▼ The introduction of time series modeling techniques made analyzing the different factors underlying the changes in mortality and incidence rates over time possible, both for analytic and predictive purposes. Age-period-cohort analyses contribute to the etiologic purpose of descriptive epidemiology making inference from the group to the individual possible. These refer to a family of statistical techniques that study the temporal trends of outcomes, such as mortality an incidence, in terms of three temporal variables: subject age, calendar period and the subject's birth cohort. Useful as it is, the age-period-cohort model is marred by a structural problem of identifiability: the variables of age, period and cohort have an exact linear dependence, i.e. "age = period - cohort". Predicting a future event is a complex and insidious process, however, it is a useful endeavor in most human activities. The information gained on probable future trends, even if unreliable or imprecise is highly valuable. Predicted future cancer incidence and mortality rates are essential tools for both epidemiology and health planning. Numerous methods to carry out age-period-cohort analysis are described in the literature, three of these are illustrated in detail and compared by applying them to real data (WHO mortality database): a method based on penalized likelihood, one using generalized additive models (GAM) and one based on partial least squares (PLS) techniques. Predictive analysis techniques are also presented and compared, using observed mortality data. Short term age-period prediction methods based on joinpoint analysis and Bayesian modelling, and a long term technique, which uses a Bayesian age-period-cohort model, are reviewed. In details, predictions through age-period method based on joinpoint analysis are carried out applying linear, Poisson and log-linear regression models. In the age-period-cohort analysis comparison, the penalized likelihood and GAM methods produce similar results, while effect estimates from the PLS model are noticeably different. These differences can be explained by looking at how the three models solve the issue of perfect collinearity between age, period and cohort parameters. On the one hand, the penalized likelihood and GAM methods use different techniques to distribute the linear drift between the period and cohort effects. The PLS method, on the other hand, solves the identifiability problem by tackling the generalized inverse, minimizing the estimated parameter variance and covariance matrix. Without a formal simulation analysis, comments are limited to stating that the two models based on linear drift distribution are more suitable for epidemiological comparisons, where the effects of age are well defined (as in the case of cancer mortality) and the major problems reside in untangling the period and cohort effects. The PLS model, on the other hand, may hypothetically prove to be a useful method to predict future trends. The gap between predicted brain age and chronological age could serve as biomarker for early-stage neurodegeneration and as potentially as a risk indicator for dementia. We assess the utility of this age gap as a risk biomarker for incident dementia in a general elderly population. The brain age is estimated from longitudinal brain magnetic resonance imaging (MRI) data using deep learning models. From the population-based Rotterdam Study, 5656 dementia-free and stroke-free participants (mean age 64.67±9.82, 54.73% women) underwent brain MRI at 1.5T, including three-dimensional (3D) T1-weighted sequence, between 2006 and 2015. All participants were followed for incident dementia until 2016. During 6.66±2.46 years of follow-up, 159 subjects developed dementia. The entire dataset was split into control (N=5497) and incident dementia (N=159) groups. We then built a convolutional neural network (CNN) model trained on the control group to predict brain age based on brain MRI. Model prediction performance was measured in mean absolute error MAE=4.45±3.59 years of brain age prediction. Reproducibility of prediction was tested using the intra-class correlation coefficient ICC=0.97 (95% confidence interval CI=0.96-0.98), computed on a subset of 80 subjects. Hereafter, we investigated the gap between model predicted age and chronological age of the incident dementia group data, compared to control group. Logistic regressions and Cox proportional hazards models were used to assess the association of the age gap with incident dementia, adjusted for years of education, ApoE4 allele carriership, GM and intracranial volume. These models showed that the age gap was significantly related to incident dementia (odds ratio OR=1.11 and 95% confidence intervals CI=1.05-1.16; hazard ratio HR=1.11 and 95% CI=1.06-1.15, respectively). Additionally, we computed the attention maps of CNN, which shows the importance of brain regions for age prediction. These were particularly focused on the amygdalae and hippocampi. We show that the gap between predicted and chronological brain age is a biomarker, associated with a risk of dementia development. This suggests that it can potentially be used as a complimentary biomarker for early-stage dementia risk screening.

