Background: Studies comparing isobaric local anaesthetics (LA, e.g., bupivacaine [B], ropivacaine [R] and levo-bupivacaine [LB]) with the addition of fentanyl (F) and the concomitant use of epidural volume extension (EVE) with normal saline in obstetric patients do not exist to date. Methods: Parturients (n=120) scheduled for elective c-section were allocated to 6 groups (n=20 per group): 3 groups received isobaric B 10 mg with 10 μg F (BF), R 15mg with 10 μg F (RF) , LB 10mg with 10 μg fentanyl (LBF) or the same LA-F combinations with epidural volume extension (EVE) with 10 ml of normal saline (N/S, e.g., BF-S, RF-S and LBF-S), respectively. Sensory and motor changes, haemodynamics and side effects were assessed. . Results: The maximum sensory block was faster in all 3 EVE groups (BF-S, RF-S and LBF-S) only to the LBF group (P < 0.001). Furthermore the BF-S group showed a significant increase in the max sensory blocks above the T2 dermatome compared to both LBF and LBF-S groups ((P < 0.05). The time of sensory regression to T10 was not significantly different among the 6 groups studied. The max Bromage block (Br-3) was similar in all 6 groups studied. As far as the complete motor block (MB) regression is concerned only the BF group showed a significant prolongation (138.8 ± 29.6min) compared to the other 5 groups (P < 0.01 to P < 0.001). All 3 EVE groups showed similar quality of intraoperative analgesia. Time to first request for supplemental analgesia was significantly shorter in all three EVE groups compared to BF, RF and LBF. Among the 3 non-EVE groups the LBF group showed the shortest time to first request for supplemental analgesia compared to BF (P<0.001) and RF (P<0.01) respectively. All parturients experienced intraoperatively a similar high satisfaction score. The LBF groups showed less haemodynamic compromise, requiring less supplemental Ringer’s Lactate or etilefrine compared to groups BF (P < 0.05) and RF (P < 0.01), respectively. The overall hypotensive events were less common in the LBF group compared to the 2 other non EVE groups respectively. The only major side-effect observed was nausea was more than twice as high in all 3 EVE groups compared to the BF, RF and LBF groups (P < 0.05 – P<0.01), respectively.Conclusions: The LBF group results in equal quality of intraoperative analgesia compared to BF and RF with the advantage of better consistency of cephalad block and less hemodynamic compromise compared to all other groups. EVE with 10 ml N/S to this group (group LBF-S) produces a faster onset of sensory block (P<0.001), it does not increase the maximum sensory block, but it produces more haemodynamic compromise compared to group LBF (P < 0.01). Finally all 3 EVE groups (BF-S, RF-S and LBF-S) showed a faster motor block recovery compared to the 3 non EVE groups, which might be of immense advantage in fast-track obstetric surgery.

THE CHANGES IN PLASMA ANGIOTENSIN II LEVELS DURING DELIBERATE HYPOTENSION WITHSODIUM NITROPRUSSIDE (SNP) AND THE RELATIONSHIP BETWEEN PLASMA ANGIOTENSIN II LEVELS AND THE REBOUND HYPERTENSION AFTER DISCONTINUATION OF SNP, WERE STUDIED. THIRTY PATIENTS (49.5 +- 1.3 YEARS) UNDERGOING ELECTIVE CRANIOTOMY FOR INTRACRANIAC ANEURYSM AND BRAIN NEOPLASM SURGERY PARTICIPATED IN THIS STUDY. 0.5 - 3 MG KG-1. MIN- 1 INTRAOPERATIVELY SNP WAS INFUSED AFTER THE INCISION OFDURAMATER. PLASMA ANGIOTENSIN II LEVELS INCREASED SIGNIFICANTLY (P < 0.01) AS COMPARED TO CONTROL VALUES AND MAP, SAP, DAP EVENTUALLY ROSE TO VALUES SIGNIFICANTLY HIGHER (P < 0.05) TO CONTROL PRIOR TO SNP INFUSION- REBOUND HYPERTENSION. II) GROUP II WOMEN, N =9, EXACTLY THE SAME RESULTS, LIKE GROUP I AND III) GROUP III (WOMEN, N=2) 60 MIN AFTER THE DISCONTINUATION OF SNP PLASMA ANGIOTENSIN II LEVELS AND MAP, SAP, DAP WERE MAINTAINED NEAR TO CONTROL LEVELS(P < 0.10), NO SIGINIFICANT DIFFERENCE. THE ROLE OF THE RENIN-ANGIOTENSIN SYSTEM IN THE REGULATION OF BLOOD PRESSURE, DURING SNP INDUCED HYPOTENSION WAS EVALUATED. IN ADDITION, THE ROLE OF ANGIOTENSIN II, WHICH IS THE POTENTIATED PRODUCT OF THIS SYSTEM, IN THE POST-SNP REBOUND HYPERTENSION WAS DEMONSTRATED.
