Les syndromes de douleur persistante sont très débilitants pour ceux qui en souffrnt et malheureusement, la pharmacothérapie pour traiter ce type de douleur n'offr pas le soulagement souhaité. Les drogues agissant par liaison à la protéine alpha[indice inférieur 2]-delta semblent conduire à un meilleur soulagement de la douleur chronique, mais leur action pour y arriver n'est pas clairement établie. En effet, un nombre restreint d'études ont étudié leur action au niveau du système nerveux central. Certaines études concluent que la prégabaline et la gabapentine agiraient seulement lorsque les neurones sont dans un état sensibilisé, tandis que certaines autres prétendent que ces médicaments agiraient aussi sur les neurones à leur état basal. Aussi, plusieurs études animales rapportent que la prégabaline agirait au niveau de la moelle épinière, mais aucune étude humaine ne le confirme clairement. Une étude réalisée chez des sujets sains a quant à elle conclu que la prégabaline agirait au niveau des centres supérieurs, ce qui pourrait laisser croire que la prégabaline produirait des effets sur la douleur autant par une action spinale que supraspinale. Malgré plusieurs hypothèses, plusieurs questions demeurent en suspens pour en arriver à comprendre clairement les mécanismes neurophysiologiques de l'analgésie par la prégabaline. Le but de cette étude est donc d'étudier les propriétés anti-hyperalgésiques et anti-nociceptives de la prégabaline, tout en investiguant les endroits du système nerveux central ou la prégabaline produit ses effets sur la douleur. Grâce à l'étude du réflexe de retrait nociceptif chez des patients prenant de la prégabaline, nous avons pu évaluer son effet au niveau de la moelle épinière et de l'encéphale par électrophysiologie, en comparant les réponses des patients obtenues avant la prise du médicament avec celles obtenues suivant quelques semaines de traitement. De plus, grâce aux changements possibles de fréquences de stimulations électriques, les neurones ont été étudiés dans un état basal et sensibilisé, permettant d'étudier les effets antinociceptifs et anti-hyperalgésiques de la prégabaline. Pour en arriver à formuler certaines hypothèses pouvant servir aux études subséquentes sur le sujet, cinq patients ont servi aux analyses finales. D'abord, la prégabaline agirait effectivement sur la moelle épinière, puis elle aurait des propriétés anti-hyperalgésiques, mais une étude réalisée avec des sujets sains pourrait permettre d'étudier plus clairement ses propriétés anti-nociceptives. Ces résultats offrnt donc des pistes très intéressantes de recherche pouvant mener à une amélioration du traitement des douleurs chroniques. Advisors/Committee Members: Marchand, Serge (advisor).

Pluchea sagittalis (Lam.) Cabrera, Asteraceae, é conhecida popularmente como quitoco, arnica ou erva lucera. Seu caule e folhas são utilizados em infusão para tratamento de dores corporais, inflamação, dispepsias nervosas e histerismo. O objetivo desse trabalho foi isolar e identificar os constituintes químicos do extrato etanólico das folhas de P. sagittalis, verificando o potencial farmacológico desse extrato, em especial, sobre a ansiedade. A planta foi coletada no Bairro Japãozinho, Aracaju SE. As folhas selecionadas foram secas em estufa a 40C, com circulação de ar, trituradas e submetidas à extração com etanol P.A., em soxleht. O extrato obtido foi fracionado por cromatografia em coluna e analisados em HPLC. O extrato etanólico e frações: aquosa, etanólica e acetônica obtidas, foram submetidas ao screening fitoquímico e a triagem farmacológica comportamental, na dose de 100 mg/kg, v.o. A atividade motora dos animais foi observada nos testes Open Field e Rota Rod. O efeito hipnótico/sedativo foi avaliado no teste Tempo de Sono Induzido por Barbitúrico e a atividade ansiolítica verificada no teste de Labirinto em Cruz Elevado e Anticonvulsivante induzido por Pentilenotetrazol. Nas análises fitoquímicas identificou-se a presença de fenóis, flavonóides, terpenos e esteróides, sendo isolado um sesquiterpeno. Na triagem farmacológica comportamental observou-se o potencial efeito depressor do Sistema Nervoso Central (SNC) de todas as amostras testadas. No Open Field, verificou-se que os animais tratados com a dose de 200mg/Kg do extrato etanólico bruto apresentaram aumento no número de quadrantes e tempo de deslocamento, não sendo observada alteração na coordenação motora ou relaxamento muscular no Rota Rod. As doses de 100 e 200 mg/Kg demonstraram-se indutoras e potencializadoras do sono induzido por barbitúrico. A ação ansiolítica foi observada na dose de 200 mg/Kg no labirinto em cruz elevado. Além do efeito protetor contra convulsões induzidas por ptentilenotetrazol, das doses de 100 e 200 mg/Kg. Conclui-se que o extrato etanólica de P. sagittalis ação ansiolítica, sendo possível a participação de terpenos nesse efeito.