▼ In the evaluation of special populations, such as neonates, infants and malingerers, audiologist often have to rely heavily on objective measurements to assess hearing ability. Current objective audiological procedures such as tympanometry, the acoustic reflex, auditory brainstem response and transient evoked otoacoustic emissions, however, have certain limitations, contributing to the need of an objective, non¬invasive, rapid, economic test of hearing that evaluate hearing ability in a wide range of frequencies. The purpose of this study was to investigate distortion product otoacoustic emissions (DPOAEs) as an objective test of hearing. The main aim was to attempt to predict hearing ability at 500 Hz, 1000 Hz, 2000 Hz and 4000 Hz with DPOAEs and artificial neural networks (ANNs) in normal and hearing-impaired ears. Other studies that attempted to predict hearing ability with DPOAEs and conventional statistical methods were only able to distinguish between normal and impaired hearing. Back propagation neural networks were trained with the pattern of all present and absent DPOAE responses of 11 DPOAE frequencies of eight DP Grams and pure tone thresholds at 500 Hz, 1000 Hz, 2000 Hz and 4000 Hz. The neural network used the learned correlation between these two data sets to predict hearing ability at 500 Hz, 1000 Hz, 2000 Hz and 4000 Hz. Hearing ability was not predicted as a decibel value, but into one of several categories spanning 10-15dB. Results indicated that prediction accuracy of normal hearing was 92% at 500 Hz, 87% at 1000 Hz, 84% at 2000 Hz and 91% at 4000 Hz. The prediction of hearing-impaired categories was less satisfactory, due to insufficient data for the ANNs to train on. The variables age and gender were included in some of the neural network runs to determine their effect on the distortion product. Gender had only a minor positive effect on prediction accuracy, but age affected prediction accuracy considerably in a positive way. The effect of the amount of data that the neural network had to train on was also investigated. A prediction versus ear count correlation strongly suggested that the inaccurate predictions of hearing-impaired categories is not a result of an inability of DPOAEs to predict pure tone thresholds in hearing impaired ears, but a result of insufficient data for the neural network to train on. This research concluded that DPOAEs and ANNs can be used to accurately predict hearing ability within 10dB in normal and hearing-impaired ears from 500 Hz to 4000 Hz for hearing losses of up to 65dB HL. ▼ Heart transplantation has become the treatment of choice for patients with end-stage heart diseases. Originally, strict recipient and donor selection criteria were developed to minimize morbidity and mortality after heart transplantation. However, the increasing demand for heart transplantation has led to the liberalization of the traditional recipient selection criteria. There has been a gradual increase of recipient age. Patients at poor health status, e.g. pretransplant diagnosis of end-stage valvular heart disease and older recipients, are more likely to have a higher morbidity and mortality due to co-existing medical problems. The increased gap between the demand and supply of donor hearts results in significant prolonged waiting time and an increased mortality of recipients on the waiting list. To increase donor supply, acceptance criteria of donor hearts have been expanded to include the sub-optimal or marginal donor hearts. Nevertheless, the impact of these trends on the outcome of heart transplantation, in particular on long term, remains uncertain. Heart transplantation should be reserved for those patients most likely to benefit in terms of survival. To enable predicting the outcome for individual patient based on recipient and donor characteristics, and their match, a prediction model may play a crucial role in selection of a donor heart to a specific recipient. The focus of this thesis is on the impact of various recipient and donor baseline characteristics on the outcome after adult heart transplantation. This thesis also includes several models that have been developed and validated to predict mortality risk after adult heart transplantation. We demonstrated that despite changing in baseline risk profiles at different transplant periods, the survival of adult heart transplantation remains stable and encouraging. Moreover, we showed that prognosis after heart transplantation in patients with end-stage valvular disease is similar to patients with other indications, whereas a longer waiting time to heart transplantation significantly improved survival. Our results also demonstrated that the outcome of heart transplantation in older recipients is less favorable than in younger patients. Therefore, the decision to offer heart transplantation to recipients older than 55 years should be considered cautiously. Furthermore, we found that heart transplantation from donors with sodium level > 155 mmol/L and using donor hearts that have been subject to cardiopulmonary resuscitation do not adversely affect the outcome of heart transplantation. Accepting such donor hearts will add to the enlargement of the donor pool, and guidelines should be adjusted accordingly. Finally, we have shown that the 30-day, 1- and 5-year mortality risk after adult heart transplantation could be predicted with accuracy based on pretransplant clinical characteristics of recipients and donors as well as perioperative data. ▼ Accurate descriptions of normal craniofacial shape are essential for accurate assessment of pathologies of the craniofacial complex. Traditionally, these descriptions, have been limited to growth curves of single