▼ To determine the prevalence of drug adherence and factors associated with poor adherence to antihypertensive treatment Objective:among adults seen in the department of medicine at UTH. To investigate patient related and health care system related factors associated with poor adherence to antihypertensive Drugs.Methods:Adult patients aged 18 and above with previous diagnosis of essential hypertension receiving outpatient care in the UTH medical clinics were recruited from the first week of November to the second week of December 2010. Data was collected from patients regarding patients‘ social demographic factors, level of education, income per month and family history of hypertension. Information was also collected regarding health care system related factors and care giver related factors to patient non adherence using self report and modified hill bone compliance scale.Data was collected from 234 participants. The mean age was 57.8 ± 12.0 SD. 51 patients (22%) had diabetes mellitus and 44 patients (19%) had the diagnosis of heart failure. The commonest side effects of drugs reported in the study were dizziness and excessive urination, affecting 35% and 31% of patients, respectively. Patients on three antihypertensive drugs were less likely to be non-adherent (odds ratio 0.21, 95% CI 0.06-0.79) than patients taking only one drug.Majority (60%) of the patients were reviewed at least twice in the last 6 months at the time of the interview. 195 (83%) patients reported that drugs prescribed were not available at the hospital pharmacy, but 186 (79%) of these were able to purchase the drugs elsewhere.221 patients (94%) were counseled by the doctor on how to take medicines. Patients counseled by the nurse were more likely to be adherent than those not counseled by the nurse, OR 2.7 (1.0-7.3). Those who were counseled for more than 5 minutes had significantly less non-adherence as reported by both self report and modified Hill Bone with OR of 0.3(95% CI 0.2-0.8) and 0.3(95% CI0.1-0.5), respectively.In multivariable analysis, participants were more likely to be non-adherent by self-report if they had attained a primary level of education, had missed appointments due to lack of transport, or had experienced the side effect of dizziness. Patients with heart failure were more likely to be non-adherent based on the modified Hill-Bone scale, whereas those taking 3 antihypertensive drugs and those who were counseled for more than 5 minutes on drugs were significantly less likely to be non-adherent.Conclusion:The prevalence of adherence among hypertensive patients was found to be higher than anticipated. The factors associated with non-adherence included side effect of dizziness, missed appointment due to lack of transport, and living at a distance of more than 10 km from the hospital. Taking 3 BP drugs and receiving more than 5 minutes of counseling about how to take medications were both associated with decreased likelihood of non-adherence. A equivalÃncia farmacÃutica entre dois medicamentos relaciona-se Ã comprovaÃÃo de que ambos contÃm o mesmo fÃrmaco na mesma dosagem e forma farmacÃutica, o que pode ser avaliado por meio de testes in vitro. No Brasil, os medicamentos alopÃticos sÃo divididos em trÃs categorias quanto ao registro junto Ã AgÃncia Nacional de VigilÃncia SanitÃria: medicamentos novos, medicamentos similares e medicamentos genÃricos. A legislaÃÃo atual dispÃe que para o registro de novos medicamentos genÃricos e similares Ã necessÃria a comprovaÃÃo da equivalÃncia farmacÃutica e a bioequivalÃncia em relaÃÃo ao medicamento de referÃncia. Produtos registrados antes de 2003 tÃm um perÃodo de adaptaÃÃo garantido, de modo que os resultados dos testes de equivalÃncia farmacÃutica e perfil de dissoluÃÃo sejam incluÃdos ao seu registro atÃ 2014. O Programa FarmÃcia Popular do Brasil Ã uma nova polÃtica de AssistÃncia FarmacÃutica, dentro do Sistema Ãnico de SaÃde que tem como objetivo facilitar o acesso da populaÃÃo aos medicamentos considerados bÃsicos e essenciais, diminuindo assim, o impacto dos preÃos dos medicamentos no orÃamento familiar. O objeivo deste trabalho foi avaliar a EquivalÃncia FarmacÃutica de comprimidos de Captopril 25 mg e Cloridrato de Propranolol 40 mg comercializados no Programa FarmÃcia Popular do Brasil, comparando-o com medicamentos de ReferÃncia e GenÃrico, visando discutir a importÃncia da qualidade dos medicamentos para a saÃde pÃblica. Realizaram-se testes fÃsicos e fÃsico-quÃmicos como: identificaÃÃo, determinaÃÃo de peso, desintegraÃÃo, dureza, friabilidade, teor, uniformidade de conteÃdo, pureza e perfil de dissoluÃÃo, segundo a FarmacopÃia Brasileira, 4a ediÃÃo. Os resultados indicaram uma baixa dureza nos comprimidos de cloridrato de propranolol da FarmÃcia Popular. Os perfis de dissoluÃÃo, analisados por ANOVA e teste de Tukey demonstraram diferenÃas significativas (p<0,001) entre os perfis de dissoluÃÃo da FarmÃcia popular em relaÃÃo ao GenÃrico e ReferÃncia nos tempos avaliados dos dois fÃrmacos em estudo. Compararam-se os perfis de dissoluÃÃo dos comprimidos da FarmÃcia Popular com os medicamentos ReferÃncia e GenÃrico atravÃs da EficiÃncia de DissoluÃÃo (ED%). A extensÃo do fÃrmaco dissolvido do medicamento FarmÃcia Popular foi significativamente menor (P < 0,001) do que o ReferÃncia para Captopril e Propranolol. O Captopril da FarmÃcia Popular, apesar de ter cumprido as exigÃncias da legislaÃÃo, foi reprovado no ensaio de ED%. Portanto, os comprimidos de Cloridrato de Propranolol e Captopril da FarmÃcia Popular nÃo foram considerados equivalentes farmacÃuticos em relaÃÃo aos medicamentos ReferÃncia e GenÃrico.

