As hepatites B e C constituem ainda importante problema de saúde pública no mundo, com cifras de portadores crônicos estimados em cerca de 350 milhões e de 170 milhões, respectivamente, fazendo com que número elevado de pessoas se encontrem sob risco de cronificação e evolução para cirrose hepática e hepatocarcinoma. Entre os fatores de risco mais conhecidos para hepatite B estão às transfusões de sangue e derivados, o contato com sangue e/ou com secreções, por relações sexuais desprotegidas, compartilhamento de seringas e/ou agulhas para uso de drogas injetáveis, tatuagem e piercing com material contaminado. A hepatite C é transmitida primordialmente por via parenteral e a sua prevenção faz-se de modo semelhante à hepatite B. Diversos fatores de risco para ambas parecem ser fazer presentes na população surda, a qual é historicamente marginalizada em termos de acesso a informações e serviços de saúde e sobre a qual não existem referências de investigações sobre hepatites virais. Esse trabalho teve como objetivos estimar a prevalência de sorologia positiva para hepatite B e C e investigar possíveis fatores de risco entre a população surda de Ribeirão Preto. Foram estudados 88 surdos, aos quais foi apresentado DVD explicativo sinalizado em Libras sobre características e riscos das hepatites B e C. Aos que concordaram em participar foi aplicado questionário padronizado e coletada uma amostra de sangue para realização de testes imunoenzimáticos para detecção dos marcadores HBsAg, anti-HBs, anti-HBc e anti-HCV, realizados no Laboratório de Sorologia do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto. A análise sorológica revelou presença de infecção atual ou pregressa por hepatite B em sete participantes, correspondendo a 8% de prevalência total de marcadores (IC95%: 2,3 13,7). Ao se analisar as possíveis variáveis de risco, encontrou-se associação entre a infecção e as variáveis: ser nascido em outra Unidade Federada que não São Paulo e antecedente de encarceramento. Os participantes mostraram grande desconhecimento sobre aspectos básicos relacionados à transmissão das hepatites virais, indicando necessidade de políticas de saúde pública voltadas para esta população, e que levem em consideração suas particularidades linguística e cultural.

▼ There are high rates of inactivity in UK adults, which can lead to a range of health problems. The main aims of this thesis were: first to review existing behaviour change intervention design, delivery, evaluation, and reporting frameworks to gauge the most effective process and/or combination; second to review the existing literature on physical activity and sedentary behaviour interventions, to see whether they work, what techniques might be effective, and how well they were reported; third to review theories of behaviour (change) in terms of completeness and suitability for physical activity and sedentary behaviour; fourth to test the chosen theory (COM-B model) in terms of the relevant components of the three constructs (Capability, Opportunity, Motivation) and how well they predicted moderate-to-vigorous physical activity (MVPA) and sedentary behaviour (in comparison to the Theory of Planned Behaviour); last to design, implement, and evaluate (including from the deliverers' perspective) a community physical activity programme, with the techniques highlighted in the review included in the content and the behavioural drivers from the theory analysis as secondary outcomes. The exploration of behaviour change intervention design frameworks concluded by summarising a nine-step process covering the most important elements from needs assessment to dissemination. The systematic review showed physical activity interventions to be effective at changing behaviour and maintaining those changes, and pointed towards behaviour change techniques that were associated with effectiveness. The theory review concluded that the COM-B contained the most comprehensive range of behavioural determinants and was ideally situated within the Behaviour Change Wheel for designing interventions. The COM-B analysis showed a strong prediction of MVPA and highlighted Psychological Capability and Reflective Motivation as important drivers. Sedentary behaviour was also predicted relatively strongly with Psychological Capability the most important driver. The Active Herts programme was then detailed and evaluated, showing improvements in physical activity, health, life satisfaction, and wellbeing at 3 and 6 months. COM-B measures predicted MVPA more strongly after intervention at 3 and 6 months, than at baseline, and were better at predicting MVPA performance than change over this period. Interviews with the Get Active Specialists delivering the programme reflected positively on the training, materials, and overall programme. Key lessons to take forward were extra support at the start with supervision and engaging referrers, and limiting the length and complexity of outcomes measures. The discussion explored the need to measure long-term outcomes of behaviour change, difficulties in measuring the constructs of the COM-B, the balance between standardisation and tailoring of interventions, and adopting a transdisciplinary approach to programme design.

