▼ The periodontitis is an inflammatory disease condition caused by a variety of
oral microorganisms. A hyper reactive host response regulation is triggering
the disease progression in a small part of the world wide population – around
10% at most – resulting either in a local or generalized aggressive form of
manifestation.
Bleeding is a final act in a process of tissue and capillary
damage initiated by bacterial presence and modulated by pro-inflammatory
cytokines. Calibrated assignment of
bleeding intensity as a parameter for the
severity of the disease is considered golden standard for diagnosing
gingivitis – an inflammatory condition restricted to the soft tissues.
Periodontal inflammation regularly causes attachment loss and is accompanied
by loss of alveolar bone and a phenomenon named “pocketing”. The severity of
this disease condition is currently characterized by assessing either the
probing pocket depths (PPD) or the attachment loss (AL). The evaluation of
bleeding tendency as
Bleeding on
Probing (
BoP) is currently an additional
dichotomous finding. The aim of the present study was to assess the
suitability of a visual calibration of a Pocket
Bleeding Index (PoB-Index) to
reflect the real inflammatory status of each single probed periodontal pocket.
By matching single grades of the index suggested – i.e. the single visually
assigned
bleeding grades - with parameters such as
bleeding duration (stop
watch) and amount of blood sampled by capillary tube correlation coefficients
were calculated. Correlations between PoB-Index and the
probing depth and PoBI
and the amount of gingival crevicular fluid (GCF) sampled as exudates that
follow a gradient were calculated also to objectify the visual calibration of
bleeding intensity. The capillary dilatation is initiated as first step in a
cascade of inflammatory reactions in front of all clinical signs and results
in a measurable increase of the GCF. Since periodontitis progresses
episodically, i.e. phases of activity are followed by phases of inactivity the
capillary dilatation follows the same pattern. The mean age of 17
participating patients recruited was 32 years; 10 of them were non-smokers
whereas 7 were smoking. All patients enrolled did not have any periodontitis
therapy or received any antibiotics prescribed for the last 12 months. All
patients underwent intensive instructions in oral hygiene repeatedly, thus the
allocated plaque index remained for the duration of the study < 25% (API <
25%). After completion of instrumentation (S/RP) all patients received
Amoxicillin (500 mg) and Metronidazol (250 mg) x 3 daily for 10 days per os
prescribed. The assessment of the study parameters was carried out at baseline
after completion of the initial therapy (before S/RP), at month 3, 6 and 9
past active therapy. In total 575 sites were evaluated and monitored. For

differentiate between healthy sites and those with active
periodontal disease are of great importance to the clinician. Due
to the dichotomous nature of the clinical response, indices
involving the determination of the presence or absence of
bleeding
have been regarded as an unbiased type of clinical assessment. It
is important to understand the relationship of
bleeding on
probing
(
BOP) to other traditional measurements of the severity of
periodontal disease. The purpose of the present study was to
determine if there is a correlation between
BOP and plaque score,
pocket depth, or subgingival microbiota present in study
participants with periodontitis. ❧ Materials and Methods ❧ Thirty
otherwise healthy adults, diagnosed with periodontitis, were
included in the study. The following clinical variables were
evaluated at baseline (Visit 1), two weeks post-baseline (Visit 2)
and 3 months post-baseline (Visit 3): medical history, dental
radiographs, number of teeth, presence or absence of plaque,
presence or absence of
bleeding on
probing, pocket depth, gingival
recession, mobility and furcation involvement. Participants
randomly received professional subgingival irrigation with either
sodium hypochlorite 0.25% (test) or water (control) at visit 1 and
visit 2 and rinsed at home twice-a-week with either sodium
hypochlorite 0.25% (test) or water (control) throughout the course
of the study. ❧ The presence of plaque and
BOP were recorded as
dichotomous variables. Pocket depth was recorded as a discrete
variable in mm and was further dichotomized into >4 and ≤ 4 mm.
Descriptive statistics were calculated at each visit. Pearson χ²
and correlation coefficient tests were used to assess the
relationship between
BOP and plaque score as well as
BOP and pocket
depth. The significance level was set at α =0.05. Additionally, the
patients were grouped into two separate cohorts according to
presence or absence of
BOP in the sites that were sampled, and the
microbiota present at these sites were analyzed within each group.
The difference within the groups between the data at baseline,
Visit 2 and whenever available, Visit 3, were compared using
Wilcoxon signed-rank sum test. Comparisons between test and control
group at each of the 3 time points were accomplished using Wilcoxon
Mann-Whitney's U test. A two-sided alternative hypothesis and a
0.05 significance level were assumed in all testing. Data analysis
was performed using SAS v9.3 and STATA© statistical software
(StataCorp, College Station, Texas, USA). ❧ Results ❧ The results
indicate that there was a statistically significant correlation
between
BOP and plaque score within each group (test, control and
pooled) at all study times (p4 mm within each group at all study