Introdução: A elevação crônica da resistência vascular sistêmica pode ser considerada como a principal alteração hemodinâmica na hipertensão arterial primária estabelecida. Investigamos os efeitos do tratamento crônico com os anti-hipertensivos atenolol (ATE), enalapril (ENA), nifedipina (NIF) e losartan (LOS) sobre a densidade capilar funcional média (DCFM) cutânea e muscular esquelética (grácil) e sobre a densidade capilar estrutural muscular esquelética e do ventrículo esquerdo de SHR. Métodos: Ratos SHR machos com 12-14 semanas receberam tratamento oral por gavagem com ATE (50 mg/kg/dia), ENA (10 mg/kg/dia), NIF (20mg/kg/dia), LOS (10mg/kg/dia) ou veículo (grupo controle) durante quatro semanas. Após o término do tratamento avaliouse a DCFM através de microscopia intravital por epi-iluminação com fluorescência. A seguir, foi avaliada a densidade capilar estrutural através de histoquímica em parafina. Resultados: O tratamento reduziu de forma similar a pressão arterial sistólica de SHR tratados com ATE, ENA, NIF ou LOS. A DCFM encontrava-se diminuída em SHR no músculo esquelético (WKY 39517 e SHR 25813 capilares/mm2, p<0,01) ou pele (WKY 39118 e SHR 21015 capilares/mm2, p<0,01). LOS e NIF reverteram completamente este quadro em ambos os tecidos (43426 e 42218 capilares/mm2 no músculo esquelético e 397 31 e 391 24 capilares/mm2 na pele, p<0,01, respectivamente), enquanto o ENA aumentou significativamente a DCFM apenas na pele de SHR (28317, p<0,05 capilares/mm2). O ATE não induziu nenhuma alteração na DCFM de SHR. Foi observada uma relação linear entre a densidade capilar funcional no músculo esquelético e na pele (r=0.654, p<0.0001). Na análise estrutural, foi observada uma relação capilar/fibra significativamente menor no músculo esquelético de SHR (WKY 1,740,08 e SHR 1,400,06, p<0,01), que foi revertida pelos tratamentos com ENA, NIF e LOS (1,650,04; 1,780,1 e 1,80,07, p<0,01, respectivamente). A razão entre a densidade de volume de capilares e a densidade de volume de fibras (Vv[cap] / Vv[fib]) do ventrículo esquerdo de SHR também foi significativamente reduzida (WKY 0,550,09 e SHR 0,420,09, p<0,01). Os tratamentos com LOS ou ENA reverteram completamente a rarefação estrutural cardíaca de SHR (0,590,03 e 0,590,03, p<0,01, respectivamente), enquanto os tratamentos com ATE e NIF não apresentaram nenhum efeito. Discussão: Os resultados obtidos indicam que os efeitos microcirculatórios de diferentes drogas anti-hipertensivas diferem entre as classes farmacológicas e vão além da redução da pressão arterial. Este conceito pode ser útil para guiar o tratamento anti-hipertensivo na tentativa de reduzir ou até mesmo reverter as lesões de órgãos-alvo.