measurements (e.g. head circumference) to which a patient can be compared. In this thesis we exploit advances in 3D image capture and image processing and also combine tools from statistical shape analysis and multivariate statistics to produce 3D growth curves of the entire head and face. These can serve as a normative frame of reference, against which craniofacial abnormality can be judged and we demonstrate their use for assessing individual patients. They are also a tool for comparing populations, which we demonstrate by examining emerging shape differences between boys and girls throughout childhood and adolescence, and for growth prediction and age estimation. In two studies we examine the development of sex differences in craniofacial shape (craniofacial sexual dimorphism). The first compared boys and girls within a database of one year-olds. The second compared growth curves of boys and girls derived from a cross-sectional database of 3D images of children aged from 0.05-18.6 years, to examine how and when sex differences emerge. Both studies confirmed the presence of sexual dimorphism at approximately one year-old. The second also demonstrated that sexual dimorphism emerges in primarily two phases between ages five and ten and from twelve onwards. The second of these constitute the most comprehensive study of the development of sexual dimorphism to date and both studies challenge the view that sexual dimorphism emerges as the result of sex hormones at puberty. We also use the growth curves to predict the growth of individuals from 3D photographs. We validate these predictions against a sample of 50 longitudinally collected images by comparing the predicted head at the second time point to the actual head at the second time point. We also develop them into an algorithm for estimating the age of individuals from 3D photographs.

▼ Childhood viral and bacterial infections remain an important public problem, and research into their dynamics has broader scientific implications for understanding both dynamical systems and associated methodologies at the population level. Measles and pertussis are two important childhood infectious diseases. Measles is a highly transmissible disease and is one of the leading causes of death among young children under 5 globally. Pertussis (whooping cough) is another common childhood infectious disease, which is most harmful for babies and young children and can be deadly. While the use of ongoing surveillance data and - recently - dynamic models offer insight on measles (or pertussis) dynamics, both suffer notable shortcomings when applied to measles (or pertussis) outbreak prediction. In this thesis, I apply the Sequential Monte Carlo approach of particle filtering, incorporating reported measles and pertussis incidence for Saskatchewan during the pre-vaccination era, using an adaptation of a previously contributed measles and pertussis compartmental models. To secure further insight, I also perform particle filtering on age structured adaptations of the models. For some models, I further consider two different methods of configuring the contact matrix. The results indicate that, when used with a suitable dynamic model, particle filtering can offer high predictive capacity for measles and pertussis dynamics and outbreak occurrence in a low vaccination context. Based on the most competitive model as evaluated by predictive accuracy, I have performed prediction and outbreak classification analysis. The prediction results demonstrated that the most competitive models could predict the measles and pertussis outbreak patterns and classify whether there will be an outbreak or not in the next month (Area under the ROC Curve of measles is 0.89, while pertussis is 0.91). I conclude that anticipating the outbreak dynamics of measles and pertussis in low vaccination regions by applying particle filtering with simple measles and pertussis transmission models, and incorporating time series of reported case counts, is a valuable technique to assist public health authorities in estimating risk and magnitude of measles and pertussis outbreaks. Such approach offers particularly strong value proposition for other pathogens with little-known dynamics, important latent drivers, and in the context of the growing number of high-velocity electronic data sources. Strong additional benefits are also likely to be realized from extending the application of this technique to highly vaccinated populations. ▼ Electrocardiography (ECG) is a non-invasive method used in medicine to track the electrical pulses sent by the heart. The time between two subsequent electrical impulses and hence the heartbeat of a subject, is referred to as an RR interval. Previous studies show that RR intervals can be used for identifying sleep patterns and cardiovascular diseases. Additional research indicates that RR intervals can be used to predict the cardiovascular age of a subject. This thesis investigates, if this assumption is true, based on two different datasets as well as simulated data based on Gaussian Processes. The datasets used are Holter recordings provided by the University of Gdańsk as well as a dataset provided by Physionet. The former represents a balanced dataset of recordings during nocturnal sleep of healthy subjects whereas the latter one describes an imbalanced dataset of records of a whole day of subjects that suffered from myocardial infarction. Feature-based models as well as a deep learning architecture called DeepSleep, based on a paper for sleep stage detection, are trained. The results show, that the prediction of a subject's age, only based in RR intervals, is difficult. For the first dataset, the highest obtained test