▼ Fasting exhibits anti-inflammatory effects in the human body. The impact on the periodontal tissues, however, has yet not been investigated. To examine the effects of prolonged multimodal clinically controlled fasting on periodontal parameters, a non-interventional clinical trial was conducted in patients with metabolic syndrome (MetS).
All 47 in-patients diagnosed with MetS were enrolled between August and November 2016 after signing an informed consent. Each patient participated in a multimodal fasting program according to Buchinger with an energy intake of 1200 KJl/d. Outcomes were accessed at baseline (T1) and after fasting (T2), and the inflammatory status (c-reactive protein – CRP) and periodontal parameters (
bleeding on
probing –
BOP, gingival crevicular fluid rate – SFFR, Plaqueindex – PI) performed by two calibrated examiners, one single-blinded, using a pressure sensitive probe were documented. Furthermore, the metabolic parameters blood pressure (BD), waist circumference (TU), fasting glucose (NGLU), triglyceride (TRG), and HDL-level were assessed.
Summary statistics of baseline characteristics were separately calculated for both men and women using the Mann-Whitney-U test. Correlation analyses were performed using Spearmans rank correlation. Differences between T1 and T2 parameters were evaluated using the Wilcoxon test.
For the statistical analysis, data of 28 women were used.
BOP and SFFR significantly decreased, while PI and CRP remained unchanged (
BOP 39 % - 33,1 % (p<0,001), SFFR 73,9 PE - 59,3 PE (p<0,05), PI 69,2 % - 69,3 % (p=0,446), CRP 4,2 mg/l - 4,9 mg/l (p=0,178)). All metabolic parameters were significantly reduced after fasting (BD systolic 135 mmHg – 120 mmHg, diastolic 80 mmHg - 75 mmHg (<0,001), TU 111 cm - 107 cm (p<0,001), NGLU 99,1 mg/dl - 76,6 mg/dl (p<0,001), TRG 173,7 mg/dl - 135,2 mg/dl (p<0,001), HDL 50,3 mg/dl - 46,4 mg/dl (p=0,002)). The reduction in
bleeding correlated with fasting glucose levels at baseline (r=0,37, p=0,049) as well as weight loss (r=0,4, p=0,04).
Prolonged fasting exhibits beneficial short-term effects on all MetS parameters and may facilitate the reduction of gingival inflammation. CRP seems to be an inadequate marker to predict inflammatory processes in periodontal tissues while fasting. To evaluate the effect of fasting on periodontal disease further studies are required.

The principal aim of this study was to explore associations between systemic inflammatory status and periodontal inflammation and tissue destruction. The study population consisted of 61 patients with chronic periodontitis, 30 periodontally healthy control subjects, and 80 subjects with type 1 diabetes mellitus (T1DM). The T1DM subjects were periodontally treated and re-examined eight weeks after completion of the treatment. The periodontal measures included plaque, probing depth (PD), bleeding on probing (BOP) and attachment level (AL). The serum levels of interleukin (IL)-6, tumor necrosis factor (TNF)-α, and interleukin (IL)-10, as well as the gingival crevicular fluid (GCF) level of matrix metalloproteinase (MMP)-8 were analyzed using commercially available ELISA assays, and serum high density lipoprotein (HDL) level using direct enzymatic methods.
Serum IL-6 level associated significantly with the extent of inflamed periodontal pockets in T1DM subjects. Moreover, serum IL-6 modulated local periodontal inflammation in T1DM patients; periodontal healing turned out to be poorer in subjects with a high level of serum IL-6 than in those with a low level. Serum TNF-α/IL-10 ratio was three times higher in chronic periodontitis patients than in periodontally healthy control subjects. In T1DM subjects a significant inverse association between serum HDL level and the extent of inflamed periodontal pockets was found; subjects with a low serum HDL level presented 50% more inflamed periodontal sites than subjects with a high serum HDL level. A significant association between GCF MMP-8 level in shallow crevices and the extent of periodontal attachment loss in chronic periodontitis patients was observed.
In conclusion, we focused on analyzing associations between systemic inflammatory status and periodontal conditions. According to our results, periodontal inflammation/infection is associated with systemic inflammatory status using serum IL-6, TNF-α/IL-10 ratio and HDL as indicators. Also, a high level of MMP-8 in GCF in shallow crevices could be indicative of higher susceptibility to periodontal infection and tissue destruction.

Tutkimuksen tavoitteena oli tarkastella hampaiden kiinnityskudosten tulehduksen (pardodontiitti) ja siihen liittyvän inflammaation yhteyttä systeemiseen tulehdustilaan. Tutkimusaineistoon kuului 61 yleistervettä potilasta, joilla oli kohtalaisesti tai pitkälle edennyt parodontiitti, 30 yleistervettä yksilöä, joiden hampaiden kiinnityskudokset olivat terveet/lähes terveet (kontrolliryhmä), sekä 80 tyypin 1 diabetes mellitus (T1DM) potilasta, joilla esiintyi vaihtelevasti parodontiittia. A eliminação do biofilme estabelecido e a descontaminação da superfície do implante é o objetivo principal no tratamento da peri-implantite.Métodos: Algunas técnicas cirúrgicas podem ser necessárias para nos proporcionar um acesso adequado para desgranular eficazmente os tecidos inflamados, bem como para descontaminar e, se indicado, modificar a superfície do implante. Antes da utilização de técnicas cirúrgicas, a terapia não cirúrgica e o reforço da higiene bucal devem ser realizados. Resultados: Como em qualquer doença, a prevenção é a melhor forma de tratamento e a peri-implantita não é exceção. O diagnóstico precoce da doença peri-implantar pode ajudar a controlar a progressão e permite a sua resolução por meios não cirúrgicos no caso de mucosite peri-implante. Conclusão: As doenças peri-implantes são doenças importantes como resultado de sua alta prevalência e da falta de um modo padrão de terapia. Um estudo completo dos fatores de risco locais e sistêmicos do paciente é importante antes de qualquer tratamento com implante.
Aim: Peri-implantitis is an inflammatory disease affecting soft and hard tissues surrounding a functional implant. The elimination of the established biofilm and decontamination of the implant surface are the main objectives in the treatment of peri-implantitis. Methods : Surgical techniques may be necessary to provide adequate access to degranulate the inflamed tissues effectively as well as to decontaminate and, if indicated, modify the implant surface. Before surgical techniques are employed, nonsurgical therapy and reinforcement of the oral hygiene should be performed. Results: As with every disease, prevention is the best treatment. Early peri-implant disease diagnosis can help in controling the progression and its resolution by non-surgical means in the case of peri-implant mucositis. Conclusion: Peri-implant diseases are important disease entities because of their high prevalence and the lack of a standard mode of therapy. Thorough study of the patient’s local and systemic risk factors is important before any implant treatment.