▼ Introduction: High-stress situations, such as military deployments, may be a risk factor for hypertension. The relationship between the stress triggered by combat deployment and hypertension is unknown. Acute stress from combat conditions can cause a temporary rise in blood pressure, which decreases within hours or days. Cross-sectional studies have shown no association between hypertension and deployment to the 1991 Gulf War or to Vietnam. Self-reported hypertension often is used as an outcome in large population studies. Fair to substantial agreement has been observed between self-reported hypertension and various sources of administrative data, medical records, and blood pressure measurements. The goal of this dissertation was to determine whether recent deployment to Iraq and Afghanistan was associated with new-onset hypertension.Methods: Baseline Millennium Cohort Study questionnaires (July 2001 to June 2003) were completed by 77,047 individuals. Follow-up questionnaires (June 2004 to February 2006) were completed by 55,021 responders. The relationship between new-onset hypertension and history of a recent military deployment was assessed through multivariable logistic regression (N=37,075). Baseline data were analyzed cross-sectionally to assess factors associated with prevalent hypertension (N=70,100). Kappa statistics were used to compare self-reported hypertension with provider diagnosed hypertension and prescription antihypertensive medication dispensed (N=41,129).Results: The 3-year incidence of hypertension was 6.9%. After adjusting for demographic and lifestyle characteristics, deployers without combat exposure were less likely to develop new-onset hypertension compared to nondeployers (odds ratio [OR]=0.74; 95% confidence interval [CI]: 0.64, 0.85). There was no association between deployment with combat exposure and hypertension compared to nondeployers (OR=0.94; 95% CI: 0.82, 1.07). Among deployers reporting combat exposures, the risk for incident hypertension was 1.31 (95% CI: 1.07, 1.61) compared to deployers not reporting combat exposures. The prevalence of self-reported hypertension at baseline among Cohort members was 10.4%. There was moderate agreement between self-reported hypertension and provider-diagnosed hypertension, as well as antihypertensive medications dispensed.Conclusion: Military service members are adversely affected by hypertension. Although deployers had lower incidence of hypertension compared to nondeployers, individuals reporting combat exposures were more likely to report new-onset hypertension. Self-reported hypertension had moderate reliability compared with provider-diagnosed hypertension and dispensed antihypertensive medication. INTRODUÇÃO: A hipertensão é um importante problema de saúde pública e os estudos de Custo-Efetividade (C/E) do tratamento antihipertensivo são raros no Brasil. OBJETIVO: Realizar análise farmacoeconômica retrospectiva do tipo C/E do tratamento medicamentoso em hipertensos. MATERIAL E MÉTODOS: Foi analisado o custo (C) dos medicamentos anti-hipertensivos, do número de visitas médicas não programadas e do tratamento dos efeitos adversos e a efetividade (E) medida pela redução média da pressão arterial em mm Hg ao término de estudo sobre eficácia do tratamento Tradicional iniciado com hidroclorotiazida e atenolol comparado ao tratamento Atual iniciado com losartan e anlodipino, administrados aleatoriamente durante 12 meses a hipertensos sem outras doenças concomitantes pertencentes ao estágios 1 e 2 (grupo HT1-2 = 140 menor ou igual PAS < 180 e 90 menor ou igual PAD < 110 mm Hg) e ao estágio 3 (grupo HT3 = PAS > 180 e PAD > 110 mm Hg). RESULTADOS: A razão C/E (R/mm Hg) no grupo HT1-2 (n = 231) para PAS/PAD dos tratamentos Atual e Tradicional foi de 112,52 ± 395,28 / 181,26 ± 358,91 e 43,05 ± 50,73 / 80,51 ± 108,31 (p < 0,05) ao passo que no grupo HT3 (n = 132) foi de 115,12 ± 254,87 / 108,14 ± 82,56 e 218,59 ± 891,93 / 173,97 ± 447,23 (p > 0,05). CONCLUSÃO: O tratamento Tradicional foi custo-efetivo em relação ao Atual nos hipertensos estágios 1 e 2. Por outro lado, nos hipertensos estágio 3 não houve diferença na razão C/E entre os tratamentos. Estes resultados foram confirmados quando foi utilizado o menor preço de aquisição dos medicamentos e quando foram considerados somente os pacientes que atingiram o controle da pressão arterial ao final do estudo.