▼ Due to emerging climate concerns, a growing pressure is put on the fashion industry to adopt circular business models which encourage sustainable usage of clothing. The clothing supply chain is currently both complicated and polluting. Paired with rising quantities of clothing sales, technological solutions will only temporarily tackle the symptoms of a deeply rooted issue, namely mass-consumerism and underutilisation. How can fashion companies encourage consumers to use their current clothes more often whilst consuming less, in a way that’s also attractive from a business perspective? This project investigates the behavioural implications of creating such a sustainable clothing service. Research is conducted in the context of King Louie: a Dutch, vintage-inspired womenswear brand. In-depth interviews and surveys focussing on women around their 30s have revealed the following: - Clothes can be appreciated due to functional needs (practical value or monetary/material value) or psychological needs (the value of self-expression, social levelling, having a hobby or aesthetics). - Most women reorganise their wardrobe bi-anually; this makes them reflect on clothes. - Friends and relatives can highly influence how clothes are valued and when new clothes are sought. For most of these women, having and using clothes is part of a hobby and their identity, which increases the desire to own more clothes. These ‘Experiential clothing users’ would benefit from a clothing service that renews their clothes’ self-expression value through a social, qualitative experience... starting with a wardrobe re-organisation. Based on this, Re:Create was developed: a digital wardrobe and peer-to-peer styling app. This service was designed by using an interdisciplinary method, combining service design with behavioural science (using the Behaviour Change Wheel approach). The behavioural science behind Re:Create focusses on the use of ‘Persuasion’, Modelling’ and ‘Training’. Furthermore, it taps into women’s sense of identity and takes them on an emotional self-discovery journey that emphasises on the value of self-expression. The sustainability vision behind Re:Create is that women can ‘shop in their own wardrobe’ before purchasing something new; this stimulates product-life extension and the reduction of consumption. If ‘new’ garments are needed, Re:Create’s second-hand webshop encourages re-using someone else’s clothes as a means of collective product-life extension. The business value of Re:Create is that it helps a cluster of high-quality, sustainable brands gain brand exposure and possibly meet new consumer segments through its ‘network effect’. In the future, revenue from a self-controlled second-hand market can elevate circular business model implications. Testing the behavioural principles of Re:Create reveals that the app can indeed be an enjoyable and valuable service. Food waste is an arguably increasing problem in society, with consumers being responsible for as much as half of all combined food waste. The problem needs to be tackled from all angles, but there is no all encompassing solution for all situations. One context consumers find themselves in is the kitchen at their place of work, where the shared nature of the refrigerator creates situations and problems not found in the home environment. It is for these shared kitchens that this study is focused on providing a physical solution based on The Behavioural Change Wheel (BCW), book providing methods and tools for designing with the goal of behavioural change. The intention of this study was to develop, test and later evaluate a prototype on its behavioural change capacity and potential, using the tools presented in the BCW. The developed prototype took form as an RFID activated labelling system, providing users with the current date and their name on a label to be adhered on the articles they place in the shared refrigerator. After the testing period, an online questionnaire was sent out and was answered by fourteen out of seventeen participants. The two week testing period and subsequent questionnaire provided no conclusive answers regarding detected behavioural change capacity, but the potential is considered high based on participants qualitative comments and attitudes, and their quantitative usage. No measurements of change in food waste is conducted, but the target behavioural changes would provide a positive development resulting in less food waste in the workplace kitchen, if not only a more orderly refrigerator.

