amitriptyline concentration during the acute phase of absorption. Factors that were found to be important were the
amitriptyline to nortriptyline (as metabolite) ratio, the nature of the specimen, and the collection site. The calculation of the dose from an acute ingestion of
amitriptyline resulting in death cannot be accurately determined from a single specimen collected at autopsy.

▼ Objetivo: Revisar ev idências de eficácia e segurança da duloxetina em comparação à amitriptilina no tratamento de fibromialgia. Métodos: Foi construída uma o verview de revisões sistemáticas de ensaios clínicos com duloxetina e amitriptilina no tratamento da fibromialgia, reunindo as evidências de eficácia e segurança existentes. Método de busca: bases de dados Cochrane, PubMed, EMBASE e SRDR. I diomas e ano de publicação : sem restrições. A aplicação dos critérios de inclusão e a condução da extração de dados e análises foram realizadas em duplicata, de forma independente . A qualidade metodológica de cada revisão sistemática foi avaliada utilizando a ferramenta AMSTAR. A qualidade da evidência foi avaliada através do G RADE adaptado abrangendo a avaliação de critérios objetivos sintetizados narrativamente . Resultados e discussão: a pesquisa identific ou 122 artigos que, após exclusão dos que não preenchiam os critérios, restaram 13 revisões que
foram lidas na íntegra. Des tas 5 foram excluídas por não atendimento de critérios ou serem versões mais antigas , restando 8 revisões analisadas. As diferenças de eficácia entre a duloxetina e a amitriptilina no tratamento da fibromialgia variam conforme o sintoma. Há recomendação a favor da amitriptilina com baixa evidência para dor, moderada evidência para sono e fadiga e alta evidência para qualidade de vida. A duloxetina tem recomendação com alta qualidade de evidência quando os pacientes possuem transtorno de humor de modo preponderante, tem maior aceitabilidade com baixa qualidade de evidência e é a primeira escolha para pacientes idosos por razões de segurança. Conclusão: ambos os medicamentos podem ser utilizados no tratamento da fibromialgia, a depender do sintoma preponderante e do perfil do paciente. Os resultados se alinham com a complexidade já conhecida desta doença. Esta overview ajuda na orientação de clínicos e tomadores de decisão em políticas de saúde pública

Objective: Review evidence of efficacy and safety of duloxetine compared to amitriptyline in the treatment of fibromyalgia. Methods An overview of systematic reviews of clinical trials with duloxetine and amitriptyline in the treatment of fibromyalgia was constructed , gathering the existing evidence of efficacy and safety. Search method: Cochrane, PubMed, EMBASE and SRDR dat abases. Lan guages and year of publication: no rest rictions. The application of inclusion criteria and the conduct of data extraction and analysis were carried out in duplicate, independently. The methodological quality of each systematic review was assessed using the AMSTAR tool. ▼ O uso de vasoconstritores associados a soluções anestésicas locais em pacientes que fazem uso de antidepressivos tricíclicos gera controvérsias por seu possível efeito sobre o sistema cardiovascular. Este estudo teve por objetivo avaliar alterações na pressão arterial e frequência cardíaca de ratos tratados ou não com antidepressivos tricíclicos, após a infiltração em fundo de sulco vestibular e injeção intravenosa de adrenalina, felipressina e fenilefrina, em doses equivalentes (DE) às quantidades presentes em 2, 8 e 32 tubetes de anestésico local. Foram utilizados 42 ratos Wistar machos pesando de 300 a 500 gramas, tratados por gavagem durante sete dias com cloridato de amitriptilina (0,3mg/Kg). No oitavo dia, após anestesia do animal, um cateter foi implantado na artéria carótida, a cânula arterial foi conectada a um transdutor de pressão acoplado ao sistema PowerLab 4/30 de registro invasivo de pressão arterial, utilizando software adequado.
Após a infiltração e injeção intravenosa, um intervalo de 30 minutos foi respeitado entre as doses para evitar efeito cumulativo. Para a análise dos resultados foi utilizada análise de variância de medidas repetidas a um ou dois critérios de classificação, seguido do teste de Holm-Sidak. Observou-se que o tratamento com amitriptilina provocou elevação significativa na frequência cardíaca no grupo tratado em relação ao controle (n=21) (258,252 ± 6,32 e 221,790 ± 7,22, respectivamente, p<0,001) e queda significativa na pressão arterial do grupo tratado comparado ao grupo controle (101,80 ± 2,52 e 110,12 ± 2,91, respectivamente, p<0,05). A adrenalina mesmo utilizada em via infiltrativa apresentou efeito cronotrópico positivo em todas as doses (2 DE 197.15 ± 3,74 e 240.67 ± 8,14, 8 DE 212.44 ± 6,72 e 238.17 ± 10,05, e 32 DE 246.45 ± 11,13 e 251.32 ± 10,46 bpm, para controle e tratado, respectivamente, p>0,05). Na injeção intravenosa, mesmo com o nítido aumento de
pressão e da frequência cardíaca, não houve diferença significativa entre o grupo controle e tratado. O tratamento com amitriptilina potencializa ligeiramente a resposta hipertensora após infiltração de adrenalina. A felipressina promove menores alterações de pressão arterial e a fenilefrina mostrou-se o vasoconstritor mais potente dos três estudados, produzindo alterações importantes de pressão arterial e de frequência cardíaca, mesmo por via infiltrativa, em doses superiores a 8 tubetes.

