O objetivo do presente estudo foi comparar dois métodos utilizados para determinar a posição do disco articular em boca fechada por meio de imagens sagitais corrigidas de Ressonância Magnética (RM). Um método analisa a localização da banda posterior em relação aos ponteiros de um relógio, e o outro analisa a localização da zona intermediária do disco articular em relação à cabeça da mandíbula e o tubérculo articular. Este estudo também avaliou a correlação entre o diagnóstico clínico e o diagnóstico obtido por cada método. Uma amostra de 20 pacientes com sinais e sintomas de disfunção temporomandibular foi examinada clinicamente e um diagnóstico clínico foi obtido. Imagens sagitais corrigidas de RM foram obtidas de ambas as articulações utilizando um aparelho de 0,3T com bobinas de superfície bilaterais. O método baseado nos ponteiros de horas de um relógio resultou numa maior percentagem de discos articulares com deslocamento anterior (52,5%) quando comparado ao segundo método (30,0%). Foi encontrada diferença estatisticamente significante entre os dois métodos. A concordância geral entre ambos os métodos foi de 77,5%, a concordância entre o diagnóstico clínico e o método do relógio foi de 70,0%, e a concordância entre o diagnóstico clínico e o método da zona intermediária foi de 87,5%. Não foi encontrada diferença estatisticamente significante entre o diagnóstico clínico e o método baseado na localização da zona intermediária.

The purpose of the present study was to compare two methods used to determine disc position with the mouth closed based on corrected sagittal Magnetic Resonance (RM) images. One method analyzes the posterior band location in terms of hours on a clock face, and the other analyzes the location of the intermediate zone of the disc in relation to the mandibular head and the articular tubercle. This study also evaluated the correlation between the clinical diagnosis with the diagnosis obtained by each method. A series of 20 patients with signs and symptoms of temporomandibular dysfunction was clinically examinated and a clinical diagnosis was reached. Corrected sagittal RM images were acquired for both joints using a 0.3T equipment with bilateral surface coils. The method based on the posterior band location in terms of hours on a clock face yielded a higher percentage of anterior displaced discs (52.5%) when compared with the second method (30.0%). Statistically significant difference between the two methods was found. The overall agreement between both methods was 77.5%, the agreement between the clinical diagnosis and the clock face method was 70.0%, and the agreement between the clinical diagnosis and the intermediate zone method was 87.5%. No statistically significant difference was found between the clinical diagnosis and the method based on the location of the intermediate zone.

A avaliação completa do sistema estomatognático é essencial para o diagnóstico e plano de tratamento ortodôntico. Não se devem apenas avaliar aspectos estáticos da oclusão, mas também os funcionais, pois estes últimos podem mostrar-se alterados e envolver um ou mais componentes do sistema estomatognático, entre eles, a articulação temporomandibular (ATM). Uma das alterações encontradas com freqüência é o deslocamento de disco articular, o que pode interferir no prognóstico e, eventualmente, comprometer o resultado do tratamento ortodôntico. Na tentativa de contribuir para o reconhecimento dessas alterações, este estudo teve como finalidade avaliar a concordância entre dois examinadores calibrados em relação ao diagnóstico de deslocamento de disco articular, comparar a percepção dos pacientes aos ruídos com os dados do exame clínico e eletrovibratografia, além de checar a possível equivalência entre o exame clínico e eletrovibratografia, como métodos de detecção do deslocamento de disco articular. A amostra foi composta por 60 voluntários, divididos igualmente em 4 grupos com 15 participantes cada, definidos por meio do exame clínico e agrupados segundo o gênero e a presença ou ausência de estalo. O exame clínico foi realizado por dois examinadores treinados. A eletrovibratografia foi realizada por um único operador treinado, e empregou-se o programa SonoPAK (Bioresearch Inc.). Para análise dos resultados de concordância entre ambas as técnicas e entre os examinadores, foi utilizado o coeficiente kappa. Os resultados mostraram excelente concordância inter examinadores na realização do exame clínico, concordância satisfatória entre os relatos de ruídos referidos pelos pacientes quando comparados ao exame clínico e eletrovibratografia e concordância também satisfatória entre os dois métodos estudados.