INTRODUCTION: Hypertension is an important public health problem and Cost-Effectiveness (C/E) studies of antihypertensive drug treatment are uncommon in Brazil. OBJECTIVE: To perform a cost-effectiveness (C/E) ratio retrospective pharmacoeconomic analysis of drug treatment in hypertensive patients. MATERIAL AND METHODS: Antihypertensive medication cost (C) was analysed, along with the number of non-scheduled medical visits, treatment of adverse effects, and effectiveness (E) based on the average reduction of arterial blood pressure measured in mm Hg at the end of the study comparing the efficacy of a Traditional Treatment with hydrochlorothiazide and atenolol versus Current Treatment using losartan and amlodipine, administered at random for 12 months to hypertensive patients with no other simultaneous diseases and presenting disease stage 1 and 2 (HT1-2 Group = 140 < or = SBP < 180 and < or = 90 DBP < 110mmHg) and stage 3 (HT3 Group = SBP maior or = 180 and DBP > or = 110mmHg). ▼ Abstract: Recent works show that an increased activation of the TGF-? is associated with most of the symptoms of the Marfan syndrome. Studies using mouse models of Marfan treated with losartan have been shown to prevent the degradation of the elastic matrix. Other investigators suggest that the metalloproteinases and the noncanonical ERK signaling are involved in the breakdown of the elastic fiber, which results in the aneurysm. In this study we investigated the role of fibrillin-1 in the arterial thrombosis model using mouse models of Marfan. We analyzed thrombus formation, platelet levels, APTT, PT and TT time, hemodynamic parameter, TGF-? levels, MMP activity as well as platelets, elastic fibers morphology and thrombus. This study demonstrated that Fbn1mg?/+ mice take about 120±21,07 minutes for the thrombus to be formed when compared with wild type, 58±7,16. When these Fbn1mg?/+ mice were treated with antihypertensive losartan and captopril the time taken for the thrombus formation was reduced in 57,5% and 67,5%, respectively. The activity of metalloproteinases and activity TGF-? was increased in the Fbn1mg?/+, however no significant difference between the hemodynamic parameters, levels of totals TGF-?, morphological platelets, elastic fiber and thrombus were observed. Finally, the results suggested that fibrillin-1 interferes with this process and antihypertensive affect the physiology of Fbn1mg?/+ mice. Hypertension is a major contributor to morbidity, mortality, and medical costs in the United States. African Americans (AAs) have the highest prevalence of hypertension in the world, exhibiting at least twice the rate for Caucasians in almost every age and sex group. AAs are also at higher risk for complications from hypertension than Caucasians. While several factors have been proposed as accounting for those differences, there is no consensus on which factors may be responsible; the reasons for disparities in hypertension control in AAs are complex, and need further investigation. This study explored relationships between multiple factors to identify how they might contribute to disparities in hypertension control between AAs and Caucasians. A secondary analysis of data from 1,287 municipal employees participating in City of Birmingham Good Health Program health screenings was used. All participants reporting either a history of hypertension or taking antihypertensive medication were included. Predictors included race, antihypertensive medication, work shift, stress, job and life satisfaction, physical activity, smoking, salty food and alcohol intake, frequency of eating out, high fat food intake, BMI, and waist circumference. The response variable was hypertension control (blood pressure less than 140/90), and covariates included age, gender, marital status, educational level, and income. Multiple logistic regression was used to identify variables associated with hypertension control and with taking antihypertensive medication, and [chi]2 was employed to test for differences in class of antihypertensive medication. The participants were predominantly AA (72%), male (72%), and obese (61%), with a mean age of 48. Fifty two percent had controlled hypertension. After controlling for covariates, AAs had a one third lower odds for controlled hypertension than Caucasians. This relationship held even in participants taking medication. Other variables independently associated with controlled hypertension included income and gender. AAs were no more likely than Caucasians to take antihypertensive medications. Independent predictors of taking antihypertensive medications included income, gender, and age. Though less likely to have controlled hypertension, AAs were no more likely to be prescribed combination antihypertensive therapy

According to the World Health Organization, hypertension is the leading cause of death in the world and it is one of the primary reasons patients seek health care. Prior to 2010, uncontrolled blood pressure (BP) rates were approximately 18%, despite the fact that prescribed medical treatment is available to adequately control patients’ BP. It has been established that medication adherence is a major factor in controlling BP, and self-BP monitoring is highly recommended by the national guideline to improve blood pressure control. However, information related to assessing hypertensive patients’ readiness to engage in health care treatment and self-monitoring is limited in the literature. In addition, there is a gap in the literature regarding self-efficacy for medication adherence and BP control among those patients with access to health care, such as municipal workers. The purpose of the study was to examine the relationships among stage of change, medication adherence, and medication adherence self-efficacy, and to determine the best predictors for the odds of BP control and self-blood pressure monitoring in hypertensive municipal workers. The Transtheoretical Model (TTM), highly acclaimed in the field of health behavior, guided this investigation. A convenience sample of 149 hypertensive workers completed self-administered questionnaires including the Stage of Change Questionnaire, Medication Adherence Self-Efficacy Scale, Morisky Medication Adherence Scale, and a Behavioral Risk Factor Surveillance System (BRFSS) based questionnaire. Descriptive statistics, correlations, and multiple logistic regression models were used for data analyses. The participants were primarily well educated, middle aged (M= 47, SD=8.4), African American, male municipal employees in the public safety department of a large southeastern US city. More than three-quarters of the participants had controlled blood pressure (75.70%) and a high proportion (70%) reported self-blood pressure monitoring. Stage of change was a significant independent predictor of self-blood pressure monitoring. Further, there was a significant correlation between stage of change and medication adherence and stage of change and medication adherence self-efficacy. Age was positively associated with medication adherence self-efficacy and with medication adherence. This study may inform development of stage-correlated interventions to achieve improved BP control in hypertensive workers.