▼ Doenças inflamatórias intestinais (DII) em humanos são reações crônicas de etiologia complexa. Trata-se de uma reação imunológica exacerbada e depende da microbiota. O sistema nervoso interage com a imunidade do intestino de um modo bidirecional. Relatos clínicos e poucos achados experimentais apontam para uma ligação entre transtornos depressivos e doenças inflamatórias intestinais, sugerindo interação neuroimunológica na patogenia deste processo. Ainda, o tratamento de Doenças inflamatórias intestinais (DIIDoença de Crohn e Colite Ulcerativa) com antidepressivos em modelos murino de colite têm sugerido bons resultados na redução da inflamação. O mecanismo da inflamação na DII e a participação do sistema nervoso ou da modulação de tal processo pelo emprego de antidepressivos ainda não está totalmente elucidado. Este estudo teve como objetivo estudar o efeito do antidepressivo amitriptilina em um modelo murino de colite. A colite foi induzida em
camundongos C57BL/6 por Dextrano Sulfato de Sódio (DSS) e a amitriptilina (AMT) foi administrada por via oral, em regime profilático ou terapêutico. Avaliamos a dose de AMT no teste de suspensão da cauda (TSC), o acúmulo de neutrófilos pela atividade de mieloperoxidase (MPO), burst oxidativo, curva de sobrevida, histopatologia do intestino, atividade da doença por sintomas clínicos, a depleção de muco intestinal, citocinas inflamatórias no cólon e no soro, fenotipagem de linfócitos T CD4+, T CD8+, e monócitos CD14+,. Resultados: A dose de AMT (200 μg/ml) e os regimes de tratamento utilizados aqui foram capazes de impedir ou diminuir a histopatologia da colite, os sinais clínicos (ganho de peso (%), comprimento e peso do cólon) e a mortalidade dos animais no modelo terapêutico do grupo inflamado e tratado com AMT. A atividade de MPO, níveis circulantes de IL- 1 β, IL- 6 e TNF- α foram reduzidas nos dois protocolos experimentais (profilático e terapêutico).
Conclusões: Este estudo incluiu um período de tratamento prolongado, visto que os antidepressivos são conhecidos por serem eficazes em seres humanos depois de várias semanas a meses de prescrição, e confirmou a eficiência da via de administração oral, uma vez que os antidepressivos são geralmente administrados por via oral a seres humanos. Este regime de tratamento melhorou o potencial anti-inflamatório de AMT na redução DSS-colite em camundongos, com base nos parâmetros estudados