▼ Objetivos: Estudar os diferenciais entre os indicadores de cárie dentária e alterações gengivais em escolares de áreas urbanas e rurais do Estado de São Paulo. Material e Métodos. Estudo de corte transversal analítico e ecológico foram os dois delineamentos utilizados. A população de estudo foram estudantes de escolas urbanas e rurais do Estado de São Paulo de 5 a 7 e de 10 a 12 anos; a fonte de informação foi o "Levantamento das Condições de Saúde Bucal - Estado de São Paulo, 1998". Foi estimada a prevalência de cárie dentária (índice ceo-d e CPO-D), do sangramento gengival durante a sondagem e de cálculo dental (índice CPI), para o estudo de associação com características sócio-demográficas dos escolares e indicador de desenvolvimento social (IDH-M) das cidades participantes do levantamento. Foram estimadas as odds ratios ajustadas e não ajustadas, por meio de análise de regressão logística de delineamento de modelos multivariados e multiníveis.
O estudo de diferenciais de prevalência entre diferentes regiões do Estado empregou uma classificação de àreas rurais homogêneas pré-existentes. Resultados: Os indicadores de saúde bucal tiveram distibuição desigual entre os estratos, indicando pior condição para a população rural. Estudar em áreas rurais, em escolas públicas, ser negro ou pardo e ser do sexo masculino associaram com a manifestação de cárie não tratada e alterações gengivais, ao nível dos indivíduos. No modelo multinível, a presença de flúor na água de abastecimento público, o Índice de Desenvolvimento Humano (IDH-M) e o Índice de Cuidado associaram com as condições de saúde bucal estudadas ao nível das cidades participantes do levantamento. Áreas rurais cujo sistema produtivo apresentava maior implementação da economia agro-industrial tiveram os melhores indicadores de saúde bucal. Conclusões: Os escolares de área rural mostraram-se mais vulneráveis para todas as condições de saúde bucal avaliadas
Objectives:
To assess differentials of prevalence of dental caries and gingival status in schoolchildren from rural and urban areas of the State of São Paulo. Material and Methods: The study population comprised schoolchildren aged 5 to 7 and 10 to 12 years old; the source of information was the Survey of Oral Health Status State of São Paulo, 1998". The prevalence of dental caries (dmft and DMFT indices), gingival bleeding on probing and dental calculus (CPI index) were assessed for the appraisal of association with socio-demographic characteristics of schoolchildren and indices of social development of participating towns. The estimation of adjusted and non-adjusted odds ratios used logistic regression analysis, and the fitting of multivariate multilevel models. Ultrasound imaging plays a major role in evaluation of pregnancies especially in the first trimester.
Vaginal bleeding in early pregnancy is a relatively common presentation in the acute gynaecological
setting, and ultrasonography remains the safest and fastest way of evaluating these patients.
OBJECTIVE
The aim of the study was to determine the ultrasound findings inpatients presenting with vaginal bleeding
in the first trimester and their proportions.
MATERIALS AND METHODS
A cross-sectional descriptive study carried out at the University of Nairobi, Department of Diagnostic
Imaging and Radiation Medicine (DDIRM, UON) and at Kenyatta National Hospital (KNH). All Patients
presenting to the radiology department for ultrasound due to vaginal bleeding in the first trimester,
between April and December, 2013, were evaluated with a pelvic ultrasound, either transabdominal,
transvaginal or both. A data collection sheet was used to record the pelvic sonographic findings and
possible predisposing factors associated with the vaginal bleeding. The data was analysed using the Stata
computer software package and the results presented in the form of tables, pie charts and bar charts.
RESULTS
A total of 231 patients were recruited into the study. The age range was 16 – 47 years with a mean of
27.83 years and a median of 27 years with a standard deviation of 5.686. Most patients were in the 26 to
35 year age group (133 cases; 57.6%). All 231 patients had abnormal findings on ultrasound, but 6 of
them had more than one finding, giving a total of 237 findings. The commonest finding was incomplete
abortion accounting for 38.7% of cases, followed by ectopic pregnancy accounting for 28.1%, while the
least common finding was uterine fibroids in pregnancy accounting for 0.4%. First trimester bleeding was
associated with a high rate of pregnancy loss with 206 of the patients (89.2%) having pregnancy loss.
CONCLUSION
Vaginal bleeding during the first trimester of pregnancy was associated with high rates of pregnancy
failure in the emergency setting, with incomplete abortion and ectopic pregnancy being common
sonographic findings. One should therefore have a high suspicion index for these conditions and apply
appropriate investigative procedures and management.

▼ Defects in haemostasis, specifically in the quantitative or qualitative properties of von Willebrand factor (VWF), result in the most commonly inherited bleeding disorder, von Willebrand disease (VWD). Of the three types of VWD, type 1 is the most common and has the mildest phenotype. Type 1 VWD has a symptomatic prevalence of approximately 1 in 1000, but a much smaller number of patients who are actually diagnosed. This discrepancy is a result of a lack of standardized diagnostic criteria. The objective of the Self-Administered Bleeding Assessment Tool (Self-BAT) Study is to develop and optimize, a standardized, self-administered BAT to quantify bleeding history, and to distinguish between normal and abnormal bleeding.
In Phase 1 of the study, the medical terminology in the expert-administered ISTH BAT was changed to a grade four reading level to produce the first version of the Self-BAT. Three subsequent versions of the Self-BAT were generated during optimization based on focus group feedback and statistical analysis. The Self-BAT was administered to a total of 38 control subjects and 20 subjects affected with type 1 VWD in Phase 1. Demographic information on all the subjects (gender, age) was collected and blood was drawn for determination of VWF antigen level, VWF ristocetin cofactor activity and factor VIII level. Bleeding scores calculated from the ISTH BAT and Self-BAT showed an excellent correlation of 0.869 in the final version.
In Phase 2 of the study, the Self-BAT was administered to 27 control subjects and 23 affected subjects with the objective to simplify the questionnaire by identifying questions which can discriminate between unaffected and affected subjects. It was determined through category-total, category-category and item-item correlations that there was not sufficient evidence to reduce the Self-BAT.
In conclusion, the Self-BAT functions as a simple and effective tool to distinguish between normal and abnormal bleeding and is suitable for adults to administer themselves. The Self-BAT has now moved on to prospective validation studies where it will be tested on subjects who have been referred to tertiary care, but not yet received a diagnosis, in order to test its ability to distinguish between normal individuals and those affected with type 1 VWD and, potentially, other mild bleeding disorders.