Matavfall är ett påtagligt ökande problem i samhället, där konsumenter ansvarar för så mycket som hälften av allt kombinerat matavfall. Problemet bör hanteras från alla möjliga håll, men det finns ingen allmän lösning för alla situationer. Ett sammanhang som konsumenterna befinner sig i är köket på deras arbetsplats, där kylskåpets gemensamma natur skapar situationer och problem som inte återfinns i hemmiljön. Det är för dessa delade kök som denna studie riktar sig på att tillhandahålla en fysisk lösning baserad på The Behavioural Change Wheel (BCW). Den innehåller metoder och verktyg för att designa med beteendeförändring som mål. Syftet med denna studie var att utveckla, testa och senare utvärdera en prototyp angående dess beteendeförändringsförmåga och potential, med hjälp av de verktyg som presenteras i BCW. One program that exists that addresses the aforementioned limitation is the Transformational Coaching Workshop (TCW; Turnnidge & Côté, 2017). A recent evaluation of the TCW yielded promising findings in regard to the workshop’s ability to change the observable leadership behaviours of coaches (Lawrason et al., 2019); however, the TCW has not been examined for its ability to change the way coaches perceive their use of transformational coaching behaviours. Thus, the purpose of this study was to use behaviour change theory to examine the TCW’s ability to target the perceptual elements that influence behaviour change in coaches. Sixty-three youth sport coaches from across Ontario participated in the study. Thirty-one coaches participated in the TCW and served as the intervention group while the remaining 32 coaches served as a comparison group and did not receive any training. All coaches were asked to complete a capability-opportunity-motivation-behaviour (COM-B; Michie et al., 2011) questionnaire at two separate time points to assess their perceptions of factors that contribute to their use of transformational coaching behaviours. A combination of dependent- and independent- samples t-tests were run to examine pre- and post-test differences within the intervention group and between the intervention and comparison groups at post-test. Results of the study unveiled that while the TCW increased the mean scores of coaches’ perceived capability (p = .02), opportunity (p = .03), and motivation (p = .34) to use transformational coaching behaviours after participation in the workshop, non-significant p values after a Bonferroni adjustment (i.e., p = .01) suggest the need for a larger sample to be included in future studies of the TCW to achieve statistical significance. However, medium to large effect sizes observed for both perceived capability (d = .53) and opportunity (d = .51) offer a pragmatic understanding of the workshop’s implications on coaches who participated. Additionally, there were no significant differences between the intervention and comparison group at post-test for perceived capability, opportunity, or motivation. Nonetheless, the TCW did yield promising results for increasing the ways coaches perceive their use of transformational coaching behaviours and thus, is considered a valuable resource for youth sport coaches looking to increase the interpersonal side of their coaching.

O sistema NADPH oxidase é um complexo enzimático gerador de superóxido. O NF-kB é um fator de transcrição envolvido no controle da expressão de diversos genes ligados à resposta inflamatória. Defeitos no eixo IL-12/23-IFN-g resultam em infecções recorrentes e à susceptibilidade mendeliana a micobacterioses, podendo diminuir a expressão do componente gp91-phox da NADPH oxidase. Estudamos qual é a relação direta do NF-kB e de defeitos no eixo IL-12/23-IFN-g na regulação dos genes CYBA, NCF1, NCF2 e NCF4 do sistema NADPH oxidase humano em células U937, células B EBV transformadas provenientes de pacientes com EDA-ID, DGC, ou de pacientes com defeitos no eixo IL-12/23-IFN-g. A expressão dos genes NCF1 e NCF2 foi diminuída em células com defeitos no eixo (IFNGR1 e INFGR2) e em células U937 IkB S32A/S36A. A expressão do gene NCF1 também foi diminuída em células EDA-ID S32I e em células EDA-ID NEMO/IKKg W420X. O NF-kB e os IFNGR1 e INFGR2 são necessários para a expressão dos genes NCF1 e NCF2 e para a ativação do sistema NADPH oxidase humano neste sistema modelo.

The NADPH oxidase system is an enzymatic complex that generates superoxide. The NF-kB is a transcriptional factor involved in the expression of several genes related to the inflammatory response. The IL-12/23-IFN-g axis defects lead to recurrent infections and to the mendelian susceptibility of mycobacterial disease (MSMD), and they can decrease the gp91-phox expression (a NADPH oxidase component). We studied the NF-kB and the IL-12/23-IFN-g axis defects consequences on the regulation of CYBA, NCF1, NCF2 and NCF4 genes of the human NADPH oxidase system in U937 cells, and in B EBV cells from patients with EDA-ID, DGC, or patients with IL-12/23-IFN-g axis defects. The NCF1 and NCF2 gene expression was decreased in IL-12/23-IFN-g axis defects cells (IFNGR1 and INFGR2) and in U937 IkB S32A/S36A cells. NCF1 gene expression was decreased in EDA-ID S32I and in EDA-ID NEMO/IKKg W420X cell lineages. The NF-kB and the IFNGR1 and INFGR2 are necessary for NCF1 and NCF2 gene expression and activation of the human NADPH oxidase in this model system.