accuracy is 37.84 per cent, with a baseline of 18.23 per cent. For the second dataset, the highest obtained accuracy is 42.58 per cent with a baseline of 39.14 per cent. Furthermore, data is simulated by fitting Gaussian Processes to the first dataset and following a Bayesian approach by assuming a distribution for all hyperparameters of the kernel function in use. The distributions for the hyperparameters are continuously updated by fitting a Gaussian Process to a slices of around 2.5 minutes. Then, samples from the fitted Gaussian Process are taken as simulated data, handling impurity and padding. The results show that the highest accuracy achieved is 31.12 per cent with a baseline of 18.23 per cent. Concludingly, cardiovascular age prediction based on RR intervals is a difficult problem and complex handling of impurity does not necessarily improve the results.

▼ Part 1 of this thesis focuses on echogenicity of the carotid artery. This measure represents the color of the intima-media complex on ultrasound images. Echolucent vascular walls appear dark, echodense vascular walls light. We showed in a group of young adults that higher age, BMI, systolic blood pressure, triglycerides and carotid intima media thickness (CIMT) were associated with a lower grey-scale median (GSM) and thus a more echolucent carotid artery wall. Higher HDL-cholesterol and male sex were associated with a higher GSM and thus a more echodense carotid artery wall. Lower GSM was associated with a higher estimated cardiovascular disease risk. Furthermore, we showed that in a cohort of elderly men that when the population was split into thirds of GSM, individuals in the highest third had (independent of cardiovascular risk factors and CIMT) a significantly higher risk of all-cause mortality and cardiovascular mortality compared to those with a GSM in the lowest third. Part 2 of this thesis focuses on CIMT, both in association with risk factors and disease and as a possible marker to improve cardiovascular risk prediction. Ethnicity appeared to slightly but significantly modify the associations between risk factors and CIMT and cardiovascular events. This has possible implications for the development of cardiovascular risk prediction models. In a population of adults under 45 years of age, we found that age, sex, diastolic blood pressure, body mass index, total cholesterol and high-density lipoprotein cholesterol were associated with higher CIMT. Furthermore, higher CIMT was independently associated with increased risk of first-time myocardial infarction or stroke in this population. We did not find clinically relevant incremental prognostic value, either for measuring mean common CIMT in individuals with hypertension, or for measuring CIMT in the internal carotid artery and/or the carotid bulb in the general population. Part 3 of this thesis focuses on vascular age and on the comparison of prediction models. First, we conducted a review of the literature to explore published concepts, definitions and clinical applications of vascular age. Vascular age is the expression of cardiovascular disease risk in years instead of a percentage. Despite sharing a common name, various studies have proposed distinct ways to define and measure vascular age. While we were unable to examine the effect of using vascular age in the communication of risk, we did examine the claim of vascular age as a way to improve risk prediction. We applied four methods of determining vascular age (two based on existing risk scores and two based on CIMT) to a study population representing the general population. Irrespective of definition vascular age does not improve cardiovascular risk prediction. Finally, we examined the metrics used to calculate incremental prognostic value of prediction models. ▼ A study of 1380 grade 3, 4, 5, and 6 primary school children at four metropolitan schools was undertaken to examine the relationship between school starting age and retention rates, referral rates (for behavioural, learning, behavioural and learning difficulties, individually or aggregated), and performance on a range of achievement tests. Six different entry age group categories were identified according to entrance age in the child's preparatory grade year. These were underage starters (younger than 4 years 6 months), younger-entering starters (4 years 6 months to 4 years 9 months), normal-entering starters (4 years 10 months to 5 years 1 month), older-entering starters (5 years 2 months to 5 years 5 months), and overage starters (older than 5 years 5 months) . The last category was split into two separate groups comprising those who had repeated kindergarten, and those who had been held back at home and had not attended kindergarten. These groups were used in computation, though some of these groups were amalgamated subsequently in some chi square analyses because of low expected cell numbers. The youngest entrants (underage and younger-entering children) were found to be significantly more likely to have been retained for a year at primary school than their normally older classmates. Risks of retention dropped with increasing age on commencement, except in the case of overage entrants who had not attended a kindergarten. Preparatory grade, grade 2 and grade 3 were the most common grades chosen for retention. The academic achievement of children retained in Preparatory grade to grade 6 was significantly lower than that of the nonretained children at grades 4, 5, and 6. There was no significant difference at grade 3 level. Retained younger-entering, underage children and girls were found to be significantly more likely to be referred for learning difficulties, and retained younger-entering and underage