▼ Hexahydro-1,3,5-trinitro-1,3,5-triazine (RDX) is soluble, nonvolatile cyclic nitramine explosive. Long-term manufacturing and various applications of RDX have resulted in RDX contamination in soil and groundwater. RDX is a possible human carcinogen; therefore, occurrence of RDX in groundwater has raised a public health concern. As RDX is biodegradable; bioremediation of RDX-contaminated groundwater has been recognized as a feasible cleanup technology. Several RDX-degrading isolates are known to have ability to utilize RDX as carbon and/or nitrogen source. However, little is known about these isolates and their roles in the natural or engineered systems during RDX degradation, or about RDX-degrading microbial communities in responding to engineered interventions. Stable isotope
probing (SIP) is a powerful culture independent method that can identify functional active bacteria in various environmental samples. In this study, we applied SIP with ^(13)C-labeled or one of the ring-, nitro- and fully-labeled ^(15)N-RDX to identify microorganisms capable of utilizing RDX and its metabolites as carbon and/or nitrogen sources in groundwater microcosms, and to associate active RDX-degrading microbial communities in responding to engineered interventions.
Derived sequences from ^(13)C-DNA were clustered in Bacteroidia, Clostridia, alpha-, beta- and delta-Proteobacteria, and Spirochaetes, which were different from previously described RDX degraders. Cheese whey amendment stimulated RDX biotransformation, altered the types of RDX-degrading bacteria, and decreased microbial community diversity. Derived sequences from ^(15)N-DNA were grouped in Clostridia, beta-Proteobacteria and Spirochaetes. In comparison to the results among ^(13)C-SIP and ^(15)N-SIP studies with presence of cheese whey, derived sequences were clustered in gamma-Proteobacteria and Bacilli. The combination of these findings suggested that RDX-degrading microorganisms in groundwater are more phylogenetically diverse than what has been inferred from studies with RDX-degrading isolates. RDX biodegradation was observed when amended microcosms with different electron acceptors: Mn(IV), Fe(III), sulfate and CO_(2) (from added succinate). Derived clones from different electron-accepting conditions were identified, which were grouped in alpha-, gamma-Proteobacteria, and Clostridia. A real-time PCR assay targeted catabolic xenB gene was validated and tested with soil and groundwater samples. The presence of xenB gene would indicate that indigenous of microbial population with xenB gene are present, which can be used to estimate the potential of natural attenuation of RDX.

▼ On-machine inspection (OMI) via on-machine probing (OMP) is a technology that has the potential to provide a step change in the manufacturing of high precision products. Bringing product inspection closer to the machining process is very attractive proposition for many manufacturers who demand ever better quality, process control and efficiency from their manufacturing systems. However, there is a shortness of understanding, experience, and knowledge with regards to efficiently implementing OMI on industrially-based multi-axis machine tools. Coupled with the risks associated to this disruptive technology, these are major obstacles preventing OMI from being confidently adopted in many high precision manufacturing environments. The research pursued in this thesis investigates the concept of enabling high precision machine tools as measurement devices and focuses upon the question of: “How can traceable on-machine inspection be enabled and sustained in an industrial environment?” As highlighted by the literature and state-of-the-art review, much research and development focuses on the technology surrounding particular aspects of machine tool metrology and measurement whether this is theory, hardware, software, or simulation. Little research has been performed in terms of confirming the viability of industrial OMI and the systematic and holistic application of existing and new technology to enable optimal intervention. This EngD research has contributed towards the use of industrial machine tools as traceable measurement systems. Through the test cases performed, the novel concepts proposed, and solutions tested, a series of fundamental questions have been addressed. Thus, providing new knowledge and use to future researchers, engineers, consultants and manufacturing professionals.

The purpose of this thesis is to develop a business model for companies that do foresee the Bottom of the Pyramid (BoP) as a promising and challenging market, that can enhance their position in the competitive context. According to Prahalad’s definition, ―world’s 4 billion poor, people who live on far less than $2 a day‖, are located at the BoP (Prahalad & Hart, first quarter 2002).
Angola, the target market of this study, is one of the world’s poorest countries and one the fastest growing economies. It started reinventing itself economically and politically, after the end of the civil war in 2002, creating the right conditions for companies to invest. Angola attracted since 2003 over 1.124 projects representing more than USD 4 billion in investment capital. This study describes the benefits Angola, as fast growing economy, brings to foreign investors and the many advantages it presents mostly to Portuguese companies due to its long-relationship with Portugal. Doing business in Angola can lead to an indirect development of other sectors, welfare increase and competition. Based on the experience of ONI Communications in vertical solutions, this study developed a business case where farmers subscribed at a farmer cooperative benefited from information on market prices, new products and farming techniques, or accessed micro-credit. The synergies resulted from the partnership among a Portuguese company, a state-owned Angolan company, and a cooperative with a big impact on the target Market at an educational level, or in both infrastructures and sector development. The long relationship between Portugal and Angola, their common bonds and similarities bring benefits to this partnership.
Angola is a market to be explored, although still living the consequences of the Civil War.
With this study a new way to explore markets similar to Angola was developed and a new way to benefit from its emerging opportunities.