Objetivo: A hipertensão arterial constitui o principal fator de risco modificável para as doenças cardiovasculares e a maioria dos hipertensos necessitará de medicamento para controlar a pressão. Este estudo analisa a prevalência da hipertensão arterial referida e a utilização de anti-hipertensivos por adultos do município de São Paulo de acordo com variáveis socioeconômicas e demográficas. Métodos: Análise de dados do Inquérito de Saúde no Município de São Paulo ISA Capital, estudo transversal, de base populacional conduzido em 2003 que possui 1668 adultos com 20 anos ou mais. Para investigar a distribuição das principais classes de anti-hipertensivos, utilização de genérico, forma de obtenção e custeio utilizou-se de um recordatório de três dias. Resultados: A prevalência de hipertensão referida foi de 16,9%, sendo maior nos indivíduos com idade mais avançada, menor escolaridade e sem ocupação. Entre os que referiam hipertensão, a prevalência do consumo de anti-hipertensivo nos três dias que antecederam a entrevista foi de 73,1%. Dos indivíduos que consumiram anti-hipertensivos 38,3% obtiveram o medicamento através do SUS e 35,3% utilizaram genéricos. As principais classes consumidas em monoterapia foram: inibidores da enzima conversora de angiotensina IECA (41,9%) e diuréticos (24,6%). As principais associações foram: IECA + diurético (36,0%) e diurético + betabloqueador (22,3%). Conclusões: A hipertensão arterial referida se distribui de maneira desigual entre diferentes subgrupos da população. No acesso a medicamento anti-hipertensivo o SUS consegue promover equidade no fornecimento dessas drogas para a população mais desfavorecida. As classes consumidas não estão totalmente de acordo com as diretrizes de hipertensão.

Objective: Hypertension is the major modifiable risk factor for cardiovascular diseases and most of hypertensive patients will require medication for the control of blood pressure. This study analyses the prevalence of self-reported hypertension and the utilization of antihypertensive agents by adults in São Paulo City, Brazil, according to socioeconomic and demographic variables. Methods: Analysis of data from the Health Survey of São Paulo City ISA Capital, a cross-sectional, population-based survey conducted in 2003 with 1668 adults aged 20 years or over. To investigate the distribution of the main antihypertensive drug classes, utilization of generic drugs and information about how the drugs were obtained, individuals who self-reported hypertension were asked about any drug use for high blood pressure in the previous three days. Results: The prevalence of self-reported hypertension was 16.9%. Advanced age, lower education and not having a job were independently associated with hypertension. The consumption of antihypertensive drugs was 73.1%. Among those who took antihypertensive drugs 38.3% and 35.3%, respectively, obtained the medication from the Brazilian Public Health System (SUS) and used generics. ▼ The physiologically most important activator of intravascular fibrinolysis is tissue-type plasminogen activator (t-PA). The endothelium synthesizes and stores t-PA and regulated release of the enzyme is an important local protective response to prevent thrombus extension. Previous work by our group has shown that both patients with primary and secondary hypertension have a reduced capacity to release t-PA upon stimulation, a defect that is likely to contribute to the enhanced risk for arterial occlusion and tissue infarction in these subjects. The mechanism of this impairment is unclear although our experimental studies have indicated that it could be a direct effect of the elevated blood pressure. In order to investigate if the impairment could be reversed by lowering the blood pressure, we used the perfused-forearm model to examine hypertensive subjects for stimulated t-PA release before and after antihypertensive treatment. The findings show that the capacity for stimulated t- PA release can be significantly improved by blood pressure lowering. Treatment increased the amount of t-PA released and also improved the rapidity of the response. The changes were of similar magnitude regardless of treatment with the angiotensin converting enzyme inhibitor lisinopril or the calcium antagonist felodipine, suggesting that the improvement was related to the blood pressure effect per se. To examine the underlying mechanism of blood pressure-induced suppression of t-PA, we explored the potential involvement of the two main haemodynamic forces tensile stress and shear stress. Using in vitro biomechanical experimental models and cultured endothelial cells we observed suppressed t-PA gene expression and protein secretion in response to prolonged cyclic strain stimulation and a magnitude dependent suppression of t-PA transcript with prolonged laminar shear stress. Moreover, all reductions of t-PA were consistently followed by inductions of the main inhibitor of t-PA, plasminogen activator inhibitor type 1 (PAI-1). Further, as hypertension is often associated with a low-grade inflammation, we investigated the impact of the prototypic proinflammatory cytokine tumor necrosis factor-a (TNF-a) on t-PA expression. Prolonged stimulation of cultured endothelial cells was observed to suppress t-PA gene and protein expression. Mechanistic experiments with pharmacologic inhibitors showed that the inhibitory effect was nuclear factor-kappaB (NF-kappaB) and p38 mitogen-activated protein kinase (p38 MAPK) dependent and indicated that potential effector molecules might be the transcription factors NF-kappaB and CREB interacting with the t-PA kappaB and CRE promoter elements, respectively. In conclusion, these findings show that the impaired capacity to release t-PA in hypertensive subjects is directly related to the elevated blood pressure. Data from experimental studies indicate that this impaired fibrinolytic response could be an effect of an enhanced tensile, shear and inflammatory stress acting on the endothelium.