children had significantly higher rates of referral for aggregated (multiple) causes than other entry age groups. Significant differences between older-entering and younger-entering children were not found with respect to academic achievement as measured on a range of achievement tests. However, significant differences favouring overage starters who had repeated kindergarten and other entry age group children were observed at all grade levels and in a variety of achievement tests, though not in a uniform manner. Younger-entering children were significantly more likely (across the whole sample) to be referred for learning difficulties and aggregated referral rates than other entry age groups. This applied to both girls and boys. ▼ Part 1 of this thesis focuses on echogenicity of the carotid artery. This measure represents the color of the intima-media complex on ultrasound images. Echolucent vascular walls appear dark, echodense vascular walls light. We showed in a group of young adults that higher age, BMI, systolic blood pressure, triglycerides and carotid intima media thickness (CIMT) were associated with a lower grey-scale median (GSM) and thus a more echolucent carotid artery wall. Higher HDL-cholesterol and male sex were associated with a higher GSM and thus a more echodense carotid artery wall. Lower GSM was associated with a higher estimated cardiovascular disease risk. Furthermore, we showed that in a cohort of elderly men that when the population was split into thirds of GSM, individuals in the highest third had (independent of cardiovascular risk factors and CIMT) a significantly higher risk of all-cause mortality and cardiovascular mortality compared to those with a GSM in the lowest third. Part 2 of this thesis focuses on CIMT, both in association with risk factors and disease and as a possible marker to improve cardiovascular risk prediction. Ethnicity appeared to slightly but significantly modify the associations between risk factors and CIMT and cardiovascular events. This has possible implications for the development of cardiovascular risk prediction models. In a population of adults under 45 years of age, we found that age, sex, diastolic blood pressure, body mass index, total cholesterol and high-density lipoprotein cholesterol were associated with higher CIMT. Furthermore, higher CIMT was independently associated with increased risk of first-time myocardial infarction or stroke in this population. We did not find clinically relevant incremental prognostic value, either for measuring mean common CIMT in individuals with hypertension, or for measuring CIMT in the internal carotid artery and/or the carotid bulb in the general population. Part 3 of this thesis focuses on vascular age and on the comparison of prediction models. First, we conducted a review of the literature to explore published concepts, definitions and clinical applications of vascular age. Vascular age is the expression of cardiovascular disease risk in years instead of a percentage. Despite sharing a common name, various studies have proposed distinct ways to define and measure vascular age. While we were unable to examine the effect of using vascular age in the communication of risk, we did examine the claim of vascular age as a way to improve risk prediction. We applied four methods of determining vascular age (two based on existing risk scores and two based on CIMT) to a study population representing the general population. Irrespective of definition vascular age does not improve cardiovascular risk prediction. Finally, we examined the metrics used to calculate incremental prognostic value of prediction models. ▼ Genetic testing plays an important role in tree breeding. Understanding basic genetic parameters for growth and fertility is a prerequisite for developing a successful breeding strategy to balance between short- and long-term gains. It is both impractical and inefficient to wait until trials are several decades old to make selection decisions. It is therefore of great value to evaluate a sample of long-term experiments to confirm selection efficiency at ages closer to rotation time. This thesis investigates the genetic expression of field performance in Scots pine (Pinus sylvestris L.) for growth and fertility in long-term experiments over time and across sites. The data revealed large variation in genetic expression over time and across sites. The additive effect was the most important source of genetic variation for growth, while estimates of dominance were small, site-specific, and decreased much with age. Thus, there is little benefit in attempting to explore dominance through specific combining ability to improve genetic productivity in northern Sweden. Progeny of plus-trees selected in northern Sweden showed faster growth compared to unimproved controls at age 30, as well as genetic differences in stem shape, such that improved trees were more slender. The genetic relationship between height and diameter in northern Sweden demonstrated the importance of considering diameter in selection to obtain greater genetic gain in volume. Overall, the results show higher heritability at older ages and genetic correlations that reveal important rank changes with time and across environments. Clonal variation in female fertility in mature seed orchards is rather small and varies as much within clones as among clones, and is heavily dependent on year of assessment. The prospects for early selection of clones for future seed cone production based on a single-year observation are low. Finally, this thesis illustrates the importance of subjecting data from long-term field tests to a multi-trait, multi-site analysis accounting for environmental effects.