▼ Introduction Capsule endoscopy (CE) is a first-line diagnostic tool for known or suspected small bowel bleeding (SBB), and its use has over time been expanded to include panenteric imaging. It offers advantages over conventional endoscopy in minimal invasiveness and ease of use. However several drawbacks remain including the lack of modalities other than imaging, inability to control or propel the capsule, lesser image quality compared to conventional endoscopy and labour-intensiveness of data interpretation. Aims and objectives This thesis aims to explore the ways in which use of CE can be optimised in the current clinical or “real world” context, focusing on its use in gastrointestinal bleeding and working within current resource and technological limitations. Methods A review and analysis of the existing literature was undertaken, examining the present state of CE technology and identifying current gaps in knowledge. Meta-analyses were undertaken examining the effectiveness of the two main methods of image enhancement in CE: the use of bowel preparation and currently available rudimentary computer-aided diagnosis. The following studies then looked into how to better select patients who should be prioritised for CE examination – a pertinent issue in today’s resource-stretched healthcare systems. A retrospective study was carried out to examine the effects of altering the timing of CE examination in patients referred for likely SBB, using cases carried out at our tertiary care centre over the past decade. Outcomes were compared between patients who had undergone CE following negative bidirectional endoscopies, or negative upper gastrointestinal tract endoscopy only. Furthermore, building on existing work, a second study was undertaken using a prospectively-designed database to collect multicentre data on findings and outcomes in young patients referred for CE with iron deficiency anaemia. This study investigated factors predictive of small bowel neoplasia in this patient group. Finally, the effect of image visualisation quality on diagnostic certainty was investigated. CE images were processed to alter image parameters, and the resulting images presented to an iii international group of expert CE readers in order to determine thresholds for acceptable image quality and the effects of differing image quality in the parameters examined. Results Currently-available image enhancement techniques: (1) Use of bowel preparation: Laxative use did not improve the diagnostic yield of CE with odds ratio (OR) 1.1 for both overall and significant findings when comparing laxative use with pre-procedural fast only. However, subjectively-determined small bowel visualisation quality improved with the use of laxatives (OR 1.60 (95%CI 1.08–2.06)), NNT 14. (2) Use of suspected blood indicator (SBI): The overall sensitivity of SBI for bleeding or potentially bleeding lesions was 0.553, specificity 0.578, DOR 12.354. ▼ Hexahydro-1,3,5-trinitro-1,3,5-triazine (RDX) is soluble, nonvolatile cyclic nitramine explosive. Long-term manufacturing and various applications of RDX have resulted in RDX contamination in soil and groundwater. RDX is a possible human carcinogen; therefore, occurrence of RDX in groundwater has raised a public health concern. As RDX is biodegradable; bioremediation of RDX-contaminated groundwater has been recognized as a feasible cleanup technology. Several RDX-degrading isolates are known to have ability to utilize RDX as carbon and/or nitrogen source. However, little is known about these isolates and their roles in the natural or engineered systems during RDX degradation, or about RDX-degrading microbial communities in responding to engineered interventions. Stable isotope
probing (SIP) is a powerful culture independent method that can identify functional active bacteria in various environmental samples. In this study, we applied SIP with ^(13)C-labeled or one of the ring-, nitro- and fully-labeled ^(15)N-RDX to identify microorganisms capable of utilizing RDX and its metabolites as carbon and/or nitrogen sources in groundwater microcosms, and to associate active RDX-degrading microbial communities in responding to engineered interventions.
Derived sequences from ^(13)C-DNA were clustered in Bacteroidia, Clostridia, alpha-, beta- and delta-Proteobacteria, and Spirochaetes, which were different from previously described RDX degraders. Cheese whey amendment stimulated RDX biotransformation, altered the types of RDX-degrading bacteria, and decreased microbial community diversity. Derived sequences from ^(15)N-DNA were grouped in Clostridia, beta-Proteobacteria and Spirochaetes. In comparison to the results among ^(13)C-SIP and ^(15)N-SIP studies with presence of cheese whey, derived sequences were clustered in gamma-Proteobacteria and Bacilli. The combination of these findings suggested that RDX-degrading microorganisms in groundwater are more phylogenetically diverse than what has been inferred from studies with RDX-degrading isolates. RDX biodegradation was observed when amended microcosms with different electron acceptors: Mn(IV), Fe(III), sulfate and CO_(2) (from added succinate). Derived clones from different electron-accepting conditions were identified, which were grouped in alpha-, gamma-Proteobacteria, and Clostridia. A real-time PCR assay targeted catabolic xenB gene was validated and tested with soil and groundwater samples. The presence of xenB gene would indicate that indigenous of microbial population with xenB gene are present, which can be used to estimate the potential of natural attenuation of RDX.

▼ RNA in nature is modified at many specific sites to order to gain extra functions or to expand the genetic code. One of such RNAs is ribosomal RNA (rRNA), which contains several modified bases, particularly around the functionally significant sites. We have focused on understanding the influences of modified base on RNA structure and function by employing helix 69 (H69), which is a good region to evaluate the roles of modified bases since it contains three pseudouridines in the loop region and exists at the core of the ribosome.
Previous model studies using small hairpin H69 showed the conformational differences of H69 loop under different conditions and revealed the significance of modified bases in H69 dynamics. Comparison of crystal structures of ribosomes indicates variable H69 conformations under different conditions. Based on these information, we performed dimethylsulfate (DMS)
probing on 50S ribosomal subunits under different pHs, temperatures and Mg2+ concentrations, showing that H69 has a dynamic RNA loop component on the ribosome level as well as the models, and its multiple conformations are dependent on the presence of modified bases. In addition, footprintings in the presence of aminoglycoside antibiotics neomycin and paromomycin also show conformational variability in H69. These results indicate that H69 exists in multiple conformational states, which could be related to the function of the ribosome in the cell.

▼ ”Base of the Pyramid” (BOP) beskriver de fyra miljarder människor som lever i extrem fattigdom. Även om de är fattiga har de tillsammans stark konsumentkraft, vilket gör BOP till en marknad som inte ska förbises. Företaget Emerging Cooking Solutions (ECS) tillverkar pellets av överbliven biomassa i Zambia som kan användas som bränsle i matlagningsspisar de säljer. Fallstudien på ECS avser att undersöka hur ett innovationsnätverk har möjlighet att påverka etablering av en innovation, i detta fall pellets, på en BOP-marknad. Från befintlig forskning kring BOP, innovationsprinciper samt de 4 A:na identifierades de fyra kategorierna: funktion, kostnad, distribution samt miljö som ett företag bör arbeta utifrån när de utvecklar och etablerar en produkt. Därefter analyseras kategorierna efter innovationsnätverkets nivåer. Studien visar att ifall en innovation är anpassad efter BOP-marknadens krav, samtidigt som den har tydligt koncept, är företaget mer benäget att attraherar rätt aktörer som kan påverka deras etablering. Ifall konceptet har andra värdefulla påföljder, såsom positiv miljöpåverkan, istället för bara ekonomiska kan företaget gynnas ytterligare av nätverket.