▼ A utilização de adjuvantes como gabapentina e amitriptilina por via sistêmica é uma ótima opção no tratamento analgésico de síndromes dolorosas crônicas que culminam com hiperalgesia e alodinia, com baixa incidência de efeitos colaterais. Entretanto, os efeitos do emprego destes fármacos em cães ainda são pouco conhecidos, bem como sua real aplicação, pois faltam estudos clínicos nesta espécie. Portanto, este estudo teve o intuito de avaliar a ação analgésica da administração de isolada de gabapentina ou amitriptilina em cães portadores de dor crônica de qualquer etiologia que não oncollógica. Avaliou-se o efeito analgésico através da escala numérica verbal (ENV), questionário de qualidade de vida e testes para verificação de alodinia. Foram também avaliados os valores do hemograma completo, funções hepática e renal dos animais incluídos no estudo antes e após o tratamento, afim de que a segurança dos fármacos seja verificada.Foram dosados níveis
séricos de TNF-α e IL-6 para comparação dos valores antes e após a terapia. Animais que necessitaram de resgate analgésico receberam dipirina 25 mg/kg. Foram utilizados 18 animais provenientes do Ambulatório de Dor e Cuidados Paliativos do Hospital Veterinário da Universidade de São Paulo. Estes animais foram distribuídos em 2grupos de forma aleatória, com 9 animais em cada grupo. Os animais participantes do presente estudo receberagabapentina na dose de 10 mg/kg a cada 12 horas, ou amitriptilina 1 mg/kga cada 24 horas pela via oral.Os retornos foram realizados semanalmente, avaliando-se valores da ENV, qualidade de vida e alodinia. Os resultados foram analisados estatisticamente pelo teste de Wilcoxon para dados não paramétricos. Com base nos resultados obtidos, pôde-se concluir que o uso isolado da gabapentina e da amitriptilina promovemelhora na qualidade de vida e redução dos escores na ENV em cães portadores de dor crônica neuropática, apesar de demonstrarem resultados
ainda melhores como parte de um protocolo de analgesiamultimodal. A amitriptilina demonstrou-se superior, pois os componentes de seu grupo não necessitaram de resgate e obtiveram maior redução de escores de dor na ENV e alodinia e melhores índices de qualidade de vida. Os animais pertencentes ao grupo da gabapentina não apresentaram melhora significativa nos parâmetros observados. Não foram observados efeitos adversos relacionados durante o período do estudo.

▼ Introduction: Toxicological analysis of post-mortem tissue can confirm whether drugs were implicated in death. Alternative toxicological specimens, such as blowfly larvae (Lucilia sericata) are used when traditional tissue or blood samples are not available for toxicological analysis. Blowfly larvae have a high tolerance to certain drugs, and can grow on tissue spiked with a doses of certain drugs that are lethal to humans. This tolerance suggests that the larvae are able to either metabolise or eliminate the drugs, preventing accumulation within the larvae. However, it is not known whether Lucilia sericata larvae are capable of metabolising drugs. If larvae can metabolise drugs this has significant implications for how future entomotoxicological assays are designed. To investigate whether Lucilia sericata larvae were capable of metabolising, the model drug
amitriptyline (AMI) was used. AMI is commonly involved in overdose, and it has a well characterised metabolic pathway.
Aim: To determine whether Lucilia sericata larvae could metabolise the model drug AMI by N-demethylation to nortriptyline (NOR).
Method: Lucilia sericata larvae were reared on AMI-spiked porcine brain tissue (2.5 mg/g). The larvae were washed and frozen at -20 ºC prior to analysis. The biological tissues; porcine brain tissue or Lucilia sericata larvae were homogenized with water (1:10 w/v) and underwent a liquid-liquid extraction (LLE) followed by quantitative analysis using high performance liquid chromatography (HPLC).
Results: The HPLC assay could quantify AMI and NOR from 0.2 µg/mL to 5 µg/mL (R2 > 0.99) extracted from the biological tissues. There was minimal conversion of AMI to NOR within the porcine brain tissue over 123 hours. However, the larvae showed a high tolerance to AMI and partially converted the AMI to NOR in-vivo within 78 hours of exposure to AMI. In contrast, larvae samples taken after 120 hours contained additional metabolites in addition to the NOR.
Conclusion: Lucilia sericata larvae were able to perform N-demethylation using AMI as a model drug. Larvae, when used as an alternative toxicological sample need to be considered metabolically active in comparison to traditional toxicological samples; such as blood or muscle from a cadaver that are no longer metabolically active.

▼ Deep brain stimulation (DBS) is currently being investigated as a therapy for treatment-resistant depression, with promising results. However, it is not clear whether or not DBS works via the same mechanisms as those induced by antidepressant medications. Processes currently implicated in antidepressant effects include neuroplastic changes and promotion of neurogenesis. We investigated the effects of chronic treatment with three different classes of antidepressants and DBS on markers of neuroplasticity (brain-derived neurotrophic factor, (BDNF), and phosphorylated cyclic-AMP regulatory element binding protein, (pCREB)) and neurogenesis (Ki-67, bromodeoxyuridine (BrdU) and doublecortin) in the rat hippocampus. No clear treatment effects were seen on BDNF, pCREB and Ki-67 levels. However all treatments caused increased levels of BrdU (range: 46%-96%) and doublecortin (8%-61%), although these effects were statistically significant only for DBS and amitriptyline, respectively. This overall pattern of results may suggest that diverse antidepressant treatments could possibly share common mechanisms involving cell survival and neuronal differentiation. Potentiated effects of DBS on cell survival may underlie its efficacy in treatment-resistant depression.