▼ Blood pressure elevation is a major risk factor for cardiovascular events and the risk increases in a dose-dependant manner. It is of importance to identify subjects prone to develop hypertension and adverse cardiovascular remodeling in order to start treatment timely. The increased risk of myocardial infarction and ischemic stroke in hypertension suggests that the condition is associated with prothrombotic mechanisms. Our research group recently discovered that the capacity for activation of the endogenous fibrinolytic system by acute release of tissue plasminogen activator (t-PA) is markedly impaired in subjects with hypertension. This impairment could contribute to the increased risk for atherothrombotic events. The predictive value of blood pressure elevation (SBP 140-160 and/or DBP 85-95 mmHg; BPE group) or normal blood pressure (SBP 110-130 and DBP 60-80 mmHg; NC group) was studied in a cohort of 20-year old men investigated in 1987. The prevalence of hypertension was 74.5% and 5.9% in the BPE and NC group, respectively, at 20 year follow-up. The difference in blood pressure level at baseline between the groups contrasted even more at follow-up. Further, the BPE group had significantly increased left ventricular mass index and intima-media thickness compared to the NC group. We investigated if the impaired fibrinolytic capacity in untreated hypertension could be restored by chronic and acute blood pressure lowering. T-PA release was stimulated by infusion of substance P in the perfused-forearm model before and during chronic and acute blood pressure lowering. Chronic antihypertensive treatment with either the calcium antagonist felodipine or the ACE-inhibitor lisinopril, increased the amount of t-PA released and improved the rapidity of the t-PA response. Changes were similar in the two treatment groups, suggesting the improvement to be related to the blood pressure lowering per se. However, acute blood pressure lowering with intravenous sodium nitroprusside did not affect the stimulated t-PA release. The results of the two studies indicate that high blood pressure decreases the cellular content of t-PA, rather than interfering with the release mechanisms of the protein. Further, we explored the impact of the tensile force component of blood pressure on the regulation of fibrinolytic proteins by studying cultured endothelial cells in an in vitro biomechanical experimental model. Prolonged cyclic strain, mimicking the hypertensive state, was found to suppress t-PA gene expression and protein secretion. In contrast, the main inhibitor of t-PA, plasminogen activator inhibitor-1 was induced, adding to the negative effects of elevated blood pressure on fibrinolysis. In conclusion, blood pressure elevation in young age predicts adverse cardiovascular remodeling and hypertension twenty years later. ▼ The current recommendations by the American Heart Association for health promotion are that all persons should partake in regular physical activity in order to reduce the risk of cardiovascular disease. Regular physical exercise reduces blood pressure and is an important component of the management of hypertension. It is therefore important that patients with hypertension participate in habitual physical exercise. Many hypertensive patients who exercise will require anti-hypertensive medication. However, some antihypertensive agents cause fatigue during exercise. In order for patients to gain the full benefits of an active lifestyle, it is important that the prescribed antihypertensive agent does not prevent them performing and enjoying sustained exercise. It has been well documented that β-blockers cause premature fatigue during physical exercise. The effects on exercise performance of other first line antihypertensive medications, such as calcium channel antagonists have not been extensively investigated. In particular, the effects of these agents on prolonged submaximal exercise endurance have not been well studied. The object of this thesis was to compare the effects of isradipine, a dihydropyridine calcium channel antagonist, to those of atenolol, a β₁-selective antagonist, on maximal and submaximal exercise performance and on short duration high-intensity exercise in physically active hypertensive patients. The study design was a crossover trial where drug treatments were double blinded and randomised. Physically active volunteers with mild to moderate hypertension were recruited. 11 subjects performed i) progressive exercise to exhaustion for determination of maximal oxygen consumption (VO₂max), maximal work load and cardiorespiratory responses to maximal exercise, ii) prolonged submaximal exercise for determination of exercise endurance, cardiorespiratory responses and ratings of perceived exertion (APE), and iii) short duration, high intensity exercise consisting of a 30 second maximal exercise test (Wingate test) to determine skeletal muscle power output, following 4 weeks ingestion of isradipine (2.5mg bd), atenolol (50mg bd) or placebo. Diastolic blood pressure at rest was reduced by both atenolol and isradipine, but was lowered to a greater extent by atenolol (83.3 vs 89.0 vs 96.1 mmHg, atenolol vs isradipine vs placebo, p<.0005). Systolic blood pressure at rest tended to be similarly reduced by both agents, but was significantly reduced during maximal and submaximal exercise by atenolol only (p<.001, atenolol vs isradipine, placebo). Heart rate at rest and during maximal and submaximal exercise was decreased by atenolol only (p<.0005, atenolol vs isradipine, placebo). Maximal exercise performance was reduced after atenolol ingestion compared to placebo but not after isradipine ingestion. Peak workload achieved during the maximal exercise test was decreased after atenolol but unchanged after isradipine ingestion (214 vs 243 W, atenolol vs placebo, p<.01). Le traitement antihypertenseur (TAH) réduit le risque cardiovasculaire (RCV). Son efficacité est établie à partir de nombreux essais et méta-analyses conduits sur différentes populations. L’effet du TAH suit en moyenne un modèle multiplicatif différent d’une classe médicamenteuse à l’autre et non constant dans le temps. Pour progresser vers une prescription personnalisée du TAH, nous avons suivi 3 objectifs: 1) La modélisation statistique de l’effet du TAH sur le risque d’accidents vasculaires cérébraux (AVC) et d’infarctus (IDM) dans différents sous-groupes de patients, selon le temps et la classe médicamenteuse. 