό, που προσαρμόζεται σε φασματοσκόπιο Raman με ανιχνευτή CCD, δίνει τη δυνατότητα επιλεκτικής συλλογής του σκεδαζόμενου φωτός, σαρώνοντας τον εμπρόσθιο θάλαμο, σε γεωμετρία σκέδασης 90ο. IN FIFTY-ONE (51) PATIENTS, WHO WERE OPERATED FOR PROSTATIC HYPERPLASIA, WAS STUDIED THE DRUG MOBILITY AND THE NORMS OF CONCENTRATION IN PROSTATIC TISSUE AND IN BLOOD PLASMA OF THE TREE (3) B-LACTAM ANTIBIOTICS (CEFTRIAXONE-CEFTAZIDIME- CEFORANIDE). THE LEVELS OF THE DRUGS USED, WERE MEASURED IN BLOOD PLASMA ANDPROSTATIC TISSUE IN TWO SPECIMENS, ONE OF THE PROSTATIC TISSUE CENTER AND ONE OF THE DISTRICT. ALL THREE ANTIBIOTICS WERE GIVEN IN DOSES OF 2 GR INTRAVENOUSLY IN THE BEGINNING OF THE OPERATION. THE RESULTS SHOWED US THAT ALL THREEANTIBIOTICS PRESENT A SATISFACTORY CONCENTRATION IN THE PROSTATIC TISSUE. THE ABSORPTION INDICATOR (A.I.) WAS IN CEFORANIDE: M=11,92+-1,03, IN CEFTRIAXONE: M =9,66+-0,71 AND IN CEFTARIDIME: M=7,74+-0,47. THE LEVELS IN ALL THREE DRUGS IN THE SERUM AND IN THE PROSTATIC TISSUE WERE MUCH HIGHER THAN THE RELEVANT MINIMAL INHIBITORS CONCENTRATION (M.I.C.). THE STUDY'S RESULTS CONFIRM THE PREVENTIONAL USE OF B-LACTAME IN THE PROSTATIC SURGERY.

Objectives: To evaluate in vitro and ex vivo data of the enhancement of the activity of antimicrobials on multidrug-resistant Pseudomonas aeruginosa by n-6 polyunsaturated fatty acids (PUFAS).Setting: Gamma-linolenic acid (GLA)) was administered intravenously with ceftazidime and amikacin after bacterial challenge in a model of experimental sepsis by one multidrug-resistant isolate. 48 rabbits were applied divided into groups of treatment; 24 for intense hemodynamic monitoring followed by sacrifice and 24 for survival study.Findings: Mean survival of untreated animals, treated with antimicrobials, co-administered GLA was 2.58, 9.52, and 9.53 days respectively. Co-administration of GLA induced higher levels of systolic, diastolic and mean arterial pressures than the other groups. Addition of n-6 PUFA significantly reduced the number of viable cells in blood, lung and mesenterial lymph nodes compared to the other groups. Estimated serum levels of antimicrobials were inadequate to achieve bacterial killing due to the level of multidrug-resistance. N-6 PUFA did not influence biosynthesis of tumor necrosis factor-alpha and serum oxidant status; addition of GLA ended in lower serum levels of nitric oxide. Conclusions: Intravenous co-administration of n-6 PUFA enhanced antimicrobial killing in experimental sepsis by multidrug-resistant P.aeruginosa conferring hemodynamic stability.

Σκοπός: H εκτίμηση της ενίσχυσης της δράσης των αντιμικροβιακών στην αντιμετώπιση της πολυανθεκτικής Pseudomonas aeruginosa, in vitro και ex vivo, μετά από χορήγηση ω-6 πολυακόρεστων λιπαρών οξέων (ΠΛΟ).Μέθοδος: To γ-λινολενικό οξύ συγχορηγήθηκε ενδοφλεβίως με τους αντιμικροβιακούς παράγοντες κεφταζιδίμη και αμικασίνη μετά τον βακτηριακό ενοφθαλμισμό, σε μοντέλο πειραματικής σήψης με πολυανθεκτικό στέλεχος P.aeruginosa. Το μοντέλο εφαρμόστηκε σε 48 κονίκλους χωρισμένους σε ομάδες θεραπείας. 24 κόνικλοι χρησιμοποιήθηκαν για την καταγραφή των αιμοδυναμικών παραμέτρων και ακολούθως θυσιάστηκαν και 24 κόνικλοι χρησιμοποιήθηκαν για την μελέτη επιβίωσης.Αποτελέσματα: Η μέση επιβίωση στην ομάδα μαρτύρων, στην ομάδα που έλαβε αντιμικροβιακά και στην ομάδα που έλαβε αντιμικροβιακά και γ-λινολενικό οξύ ήταν 2.58, 9.52 και 9.53 ημέρες αντίστοιχα. Η συγχορήγηση γ-λινολενικού οξέος οδήγησε σε υψηλότερα επίπεδα συστολικής, διαστολικής και μέσης αρτηριακής πίεσης σε σχέση με τις άλλες ομάδες θεραπείας. Η προσθήκη ω-6 ΠΛΟ οδήγησε σε σημαντική μείωση των ζώντων μικροοργανισμών στο αίμα, στους πνεύμονες και στους μεσεντέριους λεμφαδένες συγκριτικά με τις άλλες ομάδες. Τα επίπεδα των αντιμικροβιακών στον ορό δεν ήταν ικανά για να επιτύχουν τον βακτηριακό θάνατο, βάσει των επιπέδων της πολυανθεκτικότητας. Η βιοσύνθεση του ΤΝF-α και το οξειδωτικό δυναμικό του ορού δεν επηρεάστηκαν. Η προσθήκη του ω-6 ΠΛΟ οδήγησε σε μείωση των επιπέδων του μονοξειδίου του αζώτου στον ορό.Συμπέρασμα: Η ενδοφλέβια συγχορήγηση ενός ω-6 ΠΛΟ ενισχύει την δράση των αντιμικροβιακών σε πειραματικό μοντέλο σήψης από πολυανθεκτικό στέλεχος P.aeruginosa παρέχοντας ταυτόχρονα αιμοδυναμική…