▼ Background: The objective of this thesis was to explore mechanistic processes of
amitriptyline, opioids and "Burst" spinal cord stimulation (SCS) for neuropathic pain in vivo. These three therapies are commonly employed for the management of neuropathic pain without a global agreed consensus with regard to mechanism of action. Whilst there is growing body of pre-clinical evidence supporting the concept that chronic neuropathic pain is associated with neuroimmune dysfunction and / or neuroinflammation, this has yet to be translated into the clinical domain. Epidural steroid injections have demonstrated an attenuation of neuroinflammation associated with a short-term reduction in pain. To demonstrate dynamic changes within the neuroimmune interface (neuronal-glia-immune cell communications) we compared the constituents of cerebrospinal fluid (CSF) before and after treatment with
amitriptyline and Burst SCS. The mechanisms and side effects of opioids proposed for chronic pain are also related to neuroimmune interactions. Due to the fact many patients were taking opioids before enrolment, we compared the CSF of patients taking and not taking opioids prior to commencing the proposed interventional treatment. Not only would this provide data of confounding variables but give insight into opioid related phenomena and mechanisms. A more detailed characterisation of the effects of currently employed therapies may facilitate better stratification and phenotyping; as well as providing information which may enhance our understanding of the pathophysiology of neuropathic pain. Methods: This study involved taking cerebrospinal fluid samples before and after treatment and determining the cellular constituents of central peptides and immune cells. Patients were screened for neuropathic pain at outpatient clinics at St. James?s Hospital, Dublin and enrolled according to strict inclusion/exclusion criteria of the individual study. Lumbar radicular pain was the most common diagnosis for enrolled patients. Demographics and pains scores according to numerical rating scale (NRS) were recorded. After a baseline CSF sample the interventional treatment was commenced for 8 weeks. A second sample was subsequently taken after 8 weeks of treatment as well as outcome data. We compared the baseline and post intervention samples examining cellular data of T cells via flow cytometry, specific neuropeptides by ELISA and the proteome via Mass Spectrometry. In vitro work was also performed to examine the effect of
amitriptyline on T cells directly. Results: In a cohort of patients with lumbar radicular pain who were deemed responders to
amitriptyline, GO analysis identified immune system process as the most modulated proteins. This was associated with an attenuation of pro-inflammatory pathways, including the MAPK pathway identified by KEGG analysis. The chemokine eotaxin-1 was also significantly reduced in responders to