The complete evaluation of the stomatognathic system is essential to the diagnostic and orthodontic treatment plan. It should not evaluate only the occlusal static aspects, but the functional aspects, because the latter could be altered and one or more components of the stomatognathic system could be involved, including the Temporomandibular Joint (TMJ). One of the frequently found alterations founded frequently is the articular disc displacement, which could affect in the prognostic, and, eventually the result of the orthodontic treatment. Este estudo avaliou por meio da Ressonância Magnética a intensidade de sinal do tecido retrodiscal associando a anatomia da eminência articular, derrame articular e deslocamento do disco com e sem redução. Para isto, foram analisadas imagens por RM de 48 ATM de 24 indivíduos sintomáticos, sendo 19 (79,2%) do gênero feminino e 5 (20,8%) do gênero masculino. Considerando o posicionamento sagital, cerca de 5 (10,4%) das articulações eram normais, 23 (47,9%) das articulações eram DDCR e 20 (41,7%) eram DDSR. Para a forma da eminência articular, cerca de 15 (31,3%) das articulações foram classificadas como tendo formato de caixa, 12 (25,0%) foram classificadas como sigmoide, 10 (20,8%) como aplainada e 11 (22,9%) das articulações foram classificadas como deformada. A maioria das articulações apresentaram derrame cerca de 32 (66,7%). Das articulações com DDSR, 16 (51,6%) apresentaram derrame e as com DDCR 15 (48,4%). As articulações com DDCR apresentaram 9 (39,15) em forma de caixa, 6 (26,1%) como sigmóide, 3 (13,0%) estavam aplainada e 5 (21,7%) deformadas; as ATM com DDSR apresentaram 5 (25,5%) como caixa, 4 (20,0%) como sigmóide, 6 (30,0%) aplainadas e 5 (25,5%) deformadas. A forma da eminência articular não está relacionada ao posicionamento sagital (p=0,520). O posicionamento sagital do disco não está relacionado à presença de derrame (p=0,281). A intensidade de sinal do TR foi estatisticamente menor no posicionamento sagital DDCR, quando comparado ao posicionamento sagital DDSR (p=0,016). Houve uma tendência da intensidade do sinal do tecido retrodiscal ser maior nas articulações com derrame quando comparado às sem derrame (p=0,065). Não houve relação entre posicionamento sagital do disco e derrame. E por fim, o presente estudo não conseguiu estabelecer uma relação entre o sinal do tecido retrodiscal e eminência articular (p=0,159).

This study evaluated MR signal intensity of retrodiscal tissue associated with articular eminence morphology, joint effusion status, and disc displacement with and without reduction (DDwR and DDwoR, respectively). Forty-eight TMJ MR images of 24 symptomatic subjects (19 females and 5 males) were analyzed. For the sagittal position, 5 (10.4%) joints were normal, 23 (47.9%) showed DDwR and 20 (41.7%) DDwoR. Regarding articular eminence morphology, 15 (31.3%) joints were classified as box-shaped, 12 (25.0%) as sigmoid, 10 (20.8%) flattened and 11 (22.9%) deformed. The majority of TMJs (32 or 66.7%) showed effusion. Sixteen (51.6%) of the TMJs with DDwoR had effusion versus 15 (48.4%) of those with DDDwR. Of TMJs with DDwR, 9 (39.1%) were box-shaped, 6 (26.1%) sigmoid, 3 (13.0%) flattened and 5 (21.7%) deformed. Among joints with DDwoR, 5 (25.5%) were boxshaped, 4 (20.0%) sigmoid, 6 (30.0%) flattened and 5 (25.5%) deformed. As a conclusion, articular eminence shape was not related to sagittal position of the disk (p = 0.520). Sagittal position of the disk was not related to presence of effusion (p = 0.281). O presente estudo teve como objetivo avaliar por meio de imagens de ressonância magnética a prevalência e os tipos de deslocamentos de disco e alterações ósseas degenerativas que podem acometer a articulação temporomandibular (ATM) de indivíduos adultos portadores de sinais e sintomas de desordens temporomandibulares (DTM), verificando a correlação entre estes achados. Foram avaliadas 224 articulações de112 pacientes, de ambos os gêneros, com indicação para o exame de ressonância magnética. A possibilidade de visualizar simultaneamente o disco articular e estruturas de tecido duro da ATM por meio de um único exame permitiu verificar estas condições e a correlação entre ambas. Para isso um examinador calibrado avaliou as imagens atribuindo escores em relação ao deslocamento do disco articular e alterações ósseas degenerativas. O deslocamento do disco articular foi verificado em 58,42% das ATM avaliadas no corte sagital. O deslocamento anterior de disco com redução foi o mais frequente, ocorrendo em 67,18% dos casos de articulações com deslocamento do disco. Nos cortes coronais, o deslocamento do disco articular foi verificado em 56,65% das ATM avaliadas, sendo o deslocamento medial visto em 96% dos casos. Com relação às alterações ósseas degenerativas as mesmas foram verificadas em 53,94% das ATM analisadas. Houve correlação significativa entre o deslocamento de disco com redução e o aplainamento condilar; deslocamento de disco sem redução e o aplainamento condilar; deslocamento de disco sem redução e alterações degenerativas associadas (aplainamento e erosão; aplainamento, osteófito e erosão; aplainamento e osteófito; erosão e esclerose, aplainamento e esclerose; aplainamento, osteófito e esclerose) e deslocamento lateral do disco articular e osteófitos. A associação entre casos avançados de deslocamento de disco e a ocorrência de alterações degenerativas, enfatizam a importância das imagens por ressonância magnética tanto para um diagnóstico preciso e elaboração de um correto plano de tratamento, quanto para fins prognósticos, nos casos que o exame clínico não é suficiente para estes objetivos