2) La méta-analyse de l’effet du TAH sur le risque d’AVC, d’IDM et de mortalité après 80 ans, situation de prescription fréquente. 3) La mise en place, conduite et coordination de l’essai clinique IDEAL, randomisé en plan croisé et double insu dont l’objectif est d’étudier l’influence des caractéristiques individuelles sur la réponse pressionnelle à 2 classes de TAH. Les 2 premiers travaux ont été réalisés sur la base INDANA, méta-analyse sur données individuelles des essais évaluant le TAH contre placebo. Ils suggèrent sans être définitivement convaincants que la réduction du bénéfice au cours du temps sur le risque d’infarctus est plus nette chez la femme et sous bêtabloquants. Chez les patients très âgés le TAH reste efficace pour réduire le RCV, mais notre analyse de l’absence de réduction de la mortalité nous conduit à recommander d’éviter toute intensification du TAH à cet âge. L’essai IDEAL a inclus 124 patients chez lesquels la régression à la moyenne et l’évolution sous placebo expliquent une baisse de pression de même ordre que le TAH.

Antihypertensive treatment (AHT) reduces cardiovascular risk (CVR). Its efficacy is well established from numerous clinical trials and meta-analysis conducted in several populations. The AHT effect follows on average a multiplicative model. This model is different from one drug class to another, and is not constant during follow-up.In order to progress towards a personalized prescription of AHT, we followed 3 objectives: 1) The statistical modelling of AHT effect on stroke (ST) and myocardial infarction (MI) in different sub-groups of patients,depending on time of follow-up and first line drug class. 2) The meta-analysis of AHT effect on the risk of stroke,MI and total mortality in patients ages 80 years and older in whom AHT is frequently prescribed. 3) The set-up,conduct and coordination of the IDEAL study, a cross-over randomized double blind clinical trial in order to assess the influence of individual characteristics on blood pressure (BP) response to two AHT drug classes. The first two analyses were performed on the INDANA database, an individual patient data meta-analysis from trials that evaluated the effect of AHT against placebo. ▼ Cardiovascular diseases (CVD) are the main cause of death worldwide. In 2012, about 17.5 million people died from CVD, accounting for 30% of all deaths. High blood pressure (BP) is a major cardiovascular risk factor, which was responsible for 10.4 million deaths in 2013. Diet and lifestyle play an important role in the etiology of hypertension. Maintenance of a desirable body weight, physical activity, and low intake of alcohol and salt are well-known measures to avoid high BP. Whether dietary protein, or more specifically plant and animal protein, could contribute to maintaining a healthy BP is less clear. The association between BP and CVD mortality has been extensively investigated. BP in prospective studies can be analyzed using different approaches, such as single BP (measured at one moment in time), single BP adjusted for regression dilution, average BP, and trajectories of BP. It is not yet clear which of these approaches is to be preferred for CVD risk prediction. This thesis is centered on BP as a major cardiovascular risk factor. In the first part (Chapter 2, 3 and 4), the relation of dietary protein intake with BP level and change was examined. In the second part (Chapter 5 and 6), various approaches for analyzing repeated BP measurements were compared in relation to CVD and all‑cause mortality risk. The final chapter discusses the main findings and their implications. Chapter 2 describes the association of 24-h urinary urea excretion, as a biomarker of total protein intake, with 9-year incidence of hypertension. We analyzed data of ~4000 men and women aged 28–75 years, who participated in the Prevention of Renal and Vascular Endstage Disease (PREVEND) study, a prospective cohort study. BP was measured four times during 1997–2009 and participants were followed for hypertension incidence, defined as BP ≥140/90mmHg or use of antihypertensive medication. Urea excretion was assessed in two consecutive 24-h urine collections at baseline and approximately 4 years later, from which total protein intake was estimated. Protein intake based on 24-h urinary urea excretion was not associated with incident hypertension. Chapter 3 presents findings for long-term total, animal and plant protein intake in relation to 5‑year BP change. Analyses were based on 702 observations of 272 men who participated in the Zutphen Elderly Study. Participants did not use antihypertensive medication and were initially free of CVD. Physical and dietary examinations were performed in 1985, 1990, 1995, and 2000. BP was measured twice at each examination and protein intake was