▼ Atualmente, a quantidade de pacientes que são diagnosticados com alguma forma de depressão, entre elas, o transtorno depressivo maior, aumenta consideravelmente, quer seja em razão de diagnósticos mais precisos ou pela própria epidemiologia da doença. Acresça-se o fato de que muitos pacientes, apesar da quantidade de tipos de antidepressivos atualmente disponíveis para a terapêutica, são refratários ao tratamento prescrito, em razão dos efeitos adversos apresentados ou ainda em razão de simplesmente não se observar melhora com a prescrição. Em razão disso, novos tratamentos farmacológicos são disponibilizados. Para auxiliar na máxima eficácia em sua utilização, esse trabalho propôs o desenvolvimento de metodologia analítica para a determinação simultânea de antidepressivos tricíclicos e não tricíclicos, a saber: moclobemida, venlafaxina, citalopram, agomelatina, duloxetina, amitriptilina e sertralina em plasma humano por cromatografia líquida de
alta eficiência (HPLC), para posteriormente ser aplicada na monitorização de pacientes depressivos. O método consistiu na extração líquido-líquido com recuperação entre 73% a 86%, exceto para a moclobemida (55%). A separação foi obtida usando uma coluna em fase reversa LiChrospher® 60 RP-select B em LichroCART 250mm x 4mm, 5 ?m de diâmetro interno, Merck sob condições isocráticas com detecção em UV em 230 nm, com fase móvel composta por 35% de uma mistura de acetonitrila/metanol 55/5 v/v e 65% de tampão acetato 0,25M pH 4,4. As curvas padrão foram lineares em uma faixa de trabalho de 2,5-1000 ng/mL para moclobemida, 5-2000 ng/mL para venlafaxina, citalopram, agomelatina, duloxetina e amitriptilina, 10-2000 ng/mL para sertralina. A precisão intra e interensaios foi efetuada em 3 concentrações (50, 200 e 500 ng/mL). Os coeficientes de variação para a precisão intraensaio foram menores que 8,8% para todos os compostos e os coeficientes de variação para a precisão interensaios foram
menores que 8,6%. Os limites de quantificação foram de 2,5 ng/mL para a moclobemida, 5 ng/mL para venlafaxina, citalopram, duloxetina, agomelatina, amitriptilina e 10 ng/mL para sertralina com a etidocaína como padrão interno. Não se observou qualquer interferência das drogas normalmente associadas com antidepressivos. O método desenvolvido foi aplicado na monitorização de 79 pacientes em tratamento prolongado com amitriptilina, sertralina e venlafaxina. Paralelamente, foram avaliadas as concentrações plasmáticas de pacientes voluntários sadios submetidos a tratamento em dose única com agomelatina. A monitorização terapêutica se faz necessária para monitorar adesão ao tratamento já que a depressão está entre as principais causas mundiais de morbidade por incapacitação social e estimativa de prevalência crescente até 2030; caracterizando-se como um dos maiores e mais onerosos problemas de saúde pública.

A amitriptilina é o protótipo dos antidepressivos tricíclicos, classe de fármaco muito utilizada no tratamento da dor neuropática. Tem por propriedade principal a inibição da recaptação de serotonina e noradrenalina, além de interagir com diversos outros receptores. Estudos recentes demonstraram que os antidepressivos tricíclicos são potentes bloqueadores dos canais de sódio, potássio e cálcio, apresentando propriedades semelhantes aos anestésicos locais. Assim existe potencial para que esta classe farmacológica possa ser utilizada no neuroeixo como adjuvante anestésico local ou
no tratamento de síndromes dolorosas neuropáticas refratárias. Entretanto, até o presente momento, não há consenso quanto a neurotoxicidade desta classe farmacológica sobre a medula e as meninges. O objetivo desta pesquisa foi avaliar a toxicidade da amitriptilina sobre a medula e as meninges quando administrada pela via subaracnóidea em cães. Método: Vinte e um animais foram randomizados em três grupos que receberam pela via subaracnóidea: Grupo 1 - Solução fisiológica 0,9%; Grupo 2 - solução de amitriptilina 5 mmol.ml-1; Grupo 3- solução de amitriptilina 10 mmol.ml-1. A punção subaracnóidea foi realizada no espaço intervertebral L6-L7. O volume da solução injetada foi de 1 ml. Os animais foram avaliados após uma hora da administração do fármaco e durante os 21 dias que permaneceram em cativeiro quanto ao tônus do esfíncter anal, bloqueio motor e sensibilidade dolorosa. Os animais foram sacrificados por eletrocussão sob anestesia. A medula e as meninges fixadas com solução de

▼ In this thesis, rodent models of chronic pain were
used to explore analgesic mechanisms that may potentially be
engaged in spinal and peripheral compartments by
systemically-administered
amitriptyline, a tricyclic
antidepressant. The first project (Chapter 2) identified the roles
of spinal adenosine A1 and serotonin 5-HT7 receptors, as well as of
peripheral adenosine A1 receptors, in the acute antinociceptive
effects of
amitriptyline in mice. The second project (Chapter 3)
examined the potential utility of
amitriptyline as a preventive
analgesic against persistent post-surgical pain, and involved
perioperative administration of
amitriptyline after peripheral
nerve injury in rats. Changes in post-injury behavioural outcomes,
as well as spinal noradrenergic sprouting, were assessed. Overall,
spinal serotonergic pathways linked to adenosine A1 receptors, as
well as peripheral adenosine A1 receptors, appear to be important
in antinociception by
amitriptyline. Preventive analgesia by this
drug does not appear to result from anatomical changes in spinal
noradrenergic pathways.