Originally, strict recipient and donor selection criteria were developed to minimize morbidity and mortality after heart transplantation. However, the increasing demand for heart transplantation has led to the liberalization of the traditional recipient selection criteria. There has been a gradual increase of recipient age. Patients at poor health status, e.g. pretransplant diagnosis of end-stage valvular heart disease and older recipients, are more likely to have a higher morbidity and mortality due to co-existing medical problems. The increased gap between the demand and supply of donor hearts results in significant prolonged waiting time and an increased mortality of recipients on the waiting list. To increase donor supply, acceptance criteria of donor hearts have been expanded to include the sub-optimal or marginal donor hearts. Nevertheless, the impact of these trends on the outcome of heart transplantation, in particular on long term, remains uncertain. Heart transplantation should be reserved for those patients most likely to benefit in terms of survival. To enable predicting the outcome for individual patient based on recipient and donor characteristics, and their match, a prediction model may play a crucial role in selection of a donor heart to a specific recipient. The focus of this thesis is on the impact of various recipient and donor baseline characteristics on the outcome after adult heart transplantation. This thesis also includes several models that have been developed and validated to predict mortality risk after adult heart transplantation. We demonstrated that despite changing in baseline risk profiles at different transplant periods, the survival of adult heart transplantation remains stable and encouraging. Moreover, we showed that prognosis after heart transplantation in patients with end-stage valvular disease is similar to patients with other indications, whereas a longer waiting time to heart transplantation significantly improved survival. Our results also demonstrated that the outcome of heart transplantation in older recipients is less favorable than in younger patients. Therefore, the decision to offer heart transplantation to recipients older than 55 years should be considered cautiously. Furthermore, we found that heart transplantation from donors with sodium level > 155 mmol/L and using donor hearts that have been subject to cardiopulmonary resuscitation do not adversely affect the outcome of heart transplantation. Accepting such donor hearts will add to the enlargement of the donor pool, and guidelines should be adjusted accordingly. Finally, we have shown that the 30-day, 1- and 5-year mortality risk after adult heart transplantation could be predicted with accuracy based on pretransplant clinical characteristics of recipients and donors as well as perioperative data. Therefore, the decision to offer heart transplantation to recipients older than 55 years should be considered cautiously. Furthermore, we found that heart transplantation from donors with sodium level > 155 mmol/L and using donor hearts that have been subject to cardiopulmonary resuscitation do not adversely affect the outcome of heart transplantation. Accepting such donor hearts will add to the enlargement of the donor pool, and guidelines should be adjusted accordingly. Finally, we have shown that the 30-day, 1- and 5-year mortality risk after adult heart transplantation could be predicted with accuracy based on pretransplant clinical characteristics of recipients and donors as well as perioperative data. La dégénérescence maculaire liée à l’âge (DMLA) est la première cause de cécité dans les pays industrialisés. C’est une maladie complexe et multifactorielle ayant des conséquences majeures sur la qualité de vie des personnes atteintes. De nombreux facteurs de risque, génétiques et non génétiques, jouent un rôle important dans la pathogénèse des stades avancés de la DMLA. Les modèles de prédiction développés à ce jour reposent sur un nombre limité de ces facteurs, et sont encore peu utilisés dans la pratique clinique.Ce travail de thèse avait pour premier objectif d’identifier de nouveaux biomarqueurs circulants du risque de DMLA. Ainsi, à partir d’une étude post-mortem basée sur une approche de