assessed using the cross-check dietary history method. The upper tertiles of plant protein intake were associated with a mean 5‑year change in systolic BP of ‑2.9 mmHg (95% CI: ‑5.6, ‑0.2), compared with the bottom tertile. Total and animal protein intake was not associated with BP. ▼ Volume overload is a common feature in patients on haemodialysis (HD). This contributes significantly to the cardiovascular disease burden seen in these patients. Clinical assessments of the volume state are often inaccurate. Techniques such as interdialytic blood pressure, relative blood volume monitoring, bioimpedance are available to improve clinical effectives. However all these techniques exhibit significant shortcomings in their accuracy, reliability and applicability at the bed side. We evaluated the usefulness of a dual compartment monitoring technique using Continuous Segmental Bioimpedance Spectroscopy (CSBIS) and Relative Blood Volume (RBV) as a tool to assess hydration status and determine dry weight. We also sought to evaluate the role of Atrial Natriuretic Peptide (ANP) and B-type Natriuretic Peptide (BNP) as a volume marker in dialysis patients. The Retrospective analysis of a historical cohort (n = 376, 55 Diabetic) showed a significant reduction in post-dialysis weights in the first three months of dialysis (72.5 to 70kg, p<0.027) with a non-significant increase in weight between months 6-12. The use of anti-hypertensive agents reduced insignificantly in the first 3 months, increased marginally between months 3-6 and significantly increased over the subsequent 6 months. The residual urea clearance (KRU) fell and dialysis times increased. The cohort was very different to that dialysing at Tassin and showed a dissociation between weight reduction and BP control. This may relate to occult volume overload. CSBIS-RBV monitoring in 9 patients with pulse ultrafiltration (pulse UF) showed distinct reproducible patterns relating to extra cellular fluid (ECF) and RBV rebound. An empirical Refill Ratio was then used to define the patterns of change and this was related to the state of their hydration. A value closer to unity was consistent with the attainment of best achievable target weight. The refill ratio fell significantly between the first (earlier) and third (last) rebound phase (1.97 ± 0.92 vs 1.32 ± 0.2). CSBIS monitoring was then carried out in 31 subjects, whilst varying dialysate composition, temperature and patient posture to analyse the effects of these changes on the ECF trace and to ascertain whether any of these interventions can trigger a change in the slope of the ECF trace distinct to that caused by UF. Only, isovolemic HD caused a change in both RBV and ECF in some patients that was explained by volume re-distribution due to gravitational shifts, poor vascular reactivity, sodium gradient between plasma and dialysate and the use of vasodilating antihypertensive agents. This has not been described previously. These will need to be explored further. The study did demonstrate a significant lack of comparability of absolute values of RECF between dialysis sessions even in the same patient. This too has not been described previously. Chemotherapy is still one of the primary modalities for the treatment of cancer. However, the application of free anticancer drugs has several drawbacks due to the high toxicity, the lack of selectivity and the low bioavailability. The main objective of the current PhD thesis was to improve the activity mainly of antiproliferative drugs but also of other drugs and bioactive compounds. To achieve this, two axes were followed:a) Design, synthesis and biological evaluation of bioconjugates for selective drug delivery and tumour targeting: Two antiproliferative drugs were studied gemcitabine and sunitinib. Selective drug delivery was achieved by the conjugation of these drugs to gonadotropin-releasing hormone (GnRH) peptide in order to target the GnRH receptor, which is found to over express in different tumor cells. First the chemistry of drug-peptide conjugation was studied and more specific the oxime bonds, the carboxylic acid ester bonds and the carbamate bonds in chemical linkers. Then the biological evaluation of bioconjugates was tested in androgen-independent CaP cell lines followed by the pharmacokinetic study in mice. The results showed that the bioconjugates showed better anticancer activity or stability than the parent drugs. In this way, effective concentration of drugs at target can be achieved by selecting the linker with specific bonds according to required condition of particular drug.b) Molecular hybrids: In the synthesis of molecular hybrids, gemcitabine (anticancer) and quercetin (antioxidant/anticancer) were studied. The hybrid product of gemcitabine with lipoic acid (prooxidant) showed better anticancer and prooxidant activity than the parent compounds. About quercetin: Synthesis of the different hybrids for different purposes were presented. First, quercetin-aminoacids hybrids are useful for improving biological availability by improving aqueous solubility (quercetin-glutamic acid) of quercetin, which was the major cause of quercetin low bioavailability. Second, quercetin used for conjugation with the Losartan and Captopril drugs, which are used for antihypertension. As we know lowering blood pressure is the only treatment available for hypertensive patients, and therefore lowering blood pressure causes oxidative stress in vascular level in hypertension treatment. Therefore our molecular hybrid Quercetin-Losartan and Quercetin-Cprotpil will help to minimize hypertension as well as generated oxidative stress.