Antidepresantų mišinys išskirtas iš kraujo plazmos kietos fazės
ekstrakcijos metodu. KFE metodas optimizuotas keičiant eliuentą ir
eliuento pH. Atliktas optimizacijos sąlygų palyginimas. Nustatyti 3
organiniai tirpikliai, kuriais efektyviausiai eliuuojamos sorbente
sulaikytos tiriamosios medžiagos bei gauti statistiškai patikimi
rezultatai (p>0,05) : 2 proc. etano rūgšties tirpalas 100 proc.
metanolyje, 2 proc. etano rūgšties tirpalas 80 proc. metanolyje ir
2 proc. metano rūgšties tirpalas 100 proc.
metanolyje.
In this thesis was performed research of
scientific literature. Applied and validated high performance
liquid chromatography for identification and quantification of
target compounds. Antidepressant mixture extracted from blood
plasma samples using LLE and SPE methods. SPE method was optimized
by changing the elutor and its pH. Three organic solvents were
determined, which best elute target compounds from sorbent and show
statistical confidence (p>0,05): 2 proc. acetic acid in 100
proc. methanol solution, 2 proc. acetic acid in 80 proc. methanol
solution and 2 proc. formic acid in 100 proc. methanol
solution.

▼ Os medicamentos antidepressivos, especialmente os tricíclicos são indicados no tratamento farmacológico da depressão ou manejo de mialgias. Estes medicamentos têm sido utilizados em larga escala na rede sanitária pública do município de Ribeirão Preto, SP, Brasil. Os objetivos desta tese foram determinar o consumo dos psicotrópicos na rede pública de saúde de Ribeirão Preto, identificar os problemas relacionados com o medicamento amitriptilina (AMT), elaborar estratégias específicas de educação continuada para os médicos e verificar a resposta desta intervenção farmacêutica. Os dados de consumo foram obtidos das planilhas mensais das unidades de farmácia, arquivados na Divisão de Farmácia da Secretaria Municipal de Saúde de Ribeirão Preto. Para atingir os outros objetivos, esta pesquisa obteve os dados por meio de anamnese farmacêutica com os pacientes de oito unidades de saúde. Resultou na verificação da impressão dos pacientes sobre a assistência
à saúde, dos fatores determinantes da prescrição de AMT, dos conhecimentos médicos sobre os problemas relacionados com este medicamento, seu comportamento quanto à assistência ao paciente e na resposta à intervenção farmacêutica, que mostrou impacto positivo. Este trabalho ainda reafirma a atenção farmacêutica como prática profissional de grande utilidade e necessidade para a melhor assistência ao paciente e como agente no uso racional do medicamento e da farmacovigilância
The antidepressants, specially the tricyclic ones, are indicated for the pharmacological treatment of depression and pain. This drugs have been used extensively by the public heath care of Ribeirão Preto, SP, Brazil. The purposes of this thesis were: to determine the psychotropic consumption at the Heath System of Ribeirão Preto, to identify the amitriptyline (AMT) adverse reactions and other related problems, to elaborate specific strategies of pharmaceutical continuing education to the physicians and to
verify the response of this pharmaceutical intervention. Consumption data were obtained from monthly charts of the pharmacy sets, archived at the Pharmacy Division. To achieve the other purposes of this research, data were obtained from pharmaceutical anamnesis with patients from the health unities chosen. It resulted in verification of patients impression about health care received, AMT prescription determinant factors, medical knowledge about problems related to this drug and their behavior and also resulted in the pharmaceutical intervention response, which showed positive impact. This research affirms the pharmaceutical care as professional practice of great utility and necessity for better patient assistance and as agent at the rational drug use and pharmacovigilance.