The aim of this study is evaluate by means of magnetic resonance images the prevalence and types of disc displacements and degenerative bone changes that can affect the temporomandibular joint (TMJ) of patients with signs and symptoms of temporomandibular disorders (TMD),checking the correlation between these findings. Two hundred twenty-for temporomandibular joints of 112 patients of both genders, with an indication for MRI, were evaluated The possibility to simultaneously view the disc and hard tissue structures of the TMJ by a single examination has shown these conditions and the correlation between them. ▼ A Disfunção Temporomandibular (DTM) se refere a várias doenças que envolvem os músculos da mastigação e/ou a articulação temporomandibular (ATM). Os distúrbios de desarranjo do disco são considerados uma subdivisão dos distúrbios intra-articulares da ATM e são aqueles onde há relacionamento anormal do disco articular com a cabeça da mandíbula, fossa articular e eminência articular. O procedimento para diagnosticar a DTM geralmente consiste em exame clínico, mas quando informações adicionais são necessárias para confirmar esse resultado, exames de imagem são solicitados. Imagem por ressonância magnética (IRM) é considerada o exame padrão-ouro de referência de diagnóstico não invasivo de deslocamento de disco. Na literatura revisada não há justificativa de que o corte central da IRM deva ser o utilizado para classificar o posicionamento do disco e talvez o uso de vários cortes da IRM pudesse fornecer um dado importante para uma correta interpretação da posição do disco. O presente estudo avaliou a concordância entre três avaliadores para ambos os critérios de classificação (corte central versus vários cortes); verificou a concordância entre os dois critérios, além de correlacionar os resultados obtidos por ambos os critérios com o sinal clínico de estalo em pacientes diagnosticados com DTM. Para medir os níveis de concordância das proporções de imagens deslocadas entre os três avaliadores foi utilizado o coeficiente de correlação intraclasse (ICC). Os níveis de concordância entre os métodos e entre as avaliadoras foram medidos através do método Kappa. O teste utilizado para verificar associações entre as variáveis foi o teste exato de Fisher bicaudal e o nível de significância adotado foi de 0,05. Do total de pacientes examinados 70% (42/60) eram do sexo feminino e 30% (18/60) do sexo masculino. A média de idade foi de 37,2 ± 12,5, Mensurando a concordância entre os avaliadores para os critérios diagnósticos do posicionamento do disco articular da ATM o ICC obtido foi excelente (0,839) para o critério que considerou a proporção de discos deslocados. Para o critério de pelo menos um disco deslocado um Kappa substancial de 0,795 foi alcançado e para o critério do corte central um Kappa de concordância quase perfeita de 0,806 foi encontrado. Um Kappa alto (0,815) também foi atingido na concordância da classificação do posicionamento do disco na IRM em abertura. Após os critérios de avaliação para a posição do disco serem submetidos aos testes estatísticos para a averiguação entre a concordância deles, um Kappa de concordância substancial de 0,697 foi constatado. Notou-se nesse estudo que existe uma associação entre o estalo e a classificação com o disco deslocado pelo método da corte central (p=0,01) e entre o diagnóstico de deslocamento do disco articular obtido através da IMR em abertura da ATM e o estalo (p=0,011). A articulação temporomandibular (ATM) é considerada a articulação mais complexa do corpo humano. Assim como outras articulações do sistema músculo-esquelético ela pode ser acometida pelas mesmas doenças e desordens, como por exemplo, deslocamentos de disco (DD), doenças articulares degenerativas (osteoartrite OA), artrites inflamatórias e sinovites. Baseado nos laudos obtidos por meio das análises das imagens em norma sagital das ATMs de pacientes sintomáticos encaminhados para a realização de exame por ressonância magnética (RM) foram avaliadas a presença de alterações ósseas das cabeças de mandíbula, eminências articulares e fossas articulares do osso temporal e a presença de derrame articular (DA), decorrentes do posicionamento do disco articular. A amostra foi constituída por 148 ATMs de 74 pacientes sintomáticos. As alterações ósseas ocorreram com mais frequência nas cabeças de mandíbula, com destaque para o aparecimento de osteófitos e erosões nos casos de deslocamento de disco sem redução (DDSR). Os resultados dos testes estatísticos revelaram que as alterações ósseas da cabeça de mandíbula do tipo erosão e osteófito e da eminência articular anterior, assim como a presença do DA estão associadas ao posicionamento do disco articular.