Multidisciplinary cancer conferences (MCCs) are prospective meetings to discuss diagnoses and treatment plans for patients with cancer. MCCs are typically attended by surgeons, medical and radiation oncologists, pathologists and radiologists. To date, the quality of MCCs, specifically MCC decision making, has not been formally evaluated in a Canadian context. We utilized progressive knowledge translation methodology, specifically the use of theory, models and an integrated knowledge translation approach, to design, implement and evaluate an intervention, titled the KT-MCC Strategy (KT-MCC). The purpose of the KT-MCC is to improve the quality of MCC decision making. This thesis is comprised of four parts. In Part 1, we completed a generalizability study to evaluate the reliability of an MCC assessment tool (MTB-MODe) in an Ontario context. In Part 2, we conducted key informant interviews using the Theoretical Domains Framework (TDF) to identify barriers and facilitators to optimal MCC decision making. In Part 3, we mapped identified TDF barriers and facilitators to the COM-B Behavioural Change Wheel to develop the KT-MCC, an intervention aimed at improving the quality of MCC decision making. In this study, we examined the validity of the key informant findings using focus groups and surveys completed by individual MCC participants. In Part 4, we conducted a before-and-after pilot study to evaluate the feasibility and impact of the KT-MCC on MCC decision making in preparation for a possible randomized controlled trial testing the efficacy of the KT-MCC. There are few examples in the KT literature that provide a complete and detailed description of the design, implementation and evaluation of a complex KT strategy using progressive KT methods such as TDF interviews to identify barriers and facilitators to practice change; the COM-B model to identify potential interventions; and use of integrated KT with front-line workers. We describe in detail our methods to design, implement and evaluate the KT-MCC. This thesis provides a significant contribution to the knowledge translation literature and provides recommendations to improve the quality of MCCs in Ontario.

Multidisciplinary cancer conferences (MCCs) are regular meetings held by health professionals to prospectively discuss diagnoses and treatment plans for patients with cancer. The purpose of MCCs is to facilitate input from numerous experts to ensure that each patient receives an optimal treatment recommendation. [pt] Soluções poliméricas diluídas podem ser usadas na indústria de Óleo e Gás para injeção em reservatórios de petróleo como agentes de controle de mobilidade em técnicas de recuperação avançada de óleo, com o objetivo de aumentar o fator de recuperação do campo e postergar a indesejada alta taxa de produção de água. No entanto, o fenômeno macroscópico esperado, aumento da eficiência de varrido, é influenciado diretamente por fatores microscópicos que ocorrem na escala de poro. Devido às suas grandes cadeias moleculares, ao avançar através do meio poroso, soluções poliméricas de alto peso molecular dificultam o avanço da água alterando a razão de mobilidades água/óleo. Porém, o estudo de escoamento de soluções poliméricas ainda é um desafio devido ao comportamento não Newtoniano da relação vazão x diferença de pressão e à sensibilidade a diversos fatores como: tipo de polímero utilizado, concentração, razão de viscosidades, temperatura, salinidade e taxa de cisalhamento. O presente trabalho analisa a implementação de dois modelos constitutivos, Oldroyd-B e FENE-CR, aplicados ao método de solução de elementos finitos, EF, utilizando a formulação dos resíduos ponderados de Galerkin para modelar o escoamento de soluções poliméricas diluídas através de capilares com constrição e entender os fenômenos viscosos e elásticos envolvidos. Os resultados obtidos fornecem uma descrição mais detalhada da aplicação dos modelos testados e do escoamento de soluções poliméricas diluídas em gargantas de poros.

[en] Diluted polymeric solutions can be used in Oil & Gas industry for reservoir injection as mobility control agents in enhanced oil recovery technics, which goal is to increase the oil field recovery factor and postpone the high water production rates. However, the macroscopic expected phenomena, increase of the areal sweep, is directly influenced by microscopic factors that occurs at the pore scale. Due to their huge molecular chains, by advancing through the porous media, high molecular weight polymer solutions hinder the water flow changing the water/oil mobility ratio. Nevertheless, the comprehension of polymeric solutions flow is still a great challenge due to the Non-Newtonian behavior of the flow rate x pressure gradient relation and the sensibility to several variables such as: type of polymer used, concentration, viscosity ratio, temperature, salinity and shear rate. The present work analyzes the implementation of two constitutive models, Oldroyd-B and FENE-CR, coupled with the Finite Element method using the Galerkin weighted residual formulation to model the flow of diluted polymeric solutions through constricted capillaries and understand the viscous and elastic phenomena involved. The obtained results provide a more detailed portrait of the constitutive models and the flow of diluted polymeric solutions through pore throats.
