The aim of this experimental study was to investigate the effect of the local application of albumin/gloutaraldehyde (Bioglue) on healing of colonic anastomoses under obstructive condition in rats. Eighty Albino-Wistar male rats were divided into four groups, (A, B, C, D) of twenty and eight subgroups (Α1-Α2, Β1-Β2, C1-C2, D1-D2) of ten. Two segmental colonic resections and primary anastomoses were performed five cm proximal to the rectum in both groups A and C. In groups B and D the left colon was ligated at five cm above the rectum. After 24 hours the animals of those groups were reoperated. A colonic resection of the obstructed part one cm around the ligation and a primary anastomosis was performed. In groups C and D the anastomosis was covered with Bioglue. Half of the rats in subgroups (A1, B1, C1, D1) were sacrificed on the fourth and the other half (A2, B2, C2, D2) on the eighth postoperative day. The anastomosis was resected and examined macroscopically for possible rupture and the peritoneal cavity was examined for peritonitis and adhesions. In addition, bursting pressures were calculated. Afterwards, the anastomosis was sent for histological examination and evaluation of white blood cells concentration, neoangiogenesis, fibroblast proliferation and collagen deposition. Two other parts of anastomosis were sent for evaluation of hydroxyproline and collagenase concentrations. For each parameter examined, the Least Significant Difference (LSD) criterion was adopted and testing procedure was computed from the results of a two-way ANOVA. The statistical analyses were performed using the SPSS 15.0 statistical package. On the 4th and 8th postoperative day, the mean body weight decrease, the anastomotic bursting pressure and the fibroblast activity in groups C and D were statistically significant higher compared to group B. On the 4th and 8th postoperative day anastomotic rupture was noted only in animals in groups B and D, but the incidence was not statistically significant higher compared to groups A and C. On the 4th and 8th postoperative day, in groups C and D was a higher score of adhesions formation, inflammatory cells infiltration, neoangiogenesis and collagen deposition and the difference was statistically significant compared to groups A and B. The tissue hydroxyproline concentration, only on the 8th postoperative day in groups C and D was statistically significant compared to group B. On the 4th and 8th postoperative day in groups C and D a higher collagenase I concentration was noted, but the difference was not statistically significant compared to groups A and B. In conclusion, the local application of Bioglue on colonic anastomoses in rats, under obstructive conditions promotes the healing process on the 4th as well as on the 8th postoperative day.