lipidomique, nous avons identifié les composés lipidiques sanguins les plus prédictifs des concentrations rétiniennes en acides gras polyinsaturés omégas 3 (AGPI w-3). Nous avons développé un modèle de prédiction basé sur 7 espèces de lipides des esters de cholestérol. Ce modèle, obtenu en combinant pénalisation et réduction de la dimension, a ensuite été validé dans des études cas-témoins de DMLA et dans un essai clinique randomisé de supplémentation en AGPI w-3. Ces biomarqueurs pourraient être utiles pour l’identification des personnes à haut risque de DMLA, qui pourraient ainsi bénéficier d’une supplémentation en AGPI w-3.Le deuxième objectif de cette thèse était de développer un modèle de prédiction du risque de progression vers une DMLA avancée à partir de facteurs de risque génétiques, phénotypiques et environnementaux. Une originalité de notre travail a été d’utiliser une méthode de régression pénalisée – un algorithme d’apprentissage automatique – dans un cadre de survie afin de tenir compte de la multicollinéarité entre les facteurs de risque. Nous avons également pris en compte la censure par intervalle et le risque compétitif du décès via un modèle à 3 états sain-malade-mort. Nous avons ensuite validé ce modèle sur une étude indépendante en population générale.Il serait intéressant de valider ce modèle de prédiction dans d’autres études indépendantes en y incluant les biomarqueurs circulants identifiés à partir de l’étude de lipidomique effectuée dans le cadre de cette thèse. Le but final serait d’intégrer cet outil prédictif dans la pratique clinique afin de rendre la médecine de précision une réalité pour les patients atteints de DMLA dans le futur proche.

▼ In the evaluation of special populations, such as neonates, infants and malingerers, audiologist often have to rely heavily on objective measurements to assess hearing ability. Current objective audiological procedures such as tympanometry, the acoustic reflex, auditory brainstem response and transient evoked otoacoustic emissions, however, have certain limitations, contributing to the need of an objective, non¬invasive, rapid, economic test of hearing that evaluate hearing ability in a wide range of frequencies. The purpose of this study was to investigate distortion product otoacoustic emissions (DPOAEs) as an objective test of hearing. The main aim was to attempt to predict hearing ability at 500 Hz, 1000 Hz, 2000 Hz and 4000 Hz with DPOAEs and artificial neural networks (ANNs) in normal and hearing-impaired ears. Other studies that attempted to predict hearing ability with DPOAEs and conventional statistical methods were only able to distinguish between normal and impaired hearing. Back propagation neural networks were trained with the pattern of all present and absent DPOAE responses of 11 DPOAE frequencies of eight DP Grams and pure tone thresholds at 500 Hz, 1000 Hz, 2000 Hz and 4000 Hz. The neural network used the learned correlation between these two data sets to predict hearing ability at 500 Hz, 1000 Hz, 2000 Hz and 4000 Hz. Hearing ability was not predicted as a decibel value, but into one of several categories spanning 10-15dB. Results indicated that prediction accuracy of normal hearing was 92% at 500 Hz, 87% at 1000 Hz, 84% at 2000 Hz and 91% at 4000 Hz. The prediction of hearing-impaired categories was less satisfactory, due to insufficient data for the ANNs to train on. The variables age and gender were included in some of the neural network runs to determine their effect on the distortion product. Gender had only a minor positive effect on prediction accuracy, but age affected prediction accuracy considerably in a positive way. The effect of the amount of data that the neural network had to train on was also investigated. A prediction versus ear count correlation strongly suggested that the inaccurate predictions of hearing-impaired categories is not a result of an inability of DPOAEs to predict pure tone thresholds in hearing impaired ears, but a result of insufficient data for the neural network to train on. This research concluded that DPOAEs and ANNs can be used to accurately predict hearing ability within 10dB in normal and hearing-impaired ears from 500 Hz to 4000 Hz for hearing losses of up to 65dB HL.