▼ Este trabalho contempla a utilização dos modelos de Langmuir, Freundlich, Sips, Redlich-
Petersen, Toth, Dubinin–Radushkevich e Tempkin para análise de isotérmicas de adsorção
de compostos fenólicos, amitriptilina e fluoxetina em carvões ativados e negros de fumo em
fase líquida, obtidas pelo Grupo de Investigação de Química de Superfícies e Materiais da
Universidade de Évora. Foi utilizado o ajuste não linear aos dados experimentais usando o
software Solver do MS Excel. Para a avaliação da aplicabilidade dos modelos foram usadas
as funções de erro, coeficiente de determinação, soma dos erros quadrados, média do erro
relativo, desvio padrão percentual de Marquardt, função de erro fracionada hibrida, soma
dos erros absolutos, chi-quadrado, raíz quadrada da média do erro e média percentual dos
erros.
Os modelos que apresentaram melhor ajuste foram os modelos de Sips, Freundlich, Toth e
Langmuir observando-se para estes modelos as melhores correlações e minimização das
funções de erro;## Abstract:
Title: Study of the Application of Various Models and Methodologies in the Analysis of
Adsorption from the Liquid Phase.
Isotherm adsorption models of Langmuir, Freundlich, Sips, Redlich-Petersen, Toth, Dubinin–
Radushkevich and Tempkin were used to analyse isotherms of adsorption of phenolic
compounds,
amitriptyline and fluoxetine on activated carbons and carbon blacks from the
liquid phase. The samples were obtained by the Surface & Materials Chemistry Group from
University of Évora. The non-linear form of the models were used and fitted to the data using
Solver from the MS Excel software. Several error functions were used, such as the
Coefficient of Determination, the Sum of Squares of Errors, the Average Relative Error, the
Marquardt’s Percent Standard Deviation, the Hybrid Fractional Error Function, the Sum of
Absolute Errors, the Nonlinear Chi-square Test, the Residual Root Mean Square Error and
the Average Percentage Errors.
Sips, Freundlich, Toth and Langmuir models were considered to be the best-fitting, as they
gave the best correlation and minimization of the error functions of the data.

▼ Este é um estudo clínico, prospectivo, aleatório, e duplamente encoberto realizado em 80 doentes hansênicos de ambos os sexos e com idade variando entre 18 e 65 anos, portadores de dor neuropática. Os pacientes foram divididos em 4 grupos: Grupo G: pacientes tratados com gabapentina na dose de 400mg diários, Grupo C: pacientes tratados com carbamazepina na dose de 200 mg diários, Grupo GA: pacientes tratados com a associação de gabapentina 400 mg e amitriptilina 25 mg diários e Grupo CA: pacientes tratados com a associação de carbamazepina 200 mg e amitriptilina 25 mg diários e avaliados durante 4 meses quanto a intensidade da dor, consumo de prednisona, necessidade do uso de talidomida, queixa de queimação, parestesia, sensação de choque e alteração da sensibilidade, bem como da necessidade de realização de neurólise e eventos adversos relacionados ao tratamento. Os resultados permitiram verificar que o valor médio de intensidade de dor foi
semelhante em todos os grupos no momento da inclusão e no momento de encerramento do estudo, e a diminuição da dor foi semelhante em todos os grupos, não havendo superioridade de nenhum dos esquemas terapêuticos sobre o outro, portanto a carbamazepina isolada ou associada a amitriptilina e a gabapentina isolada ou associada a amitriptilina foram igualmente eficientes na redução da dor que os pacientes apresentavam no momento de inclusão no estudo, porém a duração da dor, em dias, foi maior no grupo medicado com a gabapentina isolada, foi menor e igual nos grupos medicados com a carbamazepina e com a gabapentina associada a amitriptilina e teve uma duração intermediária no grupo medicado com a carbamazepina associada a amitriptilina, embora a diferença não tenha sido significativa pela análise estatística. ▼ Many drugs such as tricyclic antidepressants and local anesthetics cause severe toxicity and/or death when taken at excessive and sometimes normal dosage levels. The
subject of this work was the development of a drug overdose treatment to counteract toxicity. Liposomes were chosen as the primary vehicle for toxicity reversal. The binding of the tricyclic antidepressants
amitriptyline, nortriptyline, imipramine, dosulepin, and opipramol to several variations of liposomes was measured. The medium of measurement was also varied from buffer to serum to assess in vivo affects. The effects of lipid type and loading, liposome size, polyethylene glycol inclusion and chain length, protein interaction, and storage were considered. Pegylated, anionic liposomes exhibited high affinity binding to all of the tricyclic antidepressants studied, despite the presence of serum proteins. Liposome size and polyethylene glycol chain length were inconsequential, while the proportion of polyethylene glycol incorporated into liposomes was optimal at about 5%. Liposome-drug binding was also found to occur to a significant extent for the local amide anesthetic drug bupivacaine. Additionally, interactions between cationic drugs and anionic liposomes were studied by measuring binding of drugs and the effect of binding on liposome permeability. Experiments and modeling indicated that, although electrostatic interactions were important, the fraction of drug sequestered in the double-layer was negligible. The majority of the drug enters the bilayer with the charged regions interacting with the charged lipid head groups and the lipophilic regions associated with the bilayer. Bupivacaine binds significantly less compared to tricyclic antidepressants because its structure is such that the charged region has minimal interactions with the lipid heads once the bupivacaine molecule partitions inside the bilayer. Conversely, the tricyclic antidepressants are linear with distinct hydrophilic and lipophilic regions, allowing the lipophilic regions to lie inside the bilayer and the hydrophilic regions to protrude out. This conformation maximizes the permeability of the bilayer, which leads to an increased release of a hydrophilic fluorescent dye from liposomes. Lastly, physiologically based pharmacokinetic models were developed for the design and optimization of liposome therapy for overdoses. The in vitro drug-binding data for pegylated, anionic liposomes and published mechanistic equations for partition coefficients were used to develop the models. The models were proven reliable through comparisons to intravenous data. Drug overdoses were simulated for various drug and liposome doses, elapsed time between drug intake and liposome treatment, and patient specific input parameters. ▼ The goal is the rapid removal of a hydrophobic drug from the bloodstream in cases of overdose. This can be accomplished with molecularly imprinted polymer (MIP) nanoparticles (NPs). The nanoparticles are made from a mixture of vinyl monomers, solvent, a photoinitiator, surfactant, and drug. The reaction is carried out as a miniemulsion polymerization, under continuous stirring, and UVA illumination. The size of the nanoparticles are determined by Dynamic Light Scattering. Drug reuptake studies by UV spectroscopy are performed with a phosphate buffered saline (PBS) solution that mimics blood plasma and contains drug hydrochloride salt. The nanoparticles demonstrate high binding affinity for the drug that they were made with, as evidenced by significant loss of drug from the supernatant after pelletizing the nanoparticles by centrifugation. This gives high partition coefficient values, or Kp, in excess of 10,000 in some batches. ( en )