Pluchea sagittalis (Lam.) Cabrera, Asteraceae, popularly known as quitoco, arnica, yerba lucera. Its stem and leaves are used in infusion to treat body pain, inflammation, nervous dyspepsia and hysteria. The purpose of this study was to isolate and identify the chemical constituents of the ethanol extract of leaves of P. sagittalis by checking the pharmacological potential of this extract, in particular on anxiety. Abstract: Rationale: The majority of women with epilepsy do not experience significant changes during pregnancy; however the persistence of seizures and the effect of antiepileptic drugs (AED) may affect the fetus. These effects include lower intrauterine growth, dysmorphisms, fetal malformation and neuropsicomotor developmental delay. Most studies report that the risk of fetal malformation is two to three times higher in fetus exposed to AED. Objectives: The goals of our study were: to identify the occurrence of fetal malformation in children of woman with epilepsy (WWE); to determine the occurrence of intrauterine developmental delay; to investigate the growth and development of these children; to describe labor complications; and to assess birth and child outcome. Methods: From May 2003 to May 2007 we evaluated 67 WWE in 69 gestations, ages ranging from 17 to 37 years, average 26.9 years. Those patients were compared with 66 non-epileptic pregnant women (control group) with 68 gestations, ages ranging from 15 to 44 years, average 26.9 years. Patients were referred from the Epilepsy Unit and Medical Genetics section of Hospital das Clínicas ¿ Unicamp. Results: Two WWE abandoned the study, although the data of one of them were recover. Forty women of the non-epileptic group quitted the study but we were able to recovered the data of 21 of these women reporting on the delivery and fetal malformations. Twenty-one women had their information lost. Follow-up was obtained in 52 WWE and 26 control women. Forty-five women were on one AED, 13 were on two AED and two were on three. Carbamazepine was used by 38 pregnant women, 26 were in monotherapy and 12 were in polytheraphy. Phenobarbital was the second most used AED, seven in monotherapy and five in polytheraphy. Three patients were on no medication. All patients were seen by the same neurologist, monthly during pregnancy and the post gestation period. In addition, anthropometric and neurological evaluation were performed by a geneticist at birth period, six month, 1, 2, 5 and 7 years of age. Three children of WWE presented complications: hypoxic ischemic encephalopathy (1), abruptio placentae (1) and pneumothorax (1). Low birth weight was identified in children of WWE. There was no difference in the Apgar scores in the first and fifth minutes, and head circunference in the two groups. Ten (16.39%) children from WWE (urogenital anomalies ¿ 2, cleft lip+cleft palate ¿ 1, gastrointestinal defects ¿ 1, skeletal defects ¿ 1, hernia ¿ 2, congenital deafness ¿ 3) and two (3.92%) of the control group (cardiac defects ¿ 2) showed major fetal malformations. Dysmorphic signs such as ear anomalies (28.85%), hypertelorism (62.96%), and small head circumference, were more frequent in children of epileptic mother, particularly associated with use of phenobarbital and complex partial seizures. Conclusions: We conclude that major fetal malformations were more frequent in children of WWE (16.39%) compared to controls (3.92%).