▼ This project is a collaborative effort between graduating student Johan van der Schaaf from Delft University of Technology, BoP Innovation Center and client organization Mhogo Foods. The project is conducted mainly in Nairobi, Kenya and focuses on building a marketing pilot for Mhogo Foods to increase access to nutrition in low-income markets. The project taps into the challenge of marketing nutrition and the difficulty of measuring effectiveness of marketing campaigns. After a short literature study and 15 interviews with the target audience that provided several insights, a design-led approach was used to create a tag line that resonates with the target audience. The combined insights resulted in a tag line that formed the key message of the campaign: ‘Mlo Wetu Halisi’ (in English: ‘Eating, in our traditional way’). The design-led approach turned out to be helpful in targeting the message to the preferences of the target audience and ensuring the tag line is inclusive. During implementation of the campaign, all materials and interventions were based on the predefined key message. The implementation of the campaign consisted of a mix of proven offline activities to gain the first market traction with digital experiments to explore new marketing opportunities. The digital experiments consisted of streaming recipe videos through Wi-Fi platforms on local buses and distributing digital coupons through several digital channels that could be redeemed at local shops. During the campaign, different digital channels were tested through the digital coupon program. An initial setup, where coupons would be distributed through Facebook, Whatsapp and the Wi-Fi platforms on buses highlighted the challenge of reaching customers through the digital channels while also educating them on the program. A second iteration, consisting of a hybrid between offline and online channels, resulted in more tangible data, with customers showing understanding and interest in the coupon program. An additional key insight was that the Wi-Fi platforms turned out to be the only digital channel that could both reach the target audience and keep the attention span of consumers for a prolonged period of time. Finally, a tool is presented that can be used in future marketing pilots to assist in building the campaign and in designing strategies for measuring effectiveness of digital channels.

factors that will influence the acceptance and impact of mobile
phone advertising to the urban Bottom of Pyramid (
BoP). The
framework incorporated awareness, availability, acceptability and
convenience as factors for effective mobile phone advertising.
Mobile phones have received unprecedented penetration rates across
all markets, including LSM 1-4. This presents companies targeting
this market with an excellent advertising medium with which to
communicate to consumers. Companies have begun advertising via
mobile phone and this research investigates the impact of this
advertising, in relation to other more established mediums, as well
as the most optimal mobile phone service to be used in delivery of
the advertising message. The type of advertising being sent to

via mobile phone advertising is analysed, along with the type of
advertising the urban
BoP market would like to receive and possible
differences identified. The research examines the recall of
advertising message across various advertising media and
comparisons are drawn. The research found that all aspects of the
framework were applicable. Convenience and acceptability were seen
as the highest drivers for effective mobile phone advertising to
the South African urban
BoP.

▼ Atrial fibrillation (AF) is the most common sustained cardiac rhythm disorder and is increasing in incidence and prevalence in ageing populations. It is estimated that 1.5-2% of the developed world suffers from AF. The risk of stroke and thromboembolism (TE) is increased 4-5 fold by non-valvular atrial fibrillation (NVAF) and nearly 15% of all strokes are caused by AF. Judicious use of antithrombotic therapy significantly reduces the risk of stroke for most patients who have AF. Conversely, underuse of antithrombotic therapy is associated with an increase in the rates of death and ischaemic events (stroke, transient ischaemic attack [TIA], or myocardial infarction [MI]). Despite the proven benefits of anticoagulant therapy among high-risk patients with AF, there have been frequent reports of discordance between guideline recommendations and anticoagulant prescribing patterns leading to problem of underutilisation in patients with AF in real-world practices.
During the time period of this study there were limited data available on the characteristics, clinical management and outcomes of patients with AF from an Australian perspective. The available literature suggested that there was underutilisation of anticoagulant therapy, although this data came from relatively small observational trials in selected patient groups. The Commonwealth Review of Anticoagulation Therapies in AF in Australia identified that stroke prevention in individuals with AF required improvement, and highlighted a range of issues to be addressed related to the assessment of patients for stroke and bleeding risk, appropriate choice of antithrombotic agent(s) in patients with multiple comorbidities, and the monitoring of patients. The review stressed the need for local data on which to base recommendations regarding the treatment of AF. Meanwhile, little was known about the clinical outcomes and safety of direct oral anticoagulants (DOACs) outside the trial setting. In 2011, dabigatran’s sponsor launched a patient familiarisation program in which over 28,000 Australians were enrolled. Thus, we established that there was a critical need for local data regarding the safety of antithrombotic medications, including the DOACs, in the treatment of AF. We designed this study, the Tasmanian AF (TAF) Study, as a starting point to providing comprehensive data describing the outcomes of stroke prevention strategies in Tasmanian patients with AF. The TAF study is an ongoing retrospective study that enrols patients from 3 different hospitals in Tasmania, Australia; the Royal Hobart Hospital (RHH), Launceston General Hospital (LGH) and North West Regional Hospital (NWRH). The rationale behind this project was to derive local data on usage pattern of antithrombotic therapy, clinical outcomes and their safety profile in Australian sub-population - initially prior to the Pharmaceutical Benefits Scheme (PBS) listing of DOACs, and then to use the established database to monitor the impact of the introduction of the DOACs into clinical practice.