▼ Depression and chronic pain share an intricate, complex relationship, the comorbidity of which is reported to affect up to 80% of patients. This thesis demonstrated enhanced nociceptive responding to mechanical and cold, but not heat, stimuli in a well-validated animal model of depression, the olfactory bulbectomised (OB) rat. In addition, early life stress in the form of maternal deprivation (MD) resulted in enhanced nociceptive responding to mechanical and heat, but not cold, stimuli. OB animals exhibited exacerbated cold, but not mechanical, allodynia following spinal nerve ligation (SNL), a clinically relevant model of neuropathic pain. In addition, MD female, but not male, rats exhibited exacerbated mechanical and cold allodynia following SNL. Thus, neuropathic pain-related behaviour is enhanced in two models of affective disorders in a stimulus- and sex-dependent manner.
Despite the high comorbidity of these disorders, the neurobiological mechanisms mediating the relationship between affective disorders and chronic pain remain unknown. Evidence suggests that monoamines and neuroimmune mediators play a role in the relationship between depression and pain. This thesis demonstrated that the enhanced SNL-induced cold allodynia observed in OB rats was accompanied by reduced serotonergic neurotransmission in the hippocampus, reduced inflammatory cytokine gene expression in the amygdala and increased cytokine gene expression and glial activation in the prefrontal cortex (PFC). In addition, early life stress and SNL differentially altered immune gene expression in the PFC and hippocampus in a sex-dependent manner. Pharmacological studies were subsequently employed in order to further probe the role of monoamines and neuroimmune mediators in the OB-related changes in nociceptive responding, prior to and following SNL. Chronic treatment with
amitriptyline (a tricyclic antidepressant and first-line treatment for neuropathic pain) and minocycline (an inhibitor of microglial activation) elicited an antidepressant-like effect, but did not alter basal nociceptive responding in the OB rat. However, chronic
amitriptyline and minocycline treatment differentially altered neuropathic pain-related behaviour in the presence or absence of a depressive-like phenotype. In brief,
amitriptyline attenuated SNL-induced cold allodynia and thermal hyperalgesia, but not mechanical allodynia, in sham (control) rats. In contrast,
amitriptyline attenuated SNL-induced mechanical allodynia, but not cold allodynia or thermal hyperalgesia in OB rats. Chronic minocycline treatment attenuated SNL-induced cold and mechanical allodynia in both sham and OB rats. Thus, enhancing monoamines and blocking microglial activation elicits antidepressant-like effects and attenuates SNL-induced mechanical allodynia in the OB model of depression.