The temporomandibular joint (TMJ) is considered the most complex articulation of the human body. Like other joints of the musculoskeletal system it can be affected by the same diseases and disorders, such as disc displacement, degenerative joint disease (osteoarthritis - OA), inflammatory arthritis and synovitis. Based on reports obtained by the analysis of images in standard sagittal TMJs of symptomatic patients referred for magnetic resonance exam were evaluated the presence of bone changes in the condyle, articular eminence and glenoid fossa, and the presence of joint effusion arising from the positioning of the disc. The sample consisted of 148 TMJs in 74 symptomatic patients. The bone changes occurred more frequently in the condyles, especially the appearance of osteophytes and erosions in cases of disc displacement without reduction. The results of statistical tests revealed that the bone changes of the condyle type erosion and osteophyte and anterior articular eminence, as well as joint effusion are associated with the disc position.

▼ The noninvasive measurement of finite displacements and strains in biomaterials by magnetic resonance imaging (MRI) may be shown to enable mathematical estimates of stress distributions and material properties within structures of the body such as articular cartilage or the intervertebral disc. Such methods will allow for non-contact and patient-specific modeling in a manner not currently possible with traditional mechanical testing or finite element techniques. Therefore, the objective of this thesis was to develop computational methods incorporating imaging-based measures of deformation, composition, and local microstructure to permit nondestructive analysis of a range of complex biomechanical systems. Finite strain-based models were developed and applied towards the analysis of several biomaterial systems of increasing material complexity. First, a model for the analysis of a homogeneous, single material system was created and applied to juvenile porcine cartilage for both linear and nonlinear material assumptions under plane stress conditions. Through this study, the viability of estimating stresses within a homogeneous material system solely from MRI-based displacement and strain measures could be established. The model was then expanded to encompass single-plane, multi-region structures and applied towards the analysis of regional stresses within a rabbit intervertebral disc degeneration model. This model incorporated imaging-based methods to estimate heterogeneous properties within the disc structure based upon local biochemical composition, and showed that the degeneration state of a tissue system could effectively be visualized through the use of finite strain-based modeling. A multi-constituent mixture-based material model was next implemented in the analysis of agarose gel constructs. Material parameter estimates from this model were found to agree with those determined by an unconfined compression validation model, establishing physical relevance of noninvasive parameter estimates produced by the models. ▼ [EN] One hundred macerated lunate bones were studied scopically and metrically. The metrical characteristics and the frequency of os lunatum Type I and II were established. Two subtypes of Type I and Type II were also determined according to the presence of one or two facets on the proximal joint bone surface. A full anthropometric characteristic was performed of the wrist bones in 36 bone sets. The metric characteristic of two types of wrist joint sets, e.g. with hamato-lunate joint and without hamato-lunate joint. The presence, localization and frequency of plicae synoviales accessoriae were established together with those of erosions on the joint surfaces and changes in discus articularis in the wrist joint complex as well as their relation to a number of anatomical factors. The adequate blood supply plicae synoviales accessoriae is discussed in relation to the angiographic and scintti- graphic investigations and their innervation is discussed in terms of their significance for the interpretation of the clinically manifested pain discomfort in the wrist. It is suggested that the presence of the folds prevents the joint surfaces on extreme loading of the wrist joints. The vascular anatomy of 100 os lunatum and 338 forearm bones are studied and the results are discussed in view of the development and treatment of Kienbock's disease. ///////////// ///////////// [BG] Изследвани са скопично и метрично 100 мацерирани луновидни кости и са установени метричните характеристики и честотата на os lunatum тип I и тип II. Установени са два подтипа на тип I и тип II в зависимост от обособяването на една или две фасетки на проксималната ставна повърхност на костта. Направена е пълна антропометрична характеристика на китковите кости в 36 костни комплекта. Установена е метричната характеристика на два типа киткови ставни комплекта: с хамато-лунатна става и без лунатна става. Установени са наличието, локализацията, честотата на plicae synoviales accessoriae, на ерозиите по ставните повърхности и промените в discus articulans в китковия ставен комплекс и зависимостта им от редица анатомични фактори. Доброто кръвоснабдяване на plicae synoviales accessoriae се обсъжда във връзка с ангиографски и сцинтиграфски изследвания, а инервацията им - със значението им при интерпретацията на клинично проявяващия се болков дискомфорт в областта на китката. Предполага се, че наличието на гънки предпазва ставните повърхности при екстремни натоварвания в китковите стави. Изследвани са васкуларната анатомия на 100 os lunatum и 338 предмишнични кости и резултатите се обсъждат с оглед развитието и лечението на болестта на Kienbock.