Uvod: Krvarenje nakon perkutanih koronarnih
intervencija (PCI) je važna komplikacija sa značajnim nepovoljnim
uticajem na prognozu. Cilj: Da se proceni prediktivna vrednost
preterano niske rezidualne reaktivnosti trombocita na dvojnu
antiagregacionu terapiju aspirinom i klopidogrelom, za krvarenje
nakon elektivne PCI. Metodologija i rezultati: Kod 481-nog
uzastopnog bolesnika sa elektivnom PCI, izvršeno je testiranje
funkcije trombocita pomoću Multiplejt analizatora, gde je
ispitivana reaktivnost trombocita indukovana arahidonskom kiselinom
(ASPI) i adenozin difosfatom (ADP), neposredno pre PCI i 24 časa
nakon PCI. Pacijenti su praćeni prosečno 15.34±7.19 meseci.
Primarni cilj studije bio je pojava svakog krvarenja, dok je
sekundarni cilj bila pojava ishemijskog neželjenog srčanog događaja
(MACE). Učestalost svih, BARK≤2, i BARK≥3 krvarenja prema BARK
klasifikaciji je bila 19%, 18% i 1%, odgovarajuće. Grupe sa svim, i
BARK≤2 krvarenjima, su imale niže prosečne vrednosti multiplejtADP
testa nakon 24 časa, u poređenju sa grupom bez krvarenja:
45.30±18.63 U prema 50.99±19.01 U; p=0.005; i 45.75±18.96 U prema
50.99 ± 18.99 U; p=0.01; odgovarajuće. Ženski pol (HR 2.11; CI
1.37-3.25; p=0.001), prethodni infarkt miokarda (HR 0.56; CI
0.37-0.85; p=0.006), niska telesna težina (HR 0.78; CI 0.62-0.98;
p=0.03), i vrednosti multiplejt ADP testa nakon 24 časa od PCI
ispod medijane (HR 0.75; CI 0.61-0.93; p=0.009) su bili nezavisni
prediktori svih krvarenja, određeni pomoću Koksove univarijantne
analize. Nakon uravnoteženja razlika između poređenih grupa,
multiplejt ADP test nakon 24h, je bio jedini nezavisni prediktor za
sva (HR 0.7; CI 0.56-0.87; p=0.002), i BARK≤2 (HR 0.71; CI
0.56-0.89; p=0.003) krvarenja putem Koksove univarijantne analize.
Zaključak: MultiplejtADP test pre i nakon PCI, je bio povezan sa
svim i BARK≤2 krvarenjima nakon elektivne PCI...

▼ IntroductionTelangiectatic matting (TM) is a morphological description referring to vessels with a small diameter of less than 0.2 mm that can appear sporadically or in well-defined patches (hence the term ‘matting’) primarily on the lower limbs. The aetiology underlying TM remains uncertain however angiogenic and inflammatory mechanisms are believed to play a pathogenic role. The aim of this study was to investigate the pathogenesis of TM and identify possible risk factors. MethodsThis study had two parts. The clinical records of consecutive patients were retrospectively analysed to identify risk factors for TM. In the second part, the haemostatic and coagulation profile of 12 patients with TM were analysed and compared with 12 controls using standard coagulation tests, platelet function and a global assay of coagulation namely rotational thromboelastometry (ROTEM).ResultsIn 352 consecutive patients presenting to a phlebology practice, 25 patients had TM (7.1%). All 25 patients were female with the median age of 45 (27-57) years. A comprehensive medical history was taken. Amongst 27 possible risk factors assessed, statistically significant associations included recurrent epistaxis, easy bruising, hypersensitivity (eczema, hives, hay fever, rhinitis), previous treatment with sclerotherapy or vascular laser for lower limb veins and a family history of telangiectasias. The haemostatic and coagulation profile of patients with TM did not differ significantly from those without TM.ConclusionTM is associated with both hypersensitivity and a
bleeding tendency. Our study revealed no significant increase in the incidence of haemostatic abnormalities in patients with TM compared with the control group. Given the significant association with hypersensitivity disorders we believe underlying mast cell hyperreactivity may contribute to both hypersensitivity and a
bleeding tendency and predispose patients to TM. Further research studying mast cell activity is warranted to establish a definite link.

Nekoliko randomizovanih kliničkih studija je
pokazalo da pacijenti sa velikim krvarenjem kao komplikacijom
akutnih koronarnih sindroma (AKS) i perkutane koronarne
intervencije (PKI) imaju lošiji klinički ishod, sa povećanim
rizikom od nastanka glavnih neţeljenih kardijalnih dogaĎaja (MACE),
koji obuhvataju smrtni ishod, infarkt miokarda, šlog i trombozu
stenta, u odnosu na bolesnike bez krvarenja. Prema tome, prvencija
krvarenja je postala isto toliko značajna kao prevencija
ishemijskih dogaĎaja. MeĎutim, napore da se ovo sprovede ometa
nedostatak jednobraznosti u definicijama krvarenja koje se danas
koriste. U cilju standardizacije brojnih definicija krvarenja kod
bolesnika koji primaju antitromboznu terapiju, tim naučnika
okupljen oko Bleeding Academic Research Consortium-a (BARC) je
nedavno konsenzusom doneo klasifikaciju krvarenja koristeći
hijerarhijski pristup pri opisu stepena teţine krvarenja. Pošto se
od nove jedinstvene definicije krvarenja očekuje da bude široko
prihvaćena u kliničkim studijama i svakodnevnoj kliničkoj praksi,
potrebno je da se njena vrednost potvrdi putem primene na već
postojećim podacima o krvarenju ili podacima sakupljenim putem
budućih kliničkih studija. Cilj: Glavni cilj ove studije je da
ispita odnos izmeĎu bolničkog krvarenja procenjenog prema BARC
klasifikaciji i kratkoročnog i dugoročnog mortaliteta/MACE-a kod
neselektovane populacije bolesnika sa akutnim infarktom miokarda sa
elevacijom segmenta ST (STAIM) podvrgnutih hitnoj koronarnoj
angiografiji i lečenih metodom primarne PKI. Metode: Studija je
obuhvatila 1808 konsekutivnih bolesnika sa STAIM, primljenih u
periodu od avgusta 2009 do januara 2011 godine na Odeljenje
urgentne kardiologije Kliničkog centra Srbije u Beogradu radi
lečenja metodom primarne PKI. Podaci o bolesnicima sakupljeni su
prospektivno i uneti u jedinstvenu elektronsku bazu podataka.
Bolesnici su grupisani prema teţini krvarenja po BARC klasifikaciji
(tip 0+1, tip 2, tip 3a, tip 3b, tip 3c, tip 5a i tip
5b)...

▼ Soil reconstruction in the Athabasca oil sands region
utilises forest floor and peat materials as surface organic
amendments to help reclaim decommissioned mine sites to upland
boreal forests. The objective of this study was to characterize the
microbial community in these two organic materials, and to
determine the relationships between two boreal tree seedlings,
aspen (Populus Tremuloides Michx.) and alder (Alnus crispa Ait),
and their respective rhizosphere microbial communities. The forest
floor exhibited a greater basal respiration than the peat, and a
distinct microbial community structure as seen with phospholipid
fatty acid analysis. Stable isotope probing showed greater carbon
flow between trees and their rhizosphere communities when seedlings
were grown in forest floor material. However, only the alder
seedlings demonstrated correspondingly greater growth in the forest
floor material. These results suggest that forest floor material
fosters a microbial community which interacts more closely with
boreal tree seedlings than peat does.

▼ The spliceosome is a eukaryotic molecular machine composed of uridine-rich small nuclear RNAs (U snRNA) and dozens of proteins that facilitate an RNA processing event known as pre-RNA splicing. By catalyzing the removal of intronic RNA the spliceosome plays an critical role in determining the final RNA sequence from pre-RNA transcripts. Proper removal of introns in pre-mRNA maintains the coding register of mature mRNAs that are eventually translated into functional proteins by the ribosome. Accurate splicing is necessary to repeatedly generate mRNAs coding for the same protein across multiple cells. Additionally, alternative splicing, the regulated inclusion and exclusion of specific exons in the final mRNA, greatly expands the coding potential of eukaryotic genomes. Introns and spliced non-coding RNAs also serve important cellular roles. During splicing the spliceosome undergoes multiple timed rearrangements to properly define the boundaries of each intron and generate the spliceosome active site. Due to the spliceosome’s structurally transient nature the precise functions and mechanisms driving many rearrangements remains undefined.Chemical probing is a powerful tool used for biomolecule structural research and is capable of characterizing spliceosome structural features and changes during splicing. During my thesis I used a new protein probing protocol designed by the Jurica lab to globally map lysine reactivity within multiple conformations of the spliceosome. From our data and published cryo-electron microscopy structures of the spliceosome we predicted how the core spliceosome scaffold Prp8 interacts with U5 snRNA and its 5 exon binding partner during active site formation. We then used our model to guide genetic experiments and tested the necessary amino acids within Prp8 for putative roles in 5 splice site selection. Our genetic data suggests a subset of lysines that change in probe reactivity between the pre-activated and catalytic spliceosome work together with specific spliceosome protein components (Isy1 and Snu114) to position the 5 splice site. In addition to chemical probing, I designed experiments to purify a more conformationally homogenous spliceosome sample for cryo-electron microscopy structure determination. During this work I added a lithium salt wash to my spliceosome purification procedure hoping the salt would wash away loosely associated proteins that bind the spliceosome sub-stoichiometrically. If correct, then 2D particle reconstruction from negative stain electron micrographs will show a more clear average of spliceosomes with higher molecular detail than spliceosomes purified without lithium washes. During the course of this project I successfully purified catalytic spliceosomes, washed them with lithium acetate then subject them to analysis via negative stain electron microscopy. From electron micrographs I generated two dimensional averaged particles that exhibited highly similar features to spliceosome purified without lithium. ▼ High-resolution techniques to characterize the three-dimensional structure of nucleic acids are critical for understanding the mechanisms of action of biologically important RNA and DNA molecules. Methods based on chemical probing have been particularly useful in gaining insight into the structures of nucleic acids in solution. The hydroxyl radical has been widely adopted as a chemical probe for DNA and RNA structure since its first application to protein-DNA footprinting. This dissertation describes efforts to improve upon the current model of how the hydroxyl radical cleaves the RNA backbone, through the use of specifically deuterated ribonucleoside triphosphates (NTPs). The synthesis and purification of deuterated NTPs are described in detail, as well as their application to the study of two RNAs: the sarcin-ricin loop (SRL) RNA - a biologically active region of ribosomal RNA - and a short RNA designed to lack secondary structure. Measurement of deuterium kinetic isotope effects (KIEs) on the cleavage of these RNAs suggests that it is possible to use this experiment to identify the GUA base triple structural motif that is commonly found in RNA.
Abstraction of a 5' ribose hydrogen atom in RNA yields a fragment containing a 5'-aldehyde terminus with the sugar and base intact. Comparison of primer extension products of cleaved SRL RNA with or without deuterium substituted at the C5' ribose position of uracil residues demonstrated that the 5' aldehyde-terminated fragment can serve as a template for reverse transcription. Implications of the presence of a 5'-aldehyde terminus on hydroxyl radical cleavage analysis are discussed in the context of reverse transcriptase-mediated primer extension, a commonly used method.
Structural features of naked DNA molecules with known protein binding sequences were explored using hydroxyl radical cleavage analyzed by capillary gel electrophoresis. An application was written in MATLAB to deconvolute and integrate cleavage intensities of hundreds of peaks in an electropherogram. In many cases, comparison of the cleavage profile to the minor groove width found in an X-ray co-crystal structure of the DNA-protein complex revealed a high degree of correlation.

pregnant with bleeding was the interplay of uncertainty and waiting
and how this experience was shaped by mediating factors such as
previous experience, staff attitudes, social support, information
provision, and lack of privacy. Implications for improving the care
of women experiencing bleeding during early pregnancy are provided.
The findings of this project contribute to our understanding of the
experience of waiting in the context of early pregnancy, underscore
the implication of this experience on women to their health care
providers, and point the way to improving women’s overall
satisfaction with their care.
